ABSTRACT Hemolytic uremic syndrome (HUS) triggered by influenza virus (iHUS) is rare. Influenza A infections have been described to trigger atypical HUS (aHUS) in individuals with an underlying genetic complement dysregulation. To date there are only few reports of Influenza B as a trigger of aHUS, all identified cases associated with mutations in the MCP or C3 gene, occasionally combined with other mutations. aHUS patients should be screened for all known disease-associated genes and screening should not be stopped after finding a mutation, to identify other genetic susceptibility factors influencing gene phenotype, particularly in patients with MCP or CFI mutations. Complement blockade using a monoclonal anti-C5 antibody, eculizumab, has greatly improved the outcome in recent years for certain groups of HUS. The decision on whether to treat or not with eculizumab should be made based on clinical and laboratorial evolution as well as molecular studies results. Influenza A and B are preventable through vaccination and strategies should be addressed for patients with complemente gene mutations identified.

Resumo Introdução: A isquemia mesentérica crónica como resultado de uma disfunção do fluxo sanguíneo nos órgãos viscerais é caracterizada por um curso clínico insidioso e, portanto, é frequentemente uma doença subestimada e subtratada. A história natural é progressiva, causada pelo desenvolvimento da aterosclerose numa população envelhecida e com múltiplas comorbilidades. Devido à extensa colateralidade, as lesões estenóticas difusas podem permanecer assintomáticas por muito tempo e manifestarem-se apenas quando ocorre um evento clínico ou cirúrgico agudo. Nestes casos, a suspeita clínica é fundamental para se chegar a um diagnóstico atempado de forma a preservar a qualidade de vida do doente e, principalmente, assegurar a sua sobrevida. Caso-clínico 1: Mulher de 48 anos de idade com antecedentes de tabagismo recorre ao serviço de urgência por dor abdominal hipogástrica, náuseas e vómitos. Foi realizado estudo por angio-tomografia computorizada (ATC) que demonstrou oclusão da artéria mesentérica superior, e estenose significativa do tronco celíaco e espessamento de ansas do intestino delgado sugestivo de sofrimento isquémico. Foi realizado uma trombectomia da artéria mesentérica superior (AMS) e stenting retrógrado do seu ostio seguido posteriormente de enterectomia extensa na laparotomia das 24 horas. Após alta hospitalar, a doente apresentou novo quadro abdominal tendo sido documentada a oclusão do stent na AMS. Foi submetida a um bypass ílio-hepático com bom resultado. Caso-clínico 2: Homem de 76 anos de idade previamente submetido a um bypass sequencial axilo-femoral e femoropopliteo direitos para tratamento de isquemia crónica com ameaça de membro. O doente recorreu ao serviço de urgência com dor abdominal, vómitos e obstipação. Foi realizado o diagnóstico de um quadro suboclusivo tendo realizado estudo por tomografia computorizada (TC) onde se observou aderência de ansas do íleon ao cego que se encontrava espessado; o tronco celíaco e AMS apresentavam estenoses suboclusivas. O doente foi submetido a um stenting primário do tronco celíaco com stent expansível em balão. Após a revascularização celíaca foi realizada uma colectomia direita com duas enterectomias segmentares. Conclusão: A isquemia mesentérica é uma patologia grave muitas vezes subdiagnosticada por falta de atenção para a doença por parte da generalidade dos médicos bem como pelos sintomas inespecíficos. A intervenção vascular visa impedir a necrose intestinal que pode mesmo levar à morte do doente. O diagnóstico atempado é assim fundamental e a revascularização deve ser realizada antes ou no mesmo tempo da cirurgia intestinal. A estreita colaboração entre as equipas de cirurgia geral e vascular é de essencial importância para o sucesso destes casos.

Abstract Introduction: Chronic Mesenteric Ischemia (CMI) resulting from an impaired blood flow of the splanchnic organs is characterized by an insidious clinical course and is often an underestimated and undertreated disease. The natural history is progressive, caused by atherosclerosis progression in a polymorbidity and aging society. Due to collateralization, diffuse stenotic lesions can remain asymptomatic for a long time and usually manifests when an acute medical or surgical event occurs. In those cases, the clinical suspicion is crucial to reach the diagnosis promptly, allowing to preserve the patients’ quality of life and, above all, the patients’ life. Clinical case 1: A 48-year-old woman with a history of smoking goes to the emergency department for hypogastric abdominal pain, nausea, and vomiting. A study was performed by computed tomography angiography (CTA) that demonstrated occlusion of the superior mesenteric artery (SMA) and significant stenosis of the celiac trunk associated with thickening of small bowel suggestive of ischemia. A thrombectomy of the superior mesenteric artery and retrograde stenting of the artery ostium was performed, followed by extensive enterectomy on 24-hour laparotomy. After hospital discharge, the patient had new abdominal complaints, and stent occlusion was documented in the SMA. She underwent an iliohepatic bypass with good results. Clinical case 2: A 76-year-old man previously submitted to a right axillofemoral and femoropopliteal sequential bypass to treat chronic limb-threatening ischemia (CLTI), went to the emergency department with complaints of abdominal pain, vomiting, and constipation. The diagnosis of a subocclusive intestinal syndrome was made, having performed a computed tomography (CT) scan where it was observed the Ileum adhered to the cecum, which was thickened; the celiac trunk and SMA had subocclusive stenosis. The patient underwent primary celiac trunk stenting with a balloon-expandable stent. After celiac artery revascularization, a right colectomy was performed with two segmental enterectomies. Conclusion: Mesenteric ischemia is a severe condition that is often underdiagnosed due to the lack of awareness of most physicians and nonspecific symptoms. The vascular intervention aims to prevent intestinal necrosis, which can lead to the patient’s death. Timely diagnosis is therefore essential, and revascularization must be performed before or at the same time as intestinal surgery. The close collaboration between the general and vascular surgery teams is essential to the success of these cases.

ABSTRACT Actinomycosis is an uncommon, endogenous, and chronic infection with varied and nonspecific clinical features such as abdominal, pelvic or cervical masses, ulcerative lesions, abscesses, draining fistula, fibrosis, and constitutional symptoms. The disease ensues when the bacteria disrupt the mucosal barrier, invade, and spread throughout interfascial planes. Currently, the diagnosis of actinomycosis is challenging because of its very low frequency and depending on the clinical presentation it may masquerade malignancies. Therapy consists initially in intravenous penicillin, followed by an oral regimen that may be extended until a year of treatment. A timely diagnosis is crucial to avoid extensive therapeutic attempt as surgery. However, a biopsy or drainage of abscesses and fistula’s tract may be required not only as a diagnostic procedure as part of the therapy. We report the case of a 72-year-old woman with an abdominal mass initially misdiagnosed as a liposarcoma. A second biopsy of a skin lesion of the abdominal wall made the diagnosis of actinomycosis, avoiding a major surgical procedure. The patient was treated with a long-term course of antibiotics with favorable outcome. Liposarcoma was ruled out after the patient’s full recovery with antibiotics and the misdiagnosis was credit to the overconfidence on the immunohistochemical positivity to MDM2.

RESUMO Objetivo: Este estudo examinou o estado de crescimento e o desenvolvimento físico de crianças brasileiras com transtornos do espectro autista entre 4 e 15 anos. Adicionalmente, examinamos se a variação nos padrões de crescimento e na massa corporal foi influenciada pelo uso de medicamentos psicotrópicos. Métodos: 120 crianças com idades entre 3,6 e 12,1 anos no início do estudo (média = 7,2 anos, DP = 2,3 anos) diagnosticadas com transtornos do espectro autista foram avaliadas em três ocasiões repetidas em um período de 4 anos. Foram considerados estatura, massa corporal e índice de massa corporal. O modelo multinível bayesiano foi utilizado para descrever os padrões de crescimento individual. Resultados: O crescimento em estatura foi comparável ao percentil 50 específico para a idade para os dados de referência do Centro de Controle e Prevenção de Doenças dos Estados Unidos até cerca de 8 anos. Porém, foi observada uma redução substancial na taxa de crescimento depois dos 8 anos, atingindo o percentil 5 específico para a idade aos 15 anos de idade. Tanto os valores de massa corporal quanto de índice de massa corporal foram, em média, maiores comparativamente ao percentil 95 específico para a idade até aos 8 anos da referência brasileira e do Centro de Controle e Prevenção de Doenças dos Estados Unidos, porém abaixo do percentil 50 específico para a idade aos 15 anos de idade. Conclusões: Os meninos brasileiros com transtornos do espectro autista entre 4 e 15 anos parecem ter retardo do crescimento na estatura após os 8-9 anos, provavelmente afeta o crescimento púbere. Foi observada uma alta prevalência de sobrepeso e obesidade na primeira infância, apesar de uma tendência de redução substancial na massa corporal e no índice de massa corporal ter sido aparente quando as crianças com transtornos do espectro autista entraram nos anos de desenvolvimento púbere.

ABSTRACT Objective: This study examined the growth status and physical development of Brazilian children with autism spectrum disorders from 4 to 15 years of age. Furthermore, it was examined whether variation in growth patterns and weight status was influenced by the use of psychotropic medications. Methods: One-hundred and twenty children aged 3.6-12.1 years at baseline (average = 7.2 years, SD = 2.3 years) diagnosed with autism spectrum disorders were measured on three repeated occasions across a 4-year period. Stature, body mass, and body mass index were considered. Bayesian multilevel modeling was used to describe the individual growth patterns. Results: Growth in stature was comparable to the age-specific 50th percentile for Centers for Disease Control and Prevention reference data until approximately 8 years, but a substantial decrease in growth rate was observed thereafter, reaching the age-specific 5th percentile at 15 years of age. Both body mass and body mass index values were, on average, higher than both the Brazilian and Centers for Disease Control and Prevention age-specific 95th percentile reference until 8 years, but below the 50th specific-age percentile at the age of 15 years. Conclusions: Brazilian boys with autism spectrum disorders between 4 and 15 years appear to have impaired growth in stature after 8-9 years of age, likely impacting pubertal growth. A high prevalence of overweight and obesity was observed in early childhood, although a trend of substantial decrease in body mass and body mass index was apparent when children with autism spectrum disorders entered the years of pubertal development.

Os autores descrevem o caso de uma menina de cinco anos observada em Dermatologia Pediátrica por crostas hemáticas e equimoses vulvares, associadas a prurido local. À observação constatou-se a existência de uma mancha acrómica vulvar e perianal, simétrica e bem definida, com atrofia cutâneo-mucosa associada a ausência de pequenos lábios e púrpura vascular na fúrcula e capuz clitoriano. A clínica e o exame objetivo permitiram fazer o diagnóstico do líquen escleroso vulvar. Os aspetos apresentados nesta entidade podem ser facilmente confundidos com estigmas de abuso sexual, pelo que importa dar a conhecer ou relembrar esta entidade a Pediatras e Médicos de Medicina Geral e Familiar.

The authors describe the case of a five-year-old girl, observed in Paediatric Dermatology Clinic due to a blood crust and vulvar ecchymosis associated with local pruritus. The observation revealed of a symmetrical and well defined white plaque around the vagina and anus, with mucocutaneous atrophy associated with the absence of labia minora and vascular purpura on the wishbone and clitorial hood. These findings were suggestive of vulvar lichen sclerosus. The aspects present in this entity can be easily confused with sexual abuse, reason why it is important to alert the Paediatricians and General Practice Doctors to the existence and the diagnostic features of this entity.

Adolescente de 12 anos de idade com diagnóstico pré-natal de agenesia renal esquerda, foi internada com uma história de quatro meses de evolução de dor abdominal recorrente e dismenorreia após a menarca. A investigação imagiológica revelou uma malformação uterina associada a defeitos de desenvolvimento do canal de Muller - útero duplicado com hemi-utero esquerdo obstruído e com conteúdo hemático (hematometra), um colo uterino único e uma vagina normal. Pretendemos alertar para a associação entre agenesia renal unilateral e malforma ções ginecológicas, a maioria uterinas, mas também vaginais. A sua detecção precoce por métodos imagiológicos permite antecipar medidas terapêuticas para alívio sintomático, sobretudo dor abdominal ou pélvica recorrente, bem como evitar as complicações associadas (endometriose, aderências pélvicas ou infecções).

A 12-year-old female with a prenatal diagnosis of left kidney agenesis was hospitalized with a history of four months of recurrent abdominal pain and dysmenorrhea after menarche. Imaging investigation revealed a gynecological malformation associated with Müllerian developmental defects - duplicated uterus with an obstructed left hemi-uterus filled with hematic content (haematometra), one cervix and a normal vagina. We pretend to alert pediatric physicians about the association of renal agenesis and gynecological malformations, most frequently uterine, but also vaginal. Early detection of these malformations by imaging will anticipate therapeutic measures to relieve symptoms, mostly recurrent abdominal or pelvic pain, and to avoid complications such as endometriosis, pelvic adherences and infections.

Introdução: A obesidade é um dos problemas de saúde mais graves, a nível mundial. A procura de soluções para a designada epidemia do século XXI contínua, sendo que a relação entre o consumo de bebidas açucaradas e o peso corporal tornou-se uma questão pertinente. Objetivos: Avaliar o impacto de uma estratégia de consciencialização sobre o teor de açúcares simples presentes num conjunto de bebidas disponibilizadas na cafetaria de um estabelecimento de ensino. Metodologia: Foram analisadas as vendas de um conjunto de bebidas antes e após a intervenção. A intervenção baseou-se na afixação de um expositor com as bebidas e a representação da quantidade de açúcares simples, em açúcar, sob o mote “O açúcar que comes quando bebes”. Procedeu-se a uma análise das proporções das vendas, com um intervalo de confiança a 95%. Resultados: Após a análise estatística dos rácios foi observada uma redução estatisticamente significativa no número de unidades vendidas, após a intervenção, de refrigerantes do tipo cola e das bebidas gaseificadas. Além disso, houve um aumento no número de unidades vendidas, com significância estatística, de néctares de frutas, chá/infusões e leite de chocolate. Conclusões: A intervenção na cafetaria da Escola de Hotelaria e Turismo de Coimbra foi bem sucedida tendo-se verificado uma redução efetiva das vendas de bebidas açucaradas.

Introduction: Obesity is one of the more serious health problems worldwide. Finding solutions to the designated epidemic of the twenty-first century persists, and the relationship between the consumption of sweetened beverages and body weight became a relevant issue. Objectives: To evaluate the impact of an awareness strategy about the content of simple sugars present on a range of drinks available in the cafeteria. Methodology: The sales of a range of beverages were analyzed, before and after intervention. The intervention was based on the exposure of a framework with drinks and the representation of the amount of simple sugars, in sugar, under the motto “The sugar that you eat when you drink”. An analysis of proportion of sales, with a confidence interval of 95% was performed. Results: After statistical analysis of the ratios was observed a statistically significant reduction in the number of units sold of soft drinks, coca-cola and carbonated drinks. In addition, there was an increase in the number of units sold, with statistical significance, of fruit nectars, tea / infusions and chocolate milk. Conclusions: The intervention in the cafeteria of the Escola de Hotelaria e Turismo de Coimbra was successful with an effective reduction in sales of sweetened beverages.
