Objectives To compare three methods of intratubular contamination that simulate endodontic infections using confocal laser scanning microscopy (CLSM). Material and Methods Two pre-existing models of dentinal contamination were used to induce intratubular infection (groups A and B). These methods were modified in an attempt to improve the model (group C). Among the modifications it may be included: specimen contamination for five days, ultrasonic bath with BHI broth after specimen sterilization, use of E. faecalis during the exponential growth phase, greater concentration of inoculum, and two cycles of centrifugation on alternate days with changes of culture media. All specimens were longitudinally sectioned and stained with of LIVE/DEAD® for 20 min. Specimens were assessed using CLSM, which provided images of the depth of viable bacterial proliferation inside the dentinal tubules. Additionally, three examiners used scores to classify the CLSM images according to the following parameters: homogeneity, density, and depth of the bacterial contamination inside the dentinal tubules. Kruskal-Wallis and Dunn’s tests were used to evaluate the live and dead cells rates, and the scores obtained. Results The contamination scores revealed higher contamination levels in group C when compared with groups A and B (p<0.05). No differences were observed between group A and B (p>0.05). The volume of live cells in group C was higher than in groups A and B (p<0.05). Conclusion The new protocol for intratubular infection resulted in high and uniform patterns of bacterial contamination and higher cell viability in all specimens when compared with the current methods.

Objective: To evaluate the response of rat subcutaneous tissue in implanted polyethylene tubes that were filled with GMTA Angelus and Portland cements containing different arsenic concentrations. Material and Methods: Atomic absorption spectrophotometry was utilized to obtain the values of the arsenic concentration in the materials. Thirty-six rats were divided into 3 groups of 12 animals for each experimental period. Each animal received two implants of polyethylene tubes filled with different test cements and the lateral of the tubes was used as a control group. After 15, 30 and 60 days of implantation, the animals were killed and the specimens were prepared for descriptive and morphometric analysis considering: inflammatory cells, collagen fibers, fibroblasts, blood vessels and other components. The results were analyzed utilizing the Kuskal-Wallis test and the Dunn's Multiple test for comparison (p<0.05). Results: The materials showed, according to atomic absorption spectrophotometry, the following doses of arsenic: GMTA Angelus: 5.01 mg/kg, WPC Irajazinho: 0.69 mg/kg, GPC Minetti: 18.46 mg/kg and GPC Votoran: 10.76 mg/kg. In a 60-day periods, all specimens displayed a neoformation of connective tissue with a structure of fibrocellular aspect (capsule). Control groups and MTA Angelus produced the lower amount of inflammatory reaction and GPC Minetti, the highest reaction. Conclusions: There was no direct relationship between the concentration of arsenic present in the composition of the materials and the intensity of the inflammatory reactions. Higher values, as 18.46 mg/kg of arsenic in the cement, produce characteristics of severe inflammation reaction at the 60-day period. The best results were found in MTA angelus.

This study compared the fluid filtration, adaptation to the root canal walls, and the push-out bond strength of two resin-based sealers and three calcium silicate-based retrograde filling materials. Fifty maxillary canines were shaped using manual instruments and the apical portion was sectioned. Retrograde cavities of 3-mm depth were prepared. The specimens were divided into five groups (n = 10): Sealer 26 (S26); MBPc (experimental); MTA; Portland cement with 20% zirconium oxide (PC/ZO), and Portland cement with 20% calcium tungstate (PC/CT). The fluid filtration was measured at 7 and 15 days. To evaluate the interfacial adaptation, sections of the teeth, 1 and 2 mm from the apex, were prepared and the percentage of gaps was calculated. The push-out bond strength at 2 mm from the apex was evaluated. Statistical analysis was performed using the ANOVA/Tukey’s test (p < 0.05). At 7 and 15 days (p = 0.0048, p = 0.006), the PC/CT group showed higher fluid filtration values when compared to other groups. At 1 mm from the apex, no statistical differences in the adaptation were found among the cements (p = 0.44). At 2 mm from the apex, the PC/ZO group presented statistically lower percentage of gaps than S26, MBPc, and MTA (p = 0.0007). The MBPc group showed higher push-out bond strength than other cements evaluated (p = 0.0008). The fluid filtration and interfacial adaptation of the calcium silicate-based cements were similar to those of the resin-based cements. The resinous cement MBPc showed superior push-out bond strength.

Objective To evaluate the biocompatibility and the setting time of Portland cement clinker with or without 2% or 5% calcium sulfate and MTA-CPM. Material and Methods Twenty-four mice (Rattus norvegicus) received subcutaneously polyethylene tubes filled with Portland cement clinker with or without 2% or 5% calcium sulfate and MTA. After 15, 30 and 60 days of implantation, the animals were killed and specimens were prepared for microscopic analysis. For evaluation of the setting time, each material was analyzed using Gilmore needles weighing 113.5 g and 456.5 g, according to the ASTM specification Number C266-08 guideline. Data were analyzed by ANOVA and Tukey's test for setting time and Kruskal-Wallis and Dunn test for biocompatibility at 5% significance level. Results Histologic observation showed no statistically significant difference of biocompatibility (p>0.05) among the materials in the subcutaneous tissues. For the setting time, clinker without calcium sulfate showed the shortest initial and final setting times (6.18 s/21.48 s), followed by clinker with 2% calcium sulfate (9.22 s/25.33 s), clinker with 5% calcium sulfate (10.06 s/42.46 s) and MTA (15.01 s/42.46 s). Conclusions All the tested materials showed biocompatibility and the calcium sulfate absence shortened the initial and final setting times of the white Portland cement clinker.

OBJECTIVES: To compare, in vivo, the accuracy of conventional and digital radiographic methods in determining root canal working length. MATERIAL AND METHODS: Twenty-five maxillary incisor or canine teeth from 22 patients were used in this study. Considering the preoperative radiographs as the baseline, a 25 K file was inserted into the root canal to the point where the Root ZX electronic apex locator indicated the APEX measurement in the screen. From this measurement, 1 mm was subtracted for positioning the file. The radiographic measurements were made using a digital sensor (Digora 1.51) or conventional type-E films, size 2, following the paralleling technique, to determine the distance of the file tip and the radiographic apex. RESULTS: The Student "t" test indicated mean distances of 1.11 mm to conventional and 1.20 mm for the digital method and indicated a significant statistical difference (p<0.05). CONCLUSIONS: The conventional radiographic method was found to be superior to the digital one in determining the working length of the root canal.

OBJECTIVE: This study compared the influence of three epoxy resin-based sealers with distinct radiopacities on the observers' ability to detect root canal filling voids during radiographic analysis. MATERIAL AND METHODS: The root canals of 48 extracted maxillary canines were prepared and divided into three groups. Each group was laterally condensed with one sealer (AH Plus®, Acroseal® or a non-radiopaque sealer), and a longitudinal void was simulated in half of the specimens from each group (n=8). Buccolingual radiographs were obtained and randomly interpreted for voids by a radiologist and an endodontist in a blinded fashion. Teeth were cut and inspected under a microscope to confirm the position of void. Differences in sensitivity and specificity between groups and examiners were compared using the Fisher's Exact and McNemar tests, respectively (α=0.05). RESULTS: Significantly lower sensitivity levels (p<0.05) were observed in the coronal portion of fillings performed with both radiopaque sealers. Specificity values for Acroseal® were significantly higher (p<0.05) in the coronal and apical portions of fillings. CONCLUSIONS: The type of root canal sealer can affect the observers' ability to detect root canal filling voids during radiographic analysis of upper single-rooted teeth.

OBJECTIVES: To evaluate the sealing ability of five root canal sealers, including two experimental cements (MBP and MTA-Obtura) using the fluid filtration method. MATERIAL AND METHODS: Teeth were divided into 5 study groups: G1-AH Plus; G2-Acroseal; G3Sealapex; G4-MBP; G5-MTA-Obtura; and two controls. Chemical-mechanical preparation was performed with ProFile rotary nickel-titanium instruments 1 mm short of the apical foramen. The sealing ability was evaluated by fluid filtration at 15, 30, and 60 days. RESULTS: The statistical analysis showed significant difference between the materials at different periods (p<0.05). AH Plus and MBP had similar leakage values at 15 and 60 days, alternating with significant reduction at 30 days, while the other materials showed progressive increase in leakage values. Acroseal and Sealapex presented the best results at 15 days and the worst at 60 days. CONCLUSIONS: All sealers evaluated presented fluid leakage, with AH Plus and MBP showing the best results at the end of the experimental period. Acroseal, Sealapex, and MTA-Obtura presented increase in leakage values at longer observation periods.

Este estudo avaliou o aumento da área do canal e a espessura de dentina na parede mesial e distal dos canais mesiais de molares inferiores após o uso de Gates Glidden (GG), La Axxess (LA), Orifice Shaper (OS). Um total de 53 canais mesiais de 27 molares inferiores foram incluídos em resina e divididos em 3 grupos. As raízes foram seccionados 3 mm abaixo da junção cemento-esmalte e as imagens foram capturadas antes e depois da instrumentação. O aumento da área do canal em percentagem e a espessura da dentina remanescente nas paredes mesial e furca foram calculados utilizando o software Image Tools. Os dados foram analisados utilizando o Kruskal-Wallis e teste de Dunn. O Nível de significância foi de 5%. Todos os instrumentos promoveram o aumento da área cervical, com diferentes quantidades de remoção da dentina nas paredes mesial e distal do canal radicular. Diferença estatística (p<0,05) foi encontrada entre LA e todos os outros instrumentos após o uso do primeiro instrumento. Quanto à espessura da dentina, não houve diferença significativa (p>0,05) entre as paredes mesial e distal de todos os instrumentos analisados. O LA 35.06 e GG 3 mostraram a menor espessura da parede e sua utilização em canais mesiais de molares inferiores deve ser feito com cuidado.

This study evaluated the increase of the instrumented area and dentin thickness in the mesial and distal aspects of mesial canals of mandibular molars after the use of Gates Glidden (GG), LA Axxess (LA) and Orifice Shaper (OS) instruments. A total of 53 canals from 27 mandibular molars were embedded in resin and divided into 3 groups. The roots were sectioned 3 mm below the cementoenamel junction and the images were captured before and after instrumentation. The increase of the instrumented area in terms of percentages and the remaining dentin thickness, in mm, at the mesial and furcal aspects were calculated using the Image tools software. Data were analyzed using the Kruskal-Wallis and Dunn's test. The significance level was set at 5%. All instruments promoted cervical flaring with different amounts of dentin removal at the mesial and distal aspects of the root canals. Statistically significant difference (p<0.05) was found between LA and all other instruments after using the first instrument. Regarding dentin thickness, no significant difference (p>0.05) was found between the mesial and distal walls in all groups. It may be concluded that LA 35.06 and GG 3 burs produced the thinnest dentin walls, and thus their use in mesial canals of mandibular molars should be done with caution.

OBJETIVES: The aim of this study was to investigate through scanning electron microscopy (SEM) the cleaning of root canal walls after the use of experimental propolis or calcium hydroxide root canal dressings. MATERIAL AND METHODS: Twenty single-rooted teeth were used. After conventional cleaning and shaping procedures and removal of the smear layer with 17% EDTA, the teeth were divided into four groups according to the medication used (N=5): Group I (control) - No drug, Group II - Calcium hydroxide dressing, Group III - Propolis paste A70D and Group IV - Propolis paste D70D. The medications were introduced into the root canals and maintained for 7 days, then removed with a K-file and 5 mL of 1% sodium hypochlorite irrigation. Finally, the canals were flushed with 2 mL of 17% EDTA for 3 min. For SEM analysis, the roots were cleaved and microphotographs from the middle third of the root canal were taken at 750x. The cleaning of the root canal walls was determined by the number of open dentinal tubules as verified with the software Image Tool 3.1. The statistical analysis was performed by ANOVA and Tukey's test (p<0.05). RESULTS: The results showed no statistically significant difference between the calcium hydroxide and propolis groups. CONCLUSIONS: The experimental propolis pastes presented acceptable physical characteristics to be used as intracanal medicaments.

OBJECTIVE: This study used dye leakage assay and scanning electron microscopy to evaluate, respectively, the sealing ability and marginal adaptation of three root-end filling materials used as apical plugs, as well as the possible correlation between these properties. MATERIAL AND METHODS: Ninety-eight single-rooted human teeth were prepared to simulate an open apex. The teeth were allocated to three experimental groups (n = 30), which received a 5-mm thick apical plug of (1) gray MTA AngelusTM, (2) CPM TM and (3) MBPc, and two controls groups (n = 4). After immersion in 0.2% Rhodamine B solution for 48 h, the teeth were sectioned longitudinally and analyzed by Image Tool 3.0 software. The marginal adaptation between apical plugs and the root canal walls were analyzed by SEM. RESULTS: MBPc had significantly less (p<0.05) apical leakage than the other materials. Regarding marginal adaptation, CPM TM showed the best numerical results, though without statistical significance from the other materials (p<0.05). There was no correlation between the two properties. CONCLUSIONS: When used as apical plugs, the tested root-end filling materials had similar marginal adaptation to the dentin walls, but MBPc had the best sealing ability, as demonstrated by the least apical leakage from all tested materials.

OBJECTIVE: The aim of this study was to compare two methodologies used in the evaluation of tissue response to root-end filling materials in rats. MATERIAL AND METHODS: Forty rats were divided into 4 groups: in Groups I and II (control groups), empty polyethylene tubes were implanted in the extraction site and in the subcutaneous tissue, respectively; in Groups III and IV, polyethylene tubes filled with ProRoot MTA were implanted in the extraction site and in the subcutaneous tissue, respectively. The animals were killed 7 and 30 days after tube implantation, and the hemi-maxillas and the capsular subcutaneous tissue, both with the tubes, were removed. Specimens were processed and evaluated histomorphologicaly under light microscopy. The scores obtained were analyzed statistically by the Kruskal-Wallis test (p<0.05). RESULTS: There were no statistically significant differences between the implantation methods (p=0.78033, p=0.72039). It was observed that the 30-day groups presented a more mature healing process due to smaller number of inflammatory cells. CONCLUSIONS: The present study showed no differences in tissue responses as far as the implantation site and the studied period were concerned. Alveolar socket implantation methodology represents an interesting method in the study of the biological properties of root-end filling endodontic materials due to the opportunity to evaluate bone tissue response.

OBJETIVO: Avaliar a radiopacidade produzida por sete cimentos comercialmente disponíveis (AH Plus, Epiphany, Sealapex, Acroseal, Sealer 26, Endomethasone and Endofill) e um em estágio experimental (MBP). METODOLOGIA: Depois de homogeneizados os materiais, o teste de radiopacidade foi executado conforme a especificação nº 57 da ANSI/ADA. Radiografou-se os espécimes junto a uma escala de Alumínio e as imagens resultantes foram digitalizadas. As densidades radiográficas expressas em milímetros de Alumínio (mm Al) foram comparadas por meio da análise de variância e teste de Tukey (α=0.05). RESULTADOS: Os cimentos AH Plus (10 mm Al) e Epiphany (9 mm Al) apresentaram as maiores radiopacidades seguidos pelo Sealapex (8 mm Al), Endofill (7 mm Al), Endométhasone (7 mm Al), MBP (7 mm Al) e Sealer 26 (6 mm Al). Acroseal (5 mm Al) mostrou a menor radiopacidade entre os cimentos testados (P<0.05). CONCLUSÃO: Concluiu-se que as diferentes radiopacidades dos cimentos testados estão acima do nível mínimo recomendado pela especificação nº 57 da ANSI/ADA.

PURPOSE: To evaluate the radiopacity produced by seven commercially available (AH Plus, Epiphany, Sealapex, Acroseal, Sealer 26, Endomethasone and Endofill) and one experimental (MBP) root canal sealers. METHODS: Sealer radiopacity testing was performed according to ANSI/ADA Specification No. 57. Specimens were radiographed with an aluminum stepwedge, and the resulting images were digitalized. Radiographic densities expressed in Aluminum mm (mm Al) were compared using an ANOVA and Tukey's test (α=0.05). RESULTS: AH Plus (10 mm Al) and Epiphany (9.0 mm Al) were the most radiopaque root canal sealers, followed by Sealapex (8 mm Al), Endofill (7 mm Al), Endomethasone (7 mm Al), MBP (7 mm Al) and Sealer 26 (6 mm Al). Acroseal (5 mm Al) showed the smallest radiopacity among the tested sealers (P<0.05). CONCLUSION: The radiopacities of the tested sealers were above the minimum level recommended by ANSI/ADA Specification No. 57.

O propósito deste estudo ex vivo foi investigar se dois cimentos obturadores de fórmulas diferentes influenciariam a percepção radiográfica de obturações de canais radiculares. Os canais radiculares de 48 caninos superiores extraídos foram preparados e divididos em 3 grupos. Em cada grupo os canais foram preenchidos através da condensação lateral da guta-percha e de um dos cimentos testados (Endométhasone, Sealer 26 e cimento não-radiopaco), e um defeito de ponta a ponta foi simulado em metade dos espécimes de cada grupo (n=8). As imagens radiográficas vestíbulo-linguais obtidas foram aleatoriamente interpretadas quanto à existência de defeitos por um radiologista e um endodontista. As diferenças em sensibilidade e especificidade entre os grupos e examinadores foram comparadas respectivamente usando-se os testes Exato de Fisher e McNemar ajustados ao nível de significância de 5%. Ambos os cimentos radiopacos ocasionaram uma redução significativa da sensibilidade na parte cervical das obturações. O uso do Endométhasone aumentou os valores de especificidade para as porções cervical e apical das obturações dos canais radiculares. Concluiu-se que os cimentos testados influenciaram de maneira distinta a percepção radiográfica de obturações endodônticas executadas com condensação lateral.

The aim of this ex vivo was to investigate if two radiopaque root canal sealers with different formulations would influence the radiographic perception of root canal fillings. The root canals of 48 extracted maxillary canines were prepared and randomly assigned to 3 groups of 16 specimens each. In each group, the root canals were filled by lateral condensation of gutta-percha and one of the tested sealers: Endométhasone, Sealer 26, or a non-radiopaque sealer. A through-and-through void was simulated in half of the specimens from each group (n=8). The buccolingual radiographic images obtained were randomly interpreted for voids existence by a radiologist and an endodontist. The differences in sensitivity and specificity between groups and examiners were compared using, respectively, Fisher's Exact and McNemar tests at 5% significance level. Both radiopaque sealers caused a significant decrease in sensitivity at the coronal part of fillings. The use of Endométhasone increased specificity values for both coronal and apical portions of the root canal fillings. In conclusion, the tested sealers influenced the radiographic perceptions of laterally condensed root canal fillings in a different way.

Um novo método para irrigação, que apresenta ativação hidrodinâmica baseado na tecnologia de pressão e aspiração, foi recentemente introduzido no mercado: o sistema RinsEndo. O presente estudo comparou a eficácia do sistema RinsEndo e irrigação convencional(manual-dinâmica) na remoção de debris do canal radicular, empregando-se microscopia eletrônica de varredura (MEV). Vinte pré-molares inferiores com raiz completamente formada foram selecionados e divididos aleatoriamente em grupo 1 (sistema RinsEndo) e grupo 2 (irrigação convencional). Os canais foram irrigados com 1 mL de solução salina a cada troca de instrumento. A instrumentação foi iniciada com uma lima tipo K #15 dilatando-se até lima tipo K #40, no comprimento de trabalho. Posteriormente os dentes foram clivados no sentido vestíbulo-lingual e as hemisecções foram metalizadas com ouro e analisadas em MEV. Os terços cervical, médio e apical foram avaliados, e os resultados foram analisados estatisticamente pelo teste de Mann-Whitney para comparação entre métodos, e o teste Kruskal-Wallis para comparação entre terços e o teste de Miller para os confrontos individuais. Foi empregado o nível de significância de 5% para todas as análises. Os resultados não mostraram diferença estatisticamente significante (p>0,05) entre métodos em cada terço, nem entre terços para cada técnica analisada individualmente. Concluiu-se que não houve diferença na capacidade de limpeza do sistema RinsEndo e da irrigação convencional.

A new device for irrigation, which presents hydrodynamic activation based on the pressure-suction technology, has recently been introduced to the market: the RinsEndo system. This study compared the efficacy of the RinsEndo system and conventional (manual-dynamic) irrigation in the removal of debris from the root canal walls, using scanning electron microscopy (SEM). Twenty mandibular premolars with completely formed roots were selected and randomly divided into group 1 (irrigation with the RinsEndo system) and group 2 (conventional irrigation). The canals were irrigated with 1 mL of saline at each change of instrument. Instrumentation started with a #15 K file and continued up to a #40 K file, which was standardized as the working length instrument. Then, the teeth were sectioned in buccolingual direction and the halves were sputter-coated with gold and examined by SEM. The apical, middle and cervical root canal thirds were evaluated, and the results were analyzed statistically by the Mann-Whitney test for comparison between methods, Kruskal-Wallis test for comparison among thirds, and Miller test for individual comparisons. A significance level of 5% was set for all analyses. The results did not show significant differences (p>0.05) between methods at each third and among thirds for each technique analyzed individually. In conclusion, there was no difference in the cleaning ability of the RinsEndo system and conventional irrigation.

O canino inferior é tido como um dente unirradiculado e com apenas um canal. Todavia, pode ocorrer o aparecimento de dois canais e, mais raramente, duas raízes. Este artigo apresenta o caso clínico de uma paciente com ambos os caninos inferiores apresentando duas raízes e dois canais radiculares. A radiografia inicial periapical dos caninos mandibulares inferiores direito e esquerdo para tratamento endodôntico revelou a presença de duas raízes em cada um deles. Uma vez realizada a abertura coronária, o preparo do terço cervical foi feito com a lima SX do sistema ProTaper® e a odontometria foi determinada e confirmada eletronicamente com o localizador apical eletrônico Root ZX. O preparo foi finalizado com a série de instrumentos do sistema ProTaper® e a obturação foi realizada com cones de guta-percha e cimento obturador resinoso sob a técnica Híbrida de Tagger. As radiografias finais mostraram dois canais bem obturados terminando nos limites apicais estabelecidos pelos localizadores apicais eletrônicos. O controle de 6 meses mostrou sucesso clínico e radiográfico do tratamento endodôntico. Os clínicos devem sempre considerar a presença de variações anatômicas em todo tratamento endodôntico. A despeito da baixa prevalência, pode haver variações no número de raízes e canais de caninos inferiores, como demonstrado neste caso clínico.

The mandibular canine is usually considered a single-rooted tooth with a single root canal. However, two canals and more rarely two roots may also occur. This paper reports the case of a patient with bilateral mandibular canines with two roots and two root canals. The initial periapical radiographs of the mandibular right and left canines for endodontic treatment revealed the presence of two roots in each tooth. After coronal opening, the cervical third was prepared with a SX file of the ProTaper® system and root canal length was confirmed using Root ZX electronic apex locator. Root canal preparation was completed with the series of ProTaper® instruments and the root canal was filled with gutta-percha and an epoxy resin-based endodontic sealer according to Tagger's hybrid technique. The final radiographs showed two well-obturated canals ending at the electronically located apexes. The 6-month posttreatment follow-up showed apparent clinical and radiographic success. Clinicians should always consider the presence of anatomical variations in the teeth during endodontic treatments. Despite the low prevalence, variations may occur in the number of roots and root canals of mandibular canines, as demonstrated in this case report.
