ABSTRACT Introduction: Head and neck neoplasms often affect fundamental functions, such as swallowing, speech, eating, and socializing. Evaluating their treatment should consider the physician’s opinion and the patient’s perspective. This difficulty in assessing the success of treatment led to the development of the FACE-Q Head and Neck Cancer Module, a questionnaire of patient-reported outcomes that measure the appearance, facial function, quality of life, and experience of care to head and neck neoplasms. The objective is to translation, cultural adaptation, and linguistic validation of the FACE-Q Head and Neck Cancer questionnaire for Brazilian Portuguese. Methods: The translation, cultural adaptation, and linguistic validation of the full questionnaire took place in four stages, using official guidelines from the World Health Organization and the International Society of Pharmacoeconomics and Outcomes Research. Results: A semantic, idiomatic, and conceptually equivalent Brazilian Portuguese version was achieved through a linguistically validated translation of the English FACE-Q Head and Neck Cancer module. Conclusion: The Brazilian Portuguese version presents a version equivalent to the original English instrument, which can be used as a critical patient-reported outcome assessment.

RESUMO Introdução: As neoplasias de cabeça e pescoço costumam afetar funções fundamentais, como engolir, falar, comer e se socializar. A avaliação do seu tratamento deve, portanto, levar em consideração a opinião do médico e a perspectiva do paciente. Essa dificuldade em avaliar o sucesso do tratamento levou ao desenvolvimento do FACE-Q - Módulo de Câncer de Cabeça e Pescoço, um questionário de resultados relatados pelo paciente que mede a aparência, função facial, qualidade de vida e experiência de cuidado para neoplasias de cabeça e pescoço. O objetivo é a tradução, adaptação cultural e validação linguística do questionário FACE- Q Câncer de Cabeça e Pescoço para o português brasileiro. Métodos: A tradução, adaptação cultural e validação linguística do questionário completo ocorreram em quatro etapas, usando as diretrizes oficiais da Organização Mundial da Saúde e da Sociedade Internacional de Farmacoeconomia e Pesquisa de Resultados. Resultados: Uma versão em português brasileiro semântica, idiomática e conceitualmente equivalente foi obtida por meio de uma tradução validada linguisticamente do módulo FACE-Q Head and Neck Cancer em inglês. Conclusão: A versão em português brasileiro apresenta uma versão com equivalente ao instrumento original em inglês, que pode ser utilizada como avaliação crítica de resultados relatados pelo paciente.

ABSTRACT Introduction: The Keystone flap is an island flap with reliable vascularization and simple dissection, first described in 2003. Despite its distinct advantages, there are few scientific publications on this matter, and it is not a common option in the clinical practice of reconstructive surgery. This article aims to report the experience of a cancer referral center with Keystone flaps in oncological reconstructions. Methods: A retrospective study was carried out data from medical records of patients who performed oncological plastic reconstruction with keystone flaps, operated by the Surgery team of the Cancer Institute of the State of São Paulo, in addition to the analysis of pre, intra and postoperative photographic records. Results: Nine patients were identified, all with comorbidities and a mean age of 52.7. Skin defects followed after oncological resections: five in the lower extremities, three in the trunk and one in the face. The mean of the skin resected area was 52.6cm2. The reconstructions were performed under shortened surgical time. There were no postoperative complications or flap losses. The average hospital stay was 2.2 days. Conclusion: The Keystone flap is technically simple and a reproducible option for covering wounds of different sizes and locations. Due to its reliability, simple and quick dissection, shortened hospital stay and low morbidity in the donor area, it should be considered for reconstructing cancer wounds from different locations in patients of all ages.

RESUMO Introdução: O retalho Keystone é um retalho em ilha, de vascularização confiável e dissecção simples, descrito pela primeira vez em 2003. Apesar de suas vantagens, é ainda pouco citado na literatura especializada e longe de se tornar opção de escolha na prática clínica da cirurgia reconstrutiva. O objetivo deste artigo é apresentar a experiência de um serviço oncológico de alta complexidade no uso de retalhos Keystone em reconstruções. Métodos: Um estudo retrospectivo foi desenvolvido por meio do levantamento de dados de prontuário de pacientes operados pela equipe de Cirurgia Plástica do Instituto do Câncer do Estado de São Paulo, além de análise de registros fotográficos pré, intra e pós-operatórios. Resultados: Nove pacientes foram identificados, todos portadores de comorbidades e média de idade de 52,7 anos. Os defeitos cutâneos se seguiram após ressecções oncológicas, sendo cinco em extremidades inferiores, três em tronco e um em face. A média da área ressecada foi de 52,6cm2. As reconstruções foram realizadas sob abreviado tempo cirúrgico. Não houve complicações pós- operatórias ou perdas do retalho e o tempo de hospitalização médio foi de 2,2 dias. Conclusão: O retalho Keystone é uma opção tecnicamente simples e reprodutível para a cobertura de ferimentos de tamanhos diversos e em localizações variadas. Devido à sua confiabilidade, dissecção simples e rápida, abreviado tempo de internação e baixa morbidade à área doadora, deve ser considerado na reconstrução de feridas oncológicas de diversas localizações, em pacientes de todas as idades.

ABSTRACT Introduction: Global sanitary crisis caused by the spread of COVID-19 induced many health services to stop performing non-urgent surgical procedures. In the scenario of plastic surgery, where most procedures are elective, socioeconomic consequences are estimated for these specialists. The objective of this study is to measure this impact. Methods: Effects of the pandemic within the clinical practice of Brazilian plastic surgeons were investigated through an online questionnaire addressed to members of the Sociedade Brasileira de Cirurgia Plástica. Results: A survey was applied to 645 surgeons. Most respondents reported operation restrictions on procedures and income reduction, especially in regions severely affected by the pandemic. Plastic surgeons with more than 10 years of experience were the most affected. High contamination rates, mental overload, decreased physical activity, and psychiatric medications have also been reported. Conclusion: COVID-19 pandemic brought changes to the personal and professional life of the Brazilian plastic surgeon. Due to the significant reduction in the workload, there were financial impacts on specialists from all country regions, besides physical and mental health issues. Adaptations were mandatory to maintain services and explore new areas of activity to supply the low demand for cosmetic surgery during the crisis.

RESUMO Introdução: Devido à crise sanitária mundial provocada pela disseminação da COVID-19, muitos serviços de saúde interromperam a realização de procedimentos cirúrgicos não urgentes. No cenário da Cirurgia Plástica, no qual a maioria das cirurgias são eletivas, estimam-se consequências socioeconômicas a estes especialistas. O objetivo deste estudo é dimensionar este impacto. Métodos: Os efeitos da pandemia dentro da prática clínica dos cirurgiões plásticos brasileiros foi investigada por meio de um questionário on-line, endereçado aos associados da Sociedade Brasileira de Cirurgia Plástica. Resultados: A pesquisa foi aplicada a 645 cirurgiões. A maioria dos entrevistados relatou restrições operacionais à realização de procedimentos e redução da renda, sobretudo nas regiões severamente afetadas pela pandemia. Cirurgiões plásticos com mais de 10 anos de formação foram os mais prejudicados. Elevada taxa de contaminação, sobrecarga mental, diminuição na prática de atividades físicas e uso de medicações psiquiátricas também foram relatados. Conclusão: A pandemia da COVID-19 trouxe mudanças no cenário pessoal e profissional do cirurgião plástico brasileiro. Devido à importante redução no volume de trabalho, houve impacto financeiro nos especialistas de todas as regiões do país, além de reflexos na saúde física e mental. Adaptações foram necessárias para manutenção dos atendimentos, além de exploração de novas áreas de atuação para suprir a baixa demanda de cirurgias estéticas durante a crise.

ABSTRACT Introduction: To present a reverse sural flap case report as an alternative to microsurgical flap in foot reconstruction after electrical trauma. The report presented is of a patient treated by the Burns and Complex Wounds groups of the Plastic Surgery Service of the Hospital das Clínicas of the Faculty of Medicine of the University of São Paulo in 2020. Case Report: Male patient, 35 years old, without comorbidities, victim of high voltage electrical trauma (1300V) at home, affecting upper and lower limbs bilaterally. Due to the severity of the injuries, he underwent left transtibial amputation and serial debridement in the contralateral limb, resulting in a defect in the medial face, hallux and entire dorsum of the right foot, with bone exposure. Due to the failure of reconstruction with a microsurgical flap of the vastus lateralis muscle, reconstruction with a reverse sural flap was chosen. It evolved with distal necrosis, requiring new debridement and re-advancement of the flap. In the follow-up, he presented a favorable evolution and is in the process of rehabilitation. Conclusion: The reverse sural flap proved to be suitable for the salvage treatment of extensive foot injuries after the failure of microsurgical therapy, offering stable coverage and excellent contour, thus allowing satisfactory patient rehabilitation.

RESUMO Introdução: Apresentar um relato de caso de confecção de retalho sural reverso como alternativa ao retalho microcirúrgico na reconstrução de pé após trauma elétrico. O relato apresentado é de um paciente atendido pelos grupos de Queimaduras e Feridas Complexas do Serviço de Cirurgia Plástica do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, no ano de 2020. Relato de Caso: Paciente masculino, 35 anos, sem comorbidades, vítima de trauma elétrico de alta voltagem (1300V) em domicílio, acometendo membros superiores e inferiores bilateralmente. Devido à gravidade das lesões, foi submetido a amputação transtibial à esquerda e desbridamentos seriados no membro contralateral, resultando em defeito em face medial, hálux e todo dorso do pé direito, com exposição óssea. Devido ao insucesso de reconstrução com retalho microcirúrgico de músculo vasto lateral, optou-se por reconstrução com retalho sural reverso. Evoluiu com necrose distal, sendo necessário novo desbridamento e reavanço do retalho. No seguimento, apresentou evolução favorável, e está em processo de reabilitação. Conclusão: O retalho sural reverso mostrou-se adequado para o tratamento de resgate de lesão extensa em pé após falha da terapia microcirúrgica, oferecendo cobertura estável e ótimo contorno, permitindo, assim, reabilitação satisfatória do paciente.

ABSTRACT Purpose: To analyze the cytotoxicity and cell in porcine-derived decellularized skin matrix. Methods: We analyzed the effect of multiple decellularization processes by histological analysis, DNA quantification, and flow cytometry. Subsequently, we analyzed the most appropriate hydrogel concentration to minimize cytotoxicity on fibroblast culture and to maximize cell proliferation. Results: After the fourth decellularization, the DNA quantification showed the lowest DNA concentration (< 50 ng/mg). Histological analysis showed no cell components in the hydrogel. Moreover, hematoxylin and eosin showed a heterogeneous structure of collagen fibers. The best hydrogel concentration ranged from 3 to 25%, and there was no significant difference between the 24 hours and seven days. Conclusions: The process of hydrogel production was effective for removing cells and DNA elements. The best hydrogel concentration ranged from 3 to 25%.

ABSTRACT Introduction: nose is the central point of the face and vulnerable to the occurence of non-melanoma skin cancer (NMSC), impacting on appearance. The paramedian forehead flap (PMFF) is considered the best option to treat extensive nasal defects. The objective of this study is to present the experience on PMFF for nasal reconstruction in the treatment of NMSC of a cancer referral center. Methods: retrospective study was carried out through data from medical records of patients who underwent nasal reconstruction with PMFF due to NMSC at the Cancer Institute of the State of São Paulo (ICESP). Results: 111 patients were identified, mostly ederly, with comorbidities and on initial tumors (T1 and T2). Basal cell carcinoma (BCC) was the predominant histological type. Dorsum and tip were the most affected subunitis. In addition to skin coverage, reconstruction of the lining and structural framework was also performed in half of the cases. Second intention healing was the technique of choice in closing the donor area. Pedicle division ocurred predominantly in the second operation and the median time to complete reconstruction was 6 months. There were low complication rates. Conclusions: the PMFF is safe and effective to treat nose NMSC, even in cases of high complexity. Since the treatment time can be prolonged and impact on quality of life, it is essential to emphasize and discuss this aspect with the patients before surgery.

RESUMO Introdução: o nariz é o ponto central da face e vulnerável à ocorrência de câncer de pele não-melanoma (CPNM), com impacto potencial na aparência. O retalho frontal paramediano (RFPM) é considerado a melhor opção para o tratamento de defeitos extensos no nariz. O objetivo deste estudo é apresentar a experiência de um centro oncológico de referência no uso de RFPM para reconstrução nasal no tratamento do CPNM. Métodos: estudo retrospectivo foi desenvolvido através do levantamento de dados de prontuário de pacientes submetidos à reconstrução nasal com RFPM devido à CPNM no Instituto do Câncer do Estado de São Paulo (ICESP). Resultados: 111 pacientes foram identificados, a maioria idosos, com comorbidades e portadores de tumores iniciais (T1 e T2). O carcinoma basocelular (CBC) foi o tipo histológico predominante. Dorso e ponta foram as subunidades mais acometidas. Além da cobertura cutânea, em metade dos casos foi realizada também a reconstrução do forro ou arcabouço do nariz. Cicatrização por segunda intenção foi a técnica de escolha no fechamento da área doadora. A liberação do pedículo foi realizada predominantemente no segundo tempo cirúrgico e o tempo para finalização do tratamento teve mediana de 6 meses. Houve baixa taxa de complicações. Conclusões: o RFPM é seguro e eficaz no tratamento do CPNM nasal, mesmo em casos de elevada complexidade. O tempo total de tratamento pode ser prolongado e impactar na qualidade de vida, sendo fundamental enfatizar e discutir este aspecto com o paciente antes da cirurgia.

ABSTRACT Purpose: To compare the effect of vein conduit filled with adipose tissue stem cells (ASC) on peripheral nerve injury regeneration. Methods: We analyzed 30 male Wistar rats surgically submitted to a 5-mm gap on the sciatic nerve. Then, the animals were divided into three groups: nerve autografting (AG, n=10), autogenous inverted glycerol-conserved vein (VG, n=10), and autogenous inverted glycerol-conserved vein + ASC (VASCG, n=10). The study endpoints were neuromotor functional analysis, gastrocnemius muscle weight, and sciatic nerve graft histomorphometry analysis. In the histologic analysis, we added a control group (naïve nerve). Results: Regarding functional analysis (Walking tract- score), the findings at week 3 showed a difference between the AG and the VG (-96.6 vs. -59.6, p=0.01, respectively) and between the VG and the inverted vein + VASCG (-59.9 vs. -88.92, p=0.02). At week 12, this study showed a difference between the AG and the VG (-64.8 vs. -47.3, p=0.004, respectively), and also a difference between the VG and the VASCG (-47.3 vs. -57.4, p=0.02, respectively). There was no difference in the histomorphometry analysis (nerve diameter, Schwann cells counting). The gastrocnemius muscles on the intervention side were more atrophic when compared to the gastrocnemius muscles on the control side. Conclusions: Our results suggested better functional recovery in the inverted vein group when compared to control group, and inverted vein + ASC group.

ABSTRACT Introduction: Reconstruction of distal wounds in lower extremities can be challenging due to the lack of tissue to perform local flaps. Fasciocutaneous and muscular flaps are some options for coverage, such as the reverse-flow fasciocutaneous sural flap. Objective: To present an 18-month experience on ankle, calcaneus, and foot reconstruction using the reverse-flow sural flap, performed by the Complex Wounds Group of the Plastic Surgery Department of the University of São Paulo Medical School. Methods: An observational, retrospective and descriptive study was performed through data survey on medical records of all patients treated between November 2018 and June 2020. Results: Nine reverse-flow fasciocutaneous sural flaps were performed. All patients were men. The mean age was 38 years old. Five patients had acute wounds for traffic collision, one electrical trauma and three chronic post-traumatic injuries. The ankle was the most common injury site (6), followed by foot (2) and calcaneus (1). Four patients had complications, three of which were partial necrosis and one distal epitheliosis. No case of total necrosis was recorded. The average hospital stay was 30.1 days. Conclusion: The reverse-flow fasciocutaneous sural flap proved to be a viable, reproducible, and reliable option for distal lower limb reconstruction. Level of Evidence IV, Case Series.

RESUMO Introdução: Reconstrução de lesões distais de extremidades inferiores podem ser desafiadoras devido à limitação de tecido para retalhos locais. Retalhos fasciocutâneos e musculares são opções, como o retalho fasciocutâneo sural reverso. Objetivo: Apresentar a experiência de 18 meses do Grupo de Feridas Complexas do Serviço de Cirurgia Plástica da Faculdade de Medicina da Universidade de São Paulo no uso do retalho sural reverso para reconstruções de defeitos em tornozelo, calcâneo e pé. Métodos: Estudo observacional, retrospectivo e descritivo, com dados de prontuário de todos os casos operados no serviço entre novembro de 2018 e junho de 2020. Resultados: Foram realizados nove retalhos fasciocutâneos sural reverso. Todos os pacientes eram do sexo masculino, com idade média de 38 anos. Cinco foram vítimas de acidente automobilístico, um de trauma elétrico e três apresentavam sequelas pós-traumáticas. Seis lesões localizadas em tornozelo, dois em pé e um em calcâneo. Quatro pacientes apresentaram complicações, sendo três necroses parciais e uma epiteliólise distal. Não houve perda total de retalho. O tempo médio de hospitalização foi de 30,1 dias. Conclusão: O retalho fasciocutâneo sural reverso mostrou-se uma opção viável, reprodutível e segura para reconstrução de lesões complexas em terço distal de perna e pé. ■ ABSTRACT Introduction: The COVID-19 pandemic was responsible for increasing social isolation rates, resulting in a reduction in traffic accidents. Trauma, in general, is among the most important public health problems worldwide. This study aims to evaluate the impact of social isolation in the city of São Paulo during the COVID-19 pandemic on the care of patients with detaching wounds. Methods: Observational, retrospective and descriptive study. All medical records of patients treated at the Emergency Room of the Hospital das Clínicas of the Faculty of Medicine of the University of São Paulo (HCFMUSP), by the Complex Wounds Group of the Plastic Surgery Service, admitted for detaching wounds and undergoing surgical procedures, were reviewed, from April to June of the years 2019 and 2020. Results: Twenty patients with detaching injuries on the lower limbs were included. In 2019, 14 patients were seen, with a mean age of 47 years, 7 of which were victims of traffic accidents, 5 of being run over, 1 of crushing and 1 of falling from standing height. In 2020, 6 patients were treated with detaching wounds during the pandemic, with a mean age of 36.16 years, 5 of which were victims of motorcycle accidents and 1 of being run over. No patient had COVID-19. Conclusion: During social isolation, there was a numerical reduction in care for detaching trauma during the quarantine period.

■ RESUMO Introdução: A pandemia da COVID-19 foi responsável pelo aumento dos índices de isolamento social, resultando na diminuição de acidentes de trânsito. Traumas em geral estão entre os problemas de saúde pública mais importantes em todo o mundo. O objetivo deste trabalho é avaliar o impacto do isolamento social na cidade de São Paulo durante a pandemia da COVID-19 nos atendimentos de pacientes com ferimentos descolantes. Métodos: Estudo observacional, retrospectivo e descritivo. Foram revisados todos os prontuários de pacientes atendidos no Pronto Socorro do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (HCFMUSP), pelo Grupo de Feridas Complexas do Serviço de Cirurgia Plástica, admitidos por ferimentos descolantes e submetidos a procedimento cirúrgico, no período de abril a junho dos anos de 2019 e 2020. Resultados: Foram incluídos 20 pacientes com ferimentos descolantes em membros inferiores. Em 2019, foram atendidos 14 pacientes, com idade média de 47 anos, sendo 7 vítimas de acidente de trânsito, 5 de atropelamento, 1 de esmagamento e 1 de queda da própria altura. Em 2020, durante a pandemia, foram atendidos 6 pacientes com ferimento descolante, com idade média de 36,16 anos, sendo que 5 foram vítimas de acidentes com motocicletas e 1 de atropelamento. Nenhum paciente apresentou COVID-19. Conclusão: Durante o isolamento social houve redução numérica nos atendimentos de traumas descolantes no período de quarentena.

■ ABSTRACT Introduction: The COVID-19 pandemic spread rapidly worldwide, bringing the need for emergency actions to control the disease at the collective and individual level. Thus, the Division of Plastic Surgery and Burns of the largest hospital complex in Latin America was physically aimed at the infected. In this context, we find the challenges of treating a large burned and polytraumatized patient, who presented arterial thrombosis in the lower limb, a rare complication associated with the thermal burn. All treatment was performed in a hospital not specialized in trauma care. Case Report: Case Report: Male patient, 18 years old, without comorbidities, with predominantly third-degree burns of 50% of body surface burned and severe neurotrauma. He presented arterial thrombosis in his right leg 24 hours after the burn. The patient underwent limb amputation and serial surgical interventions for debridement and skin grafting. Despite the prevention protocols, the patient was infected by COVID-19 during hospitalization. Conclusion: This case focuses on a rare complication related to burn injury, which does not yet have diagnostic criteria and defined prophylactic measures. Besides, the COVID-19 pandemic has impacted various health services areas, and it is essential to share knowledge during this pandemic by seeking adaptations to the crisis situation.

■ RESUMO Introdução: A pandemia de COVID-19 se espalhou rapidamente pelo mundo trazendo a necessidade de ações emergenciais para o controle da doença em nível coletivo e individual. Assim, a Divisão de Cirurgia Plástica e Queimaduras do maior complexo hospitalar da América Latina foi fisicamente destinada aos infectados. Neste contexto, encontramos os desafios de tratar um paciente grande queimado e politraumatizado, que apresentou trombose arterial em membro inferior, uma rara complicação associada à queimadura térmica. Todo tratamento foi realizado em hospital não especializado ao atendimento de trauma. Relato de Caso: Paciente masculino, 18 anos, sem comorbidades, com queimaduras predominantemente de 3º grau de 50% de superfície corpórea queimada e neurotrauma grave, que apresentou trombose arterial em perna direita 24 horas após a queimadura. Paciente submetido à amputação do membro e a seriadas intervenções cirúrgicas para desbridamento e enxertia de pele. Apesar dos protocolos de prevenção, paciente foi infectado pela COVID-19 durante a internação. Conclusão: Este caso enfoca uma complicação rara relacionada à lesão por queimadura, que ainda não possui critérios de diagnóstico e medidas profiláticas definidas. Além disso, a pandemia de COVID-19 trouxe impactos em diversos âmbitos nos serviços de saúde, sendo fundamental o compartilhamento de conhecimentos durante esta pandemia pela busca de adaptações à situação de crise.

Abstract This study presents the method used for chest reconstruction and treatment of mediastinitis following cardiac surgery at the Heart Institute of the University of São Paulo Medical School. After infection control with antibiotic therapy associated with aggressive surgical debridement and negative pressure wound therapy, chest reconstruction is performed using flaps. The advantages and disadvantages of negative pressure wound therapy are discussed, as well as options for flap-based chest reconstruction according to the characteristics of the patient and sternum. Further studies are needed to provide evidence to support the decisions when facing this great challenge.

ABSTRACT Introduction: 40% of patients undergoing radiotherapy after breast reconstruction by silicone prosthesis implant may develop prosthesis encapsulation. Several strategies have already been tested to prevent capsule contracture with unsatisfactory results. This study analyzed the effect of topical antileukotriene (AL) on capsular contracture formation in rats with silicone implants associated with irradiation. Methods: Silicone blocks were implanted in the dorsal region in 20 female rats Wistar with weights ranging from 200-250g. The animals were divided into two groups: control (injection of 0.9% saline solution into the tissue around the implant) and intervention group (injection of 10mg of AL into the tissue around the implant). Immediately after surgery, the animals were irradiated with a single dose of 10Gy. After two months, we collected capsule samples for histological analysis and gene expression analysis of the following biomarkers: iNOS, VEGF-a and MMP-9. Results: Vascular density was lower in the AL group when compared to the control group (55.4±30.0 vs. 81.8±26.7, p=0.05, respectively). Similarly, VEGF-a had the same behavior (control group - 0.34±0.1 vs. group Al - 0.02±0.001, p=0.04). Conclusion: This study suggested that treatment with AL decreases angiogenesis in animals submitted to silicone implants and underwent radiotherapy.

RESUMO Introdução: 40% dos pacientes submetidos à radioterapia após reconstrução de mama por implante de prótese de silicone podem desenvolver encapsulamento da prótese. Diversas estratégias já foram testadas para prevenir a contratura da cápsula com resultados insatisfatórios. Este estudo analisou o efeito do antileucotrieno (AL) tópico na formação de contratura capsular em ratos com implantes de silicone associados à irradiação. Métodos: Foram implantados blocos de silicone na região dorsal em 20 ratas fêmeas, espécie Wistar com peso variando de 200-250g. Os animais foram divididos em dois grupos: controle (injeção de solução fisiológica 0,9% no tecido ao redor do implante) e grupo intervenção (injeção de 10mg de AL no tecido ao redor do implante). Imediatamente após a cirurgia os animais foram irradiados com dose única de 10Gy. Após dois meses, coletamos amostras de cápsulas para análise histológica e análise da expressão gênica dos seguintes biomarcadores: iNOS, VEGF-a e MMP-9. Resultados: A densidade vascular foi menor no grupo AL quando comparado ao grupo controle (55,4±30,0 vs. 81,8±26,7, p=0,05, respectivamente). Da mesma forma, o VEGF-a teve o mesmo comportamento (grupo controle - 0,34±0,1 vs. grupo Al - 0,02±0,001, p=0,04). Conclusão: Este estudo sugeriu que o tratamento com AL diminui a angiogênese em animais submetidos a implantes de silicone e submetidos à radioterapia

ABSTRACT Introduction: Obesity is a chronic inflammatory disease associated with changes in inflammatory markers such as interleukins and CRP. This study evaluates the inflammatory response, through variations in interleukins and CRP, in patients undergoing abdominoplasty. Methods: Fourteen patients underwent abdominoplasty after weight loss achieved through bariatric surgery to maintain weight loss for at least18 months. Il4, IL6, IL10 and PCR levels were verified at times: preoperative, during surgery, 24 hours after surgery, 7th postoperative day and 14th postoperative day. Results: IL4 increased in the 24 hours postoperatively and continued on the rise until the 14th day. IL10 went up during surgery and began to fall in the 24 hours postoperatively to levels lower than the initial ones. IL6 began to rise during surgery, being more expressive in the 24 hours postoperatively, followed by a fall until the 14th day. CRP increased 24 hours postoperatively and remained discharged until the 14th day. Conclusion: Abdominoplasty reduced the chronic inflammatory systemic condition.

RESUMO Introdução: A obesidade é uma doença inflamatória crônica associada a alterações de marcadores inflamatórios como as interleucinas e PCR. O objetivo deste estudo é avaliar a resposta inflamatória, através das variações das interleucinas e PCR, em pacientes submetidas à abdominoplastia. Métodos: Quatorze pacientes foram submetidas à abdominoplastia após perda ponderal alcançada por meio da cirurgia bariátrica com manutenção da perda ponderal por, ao menos, 18 meses. Os níveis de IL4, IL6, IL10 e PCR foram verificados nos tempos: pré-operatório, durante a cirurgia, 24 horas após a cirurgia, 7o dia pós-operatório e 14o dia pós-operatório. Resultados: IL4 aumentou nas 24 horas de pós-operatório e seguiu em ascensão até o 14 o dia. IL10 subiu durante a cirurgia e começou a cair nas 24 horas de pós-operatório a níveis inferiores aos iniciais. IL6 começou a subir durante a cirurgia, mais expressivamente nas 24 horas de pós-operatório, seguida de queda até o 14o dia. A PCR aumentou nas 24 horas de pós-operatório e continuou alta até o 14o dia. Conclusão: A abdominoplastia promoveu uma amenização do quadro inflamatório sistêmico crônico.

ABSTRACT Introduction: Morbidly obese patients undergoing bariatric surgery after massive weight loss evolve with large skin folds in various body regions, including the abdomen. In patients with dermofat excesses throughout the abdominal circumference and ptosis of the gluteal region, circumferential abdominoplasty (simple or composite) has been an effective surgical solution conventional or “anchor” abdominoplasty brings unsatisfactory results in those patients with severe dysmorphia. The objective is to analyze the technical evolution of simple and composite circumferential abdominoplasty and its complications. Methods: Twenty-nine patients were evaluated, 28 females, with a mean age of 41.17 years, submitted to circumferential abdominoplasty between 2002 and 2012. This retrospective study, conducted through data collected from medical records, evaluated: surgery time, the weight of the resected surgical specimen, length of hospital stays, antibiotic therapy used, associated complications, and changes in the surgical technique in this period. Results: Composite circumferential abdominoplasty was performed in 23 patients (79.3%) and the simple one in six (20.7%). The mean surgical time was 346 minutes, and the surgical specimen’s mean weight was 4323 grams. Three patients (10.3%) had significant complications (symptomatic anemia and major suture dehiscence) and five (17.2%) minor complications (minor dehiscence, slight spontaneous bleeding, seroma, and hypertrophic scarring). Between 2002 and 2004, 75% of the complications occurred. The reoperation rate was 6.9%. Conclusion: There was a significant technical evolution in circumferential abdominoplasty performance, and the incidence of complications and the rate of reoperation were similar to those found in the literature.

RESUMO Introdução: Pacientes portadores de obesidade mórbida submetidos à cirurgia bariátrica, após perda ponderal maciça, evoluem com grandes dobras de pele em várias regiões do corpo, incluindo abdome. Nos pacientes com excessos dermogordurosos em toda circunferência abdominal e ptose da região glútea, a abdominoplastia circunferencial (simples ou composta) tem demonstrado ser uma solução cirúrgica eficaz, pois a abdominoplastia convencional ou “em âncora” traz resultados insatisfatórios naqueles pacientes com dismorfia severa. O objetivo é analisara evolução técnica da abdominoplastia circunferencial simples e composta e suas complicações. Métodos: Foram avaliados 29 pacientes, sendo 28 do sexo feminino, com média etária de 41,17 anos, submetidos à abdominoplastia circunferencial, entre 2002 e 2012. Este estudo retrospectivo, realizado através de dados colhidos dos prontuários médicos, avaliou: tempo de cirurgia, peso da peça cirúrgica ressecada, tempo de internação hospitalar, antibioticoterapia utilizada, complicações associadas e alterações ocorridas na técnica operatória neste período. Resultados: A abdominoplastia circunferencial composta foi realizada em 23 pacientes (79,3%) e a simples em seis (20,7%). O tempo cirúrgico médio foi de 346 minutos e o peso médio da peça operatória foi 4323 gramas. Três pacientes (10,3%) tiveram complicações maiores (anemia sintomática e deiscência de sutura maior) e cinco (17,2%) complicações menores (pequenas deiscências, pequeno sangramento espontâneo, seroma e cicatriz hipertrófica). Entre 2002 e 2004 ocorreram 75% das complicações. O índice de reoperação foi de 6,9%. Conclusão: Houve importante evolução técnica na realização da abdominoplastia circunferencial, sendo que a incidência de complicações e a taxa de reoperação foram similares àquelas encontradas na literatura.
