Introduction: Recent progress in the bioengineering of cardiac grafts offers a new therapeutic modality for the regeneration of cardiac tissue after myocardial infarction. CD34 is a marker that expresses all hematopoietic and endothelial precursor cells, and functions as a cell adhesion factor. The antibody corresponding to this marker is used in immunohistochemistry to evaluate the formation of new vessels and the presence of stem cells.
Objective: To evaluate the effectiveness of omentopexy in neovascularization and stem cells.
Methods: Review collecting information published on selected virtual platforms (SciELO – Scientific Electronic Library Online, Google Scholar, Pubmed and Scopus). The search began with descriptors related to the topic, identified through DeCS/MESH in the following: “myocardial ischemia; myocardial revascularization; omentopexy; immunohistochemistry; CD34; stem cells”. They were read by title and summary, with an AND or OR search followed by the full reading of those most closely related to the topic. A total of 23 articles were included.
Results: The greater omentum has a remarkable clinical property in containing sites of tissue damage. It increases its tissue volume in response to foreign and inflammatory particles involving several immunomodulatory cells with progenitor cells, in a process called “omentum activation”.
Conclusion: Cardio-omentopexy associated with mechanical abrasion and myocardial perforations proves to be efficient in inducing neovascularization. The greater omentum promotes stem cells - confirmed by CD34 -, demonstrating great potential as a future therapy to restore areas of ischemic myocardium.

Introduction: Teaching in surgery must follow the evolution of medical techniques, procedures and technology. Since the 90s videosurgery has been developing and nowadays is the gold standard for many procedures in general surgery. As there are different characteristics from conventional surgery, residents need to practice in order to adapt to simulators. The world medical literature has established many training programs among them the Fundaments of laparoscopic Surgery (FLS). Evaluation of surgical technical abilities is a major tool for the residency programs.
Objectives: To assess the effect of training acquiring skills of residents at first and second postgraduated level year (PGY1 and PGY2) before and after practice using low fidelity simulator based on tasks from FLS.
Methods: A prospective study was conducted to determinate skills acquisition of residents at first and second postgraduated level year l (PGY1 and PGY2) before and after standardized practice using low fidelity simulator.
Conclusions: There was a significant improve of the technical abilities reaching levels of improvement. Resident’s performance was evident in the sample groups. First year residents (PGY1) achieved knowledge retention after training similar to the performance of second in the initial assessment, suggesting that the sequenced training carried out is equivalent to one year of practice in the general surgery residency program

Introduction: Teaching in conventional surgery has traditionally been based on the Halstedian model of “see one, do one, and teach one”. However, with the widespread success of videolaparoscopic surgery, the teaching method needed to be adapted to a novel “Do many, always mentored” model. The modern era of surgery demands sequential training with progressively increasing complexity and supervised repetition, in order to acquire specific skills prior to the first surgical procedure.
Objective: To develop a new experimental teaching and training with low cost and low fidelity platform for laparoscopic surgery using synthetic materials, and simple manufacturing.
Method: A platform was built using low-cost materials easily found in hardware stores, consisting of a wooden board and a few hardware materials (threaded bolt, wing nut and bolt, threaded hooks), EVA sheet (ethylene vinylacetat), white crafting paint, and felt-tip pen. The sample consisted of 15 general surgeons with more than 10 years of experience in videolaparoscopy. The evaluation began with an explanatory video lasting 20 min. Next, each surgeon performed 5 recorded tasks for later analysis and scoring. Skills were measured according to the stablished score.
Results: The developed platform was useful and capable of reproducing the tasks performed based on the program of Fundaments of Laparoscopic Surgery. All the surgeons of the sample group achieved the target time proposed for the program.
Conclusion: Was shown that the proposed platform can be built using easily obtainable and low-cost materials, and it proved to be an effective model for training in laparoscopic surgery, being easy to replicate and construct. Additionally, this platform can be easily adapted to various models of tasks using materials similar to those found in other training programs, that are widely practiced and have been globally validated.

Introduction: In the event of myocardial injury, the identification of biomarkers that constitute the cardiac muscle cell, especially troponins, can be observed from blood and saliva samples. The preference for using saliva as a diagnostic fluid is supported by the ease of obtaining it using a non-invasive and potentially pre-hospital method.
Objective: To verify the expression of troponin I in salivary fluid in acute myocardial infarction in emergency and pre-hospital screenings and correlate it with plasma troponin I levels.
Method: Cross-sectional analytical study of diagnostic testing in 27 patients, 9 women and 18 men, with diagnostic confirmation of acute myocardial infarction. Blood and saliva samples were collected and stored and transported over a period of up to 24 hours for troponina I quantification.
Results: The study demonstrated that 44.4% of patients were positive for troponin I in both analyses, with equal dichotomous results in 48.1% of cases. Thus, the salivary troponin test achieved 48% sensitivity and 50% specificity.
Conclusion: It was possible to identify troponin in the saliva fluid of patients suffering from acute myocardial infarction, but the probability of a false positive result was 50% and a false negative result was 52%.

Introduction: Non-melanoma skin cancer (NMSC) comprises a group of neoplasms with a high incidence in the world population. It is divided into basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). As it is highly prevalent, understanding the process of oncogenesis and the relationship with ions, proteins and cellular receptors in CPMN can contribute to the evaluation of new therapies. Objective: To understand the oncogenesis process of non-melanoma skin tumors and its relationship with the immunolocalization of IP3R. Method: Integrative literature review with evidence synthesis. The database was PUBMED; the search strategy: “squamous cell carcinoma, AND/OR basal cell carcinoma, AND/OR IP3R, AND/OR immunohistochemistry”. Works published between 2018 and 2023 were considered for review; 40 works were included, fully read and summarized. Results: NMSC is the most common malignant tumor worldwide, with 75-80% being BCC, and up to 25% being SCC. Molecular interactions in general involve a large participation of tumor suppressor molecules, as well as procto-oncogenes. Furthermore, voltage-dependent ion channels control the cytosolic flow of ions, including calcium. The IP3R (phosphatidyl inositol-3 receptor) allows the exit of calcium from the endoplasmic reticulum so that it can be used by the cell for physiological activities such as proliferation, angiogenesis, motility and invasion capacity. Conclusion: The IP3R, due to its immunohistochemical expression characteristics, appears may also be related to the pathophysiology of NMSC.
A base de dados foi o PUBMED; a estratégia de busca: “carcinoma espinocelular, AND/OR carcinoma basocelular, AND/OR IP3R, AND/OR imunoistoquímica”.

Abstract Background Vitamin K antagonists (VKAs) are the recommended first-line treatment for left ventricular thrombus (LVT); however, direct oral anticoagulants (DOACs) have been considered an alternative therapy. Objectives To evaluate the efficacy and safety of DOACs compared with VKAs therapy in patients with LVT. Methods PubMed, Embase, and Cochrane were systematically searched for randomized clinical trials or cohort studies that compared DOACs versus VKAs for LVT. Risk ratios (RRs) were computed for binary endpoints, with 95% confidence intervals (95% CIs). Statistical significance was defined as p value < 0.05. Results A total of 4 randomized clinical trials and 29 cohort studies were included, with 4,450 patients assigned to either DOACs or VKAs. There was no significant difference between groups for stroke or systemic embolic (SSE) events (RR 0.84; 95% CI 0.65 to 1.07; p = 0.157), stroke (RR 0.73; 95% CI 0.48 to 1.11; p = 0.140), systemic embolic (SE) events (RR 0.69; 95% CI 0.40 to 1.17; p = 0.166), thrombus resolution (RR 1.05; 95% CI 0.99 to 1.11; p = 0.077), any bleeding (RR 0.78; 95% CI 0.60 to 1.00; p = 0.054), clinically relevant bleeding (RR 0.69; 95% CI 0.46 to 1.03; p = 0.066), minor bleeding (RR 0.73; 95% CI 0.43 to 1.23; p = 0.234), major bleeding (RR 0.87; 95% CI 0.42 to 1.80; p = 0.705), and all-cause mortality (RR 1.05; 95% CI 0.79 to 1.39; p = 0.752). Compared with VKAs, rivaroxaban significantly reduced SSE events (RR 0.35; 95% CI 0.16 to 0.91; p = 0.029) and SE events (RR 0.39; 95% CI 0.16 to 0.95; p = 0.037). Conclusions DOACs had a similar rate of thromboembolic and hemorrhagic events, as well as thrombus resolution, compared to VKAs in the treatment of LVTs. Rivaroxaban therapy had a significant reduction in thromboembolic events, compared to VKAs. (VKAs firstline first line LVT (LVT) however (DOACs PubMed Embase RRs (RRs endpoints 95 (95 CIs. CIs . Background: Cardiovascular diseases are the leading cause of death in the world. The use of hematopoietic precursor cells and recent advances made in heart graft bioengineering offer a new therapeutic modality for post-myocardial infarction (MI) nbsp;and cardiac tissue regeneration. CD34 is a marker expressed on all hematopoietic and endothelial precursor cells, and functions as a cell adhesion factor. The antibody corresponding to this marker is used in immunohistochemistry to assess the formation of new vessels and the presence of stem cells. Aim: To evaluate the efficacy of omentopexy as stem cell donor, on previously infarcted myocardium, using immunohistochemically analysis of CD34. Method: Myocardial infarction was generated in four pigs, by ligature of the 1st and 2nd marginal branches of the circumflex artery. In three animals, abrasion of the infarcted epicardium was performed followed by multiple myocardial perforations and the mobilization of the omentum from the abdominal cavity to the mediastinum, sutured on the infarcted area. In the fourth animal, omentopexy was not performed and only the abrasion and perforation of the infarcted area were performed. All hearts were removed for CD34 immunohistochemically evaluation. Results:nbsp; In the samples from the group submitted to omentopexy, there was a 60% increase in angiogenesis, and in the samples from the control animal there was minimal staining.nbsp; Four samples from different sites of each animal, totaling 16 histopathological samples were evaluated. All samples were immunolabelled for CD34. Conclusions: Omentopexy proved to be effective in seeding previously infarcted myocardium with stem angiogenic cells, seen through immunohistochemistry, using CD34 marker.

Introduction: currently, the most relevant applications of perinatal derivatives are in wound healing, for example, the use of amniotic membrane (AM) as a biomaterials source for use in different healing processes. The anastomotic healing compromised by antimetabolic drugs, such as 5-fluorouracil (5-FU), is a potential target of AM. Objective: To evaluate the healing effects of the AM use in rats treated with 5-FU at a dose of 20 mg/kg on the 7th postoperative day, regarding the parameters percentage of type I collagen (mature), cell viability, microvascular density and granulation tissue formation. Method: Thirty-two Wistar rats were submitted to colotomy and colorraphy, separated in 4 groups with 8t rats each and received, by 7 days, different treatments intraperitoneally, once daily from the operation day until sacrifice: 1- saline solution (C), 2- 20 mg/kg 5-FU, 3- 20 mg/kg 5-FU and 4- MA, and MA. Results: Tthe treatment with 5-FU (20 mg/kg – 7 days) induced adverse effects in the anastomotic healing process evidenced by the decrease of type I collagen (mature), cell viability, percentage microvascular density, granulation tissue formation (leukocyte-fibrin crust and angio-fibroblastic proliferation). The use of AM, under these conditions, induced an improvement in type I collagen (mature) percentage. Conclusion: The treatment with 5-FU induced adverse effects on the healing anastomotic process, and the use of AM, under these conditions, induced improvement in type I (mature) collagen percentage.

Introduction: Invasive ductal carcinoma corresponds to the most common histological type of the breast, coexisting with different forms of clinical evolution, histological grading, expression of certain tissue markers and genomic profiles that seek a better understanding of the disease. Objectives: To analyze the correlation of β-catenin and AXL markers with tumor aggressiveness, with reference to overall survival, tumor progression and histopathological prognostic factors. Methods: A study of 101 samples of invasive ductal mammary carcinoma was performed. Those with a diagnosis of ductal type, initially submitted to biopsy or definitive surgical treatment, were included. For control purposes, 20 samples of intraductal carcinoma, 35 of breast fibroadenoma and 10 of breast tissue without any alteration were included. Those undergoing neoadjuvant chemotherapy, those without a tumor sample prior to chemotherapy, those lost to follow-up, and those with incomplete data, were excluded. Results: When the β-catenin expression was analyzed, it was negative. As for AXL, different degrees of expression were observed without statistical significance between them. Conclusion: When analyzing invasive ductal breast adenocarcinoma in TMA, there was no correlation in the expression of ß-Catenin and AXL when compared to overall survival, tumor progression and histological grade.

ABSTRACT Primary cardiac tumors are rare, with an incidence between 0.0017 and 0.19%, and are asymptomatic in up to 72% of cases. Approximately 75% of tumors are benign, and nearly 50% of these are myxomas. Concerning location, 75% of myxomas are in the left atrium, 15 to 20% in the right atrium, and more rarely in the ventricles. The finding of cardiac myxomas usually implies immediate surgical excision to prevent embolic events and sudden cardiac death. Reports with documented growth rate are rare, and the actual growth rate remains a controversial issue. We report the rapid growth rate of a right atrial myxoma in an oligosymptomatic 69-year-old patient, with negative previous echocardiographic history in the last two years, who refused surgery upon diagnosis, enabling monitoring of myxoma growth.

Abstract Introduction Obesity is defined as a multifactorial metabolic syndrome in which there is an excessive number of fat cells within the tissues. It is discussed that intestinal microbiota might have a relevant relation with obesity, since it is relevantly altered in obese patients. Objectives To assess the effect of stool transplantation (ST) in the condition of obesity and its outcomes in an experimental model of cafeteria diet by analyzing histology and weight gain. Methods Forty male Wistar rats were randomly assigned to 5 groups: control (CO), control with antibiotics (CO+ATB), obesity (CAF+ATB), stool transplantation (ATB+ ST) and obesity with stool transplantation (CAF+ATB+ST). During the experiment, obesity induction groups received cafeteria diet, whereas the remaining groups had normal diet ad libitum. After 3 months, daily ST was carried out for 8 weeks by gavage procedure. The animals were euthanized, and the small intestine was harvested for further analysis. Results It was observed that before starting the ST, the cafeteria and normal diet groups had significant weight difference (p<0,0001). In the comparison between CAF+ATB and CAF+ATB+ST during the gavage period, the CAF+ATB+ST group presented lower weight gain (p=0.0017). The histopathological evaluation show that the ATB+ST group did not present intestinal crypt distortion. Conclusion Cafeteria diet resulted in an expected weight gain. In relation to the ST, it has been shown that the procedure is effective in reducing weekly weight gain. Apparently, there was no induction of disabsortive syndrome in nonobese animals that received ST.

ABSTRACT Background: Heart dysfunction and liver disease often coexist because of systemic disorders. Any cause of right ventricular failure may precipitate hepatic congestion and fibrosis. Digital image technologies have been introduced to pathology diagnosis, allowing an objective quantitative assessment. The quantification of fibrous tissue in liver biopsy sections is extremely important in the classification, diagnosis and grading of chronic liver disease. Aim: To create a semi-automatic computerized protocol to quantify any amount of centrilobular fibrosis and sinusoidal dilatation in liver Masson’s Trichrome-stained specimen. Method: Once fibrosis had been established, liver samples were collected, histologically processed, stained with Masson’s trichrome, and whole-slide images were captured with an appropriated digital pathology slide scanner. After, a random selection of the regions of interest (ROI’s) was conducted. The data were subjected to software-assisted image analysis (ImageJ®). Results: The analysis of 250 ROI’s allowed to empirically obtain the best application settings to identify the centrilobular fibrosis (CF) and sinusoidal lumen (SL). After the establishment of the colour threshold application settings, an in-house Macro was recorded to set the measurements (fraction area and total area) and calculate the CF and SL ratios by an automatic batch processing. Conclusion: Was possible to create a more detailed method that identifies and quantifies the area occupied by fibrous tissue and sinusoidal lumen in Masson’s trichrome-stained livers specimens.
