Abstract Objective: To describe the behavior of total alkaline phosphatase (tALP) in patients with metastatic castration-resistant prostate cancer receiving radium-223 therapy, in a real-world scenario, and to describe overall survival (OS) among such patients. Materials and Methods: This was a retrospective study involving 97 patients treated between February 2017 and September 2020. Patients were stratified by the baseline tALP (normal/elevated). A tALP response was defined as a ≥ 30% reduction from baseline at week 12. For patients with elevated baseline tALP, we also evaluated treatment response as a ≥ 10% reduction in tALP after the first cycle of treatment. We defined OS as the time from the first treatment cycle to the date of death. Results: There was a significant reduction in the median tALP after each cycle of treatment (p < 0.05 for all). Data for tALP at week 12 were available for 71 of the 97 patients. Of those 71 patients, 26 (36.6%) responded. Elevated baseline tALP was observed in 47 patients, of whom 19 (40.4%) showed a response. Longer OS was observed in the patients with normal baseline tALP, in those with elevated baseline tALP that showed a response to treatment (≥ 10% reduction), and in those who received 5-6 cycles of therapy. Conclusion: The tALP may be used to predict which patients will benefit from treatment with a greater number of cycles of radium-223 therapy and will have longer OS. Resumo Objetivo: Descrever o comportamento da fosfatase alcalina total (tALP) em pacientes com carcinoma de próstata metastático resistente a castração, submetidos a terapia com rádio-223 em um cenário do mundo real, e a sobrevida global (SG) desses pacientes. Materiais e Métodos: Estudo retrospectivo envolvento 97 pacientes, no período de fevereiro/2017 a setembro/2020. Os pacientes foram estratificados de acordo com a tALP basal (normal/elevada). A resposta à tALP foi definida como uma redução em relação à linha de base de ≥ 30% na semana-12. Resumo Introdução: A doença cística da adventícia é uma condição vascular rara e que afeta na maioria dos casos a artéria poplítea. Habitualmente cursa com claudicação intermitente durante o exercício. Caso clínico: Descreve-se um caso clínico de uma doente com doença cística da adventícia da artéria poplítea. A apresentação clínica foi sob a forma de claudicação intermitente gemelar direita. Procedeu-se ao tratamento cirúrgico, por abordagem posterior, com exérese do quisto mantendo a integridade da parede luminal. Assim, não se verificou a necessidade de reconstrução arterial. Discussão: Possíveis etiologias incluem traumatismo, doença sistémica, doença sinovial ou alterações do desenvolvimento embrionário. O tratamento depende do vaso afetado e a remoção do quisto com preservação ou substituição arterial tem apresentado resultados promissores. O seguimento a longo prazo é mandatório, atendendo ao risco de recidiva.

Abstract Objectives To evaluate the relationship between age at onset of cannabis use and cannabis abuse in a sample of Portuguese cannabis users, testing the mediating role of the number of friends using cannabis and comparing these relationships between male and female subsets. Method A sample of 529 Portuguese cannabis users comprising 276 males and 244 females aged from 14 to 21 years completed a sociodemographic questionnaire, the Cannabis Abuse Screening Test (CAST), and answered four questions related to cannabis use. Data were analyzed using the PROCESS macro in SPSS version 26.0. Results Age at onset of cannabis use was negatively related to the number of friends using cannabis, while the number of friends using cannabis had a positive relationship with cannabis abuse. As predicted, the number of friends using cannabis seems to have a mediating role in the relationship between cannabis onset age use and cannabis abuse, since the indirect effect was found to be significant. The pattern of the relationships among the variables observed was found to be similar for both male and female subsets of the sample. However, males reported higher cannabis abuse than females. Conclusion The results highlight the mediating role of friends’ use of cannabis in the relationship between age at onset of cannabis use and users’ abuse. These results highlight the importance of early intervention in cannabis use. In particular, the pattern of the relationships observed among the studied variables emphasizes the need to monitor and conduct peer training interventions or interventions to promote of social skills.

Resumo Introdução: Ao longo das últimas décadas houve uma evolução nas opções de tratamento da isquemia aguda de membro (IAM). Hoje em dia, a trombólise dirigida por cateter (TDC) é por vezes utilizada como alternativa à cirurgia no tratamento da IAM e pode oferecer algumas vantagens em relação à cirurgia convencional em pacientes devidamente seleccionados. Objectivo: Avaliar os dados relativos à TDC para o tratamento da isquemia aguda de membro em diferentes etiologias. Materiais e métodos: Foram consecutivamente analisados todos os pacientes submetidos a TDC na isquemia aguda de membro no nosso serviço de cirurgia vascular, entre 1 de Janeiro de 2011 e 31 de Agosto de 2017. Os resultados incluíram a patência primária aos 30 dias e um ano, complicações hemorrágicas, amputação major e mortalidade. Resultados: No total foram incluídos 128 membros de 106 pacientes. O seguimento mediano foi de 14 meses [6-31 meses]. As etiologias de isquemia incluídas no estudo foram trombose de artérias nativas, trombose de bypass PTFE ou VGS, trombose intra-stent, trombose de aneurisma ou entrapment poplíteo e embolia arterial. De acordo com a classificação de Rutherford na IAM foram observados no grau I 12,5%, grau IIa 77,3% e grau IIb 10,2%. As taxas de patência primária foram de 72,8% e 47,8% a um e três anos, respectivamente. A reintervenção foi de 27,6% no grupo da trombose de artéria nativa, 65,2% no grupo da trombose de bypass de PTFE, e 18,2% no grupo da trombose intra-stent. Não foram verificadas reintervenções para os grupos de aneurisma poplíteo ou embolia arterial. A sobrevida livre de amputação foi de 83,3% aos 27 meses, e a incidência cumulativa de morte foi de 10,1% aos 32 meses. Observaram-se complicações em 40 procedimentos (31,3%); a maioria hemorragia minor do local de acesso e uma morte devido a acidente vascular cerebral hemorrágico. Conclusão: A TDC embora associada a um risco acrescido de complicações hemorrágicas, é considerada um tratamento seguro e com resultados que podem reduzir a necessidade de tratamento cirúrgico convencional em doentes selecionados. O nosso estudo vem reforçar o uso desta terapêutica como uma opção válida na IAM em diferentes etiologias.

Abstract Introduction: Over the past few decades management options of the acute limb ischaemia (ALI) have changed. Nowadays, catheter direct thrombolysis (CDT) is commonly used as an alternative to surgery in immediate management of the ALI and may offer certain advantages over surgery in appropriately selected patients. Objective: To evaluate CDT data for the treatment of acute ischaemia in lower extremity in different aetiologies. Materials and methods: All consecutive patients who underwent CDT for acute limb ischaemia in our vascular surgery department, between 1 January 2011 to 31 August 2017 were identified and reviewed. Outcome measures included primary patency at 30 days and one year, haemorrhagic complications, major amputation and mortality. Results: In total, 128 limbs from 106 patients were included. The median follow-up was 14 months [range: 6-31 months]. The aetiologies of ischaemia included in the study were native artery thrombosis, PTFE and GSV bypass thrombosis, intra-stent thrombosis, popliteal aneurysm and entrapment thrombosis and arterial embolism. The Stages of ALI according to the Rutherford classification were 12,5% in class I, 77,3% in class IIa and 10,2% in class IIb. Primary patency rates were 71.3% and 47.8% at one and three years, respectively. The requirement for re-intervention was 27.6% in the native artery thrombosis group, 65.2% in the PTFE graft thrombosis group, and 18.2% in the intra-stent thrombosis group. No re-interventions were verified for popliteal aneurysm or arterial embolism aetiologies. The overall amputation free survival was 83.3% at 27 months, and the cumulative incidence of death was 10.1% at 32 months. Complications occurred in 40 interventions (31.3%); the majority comprised minor bleeding from the access site, and one death due to haemorrhagic stroke. Conclusions: CDT although associated with an increased risk of bleeding complications, it is a feasible and safe therapy, with clinical outcomes that may reduce the need for open surgical treatment in many patients. Our findings support this therapeutic approach as a valid option in ALI, in different aetiologies.

OBJECTIVES: To evaluate the mean concentration of 25-hydroxivitamin D [25(OH) D] and prevalence of hypovitaminosis D in individuals residing in Rio de Janeiro, Brazil. METHODS: The data of 80,000 consecutive individuals who had 25(OH) D measurements performed by electrochemiluminescence between 1/2/2018 and 2/5/2018 were selected. Patients who reported the use of therapies/supplements were excluded. Levels of 25(OH) D ≥20 ng/mL (ages <60 years) and ≥30 ng/mL (ages ≥60 years) were considered adequate. RESULTS: We analyzed the data of 24,074 individuals (1-95 years old, 64.7% female). Descriptive curves showed that, in both sexes, the mean values of 25(OH) D decreased from the first years of life until adolescence, then slightly increased, and then tended to stabilize during adulthood. Levels of 25(OH) D <20 ng/mL were observed in 6% of girls versus 3.6% of boys and in 13.6% of adolescent girls versus 12.6% of adolescent boys and 11% of adults. The percentage of seniors with serum levels of 25(OH) D <20 ng/mL was 13.6% in women and 12.7% in men; 53.2% of women and 50.6% of men had levels <30 ng/mL. CONCLUSIONS: Mean 25(OH) D values were higher in children and lower in adolescents and women. Approximately 90% of non-seniors and presumably healthy residents of the urban metropolitan region of Rio de Janeiro presented satisfactory levels of 25(OH) D during the summer months; however, in over half of the elderly, the serum concentrations of 25(OH) D were inadequate. Therefore, strategies for the prevention of hypovitaminosis D should be considered in the senior population.

ABSTRACT During the COVID-19 pandemic, special attention has been addressed in cancer care to mitigate the impact on the patient’s prognosis. We addressed our preparation to face COVID-19 pandemic in a Hematological and Stem Cell Transplant Unit in Brazil during the first two months of COVID-19 pandemic and described COVID-19 cases in patients and health care workers (HCW). Modifications in daily routines included a separation of area and professionals, SARS-CoV-2 screening protocols, and others. A total of 47 patients and 54 HCW were tested for COVID-19, by PCR-SARS-CoV-2. We report 11 cases of COVID-19 in hematological patients (including 2 post stem cell transplant) and 28 cases in HCW. Hematological cases were most severe or moderate and presented with several poor risk factors. Among HCW, COVID-19 were mostly mild, and all recovered without hospitalization. A cluster was observed among HCW. Despite a decrease in the number of procedures, the Transplant Program performed 8 autologous and 4 allogeneic SCT during the period, and 49 onco-hematological patients were admitted to continuing their treatments. Although we observed a high frequency of COVID-19 among patients and HCW, showing that SARS-CoV-2 is disseminated in Brazil, hematological patients were safely treated during pandemic times.

Resumo A pandemia e a inerente alteração de comportamentos, a par da parca previsibilidade, geraram maior ansiedade na população. Nesse sentido, este trabalho teve como objetivo analisar se os níveis de depressão, ansiedade e estresse em estudantes universitários se alteraram no período pandêmico (2020) comparativamente a períodos anteriores/normais. O estudo foi constituído por dois grupos, sendo a amostra 1 constituída por 460 sujeitos com idade média de 20,14 anos, e a amostra 2 por 159 sujeitos com idade média de 20,40 anos. Todos preencheram um questionário sociodemográfico e as escalas de ansiedade, depressão e estresse. Os estudantes que integraram o estudo no período pandêmico apresentaram níveis significativamente mais elevados de depressão, ansiedade e estresse comparativamente aos que integraram o estudo no período normal. Os resultados sugerem um impacto psicológico negativo da pandemia nos estudantes. Importa continuar a explorar as implicações da pandemia na saúde mental dos estudantes, para que se possam prevenir e minorar os seus efeitos.

Abstract The pandemic and the attendant change in behavior, together with the general unpredictability, has generated great anxiety in the population. In this context, the aim of this study was to analyze whether the levels of depression, anxiety and stress in university students have changed during the period of the pandemic (2020) compared with the previous normality. This study comprised two groups, in which sample-1 is composed of 460 subjects with a mean age of 20.14 and sample-2, 159 subjects with a mean age of 20.40. All the participants completed a sociodemographic questionnaire and were assessed by way of the Anxiety, Depression and Stress Scales. The students evaluated during the period of the pandemic presented significantly higher levels of anxiety, depression and stress, compared with students in erstwhile, normal times. Our results suggest that the pandemic has a negative psychological effect on students. It is important to continue exploring the implications of the pandemic on students’ mental health, so that its effects can be prevented, or at least mitigated.

Abstract The emergence of information and communication technologies (ICT) has generated a number of research questions, related to their use and potential risk, but also potentials for prevention or health promotion. Online social networks have become an important source of information for users as well as a tool for social relations. As traditional social networks, they can act as vehicles to improve the health of adolescents and youth, as well as play a key role in an educational context. The aim of this work is then to explore the theoretical relevance of ICT, particularly on online social networks, on disease prevention and health promotion of communicable diseases. Literature review points out the role of online social networks, particularly in the field of sexual health, body image, especially eating habits and overweight, as well as smoking and alcohol dependence. Data allow us to understand how online social network behavior and interaction is related to their burden and interventions developed in sexual health and addiction show positive results. More efforts in body image are needed in order to use these tools for prevention and promotion of health from early age.

O uso dos telemóveis é um comportamento quotidiano que nos aproxima da informação e das pessoas que nos são significativas. Com o avanço da tecnologia, tornou-se uma ferramenta presente no dia-a-dia das pessoas e por utilizadores cada vez mais jovens, associada também a alguns riscos ou mesmo a um uso excessivo. No presente trabalho, pretendemos apresentar os estudos de adaptação para a população portuguesa de um instrumento que foi desenvolvido para avaliar a dependência dos telemóveis. Para isso, recorremos a uma amostra de 907 participantes - adolescentes e jovens - a quem foi aplicado um questionário sociodemográfico e o Test of Mobile Phone Dependence (TMD; Chóliz, 2012), através de um formulário online. Os resultados permitiram encontrar uma estrutura de quatro fatores com fidelidade adequada. Permitiram ainda perceber uma relação moderada ou forte entre a pontuação total com os diferentes fatores. Encontramos ainda diferenças de género, com as raparigas a apresentar pontuações médias superiores, e uma relação negativa com a idade. Explorando diferenças entre género e idade, encontramos uma diminuição significativa entre os participantes do género feminino. Os dados recomendam a utilização do instrumento no nosso contexto e permitem estudos futuros nesta temática.

The use of mobile phones is a daily behavior that brings us closer to the information and the most significant people to us. With the developments of technology, mobile phones have become a daily tool for people and for each time younger users, also associated with some risks or even dependence. In the present work, we intend to present the adaptation studies for the Portuguese population of an instrument that was developed to evaluate the dependence of mobile phones. For this, we used a sample of 907 participants - adolescents and young people - to whom a sociodemographic questionnaire and the Test of Mobile Phone Dependence (TMD; Chóliz, 2012) were applied through an online form. The results allowed us to find a structure of four factors with adequate fidelity. They also allowed us to perceive a moderate or strong relationship between the total score and the different factors. We also found gender differences, with girls presenting higher mean scores, and a negative relation with age. Exploring differences between gender and age, we found a significant decrease among participants of the females. The data recommend the use of the instrument in our context and allow future studies in this issue.

Assistimos nas últimas décadas a uma crescente atenção sobre os idosos e os seus cuidadores, formais e informais. Com a organização das respostas, urge acompanhar os técnicos, as suas perceções e indicadores de satisfação, como stress e burnout podem dar indicações úteis para a melhoria das instituições. Existindo ainda escassos estudos sobre esta temática, o presente trabalho visa descrever e analisar a satisfação laboral e o burnout entre assistentes geriátricos, explorando, especificamente o papel de variáveis pessoais e profissionais. Foram recolhidos dados junto de uma amostra de 197 assistentes geriátricos de instituições no norte do País, com recurso a um questionário sociodemográfico, um questionário de satisfação no trabalho e o Oldenburg Burnout Inventory para avaliar o burnout. Dos resultados, destaca-se a maior satisfação com a formação profissional, com o local de trabalho e com os colegas, e menor satisfação com o salário e oportunidades de promoção laboral. No burnout, os valores encontram-se dentro da média teórica sendo superior a exaustão. Os dados permitem ainda perceber uma relação negativa entre o burnout e as habilitações literárias, e positiva entre a experiência profissional e a exaustão. Também na satisfação laboral, algumas dimensões apareceram relacionadas com as habilitações académicas e experiência profissional. A satisfação profissional apareceu como um preditor significativo do burnout, explicando 21,6% da exaustão e 34,8% do distanciamento. Entendendo a satisfação profissional como um indicador do funcionamento das instituições, os dados apresentados podem ser importantes para uma compreensão mais abrangente destas organizações e novas medidas para promover a sua qualidade.

Over the last decades e have witnessed increasing attention on the elderly and their caregivers, both formal and informal. With the organization of the institutions, it is imperative to follow the technicians, their perceptions and indicators of satisfaction, as stress and burnout can give useful indications for the improvement of institutions. Given the few studies on this subject, the present work aims to describe and analyse job satisfaction and burnout among geriatric assistants, specifically exploring the role of personal and professional variables. Data were collected from a sample of 197 geriatric assistants from institutions in the north of the country, using a sociodemographic questionnaire, a job satisfaction questionnaire and the Oldenburg Burnout Inventory to assess burnout. Among the results, we highlight the greater satisfaction with vocational training, with the workplace and with colleagues, and less satisfaction with salary and job promotion opportunities. In the burnout, the values are within the theoretical average being higher in exhaustion. The data also allow us to perceive a negative relationship between burnout and qualifications, and a positive relationship between professional experience and exhaustion. Also, in job satisfaction, some dimensions appeared related to academic qualifications and professional experience. Professional satisfaction appeared as a significant predictor of burnout, accounting for 21.6% of exhaustion and 34.8% of disengagement. Understanding professional satisfaction as an indicator of the functioning of institutions, the data presented may be important for a more comprehensive understanding of these organizations and new measures to promote their quality.

O consumo de cannabis é um dos comportamentos de risco na adolescência que mais se tem destacado nos últimos anos. Perante uma norma social permissiva, a cannabis expandiu-se e ganhou protagonismo na cultura juvenil, percecionando-se atitudes mais favoráveis ao consumo. Dado o aumento da prevalência e as implicações precoce deste consumo, pretendeu-se compreender a relação entre as atitudes e o consumo de cannabis, junto de uma amostra de 164 estudantes do ensino geral e vocacional. Para isso, foram utilizados um questionário sociodemográfico e sobre a experiência de consumo, o Cannabis Use Intention Questionnaire e o Cannabis Abuse Screening Test. Foram encontradas diferenças estatisticamente significativas no consumo de cannabis (problemático/de risco e prevalências) e nas atitudes associadas (atitude, norma subjetiva, autoeficácia percebida e intenção), em função do sexo, idade e percurso escolar. Os estudantes de sexo masculino com idade mais avançada, com maior absentismo e insucesso escolares, do ensino vocacional, apresentam maior consumo, um consumo mais problemático/de risco e atitudes globalmente mais favoráveis ao mesmo. Quanto mais favoráveis são as atitudes face ao consumo de cannabis, maior é o consumo. Este estudo reforça a necessidade de promover competências pessoais e sociais, de forma precoce e contínua, e de desenvolver programas de prevenção direcionada e seletiva, centrados nas atitudes e significado, em contexto escolar.

Cannabis use is one of the most prominent risk behaviors in adolescence in recent years. Cannabis has been expanding and gaining prominence in youth culture in the face of a permissive social norm and more favorable attitudes towards its use. Given the increasing prevalence and the early implications of this use, this study intends to understand the relationship between attitudes and cannabis use, in a sample of 164 students from general and vocational education. For this, a sociodemographic questionnaire, a questionnaire about the consumption experience, the Cannabis Use Intention Questionnaire and the Cannabis Abuse Screening Test were used. There were statistically significant differences in cannabis use (problematic/risk and prevalence) and attitudes toward cannabis (attitude, subjective norm, perceived self-efficacy and intention), according to gender, age and school level. Older male students, with greater absenteeism and school failure, with higher vocational education, tend to have a larger and more problematic use of cannabis and present overall more favorable attitudes. The more favorable are attitudes towards use, the bigger is the use. This study reinforces the need to promote personal and social skills, early and continuously, and to develop programs of targeted and selective prevention, focused on attitudes and meaning, in the school context.

Resumo Em Portugal, a extensão dos serviços de educação especial à educação pré-escolar é relativamente recente. São também escassos os estudos que avaliam as percepções dos educadores de infância sobre as práticas inclusivas. Neste trabalho, pretendemos avaliar as percepções dos educadores de infância sobre a inclusão. Recorreu-se a uma amostra de 118 educadores de infância portugueses, a quem foi administrado o Inventário de Inclusão. Os resultados indicam que a implementação de práticas inclusivas está relacionada com a percepção de apoio e utilização anterior de práticas inclusivas. Educadores mais novos, com menos experiência letiva e com menos experiência na aplicação de práticas inclusivas possuem mais crenças sobre a inclusão. Educadores mais velhos e que trabalham mais tempo com crianças com necessidades educativas especiais percebem como melhores os efeitos da inclusão. Esses resultados são discutidos à luz da literatura existente, apresentando-se também implicações para a prática educativa e para a investigação.

Abstract In Portugal, special education services were recently extended to preschool education. The studies that evaluate the perceptions of the preschool teachers about inclusive practices are also few. In this study we aim to evaluate the perceptions of preschool teachers about inclusion. A sample of 118 Portuguese preschool teachers was used. The Inclusion Inventory was administrated. The results indicate that the implementation of inclusive practices is related to perceived support and previous use of inclusive practices. Younger teachers, with less teaching experience and less time implementing inclusive practices have more beliefs about inclusion. Older professionals and teachers who spend more time working with children with special needs perceive better effects of inclusion in children. These results are discussed considering the existing literature, and implications for educational practice and research are presented.

Resumen En Portugal, la extensión de los servicios de educación especial a la educación preescolar es relativamente reciente. También son pocos los estudios que evalúan las percepciones de los maestros de la educación preescolar en las prácticas inclusivas. En este trabajo nos proponemos evaluar las percepciones de los maestros de la educación preescolar acerca de la inclusión. Se utilizó una muestra de 118 educadores portugueses, los cuales respondieran al inventario de inclusión. Los resultados indican que las prácticas inclusivas están relacionadas con la percepción de apoyo y el uso previo de las prácticas inclusivas. Los educadores más jóvenes, con menos experiencia de trabajo y menos experiencia lectiva en la aplicación de prácticas inclusivas, tienen más creencias acerca de la inclusión. Educadores con más edad y con más tiempo de servicio con niños con necesidades educativas especiales perciben mejores efectos de su inclusión. Estos resultados se discuten a la luz de la literatura existente, presentando también implicaciones para la práctica educativa y la investigación.

Este trabalho pretende apresentar os resultados de adaptação portuguesa da Escala de Orientações Gerais para Objetivos de Desempenho (EOGOD; McInerney & Yeung, 2000). Trata-se de instrumento de medida com cinco escalas de motivação: Valorização da Motivação, Motivação Geral, Orientação para a Mestria, Orientação para o Desempenho e Orientação Social. Para a sua adaptação, foi realizada análise fatorial exploratória e confirmatória e análise de consistência interna a partir de dois grupos, de 617 e 418 estudantes do Ensino Secundário. Os dados obtidos permitiram encontrar uma solução com boas propriedades psicométricas, seja em termos de validade como fiabilidade aceitáveis, assim como diferenças em função do género e relação com a idade e escolaridade. Os resultados são discutidos e apresentadas sugestões.

This paper aims to present the results of the Portuguese adaptation of General Achievement Goal Orientation Scale (GAGOS; McInerney & Yeung, 2000). This instrument has five different motivation scales: Motivation Value, Global Motivation, General Mastery, General Performance and General Social. The work of adaptation consisted in a exploratory and confirmatory factor analysis and internal consistency analysis from the data of two samples of 617 and 418 High School students. The results suggest a solution with good psychometric properties, in terms of validity as acceptable fidelity, with differences per gender and relationship with age and education level. The results discussed and suggestions presented.

Caso clínico: Homem indigente, 48 anos, com antecedentes de tabagismo e etilismo crónico. Encaminhado ao SU um mês após queda com traumatismo do MIE. Ao EO apresentava extensa úlcera (25x15cm) na face antero-medial da perna esquerda, com tecido desvitalizado e sinais de infeção no leito, bem como exposição óssea significativa da diáfise da tíbia. Sem pulso femoral no membro esquerdo. O caso foi discutido com equipa de SU de cirurgia plástica tendo sido decidido internar o doente para posterior abordagem multidisciplinar. Angiografia diagnóstica demonstrou oclusão da artéria ilíaca externa e da artéria femoral superficial esquerdas. Procedeu-se à realização de bypass aorto-bifemoral com prolongamento à artéria poplítea supra-articular com veia grande safena. Cumpriu antibioterapia EV de largo espectro e cuidados de penso com hipoclorito e soro fisiológico, seguido de limpeza cirúrgica da ferida. Posteriormente, realizou-se plastia do defeito com retalho livre de músculo grande dorsal, seguido de enxerto de pele parcial. Alta após 10 dias. Atualmente doente assintomático, com cicatrização total da lesão. Conclusão: Em casos extremos, mesmo após revascularização eficaz, o grau de lesão tecidual pode não ser compatível com um processo de cicatrização favorável. Nestes casos, uma abordagem multidisciplinar constitui a única via para evitar a amputação. Este caso é um exemplo paradigmático onde a avaliação e intervenção conjuntas por cirurgia vascular e cirurgia plástica foram decisivas para salvar o membro de um homem de 48 anos.

Clinical case: A 48 years old indigent man, with history of smoking and chronic alcohol consumption. He was taken to the Hospital one month after leg trauma. Physical examination showed extensive ulcer (25x15cm) on the anteromedial aspect of the left leg, with important signs of infection, as well as significant bone exposure of the tibial shaft. No femoral or distal pulses in the affected limb. The case was discussed with the plastic surgery team and a multidisciplinary approach was decided. Diagnostic angiogram showed occlusion of the left external iliac and superficial femoral arteries. Patient was revascularized with aortobifemoral bypass with great saphenous vein extension to the popliteal artery. Following revascularization, there was a period wound dressing with hypochlorite and large spectrum antibiotics, followed by surgical wound cleaning. Finally, plasty of the clean injury with latissimus dorsi muscle free flap, followed by partial skin graft was performed. Patient was discharged 10 days after the plastic surgery. He remains asymptomatic, with complete healing of the injury, 6 months after hospital release. Conclusion: In extreme cases, even after successful revascularization, the degree of tissue injury may not be compatible with a favorable healing process. In these cases, a multidisciplinary approach is the only way to avoid amputation. This case is a classic example where the evaluation and joint intervention of vascular and plastic surgery were decisive for the limb salvage of a 48-year-old man.
