Abstract Pollutants have adverse effects on human health and on other organisms that inhabit or use water resources. The aim of the present study was to assess the environmental quality of three watercourses in Monte Carmelo, MG, Brazil, using the micronucleus test on Tradescantia. For each treatment, 15 plants were exposed to water samples for 24 h. The control group was exposed to formaldehyde (0.2%) and the negative control to Hoagland solution. Subsequently the plants were placed in Hoagland solution for 24 h to recover. Cells were stained with 2% acetic carmine and examined by light microscopy. Three hundred tetrads were analyzed per slide. The frequency of genotoxic alterations was expressed as the number of micronuclei per 100 tetrads, and the groups were compared by ANOVA. At all sample sites for each watercourse significant genotoxicity indices were observed. The results suggest that in the Mumbuca creek, the current situation of effluent discharge should be reconsidered by the municipal environmental authorities. The increase in micronucleus frequency denoted for water samples of the Mumbuca creek, Lambari river and Perdizes river emphasizes the need to adopt environmental vigilance strategies, such as biological monitoring.

The paper describes a fundamental feature of digital cytology relevant to the implementation of the technology in the hospital net services: the electronic recording of the virtual slides (VS) in the hospital information system (HIS) through a picture archiving and communication system (PACS). Starting from the digital cytology (D-CYT) state of art and considering the most important products in the field, particular attention has been devoted in this review to the comparison with the digital radiology (D-RAD). Two main indications emerged from the study: 1. there is not a standard in the digital files relevant to the virtual slides in D-CYT, while in D-RAD the standard digital imaging and communications in medicine (DICOM), introduced by the National Electrical Manufacturers Association (NEMA), has been adopted from several years; 2. the PACS in D-CYT are not standardized from manufacturers. The study makes a proposal of a useful software architecture to improve the PACS integration for D-CYT applications, with potentialities in the HIS.

Il lavoro affronta una delle problematiche di base relative all'introduzione della tele-citopatologia negli ospedali: la registrazione elettronica del vetrino virtuale nell'HIS attraverso il PACS. Nella revisione dello stato dell'arte attuale relativo all'introduzione negli ospedali della citologia digitale si è tenuto conto dei diversi prodotti attualmente disponibili. Particolare attenzione è stata inoltre posta alla comparazione con lo stato di inserimento della radiologia digitale. Lo studio ha evidenziato attraverso questa comparazione con la radiologia digitale due importanti problematiche: 1. non esiste attualmente uno standard consolidato relativo ai vetrini virtuali in citologia digitale. In radiologia-digitale lo standard DICOM è stato adottato da diversi anni; 2. i PACS in citologia digitale non sono attualmente standardizzati e cambiano con il produttore. Lo studio termina con una proposta teorica di una architettura software utile a migliorare l'integrazione dei PACS nelle applicazione di citologia digitale con potenzialità nell'HIS.

The paper approaches a new technological scenario relevant for the introduction of the digital cytology (D-CYT) in the health service. A detailed analysis of the state of the art on the status of the introduction of D-CYT in the hospital and more in general in the dispersed territory has been conducted. The analysis was conducted in a form of review and was arranged into two parts: the first part focused on the technological tools needed to carry out a successful service (client server architectures, e-learning, quality assurance issues); the second part focused on issues oriented to help the introduction and evaluation of the technology (specific training in D-CYT, health technology assessment in-routine application, data format standards and picture archiving computerized systems (PACS) implementation, image quality assessment, strategies of navigation, 3D-virtual-reality potentialities). The work enlightens future scenarios of actions relevant for the introduction of the technology.

Il lavoro affronta il nuovo scenario tecnologico relativo all'introduzione della citopatologia digitale nell'ospedale. È stata condotta un'analisi dettagliata sullo stato dell'arte relativo all'introduzione della tecnologia negli ospedali e nei territori disagiati. L'analisi condotta in forma di revisione scientifica è stata organizzata in due parti: la prima parte ha preso in considerazione gli elementi di successo della tecnologia; (le architetture client-server, l'e-learning e la citologia digitale; il controllo di qualità e la citologia digitale); la seconda parte ha preso in considerazione gli elementi di insuccesso e quelli necessari di approfondimento della tecnologia (corsi specifici in citologia digitale, health technology assessment e citologia digitale, studi in applicazioni di routine in citologia digitale, standards e PACS in citologia digitale, controllo di qualità nelle immagini e studi sulle strategie di navigazione in citologia digitale, applicazioni della realtà virtuale 3D alla citologia digitale). Da un punto di vista globale il lavoro evidenzia gli scenari futuri di azioni relative all'introduzione della tecnologia.

Due to major advances in the information technology, telemedicine applications are ready for a widespread use. Nonetheless, to allow their diffusion in National Health care Systems (NHcSs) specific methodologies of health technology assessment (HTA) should be used to assess the standardization, the overall quality, the interoperability, the addressing to legal, economic and cost benefit aspects. One of the limits to the diffusion of the digital tele-echocardiography (T-E) applications in the NHcS lacking of a specific methodology for the HTA. In the present study, a solution offering a structured HTA of T-E products was designed. The methodology assured also the definition of standardized quality levels for the application. The first level represents the minimum level of acceptance; the other levels are accessory levels useful for a more accurate assessment of the product. The methodology showed to be useful to rationalize the process of standardization and has received a high degree of acceptance by the subjects involved in the study.

Grazie ai grandi progressi nell'information technology le applicazioni di telemedicina sono mature per un uso diffuso. Tuttavia per permettere la loro introduzione nel sistema sanitario nazionale devono essere utilizzate specifiche metodologie di health technology assessment (HTA) per valutare il grado di standardizzazione, la qualità totale, l'interoperabilità, il rispetto dei requisiti legali ed economici e il rapporto costo-beneficio. Con riferimento alla tele-ecocardiografia digitale uno dei limiti è la mancanza di una specifica metodologia di HTA. Nel presente studio, è stata proposta una soluzione che offre un HTA strutturato di prodotti di tele-ecocardiografia (T-E) digitale. La metodologia ha assicurato anche la definizione di livelli standardizzati di qualità per l'applicazione. Il primo livello rappresenta il livello minimo di accettazione; gli altri livelli riguardano aspetti accessori e sono utili per una più accurata valutazione del prodotto. La metodologia si è mostrata di utilità per razionalizzare il processo di standardizzazione ed ha ricevuto un elevato grado di accettazione dei soggetti coinvolti.

Digital tele-echocardiography (T-E) has the potentiality to allow relevant change in health care organization. The purpose of the paper is to analyze the scientific development around the digital T-E to explore the successful applications and individuate the limits which hamper the routine introduction in the National Health Care System (NHCS). A literature review was carried out by searching for studies from 1988 to 2008. The studies have been investigated according to four crucial issues: a) the employment of the digital T-E versus the traditional videotape registrations; b) the evolution of the telecommunication network and T-E; c) the quality assessment of the images after transmission; d) the economical legal and social impact of the T-E. The analysis showed a generalized increased diffusion of the digital T-E thanks to the wonderful development of the information technology. This diffusion was sometimes also accompanied by investigation studies on the diagnostic accuracy and on cost-benefit analysis with special care to the economical and social impact. The study examined some points which need to be improved to allow a better introduction of the T-E in the NHCS as a routine exam. Among these two were the most important. The first was the lacking of an easy and automatic methodology for the image quality assessment alternative to the currently used methods which are highly complex, expensive and needing a long time to be applied. The second was the lacking of a properly designed methodology for the health technology assessment in T-E, the latter, as it is well known is a very complex and heterogeneus system embedding parts from telematics, bioengineering, and medical physics.

La tele-ecocardiografia (T-E) digitale rappresenta attualmente una delle applicazioni telemediche con più ampie prospettive per il Servizio Sanitario Nazionale. Il presente lavoro è una revisione degli studi di T-E con particolare attenzione sia agli elementi di successo che di insuccesso focalizzando la ricerca sugli studi di interesse effettuati tra il 1988 ed il 2008. Gli studi sono stati organizzati secondo quattro raggruppamenti ritenuti rilevanti: a) la T-E digitale in confronto ai metodi tradizionali basati su videoregistratore; b) lo sviluppo delle reti di telecomunicazione e la T-E; c) la valutazione della qualità delle immagini dopo la trasmissione; d) gli aspetti legale, sociale e economico nella T-E. Da un punto di vista generale l'analisi ha mostrato un incremento generalizzato della diffusione della T-E grazie anche al rapido sviluppo della information technology. Questa diffusione è stata spesso inoltre accompagnata da studi di accuratezza diagnostica e di analisi costo-beneficio con particolare attenzione agli aspetti economici e sociali. L'analisi condotta inoltre ha individuato alcuni punti che devono essere migliorati per permettere una più capillare introduzione della T-E nel Sistema Sanitario Nazionale. Tra questi due sono i più rilevanti. Il primo è rappresentato dalla mancanza di una metodologia semplice ed automatica per la valutazione della qualità delle immagini alternativa ai metodi attualmente usati, i quali sono complessi, costosi e necessitano di un elevato tempo di esecuzione. Il secondo è rappresentato dalla mancanza di una metodologia strutturata di health technology assessment in T-E, quest'ultimo lo ricordiamo, è un sistema complesso ed eterogeneo che comprende anche elementi di telematica, di bioingegneria e di fisica medica.

Both in real-time and "store & forward" tele-echocardiography (T-E), a coding process has to be applied to the echocardiography videoclips in order to limit the bandwidth needed and adapt it to the bandwidths furnished by network providers. The compression process degrades the videoclips, affecting thus the quality of the videoclips and potentially compromising the diagnostic accuracy of the T-E. In this work the authors investigated on the use of automatic tools for the video quality assessment by means of objective methods with particular care to the role of the system administrator. As the use of tests on video quality assessment (based on subjective methods) is hampered by the high number of needed resources (persons, laboratories and time). The use of valid objective methods is thus desirable. The study reviewed different tools with this specific aim. One of the more suitable tool was found to be represented by a software package designed by the Institute of Telecommunication Sciences and the National Telecommunication and Information Administration, the NTIA/ITS VQM tool. This tool gives back objective-quantitative data as outcomes, however embeds models emulating the subjective perception. This study reviewed and analyzed in depth the functionalities of the tool to improve the image quality in TE over the network. The tool was also found suitable for a more general process of T-E assessment, from a health technology assessment (HTA) perspective.

In tele-ecocardiografia (T-E), sia quella in modalità di trasmissione "real-time" che "store & forward", deve essere applicato un processo di codifica ai videoclip ecocardiografici provenienti dall'elettrocardiografo, per limitare la banda necessaria alla trasmissione e per adattarsi così alle bande fornite dai fornitori di servizi di rete. Il processo di compressione degrada i videoclip, influenzando quindi la qualità di essi e compromettendo potenzialmente l'accuratezza diagnostica della T-E. Lo studio ha investigato l'uso di strumenti informatici automatici per la valutazione dei videoclip ottenuta tramite metodi oggettivi con particolare attenzione al ruolo dell'amministratore di sistema. Dato che l'uso di test basati sulla valutazione umana è limitato dal numero elevato di risorse disponibili (persone, laboratori, tempo) l'uso di metodi oggettivi è pertanto desiderabile. Lo studio ha rivisitato diversi strumenti con questo obiettivo. Uno degli strumenti più adatti è stato giudicato il pacchetto software disegnato dall' Institute of Telecommunication Sciences and the National Telecommunication and Information Administration (USA), ossia il "NTIA/ITS VQM tool". Lo studio ha analizzato con successo le funzionalità dello strumento con particolare riferimento al miglioramento della qualità delle immagini in T-E. Lo strumento è stato ritenuto anche adatto ad essere utilizzato in un più generale processo di valutazione del sistema di T-E secondo una prospettiva tipica dell'health technology assessment.

A cytogenetic study was conducted on an Astyanax eigenmanniorum population from the Caetano Stream (18° 44' 56" S/ 048° 18' 39" W) - in Uberlândia, MG, Brazil - showing a modal diploid number of 48 chromosomes in the standard male and female karyotypes. However, in several specimens it was also possible to observe metaphases with one or two B chromosomes, increasing the diploid number to 49 or 50 chromosomes, respectively. The supernumerary chromosomes were totally heterochromatic and highlighted after C-banding. The silver-stained nucleolus organizing regions (Ag-NORs) were located in at least five chromosomes of the standard karyotype, thus characterizing a multiple NOR system in the species. This is the first occurrence of an A. eigenmanniorum population with 2n = 48 chromosomes, bearing supernumerary chromosomes.
