Abstract The prevalence of openly trans and non-binary people has been increasing, and, in today's world, it is estimated that they represent 0.39% and 1.19% of the global population, respectively. Thus, it becomes increasingly imperative that primary health care physicians are prepared to receive and guide this population, in their general and specific health needs. In this sense, this work intends to study the knowledge of health professionals and their approach to trans and non-binary people in the primary care facilities associated with Centro Hospitalar Universitário do Porto, via an online questionnaire that was sent to the population under study. It was found that, although there is significant contact between residents and specialists in general and family medicine with trans and non-binary people in clinical practice, the perceived degree of knowledge and training about the specific needs of this population seems to be extremely small. It is, therefore, crucial to invest in pre-graduate training, as well as post-graduate training.

Resumo A prevalência de pessoas assumidamente trans e não binárias tem vindo a aumentar, sendo que no mundo hodierno estima-se representarem 0,39% e 1,19% da população mundial, respetivamente. Deste modo, torna-se cada vez mais imperativo que os cuidados de saúde primários estejam preparados para receber e orientar esta população nas suas necessidades de saúde gerais e específicas. Neste sentido, este trabalho pretende estudar o conhecimento dos profissionais de saúde e a sua abordagem aos utentes trans e não binários nas Unidades de Saúde Familiar adstritas ao Centro Hospitalar Universitário do Porto, tendo sido enviado um questionário online à população em estudo. Constatou-se que, embora exista um significativo contacto entre médicos internos e especialistas de medicina geral e familiar com pessoas trans e não binárias na prática clínica, o grau percecionado de conhecimentos e de formação acerca das necessidades específicas desta população parece ser extremamente diminuto. É, assim, fulcral o investimento na formação pré-graduada, bem como na formação pós-graduada.

Abstract Polyorchidism is a rare condition. Its diagnosis is mostly incidental and its management is still a matter of debate. Herein we report a case of triorchidism and its management. A 6-year-old boy presented with asymptomatic left scrotal swelling. Scrotal ultrasound confirmed hydrocele and described a supernumerary testis on the left side, along with bilateral testicular microlithiasis. Intraoperative examination of the scrotal content confirmed triorchidism: two testicular masses sharing the same epididymis and vas deferens. Preservation of both units was decided and considering the free movement between them and the inherent risk of torsion, a fixation suture was accomplished. The postoperative recovery was uneventful. According to the scarce reports on literature, since testicular drainage into a shared vas deferens was confirmed, no resection was pursued. Considering the increased risk of malignancy, an annual ultrasound follow-up was recommended.

Resumo A poliorquidia é uma condição rara. O seu diagnóstico é sobretudo incidental e a sua orientação clínica é ainda tema de debate. Neste trabalho, os autores reportam um caso de triorquidismo e a sua abordagem clínica. Um rapaz de 6 anos apresentava uma tumefacção escrotal assintomática à esquerda. A ecografia escrotal confirmou a presença de hidrocelo e descreveu um testículo supranumerário à esquerda, bem como microlitíase testicular bilateral. A exploração intra-operatória do conteúdo escrotal confirmou triorquidismo: duas unidades testiculares que partilhavam o mesmo epidídimo e canal deferente. Optou-se pela preservação de ambas as unidades e, considerando o movimento livre entre elas e o risco inerente de torção, foi realizada uma sutura de fixação. O período pós-operatorio não teve intercorrências. De acordo com a escassa literatura disponível, uma vez que se confirmou uma drenagem testicular conjunta para o mesmo canal deferente, não se realizou exérese de nenhum testículo. No entanto, considerando o risco aumentado de malignidade, recomendou-se seguimento anual através de ecografia.

Abstract: This research aims to provide updated information on caries experience and associated risk factors in children 6-12 years old. A cross-sectional and descriptive study design was carried out with a non-probabilistic, convenient sample of 209 children male and female. Clinical examinations were performed by calibrated dental students following WHO detection criteria. Caries indices dmft and DMFT were calculated. Caries Risk Assessment data was collected using an adapted CAMBRA instrument; following the International Caries Care guidelines. Descriptive statistics were performed to analyze the results and Chi-square test, Contingency Coefficient (C) and Corrected Typified Residues were calculated to determine the association between variables. 58% of the total population had dental caries lesions in its more severe stages (cavitation) and 42% were apparently healthy (AHS) without any cavitated lesions. The mean dmft index was 1.34 ± 1.93, and the mean DMFT index was 0.63 ± 1.22. Lesion severity remained between 1-2 teeth affected on both dentitions. A statistically significant association (p = 0.035) between the health condition and toothbrushing was stablished with a degree of dependence of C = 0.144. A positive standardized residual of 2.1 was evident for schoolchildren that experience caries lesion that never brush their teeth and AHS that brushed their teeth more than once. No association (p = 0.081) was found between health condition and intake of sugary snacks and beverages. A severe dental caries experience with a statistically significant association between the health condition and toothbrushing with fluoridated toothpaste 1450 ppm > 1 a day and a positive correlation in schoolchildren that experience caries lesion that never brush their teeth.

Abstract Introduction Much of the evidence on the relationship between stress, lifestyle, and other physical and mental health outcomes comes from studies conducted in high-income countries. There is therefore a need for research among populations in low and middle-income settings. Objectives To measure stress levels and identify factors associated with a high stress level and its consequences for health. Methods This was a population-based cross-sectional study carried out in 2016 with adults aged 18 years or older in a municipality in southern Brazil. A two-stage sampling strategy based on census tracts was used. Stress levels were measured with the Perceived Stress Scale (PSS-14) and classified into quartiles. The impact of the highest stress levelon each outcome was assessed with etiologic fractions (EF). Results The most stressed groups were: females (PR = 1.51, 95%CI 1.25-1.81), younger people (PR = 1.76, 95%CI 1.26-2.46), middle-aged individuals (PR = 1.60, 95%CI 1.17-2.19), those with lower schooling (PR = 1.56, 95%CI 1.20-2.02), the physically inactive (PR = 1.51, 95%CI 1.20-1.91), people who spent three or more hours watching television per day (PR = 1.29, 95%CI 1.12-1.50), and those with food insecurity (PR = 1.44, 95%CI 1.19-175). Possible consequences of high stress level were regular or poor self-perception of health (EF = 29.6%), poor or very poor sleep quality (EF = 17.3%), lower quality of life (EF = 45.6%), sadness (EF = 24.2%), and depressive symptoms (EF = 35.8%). Conclusions Stress plays an important role in several domains of health. Both public policies that target reduction of inequalities and specific stress-management interventions can reduce stress levels in populations, thereby decreasing the burden of other negative physical and mental health outcomes related to stress.

ABSTRACT Chronic kidney disease is characterized by immune dysfunction that increases predisposition to infections, virus-associated cancers and impaired response to vaccination. The altered immune response is caused by impairment of both innate and adaptive immune systems, as well as other factors that are hallmarks of renal disease, such as uremia, malnutrition, chronic inflammation, mineral bone disease and anemia. The aim of this article is to review the causes and mechanisms that lead to immune dysfunction in patients with chronic kidney disease.

Resumo Introdução embora tenhamos avançado nos estudos sobre o consumo de crack, é importante o conhecimento das características dos usuários que buscam tratamento nas diferentes modalidades assistenciais para transtornos relacionados ao uso de substâncias para planejar a operacionalização desses dispositivos. Objetivo analisar a prevalência e os fatores associados ao uso de crack em pacientes em tratamento de modalidade clínico-ambulatorial. Método estudo transversal, com análise dos prontuários dos pacientes do período de 1999 a 2015 de um ambulatório de dependência química no Sul do Brasil. Para análise dos dados foi realizado o teste exato de Fisher e regressão de Poisson. Resultados foram analisados os prontuários de 1.253 pacientes, dos quais 1.196 (95,5%) continham informações sobre o uso de crack. O uso de crack foi relatado por 47% (IC 95% [44, 50]) dos pacientes. O grupo de risco consistiu em adultos de 20 a 39 anos, sem renda, com três ou mais filhos, que não consumiam álcool ou usavam maconha, que tinham acompanhamento familiar contínuo, que buscavam espontaneamente o serviço, que já haviam sido internados em hospital ou feito acompanhamento em comunidade terapêutica ou centro psicossocial. Conclusão existe uma grande demanda no atendimento ambulatorial de pacientes que usam crack. É imprescindível que os fatores de risco orientem o planejamento do tratamento.

Abstract Background Despite the advance in studies addressing the use of crack cocaine, knowledge about the characteristics of users that seek treatment in the different modalities of care for substance use disorders is important to plan the operationalization of these services. Objective To analyze the prevalence and factors associated with the use of crack cocaine in outpatients. Method Cross-sectional study consisting in the analysis of the medical records of outpatients of a chemical dependency clinic located in the south of Brazil from 1999 to 2015. The Fisher’s exact test and the Poisson regression model were used to analyze the data. Results Medical records from 1,253 patients were analyzed, and 1,196 (95.5%) of them contained information on the use of crack cocaine. Use of this substance was reported by 47% (95% CI [44, 50]) of the outpatients. The risk group was composed of adults aged 20-39 years, with no income, who had three or more children, did not consume alcohol or marijuana, had continuous family assistance, spontaneously looked for the service, and had already been hospitalized or assisted at a therapeutic community or psychosocial center. Conclusion There is great demand for the outpatient care of crack cocaine users. It is crucial that the risk factors guide treatment planning.

Abstract This article reports a supervised internship in Psychology and Public Health. In this work, we aim to report the experience that happened with a focus on the promotion of living and dialogical spaces between teaching/services/community focused on the production of knowledge and practices to strengthen community bonds, development of support networks and promotion of mental health. Participatory methodologies were recommended in conducting the processes for surveying the health needs of the territory, planning actions, executing and evaluating based on onto epistemological assumptions and analytical categories of community psychology. We saw that in the territory community relations are crossed by the psychosocial effects of inequality and by the fear affection that weaken the weave of life in common and, in this context, we point out the importance of facilitating the improvement of community relations with a view to overcoming social inequities in health. We perceive that the valorization of history, memory and local language, as well as the ethics-aesthetics of art are potent elements in facilitating the process of producing care for oneself and the other.

Resumen O presente artigo descreve uma experiência registrada durante um estágio profissional em Psicologia e Saúde Coletiva. Neste trabalho, objetivamos relatar a experiência acontecida com foco na promoção de espaços vivenciais e dialógicos entre ensino/serviços/comunidade voltados para a produção de saberes e práticas de fortalecimento dos vínculos comunitários, desenvolvimento de redes de apoio e à promoção da saúde mental. Foram preconizadas metodologias participativas na condução dos processos de levantamento das necessidades de saúde do território, planejamento das ações, execução e avaliação com base nos pressupostos ontoepistemológicos e categorias analíticas da psicologia comunitária. Vimos que no território as relações comunitárias são atravessadas pelos efeitos psicossociais da desigualdade e pelo medo que fragilizam o tecer da vida em comum e, nesse contexto, assinalamos a importância de facilitar o aprimoramento das relações comunitárias com vistas à superação das iniquidades sociais em saúde. Percebemos que a valorização da história, memória e linguagem local, bem como, a ética-estética da arte são elementos potentes na facilitação do processo de produção de cuidado de si e do outro.

Resumo: Introdução: A infecção por Yersinia enterocolitica é uma zoonose que se pode apresentar como uma gastroenterite auto-limitada ou assumir formas mais graves. A sua propensão em afectar o íleo terminal e, consequentemente, os gânglios linfáticos regionais explica o potencial confundimento diagnóstico com a apendicite aguda. Métodos: Foram revistos todos os casos reportados de infecção por Y. enterocolitica em população pediátrica, nos últimos 12 anos. Resultados: Foram identificados 11 casos de infecção por Y. enterocolitica no período selecionado. Em 4 destes pacientes foi suspeitado inicialmente um diagnóstico cirúrgico: 1 invaginação intestinal e 3 apendicites agudas. Os doentes que apresentaram caraterísticas típicas de apendicite aguda eram mais velhos, enquanto os pacientes de menor idade mais comumente se apresentaram com diarreia e febre. Nos doentes com suspeita de apendicite, a ecografia e a tomografia computorizada abdominal foram realizadas em todos os doentes e excluíram o diagnóstico em 2/3 casos. O único paciente submetido a cirurgia tinha relato de alterações radiológicas típicas de apendicite aguda e apresentava um factor de risco importante para a infecção por Y. enterocolitica que não foi inicialmente reconhecido. Conclusão: O diagnóstico de Yersinose deve ser suspeitado em doentes com sintomas gastrointestinais que apresentem fatores de risco (sobrecarga de ferro) e o contexto epidemiológico adequado. A suspeição precoce deste diagnóstico neste contexto pode evitar intervenções cirúrgicas desnecessárias.

Abstract: Introduction: Yersinia enterocolitica infection is a zoonotic disease that varies from self-limited gastroenteritis to more severe forms. Its propensity to affect the terminal ileum and to spread to regional lymph nodes explains the potential misdiagnosis with appendicitis. Methods: We reviewed the Y. enterocolitica infection cases in a pediatric population for the last 12 years. Results: There were 11 cases of Y. enterocolitica infection in the selected period. Four patients had a suspected surgical diagnosis: 1 intussusception, 3 acute appendicitis. Patients who presented with appendicitis-like features were older, whereas younger children most commonly presented with diarrhea and fever. Ultrasound and abdominal computed tomography (CT) were performed in appendicitis-like patients and ruled out appendicitis in 2 of the 3 cases. The only patient submitted to surgery had abnormal CT findings and an important risk factor for this infection that was not recognized at presentation. Conclusion: This condition should be considered in patients with known risk factors (such as iron overload) and with the right epidemiological setting when presenting with abdominal symptoms. The suspicion of this diagnosis in these particular cases might obviate unnecessary surgical interventions.

Resumo Uma das causas mais comuns da glomerulonefrite rapidamente progressiva (GNRP) é a glomerulonefrite crescêntica (GNC) pauci-imune. Na maioria dos casos, a patologia apresenta um marcador sorológico positivo, o anticorpo anticitoplasma de neutrófilos (ANCA), mas em cerca de 10% dos pacientes não há ANCAs circulantes, perfazendo um subgrupo da patologia conhecido como GNC pauci-imune ANCA-negativa. A GNRP pode estar associada a doenças sistêmicas, mas são poucos os relatos de caso que descrevem sua associação com doença mista do tecido conjuntivo (DMTC). O presente artigo relata um caso de GNC ANCA-negativa associada a DMTC.

Abstract One of the most common causes of rapidly progressive glomerulonephritis (RPGN) is pauci-immune crescentic glomerulonephritis (CrGN). In the majority of cases, this condition has a positive serologic marker, the anti-neutrophil cytoplasmic antibodies (ANCAs), but in approximately 10% there are no circulating ANCAs, and this subgroup has been known as the ANCA-negative pauci-immune CrGN. RPGN can be associated with systemic diseases, but there are only few case reports describing the association with mixed connective tissue disease (MCTD). The authors report a case of ANCA-negative CrGN associated with a MCTD.

Objetivo: Rever a evidência sobre a eficácia da suplementação oral de magnésio na prevenção de cãibras musculares, sem patologia evidente associada, em indivíduos adultos. Fontes de dados: National Guideline Clearinghouse, Canadian Medical Association Practice Guidelines Infobase, Guidelines Finder, The Cochrane Library, DARE, Bandolier, Evidence-Based Medicine Online e PubMed. Métodos: Pesquisa realizada no dia 4 de julho de 2018 com os termos MeSH muscle cramp, magnesium e magnesium compounds de meta-análises, revisões sistemáticas, ensaios clínicos aleatorizados e controlados (ECAC), estudos de coorte e caso-controlo e normas de orientação clínica publicados desde 1 de janeiro de 2008 nas línguas portuguesa, inglesa e espanhola. Para atribuir níveis de evidência (NE) e forças de recomendação foi utilizada a Strength of Recommendation Taxonomy (SORT), da American Academy of Family Physicians. Resultados: Dos 47 artigos encontrados, cinco cumpriram os critérios de inclusão: uma meta-análise (NE 2), três revisões sistemáticas (NE 2) e um ECAC (NE 2). Não foram identificados estudos relativos a cãibras associadas ao exercício. Os estudos são consensuais na aparente ausência de eficácia do magnésio na prevenção de cãibras idiopáticas. Quanto às cãibras associadas à gravidez, a evidência é inconsistente. Contudo, o uso de amostras pequenas, períodos de follow-up curtos e a heterogeneidade das metodologias (população, tipo de suplemento, posologia, outcomes) comprometem a extrapolação e reduzem a força das conclusões obtidas. Conclusão: Perante a evidência encontrada, a suplementação oral de magnésio na população geral parece não ser eficaz na prevenção de cãibras idiopáticas (SORT B), não havendo evidência que suporte a sua prescrição por rotina na prática clínica. Nas cãibras associadas à gravidez a evidência da suplementação oral de magnésio não é clara (SORT B). Para obter conclusões mais robustas são necessários ECAC de melhor qualidade e com metodologias mais homogéneas.

Aim: Evidence review on the efficacy of oral magnesium supplementation in the prevention of muscle cramps without associated pathology, in adults. Data sources: National Guideline Clearinghouse, Canadian Medical Association Practice Guidelines Infobase, Guidelines Finder, The Cochrane Library, DARE, Bandolier, Evidence-Based Medicine Online, and PubMed. Methods: On 4th July 2018, using the MeSH terms ‘Muscle Cramp’, ‘Magnesium’ and ‘Magnesium Compounds’, we searched for meta-analyses, systematic reviews, randomized controlled trials (RCT), observational studies and clinical guidelines published in Portuguese, Spanish, and English. To assign levels of evidence (LE) and strength of recommendations, the Strength of Recommendation Taxonomy (SORT) scale of the American Academy of Family Physicians was used. Results: We found 47 articles, of which five met the inclusion criteria: one meta-analysis (LE 2), three systematic reviews (LE 2) and one RCT (LE 2). No studies related to exercise-associated cramps were found. Studies are consensual in the apparent lack of efficacy of the magnesium in the prevention of idiopathic muscle cramps. As for the pregnancy-associated cramps, the evidence is inconsistent. However, the use of small samples, short follow-up periods and the heterogeneity of methodologies (population, supplement type, posology, and outcomes) compromise the extrapolation and reduce the strength of the conclusions obtained. Conclusion: According to the evidence found, oral magnesium supplementation in the general population seems to be ineffective in preventing idiopathic muscle cramps (SORT B), therefore, with no evidence to support its routine prescription in clinical practice. In pregnancy-associated cramps, the evidence of oral magnesium supplementation is unclear (SORT B). To achieve more robust conclusions, higher quality RCT is needed, with more homogeneous methodologies.

RESUMO Objetivo: analisar informações e características sobre a linguagem de crianças com diagnóstico para altas habilidades e superdotação. Métodos: revisão integrativa da literatura realizada a partir da busca de dados em bases eletrônicas do Portal de Pesquisas da Biblioteca Virtual em Saúde, LILACS, MEDLINE, Google Scholar, utilizando combinação das palavras chaves “Fonoaudiologia e altas habilidades”, “superdotação e linguagem infantil”. Resultados: foram analisados 45 artigos que preencheram os critérios de inclusão, dentre os quais 29 foram excluídos e 16 selecionados para a revisão. A análise destes artigos permitiu verificar, ainda que superficialmente, os aspectos presentes na linguagem oral e escrita de crianças com altas habilidades e superdotação. O vocabulário e discurso destas crianças geralmente é rico, com excelente habilidade de compreensão. Há um interesse precoce pela leitura, além de notável habilidade de decodificação e compreensão da escrita. Os estudos indicam que, estas capacidades se apresentam de uma maneira atípica, quando comparadas a população tida como normal, cujo discurso tende a ser compatível com crianças 2 anos mais velhas e o desenvolvimento da linguagem ocorrendo de maneira mais acelerada. Conclusão: a linguagem das crianças diagnosticadas com altas habilidades e superdotação possui particularidades que devem ser melhor estudadas, pois o seu desenvolvimento se difere das demais crianças.

ABSTRACT Purpose: to analyze information and characteristics of the language of children diagnosed with high abilities and giftedness. Methods: an integrative literature review based on a search in LILACS, MEDLINE, and Google Scholar databases, combining the keywords “Speech-Language-Hearing and high abilities”, “giftedness and child language”. Results: a total of 45 articles that met the inclusion criteria were analyzed, of which 29 were excluded and 16 selected for the review. The analysis of these articles made it possible to verify, though superficially, the aspects in the oral and written language of children with high abilities and giftedness. These children’s vocabulary and discourse are usually rich, with excellent comprehension skills. There is early interest in reading, besides noteworthy writing comprehension and decoding skills. The studies indicate that these capacities are atypical when compared with those of the population referred to as normal. The discourse of gifted children tends to be compatible with that of children two years older, and their language develops faster. Conclusion: the language of children diagnosed with high abilities and giftedness has particularities that must be better studied, as their development differs from that of other children.

Contrast agents are widely used in ambulatory and hospitalized patients, as a complement to imaging studies, improving diagnostic accuracy. Patients with chronic kidney disease are at increased risk for adverse events related to contrast administration. In this review, we will summarize the current evidence on this topic.

RESUMO A doença renal ateroembólica (DRAE) é uma manifestação renal da aterosclerose enquanto patologia sistêmica. A DRAE é definida como uma disfunção renal secundária à embolização de cristais de colesterol seguida da oclusão da vascularização renal. O presente relato descreve o caso de um paciente com vários fatores de risco, porém sem um evento precipitante, que se apresentou com um curso clínico bastante atípico de doença ateroembólica grave de evolução espontânea e silenciosa.

ABSTRACT Atheroembolic renal disease (AERD) is a kidney manifestation of atherosclerosis as a systemic disease. AERD is defined as a renal impairment secondary to embolization of cholesterol crystals with consequent occlusion of renal vascularization. The current case report describes one patient with multiple risk factors but without any inciting event history who presents a very atypical clinical course of a severe and massive atheroembolic disease that developed spontaneously and silently.
