Abstract Austrodiplostomum Szidat & Nani, 1951 is a genus of parasitic digenetic trematodes widely distributed in the Neotropical region. Infects a wide variety of species, families and requests for freshwater fish. We identify samples of Austrodiplostomum sp, based on metacercariae isolates from eyes of tambaqui (Colossoma macropomum), a fish of high commercial importance in Brazil, and widely consumed by the population of the northern region. The sequences obtained clustered with A. compactum. This is the first report of the occurrence of diplostomids in farmed tambaqui in Amazonia.

Resumen Objetivo Traducir, adaptar culturalmente y validar el contenido de la Preterm Infant Breastfeeding Behaviour Scale (PIBBS). Métodos Estudio metodológico, cuyas etapas fueron: traducción inicial, síntesis de las traducciones, retrotraducción, revisión de las versiones traducidas, validación de contenido, prueba piloto y envío de la documentación al Comité de Revisión de Adaptación Transcultural. Tanto la escala original, como la traducida y adaptada, contienen seis ítems y cada uno contiene de tres a seis subítems con puntuación que varía de 0 a 6. La puntuación total puede variar de 0 a 20. Para la etapa de validación se realizó el cálculo de índice de validez de contenido (IVC) de todos los ítems de la escala. Se consideró IVC mínimo de 0,8 como aceptable para la evaluación de cada ítem individualmente, y de 0,90 para la evaluación general del instrumento. Resultados La versión brasileña de la escala PIBBS fue denominada “Escala Comportamental de amamentação do pré-termo” (Escala de Comportamiento de Lactancia del Recién Nacido Prematuro). La escala fue traducida y adaptada, y su contenido fue validado, con una equivalencia conceptual e idiomática que varió de 83,3 % a 100 %. El índice de validez de contenido fue de 0,93. Conclusión Después de su traducción, adaptación y validación por parte de los profesionales de la salud, la escala demostró ser válida, de fácil aplicación y con un lenguaje de fácil comprensión. La escala permitirá que los profesionales de la salud puedan observar el comportamiento del prematuro y orientar a la madre, con el objetivo de estimular la lactancia materna exclusiva, ya que el proceso de lactancia del prematuro es complejo. Su uso en investigaciones futuras y en la práctica clínica permitirá complementar análisis psicométricos más sólidos.

Foi considerado IVC mínimo de 0,8 como aceitável, para avaliação de cada item individualmente; e de 0,90, para avaliação geral do instrumento. Resultados A versão brasileira da escala PIBBS foi denominada de Escala Comportamental de amamentação do pré-termo. A escala foi traduzida, adaptada e seu conteúdo foi validado, alcançando equivalência conceitual e idiomática que variou de 83,3% a 100%. O índice de validade de conteúdo foi de 0,93. Conclusão Após a tradução, adaptação e validação pelos profissionais de saúde a escala se mostrou válida, de fácil aplicação e linguagem de fácil entendimento. A escala permitirá que os profissionais de saúde possam observar o comportamento do prematuro e orientar a mãe, estimulando assim o aleitamento materno exclusivo, já que o processo de amamentação do prematuro é complexo. Sua utilização em pesquisas futuras e prática clínica permitirá complementar as análises psicométricas mais robustas.

Abstract Objective To translate, culturally adapt and validate the Preterm Infant Breastfeeding Behavior Scale (PIBBS) content Methods This is a methodological study, whose steps were: initial translation; synthesis of translations; back-translation; review of translated versions; content validity; pre-test; and sending the documentation to the Cross-Cultural Adaptation Review Committee. Both the original scale and the translated and adapted scale have six items, and each item contains 03 to 06 sub-items with score scores ranging from 0 to 6. The total score can range from 0 to 20. For the validity step, the Content Validity Index (CVI) of all scale items was calculated. A minimum CVI of 0.8 was considered acceptable for assessing each item individually, and 0.90, for generally assessing the instrument. Results The Brazilian version of PIBBS was called the “Escala Comportamental de Amamentação do Pré-Termo”. The scale was translated, adapted and its content was validated, achieving conceptual and idiomatic equivalence ranging from 83.3% to 100%. The CVI was 0.93. Conclusion After translation, adaptation and validity by health professionals, the scale proved to be valid, easy to apply and easy to understand language. The scale will allow health professionals to observe preterm infants’ behavior and guide mothers, thus encouraging exclusive breastfeeding, since the breastfeeding process of a preterm infant is complex. Its use in future research and clinical practice will complement more robust psychometric analyses.

Resumo A esclerose peritoneal encapsulante é uma complicação incomum, mas grave, da diálise peritoneal. Na maioria dos casos, os sintomas aparecem após a suspensão da diálise peritoneal, o que dificulta seu diagnóstico. Apresentamos o caso de um homem caucasiano de 44 anos de idade que esteve em diálise peritoneal por 6 anos e 3 meses e foi transferido para hemodiálise devido a falha de ultrafiltração. Durante seus últimos meses em diálise peritoneal, ele desenvolveu anorexia e astenia, que foram inicialmente atribuídas à inadequação da diálise. Após a indução de hemodiálise, o paciente desenvolveu dor abdominal, aumento do volume abdominal, obstipação alternada com diarreia, e perda de peso. A tomografia computadorizada mostrou ascite de novo. Foi considerado um diagnóstico de esclerose peritoneal encapsulante precoce, e o tratamento foi prontamente iniciado com suporte nutricional, prednisolona oral e tamoxifeno por um ano. O paciente progrediu com resolução dos sintomas. Um mês após o término do tratamento, ele foi submetido a um transplante renal bem-sucedido e permanece sem maiores intercorrências. Um alto nível de suspeita clínica é crucial para o diagnóstico precoce da esclerose peritoneal encapsulante, uma vez que a doença pode ser fatal em estágios avançados. Este caso destaca que, com tratamento precoce, o transplante renal pode ser realizado com sucesso após um episódio de esclerose peritoneal encapsulante.

Abstract Encapsulating peritoneal sclerosis is an uncommon but serious complication of peritoneal dialysis. In most cases, the symptoms appear after peritoneal dialysis withdrawal, which hampers its diagnosis. We present the case of a 44-years-old Caucasian male who had been on peritoneal dialysis for 6 years and 3 months and was switched to hemodialysis due to ultrafiltration failure. During his last months on peritoneal dialysis, he developed anorexia and asthenia, which were initially attributed to dialysis inadequacy. After hemodialysis induction, the patient developed abdominal pain, increased abdominal volume, obstipation alternating with diarrhea, and weight loss. Computed tomography showed de novo ascites. A diagnosis of early encapsulating peritoneal sclerosis was considered, and treatment was promptly initiated with nutritional support, oral prednisolone, and tamoxifen for one year. The patient progressed with resolution of the symptoms. One month after the end of the treatment, he underwent a successful kidney transplant and remain without any major intercurrences. A high level of clinical suspicion is crucial for the early diagnosis of encapsulating peritoneal sclerosis as the disease can be fatal in advanced stages. This case highlights that with early treatment, kidney transplantation can be successfully performed after an episode of encapsulating peritoneal sclerosis.

Resumo Antecedentes A esclerose múltipla (EM) e a doença do espectro neuromielite optica (NMOSD) são as doenças autoimunes mais comuns do sistema nervoso central (SNC). Ambas apresentam curso crônico com recaídas (surtos) e exigem tratamento com drogas modificadoras de doenças (DMDs) para a prevenção de atividade inflamatória. As DMDs levam à imunomodulação ou imunossupressão através de diversos mecanismos (por exemplo deslocando e/ou suprimindo subpopulações linfocitárias ou alterando perfil de produção de citocinas). Desta forma, os pacientes com EM ou NMOSD são mais propensos a complicações infecciosas, as quais podem levar ao agravamento de suas doenças de base. Objetivo Apresentar estratégias viáveis para mitigar o risco de infecção de pacientes com EM ou NMOSD sob tratamento. Métodos Revisão bibliográfica focada em manejo de risco de infecção com ênfase em vacinação, medidas específicas de tratamento e particularidades de doenças infecciosas endêmicas do Brasil. Conclusão Propomos um calendário de vacinação, rotina de triagem infecciosa e medidas profiláticas baseadas em evidências científicas atuais. A conscientização das doenças tropicais emergentes é necessária devido a evidências de eventos desmielinizantes e possíveis casos parainfecciosos de EM e NMOSD.

Abstract Background Multiple sclerosis (MS) and neuromyelitis optica spectrum disorders (NMOSD) are the most common autoimmune diseases of the central nervous system (CNS). They present chronic relapsing courses that demand treatment with disease-modifying drugs (DMDs) to prevent inflammatory activity. Disease-modifying drugs lead to immunomodulation or immunosuppression through diverse mechanisms (e.g., shifting lymphocyte and cytokine profile, suppressing specific lymphocyte subpopulations). Thus, patients are more prone to infectious complications and associated worsening of disease. Objective To present feasible strategies for mitigating the infection risk of MS and NMOSD treated patients. Methods Targeted literature review concerning the management of infection risk with an emphasis on vaccination, therapy-specific measures, and particularities of the Brazilian endemic infectious diseases' scenario. Conclusion We propose a vaccination schedule, infectious screening routine, and prophylactic measures based on the current scientific evidence. Awareness of emergent tropical diseases is necessary due to evidence of demyelinating events and possible parainfectious cases of MS and NMOSD.

Abstract Ankylosis of the temporomandibular joint (TMJ) is characterized by a bony or fibrous fusion between the articular fossa and the condyle. It represents one of the greatest therapeutic challenges in oral and maxillofacial surgery. Several treatments have been used for its management, such as gap arthroplasty, interpositional arthroplasty, and TMJ reconstruction with autogenous bone graft and alloplastic prostheses. A 39-year-old Black male patient developed a complex mandibular symphysis fracture associated with bilateral high condylar fractures after a motorcycle accident. In the first approach, osteosynthesis was performed to reduce and fix symphysis fractures and conservatively treat the condyles. However, the patient did not proceed with physiotherapy and adequate follow-up, evolving into bilateral ankylosis. Subsequently, he underwent interpositional arthroplasty with a temporal muscle flap. The treatment of ankylosis is imperatively surgical and aims to release the joint and reanatomize the region, restoring functionality to the stomatognathic system. The choice of technique depends on many factors, including age. In children, gap arthroplasty is the most frequent choice because it helps in the centrality of condylar growth. In adult patients, gap arthroplasty with interposition of the temporal muscle is eligible, as it restores function and reduces the recurrence of bone fusion and consequent re-ankylosis. Re s u m o A anquilose da articulação temporomandibular (ATM) é caracterizada pela fusão óssea ou fibrosa da fossa articular com a cabeça da mandíbula. Representa um dos maiores desafios terapêuticos na cirurgia oral e maxilofacial. Tratamentos diversos têm sido utilizados para o seu manuseamento, tais como artroplastia de gap, artroplastia interposicional e reconstrução da ATM com enxerto ósseo autógeno e próteses aloplásticas. Paciente do sexo masculino, 39 anos, melanoderma, vítima de acidente motociclístico evoluiu com fratura complexa de sínfise mandibular associada a fraturas altas dos côndilos bilateralmente. Numa primeira abordagem, foi realizada a osteossíntese para redução e fixação das fraturas da sínfise e tratamento conservador dos côndilos. Entretanto, o paciente não prosseguiu com a fisioterapia e acompanhamento orientados, evoluindo para anquilose bilateral. Foi posteriormente submetido a artroplastia interposicional com retalho do músculo temporal. O tratamento das anquiloses é imperativamente cirúrgico e tem como objetivo a liberação da articulação e reanatomização da região, devolvendo funcionalidade ao sistema estomatognático. A escolha da técnica depende de alguns fatores, incluindo a idade. Em crianças, a artroplastia em gap é a escolha mais frequente pois auxilia na centralidade do crescimento condilar. Em pacientes adultos, a artroplastia em gap com interposição do músculo temporal é elegível, pois devolve a função e diminui a recidiva da fusão óssea e consequente re-anquilose

Abstract The greatest and noblest challenge of mining engineering for the present and the future of iron ore treatment could be considered as the pursuit of optimizing the use of mineral resources. The use of mining tailings, in the mineral sector itself, or in another industrial branch, meets the needs of circular economy as it increases the useful service life of this material. In order to investigate this scenario, a technological characterization of a sample from a tailings dam was carried out along with magnetic concentration tests in a Wet High-Intensity Magnetic Separator (WHIMS) to propose a possible concentration route. In relation to the characterization itself, the results demonstrated a relative density of 3.04 x 103 Kg/m3and an average superficial specific area of 3.75 cm3/g. The granulometric analysis classified the material as fine, with d90 of 0.110 mm and d50 ≅ 0.049 mm. Quartz, hematite, goethite, kaolinite and manganese oxide were identified in the sample. The test which presented the best results (66.83% Fe and 1.74% SiO2) consisted of the Rougher, Cleaner and Recleaner phases, using a 1.5 mm grooved GAP matrix, a 7,000 Gauss magnetic field, 30% solids, and water pressure at 0.5 kgf/cm2 (49.0 x 103 Pa). Finally, the conclusive results indicate that the studied material can be concentrated through magnetic concentration, keeping in mind the specifications concerning the pellet feed fines commercial product. Besides the financial gain, the activity prolongs the durability of the tailings dam and reduces the environmental impacts associated with these structures.

Abstract This article proposes a structure for family medicine remote consultations, exploring strategies to overcome communicational barriers. The authors propose twelve steps divided into three phases. Phase I. Preparation. Step 1. Place choice: evaluate room conditions. Step 2. Connection verification: verify telephone, computer, and printer availability and the presence of a good internet connection. Step 3. Room preparation: make sure there is good lighting and that all objects you may need are properly placed. Step 4. Organizing ideas: read the previous medical records and any important alerts. Phase II. Remote consultation. Step 5. Sessions make ready: make sure you have the correct patient’s contact, session passwords as well as the computer’s department helpline number. Step 6. First minutes: make sure video and sound are working correctly; confirm patient’s identification and ensure privacy. Step 7. Quick assessment: quick assessment if the patient is severely ill or not; if severe illnesses, write a short anamnesis and assess vital signs in order to decide whether or not to send the patient to seek medical attention. Step 8. Exploration: present illness anamnesis. Step 9. Evaluation: this is the most complex step; patient descriptions, live video images or emailed afterward, measurements with patient’s medical devices, and the use of various medical scales are of great help. Step 10. Decision: gathering all the information in other to manage the correct follow-up procedures with patients’ agreement. Phase III. Final. Step 11. Closure: make sure that all topics were discussed, and that the patient understands the plan. Step 12: Final reflection: medical records should be finished and a doctor’s self-reflection on the consultation should take place. For breaking bad news SPIKES protocol should be used. This article was thought of as a consultation guide for family medicine remote consultations in order to improve doctors’ approach during the COVID-19 pandemic.

Resumo Este artigo propõe uma estrutura para as teleconsultas de medicina geral e familiar, explorando estratégias para ultrapassar barreiras comunicacionais. Propõem-se doze passos divididos em três fases. Fase I: Preparação. Passo 1. Escolha do local: avaliar as condições do gabinete. Passo 2. Verificação da conexão: verificar a disponibilidade de telefone, computador e de impressora e a existência uma boa conexão de Internet. Passo 3. Preparação do consultório: certificar-se de que há boa luminosidade e que todos os objetos que possam a vir a ser necessários estão posicionados corretamente. Passo 4. Organização do pensamento: ler os registos médicos prévios e algum alerta importante. Fase II: Teleconsulta. Passo 5. Preparar a sessão: certificar-se de que tem o contacto correto do utente, password da sessão bem como o número telefónico de uma linha de apoio informático. Passo 6. Primeiros minutos: certificar-se de que o vídeo e o áudio estão a trabalhar corretamente; confirmar a identificação do paciente e garantir a sua privacidade. Passo 7. Avaliação rápida: avaliar rapidamente se o paciente está severamente doente ou não; se doença grave, escrever uma pequena história clínica e avaliar os parâmetros vitais de forma a decidir se deve encaminhar-se o doente para uma observação médica. Passo 8. Exploração: história da doença atual. Passo 9. Avaliação: este é o passo mais complexo; descrições do paciente, imagens de vídeo em direto ou enviadas posteriormente para o email, valores de dispositivos médicos que o paciente possua e uso de várias escalas médicas são uma excelente ajuda. Passo 10. Decisão: interpretação das monitorizações de forma a decidir o melhor seguimento em consenso com o doente. Fase III: Final. Passo 11. Encerramento: deve confirmar-se que foram abordados todos os temas propostos para a consulta e verificar se o utente compreendeu o plano. Passo 12. Reflexão final: finalizar os registos clínicos e realizar uma autorreflexão da consulta que teve lugar. Para a comunicação de más notícias deve usar-se o protocolo de SPIKES. Este artigo foi pensado como um guia de consulta para teleconsultas de medicina geral e familiar como forma de melhorar a abordagem médica durante a pandemia COVID-19.

Resumo Introdução: As sociedades de Gastroenterologia criaram recomendações de modo a reduzir a exposição não necessária ao vírus da síndrome respiratória aguda grave (SARS-CoV-2). O nosso objetivo é avaliar a perspectiva nacional dos gastroenterologistas sobre o impacto da COVID-19 e avaliar o impacto da reorganização do nosso serviço de Gastroenterologia durante a pandemia. Métodos: Para o 1° objetivo, um inquérito online foi disponibilizado aos gastroenterologistas de todo o país. Para o 2° objetivo, os autores realizaram uma análise dos procedimentos endoscópicos do serviço de Gastroenterologia do Centro Hospitalar Vila Nova de Gaia/Espinho (CHVNG/E) entre 16 de Março e 8 de Maio durante os anos de 2019 e 2020. Resultados: 67 gastroenterologistas responderam o inquérito. Destes, 14.9% eram internos. 86.6% trabalharam num hospital com doentes com COVID-19, e 16.4% dos médicos prestavam cuidados a esses doentes. Todos os departamentos sofreram alterações. 90% dos internos referiram que a atividade tinha sido alterada. 94% dos médicos afirmaram que os procedimentos endoscópicos não-essenciais foram adiados. 85.1% mantinham algumas consultas médicas presenciais, 88.1% estavam a realizar consultas não presenciais, e 11.9% não tinham nenhum tipo de consulta. No nosso serviço, o número de procedimentos endoscópicos diminui em 73.1% de 2019 para 2020. Em 2020, a proporção de procedimentos endoscópicos de urgência foi superior em relação a 2019. Conclusão: A pandemia da COVID-19 conduziu a alterações importantes dentro das atividades da Gastroenterologia em Portugal, e os serviços de Gastroenterologia mostraram cumprir as recomendações. Além disso, os gastroenterologistas portugueses acreditam que a diminuição da atividade endoscópica dos internos pode comprometer a sua futura formação. O serviço de gastroenterologia do CHVNG/E mostrou uma redução significativa do número de procedimentos endoscópicos.

Abstract Background: Several gastroenterology societies have created recommendations in order to reduce nonessential exposure to the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus. Our aim is to evaluate the national gastroenterologists’ perspective on the impact of COVID-19 and the impact of reorganization of a gastroenterology department during the COVID-19 pandemic. Methods: For the first purpose, an online survey was distributed to gastroenterologists nationwide. For the second purpose, the authors conducted an analysis of some endoscopic procedures performed at the Gastroenterology Department of the Centro Hospitalar Vila Nova de Gaia/Espinho (CHVNG/E) between March 16 and May 8 during the years 2019 and 2020. Results: Sixty-seven gastroenterologists answered our survey. Only 14.9% were residents and 86.6% worked in a hospital with COVID-19 patients, with 16.4% assigned to assist those patients. All of the departments suffered modifications. Ninety percent of the residents affirmed that their activity had changed. Ninety-four percent declared having nonessential endoscopic procedures postponed, and 85.1% maintained in-person medical visits, 88.1% were already having remote consultations, and 11.9% did not have any clinical visit. In our gastroenterology unit, the number of endoscopic procedures decreased by 73.1% from 2019 to 2020. In 2020, the proportion of urgent procedures was higher compared to 2019. Conclusion: The advent of COVID-19 has led to important changes in gastroenterology activities in Portugal, and national gastroenterology units are complying with the recommendations. Furthermore, Portuguese gastroenterologists believed that the decrease in endoscopic activity can compromise residents’ education and training. The gastroenterology department at CHVNG/E has shown a significant reduction in the number of endoscopic procedures.

ABSTRACT INTRODUCTION: Fortification is the addition of nutrients to foods and has been used to address micronutrient deficiency concerns within populations. In Portugal, food fortification is voluntary. Studies that assess the impact of fortification on micronutrient intake and prevalence of inadequacy are still lacking. The first step required to carry out such an evaluation, is to have an insight into the supply of micronutrient-fortified foods. OBJECTIVES: Identify and characterize the national supply of micronutrient-fortified foods by analyzing the food labels' database, collected under the framework of the IAN-AF 2015-2016. METHODOLOGY: The database used herein integrates information about the foods consumed by IAN-AF 2015-2016 participants, together with information obtained from the labelled foods reported as consumed. The study encompassed the analysis of 5704 labels belonging to 12 IAN-AF 2015-2016 food groups. Micronutrient-fortified foods were identified by the presence of any micronutrients allowed to be added to foods in accordance with Regulation (EC) No. 1170/2009. RESULTS: A total of 911 micronutrient-fortified foods were identified. The largest proportion of micronutrient-fortified foods was found in the Cereals, cereal products and starchy tubers (43%) food group, followed by Sweets, cakes and biscuits (15%), Dairy (14%), Non-alcoholic beverages (11%), Milk and milk products substitutes (10%) and Fats and oils (7%). The number of micronutrients added to the products ranged from 1 to 17, with 37% of food products added with one or two micronutrients and the remaining 63% with at least three. Vitamins B2 (49%), B6 (45%), B3 (44%), B9 (43%), D (42%), B1 (37%) and B12 (35%) and the minerals iron (45%) and calcium (36,7%) are the most common micronutrients used in fortified foods. CONCLUSIONS: Approximately 16% of foods reported as consumed by IAN-AF 2015-2016 participants are micronutrient-fortified foods. This study is important to investigate further the contribution of micronutrient-fortified foods to usual micronutrient intake by the Portuguese population or specific subgroups, its adequacy, and potential risks of excessive intake.

RESUMO INTRODUÇÃO: A fortificação é o processo de adição de nutrientes aos alimentos e tem vindo a ser utilizada para combater a deficiência em micronutrientes em populações. Em Portugal, a fortificação de alimentos é voluntária, não existindo estudos que avaliem o seu impacto quer na ingestão, quer nas prevalências de inadequação de micronutrientes. Para isso, é necessário conhecer-se a oferta de alimentos fortificados em micronutrientes. OBJETIVOS: Identificar e caracterizar a oferta nacional de alimentos fortificados em micronutrientes através da análise de uma base de dados de rótulos alimentares recolhidos no âmbito do IAN-AF 2015-2016. METODOLOGIA: A base de dados utilizada integra informações sobre todos os alimentos que foram consumidos pelos participantes do IAN-AF 2015-2016, juntamente com informações obtidas através dos rótulos dos alimentos relatados como consumidos. Os alimentos fortificados em micronutrientes foram identificados pela presença de quaisquer micronutrientes que podem ser adicionados aos alimentos de acordo com o Regulamento (CE) n.º 1170/2009. O estudo compreendeu a análise de 5704 rótulos pertencentes a 12 grupos de alimentos do IAN-AF 2015-2016. RESULTADOS: Foram identificados 911 alimentos fortificados em micronutrientes. A maior proporção de alimentos fortificados em micronutrientes foi encontrada no grupo Cereais, derivados e tubérculos (43%), seguido por Bolachas, bolos e doces (15%), Produtos lácteos (14%), Bebidas não alcoólicas (11%), Substitutos de leite e produtos lácteos (10%) e Óleos e gorduras (7%). O número de micronutrientes adicionados variou de 1 a 17, com 37% dos produtos fortificados com um ou dois micronutrientes e os restantes 63% com, pelo menos, três. As vitaminas B2 (49%), B6 (45%), B3 (44%), B9 (43%), D (42%), B1 (37%) e B12 (35%) e os minerais ferro (45%) e cálcio (37%) são os micronutrientes mais usados para fortificar alimentos. CONCLUSÕES: Aproximadamente 16% dos alimentos reportados como consumidos pelos participantes do IAN-AF 2015-2016 são alimentos fortificados em micronutrientes. Este estudo é o primeiro passo para se investigar a contribuição do consumo de alimentos fortificados em micronutrientes para a ingestão habitual de micronutrientes pela população portuguesa ou subgrupos específicos, a sua adequação e os potenciais riscos de ingestão excessiva.

Resumen Los accidentes causados por escorpiones representan un problema mayor de salud pública tanto en términos de daños individuales como en la esfera de la salud pública. Los eventos mórbidos pueden ocurrir con diferentes manifestaciones y condiciones clínicas que deben ser diferenciadas para un manejo adecuado. Ade más del daño potencial que ocasionan a los humanos, el veneno del escorpión ha sido estudiado con el fin de analizar su potencia aplicación para le tratamiento de algunas enfermedades. Este articulo presenta una actualización sobre los aspectos clínicos, terapéuticos y profilácticos del escorpionismo.

Abstract Accidents caused by scorpions represent a major health problem on planet, both in individual terms and in the public health sphere. Morbid events can occur in different manifestations, and different clinical conditions must be differentiated for adequate treatment. In addition to the potential injury to human hosts, scorpion venom has been investigated for the therapy of several illnesses. Based on these aspects, the present article aims to present the clinical, diagnostic, therapeutic and prophylactic aspects of scorpionism.

ABSTRACT Introduction McKittrick-Wheelock syndrome is a rare condition that arises from a hypersecretory state secondary to large colorectal tumors, mainly villous adenomas, leading to an electrolytic disorder associated with chronic diarrhea that usually persists for years. It is a relatively unknown disease that can lead to severe complications such as acute kidney injury, severe hyponatremia, and hypokalemia. In fact, it causes death in most untreated cases. Surgical removal of the tumor is the most successful treatment, and symptoms tend to disappear after proper management. Case Report A 62-year-old man with a 2-year history of mucoid diarrhea preceded by abdominal pain presented with acute kidney injury, hyponatremia, and hypokalemia. A digital rectal examination and sigmoidoscopy were performed, and revealed a large laterally-spreading tumor in the rectum. Further investigation showed a rectal tubulovillous adenoma with secondary McKittrick-Wheelock syndrome. An anterior resection of the rectum with a colonic J-pouch and a diverting ileostomy were performed, and the patient improved with the resolution of the renal failure and electrolyte disturbances. The histopathological analysis revealed an invasive rectal adenocarcinoma. Discussion McKittrick-Wheelock syndrome is a condition with a low incidence that needs early intervention and proper diagnosis. It is of extreme importance that this disease is included in the differential diagnoses for chronic diarrhea associated with an electrolytic disorder.

RESUMO Antecedentes: Doenças desmielinizantes adquiridas levam a déficits visuais, piramidais, sensitivos, autonômicos e cerebelares que se sobrepõem e podem conduzir a grave incapacidade. O diagnóstico e o início de tratamento precoces são fundamentais para a prevenção de surtos e ocorrência de incapacidade. Objetivo: Neste artigo, apresentamos uma visão geral atualizada sobre o diagnóstico diferencial de doenças desmielinizantes adquiridas. Métodos: Realizamos uma revisão crítica sobre o diagnóstico das doenças desmielinizantes mais prevalentes: esclerose múltipla (EM), doença do espectro neuromielite óptica (NMOSD) e doença associada ao anticorpo contra a glicoproteína da mielina do oligodendrócito (MOGAD). Resultados: Discutimos a investigação, os critérios diagnósticos e os novos biomarcadores atualmente empregados para o diagnóstico dessas doenças, levando em conta as particularidades da população e sistema de saúde brasileiros. Conclusão: Uma análise minuciosa do histórico médico, exame neurológico e exames biomédicos e de imagem deve ser realizada para se fazer um diagnóstico diferencial de doença desmielinizante. Critérios diagnósticos devem ser empregados cautelosamente considerando-se particularidades étnicas e ambientais de cada paciente.

ABSTRACT Background: Acquired demyelinating disorders lead to overlapping visual, pyramidal, sensory, autonomic, and cerebellar deficits and may lead to severe disability. Early diagnosis and start of treatment are fundamental towards preventing further attacks and halting disability. Objective: In this paper we provide an updated overview of the differential diagnoses of acquired demyelinating disorders. Methods: We performed a critical targeted review of the diagnoses of the most prevalent demyelinating disorders: multiple sclerosis (MS), neuromyelitis optica spectrum disorders (NMOSD) and myelin oligodendrocyte glycoprotein antibody disease (MOGAD). Results: We discuss the workup, diagnostic criteria and new biomarkers currently being used for the diagnosis of these disease entities taking into account the particularities of the Brazilian population and healthcare system. Conclusion: A comprehensive analysis of medical history, physical examination, biomedical and imaging data should be performed to obtain differential diagnosis. Diagnostic criteria should be mindfully employed considering ethnic and environmental particularities of each patient.

ABSTRACT Introduction: Dengue epidemics impose a significant burden on health services, in addition to the economic loss caused by temporary leaves from work or deaths, which interferes in the economic performance of countries. Objective: To analyze the demographic, social and epidemiological profile of dengue cases in the population of Contagem-MG; and to evaluate climatic factors in order to provide tools for planning public health strategies. Methods: An ecological study was conducted from a time series between 2013 and 2017 in Contagem-MG municipality from Brazil. Results: The most affected groups were women, adults, mestizos, people with few years of schooling, and those living in peripheral regions. More than 99% of the people suffered from dengue without warning signs. Rain does not seem to be a determinant factor in the increase on the number of cases, since it rained every year. Conclusions: To better fight against dengue seasonal epidemics, the strategic planning approach is suggested, with the selection and definition of methodologies for planning an intersectoral strategy to solve recurrent events. This could increase the efficiency of public policies and, therefore, the possibility of successful public management in the reduction of inequities.

RESUMEN Introducción: Las epidemias de dengue imponen una carga importante a los servicios de salud, además de la pérdida económica ocasionada por bajas temporales del trabajo o por años de vidas perdidas como consecuencia de la muerte, lo que interfiere en el desempeño de las economías de los países. Objetivo: Analizar el perfil demográfico, social y epidemiológico de los casos de dengue en la población de Contagem-MG, además de evaluar los factores climáticos con el fin de brindar herramientas para la planificación de estrategias de salud pública. Método: Se realizó un estudio ecológico, a partir de una serie de tiempo, entre los años 2013 a 2017, en el municipio de Contagem-MG en Brasil. Resultados: Los grupos más afectados fueron las mujeres, adultos, mestizos, individuos con pocos años de estudio y los que vivían en regiones periféricas. Más del 99 % de las personas tenía dengue sin signos de alarma. Las lluvias no parecen haber sido determinantes para el aumento del número de casos, dado que hubo lluvias en todos los años. Conclusiones: Para mejorar la lucha contra las epidemias estacionales de dengue, se sugiere el enfoque de planificación estratégica, con la elección y definición de metodologías para planificar una estrategia intersectorial en la solución de situaciones recurrentes. Así se puede incrementar la eficiencia de las políticas públicas y, por ende, las posibilidades de una gestión pública exitosa en la reducción de inequidades.

Abstract Soil-cement bricks are earth construction materials that have been gaining prominence due to their environmental advantages. The Brazilian Technical Standard NBR 8491 establishes physical and mechanical procedures to characterize these bricks. However, the thermal characterization is an important assessment, since it is related to the building’s thermal performance. The objective of this study is to thermally characterize a panel of soil-cement bricks produced with mining tailings. For such, its absorptance (α) was evaluated using the ALTA II spectrometer, and its thermal transmittance (U) and thermal resistance (RT) through the hot box method (ISO 9869). The results enabled the calculation of the thermal capacity (CT), thermal lag (φ) and solar factor (FSO) of the panel, which were compared to the Brazilian Technical Standards NBR 15575-4 and NBR 15220 for Bioclimatic Zone 8. As a result, the soil-cement body panel showed values of α = 0.71, U = 2.47 Wm-2K-1, CT = 273.4 kJm-2K-1, φ = 6.02 h and FSO = 6.81%, meeting the requirements established in the first standard, but failing to attend the second. To comply with the Brazilians specifications, it is suggested to paint bricks with lighter colors focusing on decreasing its absorptance value. Thus, soil-cement bricks may be an environmental alternative to dispose of the waste generated during the mining process, contributing to sustainability.
