Abstract As most individuals become digital media users, many struggle to find balance in such use. This study adds to emergent research on how digital disconnection experiences reflect on wellbeing (Nguyen et al., 2021; Radtke et al., 2022; Vanden Abeele, 2021) by focusing on motivations and strategies to disconnect from digital media, as well as on outcomes for well-being. We set out to understand teenagers who have voluntarily chosen to disconnect in the post-lockdown period in 2021. Our qualitative study included 20 participants from Portugal between 15 and 18 years old. Among those who consciously chose to disconnect from digital media, motivations arose from realizing that digital media was not bringing enough benefits for the amount of time that they took from users. Specific forms of digital media stood out as particularly problematic for young people, especially social media, but also videogames and pornography. This realization seems to be strongly influenced by the media and is evident in the vocabulary and associations used by our respondents. Social pressure is felt both as causing anxiety when they are connected and when they are disconnected through fear of missing out. However, the group provides support when they engage in a progressive disconnection together. Radical disconnection is rare, especially during the pandemic, and can appear as a solution to a dramatic problem in young people’s lives, but it can also be reverted. More often, participants attempted to self-regulate their use of digital when they acknowledged the advantages of those services as well as their drawbacks. This is not a linear process but rather filled with attempts and reversals as unexpected feelings such as boredom arise. When young people grow different leisure and social habits, they experience positive outcomes of disconnecting from the digital.

Resumo À medida que a maioria dos indivíduos se torna utilizador dos média digitais, muitos lutam para encontrar um equilíbrio nesse uso. Este estudo contribui para a pesquisa emergente sobre como as experiências de desconexão digital se refletem no bem-estar (Nguyen et al., 2021; Radtke et al., 2022; Vanden Abeele, 2021), concentrando-se nas motivações e estratégias para se desconectar dos média digitais, bem como nos resultados para o bem-estar. Propusemo-nos compreender os adolescentes que optaram voluntariamente por desligar no período pós-confinamentos, em 2021. O nosso estudo qualitativo incluiu 20 participantes entre os 15 e os 18 anos em Portugal. Entre aqueles que optaram conscientemente por se desconectar dos média digitais, as motivações surgiram da perceção de que esses meios não traziam benefícios suficientes para a quantidade de tempo que lhes tomavam. Algumas formas específicas de média digitais destacaram-se como particularmente problemáticas para os jovens, especialmente os média sociais, mas também os videojogos e a pornografia. Essa perceção parece ser fortemente influenciada pelos média, o que se evidencia no vocabulário e nas associações usadas pelos entrevistados. A pressão social é sentida como causadora de ansiedade quando estão conectados e quando estão desconectados por meio do fear of missing out (medo de perder o que se está a passar). No entanto, o grupo é fonte de apoio quando eles se envolvem numa desconexão progressiva em conjunto. A desconexão radical é rara, principalmente em tempos de pandemia, e pode aparecer como solução para um problema dramático na vida dos jovens, mas também pode ser revertida. Mais frequentemente, os participantes tentaram autorregular o seu uso do digital, quando reconheceram as vantagens desses serviços, bem como as suas desvantagens. Este não é um processo linear, mas sim marcado por tentativas e reversões, à medida que surgem sentimentos inesperados, como o tédio. Quando os jovens desenvolvem diferentes hábitos sociais e de lazer, experimentam resultados positivos de desconexão do digital.

Resumo: Os média alternativos abordam assuntos que não são tratados pelos meios de comunicação tradicionais e dão enfoque a temáticas voltadas à defesa dos direitos humanos. Surgidos no ambiente digital, tiram partido dos baixos custos de distribuição e das potencialidades de participação do público. De forma crescente, os média alternativos utilizam plataformas de crowdfunding ou financiamento coletivo como modelo de manutenção das atividades. Estas plataformas que permitem o patronato não alojam nem publicam conteúdo, mas estão inseridas num ecossistema com outras plataformas que visam obter dados e transformá-los em valor económico. O presente artigo mapeia a forma como média alternativos de Portugal, Espanha e Brasil articulam as plataformas de crowdfunding com as suas estratégias de financiamento, por um lado, e com as plataformas de redes sociais, por outro. O estudo pretende debater as possibilidades e limitações das diferentes plataformas de financiamento coletivo para estes meios. O desenho metodológico inclui entrevistas pessoais, observação online sobre os média alternativos e uma análise de conteúdo sobre o seu financiamento e concretamente as plataformas digitais de financiamento. Os média alternativos articulam o seu uso de plataformas de crowdfunding com as de redes sociais, num trabalho constante de demonstração da relevância do tipo de jornalismo que praticam e do seu contributo social em busca do envolvimento do público enquanto financiador.

Abstract: Alternative media appeared in the digital environment, taking advantage of the low distribution costs and the potential for public participation. Increasingly, they use crowdfunding or collective financing platforms as a model for maintaining activities. This article maps how alternative media from Portugal, Spain and Brazil articulate crowdfunding platforms with their financing strategies, on the one hand, and with social media platforms, on the other. The study aims to discuss the possibilities and limitations of the different crowdfunding platforms for these means. The methodological design includes personal interviews, online observation about alternative media and a content analysis about their financing and specifically the digital financing platforms. Alternative media articulate their use of crowdfunding platforms with those of social networks, in a constant work of demonstrating the relevance of the type of journalism they practice and their social contribution in search of public involvement as a funder.

RESUMO O fruto de gueroba pode ser utilizado na produção de farinhas com potencial para o desenvolvimento de novos produtos oriundos da sociobiodiversidade do Cerrado. Objetivou-se estimar a cinética de secagem, bem como determinar o coeficiente de difusão efetivo e a energia de ativação para a polpa dos frutos de gueroba submetida a diferentes temperaturas de secagem. Os frutos de gueroba foram despolpados manualmente, retirando o mesocarpo com o epicarpo, sendo esse material identificado como a polpa. O material foi submetido à secagem em estufa nas temperaturas de 40, 50, 60 e 70 °C. Aos dados experimentais foram ajustados modelos de regressão não linear. O modelo mais adequado foi selecionado com base no coeficiente de determinação, erro médio relativo e estimado, teste de Qui-quadrado, AIC e BIC. Com o aumento da temperatura de secagem, menor é o tempo do processamento para se atingir o mesmo teor de água, devido ao aumento da difusividade da água no interior do produto. O modelo de Midilli apresentou o melhor ajuste aos dados experimentais obtidos. Os coeficientes de difusão efetivos da polpa dos frutos de gueroba apresentaram magnitudes entre 3,11 x 10-9 a 5,84 x 10-9 m2 s-1 para as temperaturas de 40 a 70 °C. A energia de ativação do processo foi de 18,34 kJ mol-1.

ABSTRACT The ‘Gueroba’ fruit can be used to produce flours with potential for the development of new products from the ‘Cerrado’ socio-biodiversity. The objective was to estimate the drying kinetics and determine the effective diffusion coefficient and activation energy for the pulp of ‘gueroba’ fruits subjected to different drying temperatures. ‘Gueroba’ fruits were manually pulped, removing the mesocarp with the epicarp, and this material was identified as the pulp. The material was subjected to oven drying at temperatures of 40, 50, 60 and 70 °C. Nonlinear regression models were fitted to the experimental data. The most adequate model was selected through the coefficient of determination, mean relative and estimated errors, Chi-square test, AIC and BIC. As the drying temperature increases, the processing time to achieve the same moisture content decreases, due to the increase in water diffusivity inside the product. The Midilli model showed the best fit to the experimental data obtained. The effective diffusion coefficients of the pulp of ‘gueroba’ fruits showed magnitudes between 3.11 x 10-9 to 5.84 x 10-9 m2 s-1 for temperatures from 40 to 70 °C. The activation energy of the process was 18.34 kJ mol-1.

O objetivo foi descrever as características da infecção por SARS-CoV-2 entre os membros de domicílios, com um caso primário confirmado de COVID-19, em distritos com baixa carga de casos em Lima, Peru, em comparação com um distrito com alta carga. Estudo retrospectivo de revisão de banco de dados secundário. As informações foram coletadas em uma atividade de vigilância epidemiológica em contatos próximos (coabitantes), em 52 domicílios em Lima, com um único membro com COVID-19. Foi realizada uma reavaliação em 10 domicílios. Variáveis epidemiológicas e clínicas foram avaliadas; e sua associação com o resultado do teste sorológico rápido (presença de IgG, IgM ou ambos). Os casos secundários foram encontrados em 40 domicílios, representando uma taxa média de identificação de 49,9% por domicílio. Foi encontrada uma taxa de ataque secundário entre coabitantes de 53% (125 casos), com 77,6% dos casos sendo sintomáticos (relação sintomático/assintomático: 3,5). A presença de febre e/ou calafrios foi encontrada em 40% das pessoas com resultado positivo, seguida de dor de garganta em 39,2%. Ageusia e anosmia estiveram presentes em 22,4% e 20,8% dos casos, respectivamente. Quando há um caso primário de COVID-19 em casa, a taxa de ataque secundário para essa infecção é de 53%; No entanto, em uma proporção significativa dos domicílios avaliados não houve caso positivo, além do caso primário. As características epidemiológicas e clínicas encontradas neste caso foram consistentes com o que já foi relatado em outras séries internacionais.

The study aimed to describe the characteristics of SARS-CoV-2 transmission among members of households with a confirmed primary case of COVID-19 in districts with low burden of cases in Lima, Peru, compared to a district with high burden. This was a retrospective study with a secondary database review. Information was collected from an epidemiological surveillance activity in close contacts (household members) in 52 households in Lima, with a single member with COVID-19. Reevaluation was conducted in 10 households. The study evaluated epidemiological and clinical variables and their association with the result of the rapid serological test (presence of IgG, IgM, or both). Secondary cases were found in 40 households, representing mean identification of 49.9% per household. Secondary attack rate in household members was 53% (125 cases), and symptomatic individuals accounted for 77.6% of cases (symptomatic/asymptomatic ratio: 3.5). Presence of fever and/or chills was found in 40% of persons with positive test results, followed by sore throat with 39.2%. Ageusia and anosmia were present in 22.4% and 20.8% of cases, respectively. When there was a primary case of COVID-19 in the household, the secondary attack rate was 53%; however, in an important proportion of households there were no positive cases other than the primary case. The epidemiological and clinical findings were consistent with reports from other international series.

El objetivo fue describir las características de la infección por SARS-CoV-2 entre miembros de hogares, con un caso confirmado primario de COVID-19, en distritos de baja carga de casos en Lima, Perú, en comparación con un distrito de alta carga. Estudio retrospectivo de revisión de base de datos secundaria. Se recolectó información proveniente de una actividad de vigilancia epidemiológica en contactos cercanos (cohabitantes), en 52 hogares de Lima, con un solo miembro con COVID-19. En 10 hogares se realizó una reevaluación. Se evaluaron variables epidemiológicas y clínicas; y su asociación con el resultado a la prueba serológica rápida (presencia de IgG, IgM o ambas). En 40 hogares se encontraron casos secundarios, lo que representa un 49,9% de identificación en promedio por hogar. Se encontró una tasa de ataque secundaria entre cohabitantes de 53% (125 casos), siendo sintomáticos un 77,6% de casos (razón sintomáticos/asintomáticos: 3,5). La presencia de fiebre y/o escalofríos se encontró en el 40% de las personas con resultado positivo, seguido del dolor de garganta en el 39,2%. Resumo Os casos relatados descrevem a associação entre glomeruloesclerose segmentar e focal (GESF) colapsante e infecção aguda pelo vírus da dengue. Nas duas instâncias os pacientes foram diagnosticados com infecção pelo vírus da dengue e apresentaram doença renal grave, com síndrome nefrótica e insuficiência renal aguda. A biópsia renal revelou a presença de GESF colapsante. O primeiro paciente, um homem de 27 anos, foi diagnosticado com infecção pelo vírus da dengue e desenvolveu síndrome nefrótica após duas semanas de doença. Ele foi tratado com metilprednisolona por três dias e furosemida endovenosa. O paciente evoluiu bem, embora sua função renal não tenha se recuperado plenamente. O segundo paciente, um homem de 32 anos, foi diagnosticado com apresentação clínica mais discreta de infecção pelo vírus da dengue. Ele tinha histórico de síndrome nefrótica na infância, o que pode ter causado a recidiva. O paciente foi tratado com furosemida endovenosa e também não recuperou plenamente sua função renal. Os dois casos destacam a possível implicação da infecção pelo vírus da dengue na etiologia da variante colapsante da GESF. Os profissionais de saúde devem estar preparados para identificar casos semelhantes.

Abstract The reported cases describe the association between collapsing focal segmental glomerulosclerosis (FSGS) and acute dengue virus infection. In both cases, patients were diagnosed with dengue virus infection and had a severe kidney disease, with nephrotic syndrome and acute kidney injury. Kidney biopsy was performed and showed collapsing FSGS. The first patient, a 27-year-old man, was diagnosed with dengue virus infection and developed nephrotic syndrome after two weeks of illness. He was treated with methylprednisolone for three days and intravenous furosemide. This patient evolved well, although his renal function did not fully recover. The second patient, a 32-year-old man, was diagnosed with a milder clinical presentation of dengue virus infection. He had a past medical history of nephrotic syndrome in childhood, which might have caused its relapse. This patient was treated with intravenous furosemide and also did not fully recover renal function. These cases highlight the possible implication of dengue virus infection in the etiology of collapsing variant of FSGS. Healthcare professionals should be prepared to identify similar cases.

Overview and Aims: Cases of radiation-induced endometrial cancer after cervical carcinoma are characteristically different than sporadic cases of endometrial cancer as they tend to be more aggressive, diagnosed in a more advanced stage, and with a poorer outcome. Therefore, it is important to maintain surveillance, be aware of the warning signs and know how to approach these secondary cancers in order to assure the best outcome for these patients. The goal of our study is to describe a case series of patients with radiation-induced endometrial cancer after cervical carcinoma followed at our institution. Study design: Retrospective, cross-sectional study. Population: Patients with a diagnosis of endometrial cancer who had previously received definitive radiation treatment for cervical cancer. Four patients met our inclusion criteria. Methods: Analyzed parameters included patient demographics, age upon diagnosis, type of radiation therapy, histological grade and subtype of the primary and the secondary cancers. Results: The mean age at diagnosis of the primary cervical cancer was 64 years. All of the patients had received definitive radiation therapy and chemotherapy. The mean latency period between the initial diagnosis of cervical cancer and the development of the endometrial carcinoma was 5.3 years. Two patients had stage I disease, one had stage II and one had stage III. Regarding the histological type, there was one case of endometrioid carcinoma, one of carcinosarcoma and two of serous carcinoma. All of the patients underwent hysterectomy and bilateral salpingo-oophorectomy and three received chemotherapy as adjuvant therapy. Three patients maintain their follow-up in our institution without any evidence of disease. Conclusion: Regular surveillance based on anamnesis and physical examination is of the most importance in women that underwent radiotherapy for cervical carcinoma. Imaging tests can aid in this particular subset of patients as cervical stenosis might hide an underlying condition. In our sample, only one patient complained of abnormal uterine bleeding and three patients had an aggressive histological type of endometrial cancer diagnosed. The latency period between the primary and secondary cancers was shorter than expected and might be related to small size of the sample.

ABSTRACT Objective: To determine the additional diagnostic performance of a rapid serological test for detection of IgM and IgG antibodies compared to the real-time polymerase chain reaction (RT-PCR) test; for detection of SARS-CoV-2. Materials and methods: A cross-sectional study was carried out including patients hospitalized for COVID-19 in 3 hospitals, health workers exposed to the infection and outpatients who met suspicious case criteria, all of which underwent the molecular test (RT-PCR) and the rapid serological test. The additional diagnostic performance of rapid serological test was evaluated in comparison to molecular tests. Likewise, an approximation was made to the sensitivity and specificity of the rapid serological test. Results: 144 people were included. With the rapid test, 19.4% of positive results were obtained compared to 11.1% in the molecular test (p = 0.03). The rapid serological test detected 21 cases that had been negative by the initial (RT-PCR), providing an additional diagnostic performance of 56.8% compared to the RT-PCR. The additional diagnostic performance was 50.0% during the first week, 70.0% during the second week and 50.0% during the third week of symptom onset. The sensitivity of the rapid serological test was 43.8% and the specificity of 98.9%. Conclusions: The rapid serological test was able to detect a greater number of cases than those detected by the molecular test especially after the second week of onset of symptoms. It also showed high specificity. It is therefore useful as a complementary test to RT-PCR, especially during the second and third week of illness.

RESUMEN Objetivos: Determinar el rendimiento diagnóstico adicional de una prueba serológica rápida que detecta anticuerpos IgM e IgG contra SARS-CoV-2 en relación a la reacción en cadena de polimerasa reversa en tiempo real (RT-PCR). Materiales y métodos: Se realizó un estudio transversal incluyendo pacientes hospitalizados por COVID-19 en tres hospitales, trabajadores de salud expuestos a la infección y pacientes ambulatorios que cumplían criterios de caso sospechoso, a quienes se les realizó la prueba molecular (RT-PCR) y la prueba serológica rápida. Se evaluó el rendimiento diagnóstico adicional de las prueba serológica rápida en relación a la molecular. Asimismo, se realizó la estimación de sensibilidad y especificidad de dichas pruebas. Resultados: Se incluyeron 144 personas. La prueba serológica rápida obtuvo un 19,4% de resultados positivos en comparación con un 11,1% en la prueba molecular (p=0,03). La prueba serológica rápida detectó 21 casos que habían resultado negativos por el RT-PCR inicial y el rendimiento diagnóstico adicional fue de 56,8% en comparación al RT-PCR. El rendimiento diagnóstico adicional fue 50,0% durante la primera semana, 70,0% durante la segunda y 50,0% durante la tercera semana de inicio de síntomas. La sensibilidad de la prueba serológica rápida fue de 43,8% y la especificidad del 98,9%. Conclusiones: La prueba serológica rápida logró detectar un mayor número de casos respecto a la molecular, sobre todo a partir de la segunda semana de inicio de síntomas. Además, presentó una alta especificidad. Los resultados mostrarían su utilidad como prueba complementaria a la prueba molecular, especialmente durante la segunda y tercera semana de enfermedad.

La prueba serológica rápida detectó 21 casos que habían resultado negativos por el RT-PCR inicial y el rendimiento diagnóstico adicional fue de 56,8% en comparación al RT-PCR. El rendimiento diagnóstico adicional fue 50,0% durante la primera semana, 70,0% durante la segunda y 50,0% durante la tercera semana de inicio de síntomas. La sensibilidad de la prueba serológica rápida fue de 43,8% y la especificidad del 98,9%. Conclusiones: La prueba serológica rápida logró detectar un mayor número de casos respecto a la molecular, sobre todo a partir de la segunda semana de inicio de síntomas. Además, presentó una alta especificidad. Los resultados mostrarían su utilidad como prueba complementaria a la prueba molecular, especialmente durante la segunda y tercera semana de enfermedad.

The mean age of diagnosis of ovarian epithelial tumors is 63 years, but 3-17% affects women under the age of 40 years. In this age group, standard treatment conditions an iatrogenic menopause, so that conservative therapeutic options are increasingly weighted at early ages. Estrogen therapy is the most effective treatment for the symptoms associated with menopause, but doubts about its use in these women are frequent. The review of the literature shows that after treatment of ovarian cancer, the use of hormone therapy does not influence relapse rates nor overall survival.
