Resumo O texto aqui comentado reconstitui o cotidiano no Instituto Oswaldo Cruz no começo do século XX com base em depoimentos de antigos funcionários. “Os escravos são as mãos e os pés do senhor do engenho” – escreveu Antonil em 1711. Cada laboratório do instituto era como um pequeno engenho onde as mãos e os pés dos pesquisadores eram seus serventes, que executavam desde as tarefas mais desqualificadas até operações bem delicadas da pesquisa científica, atualmente confiadas a técnicos formados em escolas próprias. As habilidades dos primitivos técnicos, muitos recrutados ainda meninos nas oficinas da instituição, eram adquiridas empiricamente. O instituto era moderno por suas atividades, mas as relações de trabalho traziam as marcas de uma sociedade agrária e patriarcal, recém-egressa da escravidão.

Abstract This text reconstructs everyday routines at the Oswaldo Cruz Institute in the early twentieth century based on statements from staff at that time. As Antonil wrote in 1711, “Slaves are the hands and feet of the sugar-mill owner.” The researchers’ assistants fulfilled a similar role in the laboratories; their work ranged from unskilled tasks to extremely delicate scientific research that today requires specialized training. These early technicians, many recruited as boys in the institution’s workshops, acquired their skills empirically. While the institute carried out modern activities, labor relations there still bore the marks of an agrarian and patriarchal society that had only recently abandoned slavery.

Resumo Os primeiros casos de leishmaniose cutânea e mucocutânea autóctones das Américas foram descritos em 1909, e em 1934 a leishmaniose visceral irrompeu como problema de saúde pública na região. O Brasil tem hoje o maior número de casos da leishmaniose tegumentar americana e, junto com a Índia, a mais elevada incidência de leishmaniose visceral. A produção de conhecimentos e os esforços para controlar essas doenças mobilizaram, em nível global, profissionais de saúde, populações urbanas e rurais, instituições governamentais e agências internacionais. Recuperam-se aqui alguns desses agrupamentos, redes de troca e cooperação, incertezas e polêmicas, identificando-se mudanças na abordagem das leishmanioses do Novo Mundo.

Abstract The first autochthonous cases of cutaneous and mucocutaneous leishmaniasis in the Americas were described in 1909, but visceral leishmaniasis only erupted as a public health problem in the region in 1934. Today Brazil is the country with the most cases of American tegumentary leishmaniasis, and alongside India has the highest incidence of visceral leishmaniasis. Knowledge production and efforts to control these diseases have mobilized health professionals, government agencies and institutions, international agencies, and rural and urban populations. My research addresses the exchange and cooperation networks they established, and uncertainties and controversial aspects when notable changes were made in the approach to the New World leishmaniases.

Resumo Casos pioneiros de leishmaniose cutânea e mucocutânea nas Américas foram descritos em São Paulo, em 1909; somente em 1934, um patologista do Serviço de Febre Amarela encontrou a leishmaniose visceral no Brasil. Processos históricos concernentes a essas formas ganharam mais vigor institucional nos anos 1930. Se a Comissão para o Estudo da Leishmaniose consolidou o conceito de leishmaniose tegumentar americana, a Comissão Encarregada do Estudo da Leishmaniose Visceral Americana, chefiada por Evandro Chagas, deu origem ao Instituto de Patologia Experimental do Norte (1936) e ao Serviço de Estudo das Grandes Endemias (1938). A leishmaniose visceral ganhou crescente relevância no Nordeste brasileiro, nos anos 1950. Medidas de controle dos vetores por Dicloro-Difenil-Tricloroetano (DDT) ocorreram a reboque da campanha contra a malária, direcionadas também a cães, sacrificados massivamente, e humanos, tratados com drogas antimoniais. Grandes empreendimentos no interior do Brasil após o golpe civil-militar de 1964 transformaram a leishmaniose cutânea e mucocutânea em um problema sério na Amazônia e em outras regiões. No Brasil e em outros países, todas as formas de leishmaniose supostamente sob controle reemergiram em zonas rurais e urbanas e em áreas consideradas livres desse complexo de doenças endemoepidêmicas, devido a mudanças ambientais, migrações humanas, crescimento urbano caótico e outros processos socioeconômicos.

Abstract The first cases of cutaneous and mucocutaneous leishmaniasis in the Americas were described in São Paulo in 1909; visceral leishmaniasis was only found in Brazil in 1934, by a Yellow Fever Service pathologist. The historical processes related to these forms of leishmaniasis gained institutional strength in the 1930s. While the Leishmaniasis Study Commission solidified the concept of American tegumentary leishmaniasis, the American Visceral Leishmaniasis Study Commission (headed by Evandro Chagas) gave rise to the Institute of Experimental Pathology for the North (1936) and the Large Endemic Disease Study Service (1938). Visceral leishmaniosis gained importance in northeastern Brazil in the 1950s, and control measures against its vectors using dichlorodiphenyltrichloroethane (DDT) followed in the wake of the malaria campaign. They also targeted dogs (which were killed en masse) and humans, who were treated with antimonial drugs. Large-scale undertakings in Brazil’s hinterlands after the 1964 civilian-military coup transformed cutaneous and mucocutaneous leishmaniasis into a serious problem in Amazonia and other regions. Brazil and other countries saw a resurgence of all forms of leishmaniasis in rural and urban areas because of environmental changes, human migrations, chaotic urban growth, and other socioeconomic processes.

Resumo Harold Howard Shearme Wolferstan Thomas foi um pesquisador da Escola de Medicina Tropical de Liverpool que teve fugaz destaque na medicina britânica na época em que foi deslocado para a Amazônia (1905). Cinco anos antes, uma expedição da mesma Escola estivera na região para investigar a febre amarela. Thomas e Anton Breinl viajaram para Manaus ainda com o objetivo de estuda essa doença. Naquele intervalo, transcorreram processos muitos dinâmicos na medicina tropical, em particular no tocante às tripanossomíases. Thomas ganhou projeção ao demonstrar que o atoxyl era eficaz no tratamento de animais e humanos infectados por tripanossomos. No presente artigo, Thomas é o fio que conduz a uma trama formada por diferentes atores e doenças na América, Europa e África, cujas sinergias revelam contornos da medicina tropical e o lugar nela ocupado pela região amazônica no começo do século XX. Enfatizaremos especialmente as tripanossomíases. Até sua morte em Manaus, em 1931, Thomas envolveu-se com outros problemas de saúde locais e com médicos que lideravam a saúde pública e a medicina experimental no Amazonas - curso discrepante daquele tomado pela maioria dos médicos europeus que participaram de missões em colônias e áreas de influência das metrópoles imperiais. Nos anos 1950, Thomas foi ‘redescoberto’, ao receber (postumamente) parte de um prêmio conferido aos descobridores da cura da doença do sono. Sua memória teve outros revivals em ambientes acadêmicos.

Abstract Harold Howard Shearme Wolferstan Thomas was a researcher from the Liverpool School of Tropical Diseases who enjoyed brief prominence in British medicine at the time he was transferred to Amazonia (1905). Five years earlier, an overseas expedition from this school had been in the region to investigate yellow fever. When Thomas and his colleague Anton Breinl set sail for Manaus, their mission was still to investigate this disease. In the interval between the two expeditions, dynamic processes were underway in the realm of tropical medicine, especially with regard to trypanosomiasis. Thomas gained recognition when he showed that atoxyl was an effective treatment for humans and animals infected with trypanosomes. In this article, Thomas guides us through a web of actors and diseases in the Americas, Europe and Africa. Their synergies reveal the outlines of tropical medicine and the place occupied by the Amazon region within this field at the beginning of the twentieth century, with special emphasis on trypanosomiasis. Until his death in Manaus in 1931, Thomas involved himself with other local health problems and with physicians who were on the forefront of Amazonian public health and experimental medicine, an unusual course for most European researchers sent on missions to colonies and areas influenced by imperial metropolises. Thomas was ‘rediscovered’ two decades after his death when he was named one of the recipients of a prize from the Belgian government granted to the scientists who discovered the treatment for sleeping sickness. As we shall see, his memory was revived on other occasions in the academic sphere.

Em junho de 1899, começou a funcionar a Liverpool School of Tropical Medicine. No ano seguinte, dois de seus quadros, Herbert Edward Durham (1866-1945) e Walter Myers (1872-1901), viajaram para Belém, no Brasil, para investigar a febre amarela. Uma escala da viagem, em Havana, possibilitou o encontro com os norte-americanos que aí pesquisavam também a doença. Durham e Myers traziam a hipótese da transmissão por inseto hospedeiro, à semelhança do que acontecia com a malária, cujo modo de transmissão acabara de ser decifrado por ingleses e italianos. Mas, no Brasil, mantiveram-se presos à etiologia bacteriana e não acompanharam a ruptura na abordagem da febre amarela que o programa da medicina tropical propiciava. Tal paradoxo pode ser explicado examinando-se as trajetórias prévias de Durham e Myers, que se entrelaçam à de outro investigador, Alfredo Antunes Kanthack (1863-1898), nascido na Bahia, um dos principais protagonistas da instituição da microbiologia e imunologia na Inglaterra.

The Liverpool School of Tropical Medicine was started in June 1899. The following year, two of its staff members, Herbert Edward Durham (1866-1945) and Walter Myers (1872-1901), traveled to Belém, in Brazil, to conduct research on yellow fever. During a stop-over in Havana, they had the opportunity to meet with U.S. scientists studying the disease there. Durham and Myers brought with them the hypothesis that yellow fever was transmitted by a host insect, much like malaria, whose transmission had recently been deciphered by British and Italian researchers. Yet in Brazil, they persisted in the hypothesis of a bacterial etiology, rather than adopting the breakthrough approach to yellow fever that had been made possible by the program in tropical medicine. This paradox can be explained by examining both Durham's and Myers' backgrounds, which were interwoven with that of another researcher, Alfredo Antunes Kanthack (1863-1898), who was born in Bahia and was one of the central protagonists in the institution of microbiology and immunology in England.

Aborda o impacto da malária no âmbito da modernização republicana, basicamente nas ferrovias, que asssumiram então o papel de integrar o território e operar a expansão simbólica e material da nação brasileira. Os cientistas destacados para debelar os surtos epidêmicos não se limitaram a realizar as campanhas. Fizeram observações sobre aspectos da doença, inclusive suas relações com hospedeiros e ambientes, contribuindo com novos conhecimentos e com a institucionalização, no Brasil, de novo campo que então se estabelecia nas potências coloniais européias: a medicina tropical. O artigo articula essas inovações - especialmente a teoria da infecção domiciliária - com as campanhas em prol de ferrovias e com estágio subseqüente no enfrentamento da malária no Brasil, nos anos 1920.

The article explores the impact of malaria on infrastructure works - above all, railroads - under the republican drive towards modernization. Railways helped tie the territory together and foster the symbolic and material expansion of the Brazilian nation. The scientists entrusted with vanquishing such epidemic outbreaks did not just conduct campaigns; they also undertook painstaking observations of aspects of the disease, including its relations to hosts and the environment, thus contributing to the production of new knowledge of malaria and to the institutionalization of a new field in Brazil, then taking root in Europe's colonies: "tropical medicine." The article shows the ties between these innovations (especially the theory of domiciliary infection) and the sanitary campaigns that helped the railways, which in the 1920s were followed by a new phase in Brazil's anti-malaria efforts.

Este artigo retrata as origens familares e a trajetória de Adolpho Lutz (1855-1940) até sua transferência para o Instituto Oswaldo Cruz (IOC), em 1908. Utiliza o cientista como fio condutor para a análise da instituição das medicinas pasteuriana e tropical no Brasil. Examina seus estudos superiores e de aperfeiçoamento na Europa germânica, suas atividades como clínico e como investigador de temas relacionados à helmintologia, parasitologia, veterinária e bacteriologia no interior de São Paulo, sua estada no leprosário de Molokai, no Havaí, e controvérsias médicas de que participou durante o período em que chefiou o Instituto Bacteriológico de São Paulo, especialmente aquelas concernentes ao cólera, às disenterias, à febre tifóide, à malária e à febre amarela.

This article portrays the family origins and life story of Adolpho Lutz (1855-1940) up to his transfer to the Instituto Oswaldo Cruz in 1908. His life history is used as a motif for an analysis of the institution of pasteurian and tropical medicine in Brazil. His university and postgraduate study in German-speaking Europe are examined, as are his activities as a clinician and researcher on subjects related to helminthology, parasitology, veterinary medicine and bacteriology in the interior of São Paulo state; his stay at the Molokai leprosarium in Hawaii; and the medical controversies in which he participated as head of the Bacteriological Institute of São Paulo, especially those on cholera, dysenteries, typhoid fever, malaria and yellow fever.

O presente trabalho apresenta a série completa da correspondência da família de um portador do mal de Hansen residente no Maranhão, Nordeste do Brasil, com o médico e bacteriologista Adolpho Lutz. Fabrício Caldas de Oliveira e Numa Pires de Oliveira, pai e filho, mantiveram durante mais de vinte anos intensa troca de cartas com o cientista na busca da cura da doença que vitimava Numa desde criança. As 24 cartas aqui reunidas retratam, de maneira única, o drama médico-social enfrentado por esta família, os resultados do uso do chalmugra e outros medicamentos, a busca de tratamentos alternativos.

This project presents the complete set of letters between the family of a Hansen's disease (leprosy) sufferer in the state of Maranhão, in the northeast of Brazil, and the doctor and bacteriologist Adolpho Lutz. For more than twenty years Fabrício Caldas de Oliveira and Numa Pires de Oliveira, father and son, exchanged a steady flow of letters with the scientist in pursuit of a cure for the disease that had assailed Numa since childhood. The 24 letters compiled here paint a unique portrait of the medical and social drama confronted by this family, and the results of the use of chaulmoogra oil and other medications in their search for alternative treatments.

Este artigo analisa a instituição da microbiologia e suas conseqüências para a saúde pública brasileira durante o último quarto do século XIX e o começo do atual. O autor examina o trabalho realizado pela Escola Tropicalista Baiana, a trajetória de outra geração de médicos que, no Rio de Janeiro e em São Paulo, investigaram a febre amarela e outras doenças à luz da teoria dos germes, procurando descobrir tanto o seu micróbio específico como imunobiológicos e tratamentos eficazes. The author examines the work done by members of Escola Tropicalista Baiana and then by several constituents of another generation of physicians who, in Rio de Janeiro and São Paulo, researched yellow fever and other diseases from the perspective of the germ theory, trying to discover both its specific microbe as well as effective therapeutic and immunobiological treatments to those diseases. The article also examines the transition of the etiologic issue to the question of the means of transmission not only of yellow fever but also of malaria, correlating it with the coming of age both of Pasteurianism and Tropical Medicine. The adoption of Finlay's theory in Brazil and the successful campaigns led by Oswaldo Cruz in Rio de Janeiro, while the Brazilian capital was reshaped in accordance to a 'haussmannian' mould, initiates a new era in which Instituto Oswaldo Cruz and other medical institutions develop dynamic research programs in close syntony with European and North American Bacteriology and tropical medicine.
