RESUMO O compositor francês Maurice Ravel, no ápice de sua carreira, mostrou sinais de uma desordem progressiva que afetou sua capacidade de funcionar com linguagem verbal e musical, como notado pelo neurologista Théophile Alajouanine. O agravamento de sua condição levou a uma craniotomia, efetuada em 1937, que deixou de revelar a causa de sua doença, tendo ele falecido pouco depois. A ausência de evidência neuropatológica pós-morte impediu o diagnóstico definitivo da doença, que permaneceu enigmático. Especulações sobre o diagnóstico preciso da doença neurológica de Ravel foram baseadas sobretudo nas observações de Alajouanine, que compreendiam afasia e amusia, predominantemente expressiva, e também apraxia, enquanto o julgamento, gosto e memória musicais permaneceram relativamente intactos, implicando diferentes substratos neuroanatômicos. A possibilidade de uma subforma do complexo da degenearação lobar frontotemporal foi a sugestão diagnóstica de muitos autores. ABSTRACT The French composer, Maurice Ravel, at the peak of his career, showed signs of a progressive disorder that affected his ability to function with verbal and musical language, as noted by the neurologist Théophile Alajouanine. The worsening of the disease led to a craniotomy, performed in 1937, which failed to reveal the cause of his illness, and he died shortly thereafter. A lack of post-mortem neuropathological evidence precluded a definitive diagnosis of the illness, which remained enigmatic. Speculations about the precise diagnosis of Ravel's neurological disease have been largely based on Alajouanine's observations, which included aphasia and amusia, mostly expressive, and ideomotor apraxia, while musical judgement, taste, and memory remained relatively intact, implying different neuroanatomical substrates. A possible subform of frontotemporal lobar degeneration complex was the diagnostic suggestion of many authors. Abstract Jean-Martin Charcot, widely regarded as a leading founder of modern neurology, made substantial contributions to the understanding and characterization of numerous medical conditions. His initial focus was on internal medicine, later expanding to include neuropathology, general neurology, and eventually emerging fields such as neuropsychology and neuropsychiatry. Furthermore, Charcot's intellectual pursuits extended beyond medicine, encompassing research in art history, medical iconography, sociology, religious studies, and the arts, solidifying his status as a polymath. ABSTRACT Some of the earliest conceptual milestones in memory research with relevance to the physical means through which its preservation is made possible, namely, the ‘memory trace’ or ‘engram’, are analysed in this study. The fundamental notions were laid down by Platon and Aristoteles. While Platon regarded memory as an imprint on a ‘wax block’ in the immortal soul, Aristoteles considered memory a modification in the mortal soul, imprinted like a cast at birth time. The Roman orators were interested in mnemotechnics, and Cicero is credited for the term ‘trace’ (vestigium) used for the first time. Much later, Descartes described the (memory) ‘trace’ (trace), linking psychic, and physical processes. Finally, Semon posited innovative concepts and terms centralized by the ‘engram’ (Engramm). RESUMO Alguns dos marcos conceituais iniciais na pesquisa de memória, com relevância para o meio físico, pelo qual a preservação desta é possibilitada, a saber, o ‘traço de memória’ ou ‘engrama’, são aqui analisados. As noções fundamentais foram formuladas por Platão e Aristóteles. Enquanto Platão via a memória como uma impressão em um ‘bloco de cera’ na alma imortal, Aristóteles considerava a memória uma modificação na alma mortal, impressa como um molde ao nascimento. RESUMO Desde a publicação das últimas recomendações para o diagnóstico e tratamento da Demência Vascular pela Academia Brasileira de Neurologia em 2011, avanços significativos ocorreram na terminologia e critérios diagnósticos. O presente manuscrito é resultado do consenso entre especialistas indicados pelo Departamento Científico de Neurologia Cognitiva e do Envelhecimento da Academia Brasileira de Neurologia (2020-2022). O objetivo foi atualizar as recomendações práticas para a identificação, classificação e diagnóstico do Comprometimento Cognitivo Vascular (CCV). As buscas foram realizadas nas plataformas MEDLINE, Scopus, Scielo e LILACS. As recomendações buscam fornecer uma ampla revisão sobre o tema, então sintetizar as evidências para o diagnóstico do CCV não apenas para neurologistas, mas também para outros profissionais de saúde envolvidos na avaliação e nos cuidados ao paciente com CCV, considerando as diferentes realidades dos níveis de atenção à saúde (primário, secundário e terciário) no Brasil.

ABSTRACT Since the publication of the latest recommendations for the diagnosis and treatment of Vascular Dementia by the Brazilian Academy of Neurology in 2011, significant advances on the terminology and diagnostic criteria have been made. This manuscript is the result of a consensus among experts appointed by the Scientific Department of Cognitive Neurology and Aging of the Brazilian Academy of Neurology (2020-2022). We aimed to update practical recommendations for the identification, classification, and diagnosis of Vascular Cognitive Impairment (VCI). Searches were performed in the MEDLINE, Scopus, Scielo, and LILACS databases. This guideline provides a comprehensive review and then synthesizes the main practical guidelines for the diagnosis of VCI not only for neurologists but also for other professionals involved in the assessment and care of patients with VCI, considering the different levels of health care (primary, secondary and tertiary) in Brazil.

RESUMO. O interesse pela anatomia data desde a mais remota antiguidade. Esse conhecimento foi adquirido por meio de dissecções em animais e cadáveres humanos, por muitos pesquisadores. A anatomia macroscópica de variadas estruturas do cérebro foi identificada com o passar dos séculos, e a substância sólida predominante foi vista como amorfa e destituída de qualquer função específica até o período do Renascimento. René Descartes, um personagem com uma mente brilhante e criativa, concebeu o cérebro, sua estrutura e função, de um modo distinto ao que era conhecido no seu tempo. Ele valorizou e deu à substância sólida, pela primeira vez, uma estrutura teórica formada por elementos minúsculos, relacionada a uma função presuntiva, baseada na presença da glândula pineal e dos espíritos animais, subjacentes às atividades cognitiva, sensorial e motora. Essa vista estrutural foi, de certa maneira, endossada pelos achados microscópicos de Marcello Malpighi, o que começou a modificar a compreensão do sistema nervoso.

ABSTRACT. Interest in anatomy dates from the earliest times. Such knowledge was acquired through dissections of animals and human corpses by many researchers. The macroscopic anatomy of the varied structures of the brain were identified over the centuries, and the predominating solid substance was seen as amorphous, and devoid of any specific function, until the Renaissance. René Descartes, a personage with a brilliant and creative mind, conceived the brain, its structure and function, in a distinct manner to what was known at his time. He valued the solid matter and gave it, for the first time, a theoretical minute structure, related to a presumptive function based on the presence of the pineal gland and the animal spirits, underlying cognitive, sensory and motor activities. Such structural view was endorsed, in a given sense, by the microscopic findings of Marcello Malpighi, which begun to change the understanding of the nervous system.

RESUMO Os canabinoides compreendem os endocanabinoides, fitocanabinoides e os canabinoides sintéticos e desempenham ações no sistema nervoso central e periférico. Uma quantidade enorme de publicações tem sido lançada nos últimos anos, embora a cannabis seja conhecida por milênios. Os Departamentos Científicos da Academia Brasileira de Neurologia descreveram as evidências do uso médico em suas áreas. A literatura está em constantes mudanças e possíveis novas evidências podem surgir nos próximos dias ou meses. A prescrição dessas substâncias deve ser discutida com os pacientes e suas famílias, com conhecimento sobre eventos adversos e sua eficácia.

ABSTRACT Cannabinoids comprehend endocannabinoids, phytocannabinoids, and synthetic cannabinoids, with actions both in the central and peripherical nervous systems. A considerable amount of publications have been made in recent years, although cannabis has been known for over a thousand years. Scientific Departments from the Brazilian Academy of Neurology described evidence for medical use in their areas. Literature is constantly changing, and possible new evidence can emerge in the next days or months. Prescription of these substances must be discussed with patients and their families, with knowledge about adverse events and their efficacy.

RESUMO O estudo de longa data da anatomia macroscópica experimentou um incremento considerável com o advento do microscópio no início do século 17. A personalidade representativa dessa nova era foi, certamente, Marcello Malpighi, considerado “fundador da anatomia microscópica”. Ele estudou, com um microscópio composto rudimentar, numerosos tecidos e órgãos de diversas classes de animais, assim como plantas. Descreveu, pela primeira vez, a estrutura microscópica do sistema nervoso, identificando na substância cinzenta dos vários níveis elementos de minúsculas dimensões, que denominou “glândulas”. Deve-se lembrar que o conceito de “célula” (e de “célula nervosa”) era desconhecido naquele tempo. Muitos pesquisadores seguiram realizando estudos microscópicos, mas sem resultados melhores, e o entendimento de Malpighi foi mantido até o início do século 19, quando apareceram técnicas histológicas novas de processamento e de coloração, assim como microscópios mais aprimorados.

ABSTRACT The longstanding study of gross anatomy experienced a considerable improvement with the advent of the microscope in the early 17th century. The representative personality of this new era certainly was Marcello Malpighi, seen as “founder of microscopic anatomy”. He studied, with a rudimentary compound microscope, numerous tissues and organs of several classes of animals, as well as plants. He described, for the first time, the microscopic structure of the nervous system, identifying in the gray matter of its various levels minute elements he took as “glands”. It should be reminded that the concept of “cell” (and “nerve cell”) was unknown at his time. Many researchers followed, performing microscopic studies, but without better results, and Malpighi’s view was maintained until the beginning of the 19th century, when new histological processing and staining techniques appeared, as well as improved microscopes.

RESUMO. Os sintomas neuropsiquiátricos que podem ocorrer de modo isolado ou acompanhando manifestações cognitivas e neurológicas são ocorrências frequentes em variadas disfunções ou lesões cerebrais devidas a condições vasculares, traumáticas, neurodegenerativas ou sistêmicas. Através da história, muitos dos nomes mais proeminentes contribuíram para a definição clínica dos sintomas e síndromes mentais reconhecidos atualmente. O presente artigo visa prover uma visão abrangente sobre o desenvolvimento, desde os tempos antigos até os presentes, de alguns dos conceitos e constructos amplamente reconhecidos sobre tais transtornos neuropsiquiátricos.

ABSTRACT. Neuropsychiatric symptoms, which may appear alone or combined with cognitive and neurological manifestations, are frequent in many brain dysfunctions or lesions due to vascular, traumatic, neurodegenerative, or systemic conditions. Throughout history, many of the most prominent names have contributed to the clinical definition of the currently recognized mental symptoms and syndromes. The present paper aims at providing a comprehensive overview of the development, from ancient to modern times, of some widely known concepts and constructs about such neuropsychiatric disorders.

RESUMO A natureza da memória e a busca de sua localização tem sido objeto de interesse desde a Antiguidade. Após milênios de conceitos teóricos, mudando do coração para o cérebro e daí dos ventrículos para as partes sólidas, as estruturas centrais relacionadas com a memória finalmente começaram a ser identificadas através de métodos modernos com base científica, nos níveis diencefálico e cortical (hipocampal e neocortical), principalmente no período Moderno tardio, aproximando-se do estado atual do conhecimento sobre o tema.

After millennia of theoretical concepts, shifting from the heart to the brain, then from the ventricles to solid parts, the core memory-related structures finally began to be identified through modern scientifically-based methods at the diencephalic and cortical (hippocampal and neocortical) levels, mostly in the late Modern period, culminating in the current state of knowledge on the subject.

RESUMO. Os debates milenares sobre a localização da mente (funções superiores) chegaram a um novo caminho quando Willis localizou uma função nervosa superior (memória) no parênquima cerebral, e supostamente, no córtex cerebral. Cerca de dois séculos depois, Broca, baseado em sólido pensamento científico, localizou uma área circunscrita da 3ª circunvolução frontal do hemisfério esquerdo como o sítio da linguagem articulada, uma função superior (fala - domínio da linguagem). Ele (e Dax) também definiu a assimetria funcional (especialização) dos hemisférios, com dominância esquerda (para linguagem). O período entre os achados dessas duas personalidades não ficou quiescente, considerando que numerosos autores contribuíram, com suas pesquisas teóricas e clinicopatológicas, para criar uma atmosfera científica adequada para tal realização, devendo ser vistos como importantes. Mais estudos, nas décadas seguintes, revelaram a localização de aspectos adicionais da linguagem e de outras funções superiores (domínios cognitivos).

ABSTRACT. The age-old debates about the localization of the mind (higher functions) took a new course when Willis located a higher nervous function (memory) in the brain parenchyma, and supposedly, in the cerebral cortex. About two centuries later, Broca, founded on solid scientific reasoning, localized a circumscribed area of the 3rd frontal circumvolution of the left hemisphere as the seat of articulate language, a higher function (speech - language domain). He (and Dax) also defined the functional asymmetry (specialization) of the hemispheres, with left dominance (for language). The period between the findings of these individuals was not quiescent, as numerous authors contributed with their theoretical and clinicopathological research toward creating a conducive scientific atmosphere for this accomplishment, and should be regarded as important. Further studies, in the decades that followed, revealed the localization of additional aspects of language and of other higher functions (cognitive domains).

Resumo Os debates sobre a mente e suas funções superiores, e as tentativas de estabelecer suas localizações no corpo, foram objeto de interesse de inúmeros sábios desde os tempos mais remotos. A dúvida quanto à parte do corpo que abrigava tais funções, o coração (doutrina cardiocêntrica) ou o cérebro (doutrina cefalocêntrica), abriu as buscas. Os egípcios, há milênios, mantinham um ponto de vista cardiocêntrico. Muito tempo depois, antigos acadêmicos gregos retomaram o tema, mas permaneceram indecisos entre o coração e o cérebro, uma controvérsia que durou séculos. O ponto de vista cefalocêntrico predominou, e seguiu-se uma nova questão, quanto ao lugar dessas funções no cérebro, os ventrículos ou o tecido nervoso, que também durou séculos. Esta última localização, apesar de inicialmente equivocada, prosperou. Entretanto, esta representou apenas um começo, já que mais estudos, nos séculos seguintes, revelaram definições e localizações mais precisas para as funções mentais superiores.

Abstract The debates about the mind and its higher functions, and attempts to locate them in the body, have represented a subject of interest of innumerable sages since ancient times. The doubt concerning the part of the body that housed these functions, the heart (cardiocentric doctrine) or the brain (cephalocentric doctrine), drove the search. The Egyptians, millennia ago, held a cardiocentric view. A very long time later, ancient Greek scholars took up the theme anew, but remained undecided between the heart and the brain, a controversy that lasted for centuries. The cephalocentric view prevailed, and a new inquiry ensued about the location of these functions within the brain, the ventricles or the nervous tissue, which also continued for centuries. The latter localization, although initially inaccurate, gained traction. However, it represented only a beginning, as further studies in the centuries that followed revealed more precise definitions and localizations of the higher mental functions.

RESUMO. Introdução: O Comprometimento Cognitivo Vascular Subcortical (CCVS) é um contínuo clínico de comprometimento cognitivo de causa vascular, que inclui o Comprometimento Cognitivo Leve Vascular (CCLV) e a Demência Vascular. Déficits em Função Executiva (FE) são marcantes no quadro, mas a melhor metodologia para avaliar essa função ainda necessita ser determinada. A evolução insidiosa e quase previsível do CCVS e o conceito atual multidimensional de FE sugerem que uma dissociação temporal de comprometimentos dos domínios da FE exista nos estágios iniciais do transtorno. Objetivo: Visou-se a revisar e analisar dados da literatura sobre o desempenho de sujeitos com CCLV nos testes mais usados de FE através de uma abordagem metanalítica. Métodos: Foram realizadas buscas no Medline, Web of Knowledge e PsycINFO, usando os seguintes termos: “comprometimento cognitivo vascular” OU “comprometimento cognitivo não demência vascular” OU “transtorno neurocognitivo leve vascular” E “função executiva” OU “disexecutiva”. Metanálises foram conduzidas para cada um dos testes, usando-se modelos de efeitos aleatórios. Resultados: A revisão sistemática demonstrou grande discrepância entre os resultados dos estudos incluídos. A metanálise evidenciou desempenhos piores no Teste de Trilhas parte B e no teste de cores do Stroop em sujeitos com CCLV em comparação com controles. Conclusão: Um contínuo de comprometimentos em FE no CCVS foi proposto. Déficits iniciais parecem ocorrer em flexibilidade cognitive e controle inibitório.

ABSTRACT. Background. Subcortical Vascular Cognitive Impairment (SVCI) is a clinical continuum of vascular-related cognitive impairment, including Vascular Mild Cognitive Impairment (VaMCI) and Vascular Dementia. Deficits in Executive Function (EF) are hallmarks of the disorder, but the best methods to assess this function have yet to be determined. The insidious and almost predictable course of SVCI and the multidimensional concept of EF suggest that a temporal dissociation of impairments in EF domains exists early in the disorder. Objective: This study aims to review and analyze data from the literature about performance of VaMCI patients on the most used EF tests through a meta-analytic approach. Methods: Medline, Web of Knowledge and PsycINFO were searched, using the terms: “vascular mild cognitive impairment” OR “vascular cognitive impairment no dementia” OR “vascular mild neurocognitive disorder” AND “dysexecutive” OR “executive function”. Meta-analyses were conducted for each of the selected tests, using random-effect models. Results: Systematic review showed major discrepancies among the results of the studies included. Meta-analyses evidenced poorer performance on the Trail-Making Test part B and the Stroop color test by VaMCI patients compared to controls. Conclusion: A continuum of EF impairments has been proposed in SVCI. Early deficits appear to occur in cognitive flexibility and inhibitory control.

RESUMO. O conceito de “apoplexia”, conhecido há muito tempo, pode ser seguida desde a Antiguidade, passando pela Idade Média e a Renascença e alcançando a era Moderna e os dias presentes, com uma nova designação, “acidente vascular cerebral”. A definição de “apoplexia” pode ser dividida, pela história da autópsia, em um período precedendo essa prática, que durou da Antiguidade até a Renascença, com um conceito guarda-chuva relativamente estável baseado na clínica, e em um período de autópsia da era Moderna, quando foi alcançada a separação dessa condição em diversos subtipos. Assim, levou cerca de 2.500 anos reunindo os numerosos fragmentos de informação para chegar a um quadro satisfatoriamente bem definido. O conceito de “acidente vascular cerebral” herdou tal informação desenvolvida para a “apoplexia”, incorporando todas as aquisições históricas, para formar o presente estado de conhecimento.

ABSTRACT. The long-standing concept of “apoplexy' can be followed from Antiquity, passing through the Middle Ages and Renaissance, and reaching the Modern era and the present day, with the new designation of “stroke”. The definition of “apoplexy” can be divided, by the history of autopsy, into a period predating this practice, which spanned from Antiquity until the Renaissance, with a relatively stable clinically-based umbrella concept, and an autopsy period of the Modern era, when the condition was subdivided into several subtypes. Thus, it took about 2,500 years assembling the numerous pieces of information to achieve a fairly well-defined picture. The “stroke” concept inherited the information developed for “apoplexy”, incorporating all historical acquisitions to form the current state of this knowledge.

RESUMO. A última década vem testemunhando avanços substanciais na aquisição de marcadores biológicos para o diagnóstico da doença cerebrovascular (DCV). Os métodos de neuroimagem avançados não apenas fornecem uma contribuição estratégica para o diagnóstico diferencial do comprometimento cognitivo vascular (VCI) e demência vascular (VaD), mas contribuem substancialmente na elucidação dos mecanismos fisiopatológicos relacionados à doença de vasos pequenos (SVD) e sua progressão clínica. Objetivo: Nesta revisão, métodos de imagem estruturais e metabólicos foram descritos e sua importância diagnóstica analisada, particularmente na investigação da CVD relacionada ao envelhecimento. Métodos: uma pesquisa eletrônica de janeiro de 2010 a 2017 foi realizada através do PubMed/MEDLINE, do Instituto de Informação Científica Web of Knowledge e da EMBASE. Resultados: O emprego de estudos de multimodalidade plenamente funcional com Ressonância Magnética (MR)/ Tomografia por Emissão de Pósitrons (PET) representa uma janela para a caracterização clínica mais detalhada da VCI-VaD; com relação à neuroimagem estrutural, a ressonância magnética em 3,0 T vem permitindo varreduras com maior resolução e matrizes de imagem mais elevadas, cortes mais delgados e maior detalhamento anatômico das lesões vasculares. Conclusão: Embora a importância da maior parte dessas técnicas no cenário clínico aguarde reconhecimento, há uma grande expectativa de que o seu uso favoreça intervenções terapêuticas progressivamente mais precoces e refinadas para o gerenciamento efetivo do VCI-VaD.

ABSTRACT. The last decade has witnessed substantial progress in acquiring diagnostic biomarkers for the diagnostic workup of cerebrovascular disease (CVD). Advanced neuroimaging methods not only provide a strategic contribution for the differential diagnosis of vascular dementia (VaD) and vascular cognitive impairment (VCI), but also help elucidate the pathophysiological mechanisms ultimately leading to small vessel disease (SVD) throughout its course. Objective: In this review, the novel imaging methods, both structural and metabolic, were summarized and their impact on the diagnostic workup of age-related CVD was analysed. Methods: An electronic search between January 2010 and 2017 was carried out on PubMed/MEDLINE, Institute for Scientific Information Web of Knowledge and EMBASE. Results: The use of full functional multimodality in simultaneous Magnetic Resonance (MR)/Positron emission tomography (PET) may potentially improve the clinical characterization of VCI-VaD; for structural imaging, MRI at 3.0 T enables higher-resolution scanning with greater imaging matrices, thinner slices and more detail on the anatomical structure of vascular lesions. Conclusion: Although the importance of most of these techniques in the clinical setting has yet to be recognized, there is great expectancy in achieving earlier and more refined therapeutic interventions for the effective management of VCI-VaD.
