RESUMO A gagueira gera impactos negativos na qualidade de vida dos sujeitos e está associada a maior risco de desenvolvimento de problemas sociais e psíquicos. Nessa perspectiva, apesar da diversidade das hipóteses etiológicas e dos tratamentos da gagueira, destaca-se a interface entre psicologia e fonoaudiologia na abordagem dos aspectos bio-psíquicos envolvidos nesse quadro clínico. Pesquisas recentes indicam que a presença, especificamente de cães, pode auxiliar o paciente na elaboração simbólica de conteúdos psíquicos. Nessa perspectiva, o objetivo desse estudo é avaliar os efeitos da presença do cão na expressão de conteúdos psíquicos de um sujeito que gagueja, com a hipótese de que o enquadre pode reduzir o sintoma. O sujeito é M., sexo feminino, 45 anos, casada, sem filhos, cursou ensino fundamental completo e de auxiliar de cabeleireira. Passou pelo processo de terapia fonoaudiológica na presença do cão. Foi realizada uma entrevista semi-dirigida após o processo. Um cão coterapeuta, da raça Golden Retriever, participou de todas as sessões. Alguns conteúdos subjetivos relevantes observados no setting no decorrer do processo terapêutico fonoaudiológico, os quais parecem ter sido mobilizados pelo enquadre estabelecido pela interação entre terapeuta-paciente-cão, parecem demonstrar associação com a manifestação das disfluências. O cão fez contato físico, deu suporte, motivou e acolheu o sujeito em situações de demonstração de conflitos psíquicos. Assim, este estudo de caso clínico indica que o enquadre da presença e interação do cão favoreceu a redução do sintoma da gagueira, promovendo ambiente acolhedor possibilitando a integração psique-soma do sujeito.

ABSTRACT Stuttering has negative impacts on an individual’s quality of life and is associated with higher risk of development of social and psychological problems. From this perspective, despite the diversity of etiological hypotheses for and treatments of stuttering, the interface between psychology and speech therapy in the approach to the biopsychic aspects involved in this clinical scenario stands out. Recent research indicates that the presence, specifically of dogs, can assist patients in symbolic elaboration of psychic content. From this perspective, the aim of this study is evaluate the effects of a dog's presence on the expression of the psychic content of a stuttering person, with the hypothesis that framing may reduce symptoms. The subject is M., female, 45 years old, married, without children, hairdressing assistant, with elementary school completed. She underwent the process of speech therapy in presence of a dog. A semidirected interview was conducted after this process. A co-therapist dog of Golden Retriever breed participated in all sessions. The relevant subjective content observed in the setting during the speech therapy process, which seems to have been mobilized by the framework established by the interaction among the therapist, the patient, and the dog, seems to demonstrate an association with the manifestation of disfluencies. The dog made physical contact with, supported, motivated and welcomed the subject in situations in which psychic conflicts were demonstrated. This clinical case study indicates that the dog's presence and interaction framework favored the reduction of stuttering symptoms, promoting welcoming environment that enabled the subject's psyche-soma integration.

RESUMO Objetivo Investigar a percepção auditiva temporal, a orientação e a mobilidade de deficientes visuais. Métodos Estudo exploratório de intervenção, com amostra de conveniência composta por nove sujeitos cegos ou com baixa visão, com idades entre 23 e 64 anos, usuários de cão-guia e que realizaram os seguintes procedimentos: avaliação funcional de orientação e mobilidade, avaliação audiológica básica (audiometria tonal limiar, pesquisa do índice de reconhecimento de fala), avaliação do processamento auditivo temporal (teste de resolução temporal (random gap detection test - RGDT), teste de padrão de frequência (TPF), teste dicótico de dígitos (TDD), teste rápido de sentenças no ruído (TRIS). Resultados Todos os sujeitos apresentavam limiar audiométrico de grau normal, porém, um deles apresentava curva audiométrica do tipo neurossensorial e configuração descendente, sugerindo perda auditiva relacionada à idade. A avaliação das habilidades auditivas temporais mostrou valores dentro dos padrões de normalidade para todos os sujeitos, com exceção do teste RGDT, em que os sujeitos apresentaram valores menores (3,5 a 7,5 ms) do que os esperados para população de adultos com visão e audição normais. A avaliação da OM mostrou que todos os sujeitos necessitavam de adaptações técnicas, pois falhas foram observadas no percurso por eles executado. Conclusão A percepção auditiva temporal dos sujeitos deste estudo está adequada e a avaliação da OM mostra falhas que necessitam ser corrigidas.

ABSTRACT Purpose To investigate the temporal auditory perception, and OM of visually impaired people. Methods This is an exploratory intervention study including a convenience sample composed of nine blind or low vision subjects, aged between 23 and 64 years, guide dog users and who performed the following procedures: functional evaluation of OM, basic audiological evaluation (Pure tone audiometry, speech recognition index), temporal auditory processing assessment (random gap detection test (RGDT), frequency pattern test (FPT), dichotic digit Test (DDT), quick speech-in-noise test (QuickSIN). Results Although all subjects had normal-level audiometric thresholds, one subject had a sensorineural audiometric curve and descending configuration, suggesting an age-related hearing loss. Evaluation of the temporal auditory processing showed values within the normal for all subjects, except for the RGDT, in which test subjects had lower (3.5 to 7.5 ms.) than the expected values for the adult population with normal vision and hearing. Evaluation of OM showed that all subjects require technical adjustments as problems were observed in the route taken. Conclusion The temporal auditory processing of these subjects is appropriate and the evaluation of OM shows flaws that need to be corrected.

RESUMO Objetivo Verificar o efeito da intervenção do cão na sessão regular de terapia fonoaudiológica para a gagueira do desenvolvimento em jovens adultos. Método Foram selecionados jovens adultos e adultos com diagnóstico de gagueira do desenvolvimento. Concluíram o estudo oito participantes, seis do sexo masculino e dois do sexo feminino, com idades variando entre 16 e 45 anos. Os participantes foram divididos em dois grupos: G1 – que realizou o tratamento para gagueira com a presença de um cão terapeuta em sala de terapia e G2 – que realizou o tratamento para gagueira sem a presença do cão terapeuta. Foi incluído um grupo controle, G3 – composto por participantes fluentes, pareado em idade e sexo ao G1 e G2, para controle da variabilidade natural da fluência da fala. Resultados A análise comparativa entre os grupos indicou que o grupo que realizou o tratamento sem a presença do cão alcançou melhores índices de performance, evolução e eficácia. Conclusão Para a população pesquisada neste estudo, não foi observado efeito da intervenção do cão na terapia fonoaudiológica.

ABSTRACT Purpose To verify the effect of dog intervention on the regular session of speech therapy for developmental stuttering in adults. Methods The study involved young adults and adults with developmental stuttering. The study sample was composed of eight participants, six males and two females, ranging in age from 16 to 45 years. Participants were divided into two groups: G1 – those who underwent treatment for stuttering with the presence of a dog-therapist in the therapy room and G2 – those who underwent treatment for stuttering without the presence of the dog therapist. We included a control group, G3, composed of fluent participants, matched in age and sex to G1 and G2 to control the natural variability of speech fluency. Results Comparative results between the groups indicated that the group that performed the treatment without the presence of the dog achieved better performance, evolution and efficacy rates. Conclusion for the population in this study, the dog intervention on speech therapy did not improve treatment.

RESUMO Objetivo Avaliar a saturação de oxigênio (SatO2), a frequência cardíaca (FC), o tempo de internação e o peso dos recém-nascidos pré-termos (RNPTs) em Unidade de Terapia Intensiva Neonatal, na oferta de dieta pelas técnicas de alimentação via copo e sonda-dedo, simultaneamente ao aleitamento materno. Método Ensaio clínico randomizado simultâneo. Foram selecionados 25 prematuros internados na Unidade de Terapia Intensiva Neonatal de hospital público, no período de outubro de 2011 a fevereiro de 2012. A amostra foi dividida em dois grupos: 8 prematuros nascidos em dia par, que receberam a dieta no copo (GCP) e 17 prematuros, nascidos em dia ímpar, que receberam a dieta pela sonda-dedo (GSD). Na oferta da dieta foram anotados os valores mínimos e máximos da SatO2 e FC, antes de oferecer a dieta, durante e após a oferta. Resultados Quanto às variáveis SatO2 e FC, não foram observadas diferenças estatisticamente significativas entre os grupos, mas, no fator grupo versus tempo, os grupos apresentaram diferenças, não contínuas na variável SatO2. Em relação ao peso, foi constatado ganho estatisticamente significativo para ambos os grupos, sendo que, no GCP, o maior ganho de peso foi por causa do maior tempo de internação. Foi verificado que o GSD apresentou menor tempo de internação. Conclusão Não houve diferenças quanto à SatO2 e FC. Contudo, ao se analisar o fator tempo, os grupos apresentaram algumas diferenças, não contínuas, o que indica a necessidade de outros estudos para melhor compreensão do efeito. O GSD apresentou menos tempo de internação e os RNPTs do GCP tiveram maior ganho de peso em razão do maior tempo de internação.

ABSTRACT Purpose To evaluate the oxygen saturation, heart rate, length of hospital stay and weight preterm infants or preterm newborns (PTNBs) (in the Neonatal Intensive Care Unit in the diet supply by cup and finger feeding techniques, simultaneously with breastfeeding. Methods Simultaneous randomized clinical trial. Twenty-five preterm infants admitted to the Neonatal Intensive Care Unit of the Public Hospital from October 2011 to February 2012 were selected. The sample was divided into two groups: Eight preterm infants who received the diet in the cup probe group (CPG) who were born on the same day, and 17 by finger probe group (FPG) who were born on the odd day. In the diet offer, the minimum and maximum values of oxygen saturation (O2 Sat) and heart rate (HR) were recorded: before offering the diet, during and after the offer. Results Regarding the variables O2 Sat and HR, no statistically significant differences were observed between the groups, but in the group vs time factor, the groups showed differences, not continuous in the O2 Sat variable. Regarding weight, a statistically significant gain was observed for both groups, and in CPG the highest weight gain was due to the longer hospitalization time. It was verified that FPG presented shorter hospitalization time. Conclusion There were no differences regarding O2 Sat and HR. However, when analyzing the time factor, the groups presented some differences, not continuous, indicating the need for other studies for a better understanding of the effect. The FPG presented shorter hospitalization time and the CPG infants had greater weight gain due to longer hospitalization time.

RESUMO Objetivo Investigar a sensibilidade e consistência interna do instrumento a partir da comparação com os resultados dos instrumentos de avaliação funcional facial, escala de House-Brackmann (HBS) e Sistema de Graduação Facial, e implicações psicossociais a partir da aplicação da Escala Hospitalar de Ansiedade e Depressão (HADS). Método Pesquisa aprovada pelo comitê de ética em pesquisa, sob o protocolo nº. 196.977 e 230.982. A avaliação da sensibilidade do questionário foi realizada por meio de entrevistas fechadas em sujeitos adultos com PFP, sendo 38 selecionados para essa etapa. A análise estatística foi realizada para cada uma das etapas deste estudo, os dados foram digitados em Excel®, analisados pelos programas SPSS versão 17.0 para Windows e AMOS versão 22.0 para Windows®. Resultados Participaram 38 sujeitos, entre 19 e 78 anos, com predominância de paralisia idiopática (44,7%). Os resultados do Alfa de Cronbach mostraram uma consistência interna forte entre os grupos temáticos e as questões, no entanto a análise fatorial confirmatória alerta para questões cuja relação de causa entre os grupos temáticos foi fraca, como nos casos das questões 5 e 6 do grupo temático Aspectos Funcionais da Face, questão 17 dos Aspectos Sociais e questão 23 dos Aspectos Emocionais. Conclusão Essa pesquisa constituiu os primeiros passos para o subsídio e respaldo de um instrumento que investiga os aspectos psicossociais associados à PFP, sendo possível a elaboração de questões e ordenação em grupos temáticos. Porém, faz-se necessária a continuidade de estudos para a efetivação dos processos de validação.

ABSTRACT Purpose To investigate the sensitivity and internal consistency of the Psychosocial Scale of Facial Appearance (PSFA) based on the comparison between its results and those from other facial functional assessment instruments: House-Brackmann scale (HBS) and Facial Grading System - and the psychosocial implications measured by the Hospital Anxiety and Depression Scale (HADS). Methods The study was approved by the Research Ethics Committee of the aforementioned Institution under protocols no. 196.977 and 230.982. Thirty-eight adult individuals with Peripheral Facial Palsy (PFP) were submitted to closed interviews in order to evaluate the sensitivity of the questionnaire. Statistical analyses were conducted for each stage of this study. Data were entered in Excel® spreadsheet and analyzed using SPSS 17.0 and AMOS 22.0 for Windows®. Results Study participants were 38 individuals with PFP aged 19-78 years with predominance of idiopathic paralysis (44.7%). Results of the Cronbach's Alpha coefficient showed strong internal consistency between the thematic groups and the questions; however, Confirmatory Factor Analysis indicated some questions with week causal relationship between thematic groups, namely, questions 5 and 6 of the group Functional Aspects of Face, question 17 of the group Social Aspects, and question 23 of the group Emotional Aspects. Conclusion This study provided the first steps for the subsidy and support of an instrument designed to investigate the psychosocial aspects associated with PFP, enabling the preparation of questions and their organization into thematic groups. However, further studies are needed to conclude the validation processes.

RESUMO Objetivo Desenvolver e validar o conteúdo de instrumento de avaliação de linguagem pautado na Comunicação Suplementar e Alternativa (CSA). Métodos O instrumento foi elaborado a partir de revisão bibliográfica, em conjunto com a experiência clínica da pesquisadora. Em seguida, foi avaliado por juízes especialistas na área e uma nova versão foi construída, incorporando as colaborações dos juízes. Resultados O instrumento foi elaborado em 4 eixos de conteúdo, com instruções e sugestão de contextos e símbolos gráficos. A partir da avaliação dos juízes, o instrumento foi aprimorado e proposto, em sua versão final. Conclusão O instrumento denominado CSA_Linguagem se mostrou operacional, de fácil e rápida aplicação e baixo custo. Contudo, esta pesquisa tem limitações, na medida em que não há instrumentos de avaliação de linguagem padronizados, que considerem a utilização de símbolos gráficos como possibilidade de comunicação.

ABSTRACT Purpose Content development and validation of a language assessment instrument lined on Augmentative and Alternative Communication (AAC). Methods The instrument was elaborated from a bibliographic review, along with the clinical experience of the researcher. Then, It was evaluated by expert judges in the area and a new final version was constructed, with computed collaboration. Results The Instrument was developed in four content axes, with instructions and suggestion of contexts and graphic symbols. Based on the evaluation of judges, the instrument was improved and the final version was proposed. Conclusion The instrument, named CSA_Linguagem, was proved to be operational, easy, of rapid application and low cost. However, this research has limitations, there are no standardized language assessment tools that consider the use of graphic symbols as communication possibilities.

RESUMO JUSTIFICATIVA E OBJETIVOS: Cresce o interesse sobre Atividade, Educação e Terapia Assistida por Animais no Brasil. Somente na última década, pesquisadores brasileiros iniciaram investigações científicas com vistas a avaliar a efetividade do trabalho com Atividade, Educação e Terapia Assistida por Animais, a partir de referências da literatura internacional. A opção metodológica de desenvolver esse projeto em ambiente hospitalar vem ao encontro do interesse crescente dessas instituições em introduzir os animais durante o período de hospitalização dos pacientes, especialmente as crianças. A presença dos animais parece produzir benefícios sócioemocionais significativos como adjuvante ao tratamento clínico de pacientes hospitalizados. O objetivo deste estudo foi avaliar os efeitos da Atividade Assistida por Animais na sensação de dor em crianças e adolescentes hospitalizados. MÉTODOS: Trata-se de pesquisa de intervenção, de natureza quantitativa e qualitativa. Participaram do estudo 17 pacientes a partir de 7 anos de idade, hospitalizados no setor de internação do Hospital Infantil Sabará, com queixa de dor. Os cães coterapeutas participantes foram Bruce (porte grande) e Sheep (porte pequeno). A escala numérica de dor foi aplicada antes e depois da Atividade, Educação e Terapia Assistida por Animais. RESULTADOS: Na população estudada, há evidências de que a esta atividade demonstrou eficácia quanto à redução da dor autorreferida pelos pacientes, além de melhorar aspectos emocionais sobre a hospitalização, corroborando dados bibliográficos de pesquisas realizadas sobre o tema. CONCLUSÃO: Houve diminuição significativa da sensação de dor após a intervenção com o cão, corroborando com alguns estudos internacionais já realizados sobre o tema.

ABSTRACT BACKGROUND AND OBJECTIVES: The interest on Animal-Assisted Activity, Education and Therapy is growing in Brazil. Only in the last decade, Brazilian investigators have started scientific studies aiming at evaluating the effectiveness of Animal-Assisted Activity, Education and Therapy as from international literature references. The methodological option of developing this project in hospital environment comes to meet the increasing interest of such institutions in introducing animals during the hospitalization period of patients, especially children. The presence of animals seems to produce significant socioemotional benefits as adjuvant to clinical management of hospitalized patients. This study aimed at evaluating the effects of Animal-Assisted Activity on pain sensation of hospitalized children and adolescents. METHODS: This is an intervention, quantitative and qualitative research. Participated in the study 17 patients aged as from 7 years, hospitalized in the Hospital Infantil Sabará, with pain complaints. Participant co-therapist dogs were Bruce (large size) and Sheep (small size). Pain numerical scale was applied before and after Animal-Assisted Activity, Education and Therapy. RESULTS: There are evidences, in the studied population, that this activity was effective to decrease patients' self-referred pain, in addition to improving emotional aspects of hospitalization, confirming literature data on the subject. CONCLUSION: There has been significant pain decrease after intervention with dogs, confirming some international studies carried out on the subject.

RESUMO Poucos são os estudos fonoaudiológicos brasileiros sobre pacientes com o diagnóstico de doenças da cadeia respiratória mitocondrial. Esse quadro clínico é uma das doenças genéticas do metabolismo mais frequentes e apresenta sintomas que demandam intervenções fonoaudiológicas (alterações miofuncionais orais, auditivas e dificuldades na aquisição da linguagem oral). Neste estudo, articula-se a possibilidade de trabalho com esses sujeitos na abordagem da comunicação suplementar e alternativa. Objetivou-se descrever os resultados da utilização da comunicação suplementar e alternativa no atendimento de uma criança com doença mitocondrial. Os procedimentos terapêuticos enfatizaram o uso social da linguagem por meio de atividades significativas e contextualizadas, com o apoio de fotos e de figuras do Picture Communication Symbols de atividades cotidianas. Os resultados obtidos apontaram aumento na funcionalidade da linguagem oral do sujeito, ao favorecer suas práticas discursivas, especialmente quanto à intenção comunicativa no contexto interacional.

ABSTRACT There are few Brazilian speech-language pathology studies about patients diagnosed with mitochondrial disease. This is one of the most prevalent inherited metabolic diseases, and its clinical presentation usually includes symptoms that demand speech-language pathology interventions (myofunctional disorders and spoken language acquisition difficulties). This paper explores the possibility of employing a therapeutic approach to this disorder based on augmentative and alternative communication, by describing the results of adopting this approach when working with a child diagnosed with mitochondrial disease. The therapeutic proceedings focused on the social use of language through meaningful and contextualized activities, with the help of photos and pictures from a communication system (Picture Communications Symbols - PCS) illustrating daily activities. The results showed an increase in the functionality of the child's oral language by benefitting his discursive practices, particularly in regard to communicative intention within the interactional context.

Abstract Animal-assisted intervention (AAI) is an approach recently introduced into the hospital environment to improve the quality of hospitalization and provide important benefits for patients with chronic diseases and long-term hospitalizations. This work aims to verify the effects of animal-assisted activity (AAA) on the expression and quality of self-reported pain in hospitalized children and adolescents, while considering the subjects’ subjectivity. The participants were 17 hospitalized children/adolescents of both genders, aged 7 years and older, who complained of pain. Two therapy dogs were selected for the intervention according to the criteria of international protocols. The participants were asked an open question (“How would you describe your pain?”). After the question, an AAA session, which lasted between 5 and 10 min, was held with random activities spontaneously chosen by the subject. The open question was asked again at the end of the session, without the presence of the dog. Positive effects were observed in this population with regard to a decrease in self-reported pain. These results suggest that there is a possible symbolic elaboration of pain by the subject, in which the dog might represent acceptance and affection at a moment of great emotional suffering.

RESUMO Objetivo Analisar as respostas parentais a perguntas que investigam sinais clássicos de autismo, em dois instrumentos diferentes: Questionário de Indicadores de Risco para o Desenvolvimento Infantil (IRDI-questionário) e Modified Checklist for Autism in Toddlers (M-Chat). Métodos Quarenta e uma crianças, sendo 80% meninos, com média de idade de 2 anos e 8 meses foram avaliadas com dois instrumentos de rastreamento de autismo, recomendados pelo Ministério da Saúde. Após a aplicação integral dos instrumentos, foram selecionadas sete perguntas que compõem os instrumentos e ilustram emblematicamente sinais clássicos de transtorno do espectro do autismo (TEA), respondidas pelos pais dos sujeitos, para posterior análise. As crianças avaliadas não tinham qualquer diagnóstico fechado de TEA ou outros transtornos. Resultados Os principais preditores de importância foram questões sobre brincar de “faz de conta”, interesse da criança por outras crianças, resposta da criança ao “manhês” e troca de olhares entre mãe e criança. Conclusão Nem todas as perguntas que abordam os sinais típicos de autismo mostraram-se bons preditores de importância na análise realizada. Há necessidade de analisar o conjunto de sinais e não apenas sinais isolados, quando se está diante de uma criança com suspeita de TEA.

ABSTRACT Purpose To analyze the parental responses to questions investigating classic signs of autism using two different instruments (IRDI-questionnaire and M-Chat). Methods Forty-one children, 80% male, with a mean age of 2 years and 8 months, who were evaluated with two autism screening tools recommended by the Brazilian Ministry of Health. After administration of both tools, seven questions were selected to make up the instruments and illustrate symbolically the classic signs of the Autistic Spectrum Disorder (ASD), answered by the parents of the subjects for further analysis. The subjects did not have a formal diagnosis of ASD, or any other diagnosis. Results The main predictors of importance were questions about “make-believe” playing, interest of the child in other children, the child’s response to “motherese”, and exchange of glances between mother and child. Conclusion Not all questions referring to the typical signs of autism showed to be good predictors of importance in the conducted analysis. The data lead us to reflect on a need to analyze a set of signs and not only isolated signs when facing a child with suspected ASD.

RESUMO Objetivo Aplicar a Escala Psicossocial de Aparência Facial, para verificar aplicabilidade e reprodutibilidade, por meio de estudo piloto. Métodos Casuística: seleção de oito sujeitos adultos com paralisia facial periférica, a partir de critérios estabelecidos. Procedimentos: 1. avaliação fonoaudiológica da função facial; 2. verificação da fidedignidade da Escala Psicossocial de Aparência Facial; 3. verificação da reprodutibilidade da escala; 4. aplicação da Escala Hospitalar de Ansiedade e Depressão. Foi realizada análise descritiva dos dados e aplicado o teste t-Student pareado, entre os momentos 1 e 2, e o coeficiente de correlação intraclasse (ricc). Resultados A verificação da aplicabilidade aumentou a familiaridade com o processo de coleta de dados e auxiliou nas modificações dos procedimentos. O questionário mostrou-se fidedigno para todas as escalas analisadas: Aspectos Funcionais da Face, Aspectos Sociais, Aspectos Emocionais, Geral e Nota atribuída ao rosto. O coeficiente de correlação intraclasse (ricc) demonstrou a reprodutibilidade do instrumento, tendo apresentado, neste caso, valores excelentes para todas os grupos temáticos analisados. Conclusão Esta pesquisa ofereceu subsídios para o aprimoramento e elaboração final de instrumento que investiga os aspectos psicossociais associados à paralisia facial periférica.

ABSTRACT Purpose To apply the Psychosocial Scale of Facial Appearance in order to verify its applicability and reproducibility in a pilot study. Methods Subjects: eight adult subjects with peripheral facial palsy were selected according to established criteria. Procedures: 1. speech-language assessment of the facial function; 2. verification of the reliability of the Psychosocial Scale of Facial Appearance; 3. verification of the scale’s reproducibility; 4. application of the Hospital Anxiety and Depression Scale. A descriptive analysis of the data was performed, and the paired t-Student test (between times 1 and 2) and the intraclass correlation coefficient (ricc) were applied. Results Assessing the applicability increased the familiarity with the data collection process and contributed to modifications in the procedures. The questionnaire proved to be reliable for all the scales analyzed: Functional, Social, and Emotional Aspects of Face, General, and Note Assigned to the Face. The intraclass correlation coefficient (ricc) demonstrated the reproducibility of the instrument by showing excellent values for all thematic groups analyzed. Conclusion This study provided subsides for the improvement and final elaboration of an instrument that aims to investigate the psychosocial aspects associated with peripheral facial palsy.

Resumo: A Síndrome de Werdnig-Hoffman é uma doença neuromuscular hereditária, caracterizada pela atrofia e fraqueza muscular progressiva, que inviabiliza o desenvolvimento de habilidades motoras. O desenvolvimento mental encontra-se preservado, vivacidade e inteligência destacam-se, em contraste à precária atividade motora. O sujeito acometido por essa síndrome tem limitações físicas para a efetiva interação com o outro, o que pode acarretar problemas de comunicação. Este estudo tem como objetivo analisar as possibilidades comunicativas de uma criança com Síndrome de Werdnig-Hoffman no decorrer do processo terapêutico fonoaudiológico. É um estudo de caso de uma criança com quatro anos de idade, gênero feminino, filha única (de casal jovem), atendida em terapia fonoaudiológica em centro de reabilitação de deficiências múltiplas no período de agosto de 2013 a abril de 2014. Na avaliação fonoaudiológica observou-se que a paciente comunica-se por expressões faciais e raras vocalizações. No decorrer do processo terapêutico, a interação foi sustentada dialogicamente por meio de perguntas da fonoaudióloga respondidas afirmativa ou negativamente pela paciente. Para "sim", elevava a sobrancelha concomitante à maior abertura dos olhos e um discreto sorriso. Para "não", contraia a sobrancelha e realizava leve movimento de negação com a cabeça. Embora predominantemente silente e corporalmente inerte, a paciente expressa sua subjetividade por meio de condutas comunicativas que, mesmo precárias, indicam atividade simbólica na inserção e interpretação da realidade.

Abstract: Werdnig-Hoffman Syndrome (WHS) is a hereditary neuromuscular disorder characterized by progressive muscle weakness and atrophy, which prevents motor skills development. Mental development is preserved, vivacity and intelligence stand out in contrast to poor motor activity. The subject affected by this syndrome, has physical limitations for effective interaction, which can cause communication problems. The purpose of this study is to analyze the communicative possibilities of a child with Werdnig-Hoffman Syndrome during the speech and language therapeutic process. It is a case study of four years old female child, female, attended speech and language therapy service in a multiple disabilities rehabilitation center, from August 2013 to April 2014. The clinical assessment showed that the patient communicated by facial expressions and rare vocalizations. During the therapeutic process, the interaction was dialogically sustained through responses to yes/no questions: raising the eyebrow concomitant to eyes wider open for a "yes" and eyebrow contraction for "no", with light denial head movement. Though predominantly silent and motionless, the patient expressed her subjectivity by means of a communicative conduct that although incipient indicated symbolic activity related to integration and interpretation of reality.

Objetivo Avaliar a sensibilidade do IRDI-questionário para pais, para rastreamento de transtornos do espectro do autismo. Métodos Participaram do estudo 72 sujeitos, familiares de crianças na faixa etária de 2 anos e 11 meses a 7 anos e 7 meses, divididos em dois grupos: grupo pesquisa e grupo controle. O IRDI-questionário foi aplicado nos dois grupos estudados e a CARS-BR foi aplicada no grupo pesquisa. Os dados foram submetidos à análise estatística e, para a validade, foi realizado o coeficiente de correlação de Pearson, entre o IRDI-questionário e a CARS-BR, somente para o grupo pesquisa. Na definição do ponto de corte do escore gerado pelo instrumento IRDI-questionário, foi utilizada a curva ROC e calculados os valores de sensibilidade, especificidade e valor preditivo positivo. Na análise de concordância para o IRDI-questionário, foi utilizado o coeficiente de Kappa e concordância total. Resultados O IRDI-questionário mostrou-se de fácil aplicação, rapidez no preenchimento e baixo custo. A análise da consistência interna das questões do instrumento apresentou bom índice. Na comparação com a CARS-BR, apresentou correlação positiva. A análise da curva ROC identificou bom desempenho. Conclusão A avaliação da sensibilidade do instrumento, para fins de rastreamento de transtornos do espectro do autismo, mostrou bons índices, a partir do ponto de corte estabelecido no estudo.

Purpose To assess the sensitivity of the Clinical Risk Indicators for Early Childhood Development (IRDI) questionnaire for parents in identifying autism spectrum disorders. Methods Seventy-two subjects, which comprised parents of children aged 2 years 11 months to 7 years 7 months, participated in the study divided into two groups, namely the study and control groups. The IRDI questionnaire was administered in both groups, and the CARS-BR was applied in the study group. The data were subjected to statistical analysis, and the Pearson correlation coefficient between the IRDI questionnaire and CARS-BR was performed only for the study group to calculate validity. In defining the cutoff score generated by the IRDI questionnaire, the receiver-operating characteristic (ROC) curve was used, and sensitivity, specificity, and positive predictive value were calculated. In the reliability analysis for the IRDI questionnaire, the Kappa coefficient and overall concordance was used. Results The IRDI questionnaire was easy to use, quick to complete, and inexpensive. Analysis of the internal consistency of the questions of the instruments presented good content. The IRDI questionnaire was positively correlated with the CARS-BR. The ROC curve analysis revealed good performance. Conclusion The evaluation of the sensitivity of the instrument for purposes of identifying autism spectrum disorders showed good performance according to the established cutoff point in the study.

Objetivo:Conhecer a opinião das pessoas que gaguejam sobre os tratamentos da gagueira, a partir de resposta a duas questões abertas.Métodos:Participaram do estudo 40 adultos, autoavaliados como gagos, sem distinção de gênero. Na primeira fase da pesquisa, foram feitos contatos com duas Organizações Não Governamentais brasileiras de apoio às pessoas com gagueira: Associação Brasileira de Gagueira (ABRAGAGUEIRA) e Instituto Brasileiro de Fluência (IBF), que se dispuseram a participar do estudo divulgando a proposta para seus membros via correio eletrônico. O primeiro contato com os participantes esclareceu o objetivo e método da pesquisa e, após obtenção da concordância na participação da mesma, foi enviada nova correspondência contendo duas questões a serem respondidas, que envolviam opiniões sobre cura, tratamentos realizados e seus resultados.Resultados:As respostas obtidas indicaram que pessoas que gaguejam acreditam na cura da gagueira; a terapia ideal seria a que levasse ao desaparecimento do sintoma e que o tratamento mais indicado é o fonoaudiológico. Os resultados indicaram ainda que, para a maioria dos participantes, embora já tenham realizado tratamento fonoaudiológico, não foi observada melhora significativa do quadro e nem satisfação positiva com o tratamento realizado.Conclusão:Os resultados indicaram que os participantes, ao emitirem suas respostas, não se basearam no conhecimento científico do distúrbio e sim no desejo de que "algo possa acontecer" e a gagueira venha a "desaparecer". Embora a pesquisa não tenha focado no tipo de terapia realizada, os resultados indicaram que a determinação dos componentes que contribuem para um tratamento eficaz para gagueira são controversos.

Purpose:To investigate the opinion about stuttering treatments in people who stutter, based on their answers to two open questions.Methods:The participants were 40 adults of both genders, with self-reported stuttering. During the first phase of the research, we contacted two Brazilian nongovernmental organizations: the Brazilian Stuttering Association (ABRAGAGUEIRA) and the Brazilian Fluency Institute (IBF). These associations agreed to participate and were responsible for sending the research questions to their members via electronic mail. The first contact with the participants elucidated the purpose and method of our research and, after obtaining informed consent from participants, the two questions were sent. The research questions involved their opinion about cure, treatments to which the participants had been submitted, and their outcome.Results:After analysis, the answers obtained indicated that people who stutter believe in a cure for stuttering; that the ideal therapy would be the one that led to the disappearance of the symptoms; and the most frequently reported professional to treat the disorder is the speech-language pathologist. The results also indicated that although most of the participants had undergone speech-language treatment for stuttering, neither significant improvements were observed nor satisfaction was positive.Conclusion:The results indicate that the answers presented by the participants were not based on scientific knowledge about the disorder but on their wish that "something could happen" to make stuttering "disappear." Although in this study we did not investigate the type of treatment to which the patients were submitted, the results suggest that the factors that contribute to an effective treatment are contentious.
