ABSTRACT Chronic kidney disease (CKD) is highly prevalent and is predicted to become the fifth highest cause of premature mortality globally by 2040. There are several risk factors for CKD and the environmental conditions play an important role on the development of kidney injury. Due to increasing use of earth’s natural resources and pollutants emissions, extreme weather events are becoming more frequent and severe. These events, such as hurricanes or floods, cause infectious diseases outbreaks that increase the risk of acute kidney injury and chronic kidney disease. In addition, rising average temperatures and heat waves cause a higher incidence of acute kidney injury and nephrolithiasis. Patients with CKD are particularly prone to these events, with a special focus on patients undergoing peritoneal dialysis or hemodialysis. Mitigation plans are needed to reduce the effect of climate change on kidney health. Nephrologists have an important role in both reducing the environmental impact of nephrology and in preparing patients for the effect of climate changes on their diseases and for the accessibility problems to health institutions.

ABSTRACT Introduction: Healthcare facilities are among the greatest contributors to resource consumption and waste generation. Nephrology is one of the most polluting medical areas, mostly due to hemodialysis (HD). Incremental hemodialysis (iHD) has gained attention due to several clinical advantages, by decreasing the number of sessions while the patient still has residual kidney function. Beyond these benefits, this regímen might also be both more economical and environmentally friendly. The aim of this study was to evaluate the potential reduction of resource consumption, waste generation and carbon footprint associated with the implementation of iHD. Methods: We conducted a single-center retrospective analysis of 157 incidental patients initiating HD in our centre in 2019 (pre-pandemic). Retrospectively, a set of criteria identifying eligibility for an iHD program were applied to those patients. Results: Twenty-three patients (15%) would have been eligible for iHD instead of starting a standard HD program. In that scenario, iHD implementation would have reduced from 36 to 85 tons of CO2 equivalent emissions and 4186 kg of contaminated waste yearly, at our unit. iHD would also allow savings of almost 60 000€ and a reduction in water consumption of 418 600 L in one year. Conclusion: This study shows that iHD can significantly attenuate the environmental and economic impact of HD. These findings encourage kidneycare programs and policymakers to adopt greener options but should never be detrimental to the clinical decision of referencing a patient to iHD.

ABSTRACT Introduction: Healthcare services have a significant environmental impact (EI). The increasing prevalence of chronic kidney disease makes nephrology one of the clinical areas with higher EI. The aim of this study was to document Portuguese nephrologists’ current practice regarding environmental sustainability. Methods: Two surveys were developed: one for chiefs of departments and another for all nephrologists registered in the Portuguese Society of Nephrology. Results: There were 153 responders (25.5% response rate): 17 (11%) were chiefs of departments, 36 (23.5%) nephrologists in coordination roles, 60 (39%) nephrologists and 40 (26%) nephrology fellows. The median priority attributed to environmental sustainability was 4 (IQR 3-5). Only one department had established a green group, but 13 were currently undertaking at least one green project (76.5%). Only 6% of responders (8/136) had received formal training on this topic, but more than 90% would like to have more information. The most reported barriers to environmental sustainability practices were inadequate information, lack of adequate facilities, concerns about security and legal norms. Only two facilities (11.8%) declared that environmental sustainability was considered in procurement decisions. More than 70% of the Units did not have regular access to any kind of environmental sustainability audit. Only 3 surveyed units (17.6%) have set up systems to reuse the reverse osmosis rejected water and only half had local on-line dialysate production (9/17, 53%). Conclusion: Portuguese nephrologists wish to incorporate environmental sustainability in their daily practice, however there is a big gap between their intention and current behavior, likely because of the identified barriers. ABSTRACT Introduction: With the development of society, the concept of quality has become essential within health care organizations. However, little is known about the impact of the structural component of the Donabedian model on the quality of care provided by the Specialist Nurses in Rehabilitation Nursing. The purpose of this study is to identify and analysis the factors related to the quality of rehabilitation nursing care in Physical Medicine and Rehabilitation Units in the light of the Donabedian model in the structure dimension. Methodology: Case study of a qualitative nature. We used a non-probability convenience sample, with all participants being RNs from the same service. Data were collected through a semi-structured interview, composed of eight open-ended questions and a set of eight questions for socio-demographic and professional characterization. In data analysis, we used the guidelines for content analysis proposed by Bardin (2009) using the Atlas.ti software. Results and Discussion: From the analysis of the speeches of the 16 participants, in light of the Donabedian model in the thematic area Structure, the following categories emerged: people management and leadership in the service and physical environment and conditions for the functioning of nursing practices. The Donabedian model supported the discussion in conjunction with other references. We found discrepancies in the participants' discourses, specifically regarding the work methodologies and workload distribution related to rehabilitation nursing care. The work method emerges as a hampering contribution for some nurses, while, for others, it is an added value for the accountability of the autonomous practices developed. Conclusion: We can conclude that the nurses identify facilitating and complicating factors within the structure of the service, which interfere with the quality of care.

RESUMEN Introducción: Con el desarrollo de la sociedad, el concepto de calidad se ha convertido en esencial dentro de las organizaciones sanitarias. Sin embargo, poco se sabe sobre el impacto del componente estructural del modelo de Donabedian en la calidad de los cuidados prestados por las Enfermeras Especialistas en Enfermería de Rehabilitación. El objetivo de este estudio es identificar y analizar los factores relacionados con la calidad de los cuidados de enfermería de rehabilitación en las Unidades de Medicina Física y Rehabilitación a la luz del modelo de Donabedian en la dimensión estructura. Metodología: Estudio de casos de carácter cualitativo. Se utilizó una muestra no probabilística de conveniencia, siendo todos los participantes EEER del mismo servicio. Los datos se recogieron a través de una entrevista semiestructurada, compuesta por ocho preguntas abiertas y un conjunto de ocho preguntas de caracterización sociodemográfica y profesional. En el análisis de los datos, se utilizaron las directrices para el análisis de contenido propuestas por Bardin (2009) utilizando el software Atlas.ti. Resultados y Discusión: Del análisis de los discursos de los 16 participantes, a la luz del modelo de Donabedian en el área temática Estructura, surgieron las siguientes categorías: gestión de personas y liderazgo en el servicio y entorno físico y condiciones para el funcionamiento de las prácticas de enfermería. El modelo de Donabedian apoyó la discusión junto con otras referencias. Encontramos discrepancias en los discursos de los participantes, específicamente en lo que se refiere a las metodologías de trabajo y a la distribución de la carga de trabajo relacionada con los cuidados de enfermería de rehabilitación. El método de trabajo surge como una contribución obstaculizadora para algunas enfermeras, mientras que, para otras, es un valor añadido para la responsabilización de las prácticas autónomas desarrolladas. Conclusión: Podemos concluir que los enfermeros identifican factores facilitadores y complicadores dentro de la estructura del servicio, que interfieren en la calidad de los cuidados.

RESUMO Introdução: Com o desenvolvimento da sociedade, o conceito de qualidade tornou-se fundamental no âmbito das organizações de saúde. Contudo pouco ainda se sabe sobre o impacto da componente estrutura do modelo de Donabedian na qualidade dos cuidados prestados pelos Enfermeiros Especialistas em Enfermagem de Reabilitação (EEER). O objetivo deste estudo consiste em identificar e analisar os fatores relacionados com a qualidade dos cuidados de enfermagem de reabilitação nas Unidades de Medicina Física e Reabilitação (MFR) à luz do modelo de Donabedian na dimensão estrutura. Metodologia: Estudo de caso de natureza qualitativa. Utilizamos uma amostra não probabilística de conveniência, sendo todos os participantes, EEER do mesmo serviço. Os dados foram recolhidos através de uma entrevista semiestruturada, constituída por oito perguntas abertas e um conjunto de oito perguntas para a caraterização sociodemográfica e profissional. Na análise dos dados utilizamos as orientações para a análise de conteúdo propostas por Bardin (2009) com recurso ao software Atlas.ti. Resultados e Discussão: Da análise dos discursos dos 16 participantes, à luz do modelo de Donabedian na área temática Estrutura emergiram as seguintes categorias: gestão de pessoas e liderança no serviço e ambiente físico e condições para funcionamento das práticas de enfermagem. O modelo de Donabedian suportou a discussão em conjugação com outros referenciais. Encontramos discrepâncias nos discursos dos participantes, especificamente em relação às metodologias de trabalho e de distribuição da carga de trabalho relativa aos cuidados de enfermagem de reabilitação. O método de trabalho surge como contributo dificultador para alguns enfermeiros, enquanto, para outros é uma mais-valia para a responsabilização das práticas autónomas desenvolvidas. Conclusão: Podemos concluir que os enfermeiros identificam fatores facilitadores e dificultadores no âmbito da estrutura do serviço, que interferem com a qualidade dos cuidados.

Abstract Objective: Through a literature review, make recommendations regarding immunizations in people living with Inborn Error of Metabolism (IEM) in Brazil, assess the possible impact on metabolic decompensations after immunization, and if this specific population may have an impaired immune response to vaccines. Source of data: The MeSH Terms vaccination OR vaccine OR immunization associated with the term inborn error of metabolism AND recommendation were used in combination with search databases. Only articles published after 1990, in the languages English, Spanish, French or Portuguese, human-related were included. Synthesis of data: A total of 44 articles were included to make the following recommendations. Individuals with IEMs need to be up to date with their immunizations. Regarding which vaccines should be offered, children and adults should follow the routine immunization schedules locally available, including the COVID-19 vaccines. The only exception is the rotavirus vaccine for hereditary fructose intolerance. The benefit of immunization outweighs the very low risk of metabolic decompensation. Since not all patients will have an adequate immune response, measuring antibody conversion and titers is recommended Conclusions: All patients should receive age-appropriate immunizations in their respective schedules without delays. The only situation when vaccination may be contraindicated is with oral rotavirus vaccine in hereditary fructose intolerance. Monitoring the levels of antibodies should be done to detect any immune dysfunction or the necessity for boosters. A personalized immunization schedule is ideal for patients with IEMs. The reference organizations could improve their recommendations to address all IEMs, not only some of them.

ABSTRACT INTRODUCTION: Adequate knowledge about food and nutrition can be an important factor in supporting healthier food choices. OBJECTIVES: To evaluate the level of knowledge about food and nutrition of students in the 1st cycle of studies in Health Sciences and its associated factors. METHODOLOGY: An observational cross-sectional study was conducted, which included students of the 1st year of Higher Education in Health Sciences in the 2019/2020 school year (n=150). Data were collected through a structured self-completed questionnaire. Knowledge about food/nutrition was assessed using a scale previously tested in the Portuguese population. Categorical variables were compared using the Chi-square test and quantitative continuous variables with the ANOVA test. RESULTS: The mean percentage of correct answers on the food/nutrition knowledge scale was 77.9% (standard deviation=12.57) (range: 36-100%). When analyzing the factors associated with higher food/nutrition knowledge (>90% correct answers), it was found that this was significantly higher in individuals who reported higher average consumption of fruit and vegetables (4.6 vs. 3.2 portion/ day, p=0.019), particularly soup, among students with more educated mothers (12.7 vs. 11.3 average years of schooling, p=0.039), and among students of the Faculty of Health Sciences compared to those in the Higher School of Health (55.0% vs. 45.0%, p=0.018). CONCLUSIONS: The level of food and nutrition knowledge was relatively high in Health Sciences students. Extreme knowledge was associated with higher consumption of fruit and vegetables, higher maternal education, and the College/School they attended.

RESUMO INTRODUÇÃO: Um nível adequado de conhecimentos sobre alimentação e nutrição pode ser um fator importante para suportar escolhas alimentares mais saudáveis. OBJETIVOS: Avaliar o nível de conhecimento sobre alimentação e nutrição de estudantes do 1.º ciclo de estudos da área das Ciências da Saúde e os seus fatores associados. METODOLOGIA: Foi conduzido um estudo observacional transversal, que incluiu estudantes do 1.º ano do Ensino Superior da área das Ciências da Saúde no ano letivo de 2019/2020 (n=150). Os dados foram recolhidos através de um questionário estruturado de autopreenchimento. Os conhecimentos sobre alimentação/nutrição foram avaliados através de uma escala previamente testada na população portuguesa. As variáveis categóricas foram comparadas através do teste de Qui-quadrado e as variáveis quantitativas contínuas através do teste ANOVA. RESULTADOS: A percentagem média de respostas corretas na escala de conhecimentos sobre alimentação/nutrição foi de 77,9% (desvio-padrão=12,57) (variação 36%-100%). Ao analisarmos os fatores associados a um conhecimento em alimentação/nutrição elevado (>90% respostas corretas), verificou-se que este foi significativamente superior nos indivíduos que reportaram um consumo médio mais elevado de fruta e produtos hortícolas (4,6 vs. 3,2 porções/dia, p=0,019), particularmente de sopa, nos estudantes com mães mais escolarizadas (12,7 vs. 11,3 anos médios de escolaridade, p=0,039) e nos estudantes da Faculdade de Ciências da Saúde comparativamente aos da Escola Superior de Saúde (55,0% vs. 45,0%, p=0,018). CONCLUSÕES: Os estudantes da área das Ciências da Saúde mostraram um nível de conhecimentos sobre alimentação/nutrição relativamente elevado. Um maior conhecimento foi associado a um maior consumo de fruta e produtos hortícolas, uma maior escolaridade da mãe e ao local de Ensino (Faculdade/Escola).

ABSTRACT Purpose: to examine the influence of familial environment, parental perception, and socioeconomic conditions on the receptive vocabulary of elementary school children. Methods: an analytical cross-sectional observational study. The students were selected by stratified proportional sampling, using the Receptive Vocabulary Test. Those responsible for the children answered the anamnesis and the inventory of family environmental resources. Initially, a bivariate analysis was performed using the chi-square test, considering p<0.20. In the multivariate analysis of binary logistic regression, p<0.05 was considered, and the quality of the model was evaluated through the Hosmer-Lemeshow method. Results: out of the 263 students, 131 were males, 142 were in the third grade and were on average 7.6 years old (±0.57), and 111 had a low receptive vocabulary. In the bivariate analysis, there was a relationship between parental perception about learning to read and write, school failure, and the outcome (p<0.05). The variables environment, resources, and stability of family life were also used for multivariate analysis (p<0.20). Conclusion: Parental perception of difficulty in reading and writing and a lack of family stability were factors associated with poor receptive vocabulary.

RESUMO Objetivo: verificar a influência do ambiente familiar, percepção parental e condições socioeconômicas no vocabulário receptivo de escolares do ensino fundamental. Métodos: trata-se de estudo observacional transversal analítico. Os escolares, selecionados por amostragem proporcional estratificada, realizaram o Teste de Vocabulário Receptivo. Os responsáveis responderam à anamnese e ao Inventário de Recursos do Ambiente Familiar. Inicialmente, realizou-se análise bivariada pelo teste Qui-quadrado, considerando p<0,20. Na análise multivariada de regressão logística binária, considerou-se o valor p<0,05 e a qualidade do modelo foi avaliada pelo método Hosmer-Lemeshow. Resultados: dos 263 escolares, 131 são do sexo masculino, 142 do terceiro ano, possuem idade média de 7,6 anos (±0,57) e 111 apresentam vocabulário receptivo rebaixado. Na análise bivariada, observou-se relação entre percepção parental acerca do aprendizado da leitura, da escrita, repetência escolar e o desfecho (p<0,05). Também foram para análise multivariada as variáveis recursos do ambiente e estabilidade da vida familiar (p<0,20). Conclusão: a percepção parental de dificuldade de leitura e de escrita e a falta de estabilidade familiar foram fatores associados ao pior vocabulário receptivo.

RESÚMEN: Nuestro trabajo tiene como objetivo analizar los procesos de (trans) formación de futuros profesores de lengua materna en el curso de Letras para prácticas de literacidad didáctico-digitales, a partir de la escritura reflexiva en reseñas críticas. Ubicadas en el campo interdisciplinario de la Lingüística Aplicada, desarrollamos un conjunto de eventos de literacidad en, para y sobre el lugar de trabajo docente durante un Programa de Multiliteracidades Didácticas, que permitió la participación de futuros profesores en proyectos didácticos colaborativos en contextos escolares reales. Los resultados revelaron que se puede construir la literacidad didáctico-digital a partir de las representaciones del ser y actuar de los profesores, orientados a las formas en que estos sujetos significan, proyectan y operan la escritura reflexiva en las prácticas de enseñanza y aprendizaje de la lengua materna para las multiliteracidades didácticas. Argumentamos que estas prácticas pueden colaborar en los usos receptivos y responsables de la escritura en los contextos formativos de los (futuros) maestros.

ABSTRACT: Our work aims to analyze the processes of (trans) formation of future Portuguese language teachers in the Languages course for didactic-digital literacy practices from reflective writing in critical reviews. Situated in the interdisciplinary field of Applied Linguistics, we developed a set of literacy events at, for, and about the teaching workplace during a Didactic Multiliteracies Program, which enabled the engagement of future teachers in collaborative didactic projects in real school contexts. The results revealed that didactic-digital literacy can be constructed from the representations of teaching being and acting, oriented to how these subjects signify, project, and operate reflective writing in the teaching and learning practices of the Brazilian mother tongue for the didactic multiliteracy materials. We argue that these practices can collaborate in the responsive and responsible uses of writing in formative contexts of (future) teachers.

RESUMO: Nosso trabalho visa a analisar processos de (trans)formação de futuros professores de língua materna no curso de Letras para práticas de letramentos didático-digitais, a partir da escrita reflexiva em resenhas críticas. Situadas no campo interdisciplinar da Linguística Aplicada, desenvolvemos um conjunto de eventos de letramentos no, para e sobre o local de trabalho docente durante um Programa de Multiletramentos Didáticos, os quais oportunizaram o engajamento de futuros professores em projetos didáticos colaborativos em contextos escolares reais. Os resultados revelaram que o letramento didático-digital pode ser construído a partir das representações de ser e agir docentes, orientadas para os modos como esses sujeitos significam, projetam e operam a escrita reflexiva nas práticas de ensino e aprendizagem de língua materna para os multiletramentos didáticos. Argumentamos que estas práticas podem colaborar nos usos responsivo e responsável da escrita nos contextos formativos de (futuros) professores.

ABSTRACT OBJECTIVE To assess the availability of different promotional strategies applied for UPF sales in Brazilian food retailers. METHODS Information available on food packaging was gathered from all packaged products sold in the five largest food retail chains in Brazil in 2017. UPF were identified using the NOVA food classification system. From this sample, data related to promotional characteristics, nutrition claims and health claims were collected and coded using the INFORMAS methodology. Additional claims referring to the Brazilian Dietary Guidelines were also collected. RESULTS This study evaluated the packaging of 2,238 UPF, of which 59.8% presented at least one promotional strategy. Almost one third denoted a simultaneous use of different promotional strategies in the same packaging. Nutrition claims were the most commonly found promotional strategy, followed by health claims and the use of characters. The food subgroups comprising the highest prevalence of promotional strategies on their labels were: noncaloric sweeteners (100.0%), breakfast cereals and granola bars (96.2%), juices, nectars and fruit-flavoured drinks (92.9%), other unsweetened beverages (92.9%), and other sweetened beverages (92.6%). CONCLUSIONS Considering the poor nutritional quality of UPF, the widespread presence of promotional features on their packaging highlights the need for marketing restrictions on this kind of product.

Resumo A heterogeneidade e vulnerabilidade da população em situação de rua revelam a complexidade de viver nessa condição e exige que o Estado tenha papel fundamental na oferta de políticas de saúde e assistência social. O objetivo do estudo, a partir de uma revisão integrativa da literatura, foi avaliar a intersetorialidade entre políticas de saúde, proteção social e ações da sociedade civil organizada para a população em situação de rua no Brasil. Foi realizada uma busca nas bases SciELO, Lilacs e PubMed, sendo selecionados dez artigos, publicados entre 2004-2021, todos de caráter qualitativo. Observou-se indícios da intersetorialidade entre as políticas para a população em situação de rua, descritas principalmente como uma ponte de acesso entre os serviços ofertados. Entretanto, as intervenções ainda são pouco exploradas e sistematizadas. Nesse sentido, recomenda-se que estudos de impacto voltados à avaliação de políticas públicas intersetoriais sejam desenvolvidos para que se permita a avaliação da efetividade da intersetorialidade entre os serviços de saúde, assistência social e sociedade civil organizada na qualidade de vida da população em situação de rua.

Abstract The heterogeneous and vulnerable nature of the unhoused population reveal the complexity of living on the street and call for the State to play a fundamental role in the provision of health and social care policies. The scope of this study was to evaluate the intersectorality between health policies, social protection and organized civil society actions for the unhoused population in Brazil, based on an integrative literature review. To achieve this, a search was conducted in the SciELO, LILACS and PubMed databases, from which ten articles published between 2004-2021, all of a qualitative nature, were selected. From the analysis of the chosen articles, the evidence of intersectorality between the policies for the unhoused population was observed, mainly described as a point of intersection between the services offered. However, interventions are still poorly explored and systematized. It is recommended that quantitative studies aimed at the evaluation of intersectoral public policies be developed to allow the assessment of the impact of intersectorality between health services, social assistance and organized civil society on the quality of life of the unhoused population.

Resumo: A Estratégia de Prevenção e Atenção à Obesidade Infantil (PROTEJA) visa promover a implementação de um pacote de ações para prevenção da obesidade no Brasil. Este estudo apresenta as etapas de desenho operacional, proposta geral, avaliação e monitoramento do PROTEJA realizadas pela coordenação técnica do Ministério da Saúde. Trata-se de um estudo qualitativo baseado em análise descritiva documental. A formulação da estratégia envolveu análise de dados epidemiológicos de crianças brasileiras, análise de políticas existentes, e uma revisão da literatura científica e das recomendações O PROTEJA foi então formulado e aprovado pelo Ministério da Saúde e 1.320 municípios se comprometeram a implementar 20 ações essenciais e 5 complementares das 41 ações, incluindo algumas estruturais para melhoria dos ambientes. A estratégia é coordenada pelo Ministério da Saúde em parceria com governos subnacionais e universidades. Sua implementação conta com apoio de uma equipe em nível local, e prevê a avaliação da implementação e de impacto. As ações serão monitoradas anualmente, e os indicadores também implicarão nos incentivos financeiros. O PROTEJA é uma estratégia forte, baseada em evidências e inovadora que visa promover ambientes saudáveis nas cidades, mas sua implementação é desafiadora. Não obstante, apresenta um possível caminho para a reversão da obesidade infantil. Também pode agregar à ciência de implementação e contribuir para o desenvolvimento e aprimoramento de políticas públicas de prevenção da obesidade.

Resumen: La Estrategia Brasileña para la Prevención y Atención de la Obesidad Infantil (PROTEJA) tiene como objetivo promover la implementación de un conjunto de acciones para prevenir la obesidad en Brasil. Este estudio presenta las etapas del diseño operativo, la propuesta general, la evaluación y el seguimiento de PROTEJA realizadas por la coordinación técnica del Ministerio de Salud brasileño. Se trata de un estudio cualitativo basado en el análisis descriptivo de documentos. La formulación de la estrategia implicó el análisis de datos epidemiológicos de niños brasileños, el análisis de las políticas existentes y una revisión de la literatura científica y de las recomendaciones. PROTEJA fue entonces formulado y aprobado por el Ministerio de Salud, y 1.320 municipios se comprometieron a implementar 20 acciones esenciales y 5 complementarias de las 41 acciones, incluidas algunas estructurales para mejorar los entornos. La estrategia es coordinada por el Ministerio de Salud en colaboración con gobiernos subnacionales y universidades. Su implementación cuenta con el apoyo de un equipo a nivel local, y prevé una evaluación de la implementación y de impacto. Las acciones se monitorearán anualmente, y los indicadores también implicarán incentivos económicos. PROTEJA es una estrategia sólida, basada en evidencias e innovadora que busca promover entornos saludables en las ciudades, pero su implementación es un desafío. Sin embargo, presenta una posible vía para revertir la obesidad infantil. También puede agregar a la ciencia de implementación y contribuir al desarrollo y a la mejora de políticas públicas para prevenir la obesidad.

Abstract: The Brazilian Strategy for the Prevention and Care of Childhood Obesity (PROTEJA) aims to implement a set of actions to prevent obesity in Brazil. As such, this qualitative and descriptive documentary study presents the Strategy’s stages of the operational design, general proposal, evaluation and monitoring conducted by the Brazilian Ministry of Health’s technical coordination. After analyzing the epidemiological data on children and the existing policies aimed at obesity prevention, and reviewing the scientific literature and recommendations, PROTEJA was formulated and approved by the Brazilian Ministry of Health, and 1,320 municipalities committed to implementing 20 essential and 5 complementary actions, from 41, including some structural to improve environments. Coordinated by the Brazilian Ministry of Health in partnership with subnational governments and universities, the Strategy also relies on a local team for implementation support, as well as implementation and impact evaluations. Actions will be monitored annually, and the indicators will impact financial incentives. As a strong, evidence-based and innovative strategy aiming to promote healthy environments in cities, PROTEJA has the potential to open a path to childhood obesity reversal, as well as add to the implementation science and contribute to the development and improvement of public policies for obesity prevention; however, its implementation remains a challenge.

ABSTRACT OBJECTIVE This study aims to evaluate the use of “whole grains” claims in food products marketed in Brazil and evaluate the nutrient profile of these products. METHODS Data from 775 grain-based packaged foods collected in Brazil from April to July 2017 were analyzed. Based on the INFORMAS protocol for food labeling, the prevalence of packaged foods with “whole grains” claims was estimated. Information on the list of ingredients was analyzed to evaluate the presence and amount of whole or refined grains in six food groups. The nutrient profiles of the products with and without “whole grains” claims were compared using the Pan American Health Organization (PAHO) nutrient profile model. RESULTS The packages of about 19% of the evaluated products showed “whole grains” claims in their front panel. Of these, 35% lacked any whole grains among their top three ingredients. Breakfast cereals, granola bars, bread, cakes and other bakery products, cookies, and pasta had higher amounts of refined flour than whole grain ingredients in their compositions.We found 66% of products with “whole grains” claims were high in nutrients of concern according to PAHO’s criteria. CONCLUSION Our results showed that over a third of the products in Brazil with “whole grains” claims lacked whole grains as one of their main ingredients. Most had a high content of nutrients associated with noncommunicable chronic disease risk factors, indicating the overestimation of their health benefits.

Um estudo quantificou e caracterizou aditivos alimentares encontrados nos rótulos de 9.856 alimentos e bebidas embalados disponíveis nos supermercados brasileiros. nele, foi adotado um diário de campo para registro de falhas e inconformidades nas informações presentes nas listas de ingredientes, sendo, por fim, analisadas qualitativamente e descritas de forma narrativa. Com base nisso, o objetivo deste comentário é apresentar e discutir o uso desses aditivos identificados nos rótulos e as limitações da legislação brasileira, que deveria garantir o direito à informação e a saúde da população.

Resumo: Tendo em vista a discussão histórica sobre bibliotecas e censura, este texto apresenta e explora, a partir da exposição e discussão de casos recentes, alguns aspectos que envolvem contemporaneamente a interdição de livros e informações e suas implicações para o exercício profissional do(a) bibliotecário(a). Dentre eles destacam-se: (1) as formas renovadas de censura na contemporaneidade; (2) a instrumentalização de obras literárias e informações na mobilização/polarização política organizada em torno das guerras culturais; (3) o papel dos novos mediadores dos fluxos informacionais na internet e o combate à desinformação; (4) os desafios éticos que os bibliotecários enfrentam em um cenário informacional complexo. A partir destes aspectos, intenta suscitar e discutir questões pertinentes à censura no tempo presente e suas implicações para a ação bibliotecária.

Abstract: Bearing in mind the historical discussion about the library and censorship, this text presents and explores, based on the exposition and discussion of recent cases, some aspects that contemporaneously involve the interdiction of books and information and their implications for the librarian professional exercise. Among them, the following stand out: (1) the renewed forms of censorship in contemporary times; (2) the instrumentalization of literary works and information in the political mobilization/polarization organized around the culture wars; (3) the role of the “new mediators” of information flows on the Internet; (4) the ethical challenges that librarians face in a complex informational scenario. From these aspects, it intends to raise and discuss issues relevant to censorship in the present time and its implications for librarian action.
