INTRODUÇÃO: a cefalometria pode ter uso clínico limitado em Ortodontia por demandar interpretação subjetiva. Um sistema de Inteligência Artificial (IA), o sistema Decodify®, foi desenvolvido para prover uma avaliação quantitativa individualizada de dados cefalométricos contextualizados. Nesse artigo, o sistema é testado como ferramenta administrativa em consultórios de Ortodontia. MÉTODOS: o desenvolvimento de algoritmos incluiu a modelagem de normas e desvios-padrão de dados cefalométricos brasileiros, medidos em radiografias laterais. Para testar o sistema, cefalogramas iniciais de 60 pacientes ortodônticos de dois consultórios diferentes (30 casos cada) foram processados e reprocessados por três técnicos diferentes. A reprodutibilidade e a confidencialidade intraexaminador e interexaminadores foram testadas de modo pareado. O risco em cada caso ortodôntico, produto da análise eletrônica, foi comparado por matrizes de covariância e coeficientes de confiança. RESULTADOS: os níveis de correlação pareada interexaminadores (contra padrão-ouro) para 23 pares de variáveis foram de 0,68 (distância S-Go) a 0,98 (distância Na-Me) em uma clínica ortodôntica (JU) e de 0,66 (ângulo L1.APg) a 0,98 (distância S-Go) em outra clínica ortodôntica (SP). Os níveis de correlação pareada intraexaminadores para 23 pares de variáveis foram de 0,52 (ângulo L1.APg) a 0,98 (distâncias S-Go e Na-Me) em uma clínica (JU) e de 0,55 (ângulo U1.Na) a 0,92 (distância Wits) em outra (SP). Todas as correlações foram significativas em nível de p<0,001. A média dos coeficientes de confiança dos riscos mensurados foi 0,78 para uma clínica (JU) e 0,75 para outra (SP). Os coeficientes de confiança foram significativos em nível de p<0,0001. CONCLUSÕES: os resultados dessa pesquisa suportam que as análises providas pelo sistema Decodify® são reprodutíveis e confiáveis. Assim, o sistema pode ser aplicado para contextualizar medidas cefalométricas convencionais e gerar índices de risco individualizados. O sistema pode ser usado pelo ortodontista como uma ferramenta administrativa em suas avaliações profissionais diárias.

INTRODUCTION: Cephalometrics may have limited clinical application in Orthodontics because it demands subjective interpretation. An artificial intelligence (A.I.) system, Decodify®, was developed to provide an individual quantitative interpretation of contextualized cephalometric data. In this paper the system is tested as a valid management tool in orthodontic offices. METHODS: The development of algorithms included modeling of norms and standard deviations of Brazilian cephalometric data measured on lateral radiographs. In order to test the system, initial cephalograms of 60 orthodontic patients from two orthodontic offices (30 cases each) were processed and re-processed by three different technicians. Reproducibility and reliability intra-observer and inter-observers were paired checked. The risk in each case, product of the electronic analysis, was compared by covariance matrices and reliability coefficients. RESULTS: Levels of inter-observers paired correlation (against golden pattern) for 23 pairs of variables, ranged from 0,68 (S-Go distance) to 0,98 (Na-Me distance) in an orthodontic clinic (JU) and from 0,66 (L1.APg angle) to 0,98 (S-Go distance) in other orthodontic clinic (SP). Levels of intra-observer correlation for 23 pairs of variables ranged from 0,52 (L1.APg angle) to 0,98 (S-Go and Na-Me distances) in an orthodontic clinic (JU) and from 0,55 (U1.Na angle) to 0,92 (Wits distance) in other (SP). All the correlations were significant at the p<0,001 level. The reliability coefficients of the assessed risks were 0,78 for a clinic (JU) and 0,75 for other clinic (SP). The reliability coefficients were significant at the p<0,0001 level. ? CONCLUSIONS: The results of this research support that the outcomes of the Decodify® system are reproducible and reliable. Therefore, the system can be applied to contextualize conventional cephalometric data and to generate case-specific risk. It can be used by the orthodontist as an important administrative tool for daily professional evaluations.

INTRODUÇÃO: avaliações paramétricas clássicas e variáveis cefalométricas isoladas podem não prover a melhor informação em morfologia craniofacial. Por outro lado, uma cefalometria contextualizada pode ser bem mais promissora, uma vez que permite a integração de variáveis cefalométricas ponderadas. OBJETIVO: a proposta principal desse artigo é apresentar a aplicação de um modelo matemático não-trivial em cefalometria, permitindo a mineração de dados através da filtragem de certeza e contradição em cada "nó" da rede. MÉTODOS: nessa "rede neural" proposta, cada "célula" é conectada a outras "células" através de "sinapses". Tal sistema de tomada de decisão é uma ferramenta de inteligência artificial ajustada para potencialmente aumentar o significado dos dados coletados. RESULTADOS: a comparação entre o diagnóstico final fornecido pela rede neural paraconsistente e as opiniões de três examinadores foi heterogênea. O índice de concordância Kappa foi regular para discrepâncias anteroposteriores; substancial ou regular para discrepâncias verticais; e moderado para discrepâncias dentárias. Para a protrusão bimaxilar dentária, a concordância foi quase perfeita. Similarmente, a concordância entre as opiniões dos três examinadores sem a ajuda de nenhuma ferramenta de diagnóstico foi apenas moderada para discrepâncias esqueletais e dentárias. Como exceção, a concordância para protrusão dentária foi quase perfeita. CONCLUSÕES: a avaliação de desempenho do sistema tecnológico desenvolvido suporta que a ferramenta eletrônica apresentada pode se igualar às decisões humanas na maioria das situações. Como uma limitação esperada, tal ferramenta matemática-computacional se apresentou menos efetiva para discrepâncias esqueletais do que para discrepâncias dentárias.

INTRODUCTION: Classical parametric assessments and isolated cephalometric variables may not provide the best information in craniofacial morphology. Rather, contextualized cephalometrics can be more promising, since it allows integration among balanced cephalometric variables. OBJECTIVE: The main purpose of this manuscript is to present the application of a non-trivial mathematical model in cephalometrics, providing data mining by filtering certainty and contradiction in each network "node". METHODS: In the proposed "neural network", each "cell" is connected to others "cells" by "synapses". Such decision-making system is an artificial intelligence tool tailored to potentially increase the meaning of assessed data. RESULTS: The comparison between the final diagnose provided by the paraconsistent neural network with the opinions of three examiners was heterogeneous. Kappa agreement was fair for antero-posterior discrepancies, substantial or fair for vertical discrepancies and moderate for dental discrepancies. For the bimaxillary dental protrusion, the agreement was almost perfect. Similarly, the agreement among the three examiners, without any software aid, was just moderate for skeletal and dental discrepancies. An exception was dental protrusion, which agreement was almost perfect. CONCLUSION: In conclusion, the analysis of performance of the developed technology supports that the presented electronic tool might match human decisions in the most of the events. As an expected limitation, such mathematical-computational tool was less effective for skeletal discrepancies than for dental discrepancies.

INTRODUÇÃO: tanto as avaliações profissionais quanto as avaliações de pessoas leigas não indicam um claro relacionamento entre as características de perfil duro e do perfil mole em pacientes ortodônticos. Por outro lado, há evidências concretas de que o ortodontista possa alterar o perfil facial de seus pacientes. OBJETIVOS: este trabalho visou mensurar a contribuição das características ósseas da maxila e a inclinação dos incisivos superiores à posição do lábio superior. MÉTODOS: uma amostra de 147 pacientes adultos, 58 homens e 89 mulheres, com idades de 15 a 49 anos, sendo a maioria caucasiana, foi retrospectivamente selecionada na clínica particular de um dos autores. Presumiu-se que o perfil estético facial depende dos tecidos ósseos que o suportam e de sua própria constituição, como espessura, tonicidade e composição. O modelo de pesquisa não havia sido desenhado para controlar as características intrínsecas do tecido mole. Para compor as linhas estéticas de Ricketts e de Burstone, as variáveis cefalométricas de maior interesse foram SNA e U1PP, quando simultaneamente avaliadas. RESULTADOS: os coeficientes de regressão, embora estatisticamente significativos, não contribuíram definitivamente para explicar as variáveis de interesse, as linhas estéticas pré-determinadas. Além disso, os resultados encontrados sugeriram uma correlação negativa entre a posição maxilar (SNA) e a inclinação anteroposterior dos incisivos superiores (U1PP), possivelmente devido às compensações impostas pelos lábios e pela língua. CONCLUSÕES: os resultados não apresentaram evidências científicas conclusivas sobre a contribuição dos tecidos duros para o perfil facial de tecido mole.

INTRODUCTION: Professional evaluations and lay person evaluations as well do not show a clear relationship between hard tissue and soft tissue profiles in orthodontic patients. However, there are concrete evidences that the orthodontist can change the facial profile of the patients. AIM: This research project aimed to measure the contribution of the bone structure of the maxilla and the upper incisors inclination to the upper lip profile. METHODS: A sample of 147 adult patients, 58 male and 89 female, aged from 15 to 49 years, the most of them Caucasians, was retrospectively selected from the private practice of one of the authors. It was assumed that the esthetic facial profile depends of the hard tissues that support it besides its own features as thickness, strength and composition. The present research model was not set to control the intrinsic features of the soft tissue. To compose the Ricketts and Burstone esthetic lines, the main cephalometric variables were SNA and U1PP, when concomitantly considered. RESULTS: The regression coefficients, although statistically significant, did not contributed expressively to explain the regard variables, the pre-defined esthetic lines. Furthermore, the results suggested a negative correlation between the maxillary position (SNA) and the anteroposterior inclination of the upper incisors (U1PP), possibly due to the compensatory action of the lips and the tongue. CONCLUSION: The results did not presented conclusive scientific evidences about the contribution of the hard tissues for the soft tissue facial profiles.

A literatura que descreve os resultados das correções cirúrgicas nos tecidos moles é limitada, pois apresenta significativa variação e não oferece conclusões definitivas. O propósito desse artigo é estudar retrospectivamente os efeitos das cirurgias ortognáticas maxilares tipo Le Fort I nos lábios superiores e inferiores de pacientes adultos e discutir os potenciais problemas envolvidos com a metodologia aplicada. Foram estudados 19 pacientes brancos de 19 a 42 anos de idade, sendo 10 homens e 9 mulheres, submetidos a cirurgias maxilares no Centro de Pesquisa e Tratamento das Deformidades Buco-Faciais - CEDEFACE (Araraquara, SP). A grande maioria dos casos apresentou, no mínimo, uma discrepância significativa, seja ântero-posterior ou vertical, reforçando a sua indicação cirúrgica. As alterações nos tecidos moles foram significativas quando as movimentações horizontais ou verticais da base óssea maxilar foram significativas, com uma relação de aproximadamente 0,6:1 entre as alterações ântero-posteriores do lábio superior e a movimentação da base óssea. Alterações verticais na maxila, sem significativas alterações horizontais, não foram suficientes para aumentar verticalmente o lábio superior. Ocorreram alterações ântero-posteriores do lábio inferior em conseqüência do deslocamento anterior e inferior da maxila, o que permitiu um melhor selamento labial. Alterações verticais não foram quantificadas no lábio inferior.

The literature has described that the predictability of the outcomes of orthognatic corrections in soft tissues is limited, because it shows significant variation and it does not offer final conclusions, limiting its clinical applications. The goal of this article is to retrospectively study the effects of maxillary orthognathic surgeries, Le Fort I type, in upper and lower lips of adult patients. Moreover, it intends to discuss the problems involved in this research area. The sample was constituted of 19 Caucasian adults, aging from 19 to 42 year-old, being10 male and 9 female, that undergone maxillary orthognathic surgery in CEDEFACE (Araraquara - SP). The vast majority of the cases presented a significant discrepancy, antero posterior, vertical or both, reinforcing its surgical recommendation. Significant changes in soft tissues were observed when skeletal changes, antero posterior or vertical, were significant. The proportion of approximately 0.6:1 soft to hard tissue changes was observed in the upper lip. Vertical changes in the maxilla without significant horizontal changes were not sufficient to vertically increase the upper lip size. Antero posterior changes in the lower lip as a consequence of the forward and downward maxillary relocation allow better lip posture. Vertical changes in the lower lip were not quantified.
