Osteochondromas are the most common benign tumor of the bone. They are sometimes responsible for vascular complications involving either veins or arteries, principally around the knee. Pseudoaneurysms are considered a rare condition. The authors describe the occurrence of a pseudoaneurysm of the popliteal artery in association with a femoral osteochondroma in a 30-years-old man.

Paciente do sexo feminino, 73 anos, negra, lavadeira/lavradora, hipertensa, apresentava tumor pulsátil de 1,5 x 0,5 cm em região de tabaqueira anatômica da mão direita há 10 anos, de crescimento lento e progressivo, associado a dor local. Não apresentava alterações neurológicas, cianose de extremidades, sinais de infecção ou trauma local. O teste de Allen resultou negativo, e o Eco-Doppler colorido demonstrou aneurisma de artéria radial na tabaqueira anatômica. Procedeu-se a aneurismectomia de artéria radial na tabaqueira anatômica com ligadura dupla proximal e distal. Houve boa evolução operatória, sem sinais de isquemia digital. O exame anatomopatológico confirmou diagnóstico de parede arterial (aneurisma verdadeiro). O paciente encontra-se em acompanhamento ambulatorial, no momento assintomático. Não há definição de quais aneurismas distais à artéria axilar possam ser acompanhados sem conduta cirúrgica. Como a paciente, neste caso, apresentava teste de Allen normal e dor local, optou-se pela ligadura proximal e distal, com bom resultado.

We report a case of a 73-year-old female, laundress/farmer, hypertensive, with symptomatic pulsatile lump of 1.5 x 0.5 cm at the right anatomical snuffbox of the right hand, with slow and progressive growth and local pain in the past 10 years. The patient presented no neurological symptoms, cyanosis, infection or local trauma history. Allen"s test was negative, and color Doppler ultrasound confirmed presence of radial artery aneurysm in the anatomical snuffbox. Surgical treatment via aneurysmectomy of radial artery in the snuffbox with proximal and distal ligation was carried out. She had good operative course with no signs of digital ischemia. Histopathological test confirmed diagnosis of arterial wall (true aneurysm). She remains asymptomatic in outpatient follow-up. There is no definition as to which aneurysms distal to the axillary artery could be followed without a surgical procedure. As the patients had normal Allen"s test and local pain, proximal and distal ligation was chosen and had good outcome.

A síndrome do aprisionamento da artéria poplítea ocorre em função de compressão extrínseca dos vasos poplíteos, que resulta em lesão vascular. Trata-se de uma das causas mais freqüentes de claudicação intermitente em pacientes jovens. Os autores descrevem um caso de síndrome bilateral devida à posição anômala do músculo gastrocnêmio, com deslizamento de sua cabeça média (tipo III da classificação de Rich). Durante a cirurgia, o lado direito ocluído foi reconstruído por derivação da veia safena autóloga da artéria superficial femoral para a artéria peroneal e, do lado esquerdo, o músculo que sofreu o deslizamento foi secionado através de via posterior. A síndrome do aprisionamento da artéria poplítea deve ser tratada por cirurgia, independente do grau dos sintomas. A técnica de tratamento cirúrgico liberou o vaso, extraindo o músculo que causava o aprisionamento e reconstruindo o lúmen estreito por derivação.

Popliteal artery entrapment syndrome occurs due to an extrinsic compression of the popliteal vessels that results in vascular damage. It is one of the most frequent causes of intermittent claudication in young patients. The authors describe a case of bilateral syndrome by anomalous position of the gastrocnemius muscle, with abnormal slip of its medial head (Rich's type III). During the operation the occluded right side was reconstructed by autologous saphenous vein bypass from femoral superficial to peroneal artery and on the left side the slip muscle was transected by posterior approach. Popliteal artery entrapment syndrome should be treated by surgery despite the degree of symptoms. Surgical treatment technique has released the vessel by extracting the muscle that caused entrapment, and reconstructing the narrow lumen bypass grafting.

OBJETIVO: Estudar os efeitos da radiação gama de fonte externa no reimplante de artéria femoral. MÉTODOS: Foram utilizados 62 ratos machos Wistar, distribuídos em dois grupos I (Controle) e II (Irradiado), constituídos de três subgrupos de observação: 2, 7 e 21 dias de pós-operatório (PO). As artérias femorais direitas foram retiradas e reimplantadas (anastomose término-terminal) por meio de técnica microcirúrgica. Os animais do grupo II, no primeiro dia de PO, foram irradiados na dose única de 15Gy, emitida de fonte externa. Realizou-se análise descritiva e qualitativa histológica nas lâminas coradas pelos métodos da hematoxilina e eosina (HE), Verhoeff e tricrômio de Masson. As alterações morfológicas da parede vascular foram quantificadas, a partir das imagens capturadas de um microscópio, usando-se um programa de computador, nos mesmos cortes histológicos corados pela HE. Analisou-se a camada íntima (neo-íntima e células endoteliais neoformadas) e a porcentagem de núcleos das células musculares lisas foi determinada na camada média e na hiperplasia intimal. Para análise estatística dos dados, aplicaram-se os testes exato de Fisher, qui-quadrado, Mann-Whitney, Kruskal-Wallis e análise de variância. RESULTADOS: Observou-se que, no grupo irradiado, a presença de células endoteliais neoformadas foi menor; a porcentagem de presença de núcleos de células musculares lisas na hiperplasia intimal foi menor comparado ao grupo controle, embora não significante; a estenose do lúmen vascular e a ocorrência de hiperplasia intimal foi significantemente menor e a porcetagem de células musculares lisas, na camada média, mostrou-se significantemente menor aos 7 e 21 dias de observação. CONCLUSÃO: A radiação de fonte externa emitida no primeiro PO, na dose única de 15 Gy, inibiu a regeneração de células endoteliais e a estenose do lúmen e a hiperplaisa intimal, aos 2, 7 e 21 PO no reimplante de artéria femoral de ratos.

PURPOSE: To investigate the effects of external gamma radiation on rat femoral artery reimplant. METHODS: Sixty-two male Wistar rats were distributed in two groups I (Control) and II (Irradiated), both formed by three observation subgroups: 2 (10 animals), 7 (11) and 21(10) postoperative days (PO). The right femoral artery of each animal was split up and reimplanted (end-to-end anastomosis), through microsurgery technique. In the first PO day group II animals were irradiated with a single dose of 15Gy, external source. The histological analysis, qualitative and descriptive analysis, was accomplished through hematoxylin and eosin (HE), Verhoeff and Masson trichromium methodology. Morphometric analysis was realized in the same slides used to histological analysis stained by HE. Computer software was used to quantify morphological alterations of the vascular wall, by processing captured images from a microscope. It was analyzed the intimal layer (intimal hyperplasia and endothelium cells formation) and the nuclei percentage of smooth muscular cell in the medium layer and in the intima hyperplasia. The data were treated applying the statistical tests: Fisher's exact, Chi-square, Mann-Whitney, Kruskal-Wallis and analysis of variance (p < .05). RESULTS: The patency graft was observed by microscope distal to the autograft in 93.5% (29/31). It was observed that the gamma-radiation reduced the degree of covering of the vascular wall for the neo-formed endothelium. The nuclei percentage of smooth muscular cells, in the media, was smaller compared to control animals, although it was no significant for the studied sample size. The irradiated group showed a significant lower luminal stenosis and the intimal hyperplasia occurrence. In this group the percentage of smooth muscular cells proliferation, in the media, were significantly lower for 7 and 21 days of observation. CONCLUSION: The external single dose of 15 Gy gamma-radiation delivered at first postoperative day may inhibit the endothelial cells regeneration and the percentage of lumen stenosis and prevents intimal hyperplasia, in the 2, 7 and 21 PO on femoral artery autograft in rats.

A hiperplasia intimal ocorre em resposta à lesão vascular, pode complicar significantemente os procedimentos vasculares abertos e endovasculares, levando a diferentes graus de estenose do lume vascular e isquemia. De certo modo, a hiperplasia intimal está presente em todos os tipos de reconstruções vasculares, incluindo enxertos autólogos, homólogos e sintéticos, após angioplastias com ou sem colocação de stent e em órgãos transplantados. Um dos maiores desenvolvimentos experimentais nos últimos anos, para prevenção da hiperplasia intimal, é a utilização da radiação ionizante. A radiação ionizante inibe a proliferação de muitos tipos celulares, incluindo fibroblastos e células musculares lisas in vitro e a síntese de colágeno em culturas de fibroblastos. Modelos animais de experimentação têm sido utilizados para o desenvolvimento de novas estratégias de tratamento e entendimento dos mecanismos fisiopatológicos básicos da hiperplasia intimal. Assim, o objetivo desta revisão e atualização da literatura é o de expor os diferentes modelos de animais de experimentação para o desenvolvimento de hiperplasia intimal, o tipo de lesão empregada, as artérias mais utilizadas e os efeitos encontrados com o uso de diferentes tipos de fontes de radiação ionizante.

Most complications that may occur in response to vascular injury of endovascular and open vascular procedures are due to intimal hyperplasia. To a certain extent, intimal hyperplasia is present in all types of vascular reconstruction, including autologous grafts, allografts, and prosthetic grafts found in solid organs transplanted, having a substantial role in chronic rejection and following angioplasty with or without stenting. One of the greatest developments in recent years towards prevention of intimal hyperplasia is the use of ionizing radiation. Ionizing radiation inhibits proliferation of many cell types including fibroblasts and smooth muscle cells in vitro, and also suppresses the synthesis of collagen by cultured fibroblasts. Animal models have been a cornerstone to develop strategies aimed at understanding the basic physiopathologic mechanisms of intimal hyperplasia and at evaluating novel treatment strategies for clinical conditions. So, the aim of this study was to analyze animals models of intimal hyperplasia, type of injury, artery segments most used, as to the effects of different kinds and sources of ionizing radiation.

Com o objetivo de avaliar a eficácia do pericárdio bovino (PB) para corrigir defeitos arteriais, provocado nas artérias femorais cão, foram realizados remendos de PB e veia cefálica autóloga (VA). Foram utilizados vinte cães, Canis familiaris (oito machos e doze fêmeas), divididos em dois grupos de dez animais (I e II), para serem reoperados no 7º e 30º dias de pós-operatório (PO), respectivamente. Para análise dos dados, aplicou-se o teste do qui-quadrado ou teste exato de Fisher (a£ 0,05). Nas avaliações clínicas não foram observadas alterações significantes, independentemente, do tipo de enxerto utilizado e tempo de observação PO. As avaliações histológicas, entre os enxertos de PB e VA, não diferiram significantemente quanto à presença de infiltrado inflamatório agudo e trombo mural, que predominaram no 7º PO, enquanto a presença de tecido de granulação, placa miofibroblástica e células endoteliais predominaram no 30º PO. A presença de necrose, no 7º PO, entre os enxertos de PB e VA, foi significantemente maior no PB. Os enxertos não sofreram calcificação. Ocorreu significante reação tipo corpo estranho ao fio de polipropileno, no 30º PO, em ambos os enxertos. O PB mostrou-se eficaz como substituto arterial em cães, comparável à VA, para o período de observação estabelecido.

The aim of this work was to evaluate the efficacy of bovine pericardium (BP) patch graft to treat arterial defects created in the femoral artery of dogs, compared to the cephalic vein autograft (CVA) on both side of the arteries. Twenty mongrel dogs (eight males and twelve females), were used and disbuted in two groups of ten animals (I and II), to be re-operated on 7th and 30th post operative days, respstively. Clinical and histolgical data analysis were done using the chi-square an exact Fisher test (a£ 0,05). Significant alterations wasn't found in the clinical examinations, independetly, of the kin of graft used and post operative observation period. It wasn't found any significant difference between the BP and CVA grafts in the histological analyses concerning the presence of inflammatory cell infiltration and mural thrombi that predominated on the 7th post operative day. Whilst, the presence of granulation tisseu, myofibroblastic plaque and endothelization predominated on the 30th post operative day. The presence of necrosis was found significant for both BP and CVA, on the 7th post operative day, being higher for BP. Calcification of the grafts was not observed. A significant reaction of strange body kind to the polypropylene yarn take place on the 30th PO for both grafts. These results sugested that BP was efficient as artery substitute for dogs, which was comparable to CVA, for the settled period of observation.
