In 2005, odontogenic cyst was classified as keratocyst odontogenic tumor due to being aggressive and recurrent. The keratocyst odontogenic tumor has characteristics, with slow development, does not cause metastases and provides great bone destruction. The aim of this study was to discuss the aspects regarding the diagnosis, prognosis and treatment of odontogenic keratocyst tumor, through the report of two cases. Both initially underwent decompression of the lesion present proximity of anatomical structures to be great and noble, aiming to prevent pathological fractures. We carried out the clinical-radiographic and after regression of the lesion, patients underwent enucleation total.

El año 2005 quiste odontogénico fue clasificado como un tumor queratoquiste odontogénico (TQO) debido a su agresividad y recurrencia. El TQO tiene las características del tumor: crecimiento lento, no causa metástasis y proporciona una gran destrucción ósea. El objetivo de este estudio fue examinar los aspectos relacionados con el diagnóstico, pronóstico y tratamiento del TQO mediante la presentación de dos casos. Ambos casos fueron inicialmente sometidos a descompresión debido al gran tamaño de la lesión y la proximidad de ésta con estructuras anatómicas importantes, con el objetivo de prevenir las fracturas patológicas. Se llevó a cabo un seguimiento clínico-radiográfico y después de tener una regresión de la lesión postdescompresión, los pacientes fueron sometidos la enucleación total.

A síndrome de Prader-Willi (SPW) é uma doença neurocomportamental genética que afeta o desenvolvimento da criança, resultando em obesidade, estatura reduzida, hipotonia, distúrbios endócrinos e déficit cognitivo que podem comprometer a integridade da cavidade oral. O presente estudo tem como finalidade apresentar um caso de paciente branco, masculino, 15 anos de idade portador da referida síndrome cujo exame clínico intra-oral evidenciou presença de placa bacteriana, gengivite, má-oclusão, salivação viscosa e múltiplas lesões ulceradas em lábio, mucosa jugal, gengiva inserida, dorso e ventre lingual e lesões papulares ulceradas em borda lateral da língua. Após realização de biópsia excisional, foi constatada a presença de lesão herpética em cavidade oral e lesões cutâneas típicas do herpes que foram associadas a possível causa de encefalite herpética. Assim, observou-se que grande parcela dos efeitos deletérios da SPW podem ser amenizados com o diagnóstico correto e intervenções terapêuticas e educacionais precoces, sendo importante a atuação de uma equipe multiprofissional integrada e o desenvolvimento de protocolos assistenciais para melhor manejo dos pacientes portadores da síndrome de Prader-Willi.

Prader-Willi syndrome is a genetic neurobehavioral disease affecting children's development and resulting in obesity, reduced height, hypotonia, endocrine disorders and cognitive deficits, which may impair oral integrity. This study aims to report on a case involving a white male 15-year-old patient with Prader-Willi syndrome whose oral examination revealed bacterial plaque, gingivitis, poor occlusion, viscous salivation and multiple lip, jugal mucosa, inserted gum and tongue ulcerations. An excision biopsy revealed oral ulcerations typical of herpes, which were considered to be likely to correlate with herpes encephalitis. This result demonstrates that a large portion of the deleterious effects of Prader-Willi syndrome can be attenuated by appropriate diagnosis and early therapeutic intervention, highlighting the role of an integrated multidisciplinary team in the development of therapeutic protocols for Prader-Willi syndrome patients.

Calvaria grafts provide good bone quantity for the reconstruction of the atrophic maxilla, and have lower morbidity and resorption rates when compared to iliac crest. The aim of this paper is to present the technique for obtaining a graft of the skull. Initially, the depth of the osteotomy is determined by a manually conducted bur, which establishes the limits of the structures of the skull (outer table, diploe and inner table), making the removal of bone blocks easier and safer. Thus, osteotomies of the blocks are made with greater security, avoiding the complications inherent to surgical technique. The case that will be presented it is from a male patient of 65 years who refused to submit to the iliac crest graft, opting for the calvaria, despite being bald, that is a contraindication for this treatment modality. A delicate suture associated with placement of titanium mesh to maintain the conformation of the patient's skull in the region of the bone defect, created after removal of the graft, provided a good cosmetic result at the donor site. The use of titanium mesh for re-anatomization of bone defects created in the grafts is well indicated for bald patients.

Los injertos de calvaria proporcionan una buena cantidad de hueso para la reconstrucción del maxilar atrófico, y tienen menores tasas de morbilidad y reabsorción en comparación con la cresta ilíaca. El objetivo de este trabajo es presentar la técnica para la obtención de un injerto del cráneo. Inicialmente, la profundidad de la osteotomía se determina mediante una fresa manualmente, lo que establece los límites de las estructuras del cráneo (tabla externa, diploe y tabla interna), lo que hace la eliminación de bloques óseos más fácil y segura. Por lo tanto, las osteotomías de los bloques se hacen con mayor seguridad, evitando las complicaciones inherentes a la técnica quirúrgica. Se presenta el caso de un paciente hombre de 65 años que se negó a ser sometido a injerto de cresta ilíaca, optando por la bóveda craneal, a pesar de ser calvo, que es una contraindicación para esta modalidad de tratamiento. Una sutura cuidadosa asociada con la colocación de malla de titanio para mantener la conformación del cráneo del paciente en la región del defecto óseo creado después de obtención del injerto, deja un buen resultado cosmético en el sitio donante. El uso de malla de titanio para la re-anatomización de defectos óseos creados en los injertos está bien indicada para pacientes calvos.

The occurrence of diseases and variations from normality is very common in the maxillofacial complex, and this phenomenon is closely associated to peculiar genetic determinants and influenced by genetic isolation. Studies on the distribution of oral diseases and variations from normality in South American natives are generally directed mainly to the study of dental caries in the children and periodontal diseases in the adults, disregarding a wide range of other oral diseases. The aim of this study was to evaluate the distribution of lesions and anatomical variations from normality in eight ethnic groups living in the Umutina Indian Reservation, State of Mato Grosso, Brazil, through physical and oral examinations besides cytological evaluation and assessment of diet and habits, such as tobacco and alcohol consumption. A total of 291 Indians, of both genders, with ages ranging from 1 to 96 years, were examined, being identified 132 variations from normality, evidencing a high occurrence of ankyloglossia, which was present in 108 cases (37.1%), followed by benign migratory glossitis in 5 cases (1.7%); mandibular torus and candidiasis in 3 cases (1.0%). No malignant lesion was identified in the population.

La ocurrencia de enfermedades y variaciones de normalidad son muy comunes en el complejo maxilofacial. Este fenómeno se asocia estrechamente a determinantes genéticos peculiares y son influenciados a través del aislamiento genético. Estudios sobre la distribución de enfermedad oral y variaciones de la normalidad en nativos Sudamericanos son generalmente dirigidos al estudio de la caries dental en niños y enfermedad periodontal en adultos, dejando de lado una amplia gama de otras enfermedades orales. El objetivo de este estudio fue evaluar la distribución de lesiones y las variaciones anatómicas de normalidad en ocho grupos étnicos indígenas pertenecientes a la reserva Umutina, Estado de Mato Grosso, Brasil, a través del examen físico y oral, además de la evaluación citológica y valoración de dieta y hábitos como tabaco y consumo de alcohol. Un total de 291 indígenas de ambos sexos, con edades que van de 1 a 96 años fueron examinados, identificándose 132 variaciones de normalidad, evidenciando una alta ocurrencia de anquiloglosia, la cual se presentó en 108 casos (37,1%), seguido por glositis migratoria benigna en 5 casos (1,7%); torus mandibular y candidiasis en 3 casos (1,0%). Ninguna lesión maligna se identificó en la población.

A doença de von Willebrand (DvW) é uma coagulopatia hereditária. Não há predileção por sexo. Clinicamente caracteriza-se por hemorragias mucocutâneas, sobretudo nasais, menorragias e hemorragias pós-trauma. Este artigo relata um caso clínico de DvW em paciente de 52 anos de idade, leucoderma, do sexo masculino, que apresentou extensa hemorragia em bordo lateral de língua após ferimento lacerante, além de descrever alguns aspectos clínicos, patológicos e terapêuticos da DvW. Após repetidas tentativas de sutura do ferimento e reposição dos fatores de coagulação, optou-se pela ligadura da artéria carótida externa ipsilateral ao ferimento, com resolução favorável do caso, notando-se bom aspecto cicatricial 2 meses após a ligadura. Este caso reforça que é de extrema importância a realização de anamnese criteriosa, buscando-se identificar possíveis distúrbios hemorrágicos prévios ou antecedentes familiares.

The von Willebrand disease (vWD) is a hereditary coagulopathy. There is no gender predilection. Clinically characterized by mucocutaneous bleeding, especially nose bleeding, menorrhagia and bleeding after trauma. This article reports a case of a 52-year-old Caucasian male patient with vWD, who presented with extensive bleeding in the tongue after a lacerating injury caused by accidental biting, and describes some clinical, pathological and treatment aspects of vWD. After repeated attempts to suture the wound and replace clotting factors, a decision was made to perform the ligature of the external carotid artery ipsilateral to the injury. There was favorable resolution of the case, with a good aspect of the scar 2 months after ligation. This case reinforces that it is extremely important to make a thorough review of medical history of all patients, searching for possible bleeding disorders or previous family history.

OBJECTIVES: This study evaluated the resistance to antimicrobials of aerobes and facultative anaerobes isolated from patients wearing complete dentures, patients with gingivitis and periodontitis, and periodontally health subjects. MATERIAL AND METHODS: Three hundred and four isolates were tested. The minimal inhibitory concentrations of the drugs were evaluated through the agar dilution method using Mueller-Hinton agar. RESULTS: The most active antimicrobial drugs were the carbapenems (meropenem and imipenem), and resistance to these drugs was restrict to 1.6-2.3% of the isolates, as well as ciprofloxacin and rifampin. Microbial resistance to ampicillin, amoxicillin/clavulanic acid, cefoxitin, cephalothin, amikacin, chloramphenicol and nalidixic acid was particularly high. In most cases, the resistance to β-lactams was mediated by the production of hydrolytic enzymes, especially in gram-negative enteric rods, while enterococci did not evidence production of these enzymes. The association amoxicillin/clavulanic acid was not effective in 28.3% of the tested isolates. CONCLUSIONS: The results of this investigation confirmed that the oral cavity of patients with periodontitis and gingivitis, and particularly edentulous patients wearing complete dentures could harbor microorganisms with several antimicrobial resistance markers, and these microorganisms are frequently implicated in multiresistant, systemic, oral or nosocomial infections.

Chronic osteomyelitis of maxilla and mandible is rare in industrialized countries and its occurrence in developing countries is associated with trauma and surgery, and its microbial etiology has not been studied thoroughly. The aim of this investigation was to evaluate the microbiota associated with osteomyelitis of mandible or maxilla from some Brazilian patients. After clinical and radiographic evaluation, samples of bone sequestra, purulent secretion, and biopsies of granulomatous tissues from twenty-two patients with chronic osteomyelitis of mandible and maxilla were cultivated and submitted for pathogen detection by using a PCR method. Each patient harbored a single lesion. Bacterial isolation was performed on fastidious anaerobe agar supplemented with hemin, menadione and horse blood for anaerobes; and on tryptic soy agar supplemented with yeast extract and horse blood for facultative bacteria and aerobes. Plates were incubated in anaerobiosis and aerobiosis, at 37ºC for 14 and 3 days, respectively. Bacteria were cultivated from twelve patient samples; and genera Actinomyces, Fusobacterium, Parvimonas, and Staphylococcus were the most frequent. By PCR, bacterial DNA was detected from sixteen patient samples. The results suggest that cases of chronic osteomyelitis of the jaws are usually mixed anaerobic infections, reinforcing the concept that osteomyelitis of the jaws are mainly related to microorganisms from the oral environment, and periapical and periodontal infections may act as predisposing factors.

osteomyelitis of the mandible and maxilla are common in developing countries and their treatment may be long-standing and difficult. Thus, the aim of this study was to discuss the main biological aspects of the chronic osteomyelitis of the jaws of especial interest for dental team. These infections are associated with a complex microbiota composed mainly by anaerobic bacteria, sometimes associated with microorganisms originated from the skin and digestive tract. These data suggest that chronic osteomyelitis of the mandible and maxilla should be treated as anaerobic infections in most cases. In addition, local surgical treatments are relevant in the therapy outcome, associated to the use of antimicrobial agents, and the failure to accomplish them is a major cause of treatment failure.

La osteomielitis maxilar y mandibular son comunes en países en desarrollo y su tratamiento puede ser difícil y de larga duración. Así, el objetivo del presente estudio fue discutir los principales aspectos biológicos de especial interés odontológico de la osteomielitis crónica del maxilar. Estas infecciones están asociadas a una microbiota compleja compuesta principalmente de bacterias anaerobias, algunas veces asociadas a microorganismos oriundos de la piel y del tracto digestivo. Estos datos sugieren que la osteomielitis crónica maxilar y mandibular deben ser tratadas como infecciones anaerobias en la mayoría de los casos. Además, tratamientos quirúrgicos locales son relevantes en el éxito de la terapia, asociados al uso de agentes anti-microbianos, e cuyos resultados no satisfactorios representan una de las principales causas de un tratamiento fallo o no adecuado.
