SUMMARY OBJECTIVE: The aim of this study was to investigate the levels of physical activity (PA) and quality of life (QOL) in adults and elderly individuals with lower limb amputation (LLA). METHODS: This was a cross-sectional observational study. Participants completed three surveys as follows: a demographic survey, the International Physical Activity Questionnaire, and the World Health Organization Quality of Life. Thirty-six individuals with lower limb amputation were separated into two different groups as follows: Adults-lower limb amputation (n=12), composed of individuals with lower limb amputation who aged from 18–59 years, and Elderly-lower limb amputation (n=24), composed of individuals with lower limb amputation who aged 60 years and above. Statistical differences were determined as p<0.05. RESULTS: Age and number of individuals with a low level of functional independency were higher in the Elderly-lower limb amputation group (p<0.05). The International Physical Activity Questionnaire scores were reduced in the Elderly-lower limb amputation group (p<0.05). The Pearson's correlation test between low metabolic equivalent task (MET), time since amputation, and family income presented positive significant results in the Elderly-lower limb amputation (p<0.05). Adults-lower limb amputation just presents a positive significant correlation with the low family income (p<0.05). CONCLUSION: Elderly individuals with lower limb amputation are more susceptible to present negative health outcomes than adults with lower limb amputation.

Human papillomavirus (HPV) infection is the most common sexually transmitted disease worldwide and there is a strong link between certain high-risk viral types and cervical carcinogenesis. Although there are several typing methods, it is still unclear which test is the best. This study compared the effectiveness of type-specific PCR (TS-PCR) and sequencing, with a focus on their clinical application. A total of 260 cervical samples from HPV-positive patients were tested for types 6, 11, 16, 18, 31, 33 and 35 using TS-PCR and sequencing. The genotype was identified in 36% of cases by TS-PCR and in 75% by sequencing. Sequencing was four times more likely to identify the viral type in positive samples than TS-PCR (p = 0.00). Despite being more effective for virus genotyping, sequencing was unable to identify viral types in multiple infections. Combining both techniques resulted in highly sensitive detection (87% of cases), showing that they are complementary methods. HPV genotyping is an important step in HPV management, helping to identify patients with a higher risk of developing cervical cancer and contributing to the development of type-specific vaccines.

A infecção pelo vírus da imunodeficiência humana (HIV) tem aumentado significativamente entre mulheres de meia-idade. Nesta revisão foi feito um levantamento de estudos recentes que buscam identificar possíveis fatores de risco associados à infecção pelo HIV em mulheres climatéricas. São abordados diversos fatores associados, como: sintomas climatéricos, mudança de comportamento do parceiro sexual frente a novos medicamentos, negociação sobre o uso de preservativos em relações sexuais, comportamento de risco para HIV, autoestima global e autoestima sexual, história pregressa de abuso sexual, uso de drogas, estereótipo da vida sexual na maturidade, uso de terapia antirretroviral e abordagem da função sexual.

Human immunodeficiency virus (HIV) infection has increased considerably among middle-aged women. In this work we reviewed recent studies aimed at identifying possible factors related to HIV infection in climacteric women. Several associated factors are considered, such as: climacteric symptoms, partner's change in sexual behavior in face of new drugs, negotiation for use of condoms in sexual intercourse, risk behavior for HIV, global self-esteem and sexual self-esteem, history of sexual abuse, use of drugs, stereotypes of sex life in maturity, use of antiretroviral therapy and approach to sexual function.

OBJETIVO: avaliar a prevalência do HPV e seus genótipos, utilizando a técnica de reação em cadeia de polimerase (PCR), em pacientes soropositivas e soronegativas para o HIV. MÉTODOS: estudo transversal no qual foram avaliadas 79 mulheres. Dentre elas, 41 eram portadoras do vírus HIV constituindo o grupo estudado, e 38 soronegativas para o HIV, atendidas em uma Unidade Básica de Saúde, e compuseram o grupo controle. Todas as participantes foram submetidas a teste sorológico para detecção do HIV e procuraram de forma espontânea, e pela primeira vez, o atendimento de ginecologia nos serviços mencionados. Estas mulheres responderam ao questionário padrão e foram submetidas a exame ginecológico, com coleta de material da cérvice uterina para citologia oncótica e para detecção do DNA-HPV e seus genótipos. Para análise estatística realizaram-se os testes de Kruskal-Wallis, do chi2 ou o exato de Fisher. Foi considerada significância estatística p<0,05. RESULTADOS: os dois grupos analisados demonstraram semelhanças quanto às características demográficas, antecedentes obstétricos e ginecológicos, exceto DST prévia. Diferenciaram-se quanto ao exame ginecológico atual e à citologia oncótica cervical. Houve diferença significante em relação à presença do DNA-HPV nos dois grupos (p<0,05). Das pacientes infectadas pelo HIV, 73,2% apresentaram resultado positivo para o DNA-HPV, comparado a 23,8% dentre as soronegativas (OR=8,79; IC 95%: 2,83-28,37). Em relação aos genótipos não se percebeu nítido predomínio de um subtipo específico de HPV em relação aos grupos infectados ou não pelo HIV, assim como foi similar a freqüência dos tipos não identificáveis. Constatou-se predominância não significante (p>0,05) da infecção múltipla por HPV nas soropositivas (50,0%), e a combinação mais freqüentemente encontrada foi a dos tipos 6, 11 e 16. A infecção simples por HPV nas soronegativas ocorreu em 66,6% das pacientes. O tipo 16 foi o mais freqüentemente encontrado, representando 44,4% do total das infecções simples nos dois grupos. CONCLUSÕES: as mulheres soropositivas pelo HIV apresentaram maior prevalência do DNA-HPV na cérvice uterina, em relação às soronegativas. Não houve maior predominância de tipos específicos de HPV quando os dois grupos foram comparados. Houve tendência de infecção por múltiplos tipos de HPV nas portadoras de HIV, ao passo que a infecção simples predominou nas soronegativas.

PURPOSE: to estimate the prevalence of HPV and its genotypes in HIV-infected and non-infected women, using the Polymerase Chain Reaction (PCR) technique. METHODS: a sectional study with 79 enrolled women: a study group, with 41 HIV-infected women, and a control group, with 38 non-infected women attended at a Basic Health Unit. All were submitted to a serologic test for the detection of HIV and spontaneously looked for gynecological attendance at those units, for the first time. They answered a standard questionnaire and were submitted to a gynecological examination with a cervical swab and specimen for the detection of DNA-HPV and its genotypes. Statistical analysis was performed using Kruskal-Wallis, chi2 or Fisher's exact tests. Statistical significance was considered at p<0.05. RESULTS: the demographic characteristics, obstetric and gynecological previous history were similar in both groups except for previous STD, but different as to the gynecological examination and cervical cytological analysis. The presence of DNA-HPV was significantly different (p<0.05) in the two groups. Among HIV-infected patients, 73.2% presented DNA-HPV positive results, as compared with 23.8% of non-infected women (OR=8.79; 95% IC: 2.83 28.37). Concerning HPV genotypes, there was no clear predominance of a specific HPV subtype in the HIV-infected or in the HIV non-infected groups, and the frequency of unidentified types was similar in both groups. Non-significant predominance of HPV multiple infections (p>0.05) was detected in the HIV-infected women (50.0%) and the most frequently found combination was of types 6, 11 and 16. HPV simple infection occurred in 66.6% of HIV-non-infected patients. The most frequent type found in both groups was 16, representing 44% of all the simple infections in both groups. CONCLUSIONS: HIV-infected women showed higher DNA-HPV prevalence in the uterine cervix, as compared to non-infected women. There was no difference in the predominance of specific types of HPV when both groups were compared. There was a tendency to HPV multiple infections in the HIV-infected women, whereas simple infection predominated in the non-infected patients.

OBJETIVOS: verificar se a contagem de linfócitos T CD4+ e a carga viral do HIV têm influência na presença de lesões intra-epiteliais cervicais (SIL). MÉTODOS: estudo transversal, no qual foram selecionadas 134 mulheres HIV-positivas, todas submetidas à biópsia do colo uterino, quantificação da carga viral do HIV e contagem de linfócitos T CD4+. Os valores laboratoriais da quantificação da carga viral e da contagem de linfócitos T CD4+ foram obtidos antes da realização da biópsia, tendo sido estabelecidos cortes para o estudo da carga viral (<400 cópias/mL; 401 a 50.000 cópias/mL; >50.000 cópias/mL) e contagem de linfócitos T CD4+ (<200 células/mm³; 200 a 350 células/mm³; >350 células/mm³). Foram realizados os testes chi2, chi2 de tendência linear, chi2 de Mantel-Haenszel e análise de variância. Estabeleceu-se significância estatística para p<0,05 e intervalo de confiança a 95%. RESULTADOS: não houve tendência de risco para as mulheres HIV-positivas apresentarem SIL com o aumento da carga viral ou diminuição dos linfócitos T CD4+. Comparando-se a carga viral com a presença ou ausência de SIL, estratificada pelo tempo em que foi quantificada, houve diferença significante para valores acima de 400 cópias/mL (OR: 3,17; IC 95%: 1,02-9,93; p=0,048). Nenhuma associação foi encontrada para a contagem de linfócitos T CD4+ com a presença da SIL. CONCLUSÃO: as pacientes com carga viral do HIV maior que 400 cópias/mL, quantificada antes da biópsia do colo uterino, apresentaram chance 3,17 vezes maior de desencadear SIL. A contagem de linfócitos T CD4+ não influenciou no aparecimento da SIL.

PURPOSE: to evaluate CD4+ T lymphocyte cell count and HIV viral load influence on the presence of cervical squamous intraepithelial lesions (SIL). METHODS: cross-sectional study of 134 HIV-infected women submitted to uterine cervical biopsy, HIV viral load quantification and CD4+ T lymphocyte cell count. Viral load and CD4+ T lymphocyte cell count were performed before biopsy timing. Three different levels of viral load (<400 copies/mL; 401 to 50,000 copies/mL; >50,000 copies/mL) and CD4+ T lymphocyte count (<200 cells/mm³; 200 to 350 cells/mm³; >350 cells/mm³) were defined. Data were statistically analyzed by the chi2 test, linear tendency chi2 test, Mantel-Haenszel test, and analysis of variance, with level of significance set at p<0.05 and 95% confidence interval. RESULTS: there was no risk tendency for HIV-infected women to show SIL with viral load level increase or CD4+ T lymphocyte reduction. Comparing viral load with the presence or absence of SIL, stratified by quantification timing, there was a significant difference for values over 400 copies/mL (p=0.048; OR: 3.17; 95% CI: 1,02-9.93). No association was found between CD4+ T lymphocyte cell count and SIL. CONCLUSION: patients with HIV viral load higher than 400 copies/mL, performed before uterine cervical biopsy, showed a 3.17 times greater chance to develop SIL. CD4+ T lymphocyte count had no influence on the development of SIL.

OBJETIVOS: identificar as alterações ultra-estruturais glomerulares em pacientes com pré-eclâmpsia grave, assim como avaliar a evolução destas lesões e sua relação com o momento de realização da biópsia renal. MÉTODOS: dentre as 72 gestantes com síndrome hipertensiva que se submeteram à biópsia renal no puerpério, em 39 o material obtido mostrou-se adequado para exame à microscopia eletrônica de transmissão, assim distribuídas: 25 eram portadoras de pré-eclâmpsia pura e 14 tiveram diagnóstico de pré-eclâmpsia superposta à hipertensão arterial crônica. As apresentações morfológicas estiveram representadas por rim normal, edema das células endoteliais, expansão do mesângio, interposição mesangial, depósitos subendoteliais e fusão dos pés dos podócitos. RESULTADOS: as alterações mais freqüentes nos dois grupos foram os depósitos subendoteliais e a fusão dos pés dos podócitos. O edema da célula endotelial ocorreu em 84% das pacientes com pré-eclâmpsia pura e 92,9% das pacientes com pré-eclâmpsia superposta. Não foi observada associação entre a gravidade da doença e a intensidade do edema da célula endotelial. Verificou-se tendência de interposição mesangial em pacientes biopsiadas após o sétimo dia pós-parto. A fusão dos podócitos apresentou associação significativa com a proteinúria de 24 horas. CONCLUSÕES: as alterações glomerulares citadas representam espectro de lesões complexas e dinâmicas que, em conjunto, constituem as características ultra-estruturais da pré-eclâmpsia, que não deve, pois, ser identificada somente pela presença do edema da célula endotelial.

PURPOSE: to investigate the glomerular alterations in patients with severe preeclampsia, as well as to evaluate the evolution of these lesions, relating them to the moment of the renal biopsy. METHODS: seventy-two pregnant women with hypertensive syndrome underwent renal biopsy in the puerperium. Appropriate samples for electron microscopic examination were obtained from 39 patients and grouped as follows: 25 with preeclampsia and 14 with superimposed preeclampsia. Biopsy findings were classified into: normal kidney, endothelial cell edema, mesangial expansion, mesangial interposition, subendothelial fibrinoid deposits, and podocyte fusion. RESULTS: the most frequent alterations found in both groups were subendothelial fibrinoid deposits and podocyte fusion. Endothelial edema was present in 84% of the preeclampsia patients and in 92.9% of the superimposed preeclampsia cases. There was no association between the degree of hypertension and the severity of endothelial edema. A tendency to mesangial interposition was observed in patients who had a biopsy after the seventh day after delivery. Podocyte fusion showed a significant association with 24-hour proteinuria. CONCLUSIONS: the above mentioned glomerular alterations represent a spectrum of complex and dynamic lesions that together represent the ultrastructural characteristics of preeclampsia which should no longer be diagnosed based only on the presence or absence of endothelial edema.

OBJETIVO: avaliar a taxa de detecção do papilomavírus humano (HPV) de alto risco oncogênico em pacientes portadoras de neoplasia intra-epitelial cervical (NIC) e verificar se existe associação entre a presença do vírus e a carga viral com a gravidade da lesão cervical, assim como qual o melhor ponto de corte para predizer a gravidade desta lesão. MÉTODOS: estudo de corte transversal, no qual foram selecionadas 110 mulheres com citologia e/ou biópsia com diagnóstico de NIC. Todas foram submetidas à coleta de nova citologia oncológica, captura híbrida II (CH II), colposcopia e conização pela cirurgia de alta freqüência com alça. RESULTADOS: a taxa global de detecção do HPV de alto risco oncogênico na população estudada foi de 77,3%. À avaliação histopatológica, 81 (73,7%) mulheres apresentavam NIC e, nestas mulheres, a taxa de detecção do DNA-HPV foi de 87,6%, sendo de 85,9% nas mulheres com NIC 2 ou 3. A CH II apresentou sensibilidade de 87,7%, especificidade de 56%, valor preditivo positivo de 86,6% e valor preditivo negativo de 58,3%, com odds ratio (OR) de 7,76 (2,47 < OR < 25,15) no diagnóstico de NIC 2 ou 3. Com ponto de corte de carga viral de 20 pg/mL, selecionado a partir da curva ROC, o valor preditivo positivo da CH II no diagnóstico da NIC 2 ou 3, na população estudada, foi de 81,3%. CONCLUSÕES: a taxa de detecção do DNA-HPV nas pacientes portadoras de NIC foi de 77,3% e, em mulheres com NIC 2 e 3, foi de 85,9%. O melhor ponto de corte da carga viral para a predição da lesão cervical de alto grau foi de 20 pg/mL. Acima deste ponto a probabilidade de detecção do HPV de alto risco oncogênico é maior que 80%.

PURPOSE: to evaluate the high-risk oncogenic human papillomavirus (HPV) detection rate of patients with cervical intraepithelial neoplasia (CIN), checking the association between high-risk HPV, viral load and severity of the lesion, as well as the best viral load cutoff to predict lesion severity. METHODS: this is a cross-sectional study. One hundred and ten patients were selected by cytology and/or biopsy with CIN diagnosis. All of them were submitted to a new oncologic cytology, hybrid capture II (HC II), colposcopy, and loop electrosurgical excision and fulguration procedures (LEEP). RESULTS: the global detection rate of high-risk oncogenic HPV in these women was 77.3%. Eighty-one women (73.7%) had CIN with a detection rate of HPV-DNA of 87.6%. In women with CIN 2 or 3 the detection rate was 85.9%. HC II had a sensitivity of 87.8%, specificity of 56.0%, predictive positive value of 86.6% and predictive negative value of 58.3%, with an odds ratio of 7.76 (2.47 < OR < 25.15) for CIN 2 or 3 diagnosis. Using a receiver operator characteristic curve a viral load cutoff was set at 20 pg/mL in this population, with a predictive positive value of 81.3%. CONCLUSIONS: HPV DNA detection rate of patients with CIN was 77.3%. In women with CIN 2/3 it was 85.9%. The best viral load cutoff to predict cervical lesion severity was 20 pg/ml. Above this level the probability of high-risk oncogenic HPV detection is greater than 80%.

OBJETIVO: mostrar, longitudinalmente, os perfis e os parâmetros do ritmo pressórico e da freqüência cardíaca (FC) de grávidas normotensas nos três trimestres da gestação pela monitorização ambulatorial da pressão arterial (MAPA). MÉTODOS: o estudo, longitudinal e aleatório, incluiu 23 grávidas normotensas com média de idade de 23,3 ± 3,9 anos, recrutadas no pré-natal da Maternidade-Escola Hilda Brandão da Santa Casa de Belo Horizonte/MG. Realizou-se MAPA em cada trimestre (até a 14ª semana, entre a 18ª e a 28ª semana e entre a 32ª e a 40ª semana), utilizando o monitor SpaceLabs, modelo 90207. RESULTADOS: Observou-se aumento significativo (p<0,01) nas pressões arteriais sistólica (115 e 104 mmHg), diastólica (73 e 61 mmHg) e média (87 e 77 mmHg), diurna e noturna, respectivamente, no terceiro trimestre da gestação. A pressão arterial média (PAM) e a FC materna diurnas (PAM: 83, 84 e 87 mmHg; FC: 94, 95 e 93 bpm) foram significativamente maiores que as noturnas (PAM: 72, 72 e 77 mmHg; FC: 74, 79 e 79 bpm), nos três trimestres. A freqüência cardíaca materna diária e a diurna não se alteraram com o evoluir da gravidez. CONCLUSÃO: observamos aumento dos níveis pressóricos no terceiro trimestre da gestação, independente do período do dia avaliado. Quanto à freqüência cardíaca, não foram constatadas diferenças significativas entre os três trimestres na avaliação diária e na diurna. No período noturno verificou-se freqüência cardíaca significativamente inferior no primeiro trimestre quando comparada com os demais.

PURPOSE: to show longitudinally the profiles and parameters of pressure rhythm and heart rate in normotensive pregnant women during the three trimesters of pregnancy. PATIENTS AND METHODS: the longitudinal and random study involved 23 normotensive pregnant women, mean age 23.3 ± 3.9 years, recruited from the prenatal clinics of the "Maternidade-Escola Hilda Brandão - Santa Casa de Belo Horizonte/MG". Ambulatory blood pressure monitoring was done every three months (up to 14 weeks, between 18 and 28 weeks and between 32 and 40 weeks, using the SpaceLabs monitor, model 90207. RESULTS: a significant increase (p<0.01) in the 24-h systolic (115 and 104 mmHg), diastolic (73 and 61 mmHg) and average diurnal and nocturnal blood pressures (87 and 77 mmHg respectively), was noticed in the third trimester of pregnancy. Mother's diurnal blood pressure and heart rate (83, 84 and 87 mmHg; 94, 95 and 93 bpm) were significantly higher than the nocturnal measurements (72, 72 and 77 mmHg; 74, 79 and 79 bpm), in the three trimesters. Mother's heart frequency did not change during progress of pregnancy. CONCLUSION: increase in blood pressure during the third trimester of pregnancy could be shown. Maternal daily and diurnal heart rate did not change when the three trimesters were compared. The nocturnal heart rate was significantly lower in the first trimester as compared to the other trimesters.

OBJETIVO: apresentar os achados ginecológicos mais freqüentes de uma coorte de 300 mulheres infectadas pelo HIV, acompanhadas ambulatorialmente. MÉTODOS: estudo prospectivo e descritivo de mulheres HIV-soropositivas atendidas em clínica ginecológica, no período de novembro de 1996 a dezembro de 2002. As pacientes foram submetidas ao protocolo de atendimento ginecológico que incluiu anamnese, exame ginecológico, colpocitologia oncótica (Papanicolaou), pesquisa de HPV (PCR) e colposcopia. A biópsia cervical, quando necessária, foi realizada. Os dados foram armazenados e analisados no Epi-Info, versão 6.0. RESULTADOS: a idade média foi 34,5 anos. Destaca-se o pequeno número de parceiros sexuais, média de três parceiros, e a predominância do contágio heterossexual: 271 (90,6%) pacientes adquiriram o vírus por meio do contato sexual com seus parceiros. Foi alta a prevalência de neoplasias intra-epiteliais cervicais (NIC), representando 21,7% do total da amostra. Dentre as 109 pacientes submetidas a PCR encontraram-se 89 (81,7%) com algum genótipo do HPV. Esfregaço inflamatório à citologia esteve presente em 69% das pacientes. CONCLUSÕES: a infecção pelo HIV se associa com freqüência a NIC e a processos infecciosos genitais, em especial o HPV.

PURPOSE: to present the most frequent gynecologic results in a cohort of 300 outpatient HIV-infected women. METHODS: this is a prospective and descriptive study of HIV-infected women that have been followed up at the gynecological clinic from November 1996 to December 2002. These patients were subjected to a specific protocol which included an interview, a gynecological evaluation and a collecting cervical sample for Pap smear, research of HPV (PCR) and colposcopy. Cervical biopsy was performed when necessary. Data were stored and analyzed by Epi-Info, version 6.0. RESULTS: the mean age was 34.5 years. The small number of sexual partners, average of three partners, and the predominance of heterosexual contagion should be mentioned: 271 (90.6%) patients were contaminated through sexual contact with their partners. There was a high prevalence of cervical intraepithelial neoplasia (CIN) representing 21.7% of the total group. Of 109 patients subjected to PCR, 89 (81.7%) were found to have some HPV genotype. An inflammatory smear was present in 69% of the patients. CONCLUSIONS: there is a high frequency of CIN and genital infectious diseases among HIV-infected women, mainly HPV.

Objetivos: avaliar a acuidade diagnóstica da histerossonografia como método de avaliação da cavidade uterina em pacientes menopausadas com cavidade uterina alterada à ultra-sonografia endovaginal convencional. Métodos: este estudo consistiu na avaliação de 99 pacientes menopausadas com cavidade uterina anormal à ultra-sonografia endovaginal convencional, caracterizada por espessura endometrial maior ou igual a 5 mm em pacientes sem terapia de reposição hormonal, ou espessura endometrial maior ou igual a 8 mm em pacientes em terapia de reposição hormonal, com sangramento irregular. Estas pacientes foram submetidas à histerossonografia e após, foram obtidas amostras para avaliação histopatológica por biópsia dirigida por histeroscopia em 92 pacientes, biópsia endometrial em quatro pacientes e histerectomia em três pacientes. Os resultados da histerossonografia foram comparados com os resultados do exame histopatológico, considerado como "padrão-ouro". Resultados: houve oito casos de cavidade uterina normal e 20 casos de atrofia endometrial e a histerossonografia teve altos níveis de especificidade (97,8 e 97,5%) e baixa sensibilidade (35 e 25%). Houve altos níveis de sensibilidade (92,3 e 75,0%) e especificidade (94,1 e 97,9%) em pólipos (65 casos) e miomas submucosos (quatro casos). Houve três casos de câncer de endométrio e a histerossonografia teve sensibilidade e especificidade de 100%. Conclusões: a histerossonografia mostrou boa acuidade no diagnóstico de doenças focais (pólipos endometriais e miomas submucosos), com altos níveis de sensibilidade e especificidade. Houve três casos de câncer endometrial, e a histerossonografia diagnosticou corretamente todos eles. Mostrou também ser método acurado para excluir anormalidades endometriais. Entretanto, nos casos de espessamento endometrial difuso, a acuidade é baixa, porque endométrios atróficos ou normais freqüentemente aparecem como tendo espessamento difuso à ultra-sonografia endovaginal e à histerossonografia. A histerossonografia não teve complicações durante e após o procedimento.

Purpose: to evaluate the diagnostic accuracy of sonohysterography as a diagnostic method for the evaluation of the uterine cavity in postmenopausal women with abnormal uterine cavity at conventional endovaginal sonography. Methods: this study consisted of the evaluation of 99 postmenopausal patients with abnormal uterine cavity on conventional endovaginal sonography, that was defined as endometrial thickness equal to or larger than 5 mm in a postmenopausal patient not on hormone replacement therapy, or endometrial thickness equal to or larger than 8 mm in patients on hormone replacement therapy, with irregular bleeding. These patients were subjected to sonohysterography, and specimens were obtained for pathologic examination by biopsy guided by histeroscopy in 92 patients, endometrial biopsy in four patientes and hysterectomy in three patients. The results of sonohysterography were compared with the pathologic findings, considered "gold standard". Results: there were eight cases of normal uterine cavity and 20 cases of atrophic endometrium and sonohysterography had high levels of specificity (97.8 and 97.5%) and low sensitivity (35 and 25%). There were high levels of sensitivity (92.3 and 75.0%) and specificity (94.1 and 97.9%) for polyps (65 cases) and submucous myomas (four cases). There were three cases of endometrial carcinoma and the sonohysterography had a sensitivity and specificity of 100%. Conclusions: sonohysterography showed to be accurate in the diagnostic of focal diseases (endometrial polyps and submucous myomas). There were three cases of endometrial cancer, and sonohysterography correctly diagnosed all of them. This method was also accurate to exclude endometrial abnormality. However, in the cases of diffusely thickened endometrium, the accuracy was low, because atrophic and normal endometrium on histopathology frequently appears as diffusely thickened endometrium at endovaginal sonography and sonohysterography. Sonohysterography did not lead to complications during and after the procedure.

Objetivos: acompanhar a incidência de efeitos colaterais e aceitabilidade (taxa de continuidade) do acetato de medroxiprogesterona de depósito (DMPA) como contraceptivo injetável trimestral em adolescentes do nosso meio. Método: quarenta adolescentes (70% lactantes) iniciaram o uso do DMPA, tendo sido acompanhadas por uma média de 14,2 meses. As queixas espontâneas, as alterações menstruais, dados do exame físico e exames laboratoriais foram coletados e analisados pelos testes de Wilcoxon ou McNemar. Resultados: as queixas mais freqüentes foram cólica (16,6%) e cefaléia (15,2%). Os padrões menstruais predominantes foram o spotting e a oligomenorréia. Não foi observada variação significativa da pressão arterial sistólica durante o acompanhamento, mas houve uma ligeira queda nos níveis da pressão arterial diastólica, no limite da significância. Não houve alterações significativas da glicemia de jejum, e foi observada uma tendência ao aumento da taxa de hemoglobina. O ganho de peso (média de 3,9 kg em 12 meses) e a irregularidade menstrual ( > ou = 70% em qualquer retorno) foram os principais motivos para a desistência do método. Vinte e sete pacientes foram acompanhadas por 12 meses e a taxa de continuidade nessa época foi de 81,48%. Conclusão: o acetato de medroxiprogesterona de depósito mostrou-se um contraceptivo satisfatório para as adolescentes.

Purpose: to evaluate the incidence of side effects and acceptance (continuity rate) of depot medroxyprogesterone acetate (DMPA) as an injectable three-monthly contraceptive given to adolescents in our milien. Method: forty adolescents (70% lactating) started to use DMPA and were followed-up for a mean of 14.2 months. Spontaneous complaints, menstrual changes, physical examination and laboratory data were collected and studied using Wilcoxon or McNemar tests. Results: the most frequent complaints were abdominal pain (16.6%) and headache (15.2%). Predominant menstrual patterns were spotting and oligomenorrhea. Significant variation of the systolic blood pressure was not observed during the follow-up. There was a slight fall in the levels of diastolic blood pressure, at the limit of significance. Significant deviations from baseline regarding fasting glucose were not noted, but the mean hemoglobin concentration tended to increase. Weight gain (mean 3.9 kg at 12 months) and menstrual irregularity (occurred in more than 70% of all visits) were the main reasons for discontinuation of the method. Twenty-seven patients were accompanied during 12 months and the continuity rate at that time was 81.5%. Conclusion: depot medroxyprogesterone acetate is a satisfactory contraceptive method for adolescents.
