ABSTRACT Purpose To present the translation into Brazilian Portuguese and cross-cultural adaptation of the Apraxia of Speech Rating Scale (ASRS) version 3.5. Methods Validation study restricted to translation and cross-cultural adaptation. The following steps were carried out: translation and synthesis of translations; verification of applicability of the scale synthesis by judges recruited for this purpose; analysis of the relevance and feasibility of the scale calculated by the Content Validity Index (CVI), individual (CVI-I) and total (CVI-T). Eighteen speech therapists were selected. Their answers were used for the analysis of agreement (intraclass correlation coefficients - ICC) and for the calculation of the Content Validity Index (CVI). Finally, the synthesis of the translation was matched in terms of semantic, idiomatic, experiential, conceptual, syntactic, grammatical, and operational equivalence. Results The ICC ranged between 0.83 and 0.94. Six items obtained values higher than 0.9. The other items presented values between 0.8 and 0.9. The CVI-I and CVI-T had excellent values (CVI ≥ 0.78) for relevance and feasibility. Conclusion The Brazilian version of the ASRS 3.5 presents semantic, idiomatic, experiential, conceptual, and syntactic/grammatical equivalence to the original document. Thus, it is ready for the next validation steps.

RESUMO Objetivo Apresentar a tradução e adaptação transcultural do Apraxia of Speech Rating Scale (ASRS) versão 3.5 para o português brasileiro. Método Estudo de validação restrito à tradução e adaptação transcultural. Foram realizadas as seguintes etapas: tradução e síntese das traduções; verificação da aplicabilidade da síntese da escala por juízes, recrutados para tal finalidade; análise da relevância e da viabilidade da escala, calculadas pelo Índice de Validade de Conteúdo (IVC) individual (IVC-I) e total (IVC-T). Foram selecionados 18 fonoaudiólogos, cujas respostas foram utilizadas para a análise de concordância (coeficientes de correlação intraclasse - CCI) e o cálculo do Índice de Validade de Conteúdo (IVC). Por fim, a síntese da tradução foi equiparada quanto à equivalência semântica, idiomática, experiencial, conceitual, sintática, gramatical e operacional. Resultados O CCI variou entre 0,83 e 0,94. Seis itens obtiveram valores superiores a 0,9. Os demais itens apresentaram valores entre 0,8 e 0,9. O IVC-I e IVC-T apresentaram excelentes valores (IVC ≥ 0,78) para a relevância e viabilidade. Conclusão A versão brasileira do ASRS 3.5 apresenta equivalência semântica, idiomática, experiencial, conceitual e sintática/gramatical em relação ao original, dessa forma, está apta para as próximas etapas de validação.

RESUMO Objetivo: desenvolver um protocolo de revisão de escopo que busque identificar quais procedimentos e com que finalidade as tecnologias vestíveis disponíveis na literatura estão sendo usadas nos músculos masseter e temporal. Métodos: seguirá a abordagem recomendada pelo The Joanna Briggs Institute e as diretrizes PRISMA-P. As bases de dados a serem pesquisadas incluem PubMed, Cochrane Library, LILACS, Scopus, Web of Science, Embase e literatura cinzenta. Em uma primeira etapa, dois revisores, de forma independente, avaliarão os artigos pelos títulos e resumos, em seguida, será realizada a revisão na íntegra dos artigos selecionados de acordo com os critérios de inclusão e exclusão. Quaisquer incertezas ou diferenças de opinião serão discutidas para consenso. Serão incluídos artigos publicados sem restrição de data e idioma. Todos os componentes serão apresentados em tabelas e fluxogramas. Além disso, um resumo narrativo de cada componente será incluído para mais detalhes. Considerações finais: este protocolo irá apresentar o estado geral da literatura acerca do uso das tecnologias vestíveis na musculatura mastigatória de masseter e temporal.

ABSTRACT Purpose: to develop a scoping review protocol that seeks to identify which procedures and for what purpose wearables available in the literature are being used on the masseter and temporal muscles. Methods: the approach recommended by The Joanna Briggs Institute and the PRISMA-P guidelines, will follow. Databases to be searched include PubMed, Cochrane Library, LILACS, Scopus, Web of Science, Embase and grey literature. In the first step, two reviewers will independently evaluate the articles by titles and abstracts, then, a full review of the selected articles will be performed according to the inclusion and exclusion criteria. Any uncertainties or differences of opinion will be discussed for consensus. Articles published without date or language restrictions will be included. All components will be presented in tables and flow charts. In addition, a narrative summary of each component will be included for further details. Final Considerations: this protocol will present the general state of the literature on the use of wearables in masseter and temporal masticatory muscles.

RESUMO Objetivos Revisar sistematicamente a literatura sobre o impacto do tratamento medicamentoso nas funções de voz, fala e deglutição de indivíduos adultos com esclerose lateral amiotrófica esporádica, mensuradas por meio de escalas e seus respectivos escores, em relação ao grupo placebo. Estratégia de pesquisa A busca foi realizada com base na estratégia PICO (problema/população/paciente; intervenção; comparação/controle; desfecho/outcome). As palavras-chave foram selecionadas a partir de consulta aos Descritores em Ciências da Saúde (DeCS) e ao Medical Subject Headings (MeSH). Dois pesquisadores independentes fizeram busca na American Speech-Language-Hearing Association (ASHA), Cochrane, LILACS, PubMed, Scopus e Web of Science, em inglês, espanhol e português. Critérios de seleção Foram incluídos ensaios clínicos randomizados, realizados em adultos, e excluídos artigos cujos desfechos estavam relacionados à autoavaliação e à qualidade de vida, teses, dissertações, apenas resumos disponíveis, estudos de caso, estudos experimentais, capítulos de livro, enciclopédias e comunicações breves. Os estudos foram avaliados por meio das ferramentas Robins II (Risk Of Bias In Non-randomized Studies II) e GRADE (Grading of Recommendations Assessment, Development and Evaluation). Resultados dos 9824 artigos encontrados, 5 realizaram a intervenção medicamentosa e foram selecionados para análise. Observou-se ausência de estudos voltados para reabilitação das funções bulbares. A qualidade de evidência gerada variou de alto a baixo risco e o nível de evidência, de baixo a muito baixo. Conclusão a maioria dos estudos demonstra que o tratamento medicamentoso atrasa a degeneração das funções bulbares, com relação ao placebo, embora tal achado não tenha sido observado nos escores de escalas que mensuram tais funções. Os estudos apresentam risco de viés de seleção e muito baixa/baixa qualidade metodológica, limitando a confiança nos achados.

ABSTRACT Purpose To carry out a systematic review of the literature on the impact of drug treatment on the voice, speech, and swallowing functions of adult individuals with sporadic ALS, measured through scales and their respective scores, concerning the placebo group. Research strategy The search strategy was created based on the PICO strategy. The keywords were selected from a consultation with the health sciences descriptors – DECS and the medical subject headings – MeSH. Two independent researchers searched ASHA, Cochrane, Lilacs, Pubmed, Scopus and Web of Science, in English, Spanish and Portuguese. Selection criteria Randomized clinical trials, carried out on adults, were included, and articles with outcomes related to selfassessment and quality of life, theses, dissertations, abstracts only , case studies, experimental studies, book chapters, encyclopedia and brief communication were excluded. The studies were evaluated using the Robins II and Grade tool. Results Of the 9824 articles found, 5 were selected for analysis and underwent drug intervention. It is noticed the absence of studies aimed at the rehabilitation of bulb functions. The quality of evidence generated varied from high to low risk and the level of evidence low and very low. Conclusion Most studies show a delay in the degeneration of bulbar functions in relation to placebo, although this finding has not been observed in the scores of scales that measure such functions. Studies are at risk of selection bias and very low/low methodological quality makes the findings questionable.

RESUMO Objetivo Identificar os termos referidos pela população em geral para a qualidade vocal saudável, rugosa e soprosa. Métodos foi realizado um teste, de modo presencial, com 50 participantes sem vínculos acadêmicos ou profissionais com a Fonoaudiologia. A tarefa consistia em ouvir três vozes e defini-las livremente. A primeira voz apresentada era predominantemente soprosa; a segunda, predominantemente rugosa e a terceira, vocalmente saudável. Apresentou-se a emissão sustentada da vogal /Ɛ/ e a contagem de 1 a 10. Cada participante deveria responder ao comando: “Ouça essa voz. Com qual termo você a nomearia?”, digitando a resposta em uma linha disposta na tela do PowerPoint. Resultados para a voz saudável, o termo que mais se repetiu foi “normal” (36%); outros termos foram: “limpa”, “comum”, “padrão”, “clara”, “límpida”, “firme”, “boa”, “som aberto”, “definida”. Para a voz rugosa, 25 participantes (50%) responderam com o termo “rouca” e os demais se dividiram em termos como “ruidosa”, “chiada”, “voz de fumante”, “grave”, “idosa”, “cavernosa”, “anormal”, entre outros termos similares. Para a voz soprosa, 24 participantes (48%) usaram o termo “cansada”; cinco atribuíram o adjetivo “fraca”; três responderam com o termo “sem fôlego”; houve duas correspondências aos termos “arrastada” e “doente” e os demais participantes responderam com termos semelhantes: “exausta”, “preguiçosa”, “sonolenta”, “fatigada” e afins. Conclusão os termos “normal” para voz saudável, “rouca” para voz rugosa e “cansada” para voz soprosa possibilitam a percepção mais usual desses parâmetros clínicos de qualidade vocal, para indivíduos alheios à linguagem técnico-científica da Fonoaudiologia

ABSTRACT Purpose Identify the terms mentioned by the general population for healthy, rough and breathy vocal quality. Methods A test was carried out with 50 participants, in person, without academic or professional ties with Speech Therapy. The task was to hear three voices and define them freely. The first voice presented was predominantly breathy; the second, predominantly rough and the third, vocally healthy. The sustained emission of the vowel / Ɛ / and the count from one to ten were presented. Each participant should respond to the command: “Listen to that voice. Which term would you name it?”, Typing the answer on a line displayed on the PowerPoint screen. Results For the healthy voice, the term that was repeated the most was “normal” (36%), other terms were: “clean”, “common”, “standard”, “clear”, “clear”, “firm”, “good”, “open sound”, “defined”. For the rough voice, twenty-five participants (50%) responded with the term “hoarse” and the others were divided into terms such as “noisy”, “smoker's voice”, “deep”, “elderly”, “cavernous”, “abnormal”, among other similar terms. For the breathy voice, twenty-four participants (48%) used the term “tired”; five participants assigned the adjective “weak”; three responded with the term “out of breath”; there were two correspondences to the terms “dragged” and “sick”; and the other participants responded with terms similar: “exhausted”, “lazy”, “sleepy”, “fatigued” and the like. Conclusion The terms “normal” for a healthy voice, “hoarse” for a rough voice and “tired” for a breathy voice, allow a more usual perception of these clinical parameters of vocal quality, for individuals outside the technical-scientific language of Speech Therapy.

RESUMO Objetivo Investigar o efeito imediato da vibração sonorizada de língua (VSL), do canudo de alta resistência no ar (CAR) e da sobrearticulação (SA) sobre o espaço vocálico de mulheres vocalmente saudáveis (MVS) e com nódulos vocais (MNV). Método Participaram 12 mulheres no MNV e 12 mulheres no MVS, alocadas para execução dos exercícios vocais de VSL, CAR e AS. Cada participante realizou apenas um dos três exercícios propostos, durante 5 minutos, antecedidos e sucedidos pela gravação de uma sequência de frases-veículo para extração dos formantes (F1 e F2) dos segmentos vocálicos [a, i, u]. O espaço vocálico foi analisado por meio das diferenças entre as medidas dos formantes das vogais. Resultados houve redução de F1 no intervalo [a]-[i] e [i]-[u] e de F2 entre as vogais [a]-[u] e [i]-[u] no MVS, após realização do CAR. No MNV, observou-se redução de F2 no intervalo [a]-[i] após VSL. Na análise intergrupo, houve maiores valores de F1 entre os intervalos das vogais [a]-[i] e [i]-[u] no MVS, antes da realização do CAR, e após exercício apenas no intervalo [a]-[i]. Observou-se maior valor de F1 e F2 no intervalo entre as vogais [i]-[u] no MNV após VSL. Conclusão O exercício de VSL diminuiu o espaço vocálico em mulheres do MNV. O CAR reduziu o espaço vocálico de mulheres do MVS. O MNV apresentou menor espaço vocálico em relação ao MVS, antes e após a realização do CAR. Houve redução do espaço vocálico no MNV em relação ao MNV após o exercício de VSL.

ABSTRACT Purpose To investigate the immediate effect of voiced tongue vibration (VSL), high-resistance straw in the air (CAR), and overarticulation (OA) on the vocal space of vocally healthy women (MVS) and with vocal nodules (MNV). Methods 12 women participated in the MNV and 12 women in the MVS, allocated to perform the vocal exercises of VSL, CAR, and OA. Each participant performed only one of the three proposed exercises, for 5 minutes, preceded and followed by recording a sequence of vehicle sentences for extracting formants (F1 and F2) from the vowel segments [a, i, u]. The vowel space was analyzed through the differences between the measures of the formants of the vowels. Results we observed a reduction of F1 in the interval [a]-[i] and [i]-[u] and of F2 between the vowels [a]-[u] and [i]-[u] in the MVS, after performing the CAR. In MNV, we observed a reduction of F2 in the interval [a]-[i] after VSL. In the intergroup analysis, there were higher F1 values between the intervals of the vowels [a]-[i] and [i]-[u] in the MVS, before performing the CAR, and after exercise only in the interval [a]-[i]. A higher value of F1 and F2 was observed in the interval between the vowels [i]-[u] in the MNV after VSL. Conclusion The VSL exercise reduced the vowel space in MNV women. CAR reduced the vocal space of women in the MVS. The MNV had a smaller vowel space compared to the MVS before and after the CAR. We observed a reduction in the vowel space in the MNV compared to the MNV after the VSL exercise.

Resumo: Introdução: Um simulador virtual, ou baseado em realidade virtual, pode recriar computacionalmente contextos reais. Objetivo: Este estudo teve como objetivo analisar trabalhos sobre simulações virtuais para treinamento de procedimentos clínicos, com foco na avaliação de habilidades do usuário. Método: Trata-se de uma revisão integrativa da literatura realizada entre 2010 e 2020. Foram selecionados 56 estudos. Resultado: Observamos nos estudos selecionados que as variáveis e os parâmetros dos simuladores virtuais são geralmente obtidos por consulta a especialistas ou pela literatura médica. Esses simuladores se concentram principalmente no desenvolvimento de habilidades psicomotoras e na avaliação do desempenho do aluno por meio de alertas em tempo real, indicadores de progresso e relatórios de desempenho após o final de cada treinamento. Conclusão: Considerar o conhecimento do especialista para definir exclusivamente os requisitos dos simuladores virtuais pode limitar a confiabilidade e precisão destes. A participação de especialistas nesses projetos não obedece a padrões de seleção e periodicidade com que colaboram. Poucos simuladores fornecem feedback perspicaz e pertinente sobre o desempenho do usuário.

Abstract: Introduction: A virtual simulator, or one based on virtual reality, can computationally recreate real contexts. Objective: To analyze works on virtual simulations for training clinical procedures, focusing on the assessment of user skills. Method: Integrative literature review, carried out between 2010 and 2020. A total of 56 studies were selected 56 studies. Results: The selected studies showed that the variables and parameters of virtual simulators are usually obtained by consulting experts or through medical literature. These simulators mainly focus on developing psychomotor skills and assessing the learner’s performance through real-time alerts, progress indicators, and performance reports after the end of each training. Conclusion: Considering the expert’s knowledge exclusively to define the requirements of virtual simulators can limit their reliability and accuracy. The participation of experts in these projects does not follow standards regarding the selection and frequency with which they collaborate. Few simulators provide insightful and pertinent feedback on user performance.

RESUMO Objetivo Comparar as medidas acústicas e perceptivo-auditivas de mulheres com e sem nódulos vocais. Métodos Participaram do estudo 12 mulheres com nódulos vocais (grupo MNV) e 12 sem nódulos vocais (grupo MSNV). Foram submetidas à gravação da vogal /a/ sustentada, com o objetivo de extrair a média da frequência fundamental (f0), desvio padrão de f0 (DP f0), jitter, shimmer, Glottal Noise Excitation, medidas relacionadas ao Cepstral Peak Prominence-Smoothed, medidas espectrais das diferenças do primeiro e segundo harmônico (H1-H2) e gravação das frases-veículo: “Digo papa baixinho”, “Digo pipa baixinho” e “Digo pupa baixinho”, para extração do primeiro (F1) e segundo formante (F2) dos segmentos vocálicos /a, i, u/. Para avaliação perceptivo-auditiva, utilizou-se a Escala Visual Analógica. Resultados A análise comparativa entre os grupos evidenciou maiores valores para o grupo MNV nos parâmetros grau geral, rugosidade e soprosidade e para a medida acústica shimmer. Os valores de F1 para as vogais /a/ e /u/ e os valores de F2 para a vogal /a/ apresentaram-se mais elevados no mesmo grupo. Conclusão mulheres com nódulos apresentaram vozes mais desviadas, com presença de rugosidade e soprosidade e modificações nos ajustes do trato vocal, com possível redução na amplitude dos articuladores, quando comparadas às mulheres sem nódulos vocais.

ABSTRACT Purpose To compare the acoustic and auditory-perceptual measures of the voice of women with and without vocal nodules. Methods Twelve women with vocal nodules (MNV group) and 12 without vocal nodules (MSNV group) participated in the study. They were submitted to the recording of their sustained /a/ vowel, in order to extract the mean of the fundamental frequency (f0), standard deviation of f0 (SD f0), jitter, shimmer, GNE, cepstral measure of CPPS, and spectral measures of differences of the first and second harmonics (H1-H2); and recording of the carrier phrases: “I say papa baixinho”, “I say pipa baixinho” and “I say pupa baixinho”, to extract the first (F1) and second formant (F2) of the vowel segments /a, i, u/. For auditory-perceptual assessment, the visual-analog scale (VAS) was used. Results The comparative analysis between the groups shows higher values for the MNV in the parameters general degree, roughness and breathiness, and for the shimmer acoustic measure. The F1 values for the vowels /a/ and /u/, and the F2 values for the vowel /a/ were higher in the same group. Conclusion According to the data observed in the investigated sample, women with nodules have more deviated voices, with the presence of roughness and breathiness, and changes in vocal tract adjustments, with a possible reduction in the amplitude of the articulators, when compared to women without vocal nodules.

RESUMO Objetivo Apresentar uma proposta de reabilitação fonoaudiológica denominada Programa de Terapia para Manejo da Tosse Crônica (TMTC) para tratamento da tosse crônica refratária. Método O TMTC foi elaborado com base em duas etapas: análise de literatura e análise clínica. A etapa de análise de literatura foi realizada por meio dos procedimentos: busca eletrônica e manual por artigos e livros publicados até junho de 2019 que contemplassem participantes adultos com tosse crônica, e intervenção com terapia fonoaudiológica; seleção das fontes; análise dos artigos e livros incluídos; e, elaboração da versão inicial do programa de terapia. A versão inicial do programa de terapia resultante da etapa de análise de literatura foi submetida a etapa de análise clínica por meio dos procedimentos: análise de três juízes; revisão da proposta pelos autores; reanálise dos juízes; elaboração da versão final denominada Programa de Terapia para Manejo da Tosse Crônica (TMTC). Resultados O TMTC sintetiza o conhecimento científico e a experiência clínica atual sobre o manejo comportamental da tosse crônica refratária em um programa com três componentes principais, cada um com objetivos e estratégias específicos. O TMTC é composto por quatro sessões, com frequência semanal, e duração entre 30 e 45 minutos por sessão, executadas em fases hierárquicas, com objetivos e estratégias específicos por sessão. Conclusão O TMTC é um programa estruturado para oferecer uma reabilitação global da tosse. Faz-se necessário obter evidências científicas sobre sua efetividade para que ele possa ser utilizado na prática clínica.

ABSTRACT Purpose To present a proposal for speech-language pathology rehabilitation called the Therapy Program for Management of Chronic Cough (TMCC) for the treatment of refractory chronic cough. Methods TMCC was developed based on two stages: literature and clinical analysis. The literature analysis stage was carried out through the following procedures: electronic and manual search for articles and books published until June 2019 that included adult participants with chronic cough and intervention with speech-language pathology therapy; selection of sources; analysis of articles and books included; and, preparation of the initial version of the therapy program. The initial version of the therapy program resulting from literature analysis stage was submitted to clinical analysis stage through the following procedures: analysis by three judges; revision of the proposal by the authors; judges’ re-analysis; elaboration of the final version called the Therapy Program for Management of Chronic Cough (TMCC). Results TMCC synthesizes scientific knowledge and current clinical experience on the behavioral management of refractory chronic cough into a program with three main components, each one with specific objectives and strategies. TMCC consists of four sessions, with weekly frequency and duration between 30-45 minutes per session, executed in hierarchical phases, with specific objectives and strategies per session. Conclusion TMCC is a program structured to offer global cough rehabilitation. To obtain scientific evidence about its effectiveness is necessary, so that it can be used in clinical practice.

RESUMO Objetivo Avaliar a efetividade da terapia de grupo na redução dos sintomas vocais em pacientes com disfonia comportamental e compará-la a uma modalidade de terapia tradicional/individual. Métodos Trata-se de uma pesquisa explicativa, quantitativa e de intervenção. Participaram 99 pacientes com disfonia comportamental, de ambos os sexos, alocados em dois grupos: Terapia Individual (TI) e Terapia de Grupo (TG). Todos foram submetidos à terapia com abordagem eclética. Foram realizadas oito sessões, sendo a primeira e a última destinadas à avaliação e as outras seis de intervenção. Realizou-se análise estatística descritiva e inferencial para comparar os grupos e os momentos pré e pós-intervenção. Resultados A maioria dos participantes era do sexo feminino, não usava a voz profissionalmente e com lesão na porção membranosa da prega vocal. Ambos os grupos apresentavam escores médios semelhantes, no início da terapia, fato que mostra a homogeneidade dos grupos. Houve redução de todos os escores da Escala de Sintomas Vocais (ESV) no momento pós-intervenção individual e em grupo. Não foi observada diferença significativa, ao comparar as médias dos escores da ESV pós-terapia entre os grupos. Em relação aos itens da ESV, foi possível observar que alguns podem não detectar as diferenças entre os momentos pré e pós-intervenção. Conclusão Tanto a TI, quanto a TG foram efetivas na redução significativa dos sintomas vocais. Não houve diferença ao comparar os grupos. Alguns itens da ESV, principalmente no domínio limitação, foram mais sensíveis nos momentos pós-intervenção, em ambas modalidades. O tipo de intervenção influencia a redução dos sintomas vocais.

ABSTRACT Purpose To evaluate the effectiveness of two therapeutic approaches in reducing vocal symptoms in patients with behavioral dysphonia. Methods This was an explanatory, quantitative and interventional study. A total of 99 patients of both sexes with behavioral dysphonia who sought speech therapy participated in this study. These patients were allocated into two groups: individual therapy (IT) and group therapy (GT). All participants were subjected to therapy with an eclectic approach. Eight sessions were conducted, comprising a first and a last session for evaluation, with six intervention sessions in between them. A descriptive and inferential statistical analysis was performed to compare the groups and the moments before and after the intervention. Results The majority of participants were female, and there was a predominance of patients who did not use their voice professionally and who had a diagnosis of a laryngeal lesion in the membranous portion of the vocal fold. It should be noted that patients presented similar mean scores at the beginning of therapy regardless of the group to which they were allocated, which indicated the homogeneity of the groups. There were reductions in all Vocal Symptoms Scale (VoiSS) scores after individual and group therapy. No significant differences were observed when comparing the mean posttherapy VoiSS scores between the groups in either domain. Some VoiSS items were unable to detect differences between the pre- and posttherapy timepoints. Conclusion Individual and group therapeutic modalities are effective in significantly reducing self-reported vocal symptoms. The type of intervention influences the reduction in vocal symptoms. Some items of the VoiSS, mainly in the area of limitations, were more sensitive at the posttherapy timepoint in both modalities.

RESUMO Objetivo Verificar se existe associação entre a presença de alteração laríngea, a análise perceptivo-auditiva da qualidade vocal e a classificação espectrográfica do sinal vocal em indivíduos com distúrbio de voz. Métodos Participaram 478 pacientes com distúrbios de voz. Foi realizada gravação da vogal /Ɛ/ sustentada e o exame médico para estabelecimento de diagnóstico laríngeo. Os espectrogramas da vogal foram utilizados para classificação dos sinais em Tipo I, II, III e IV. Resultados Vozes de indivíduos sem alteração laríngea foram classificadas, predominantemente, como Tipo I e Tipo II, enquanto sinais de indivíduos com alteração laríngea foram classificados nos Tipos III e IV. Vozes desviadas foram classificadas, predominantemente, como Tipo II, enquanto os sinais de pacientes com desvio vocal foram categorizados, predominantemente, como Tipos II e III. Apenas os sinais de indivíduos com desvio vocal foram classificados como Tipo IV. Sinais Tipo III e IV apresentaram valores mais elevados no grau geral do desvio e nos graus de rugosidade e soprosidade, em relação aos sinais Tipo I e Tipo II. Os sinais Tipo IV apresentaram maior grau geral e graus de rugosidade e soprosidade, em comparação aos sinais Tipo III. Apenas os sinais Tipo IV apresentaram valores mais elevados no grau de tensão, em relação aos sinais Tipo I, II e III. Conclusão Há associação entre a presença de alteração laríngea, a análise perceptivo-auditiva e a classificação espectrográfica do sinal vocal em indivíduos com distúrbio de voz.

ABSTRACT Purpose To verify whether there is an association between the presence of laryngeal alteration, auditory-perceptual analysis of vocal quality, and the spectrographic classification of the vocal signal in individuals with voice disorders. Methods 478 patients with voice disorders participated in the study. A recording of the sustained vowel /Ɛ/ and a medical examination were performed to establish a laryngeal diagnosis. The vowel spectrograms were used to classify the signals into type I, II, III and IV. Results Voices of individuals without laryngeal disorders were predominantly classified as type I and type II, while signals of individuals with laryngeal disorders were classified as types III and IV. Deviated voices were predominantly classified as type II, while the signals of patients with vocal deviation were predominantly categorized as types II and III. Only the signals of individuals with vocal deviation were classified as type IV. Type III and IV signals showed higher values for the general degree of deviation and for degrees of roughness and breathiness in relation to type I and type II signals. Type IV signals showed a higher general degree and degrees of roughness and breathiness compared to type III signals. Only type IV signals showed higher values in the degree of tension compared to types I, II and III signals. Conclusion There is an association between the presence of laryngeal alteration, auditory-perceptual analysis, and the spectrographic classification of the vocal signal in individuals with voice disorders.

RESUMO Objetivo Analisar se existe associação entre a presença, a intensidade e o tipo de desvio vocal e as medidas cepstrais em amostras de indivíduos com queixa vocal. Método Foram utilizadas 376 amostras da vogal /ε/ de indivíduos de ambos os gêneros, com queixa vocal. Utilizou-se uma escala analógico-visual para análise perceptivo-auditiva das vozes quanto à intensidade do desvio vocal (GG), graus de rugosidade (GR), soprosidade (GS) e tensão (GT), incluindo-se a determinação da qualidade vocal predominante (rugosa, soprosa ou tensa). Foram extraídas as medidas relacionadas ao Cepstral Peak Prominence-Smoothed (CPPS) e o declínio espectral das amostras vocais. Resultados Houve diferença dos valores do CPPS entre os grupos com e sem desvio vocal, assim como entre as diferentes intensidades e tipos de desvio vocal. Os valores do CPPS foram mais reduzidos em função da presença e intensidade do desvio vocal. Os valores do CPPS diferenciaram vozes rugosas x soprosas, rugosas x tensas e soprosas x tensas. O declínio espectral apenas diferenciou vozes soprosas x tensas. O CPPS se correlacionou de modo positivo e forte com os GG e GS, de modo negativo moderado com o GR, e de forma negativa fraca com o GT. O declínio espectral apresentou correlação positiva moderada com o GT e correlação negativa fraca com o GS. Conclusão Existe associação entre a presença de desvio vocal, o GG, a qualidade vocal predominante e o CPPS. De modo especial, o GG é fortemente correlacionado ao CPPS. O declínio espectral está associado apenas aos parâmetros de soprosidade e tensão.

ABSTRACT Purpose To analyze whether there is an association between the presence, intensity and type of voice disorder and the cepstral measures in samples of individuals with voice complaints. Methods We used 376 vowel /Ɛ/ samples from individuals of both genders that had voice complaints. An analogue-visual scale was used for the auditory-perceptual analysis of voices regarding the overall grade of dysphonia (G) and the grades of roughness (R), breathiness (B), and strain (S), including a determination of voice quality (rough, breathy or strained). Measures related to cepstral peak prominence smoothed (CPPS) and spectral decline of vocal samples were extracted. Results There were differences in the CPPS values between the groups with or without voice disorders as well as between the different intensities and types of voice disorder. CPPS values were lower because of the presence and intensity of voice disorders. The CPPS values differentiated the following voices: rough x breathy, rough x strained, and breathy x strained. The spectral decline only differentiated breathy x strained voices. CPPS correlated positively and strongly with G and B; moderately and negatively with R, and negatively and weakly with S. The spectral decline had a moderate positive correlation with S and a weak negative correlation with B. Conclusion There is association between voice disorder, G, predominant voice quality, and CPPS. In particular, G is strongly correlated with CPPS. Spectral decline is associated only with the parameters B and S.

RESUMO Objetivo Analisar a configuração acústico-articulatória das vogais em mulheres com nódulos vocais e vocalmente saudáveis. Método Participaram do estudo 12 mulheres com nódulos vocais (GE) e 12 vocalmente saudáveis (GC). Todas as mulheres gravaram frases-veículo com as vogais /a/, /i/ e /u/ em posição tônica, sucedidas e precedidas da oclusiva /p/: “Digo papa baixinho”, “Digo pipa baixinho” e “Digo pupa baixinho”. Posteriormente, foram extraídos os três primeiros formantes (F1, F2 e F3) dessas vogais. Resultados Observou-se diferença nas medidas de F1 para as vogais /a/ e /u/ e F2 para a vogal /a/ entre os dois grupos estudados. Mulheres com nódulos vocais apresentam menor valor dessas medidas em relação às mulheres vocalmente saudáveis. Pacientes com nódulos vocais apresentaram menor intervalo nos valores de F1 e F2 entre as vogais /a/, /i/ e /u/ em relação às mulheres vocalmente saudáveis. Conclusão Mulheres com nódulos vocais apresentam menores valores de F1 e F2, e menor amplitude de movimentação dos articuladores na produção vocálica em relação às mulheres vocalmente saudáveis.

ABSTRACT Purpose To analyze the acoustic-articulatory configuration of vowels in women with vocal nodules and with healthy voice. Methods Twelve women with vocal nodules (EG) and twelve vocally health women (CG) participated of this study. All women recorded vehicle phrases with the vowels /a/, /i/, and /u/ in stress position, preceded and followed by the occlusive consonant /p/: “Digo papa baixinho”, “Digo pipa baixinho”, and “Digo pupa baixinho”. Subsequently, the first three formants (F1, F2, and F3) were extracted from these vowel targets. Results Between the two groups studied, F1 measures differed for vowels /a/ and /u/, and F2 measures differed for the vowel /a/. Women with vocal nodules showed lower values for these measures compared to vocally healthy women. Patients with vocal nodules showed a smaller interval in F1 and F2 values between vowels /a/, /i/, and /u/ compared to vocally healthy women. Conclusion Women with vocal nodules show lower F1 and F2 values and lower range of motion of the articulators during vowel production compared to vocally healthy women.

RESUMO Objetivo Analisar a frequência combinada de queixas relacionadas à deglutição e voz, antes da tireoidectomia. Métodos Foi realizada uma análise retrospectiva de 51 entrevistas de pacientes encaminhados para tireoidectomia parcial ou total. A partir da mediana da quantidade total de cada grupo de sintomas de deglutição e voz, determinou-se a frequência combinada de queixas relacionadas à deglutição e voz. Investigou-se, ainda, como os pacientes classificaram cada uma das duas funções (excelente, muito boa, boa, razoável ou ruim) e se esta classificação se relacionava com a quantidade de sintomas e com a frequência combinada. Para a análise bivariada, foram utilizados os testes Qui-quadrado de Pearson ou exato de Fisher e o teste não paramétrico de Mann-Whitney. O nível de significância foi de 5%. Resultados A frequência combinada de queixas relacionadas à deglutição e voz antes da tireoidectomia foi de 31,4%. Os sintomas mais citados, relacionados à deglutição, foram pigarro após deglutir (39,3%), esforço para deglutir (37,2%) e engasgo (35,3%) e os relacionados à voz foram garganta seca (72,6%), pigarro na garganta (72,5%) e coceira na garganta (47%). A quantidade de sintomas foi significativamente maior entre os que classificaram a deglutição (3,13 ± 2,21) e a voz (5,91 ± 2,81) de forma negativa. Houve associação entre autoavaliação negativa da voz e a frequência combinada de queixas relacionadas à deglutição e voz (p=0,003). Conclusão A combinação de queixas relacionadas à deglutição e voz antes da tireoidectomia ocorre em um terço dos pacientes e está associada à autoavaliação negativa da voz.

ABSTRACT Purpose To analyze the combined frequency of patient-reported swallowing and voice complaints before thyroidectomy. Methods This is a retrospective analysis of 51 interviews of patients referred for partial or total thyroidectomy. The combined frequency of patient-reported swallowing and voice complaints was determined from the median of the total number of related symptoms in each group. We also investigated how patients rated each function (excellent, very good, good, fair, poor) and whether this rating was related to the number of symptoms and the combined frequency. For bivariate analysis, Pearson’s chi-square test or Fisher’s exact test and the nonparametric Mann-Whitney test were used. The level of significance was 5%. Results The combined frequency of patient-reported swallowing and voice complaints before thyroidectomy was 31.4%. The most commonly swallowing-related symptoms were throat clearing after swallowing (39.3%), swallowing effort (37.2%), and choking (35.3%). The most commonly voice-related symptoms were dry throat (72.6%), throat clearing (72.5%), and itchy throat (47%). The total number of symptoms was significantly higher among those who rated swallowing (3.13 ± 2.21) and voice (5.91 ± 2.81) negatively. Negative self-assessment of the voice was associated with the combined frequency of patient-reported swallowing and voice complaints (p=0.003). Conclusion The combined frequency of patient-reported swallowing and voice complaints before thyroidectomy occurs in one third of the patients and is associated with negative self-assessment of the voice.

Resumo Introdução Os distúrbios de voz alteram o sinal sonoro de diversas formas, combinam variados tipos de perturbações e ruídos na emissão vocal. O diagrama de desvio fonatório é um gráfico bidimensional que possibilita a avaliação do sinal vocal a partir da combinação de medidas de periodicidade (jitter, shimmer e coeficiente de correlação) e de ruído (Glottal to Noise Excitation). O uso de sinais sintetizados, no qual se tem um maior controle e conhecimento das condições de produção, pode possibilitar uma maior compreensão dos mecanismos fisiológicos e acústicos subjacentes à emissão vocal e seus principais correlatos perceptivo-auditivos quanto à intensidade do desvio e aos tipos de qualidade vocal. Objetivo Analisar o desempenho do diagrama de desvio fonatório na discriminação da presença e do grau de rugosidade e soprosidade em vozes sintetizadas. Método Foram utilizados 871 sinais vocais sintetizados correspondentes à vogal/ɛ/. Realizou-se a análise perceptivo-auditiva do grau de rugosidade e soprosidade dos sinais sintetizados, com uma escala visual analógica. Posteriormente, os sinais foram categorizados quanto à presença/ausência desses parâmetros a partir dos valores de corte da escala visual analógica. A análise acústica foi realizada por meio da avaliação da distribuição dos sinais vocais de acordo com a área, quadrante, forma e densidade do diagrama de desvio fonatório. Executou-se o teste de igualdade de proporções e o teste qui-quadrado para comparar as variáveis. Resultados Sinais vocais rugosos e soprosos localizaram-se predominantemente fora da área de normalidade e no quadrante inferior direito do diagrama de desvio fonatório. Vozes com maiores graus de rugosidade e soprosidade localizaram-se fora da área de normalidade, no quadrante inferior direito e apresentaram densidade concentrada. Conclusão A área de normalidade e o quadrante do diagrama de desvio fonatório são capazes de discriminar vozes saudáveis de rugosas e soprosas. Vozes com maior grau de rugosidade e soprosidade localizam-se proporcionalmente fora da área de normalidade, no quadrante inferior-direito e com densidade concentrada.

Abstract Introduction Voice disorders alter the sound signal in several ways, combining several types of vocal emission disturbances and noise. The phonatory deviation diagram is a two-dimensional chart that allows the evaluation of the vocal signal based on the combination of periodicity (jitter, shimmer, and correlation coefficient) and noise (Glottal to Noise Excitation) measurements. The use of synthesized signals, where one has a greater control and knowledge of the production conditions, may allow a better understanding of the physiological and acoustic mechanisms underlying the vocal emission and its main perceptual-auditory correlates regarding the intensity of the deviation and types of vocal quality. Objective To analyze the performance of the phonatory deviation diagram in the discrimination of the presence and degree of roughness and breathiness in synthesized voices. Methods 871 synthesized vocal signals were used corresponding to the vowel /ɛ/. The perceptual-auditory analysis of the degree of roughness and breathiness of the synthesized signals was performed using visual analogue scale. Subsequently, the signals were categorized regarding the presence/absence of these parameters based on the visual analogue scale cutoff values. Acoustic analysis was performed by assessing the distribution of vocal signals according to the phonatory deviation diagram area, quadrant, shape, and density. The equality of proportions and the chi-square tests were performed to compare the variables. Results Rough and breathy vocal signals were located predominantly outside the normal range and in the lower right quadrant of the phonatory deviation diagram. Voices with higher degrees of roughness and breathiness were located outside the area of normality in the lower right quadrant and had concentrated density. Conclusion The normality area and the phonatory deviation diagram quadrant can discriminate healthy voices from rough and breathy ones. Voices with higher degrees of roughness and breathiness are proportionally located outside the area of normality, in the lower right quadrant and with concentrated density.
