Resumo O relato descrebeu o primeiro curso presencial visando capacitar profissionais de saúde pública na realização de vigilância genômica em tempo real, durante períodos pandêmicos. Relato de experiência sobre um curso teórico-prático com foco em pesquisa e vigilância genômica, incluindo tecnologias de sequenciamento móvel, bioinformática, filogenética e modelagem epidemiológica. O evento contou com 162 participantes e foi o primeiro grande treinamento presencial realizado durante a epidemia de covid-19 no Brasil. Não foi detectada infecção pelo SARS-CoV-2 ao final do evento em nenhum participante, sugerindo a segurança e efetividade de todas as medidas de segurança adotadas. Os resultados do evento sugerem que é possível executar capacitação profissional com segurança durante pandemias, desde que seguidos todos os protocolos de segurança.

Abstract The objective of this report was to describe the first face-to-face course aimed at training public health professionals in performing real-time genomic surveillance during the pandemic period. Experience report on a theoretical-practical course focusing on genomic research and surveillance, including mobile sequencing technologies, bioinformatics, phylogenetics and epidemiological modeling. There were 162 participants in the event and it was the first major face-to-face training course conducted during the COVID-19 epidemic in Brazil. No cases of SARS-CoV-2 infection was detected among the participants at the end of the event, suggesting the safety and effectiveness of all safety measures adopted. The results of this experience suggest that it is possible to conduct professional training safely during pandemics, as long as all safety protocols are followed.

ABSTRACT BACKGROUND: Although bariatric surgery is highly effective for the treatment of obesity and its comorbidities, preoperative weight loss has an impact on its results. AIMS: The aim of this study was to correlate preoperative weight loss with the outcome of bariatric surgery using the Bariatric Analysis and Reporting Outcome System scores. METHODS: This is a cross-sectional, observational study with 43 patients undergoing bariatric surgery that compared a group of 25 patients with a percentage of preoperative excess weight loss ³8% with a group of 18 patients with a percentage of preoperative excess weight loss <8% or with weight gain. The research took place at the bariatric surgery outpatient clinic of the Oswaldo Cruz University Hospital with patients 1 year after the surgery. RESULTS: Patients had a mean age of 40.8 years (42.7 percentage of preoperative excess weight loss ≥8% vs. 38.2 percentage of preoperative excess weight loss <8%, p=0.095). No significant difference was found between the two groups regarding preoperative comorbidities and body mass index at entry into the program. Higher preoperative body mass index (48.69 vs. 44.0; p=0.029) was observed in the group with percentage of preoperative excess weight loss <8%. No significant difference was found regarding the percentage of excess weight loss (71.4±15.4%; percentage of preoperative excess weight loss ≥8% vs. 69.47%±14.5 percentage of preoperative excess weight loss <8%; p=0.671), the result of the surgery according to the Bariatric Analysis and Reporting Outcome System scores protocol, the resolution of comorbidities, the quality of life, and the surgical complications between the two groups. CONCLUSIONS: Based on the available data, it is reasonable that bariatric surgery should not be denied to people who have not achieved pre-established weight loss before surgery. RESUMO RACIONAL: Apesar da cirurgia bariátrica ser altamente eficaz para o tratamento da obesidade e suas comorbidades, ainda não está bem estabelecido o impacto da perda de peso pré-operatória em seus resultados. OBJETIVOS: Correlacionar a perda de peso pré-operatória com o resultado da cirurgia bariátrica pelos escores do método Bariatric Analysis and Reportig Outcome System. MÉTODOS: Estudo observacional transversal com 43 pacientes submetidos a cirurgia bariátrica que comparou um grupo de 25 pacientes com percentual de perda do excesso de peso pré-operatória igual ou maior a 8% com um grupo de 18 pacientes com percentual de perda do excesso de peso pré-operatória menor a 8% ou com ganho de peso. A pesquisa ocorreu no ambulatório de Cirurgia Bariátrica do Hospital Universitário Oswaldo Cruz com pacientes após um ano da cirurgia. RESULTADOS: Os pacientes tinham uma média de idade de 40,8 anos (42,7 percentual de perda do excesso de peso pré-operatória ≥8% vs 38,2 percentual de perda do excesso de peso pré-operatória <8%, p=0.095). Não foram encontradas diferenças significativas entre os dois grupos em relação às comorbidades pré-operatórias e o IMC na entrada do programa. Abstract The Serra Branca pegmatite in northeastern Brazil is unique because of its megacrysts of bluish-green amazonite currently exploited for ornamental purposes and as high-quality gemstones. Here we analyze microtextural, spectroscopic, and geochemical data on specimens from two magmatic generations of amazonite from this pegmatite in order to characterize their mineralogy and propose a schematic evolution diagram for twinning during their development. The amazonite consists of intergrowths of the low microcline and low albite XRD varieties that evolved according to a twin coarsening process, in which the first generation (I-tg) of ± A/P twinning developed at the monoclinic-triclinic transformation, whereas the second generation (II-tg) gave rise mainly to ± A twins. The bluish-green color of the amazonite was preserved during the process of microcline twinning. In addition to the irrational II-tg twinning, which is typically found in microcline from anorogenic settings, the serra branca amazonite crystals also show elevated contents of Rb, Pb, Fe, Cs, and Tl in both generations, whereas albitic intergrowths present high concentrations of Fe, Pb, Sr, and Ga. Finally, our chemical and spectroscopic results are compatible with the hypothesis that Pb-water centers account for the bluish green color in amazonite.

The article investigates the solvency and sustainability of the investment policy carried out by subnational governments in Brazil between 2008 and 2016, limited by changes in public accounting that avoid the comparison of some items of the public revenue when including 2017. Regarding the methodology for assessing the payment capacity of the subnational governments in Brazil (CAPAG), an empirical analysis is conducted, adapting Bohn's proposal (1999 and 2007). The results in four specifications of fiscal reaction functions attest to the unsustainability of the fiscal policy of the states, an innocuous fiscal policy of the main cities when using the committed expenditures, but evidence of a solvent behavior in using the values settled by these administrations. Finally, for these two levels of government, the virtuous cycle of investments is verified, which confirm feasible increases in revenues that overlap the respective increases due to investments’ costing.

Resumo Este estudo centra-se na análise das condições sobre as quais trabalhadores brasileiros têm sido identificados como sobreeducados em seus primeiros empregos, bem como na probabilidade de permanecerem com esse status nos anos de 2006 e 2015. Busca-se também investigar os diferenciais de efeitos entre trabalhadores inicialmente sobreeducados, mas que foram primeiramente engajados em empresas com capacidade produtivas diferenciadas. São considerados sobreeducados os trabalhadores que desempenham funções que exigem abaixo de seu nível de escolaridade. Informações provenientes da Relação Anual de Informações Sociais (RAIS) e da Classificação Brasileira das Ocupações (CBO) de 2002 alimentaram um modelo probit bivariado recursivo que permitiu a realização empírica do estudo. Os resultados mostram que o status de sobreeducação no primeiro emprego em 2006 aumenta em 43,6% a probabilidade de o trabalhador repetir a mesma incompatibilidade no emprego em 2015. Além disso, trabalhadores sobreeducados aceleram suas transições para empregos condizentes ao engajarem o primeiro emprego em empresas de grande porte.

Abstract This study focuses on analyzing the conditions under which Brazilian workers have been identified as overeducated in their first job, as well as the probability of remaining with this status (Overeducated) in the years 2006 and 2015. It also seeks to investigate the differentials in effects between workers who are initially overeducated but who were first engaged in firms with differentiated productive capacity. Overeducated workers are those who perform jobs that require less than their level of education. Information from the Annual Social Information Report (RAIS) and the 2002 Brazilian Classification of Occupations (CBO) fed a recursive bivariate probit model that allowed the empirical conduct of the study. The results show that overeducated status in the first job in 2006 increases by 43.6% the probability that the worker repeats the same mismatch in the job in 2015. Moreover, overeducated workers accelerate their transitions to matching jobs by engaging their first job in large firms.

RESUMO A existência de milhões de jovens que nem estudam nem trabalham, os denominados nem-nem, tem implicações danosas para a sociedade e para a economia. Este estudo foca nos principais custos econômicos associados à ociosidade desses jovens por meio do Propensity Score Macthing. Os resultados revelam que os jovens nem-nem representaram um custo de 0,6% do PIB em 2015. Considerando esse significante custo anual, e a trajetória crescente desse contingente juvenil nos últimos anos, há que se clamar pela implementação de políticas que contenham esse crescimento, a fim de se evitar severos prejuízos de ordem econômica e social no longo prazo.

ABSTRACT The existence of millions of youths remain in the NEET status brings severe implications for the society and for the economy. Thus, this paper focuses on the main costs associated with the neet youth idleness through the Propensity Score Matching. The results reveal that the neet youths accounted for a cost of 0.6% of the GDP in 2015. Considering this significant annual cost and the increasing amount of neet youths in the last years, there is to be claimed for policies that hinder such a growing path in order to avoid severe economic and social losses in the long run.

Abstract The Vieirópolis pegmatite field is characterized by amazonite pegmatites inserted in the Borborema Province. The Serra Branca pegmatite is hosted by the Serra Branca granites, which intruded partially the Serra Negra granites, and stands out by its amazonite mineralization. To comprehend the crystallization conditions and the source of this pegmatite and associated Serra Branca and Serra Negra granites, petrographic, mineral chemistry, geochronological and whole-rock geochemical data were used. The Serra Branca pegmatite is Cs-, Rb-, Pb-, Nb-, and Ta-rich. The Serra Branca granites (563 ± 2 Ma) are metaluminous to weakly peraluminous, crystallized under high fO2 conditions, pressure of 2.68-2.80 kbar and temperatures ranging from 667-670°C, with many zircon grains showing inherited cores of Paleoproterozoic ages. The Serra Negra granites (594 ± 4 Ma) comprises metaluminous granites crystallized under high fO2 conditions, temperatures within 711-751°C, pressure varying from 4.72 to 5.42 kbar. Both granites resulted from distinct partial melting degrees of a source similar to the Caicó Complex orthogneisses. The Serra Branca pegmatite geochemical data suggests that it was originated by extreme fractionation of the Serra Branca granites magma. The Serra Branca amazonite pegmatite is the first pegmatite mineralogically and geochemically characterized as Nb-Y-F (NYF)-type from the gadolinite subtype in the Borborema Province.

RESUMO A reciclagem da fração mineral de resíduos da construção civil é vista como uma potencial fonte de geração de agregados. Os Agregados de Resíduo de Concreto (ARC) são geralmente indicados para se utilizar na produção de novos concretos. No entanto, devido à argamassa antiga aderida aos ARC, estes possuem massa específica menor e maior porosidade quando comparado ao agregado natural. Na busca de melhorar a qualidade dos ARC, várias técnicas vêm sendo desenvolvidas, sendo uma das alternativas através da modificação da superfície. Este estudo objetivou verificar o efeito da substituição de 20 % de agregado graúdo natural por ARC e realizar a modificação da superfície dos ARC através da abordagem de mistura dois estágios. A substituição dos agregados foi realizada mantendo a distribuição granulométrica do agregado graúdo natural. O tratamento do ARC foi realizado no período de pré-molhagem com os teores de 25 e 100 % de cimento Port-land. Foram realizados ensaios para avaliar o comportamento dos concretos em relação às propriedades mecânicas e parâmetros de durabilidade. Ficou evidente que a utilização de 20 % de ARC na produção de um novo concreto produz efeitos negativos nas propriedades avaliadas quando comparado ao concreto conven-cional. O tratamento nos ARC através da abordagem de mistura dois estágios com 25 % de cimento Portland proporcionou melhoria no concreto quando comparado à abordagem de mistura normal. Por outro lado, a abordagem de mistura dois estágios com 100 % de cimento Portland prejudicou o comportamento do concreto. Foi concluído que a abordagem de mistura dois estágios com 25 % de cimento Portland mostrou-se como uma alternativa viável para proporcionar melhorias nos ARC. Além de resultar em um concreto com boas características de propriedades mecânicas e durabilidade, também viabiliza a utilização de agregados reciclados e não gera custo adicional no processo de produção do concreto.

ABSTRACT The recycling of the mineral fraction of civil construction residues is seen as a source potential of generation of aggregates. The Recycled Concrete Aggregates (RCA) are generally indicated for using in the production of new concretes. However, due to the old mortar adhered to RCA, these have smaller specific mass and higher porosity when compared to the natural aggregate. In the seek of improving the quality of RCA, various techniques have being developed, being one of the alternatives through of surface modification. This study objectified to verify the effect of the substitution of 20% of natural coarse aggregate by RCA and to perform the surface modification of the RCA through the two stage mixing approach. The replacement of aggregates was performed maintaining the particle size distribution of the natural coarse aggregate. The RCA treatment was performed in the pre-wetting period with contents of 25 and 100% of Portland cement. It were performed tests to evaluate the behavior of concretes in relation to mechanical properties and durability parameters. It was evident that the utilization of 20% of RCA in the production of a new concrete produces negative effects on the evaluated properties when compared to conventional concrete. The treatment in the RCA though the two stage mixing approach with 25% of Portland cement provided improvement in the concrete when compared to the normal mixing approach. On the other hand, the two stage mixing approach with 100% of Portland cement impaired the concrete behavior. It was concluded that the two stage mixing approach with 25% of Portland cement showed as a viable alternative to provide improvements in the RCA. Besides resulting in a concrete with good characteristics of mechanical properties and durability, it also enables the utilization of recycled aggregates and it does not generates additional cost in the process of concrete production.

Abstract Since the emergence of the chikungunya virus in Brazil in 2014, more than 700,000 cases have been reported throughout the country, corresponding to one-third of all cases reported in the Americas. In addition to its high attack rates, resulting in hundreds of thousands of cases, the disease has high chronicity rates with persistent joint manifestations for more than 3 months, which can spread to more than half of the patients affected in the acute phase. Pain associated with musculoskeletal manifestations, often disabling, has an effect on patients’ quality of life at different stages of the disease. Currently, the challenge faced by specialists is identifying the best therapy to be instituted for symptom relief despite the limited number of published intervention studies. In 2016, a multidisciplinary group published pharmacological treatment protocols for pain in patients with chikungunya, which was incorporated into the guidelines for clinical management of the Brazilian Ministry of Health in 2017; in that same year, a consensus was published by the Brazilian Society of Rheumatology about diagnosis and treatment. After 5 years of experience with chikungunya epidemics, in 2019, specialists involved in the protocols of the Brazilian Society of Rheumatology and Brazilian Ministry of Health prepared an update with the main objective of developing flowcharts for the therapeutic approach of musculoskeletal manifestations in adult patients to enable specialists at different levels of healthcare to spread and apply this guideline in a systematic and simplified manner.

Abstract INTRODUCTION: Since 2016, Brazil has been in the midst of its largest sylvatic yellow fever epidemic ever, found predominantly outside the Amazon region. Cases originating from Brazil have been reported in France, the Netherlands, Romania, Switzerland, Argentina, and Chile. The epidemic began in the Central-West region of Brazil in 2014, spreading into the Southern region, with significant non-human primate transmission continuing towards Paraguay and Argentina. METHODS: This report is an integrative review of Pan American Health Organization cooperation during a sylvatic yellow fever epidemic. RESULTS: The Pan American Health Organization has played a central role in handling the yellow fever emergency, collaborating with the Ministry of Health and various research groups in supporting interventions of different response areas. The Pan American Health Organization's technical cooperation included: training and workshops to exchange experiences, carrying out technical cooperation in patient management and epidemiological, entomological, laboratory, and epizootic surveillance, organizing the assistance network, and acquiring strategic inputs. The Pan American Health Organization’s technical cooperation supported the Ministry of Health’s decision to adopt a single-dose vaccine and use fractional doses to support the vaccination needs of more than 39,000,000 people. The coronavirus disease 2019 pandemic contributed to the failure of reaching the yellow fever vaccination goals and made it difficult to integrate the yellow fever vaccine into recommended areas. CONCLUSIONS: Given the ongoing coronavirus disease 2019 pandemic, it is necessary to strengthen measures for the surveillance, prevention, and control of yellow fever with multilateral cooperation between countries.

BACKGROUND There is little information on the effect of using deltamethrin-impregnated dog collars for the control of canine visceral leishmaniasis. OBJECTIVES The objective of this study was to evaluate the effectiveness of the use of 4% deltamethrin-impregnated collars (Scalibor®) in controlling visceral leishmaniasis in Lutzomyia longipalpis by comparing populations in intervention and non-intervention areas. METHODS Phlebotomine flies were captured over 30 months in four neighbourhoods with intense visceral leishmaniasis transmission in Fortaleza and Montes Claros. We calculated the rates of domicile infestation, relative abundance of Lu. longipalpis, and Lu. longipalpis distribution in each site, capture location (intra- and peridomestic locations) and area (intervention and non-intervention areas). FINDINGS In the control area in Fortaleza, the relative abundance of Lu. longipalpis was 415 specimens at each capture site, whereas in the intervention area it was 159.25; in Montes Claros, the relative abundance was 5,660 specimens per capture site in the control area, whereas in the intervention area it was 2,499.4. The use of dog collars was associated with a reduction in captured insects of 15% (p = 0.004) and 60% (p < 0.001) in Montes Claros and Fortaleza, respectively. MAIN CONCLUSIONS We observed a lower vector abundance in the intervention areas, suggesting an effect of the insecticide-impregnated collars.

Introdução: A presença de estenose aterosclerótica sintomática da artéria inominada é uma patologia pouco frequente. Os autores apresentam a utilização da técnica kissing stent nas artérias subclávia e carótida comum direitas com stents cobertos com proteção cerebral por clampagem direta da carótida comum, como alternativa endovascular “híbrida” para o tratamento da estenose sintomática da artéria inominada. Caso Clínico: Doente de 75 anos, sexo masculino, admitido no Serviço de Cirurgia Vascular por acidentes isquémicos transitórios (AIT's) de repetição do hemisfério direito, resultando em hemiparesia esquerda transitória. A angiografia por tomografia computorizada (angioTC) excluiu lesões carotídeas significativas e revelou estenose da artéria inominada associada a trombo mural com extensão para a bifurcação deste vaso. Realizou-se exposição cirúrgica da artéria carótida comum e axilar direitas. A proteção cerebral efetuou-se por clampagem direta da carótida comum e obteve-se acesso endovascular retrógrado carotídeo e axilar. Foram colocados dois stents cobertos expansíveis por balão nas artérias inominada, subclávia e carótida comum direitas, utilizando a técnica kissing stent. No final do procedimento verificou-se a boa permeabilidade dos eixos revascularizados e foi realizada expurga direta de eventual material embólico antes da desclampagem. O pós-operatório decorreu sem complicações. Conclusão: A técnica kissing stent, com stents cobertos, nas artérias subclávia, carótida comum e inominada com proteção direta da carótida comum por clampagem é uma solução possível e minimamente invasiva da estenose sintomática da artéria inominada.

Introduction: The presence of symptomatic atherosclerotic stenosis of the innominate artery is an uncommon condition. The authors report the use of kissing stent technique in right subclavian and common carotid arteries with covered stents and cerebral protection by direct clamping of common carotid artery, as a hybrid endovascular alternative to treat symptomatic stenosis of the innominate artery. Case Report: A 75-year-old male was admitted to Vascular Surgery Department with repeated transient ischemic attacks (TIA's) of the right cerebral hemisphere, resulting in transitory left hemiparesis. Computerized tomography angiography (CTA) excluded significant carotid lesions and revealed stenosis of the innominate artery associated with mural thrombus that extended to the bifurcation. The right common carotid artery and axillary artery were surgically exposed. Brain protection was achieved by direct clamping of the common carotid artery and subsequently carotid and axillary retrograde endovascular access was obtained. Two balloon-expandable covered stents were placed in the innominate, subclavian and right common carotid arteries, using the kissing stent technique. At the end of the procedure, a good permeability of the revascularized arteries was verified and a direct expulsion of eventual embolic material was performed prior to declamping. The postoperative period was uneventful. Conclusion: The kissing stent technique, with covered stents, in innominate, right subclavian and common carotid arteries with direct protection of the common carotid by clamping is a possible and minimally invasive solution for the treatment of symptomatic stenosis of the innominate artery.

Introdução: Até 30% dos doentes submetidos a EVAR electivamente têm Doença Renal Crónica (DRC). A nefropatia induzida por contraste é uma complicação do EVAR. É possível realizar EVAR sem contraste ou minimizar a quantidade ultilizada, recorrendo à angiografia com CO2 e/ou IVUS (Intravascular ultrasound), em vez da angiografia convencional. Contudo, estes meios não estão disponíveis na maioria dos Hospitais, ao contrário do eco-Doppler que, em doentes com biótipo favorável e realizado por um profissional experiente, permite excluir endoleaks e confirmar a permeabilidade das artérias renais e hipogástricas. Objectivos: mostrar a exequibilidade do planeamento, EVAR e follow-up sem contraste, através da apresentação da técnica utilizada num caso clínico. Material e Métodos/Resultados: Doente do sexo masculino de 70 anos, com DRC por nefropatia por IgA, foi admitido no nosso hospital por AAA assintomático com 57 mm de maior diâmetro, após achado num TC sem contraste. Como o doente apresentava factores de risco para nefropatia induzida por contraste, uma anatomia favorável para EVAR e um bom biótipo para eco-Doppler abdominal, foi proposto para EVAR guiado por eco-Doppler e fluoroscopia, sem utilizaç- ção de contraste. O planeamento foi realizado por reconstrução no centre-lumen line e as medições realizadas foram outer-to-outer no TC sem contraste. O trombo no colo proximal, o diâmetro da bifurcação, bem como a permeabilidade das renais e ilíacas foram avaliados por eco-Doppler. Intraoperatoriamente, procedeu-se à cateterização da artéria renal mais distal, confirmada por eco-Doppler, ficando um fio guia hidrofílico como referência. O corpo principal da endoprótese (Gore Excluder®) foi introduzido e libertado em posição infrarenal, tendo como referencia o guia na renal, sob monitorização por eco-Doppler. De seguida, procedeu-se à cateterização de ambas as hipogástricas e um guia hidrofílico foi deixado como referência para a extensão bilateral. Um eco-Doppler final exclui a existência de endoleak tipo 1 e 3 e confirmou a permeabilidade das artérias renais e hipogástricas. Antes da alta, o doente foi submetido a eco-Doppler e TC que demonstraram a posição infrarenal adequada da endopróte- se e a permeabilidade das renais e hipogástricas. Não houve alteração da função renal no internamento. Conclusões: O planeamento, o EVAR e o Follow-up são exequíveis sem recorrer ao contraste. Esta técnica justifica-se em casos com elevado risco para nefropatia induzida por contraste e doentes com um biótipo favorável para ecografia abdominal e uma anatomia favorável para EVAR.

Introduction: Up to 30% of patients undergoing elective EVAR have prior Chronic Kidney Disease (CKD). The contrast-in- duced nephropathy is a complication of EVAR. It is possible to perform EVAR without contrast or minimize its amount during the procedure, using intraoperative angiography with CO2 and/or IVUS instead of conventional angiography. However, these techniques are not available in most hospitals, unlike Dupplex ultrasound (DUS). In patients with favorable biotype and performed by an experienced professional, DUS allows the detection of endoleaks and confirms patency of the renal and hypogastric arteries during EVAR. Objectives: show the feasibility of planning, execution and follow-up of EVAR without contrast through the presenta- tion of the technique used in a clinical case. Case Report: A 70-year-old man with a history of CKD due to IgA nephropathy, presented in our hospital with a 57mm asymptomatic infrarenal aortic aneurysm diagnosed on a non-contrast CT. As the patient had risk factors for contrast induced nephropathy, a favorable anatomy and a good biotype for abdominal DUS, he was proposed for EVAR. Planning was performed using centre-lumen line reconstruction and outer-to-outer measurements on non-contrast CT. Proxi- mal neck thrombus, lumen diameter and iliac patency were evaluated by DUS. During surgery the left (lowermost) renal artery was cannulated under fluoroscopy and confirmed by DUS, and a hydrophilic guidewire was left as reference. The main body (Gore Excluder®) was advanced and subsequently deployed under DUS monitoring. Afterwards, both hypogas- tric arteries were cannulated and used as reference for bilateral extension. A final DUS excluded type 1 or 3 endoleaks. Before discharge he underwent DUS and non-contrast CT which demonstrated the adequate positioning of the endo- prosthesis just below the lowermost renal artery, and renal and hypogastric artery permeability. There was no change in renal function during hospitalization. Conclusion: The planning, execution and and follow-up of EVAR are feasible without contrast. This technique is justified in cases with high risk for contrast-induced nephropathy in patients with a favorable biotype for abdominal ultrasound and a favorable anatomy for EVAR.
