O presente estudo teve por objetivo conhecer as vivências de luto de mulheres que interromperam a gestação sob autorização judicial, devido à malformação fetal incompatível com a vida. Participaram do estudo dez mulheres atendidas no Setor de Medicina Fetal do Hospital das Clínicas de Botucatu. Para coleta dos dados foi realizada entrevista semiestruturada quarenta dias após a interrupção. As entrevistas foram audiogravadas, transcritas na íntegra e tiveram os dados analisados na perspectiva da análise temática de conteúdo. Os resultados revelaram que as mães buscaram explicações e significados para a perda, sendo muito frequentes respostas religiosas e autoculpabilizantes. Os relatos marcaram sentimentos de tristeza, saudade e sensação de vazio pela perda do filho, revelando também a necessidade das mães de manterem-se ligadas a ele. As mães estavam e continuaram vinculadas aos seus filhos e a interrupção da gestação, embora tenha sido uma escolha para minimizar a dor de uma perda inevitável, não as poupou de vivências de grande sofrimento. O estudo traz subsídios para a discussão e planejamento de abordagens e cuidados com a saúde de mulheres que interrompem a gestação por malformação fetal letal, mediante autorização judicial.

The scope of this study was to investigate the grieving experiences of women who terminated pregnancies under judicial authorization, due to life-incompatible fetal malformation. Ten women attended in the Fetal Medicine Department of Botucatu Clinical Hospital participated in the study. Data collection was conducted by means of semi-structured interviews forty days after termination. The interviews were recorded and transcribed in full, with the data analyzed from the thematic content analysis perspective. The results revealed that the mothers sought explanations and meanings for the loss, with religious responses and self-blame being very frequent. The reports were marked by feelings of sadness, longing and sensations of emptiness due to the loss of the child, revealing the need of the mothers to dwell on the issue. The mothers were and continued to be linked to their children; the termination of the pregnancy, although being a choice to minimize the pain of an inevitable loss, did not spare the women from experiences of great suffering.>The study includes input for the discussion and planning of health approaches and care for women who terminate their pregnancy due to lethal fetal malformation, by means of judicial authorization.

OBJETIVO: estudar os efeitos maternos (composição corporal e capacidade cardiovascular) e perinatais (peso e prematuridade) da prática da hidroterapia na gestação. MÉTODOS: estudo prospectivo, coorte, aleatorizado, com 41 gestantes de baixo risco e gestação única, praticantes (grupo estudo, n=22) e não-praticantes (grupo controle, n=19) de hidroterapia. Avaliações antropométricas definiram-se os índices de peso corporal, massa magra e gordura absoluta e relativa. Por teste ergométrico, definiu-se os índices de consumo máximo de oxigênio(VO2máx), volume sistólico (VS) e débito cardíaco (DC). Como resultado perinatal observaram-se ocorrência de prematuridade e recém-nascidos pequenos para a idade gestacional. Compararam-se os índices iniciais e finais entre e dentro de cada grupo. As variáveis maternas foram avaliadas pelo teste t para amostras dependentes e independentes e empregou-se o chi ² para estudo das proporções. RESULTADOS: a comparação entre os grupos não evidenciou diferença significativa nas variáveis maternas no início e no final da hidroterapia. A comparação dentro de cada grupo confirmou efeito benéfico da hidroterapia: no grupo estudo os índices de gordura relativa foram mantidos (29,0%) e no grupo controle aumentaram de 28,8 para 30,7%; o grupo estudo manteve os índices de VO2máx (35,0%) e aumentou VS (106,6 para 121,5) e DC de (13,5 para 15,1); no grupo controle observaram-se queda nos índices de VO2máx e manutenção de VS e de DC. A hidroterapia não interferiu nos resultados perinatais, relacionados à prematuridade e baixo peso ao nascimento. CONCLUSÕES: a hidroterapia favoreceu adequada adaptação metabólica e cardiovascular materna à gestação e não determinou prematuridade e baixo peso nos recém-nascidos.

PURPOSE: to study maternal (body composition and cardiovascular capacity) and perinatal (weight and prematurity) effects of hydrotherapy during pregnancy. METHODS: a prospective, random cohort study, with 41 low-risk pregnant women in their first pregnancy, practicing (study group, n=22) and not (control group, n=19) hydrotherapy. Anthropometric evaluation was used to assess lean mass, and absolute and relative body fat. Ergometric tests were used for maximum oxygen consumption (VO2max), stroke volume (SV) and cardiac output (CO). Perinatal results showed premature births and small for gestational age newborns. Initial and final indexes within and between groups were compared. Maternal variables were evaluated using the t test for dependent and independent values; the chi ² test was used to study proportions. RESULTS: there were no significant differences between the groups for maternal variables at the start and end of hydrotherapy. Comparison within each group confirmed the beneficial effect of hydrotherapy. In the study group, relative fat index was maintained at 29.0%; the control group showed an increase from 28.8% to 30.7%; the study group maintained VO2max at 35%, and increased SV from 106.6 to 121.5 and CO from 13.5 to 15.1; the control group showed a drop in VO2max and no change in SV and CO. There was no relationship between hydrotherapy and perinatal results. CONCLUSIONS: hydrotherapy adequately assisted metabolic and cardiovascular maternal adaptation to pregnancy and did not cause prematurity or weight loss in newborns.

OBJETIVO: estudar os efeitos de técnicas fisioterápicas sobre os desconfortos músculo-esqueléticos na gestação. MÉTODOS: estudo coorte prospectivo, com 71 gestantes nulíparas, de baixo risco, distribuídas conforme a participação (estudo; n=38) ou não (controle; n=33). O Programa Multidisciplinar de Preparo para o Parto e Maternidade constou de 10 encontros (da 18ª à 38ª semana), com atividades educativas, fisioterápicas e de interação. Compararam-se, por questionário específico, no início e final do programa, a ocorrência (presença ou ausência), as características (local, tipo, intensidade, freqüência e duração) e a evolução dos desconfortos músculo-esqueléticos. As médias aferidas na avaliação inicial foram comparadas pela análise de variância (ANOVA) seguida pelo teste do F. Para o estudo entre as proporções, verificadas no início e no final do programa, utilizou-se o teste do c². A significância estatística foi definida pelo limite de 5% (p<0,05). RESULTADOS: no início do programa, 63,6% das gestantes do grupo controle e 84,2% do Estudo relataram sintomas músculo-esqueléticos (p=0,05), caracterizados por dor na região lombossacra. No grupo controle predominaram a intensidade leve (18,2%) e grave (18,4%), e no grupo estudo, a grave (36,8%) e isolada ou associada (31,6%). No final, o grupo controle tinha sintomas de intensidade grave (60,6%), com freqüência diária (42,4%) e duração maior que três horas (69,7%) (p<0,05). O grupo estudo referia intensidade leve (57,9%) e freqüência quinzenal (50,0%) com duração máxima de uma hora (55,3%) (p<0,05). A evolução dos sintomas foi diferenciada, confirmando-se piora em 63,6% das gestantes do Controle e melhora em 65,8% das participantes do Programa (p<0,05). CONCLUSÕES: as técnicas fisioterápicas do Programa Multidisciplinar de Preparo para o Parto e Maternidade se relacionaram à diminuição da intensidade, freqüência e duração e à melhor evolução dos desconfortos músculo-esqueléticos na gestação.

PURPOSE: to study the effects of physiotherapeutic techniques applied by the Multidisciplinary Program of Preparation for the Childbirth and Maternity on musculoskeletal discomforts during pregnancy. METHODS: prospective cohort study, with 71 low-risk nulliparous women, distributed according to participation (study: n=38) or not (control; n=33). The Multidisciplinary Program of Preparation for Childbirh and Maternity had 10 meetings (18th to 38th week), with educational, physiotherapeutic, and interaction activities. Occurrence, characteristics, and evolution of musculoskeletal discomforts were compared by means of a specific questionnaire, both at the beginning and at the end of the program. The average of results of the initial assessment was compared through analysis of variance (ANOVA) followed by the F test. For the study between proportions in the beginning and at the end of the program the c² was used. The statistical significance was determined at 5% of limit (p<0.05). RESULTS: at the beginning of the program, 63.6% of the pregnant women of the control group and 84.2% of the study group reported musculoskeletal symptoms (p=0.05), characterized by back and posterior pelvic pain. In the control group, light intensity (18.2%) and serious intensity pain (18.4) were predominant, while in the study group, the serious was 36.8%, and the isolated or associated was 31.6%. At the end, the control group showed symptoms of serious intensity (60.6%), with daily frequency (42.4%) and length of more than 3 h (69.7%; p<0.05). The study group reported light intensity (57.9%) and bimonthly frequency (50.0%) with a maximum length of one hour (55.3%) (p<0.05). Symptom evolution was also differentiated and there were worsening in 63.6% of pregnant women of the control group and improvement in 65.8% of participants of the program (p<0.05). CONCLUSIONS: the physiotherapeutic techniques of the Multidisciplinary Program of Preparation for Childbirth and Maternity were related to a decrease in intensity, frequency and length and to a better evolution of musculoskeletal discomforts during pregnancy.
