ABSTRACT Objective Paraoxonase 1 (PON1) polymorphisms are associated with an increased susceptibility to cardiovascular disease. PON1 Q192R polymorphism (rs662) partially determine PON1 hydrolytic activity and protect against oxidation of LDL and HDL. This study aimed to delineate the association of PON1 status (functional 192 genotype and plasma activity levels) and atherogenicity in urbans residents aged 40 years or more. Materials and methods Anthropometric data, lipid profiles, the atherogenic index of the plasma (AIP) and Framingham score risk were measured. Three kinetic assays were conducted to assay PON1 status using phenylacetate and 4-(chloromethyl)phenyl acetate as substrates. Results Smoking per se did not significantly impact the AIP but the interaction PON1 genotype by smoking significantly increased the AIP. In subjects with the RR genotype smoking increased the AIP index from (estimated mean ± SEM) -0.038 ± 0.039 to 0.224 ± 0.094. The QR genotype increased the Framingham risk index by around 1.3 points. Smoking by RR genotype carriers significantly increased the Framingham risk score (17.23 ± 2.04) as compared to smoking (13.00 ± 1.06) and non-smoking (7.79 ± 0.70) by QQ+QR genotype carriers. The interaction RR genotype by smoking was a more important predictor (odds ratio = 7.90) of an increased Framingham risk score (> 20) than smoking per se (odds ratio = 2.73). The interaction smoking by RR genotype carriers significantly increased triglycerides and lowered HDL cholesterol. Conclusion Smoking per se has no (AIP) or a mild (Framingham risk score) effect on atherogenicity, while the interaction smoking by PON1 RR genotype has a clinically highly significant impact on atherogenicity.

Resumo Introdução: Pacientes com doença renal em estágio terminal têm níveis de inflamação e estresse oxidativo maiores do que a população geral. Muitos fatores contribuem para isso, e o hormônio paratireoidiano (PTH) é um deles. Objetivo: Estudo foi realizado para avaliar a relação entre os níveis de PTH e níveis de inflamação e estresse oxidativo em pacientes em hemodiálise. Métodos: estudo transversal com pacientes de duas unidades de hemodiálise de Londrina, Brasil. Pacientes com condições causadoras de inflamação e estresse oxidativo foram exclusos. Níveis plasmáticos de PTH e parâmetros bioquímicos de inflamação (interleucina 1 e 6, fator de necrose tumoral alfa) e estresse oxidativo (capacidade antioxidante plasmática total, dialdeído malônico, hidroperoxidação lipídica, produtos avançados da degradação proteica, quantificação de metabólitos de óxido nítrico e 8-isoprostano) foram dosados antes da sessão de hemodiálise. Realizou-se análise de correlação entre os níveis de PTH - tanto como variável continua como variável categórica em tercis - e os parâmetros de inflamação e estresse oxidativo. Resultados: Não houve correlação do PTH com nenhum dos parâmetros testados, nem como variável contínua, nem como categórica. Conclusão: Neste estudo descritivo, os resultados sugerem que a inflamação e o estresse oxidativo em pacientes em hemodiálise provavelmente tem origem em mecanismos que não incluem o hiperparatireoidismo secundário.

Abstract Introduction: Patients at end stage renal disease have higher levels of inflammation and oxidative stress than the general population. Many factors contribute to these issues, and the parathyroid hormone (PTH) is also implicated. Objective: The study was conducted in order to assess the relationship between PTH levels and inflammation and oxidative stress in hemodialysis patients. Methods: Cross-sectional study with patients of two hemodialysis facilities in Londrina, Brazil. Patients with other conditions known to generate oxidative stress and inflammation were excluded. Blood levels of PTH and biochemical parameters of inflammation (interleukins 1 and 6, tumor necrosis factor-alpha) and oxidative stress (total plasma antioxidant capacity, malonic dialdehyde, lipid hydroperoxidation, advanced oxidation protein products, quantification of nitric oxide metabolites, and 8-isoprostane) were measured before a dialysis session. Then, we made correlation analyses between PTH levels - either as the continuous variable or categorized into tertiles-, and inflammatory and oxidative stress biomarkers. Results: PTH did not show any correlation with the tested inflammation and oxidative stress parameters, nor as continuous variable neither as categorical variable. Conclusion: In this descriptive study, the results suggest that the inflammation and oxidative stress of hemodialysis patients probably arise from mechanisms other than secondary hyperparathyroidism.

OBJETIVO: Avaliar os indicadores de estresse oxidativo e marcadores inflamatórios em obesos antes e depois da gastroplastia vertical com bandagem em Y-de-Roux. MÉTODOS: Vinte obesos classe III foram submetidos à gastroplastia vertical com bandagem em Y-de-Roux, sendo sete homens e 13 mulheres, com idade média de 39 anos e 20 indivíduos não obesos, nove homens e 11 mulheres, média de idade de 38 anos. Foram determinados os níveis de malondialdeído no plasma, índice de estresse, capacidade antioxidante total, catalase e glutationa reduzida e oxidada, e marcadores inflamatórios (proteína C reativa e á-1 glicoproteína ácida). No grupo obeso, estes parâmetros foram determinados antes e 2, 6 e 12 meses após a gastroplastia vertical com bandagem em Y-de-Roux. RESULTADOS: No pré-operatório, o grupo obeso apresentou níveis elevados de marcadores inflamatórios, de estresse oxidativo (níveis de malondialdeído e índice de estresse) e menores níveis de indicadores de defesa antioxidante em relação ao grupo controle. O emagrecimento foi acompanhado de redução progressiva do níveis de malondialdeído e do índice de estresse. Foi observado aumento da concentração de glutationa reduzida e da capacidade antioxidante total e redução dos níveis de marcadores inflamatórios. CONCLUSÃO: A redução do peso melhora o estado inflamatório e os níveis de estresse oxidativo.

OBJECTIVE: To evaluate the oxidative stress and inflammatory markers in obese patients before and after Roux-en-Y vertical banded gastroplasty. METHODS: We studied 20 class III obese patients undergoing Roux-en-Y vertical banded gastroplasty, seven men and 13 women, mean age 39 years, and 20 non-obese subjects, nine males and 11 females, mean age 38 years. We determined the plasma levels of malondialdehyde, stress an index, total antioxidant capacity, catalase, reduced and oxidized glutathione and inflammatory markers (C reactive protein and á-1 acid glycoprotein). In the obese group, these parameters were determined before and 2, 6 and 12 months after gastroplasty. RESULTS: Preoperatively, the obese group showed elevated levels of inflammatory markers of oxidative stress (malondialdehyde levels and stress index) and lower levels of indicators of antioxidant defense compared to the control group. Weight loss was accompanied by gradual reduction in the levels of malondialdehyde and stress index. We found an increased concentration of reduced glutathione and total antioxidant status and reduced levels of inflammatory markers. CONCLUSION: Weight loss improves the inflammatory state and oxidative stress levels.

OBJETIVO: Oferecer informações descritivas e investigar a extensão com que os fatores de risco para doenças cardiovasculares de natureza comportamental estão associados aos fatores de risco biológicos na população jovem. MÉTODOS: Amostra de 452 adolescentes (246 moças e 206 rapazes) com idades entre 15 e 18 anos, selecionados de uma escola de ensino médio da cidade de Londrina, Paraná. Fatores de risco de natureza comportamental foram analisados mediante prática insuficiente de atividade física, ingestão excessiva de gorduras e de colesterol e uso de tabaco. Como indicadores dos fatores de risco biológicos recorreu-se ao sobrepeso, aos níveis elevados de pressão arterial e concentrações não-favoráveis de lipídios-lipoproteínas plasmáticas. RESULTADOS: Por volta de 20% das moças e 16% dos rapazes apresentaram pelo menos um fator de risco biológico para o desenvolvimento de doenças cardiovasculares. O sobrepeso se associou significativamente com a ingestão excessiva de gorduras, enquanto a pressão arterial elevada se relacionou com o estilo de vida sedentário e o uso de tabaco. A excessiva ingestão de gorduras e de colesterol indicou risco aumentado de concentrações indesejáveis de lipídios-lipoproteínas plasmáticas. Adolescentes fumantes tenderam a demonstrar risco de pressão arterial e de lipídios-lipoproteínas plasmáticas alterados duas vezes maior que não-fumantes. CONCLUSÃO: Os resultados reforçam a necessidade de que intervenções direcionadas à adoção de um estilo de vida saudável, incluindo prática regular de atividade física, padrões dietéticos adequados e abstenção ao uso de tabaco, deverão ser iniciadas em idades jovens.

OBJECTIVE: To provide descriptive information and investigate to what extent behavioral risk factors for cardiovascular diseases are associated with biological risk factors in the young population. METHODS: The sample included 452 adolescents (246 girls and 206 boys) between 15 and 18 years of age selected in a high school in the city of Londrina, Paraná. Risk factors of a behavioral nature were analyzed as regards insufficient physical activity, excessive intake of fat and cholesterol and smoking. As biological risk factor indicators we used overweight, high arterial pressure levels, and adverse concentration of serum lipids and lipoproteins. RESULTS: Approximately 20% of the girls and 16% of the boys presented at least one biological risk factor for development of cardiovascular diseases. Overweight was significantly associated with excessive intake of fats, while elevated arterial pressure was related to sedentary lifestyle and smoking. Excessive intake of fat and cholesterol indicated increased risk of undesirable concentrations of serum lipids and lipoproteins. The risk associated with altered blood pressure and serum lipids and lipoprotein was increased two-fold in smokers when compared with nonsmokers. CONCLUSION: The results reinforce the need for interventions targeted at the adoption of a healthy lifestyle, including the regular practice of physical exercise, appropriate dietary patterns, and abstention from smoking as of an early age.

Benefícios da prática de atividade física e riscos do sedentarismo associados à saúde e ao bem-estar são amplamente documentados na literatura. No entanto, maior número de estudos procura envolver sujeitos adultos. Pouco se conhece com relação aos hábitos de prática de atividade física de adolescentes. O objetivo do estudo foi analisar níveis de prática de atividade física habitual em amostra representativa de adolescentes matriculados em escola de ensino médio do município de Londrina, Paraná. A amostra foi constituída por 281 adolescentes (157 moças e 124 rapazes) com idade entre 15 e 18 anos. Informações acerca da atividade física habitualmente realizada foram obtidas mediante instrumento retrospectivo de auto-recordação das atividades diárias. Estabeleceram-se estimativas quanto à demanda energética (kcal/kg/dia) com base no custo calórico associado ao tipo e à duração das atividades registradas pelos adolescentes. Os resultados revelam que rapazes foram consistentemente mais ativos fisicamente que moças. Por volta de 54% dos rapazes envolvidos no estudo foram classificados como ativos ou moderadamente ativos, enquanto aproximadamente 65% das moças analisadas mostraram ser inativas ou muito inativas. Os rapazes demonstraram significativamente maior envolvimento na prática de exercícios físicos e de esportes que moças (3:20 vs. 0:48 horas/semana). Os níveis de prática de atividade física habitual tenderam a reduzir-se com a idade, sobretudo entre moças. As moças permaneceram menor tempo em frente da TV e do vídeo que rapazes (3:30 vs. 4:00 horas/dia). Rapazes, porém não moças, pertencentes a classe socioeconômica familiar mais baixa mostraram ser menos ativos fisicamente que seus pares de classe socioeconômica familiar mais privilegiada. A maioria dos adolescentes (97% das moças e 74% dos rapazes) não atende às recomendações quanto à prática de atividade física que possa alcançar impacto satisfatório à saúde. Como conclusão, a elevada incidência de sedentarismo observada na amostra analisada sugere ações intervencionistas que venham incentivar a prática adequada de atividade física na população jovem.

The benefits of physical activity to the health and well-being of the general population and the risks of inactivity are now well documented. However, evidence concerning physical activity levels has focused largely on adults. Relatively little is known about the physical activity patterns of adolescents. The purpose of this study was to analyze levels of regular physical activity in representative samples of adolescents selected from a secondary school in the city of Londrina, Paraná, in the southern region of Brazil. A total of 281 adolescents (157 girls and 124 boys) ages ranging from 15 to 18 years were included in the study. The levels of regular physical activity were determined by means of a self-report instrument which provided an estimate of energy expenditure and of the time spent in different activities. Results showed that boys were consistently more active than girls. A percentage of 54% of the boys was considered active or moderately active based on their estimated daily average energy expenditure (kcal/kg/day), while approximately 65% of the girls were found to be inactive or very inactive. Boys spent a significantly longer time in physical exercise and in sports than girls (3:20 vs 0:48 hours/week). Levels of regular physical activity decreased significantly with age, and the decline was greater in girls. Girls spent less time watching TV than boys (3.3 vs 4.0 hours/days). Boys, but not girls, from lower socioeconomic families performed less physical activity than boys from higher socioeconomic families. Most adolescents (girls 97%; boys 74%) did not practice sufficient physical exercise so as to achieve the health benefits suggested by current guidelines and recommendations for young people. In conclusion, the levels of physical inactivity reported constitute an important issue for social agencies and organizations involved in the promotion of health-enhancing physical activity.
