Resumo O presente trabalho tem por escopo a análise do novo modelo de trabalho em época de Covid-19. O presente estudo aponta as alterações sofridas nas relações de trabalho, bem como observa, de modo mais acurado, quais as formas que estão sendo projetadas ou utilizadas para o melhor aproveitamento dos trabalhadores. Com isso, se observará que o teletrabalho tem se apresentado como fator importante para a manutenção do emprego e renda, bem como para manter a economia ativa e não estagnada. Para tanto, foi feita uma pesquisa bibliográfica e documental, como forma de compreender os impactos do Covid-19 nas relações humanas e, especialmente nas relações laborais trabalho. Por fim será observado a necessidade de adaptação de toda sociedade para uma nova realidade empregatícia.

Abstract The present work aims to analyze the new model of work at the time of Covid-19. The present study points out the changes suffered in labor relations, as well as observing, in a more accurate way, which forms are being designed or used for the better use of workers. With that, it will be observed that teleworking has presented itself as an important factor for the maintenance of employment and income, as well as to keep the economy active and not stagnant. To this end, a bibliographic and documentary research was carried out, as a way to understand the impacts of Covid-19 on human relations and, especially on labor relations. Finally, the need to adapt the entire society to a new employment reality will be observed.

Resumo O Herbário Fitopatológico Verlande Duarte Silveira (UFRJ) é considerado um dos dez mais antigos do Brasil, tendo iniciado sua coleção em 1916 na antiga Escola Nacional de Agronomia (ENA), hoje Universidade Federal Rural do Rio de Janeiro (UFRRJ). O seu acervo é de inestimável valor histórico e científico por abrigar coleções de Arsène Puttemans, Heitor Vinícius da Silveira Grillo, André Maublanc e Eugenio Rangel além de Verlande Duarte Silveira. Ciente da sua importância para a comunidade científica e da necessidade de tomar ações que promovam a sua recuperação e funcionalidade, atualmente, o herbário UFRJ vem passando por melhorias como a criação de um banco de dados (BD) para catalogação das informações contidas nas exsicatas, substituição dos envelopes deteriorados e, organização física do seu acervo, assim como a adoção de procedimentos padronizados. Destacamos a criação do BD no Programa Microsoft® Office Access®, pois facilita a busca pelas exsicatas contidas no acervo e demonstramos o resultado com a seleção de registros contendo vários gêneros de importância fitopatológica, dentre os quais, o gênero Colletotrichum. Todas estas ações visam otimizar seu uso e tornar acessível, principalmente para a comunidade científica, toda a informação contida neste acervo.

Abstract The “Verlande Duarte Silveira” Phytopathological Herbarium (UFRJ) is considered one of the ten oldest in Brazil, having started its collection in 1916 in the former National School of Agronomy (ENA), now Federal Rural University of Rio de Janeiro (UFRRJ). Its collection is of inestimable historical and scientific value because it houses collections of Arsène Puttemans, Heitor Vinícius da Silveira Grillo, André Maublanc, Eugenio Rangel, and Verlande Duarte Silveira. Aware of its importance to the scientific community and the needs of taking action to promote its recovering and restore its functionality, the UFRJ Herbarium has been undergoing improvements such as the implementation of databases to store all information contained in the exsiccates, the replacement of damaged envelopes and the physical organization of its collection, as well as the adoption of standardized procedures. We highlighted the creation of the databases using Microsoft® Office Access® Program, because it makes the search for the exsiccates contained in the collection feasible, which is demonstrated with the selection of records containing several genera of phytopathological importance, including the genus Colletotrichum. All these actions aim to optimize its use and facilitate the access to information.

Este artigo tem como objetivo avaliar a ideia de “sistema deliberativo”, considerada aqui como parte do esforço da teoria deliberativa em lidar com um conjunto de problemas relativos à coordenação de múltiplas formas de comunicação, à escala e à inclusão política. Além de contextualizar o surgimento da proposta sistêmica no interior da teoria deliberativa (1) e apresentar suas diferentes versões (2), este artigo identifica alguns limites da ideia chamando atenção particularmente para o problema das conexões entre suas partes (3). Feito isso, o artigo oferece três possibilidades de conexão: comunidades e coalizões políticas, representação de atores e temas e desenho institucional (4) buscando analisá-los do ponto de vista tanto das potencialidades quanto dos limites apresentados.

This article evaluates the idea of “deliberative system”, which is considered an attempt of the deliberative theory to offer some theoretical and practical answers to different constraints related to the deliberative practices in a complex world, characterized by cultural and ethnical pluralisms, and by social inequality. Solutions such as the pluralization of arenas and patterns of communication are introduced by the idea of the deliberative system. However, new limits have arisen that created new analytical challenges. This article responds one of them through three steps: first, the article contextualizes and analyzes the motivations that created the idea of the deliberative system. Second, the article discusses some limits of this idea with a special attention to the possibilities of connections between the arenas and the possibility of scaling up the deliberative practices. Third, the article analyses three connectors that may link different arenas and patterns of communication to deal with the analyzed limits.

Cet article vise à évaluer l’idée du « système délibératif », considéré ici comme faisant partie de l’effort de la théorie délibérative à aborder un ensemble de problèmes relatifs à la coordination de multiples formes de communication, de l’échelle et de l’inclusion politique. L’article contextualise non seulement l’émergence de proposition systémique au sein de la théorie délibérative (1) et présente ses différentes versions (2), mais il identifie certaines limites de l’idée en portant l’attention du lecteur sur le problème des connexions entre les parties (3). Ceci dit, l’article offre trois possibilités de connexion : communautés et coalitions politiques, représentation d’acteurs et des thèmes et la conception institutionnelle (4), tout en cherchant à les analyser du point de vue du potentiel et des limites présentées.

Abstract Diabetic peripheral neuropathy (DPN) is a common complication of diabetes mellitus when glycemic levels are poorly controlled. Sometimes DPN is accompanied by vasculopathy (DPV), which can worsen the clinical prognosis. The aim of this study was to analyze the gait parameters of nondiabetic individuals and diabetic individuals with DPN with or without DPV. METHOD The study included 68 individuals (50 to 65 years old) divided into three groups: people without diabetes mellitus (n = 33), diabetic patients with DPN (n = 18), and diabetic patients with both DPN and DVP (n = 17). The participants underwent a gait evaluation using electronic baropodometry to obtain the single and double support, velocity, and pressure-time integral. RESULTS The pressure-time integral, velocity, and single support variables were lower, and the double support and double support/single support ratio were higher in the diabetic neuropathy and vasculopathy group. The velocity was lower the greater the degree of impairment of the diabetic foot. Some correlations were identified with velocity. CONCLUSION In diabetic individuals, there was a significant worsening of the gait parameters analyzed according to increasing degree of clinical impairment.

Aim: To evaluate of the effect of two different occlusal splints on police officers with TMD. Methods: Thirty police officers were selected based on Research Diagnostic Criteria for TMD and on clinical exams. Volunteers (ten per group) were distributed according to occlusal splints: group A - Control, group B - Michigan Occlusal Splint (MOS), and group C - Planas Appliance (PA). Experimental groups were analyzed using a visual analog pain scale (VAPS), subject to a clinical evaluation of temporomandibular joint. Bilateral surface electromyographic activities of anterior and posterior temporal, masseter and suprahyoid muscles were analyzed at rest and during clenching, before and after four weeks using the occlusal splints. Results: The left and right temporal and masseter muscles sensitivity decreased after using both splints. Pain symptoms increased for group A (Control) and decreased for group C. Conclusions: Planas Appliance was more efficient on pain reduction than the Michigan Occlusal Splint.

A teoria democrática contemporânea abriga pelo menos duas abordagens que vinculam a presença de espaços participativos à qualidade da democracia. Ambas, a abordagem participativa e a deliberativa, apostam na centralidade destes espaços para conferir legitimidade às decisões políticas que vinculam uma coletividade. Ao apostarem na centralidade da participação e da discussão para a legitimidade do processo decisório, tais abordagens tornam-se ferramentas analíticas úteis para analisarmos mais uma inovação democrática no Brasil contemporâneo: as Conferências de Políticas Públicas. O objetivo deste artigo é avaliar a estrutura institucional destas conferências para aferir como estes espaços integram diferentes tipos de ação e se eles conseguem formar um sistema integrado de participação e deliberação, entre os diferentes níveis da federação.

Contemporary democratic theory includes at least two approaches that link the presence of participatory spaces to the quality of democracy. Both the deliberative and participatory approaches consider these spaces important to the legitimacy of policy decisions that bond a collectivity. By investing in the centrality of participation and discussion to the legitimacy of decision making, such approaches become useful tools to analyze yet another democratic innovation of contemporary Brazil: Public Policy Conferences. The aim of this paper is to evaluate the institutional structure of those Conferences, in order to see how they combine different types of action and whether or not they form an integrated system of participation and decision between the different levels of the federation.

A teoria deliberativa passa por mudanças significativas em seus pressupostos fundamentais. Para acomodar conceitos como autointeresse, poder e conflito e outras formas de comunicação, tais como barganha, essa teoria revisa atualmente os pressupostos que a orientava. Nesse movimento, ela se aproxima de outras abordagens clássicas da teoria democrática, como o pluralismo e o neocorporativismo. Este artigo tem um duplo objetivo: (1) analisar tais mudanças e (2) aferir as potencialidades e os limites das mesmas para a teoria em tela.

A deliberative theory has undergone significant changes in their fundamental assumptions. To accommodate concepts such as self-interest, conflict and power and other forms of communication as a bargain, this theory has revised the initial assumptions that oriented it. In this move, it becomes more similar to other traditional approaches to democratic theory, such as pluralism and neo-corporatism. This article has two aims: (1) to analyze such changes; and (2) to evaluate the potential and limits of such changes to deliberative theory.

INTRODUÇÃO: Pacientes com síndrome de Down (SD) geralmente apresentam TSH elevado (de origem hipotalâmica), uma desordem chamada de disfunção neurossecretora de TSH (TSH-nd). Nesses casos, há uma leve elevação do TSH (5-15 µUI/mL), com T4 livre normal e anticorpos antitireoide (AB) negativos. OBJETIVO: Reconhecer o risco de progressão para a tireoidite de Hashimoto (HT). SUJEITOS E MÉTODOS: Analisamos retrospectivamente 40 pacientes com SD (idade média = 4,5 anos), acompanhados por 6,8 anos. RESULTADOS: A HT foi diagnosticada em 9/40 pacientes, três logo no início da avaliação e seis durante a evolução. Em 31/40 dos pacientes, o diagnóstico de TSH-nd permaneceu estável durante os anos, com valores máximos de TSH variando de 5 a 15 µUI/mL. Neste grupo, o T4 livre também permaneceu normal e os AB foram negativos. Houve uma redução significativa do TSH (p = 0,017), e concentrações normais de TSH (< 5,0 µUI/mL) foram observadas em 29/31 pacientes, em pelo menos um momento. Nenhum paciente apresentou TSH > 15 µUI/mL. CONCLUSÃO: Pacientes com SD e TSH-nd apresentam baixo risco de progressão para a HT (10% para o sexo feminino e 6% para o sexo masculino).

INTRODUCTION: Patients with Down syndrome (DS) often have elevated TSH (hypothalamic origin), which is called TSH neurosecretory dysfunction (TSH-nd). In these cases, there is slight elevation in TSH (5-15 µUI/mL), with normal free T4 and negative thyroid antibodies (AB). OBJECTIVE: To recognize the risk of progression to Hashimoto's thyroiditis (HT). SUBJECTS AND METHODS: We retrospectively analyzed 40 DS patients (mean age = 4.5 years), followed up for 6.8 years. RESULTS: HT was diagnosed in 9/40 patients, three early in monitoring, and six during evolution. In 31/40 patients, TSH-nd diagnosis remained unchanged over the years, with maximum TSH values ranging from 5 to 15 µUI/mL. In this group, free T4 also remained normal and AB were negative. There was a significant TSH reduction (p = 0.017), and normal TSH concentrations (< 5.0 µUI/mL) were observed in 29/31 patients, in at least one moment. No patient had TSH > 15 µUI/mL. CONCLUSION: DS patients with TSH-nd present low risk of progression to HT (10% for females and 6% for males).

As reações alérgicas ao GH são raras e usualmente representadas por reações de hipersensibilidade tipo I (IgE mediadas), passíveis de tratamento por dessensibilização. Neste relato de caso, descrevemos a presença de reação alérgica ao GH mediada por imunocomplexo (hipersensibilidade tipo III). Nesta situação, a tentativa de dessensibilização pode perpetuar a formação de imunocomplexo, cujo depósito pode determinar insuficiência renal e respiratória.

Allergic reactions against GH are rare, and usually represented by the hypersensitivity type I (IgE-mediated). This type of reaction can be treated by desensitization. In this case report, we present a patient showing an allergic reaction soon after starting GH therapy mediated by immune complex (hypersensitivity type III reaction). In this condition, the attempt to perform the desensitization procedure can perpetuate immune complex deposition determining a life threatening renal and respiratory insufficiency.

OBJETIVOS: Determinar a sensibilidade aos glicocorticóides (GC) utilizando teste de supressão com dexametasona em doses muito baixas (IV-VLD-DST) em pacientes com artrite reumatóide (AR) e sua correlação com a expressão gênica da isoforma alfa do receptor glicocorticóide (GRα). MÉTODOS: Foram avaliados 20 controles saudáveis e 32 pacientes com AR com Health Assessment Questionnaire (HAQ) e Disease Activity Score 28 joints (DAS), IV-VLD-DST e expressão do GRα em células mononucleares. RESULTADOS: Cortisol basal e porcentagem de redução do cortisol após IV-VLD-DST foram menores no grupo AR do que nos controles, enquanto a expressão de GRα foi similar entre eles. No grupo com AR, ocorreu correlação negativa entre a expressão do GRα e a porcentagem de supressão do cortisol, enquanto nos controles não houve correlação. Ocorreu relação direta entre DAS e expressão de GRα . CONCLUSÕES: Sugerimos que os mecanismos envolvidos na resistência aos GC observada na AR não estejam ao nível da expressão gênica do GRα, já que esta é igual entre os grupos e aumenta com a gravidade da doença.

OBJECTIVES: To analyze glucocorticoid (GC) sensitivity using intravenous very low dose dexamethasone suppression test (IV-VLD-DST) in patients with rheumatoid arthritis (RA) and its correlation with glucocorticoid receptor alpha-isoform (GRα) gene expression. METHODS: We evaluated 20 healthy controls and 32 RA patients with Health Assessment Questionnaire (HAQ) and Disease Activity Score 28 joints (DAS) scores and IV-VLD-DST and GRα expression in mononuclear cells. RESULTS: Basal cortisol and the percentage of cortisol reduction after IV-VLD-DST were lower in RA patients than in controls, whereas GRα expression was similar among groups. In the RA group there was an inverse correlation between GRα expression and the percentage of cortisol suppression that was not observed in controls. There was a direct relationship between DAS and GRα expression. CONCLUSIONS: Mechanisms involved in GC resistance observed in patients with RA are possibly not at the level of GRα gene expression, since it was similar among groups and GRα increased with disease activity.

O tratamento da hiperplasia adrenal congênita (HAC) por deficiência da 21-hidroxilase forma clássica é habitualmente realizado com acetato de hidrocortisona. A hidrocortisona oral, em nosso meio, só está disponível em farmácias de manipulação. A prednisolona possui solução oral estável, comercialmente disponível, e tem como vantagem poder ser utilizada em dose única diária. O objetivo desse estudo foi comparar a eficácia da prednisolona aos resultados obtidos com o acetato de hidrocortisona. Foram estudados 15 pacientes, idade cronológica média (DP) de 7,2 anos (3,6), em dois períodos consecutivos de um ano, inicialmente utilizando a hidrocortisona (17,5 mg/m²/dia, divididos em três doses), seguida do uso de prednisolona (3 mg/m²/dia, dose única matinal). A avaliação dos tratamentos foi realizada por meio das variações do escore Z de estatura para idade cronológica (deltaZE), do escore Z de estatura para a idade óssea (deltaZEIO) e do escore Z do Índice de massa corporal (IMC) (deltaZIMC), bem como os valores da androstenediona em cada período. Não houve diferença na deltaZE, na deltaZEIO e na deltaZIMC entre os dois períodos, assim como nos valores de androstenediona. Concluiu-se que a prednisolona em dose única diária apresenta eficácia semelhante à obtida com a hidrocortisona utilizada três vezes ao dia, podendo ser considerada uma opção terapêutica nos pacientes com HAC por deficiência da 21-hidroxilase.

Hydrocortisone acetate is usually employed in the treatment of classic congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency. In Brazil, however, oral hydrocortisone acetate is only available from manipulation pharmacies. Prednisolone has stable oral pharmaceutical formulations commercially available, with the advantage of a single daily dose. The aim of this study was to compare the efficacy of oral prednisolone and oral hydrocortisone in the treatment of CAH due to 21-hydroxylase deficiency. Fifteen patients with mean (SD) chronological age of 7.2 (3.6) years, were evaluated in two consecutive 1-year periods. In the first year, hydrocortisone (17.5mg/m²/day, divided in three doses) was used in the treatment, followed by the use of prednisolone (3 mg/m²/day, once in the morning) in the second year. The comparison between the two treatments was assessed after a one-year treatment period by: variation of height standard deviation score (SDS) (delta Height SDS), variation of height SDS according to bone age (delta BA SDS), variation of body mass SDS (delta BMI SDS) and serum levels of androstenedione. No significant difference was observed in relation to the delta Height SDS, delta BA SDS and delta BMI SDS. No significant difference was observed in the serum levels of androstenedione. We conclude that the efficacy of prednisolone administered once a day orally is comparable to the oral use of hydrocortisone three times a day. Oral prednisolone may be an option for patients with CAH due to 21-hydroxylase deficiency.

As potências antiinflamatória e imunossupressora dos glicocorticóides (GC) já foram bem estabelecidas previamente. No entanto, os GC também possuem atividade reguladora da proliferação celular e da morte celular programada (apoptose). O objetivo deste estudo foi determinar a potência relativa de diferentes GC na modulação da sobrevida celular. Linfoblastos cortico-sensíveis (linhagem celular CEM-C7/14) foram mantidos em cultura prolongada e submetidos ao tratamento com GC por 48h, em doses variando entre 10-8 e 10-5 molar. O índice de sobrevida celular foi quantificado pelo teste MTT (DimetilTiazol-Tetrezolium). Para cada GC avaliado, foram realizados pelo menos quatro experimentos em quadruplicata. A resposta celular aos diferentes GC foi analisada através do teste estatístico ANOVA on Ranks, enquanto a resposta ao mesmo GC em concentrações diferentes foi analisada pelo teste ANOVA for repeated measures. O EC50 de cada GC foi calculado utilizando-se o software GraphPad Prism 3.0. Durante o uso de concentrações baixas (10-8 e 10-7 molar), observou-se sobrevida semelhante dos linfoblastos após tratamento com hidrocortisona ou metilprednisolona. Nestas mesmas concentrações baixas, a sobrevida celular foi menor quando utilizou-se dexametasona, betametasona, budesonida ou mometasona. A mometasona e a metilprednisolona foram os dois GC que determinaram maior redução da sobrevida linfoblástica. Nossos resultados sugerem que as potências antiproliferativa e pró-apoptótica dos GC sejam diferentes dos efeitos antiinflamatórios e imunossupressores previamente estabelecidos para estes GC.

Relative antiinflammatory and immunosuppressive potencies of glucocorticoids (GC) were previously well defined. Nonetheless, GC also regulate cell proliferation and programmed death (apoptosis). The aim of this study was to determine the relative potency of different GC on the modulation of cell survival. The GC-sensitive lymphoblast cell line CEM-c7/14 was submitted to 48h-exposure to GC (dose-response curve from 10-8 to 10-5M). Cell survival was analyzed employing the DimethylTiazol-Tetrazolium (MTT) test. For each GC at least 4 experiments were performed in quadruplicate. Responses to different GC at the same molarity were analyzed by ANOVA on Ranks. Cell responses to the same GC in different concentrations were tested by repeated measures ANOVA. The EC50 for each GC was calculated with the GraphPad Prism 3.0 software. The use of low concentrations (10-8 and 10-7M) of hydrocortisone and methylprednisolone determined a similar effects on cell survival, which was less prominent than that observed with betamethasone, budesonide or momethasone. Momethasone was the most potent GC, inducing the most intense dexamethasone reduction on cell survival at the lowest concentration (10-8M). Momethasone and methylprednisolone were the two GC with the strongest impact on cell survival. Our findings suggest that antiproliferative and apoptotic potencies of GC are different from those previously reported antiinflammatory and immunosuppressive actions.
