ABSTRACT Purpose: to investigate the linguistic profile (linguistic symptoms and risk factors) of children with communication, language, and speech disorders and verify their family’s participation in the therapeutic process. Methods: an integrative literature review including studies on linguistic symptoms of children with developmental language disorder (DLD), autism spectrum disorder (ASD), and phonological disorders (PD), published in Portuguese in the last 10 years (2012 to 2022) in the PubMed, SciELO, and VHL databases. Literature Review: 24 studies were included, according to the eligibility criteria, the children having different linguistic symptoms. In PD, there were more phonological processes, slower speech, and difficulties in auditory perception and morphosyntactic and semantic development. In DLD, there were changes in nominal morphology, morphosyntactic comprehension, greater use of intransitive verbs, omission of verb objects, and difficulties in solving conflicts. In ASD, there were difficulties in social interaction, imitation, prosodic changes, and decreased responsiveness. The main risk factor was prematurity, and the family was considered crucial as a support network in the therapeutic process. The family’s attitudes were facilitators or barriers to the patients’ improvement, depending on how they responded to the therapists’ instructions. Conclusion: the linguistic profile depends on the various language/speech disorders. Each child’s singularities must be considered, and the family’s participation is crucial to the therapeutic process.

RESUMO Objetivo: investigar o perfil linguístico (sintomas linguísticos e fatores de risco) de crianças com transtornos de comunicação, linguagem e fala e verificar a participação da família no processo terapêutico. Métodos: foi realizada uma revisão integrativa da literatura. Foram incluídos estudos sobre sintomas linguísticos de crianças com Transtorno do Desenvolvimento da Linguagem (TDL), TEA (Transtorno do Espectro Autista) e Transtornos Fonológicos (TF), publicados nos últimos 10 anos (2012 a 2022) em língua portuguesa nas bases de dados: PubMed, Scielo e BVS. Revisão da Literatura: aplicando os critérios de elegibilidade, foram incluídos 24 estudos. As crianças apresentaram diferentes sintomas linguísticos. No TF verificaram-se quantidade maior de processos fonológicos, menor velocidade de fala, dificuldades na percepção auditiva e no desenvolvimento morfossintático e semântico. No TDL verificaram-se alterações na morfologia nominal, na compreensão morfossintática, maior utilização de verbos intransitivos, omissões de complementos verbais e dificuldade em resolução de conflitos. No TEA observaram-se dificuldades na interação social, imitação, alterações prosódicas e diminuição na responsividade. O principal fator de risco foi a prematuridade e a família foi considerada crucial como rede de apoio no processo terapêutico. As atitudes da família foram facilitadoras ou barreiras para a melhora do paciente, dependendo da forma como as famílias correspondiam às orientações terapêuticas. Conclusão: o perfil linguístico depende dos diferentes transtornos de linguagem/fala, as singularidades de cada criança devem ser consideradas e a participação da família é crucial no processo terapêutico.

Este estudo teve como objetivo comparar as mudanças fonológicas decorrentes da aplicação de uma abordagem de terapia fonoaudiológica baseada em traços distintivos, utilizando dois tipos de sons-alvo (que enfatizam o contraste e que enfatizam o reforço de traços distintivos) no tratamento do desvio fonológico. A amostra foi constituída por sete crianças (quatro meninos e três meninas) com desvio fonológico, com idades entre 3 anos e 10 meses e 6 anos e 9 meses. As crianças foram classificadas de acordo com o grau do desvio fonológico e posteriormente, foram submetidas ao tratamento por meio do Modelo de Oposições Máximas Modificado. Em cada grau foram agrupados dois sujeitos, sendo que um foi tratado pelo "contraste" e o outro pelo "reforço" dos traços distintivos que apresentavam dificuldade. Somente o grau moderado-severo foi composto por apenas um sujeito. Após 20 sessões terapêuticas foram analisadas as mudanças fonológicas pré e pós-tratamento, considerando-se a forma de apresentação do estímulo ("contraste" e "reforço"). Todos os sujeitos tratados pelo "contraste" e pelo "reforço" apresentaram aumento no Percentual de Consoantes Corretas, no número de sons adquiridos e nas generalizações em seus inventários fonológicos. Verificou-se que ambos os grupos apresentaram mudanças em seus inventários fonológicos. Na análise comparativa entre os grupos foi observado que os dois grupos, tratados pelo "contraste" e pelo "reforço", demonstraram diferenças em relação aos tipos de generalizações apresentadas.

This study aimed to compare the phonological changes due to the application of a speech therapy approach based on distinctive features, using two types of target sounds (the ones which emphasize the contrast, and others which reinforce the distinctive features) in the treatment of phonological disorder. The sample was constituted by seven children with phonological disorder (four boys and three girls), with ages between 3 years and 10 months and 6 years and 9 months. The children were classified according to the severity of the phonological disorder and then underwent treatment based on the Modified Maximal Oppositions Model. Two subjects were grouped for each degree; one subject was treated by "contrast" and the other one by "reinforcement" of the distinctive features in which they showed difficulties. The moderate-severe degree was the only one to include only one subject. After 20 therapy sessions, the phonological changes before and after the treatment were analyzed, considering the type of stimulus presented ("contrast" or "reinforcement"). All subjects, either treated by "contrast" or "reinforcement", showed an increase in their Percentage of Consonants Correct (PCC), in the number of acquired sounds and of generalizations in their phonological inventories. It was verified that both groups presented changes in their phonological inventories. On the comparative analysis between the groups, it was observed that both groups, treated by "contrast" and by "reinforcement", demonstrated differences regarding the types of generalizations studied.

TEMA: terapia fonológica em crianças com desvios fonológicos. OBJETIVO: comparar as mudanças referentes ao sistema fonológico de crianças com desvio fonológico, com base na comparação do número de fonemas adquiridos, número de sons estabelecidos no inventário fonético e traços distintivos alterados, antes e após a terapia, e verificar se houve diferença em relação a estas mudanças de acordo como o modelo de terapia utilizado - Ciclos Modificado, Oposições Máximas e ABAB - Retirada e Provas Múltiplas. MÉTODO: o grupo pesquisado foi constituído por 21 sujeitos, sendo 15 do sexo masculino e 6 do sexo feminino, com desvios fonológicos que já haviam recebido alta do atendimento fonoaudiológico. Foram comparadas a avaliação fonológica inicial e a avaliação após a terapia em relação ao número de sons estabelecidos nos sistemas fonológicos, o número de sons presentes nos inventários fonéticos e os traços distintivos alterados; também foram comparadas as mudanças fonológicas resultantes da aplicação dos três modelos terapêuticos. RESULTADOS: observou-se diferença estatisticamente significante entre as avaliações iniciais e finais nos três modelos quanto aos fonemas estabelecidos no sistema fonológico e traços distintivos alterados, enquanto que no inventário fonético, houve diferença estatística significante somente entre os modelos ABAB - Retirada e Provas Múltiplas e Oposições Máximas. Não houve diferença estatística entre os modelos terapêuticos. CONCLUSÃO: os modelos de terapia foram efetivos no tratamento das crianças com desvio fonológico, pois estas apresentaram evolução nos seus sistemas fonológicos, inventários fonéticos e traços distintivos alterados, não havendo diferença estatisticamente significante entre os modelos.

BACKGROUND: phonological therapy in children with phonological disorders. AIM: to verify changes in the phonological system of children with phonological disorders, based on the comparison of number of acquired phonemes, number of sounds in the phonetic inventory and altered distinctive features, before and after therapy, and to verify differences between therapy models: Modified Cycles, Maximal Oppositions and ABAB - Withdrawal and Multiple Probes. METHOD: the research group was composed of 21 subjects, 15 male and 6 female, with phonological disorders, who had already completed speech-language treatment. The initial and final phonological assessments were compared taking into consideration the number of phonemes in the phonological systems, the number of sounds in the phonetic inventory and the altered distinctive features. Phonological changes that resulted from the application of the three therapeutic models were also compared. RESULTS: statistically significant differences were observed between the initial and final assessments in all of the three therapeutic models considering the number of acquired phonemes and the altered distinctive features, whereas in the phonetic inventory a statistically significant difference was observed only between the ABAB - Withdrawal and Multiple Probes and the Maximal Oppositions model. No statistical difference was observed between the therapy models. CONCLUSION: the three therapy models were effective in the treatment of children with phonological disorders, once all of the children presented improvement of their phonological system, phonetic inventory and altered distinctive features. No significant statistical difference was found between the models.
