Resumo: O objetivo deste trabalho de pesquisa avaliativa foi obter dados e informações referentes ao programa de pós-graduação em patologia da Faculdade de Medicina do Triângulo Mineiro (CPGP/FMTM) em Uberaba (MG) e avaliar a eficiência, eficácia e qualidade, além de suas relações com o desenvolvimento institucional. Foi realizado um levantamento de dados referente a história de vida, proposta do programa, recursos docente e discente, teses, linhas de pesquisa, espaço físico e infra-estrutura de apoio acadêmico. Na análise dos dados obtidos, foram enfocados o contexto e o produto-resultado, utilizando-se os critérios de avaliação de programa, eficiência e eficácia. Com respeito à qualidade do programa, foram considerados na avaliação a reputação, recursos, resultados e valor agregado, tendo sido estabelecida sua correlação com o desenvolvimento institucional. Os resultados deste trabalho favorecem a importância de uma revisão regular dos programas de pós-graduação das instituições de ensino superior em geral e, em particular, do CPGP/FMTM.

Abstract: The aim of this evaluative research work was to obtain data and informations about the program of the Pathofogy Graduation Course of Faculdade de Medicina do Triângulo Mineiro (PGC/FMTM) in Uberaba (MG) and evaluate the effiCIency, efficacy and qualIty levels, in addition to their relationships with the institutional development. The authors perfomed a retrospective study related to life histories, program proposals, docent and discent numbers, theses, research lines, physical spaces, and academic infra structural resources. The data obtained were analysed focusing the context and endresults, using avaliation criteria for program efficiency and efficacy. With respect to program quality, reputation, resources, results and aggregated values were all considered in the evaluation. The results were discussed and their correlation with the institutional development was also performed. The resulls of this work favor the importance of a regular revision in the postgraduatíon programs of brazilian superior teaching institutions in general, and particularfy in PGC/FMTM.

Com os objetivos de conhecer melhor a morfologia do megacólon chagásico e contribuir para o conhecimento da patogênese da fibrose dos megas, realizou-se estudo morfométrico na muscular circular do cólon, contando-se o número de mastócitos e medindo o conjuntivo fibroso nessa camada. Foram avaliados anéis do sigmóide distal de 15 chagásicos com megacólon (MCC), 15 sem megacólon (CSMC) e 15 não chagásicos (NC). Os anéis foram fixados em formol, incluídos em parafina, cortados com 7mm de espessura e corados por Azan-Heidenhain e Giemsa. O número de mastócitos e a fibrose foram maiores no grupo com MCC em relação ao CSMC e NC (p < 0,05; teste de Kruskal-Wallis); não houve diferença significante entre os dois últimos grupos. Diante destes achados, é possível, que haja relação entre mastocitose e fibrose no megacólon chagásico, como já se demonstrou em outras doenças.

A morphometric study of the circular colon musculature was performed, in which the mast cell count was determined and the connective fibrous tissue in this layer was measured. The objective was to gain better understanding of Chagas megacolon morphology and contribute towards the knowledge of fibrosis pathogenesis in Chagas megas. An evaluation was made of 15 distal sigmoid rings from Chagas patients with megacolon (MCC), 15 without megacolon (CSMC) and 15 non-Chagas patients (NC). The rings were fixed in formol, embedded in paraffin, and 7mm thick sections were cut and stained using Azan-Heidenhain and Giemsa. The mast cell count and fibrosis were greater in the MCC group than in the CSMC and NC groups (p < 0.05; Kruskal-Wallis test) and there was no significant difference between the latter two. The fibrosis and increased mast cell count in the colon musculature of the MCC group possibly indicates that there is a relationship between mastocytosis and fibrosis, as has already been demonstrated in other pathologies.

We carried out a morphometric study of the esophagus of cross-bred dogs experimentally infected or consecutively reinfected with Trypanosoma cruzi 147 and SC-1 strains, in order to verify denervation and/or neuronal hypertrophy in the intramural plexus. The animals were sacrificed in the chronic stage, 38 months after the initial infection. Neither nests of amastigotes, nor myositis or ganglionitis, were observed in all third inferior portions of esophageal rings analyzed. No nerve cell was identified in the submucous of this organ. There was no significant difference (p>0.05) between the number, maximum diameter, perimeter, or area and volume of the nerve cells of the myenteric plexus of infected and/or reinfected dogs and of the non-infected ones. In view of these results we may conclude that the 147 and SC-1 strains have little neurotropism and do not determine denervation and/or hypertrophy in the intramural esophageal plexuses in the animals studied, independent of the reinfections.

Pesquisou-se a ocorrência, na parede das veias do plexo pampiniforme de chagásicos crônicos, de acúmulos intracelulares de Trypanosoma cruzi e sinais inflamatórios (flebite). Para tal fim, colheram-se, à necropsia, 23 pares de funículos espermáticos, epidídimos e testículos, sendo 17 de chagásicos crônicos e 6 de controles (não chagásicos). Em cada caso, foram feitos múltiplos cortes das gônadas e dos vasos; fez-se pesquisa de T. cruzi por imuno-histoquímica nos funículos espermáticos de todos os casos. Não se observaram parasitos nas paredes vasculares. Notou-se flebite crônica inespecífica, focal e discreta, em cinco chagásicos (bilateral em três pacientes) e dois controles; havia infiltração mononuclear discreta do interstício funicular em treze chagásicos e cinco controles. A análise estatística dos resultados (chi2) não revelou diferenças significativas. Conclui-se que o ambiente hormonal devido à testosterona não parece favorecer a infecção da parede dos vasos gonadais por Trypanosoma cruzi, embora, segundo a literatura, o referido hormônio pareça ter ações imunodepressoras.

The occurrence of Trypanosoma cruzi intracellular clusters and phlebitis was searched for on pampiniform plexus vein walls of chronic chagasic patients. For this purpose, 23 pairs of spermatic cords, epididymides and testes (17 from chagasic patients and 6 from non-chagasic controls) were obtained, at autopsy. Trypanosoma cruzi was investigated by immuno-histochemistry on slides obtained from several sections of the gonads and vessels of each case. Only discrete and focal undetermined chronic phlebitis was observed, with no parasites, in 5 chagasics (bilateral in 3) and 2 controls (chi2: p < 0.10), and discrete mononuclear interstitial infiltration in the funiculi of 13 chagasics and 5 controls (chi2: p < 0.75). In conclusion, on the contrary to that published regarding the supra-renal central veins, it seems that the hormonal environment provided by testosterone does not favor the infection of the gonadal vessel wall.

Relata-se caso de larva migrans ou dermatite linear serpiginosa no couro cabeludo de adolescente, no qual o ancilostomídeo foi encontrado no interior de glândula sebácea. Destaca-se a possibilidade do helminto sediar-se em locais pouco usuais, das glândulas sebáceas serem via de penetração de larvas na pele e discute-se o provável mecanismo pelo qual o agente implantou-se no anexo cutâneo.

A case of larva migrans or serpiginous linear dermatitis on the scalp of a teenager is reported. An ancylostomid larva was found within a sebaceous gland acinus. The unusual skin site for larva migrans as well as the penetration through the sebaceous gland are highlighted. The probable mechanism by which the parasite reached the skin adnexa is discussed.

Revendo a literatura não encontramos estudos anatômicos dos gânglios intrapancreáticos na forma crônica da doença de Chagas; lesões dos mesmos poderiam explicar, ao menos em parte, os distúrbios funcionais do pâncreas exócrino e endócrino descritos nesta forma da doença. Decidimos então analisar morfologicamente tais gânglios. Para isso, estudamos segmentos transversais da cabeça, corpo e cauda do pâncreas de doze chagásicos crônicos, com idade média de 46,5 ± 9,1 anos, e quatorze controles, com idade média de 41,2 ± 11,0 anos. Os segmentos foram processados histologicamente e seccionados de forma seriada até o esgotamento, analisando-se os cortes múltiplos de sete. Para análise estatística, usamos o teste não-paramétrico de Mann-Whitney. Na cabeça do pâncreas, a contagem de neurônios teve média de 57,3 ± 50,8 para o grupo chagásico e 117,5 ± 99,0 para o grupo controle (p < 0,05); no corpo, 25,9 ± 19,4 para o grupo chagásico e 54,7 ± 47,8 para o controle (p < 0,05); na cauda, 23,4 ± 16,3 para o chagásico e 54,1 ± 29,2 para o controle (p < 0,01), sendo a contagem total de 106,6 ± 71,1 para o chagásico e 226,3 ± 156,5 para o controle (p < 0,01). Nossos achados nos permitiram concluir que: a) ocorreu despopulação neuronal estatisticamente significante no grupo chagásico em relação ao controle, em cada segmento pancreático analisado, bem como no órgão como um todo; b) 50% dos chagásicos tiveram número total de neurônios inferior ao menor número dos controles (80); c) 75% e 91,6% dos chagásicos tiveram número de neurônios inferior, respectivamente, à mediana (171) e à média (226) do grupo controle; d) assim, a despopulação neuronal pancreática foi frequente, porém não constante; e) o fator idade não pareceu ter sido o responsável pela despopulação neuronal dos chagásicos.

We have not found any anatomical studies about the intrapancreatic ganglia in the chronic Chagas' disease. The lesions in these structures could explain at least in part the functional disturbances in the exocrine and endocrine pancreas described in this form of the disease. Thus we decided to morphologically analyze these ganglia. For this analysis, we studied transversal segments of the head, body and tail of the pancreas of twelve chronic chagasics whose mean age were 46,5 ± 9,1 years and fourteen controls, mean age 41,2 ± 11,0 years. These segments were histologically processed and cut into sections in a seriate form up to the end and one cut of each seven was analyzed. For statistical analysis we used the non-parametric test of Mann-Whitney. In the head of the pancreas, the mean count of neurons was 57,3 ± 50,8 in the chagasic group and 117,5 ± 99,0 for the control group (p < 0,05); in the body 25,9 ± 19,4 for the chagasic group and 54,7 ± 47,8 for the control group (p < 0,05); in the tail 23,4 ± 16,3 for the chagasic group and 54,1 ± 29,2 for the control group (p < 0,01), the total count being 106,6 ± 71,1 for the chagasic group and 226,3 ± 156,5 for the controls (p < 0,01). Our data permitted us to conclude that: a) there was a statistically significant neuronal depopulation in the chagasic group, as compared to the control group, in each pancreatic segment that was analyzed, as well as in the organ as a whole; b) 50% of the cagasics had the total number of neurons smaller than the lowest number observed in the controls (80); c) 75% and 91.6% of the chagasics had the number of neurons smaller than, respectively, the median (171) and the mean (226) of the control group; d) therefore, the pancreatic neuronal depopulation was common, but not constant; e) the variable age was apparently not responsible for the neuronal depopulation of the chagasics.

Relatamos o caso de uma ursa polar (Ursus maritimus) de 24 anos de idade, que contraiu a infecção chagásica no Zoológico de Guadalajara, em Jalisco, no México, e morreu de cardite chagásica aguda 15 dias após o início da sintomatologia. Os achados histopatológicos são descritos, bem como a presença de triatomíneos (Triatoma longipennis Usinger) infectados por Trypanosoma cruzi coletados a 5 metros do local onde o animal vivia, na cidade de Guadalajara.

We report a 24 year old female polar bear (Ursus maritimus) who contracted Chagas' infection at the Guadalajara Zoo, in Jalisco, México, and died of acute Chagas' carditis 15 days later. The histopathological findings are described, as well as the presence of triatomines (Triatoma longipennis Usinger) infected with Trypanosoma cruzi collected within 5 meters from the place where the animal lived in the city of Guadalajara.

Descrevem-se as alterações anatomopatológicas do coração obseivadas em um caso agudo de doença de Chagas (DC), com ênfase para as lesões do sistema excito-condutor (SEC) e do sistema newoso autônomo intracardíaco (SNAIC). O exame microscópico, evidenciou pancardite aguda, com numerosas formas amastigotas de Trypanosoma cruzi em miocardiócitos. Constataram-se múltiplos focos inflamatórios no SEC, com ninhos parasitários no nódulo átrio-ventricular e no ramo esquerdo do feixe de His. Na análise das estruturas autonômicas atriais, verificaram-seperiganglionite eperineurite acentuadas, com ou sem infiltração periférica de gânglios e newos pelo exsudato. Não pareceu ocorrer despopulação neuronal cardíaca. O estudo epidemiológico sugeriu transmissão vetorial por Rhodnius pictipes. Este ê o primeiro caso de DC aguda da região amazônica em que se faz a avaliação morfológica sistematizada e concomitante do SEC e do SNAIC.

The histopathology of the heart is described in an acute case of Chagas'disease (DC). Lesions involving the conducting system (SC) and the autonomic intracardiac newous system (SNAIC) are emphasized. Light microscopy showed acute pan-carditis with plenty of Trypanosoma cruzi amastigotes within heart muscle cells. Multiple inflammatory foci were found in the SC with parasitic nests within the atrioventricular node and left his bundle. There were also severe atrial periganglionitis and perineuritis with or unthout peripheral involvement of those structures. Apparently there was no cardiac neuronal depopulation. The epidemiological study suggested transmission through Rhodnius pictipes. To the best of our knowledge, this is the first reported case of acute DC from the amazonic basic with systematized microscopy study of the SC and SNAIC.

Realizou-se estudo comparativo da freqüência e etiologia das mortes súbitas ocorridas no Município de Uberaba, Minas Gerais, em maiores de 15 anos de idade, nos anos de 1980 e de 1990, com o objetivo fundamental de analisar a freqüência atual do óbito súbito em nossa região e o eventual papel de medidas profiláticas e terapêuticas na morte súbita por doença de Chagas. Dos 1226 óbitos de 1980, 54 (4,4%) foram súbitos e destes, 13 (24,1%) etiologia atribuída à doença de Chagas. Em 1990foram pesquisados 1740 óbitos, dos quais 44 (2,5%) foram súbitos; destes, somente 3 (6,8%) devidos â tripanosomose cruzi. Os resultados indicam redução significativa na freqüência tanto da morte súbita em geral, como daquela devida à doença de Chagas, no ano de 1990, em relação ao de 1980, discutindo-se suas prováveis razões.

A comparison was made between the years 1980 and 1990for the frequency and causes of sudden death occurring in the urban and rural areas of the city of Uberaba in individuals older than 15 years. It aims mainly to analyse the current frequency of sudden death in that region and to evaluate the impact, it any, of prophylaxis and therapy on sudden death due to Chagas' disease. For the 1226 deaths cases studied from our 1980, 54 (4.4%) were sudden ones; out of these, 13 (24.1%) were supposedly due to Chagas' disease. For the 1740 death cases studied form our 1990 series, 44 (2.5%) were sudden ones; out of these, only 3 (6.8%) were considered to be due to Chagas' disease. The results indicate a significant decrease in the frequency both for sudden death in general and for sudden death due to Chagas' disease when the year 1990 is compared with 1980. Probable explanations for the findings are discussed.

Em cortes histológicos de fragmentos transmurais congelados da parede livre do ventrículo esquerdo, obtidos em necrópsias de 10 chagásicos crônicos cardiopatas, foram contados os linfócitos CD4+ e os CD8+ do infiltrado miocãrdico. As células foram marcadas com anticorpos monoclonais, usando-se a técnica estreptavidina-biotina. Contou-se: l)os linfócitos em todo o exsudato (LTE) e, separadamente, 2) os linfócitos que estavam em contato ou muito próximos das miocélulas (LCMPM). Foram considerados muito próximos aqueles linfócitos cujos núcleos distavam, dasmiocélulas, menos que seus próprios menores diâmetros nucleares. Os linfócitos CD8+foram mais numerosos do que os CD4+, principalmente entre os LCMPM. A relação CD4/CD8 foi de 0,37 ± 0,20 entre os LTE, mas foi menor quando considerados apenas os LCMPM (0,23 ± 0,11). Dentre os LTE, 34 ± 11% dos CD8+ (contra 24 ± 12% dos CD4+) eram LCMPM. Todas essas diferenças foram estatisticamente significantes. Linfócitos das duas subpopulações foram vistos em íntima relação com miocélulas íntegras ou rotas, e nãoforam observados parasitas nos cortes examinados. Estes achados concordam com a idéia de que a lesão dos miocardiócitos na forma cardíaca da doença de Chagas humana é mediada principalmente por linfócitos T citotóxicos.

Myocardialexsudate CD4+ andCD8+ lymphocytes were counted in transmural left ventricular free wall frozen sections taken from 10 necropsied chronic cardiac chagasic patients. The cells were labeled with monoclonal antibodies using a streptavidin-biotin technique. We counted: 1) lymphocytes in the total exsudate (LTE) and, separately, 2) the lymphocytes touching orvery near to my oc ells (LTVNM). Lymphocytes were considered very near whenever their own nuclear shortest nuclear diameter was larger than their distance from myocells. CD8+ lymphocytes were more numerous than CD4+ lymphocytes, especially among the LTVNM. The LTE CD4/CD8 ratio was 0,37 ± 0,20, but the LTVNM CD4/CD8 ratio was smaller (0,23 ± 0,11). Among theLTE, 34 ± 11% ofCD8+ (against24 + 12% of CD4+) were LTVNM. All these differences were statistically significant. Both subtypes ofT-lymphocytes were found to have an intimate relationship with both ruptured and unruptured myocells, and parasites were not seen. These findings are in accordance with the idea that the myocardial cell lesions in the cardiac form of human Chagas' disease are mediated mainly by T- cytotoxic lymphocytes.

Descreve-se caso de cardiopatia chagásica em menino de nove anos, natural e procedente do sulde Goiás, que desenvolveu insuficiência cardíaca congestiva quatro meses antes do óbito. As reações sorológicaspara doença de Chagas eram reagentes, epositivo o xenodiagnóstico. Os eletrocardiogramas mostraram taquicardia sinusal, extra-sístoles ventriculares e supraventriculares, hemibloqueio anterior esquerdo, bloqueio completo do ramo direito e sinais de sobrecarga de câmaras. O exame ecocardiográfico evidenciou dilatação de câmaras com hipocontratilidade difusa. O quadro se agravou progressivamente, complicando-se por vários episódios pneumônicos, o último dos quais provocou o óbito. A necrópsia, verificou-se, no coração, inflama ção crônica dos três folhetos, com miocardite crônica fibrosante predominando no septo interventricular e no ventrículo esquerdo. As estruturas componentes do sistema excito-condutor mostraram processoflogístico crônico, essencialmente exsudativo, ora discreto, ora moderado. No sistema nervoso autônomo intracardíaco constataram-se focos esparsos de discreta periganglionite crônica, e raros fenômenos degenerativos dos neurônios sem despopulação neuronal.

A case of decompensated chagasic cardiopathy in a nine-year-old boy from the south of the State of Goiás, is described. He developed congestive heart failure four months before death. The sorological reaction for Chagas ' disease and the xenodiagnosis were positive. Electrocardiograms showed sinusal tachycardia, ventricular and supraventricular extrasystoles, left anterior hemiblock, complete right bundle branch block and signs of chambers overload. The echocardiogram demonstraded chamber dilatation with diffuse hypocontractility. He presented a downhill course complicated with several pneumonic episodes, the last one just before death. At necropsy, the heart, exhibited a chronicpancarditis with fibrosing chronic myocarditis involving mainly the interventricular septum and left ventricle. The heart conduction system showed slight to moderate exsudative, inflammatory changes. Scattered foci of slight chronic periganglionitis and rare degenerative phenomena of ganglionar cells were found in intracardiac autonomic nervous, without neuronal depopulation.

Mulher de 80 anos de idade, com doença de Chagas aguda diagnosticada à necrópsia, adquirida, provavelmente, através de triatomíneos no município de Zacoelo de Torres, no Estado de Jalisco, México. Assinala-se a raridade do encontro de casos de doença de Chagas agudo, na faixa etária da paciente. O exame anatomopatológico mostrou comprometimento do coração, esôfago e intestino grosso. Encontrou-se lesões no sistema nervoso autônomo intramural do esôfago e do intestino grosso, sendo estes achados de interesse, por ocorrer em área geográfica onde os megas tem sido pouco relatados.

A case of acute Chagas' disease, diagnosed by necropsy, in a 80-year-old woman, is reported. lt is assumed that infection was acquired through triatomine bite in Zacoelo de Torres, Jalisco State, Mexico. There were lesions due to american trypanosomiasis in the heart, esophagus and bowel. Autonomic nervous lesions were detected in the esophagus and bowel. lt is emphasized the importance of these findings in an area where few cases of megas were reported

Procedeu-se à análise do sistema nervoso autônomo intracardíaco (SNAIC) em 150 cortes histológicos obtidos a partir de fragmentos atriais de homem de 74 anos, falecido de insuficiência cardíaca consecutiva à doença de Chagas aguda (DCA), provavelmente adquirida por via digestiva. Em 10 lâminas havia discretos infiltrados de mononucleares em torno de gânglios e/ou filetes nervosos sem alterações morfológicas significativas dos neurônios; em um preparado constatou-se ganglionite e periganglionite, de moderada intensidade, associadas a alterações neuronais. Epicardite focal, em geral discreta, foi observada em todas as lâminas. Os achados sugerem que a inflamação dos gânglios e fibras do SNAIC na DCA ocorre, pelo menos em parte, da propagação da epicardite adjacente e, que mesmo nos casos fatais da Tripanosomose cruzi as lesões morfológicas do SNAIC podem ser discretas.

An analysis of the intracardiac autonomic nervous system (ICANS) has been made in 150 histological sections obtained from atrial fragments of a 74 year-old man who died of cardiac failure, as a consequence of acute Chagas' disease (ACD), probably acquired via digestive tract. Small quantities of mononuclear infiltrate around ganglia and/or nerve branches without significam morphologic alterations of the neurons were found in 10 slides; another slide showed ganglionitis and periganglionitis of moderate intensity associated to neuronal alterations. Focal epicarditis, usually of slight degree, was observed in ali slides. The findings suggest: a) that the inflammation of ganglia and fibers of the ICANS in the ACD occurs aí least in part by expansion from adjacent epicarditis; b) that even in the fatal cases of the Tripanosomiasis cruzi the pathologic lesions ofthe ICANS may be slight.

Foi feito o estudo quantitativo de mastócitos e de granulócitos eosinófilos no miocárdio de 40 ratos albinos Wistar. Trinta animais encontravam-se nafase tardia da infecção (8º mês) produzida pelas cepas São Felipe, Y e Colombiana do Trypanosoma cruzi, e apresentavam graus variáveis de miocardite crônica; 10 ratos serviram como controles. Miocardite crônica fibrosante (fibrose) ocorreu em 40% dos animais infectados e detectou-se aumento do número de mastócitos nos animais, chagásicos, o qual se associou à infecção mas não à fibrose miocárdica. Não se constatou aumento do número de eosinófilos.

We did mast cell and eosinophil granulocyte counts in the myocardium of forty chronic chagasic Wistar albino rats. We used 10 controls rats and 30 animals with late stage (8th month) infection of São Felipe, Y and Colombian Trypanosoma cruzi strains, with variable degree of chronic myocarditis. We found chronic fibrosing myocarditis (fibrosis) in 40% of the infected animals. It was detected increased mast cell count in the chagasic rats associated with infection and not related with myocardial fibrosis. There was no increase in the eosinophil counts.
