Introdução: O refluxo gastro-esofágico (RGE) é uma entidade decorrente de falha anatômica e/ou funcional dos mecanismos de contenção do conteúdo gástrico. As manifestações otorrinolaringológicas do RGE são globus em orofaringe, regurgitação, disfonia, pirose orofaríngea, disfagia e tosse crônica. Objetivo: Pretende-se avaliar os sintomas otorrinolaringológicos do refluxo em pacientes com indicação cirúrgica de hérnia hiatal e comparar com pacientes apenas com queixas laríngeas do RGE. Forma de Estudo: prospectivo clínico randomizado. Material e método: Foram avaliados dois grupos. O grupo A, com 18 pacientes admitidos nos Serviços de Cirurgia do Aparelho Digestivo e Cirurgia Geral que necessitavam de tratamento cirúrgico de esofagite graus III e IV, quanto aos sintomas otorrinolaringológicos do RGE. O grupo B, com 44 pacientes do Serviço de Otorrinolaringologia com queixas laríngeas de RGE. Os pacientes foram submetidos a um protocolo, videolaringoscopia e endoscopia digestiva alta (EDA). Os dados obtidos foram tabulados e comparados com a literatura. Resultados: Os sintomas referidos pelos pacientes do grupo B foram os mesmos do grupo A, porém, disfonia, tosse, regurgitação e pirose em orofaringe foram mais prevalentes no grupo B. As alterações laringoscópicas dos pacientes do grupo B foram mais significativas. As alterações de EDA foram mais prevalentes nos pacientes do grupo A. A partir destes dados nota-se que não existe correlação entre a doença esofageana e os sintomas laríngeos. Conclusão: O trabalho demonstrou a correlação entre sintomas otorrinolaringológicos e o refluxo gastro-esofágico, porém a comparação entre os dois grupos permitiu concluir que, apesar dos quadros apresentarem fisiopatologia semelhante, não se tratam de estágios evolutivos da mesma doença.

Introduction: Gastroesophageal reflux disease (GRD) is an entity due to anatomical or functional failure of contention mechanisms of gastric content. Otolaryngology manifestations of GRD are oropharynx globus, dysphonia, oropharyngeal pirosis, dysfagia, chronic cough. The research intends to evaluate the otolaryngology symptoms of reflux in patients with surgical indication of hiatal hernia and compare the patients’ complaints. Study design: prospective clinical randomized. Material and method: The patients were divided in two groups. Group A, with 18 patients admitted in the General Surgery Service with surgical treatment indication for esophagitys, degrees III and IV and group B, with 40 patients of the Otolaryngology Service with reflux laryngitis complaints. Patients were submitted to a protocol, videolaryngoscopy and upper endoscopy (UE). Obtained data were tabulated and compared to the literature. Results: Symptoms referred by the patients of group B were the same of the group A, but dysphonia, cough, oropharynx pirosis were more prevalent in group B. The laryngoscopic evaluation of group B has shown more altered findings than group A. Altered UE were more prevalent in patients of the group A. Considering the results it is possible to verify that the degree of esophageal disease has no correlation with laryngeal disease. Conclusion: The comparison among the two groups allowed concluding that, despite the similarity of the clinical picture and fisiopatology, the gastroesophageal reflux and the laryngeal reflux must not be considered different stages of the same disease.

Objetivo: o propósito desse estudo foi comparar os padrões de deglutição em crianças com paralisia cerebral (PC) e crianças sem distúrbios neurológicos, relacionando a disfunção motora oral, a quadriplegia e a consistência dos alimentos com o tempo de deglutição. Material e método: Setenta e seis crianças participaram desse estudo, com idades variando de 1 a 5 anos, sendo 57 com paralisia cerebral e 19 sem distúrbios neurológicos. Três consistências de alimentos foram examinadas: líquida (suco), pastosa (iogurte) e sólida (bolacha), sendo cronometrado o tempo gasto para deglutir cada uma delas, durante o horário normal de lanche das mesmas. Resultados: Os resultados mostraram que: 1) o grupo com PC levou até 14,2 vezes mais tempo para deglutir os alimentos que o outro grupo; 2) quanto maior era a disfunção motora oral das crianças, maior era o tempo gasto para deglutir os alimentos e, 3) das três consistências pesquisadas, o alimento pastoso foi aquele, em que os tempos de deglutição obtidos de cada grupo estiveram mais próximos.

Aim: the purpose of this study is to compare deglutition patterns in children with cerebral palsy and children without neurological problems and to investigate the influence of oral motor dysfunction, quadriplegia and food texture, in the time spent for swallowing. Material and method: Seventy-six children aged 1-5 years participated in the study (57 with cerebral palsy and 19 without neurological problems). Three food textures were examined during children’s snack time: liquid (juice), paste (yogurt) and solid (cracker). Result: The result demonstrates, for the cerebral palsy group, their feeding took up to 15 times longer than normal. The more severe the degree of oral motor dysfunction, the longer was the time spent for swallowing. From the textures studied, paste was the one in which children with cerebral palsy were closer in time to the group without neurological problems. It suggests that there were differences in the time spent for swallowing between the groups.

A distonia laríngea (disfonia espasmódica) é distúrbio do movimento caracterizado por contrações involuntárias da musculatura laríngea envolvida no processo de vocalização. A utilização da toxina botulínica no tratamento da distonia laríngea trouxe consideráveis benefícios clínicos. Descrevemos os resultados preliminares do uso terapêutico da toxina botulínica no tratamento da distonia laríngea em 12 pacientes. Após investigação clínica, os pacientes foram submetidos a videolaringoestroboscopia para confirmação diagnóstica e as injeções de toxina botulínica foram realizadas através de punção da membrana cricotireóidea em direção ao músculo tireoaritenóideo, com uso de eletromiografia. A maioria dos pacientes submetidos ao tratamento com toxina botulínica apresentou melhora significativa da distonia laríngea (83% dos casos), com duração média do efeito de quatro meses, sem efeitos colaterais significativos.

Laryngeal dystonia (spasmodic dysphonia) is a movement disorder characterized by involuntary contractions of laryngeal muscles involved with vocalization. The introduction of botulinum toxin in the treatment of laryngeal dystonia had a major clinical impact due to the striking improvement of symptoms. We report the preliminary results of therapeutical use of botulinum toxin in the treatment of twelve patients with laryngeal dystonia. After an extensive clinical evaluation, the patients underwent a videostroboscopic exam for diagnostic confirmation. Botulinum toxin was injected in the cricothyreoid membrane, directed towards the thyreoaritenoid muscle, with the aid of eletromyography needles. Most of patients who underwent botulinum toxin injection had a significant improvement of their symptoms (83%), with effects lasting for four months in average and without important side effects.
