ABSTRACT Objective: To assess the trend in motorcyclist mortality rate from traffic accidents in the state of São Paulo, Brazil, from 2015 to 2020. Methods: This is an ecological time series study with secondary data from the Traffic Accident Management Information System of the State of São Paulo (INFOSIGA), referring to motorcyclists’ deaths due to road traffic injuries in the state of São Paulo, Brazil, from 2015 to 2020. The Annual Percent Change was calculated according to the Prais-Winsten regression model, using the Stata 14.0 software. Results: A total of 11,343 deaths of motorcyclists due to road traffic injuries were reported. The highest proportion of deaths occurred among men (88.1%), aged between 18 and 24 years (27.9%), in the two most populous and urbanized regions of the state. The distribution of mortality showed minimal variation in the analyzed period, from 4.22 to 4.42 deaths/100 thousand inhabitants. Among the analyzed sociodemographic variables, the mortality trend of motorcyclists was mostly stationary. Conclusion: The analysis of the mortality of motorcyclists due to road traffic injuries in the state of São Paulo showed a stationary trend.

RESUMO OBJETIVO Analisar o nível de conhecimento e aceitabilidade da vacina contra o HPV entre adolescentes vacinados e não vacinados na Amazônia Ocidental. MÉTODOS Estudo transversal com adolescentes de 10 a 19 anos. O instrumento usado para coletar dados contém informações demográficas e socioeconômicas e 27 perguntas que avaliam o conhecimento e a aceitabilidade da vacina contra o HPV. Para comparar a prevalência de adolescentes vacinados com as respostas corretas para perguntas sobre conhecimento, aceitabilidade e vacinação contra o HPV, o modelo de regressão de Poisson de variância robusta foi utilizado no software Stata 13.0. Resultados: Participaram do estudo 190 adolescentes, 60,5% (n=115) na faixa etária de vacinação recomendada pelo governo brasileiro; dentre eles, 53,9% (n=62) relataram não ter sido vacinados (p<0,001). A proporção de resultados corretos entre os adolescentes foi de 78,9% (n=150), que reconheceram o HPV como vírus; 63,7% (n=121) relataram o HPV como causa de câncer do colo do útero. Os participantes que sabem que o HPV é um fator causal no câncer do colo do útero têm 1,94 (IC 1,18-3,18; p=0,009) vez mais chances de terem sido vacinados do que aqueles que não sabem. Entre os entrevistados, as principais fontes de conhecimento sobre a vacina foram escolas (51,6%) e profissionais de saúde (22,6%). CONCLUSÃO Adolescentes não vacinados apresentam lacunas de conhecimento sobre o HPV e sua vacina quando comparados aos vacinados. Nossos resultados enfatizam a necessidade de campanhas eficazes para fornecer informações adequadas sobre o HPV e sua vacina a adolescentes, pais e profissionais de saúde.

SUMMARY PURPOSE To analyze the level of knowledge about and the acceptability of the HPV vaccine among vaccinated and unvaccinated adolescents in the Western Amazon. METHODS A cross-sectional study on adolescents aged 10 to 19 years. The instrument used to collect data contains demographic and socioeconomic information and 27 questions that assess the knowledge and acceptability of the HPV vaccine. To compare the prevalence of vaccinated adolescents with the correct answers to questions about HPV and acceptability and vaccination, the robust variance Poisson regression model was used in the Stata 13.0 software. FINDINGS A total of 190 adolescents participated in the study, 60.5% in the age group of vaccination recommended by the Brazilian government, among them, 53.9% reported not having been vaccinated (p <0.001). A total of 150 (78.9%) adolescents correctly recognized HPV as a virus; 121 (63.7%) recognized HPV as a cause of cervical cancer. Participants who know HPV is a causative factor for cervical cancer are 1.94 times more likely to have been vaccinated than those who do not. Among the interviewees, the main sources of knowledge about the vaccine were schools (51.6%) and health professionals (22.6%). CONCLUSION Unvaccinated adolescents have knowledge gaps about HPV and its vaccine when compared to those vaccinated. Our results emphasize the need for effective campaigns to deliver adequate information about HPV and its vaccine to adolescents, their parents, and health professionals.

RESUMO INTRODUÇÃO A associação entre diagnósticos ginecológicos e sua distribuição nos setores de saúde proporciona benefícios no campo da promoção de saúde e na identificação de temas para educação continuada na assistência. OBJETIVO Identificar os diagnósticos em saúde e o fluxo de encaminhamento de mulheres no climatério. MÉTODO Trata-se de estudo transversal realizado no Ambulatório de Saúde da Mulher do Hospital Universitário da Universidade de São Paulo, de referência em ginecologia e de treinamento para residentes de Medicina de Família e Comunidade, entre 2017-2018. A casuística foi realizada a partir de 274 prontuários de mulheres atendidas e foram processados informações sociodemográficas e clínicas, diagnósticos ginecológicos e distribuição dos serviços de saúde (primário, secundário e terciário). O teste qui-quadrado e razão de chance foram utilizados para estatística. RESULTADOS O tabagismo (OR=2,27, IC95% 1,05;4,89, p=0,035) foi associado ao encaminhamento de mulheres no climatério para a maior complexidade. Em relação aos tipos de diagnóstico, a chance de serem encaminhadas para a média e alta complexidade foi de 135% (OR=1,69, IC95% 0,93;3,08) nos transtornos não inflamatórios do trato genital feminino, 200% (OR=0,98, IC95% 0,23;4,02) nas doenças da mama, 300% (OR=1,51, IC95% 0,47;4,83) nos transtornos inflamatórios do trato genital feminino, sem predomínio entre os diagnósticos. CONCLUSÃO As mulheres climatéricas e na pós-menopausa acima de 50 anos e tabagistas com diagnósticos de transtornos não inflamatórios do trato genital feminino e inflamatórios, bem como doenças da mama, foram as mais direcionadas para ambulatório cirúrgico na média e alta complexidade.

SUMMARY OBJECTIVE The association between gynecological diagnoses and their distribution across healthcare sectors benefits health promotion and the identification of topics for continued education of gynecological care. This study aimed to identify healthcare diagnoses and referral flow in climacteric women. METHODS This is a cross-sectional study conducted at the Women’s Health Clinic of the University Hospital, University of São Paulo, with a reference to gynecology and training for Residents of Family and Community Medicine, between 2017 and 2018. The medical records of 242 women whose sociodemographic and clinical information, gynecological diagnoses, and distribution of healthcare services (primary, secondary, and tertiary) had been processed were collected. Statistical analysis included the chi-square test and odds ratio. RESULTS Smoking (OR = 2.27, 95% CI 1.05–4.89; p = 0.035) was associated with the referral of climacteric women to higher complexity services. Considering the distribution of non-oncological diagnoses in climacteric patients, the chance of women being referred to medium- and high-complexity health services presented a 2-fold increase in cases of breast diseases, a 2.35-fold increase in cases of noninflammatory disorders of the female genital tract, and a 3-fold increase in cases of inflammatory diseases of the pelvic organs. CONCLUSION Climacteric women aged over 55 years, postmenopausal women, and smoking women were most frequently referred to medium- and high-complexity outpatient surgery.

Abstract Background: The aim of this study was to evaluate the QoL of patients undergoing heart valve replacement using the SF-36 questionnaire, compare it between patients with mechanical prosthesis and patients with bioprosthesis, and correlate the results with sociodemographic variables. Objective: To assess the QoL of patients undergoing heart valve replacement and compare it between patients with bioprosthetic valves and patients with mechanical prosthetic valves. Methods: We included 36 consecutive patients (16 men) with a mean age of 51 years and six months, who underwent mitral or aortic valve replacement from September 2007 to December 2011. The study was conducted between March and May 2012 and involved the application of the SF-36 survey and a sociodemographic questionnaire. Statistical tests were performed, and data are expressed as absolute frequency and percentile, and median and interquartile range (P25 and P75) (Mann-Whitney test), considering a significance of 95%. Results: The average time of surgery was 32.5 months (8-61 months). Participants were asked about the practice of physical activity, and 41.7% were physically active. For the SF-36 domains, the highest scores were observed for the social domain whereas the lowest scores were found for mental health, with a mean of 89.25 and 54.44, respectively. In the statistical analysis, we found statistically higher values in emotional functional for patients with mechanical valve prosthesis (p = 0.0084). Conclusion: The QoL of the patients undergoing heart valve replacement improves considerably after the surgery, except for the mental health domain, probably due to the low practice of physical activity. The type of prosthesis seems not to influence the QoL or the patients in the late postoperative period.

RESUMO Objetivo: Analisar a associação da anemia com a vulnerabilidade socioeconômica de pré-escolares de creches públicas da cidade de Taubaté, SP, Brasil. Métodos: Trata-se de um estudo transversal com amostra probabilística que analisou 363 crianças assistidas em creches públicas de regiões de baixa e alta renda de Taubaté. A concentração de hemoglobina (Hb), variável dependente, foi obtida por meio de punção digital, considerando anêmicas as crianças com concentração de Hb<11,0 g/dL. As variáveis independentes como condições socioeconômicas e demográficas foram coletadas por questionário semiestruturado. Resultados: A prevalência de anemia por deficiência de ferro foi de 19,3% entre os pré-escolares. As crianças das creches de elevada vulnerabilidade socioeconômica apresentaram concentração de Hb estatisticamente menor do que aquelas de região não vulnerável (p<0,05). Conclusões: A menor concentração de Hb em pré-escolares está associada à localização das creches em região de vulnerabilidade socioeconômica, uma vez que as crianças que frequentam tais creches enfrentam condições adversas familiares, como baixa renda, mães que trabalham e com baixa escolaridade, embora recebam benefício social e acompanhamento pelos serviços públicos de saúde.

ABSTRACT Objective: To analyze the association of anemia with the socioeconomic vulnerability of preschoolers in public day care centers, in the city of Taubaté, SP, Brazil. Methods: This is a cross-sectional study with a probabilistic sample that analyzed 363 children assisted in public day care centres in low-income and high-income areas of Taubaté, SP, Brazil. The hemoglobin concentration (Hb), dependent variable, was obtained by digital puncture, considering anemic children with Hb concentration <11.0 g/dL. The independent variables such as socioeconomic and demographic conditions were collected by a semi-structured questionnaire. Results: The prevalence of iron deficiency anemia was 19.3% among preschoolers. Children from day care centers with high socioeconomic vulnerability had lower Hb concentration than those from a non-vulnerable area (p<0.05). Conclusions: The lower concentration of hemoglobin in preschoolers is associated with the location of day care centers in an area of socioeconomic vulnerability. Children attending these day care centers face adverse family conditions such as low income, working mothers, and mothers with low schooling, though they receive social benefits and monitoring by public health services.

Abstract We present a case of a female adolescent with severe acute Chagas myocarditis, acquired by oral transmission in an endemic area in the Brazilian western Amazon, who had electrocardiographic changes normalized after empirical treatment with the antiparasitic drug benznidazole combined with conventional treatment for severe heart failure.

OBJECTIVE: To evaluate the knowledge and acceptability of the human papillomavirus (HPV) vaccine among health professionals from western Amazonia. METHODS: A cross-sectional study was conducted in the Sistema Assistencial è Saúde da Mulher e da Criança (Health Care System for Women and Children; SASMC) in Acre, Brazil, in 2017. The participants comprised 196 health professionals. The data collection instrument contained 31 questions about HPV, its clinical repercussions for women, and the HPV vaccine. Quantitative variables were presented as medians and 95% confidence intervals (CIs), and p<0.05 was considered statistically significant. For the analyses, chi-square tests and Mann-Whitney tests were used. The collected data were analyzed using Stata®11.0. RESULTS: Of the 196 health professionals, 39.8% (n=76) were physicians and 61.2% (n=120) were other health professionals. The interviewees were mostly female (n=143, 73%, 95% CI 66.1 to 78.9%) who worked in the medical field (n=81, 41.3%, 95% CI 34.4 to 48.6%), and the median age was 38 years (95% CI 36.0 to 39.7). Physicians had increased knowledge regarding only the statement "cervical cancer is one of the main causes of cancer in women", with a proportion ratio of 0.88 (0.80; 0.97) and p<0.001. Regarding clinical knowledge of the HPV vaccine, a low proportion of correct answers was obtained for all the questions, and no significant differences were found between the groups. CONCLUSION: Acceptability and knowledge of HPV and its vaccine were similar among health professionals, with knowledge gaps in questions about the relation between smoking and cervical cancer and specific clinical knowledge.

OBJECTIVES: We aimed to evaluate the effects of cataract surgery on cardiac autonomic modulation. METHODS: A cross-sectional and analytical study was conducted at the Hospital Maria Braido in the city of São Caetano do Sul, São Paulo, between 2015 and 2016. We investigated 19 patients of both sexes who were all over 50 years old; all patients had a diagnosis of senile or bilateral cataracts and were recommended to undergo implantation of the intraocular lens. Heart rate variability (HRV) was evaluated before, during and after cataract surgery. RESULTS: There were no significant changes in the time and geometric domains of HRV before, during or after surgery. The high-frequency (HF) band in normalized units (nu) on the spectral analysis significantly increased (p=0.02, Cohen’s d=0.9, large effect size). However, the low-frequency (LF) band in nu significantly decreased during surgery (p=0.02, Cohen’s d=0.9, large effect size). CONCLUSION: Throughout the intraocular lens implantation cataract surgery, there was an increase in parasympathetic modulation and a decrease in the sympathetic component of the heart rate (HR). We propose that this result is attributable to the supine position of the patients during surgery and the trigeminal reflex.

OBJECTIVE: To evaluate the effect of normal saline lavage of the distal vas deferens ampulla in patients undergoing vasectomy on the time to achieve azoospermia. METHODS: A prospective randomized study of 60 men divided into two groups, group lavage (GL, n=30) in which distal vas deferens ampulla lavage was performed with 10 ml of normal saline during the vasectomy, and group without lavage (GWL, n=30) in which control patients received only a vasectomy. The patients provided sperm for semen analysis at the 5th, 10th, 15th, 20th and 25th ejaculations. RESULTS: Fifteen participants in GL and 16 in GWL, for a total of 31 patients, were excluded due to not completing the control spermiogram. The tests carried out at the five ejaculations showed immobile spermatozoa in 40 and 85.71%, 66.67 and 78.57%, 93.33 and 85.71%, 86.67 and 71.43%, and 93.33 and 85.71% of participants in GL and GWL, respectively. CONCLUSION: Vas deferens duct lavage with 10 ml of normal saline during vasectomy did not decrease the time required to achieve postoperative azoospermia.

New technologies designed to improve the communication of autistic children can also help to promote interaction processes and cognitive and social development. The aim of this study was to analyze the instruments used to improve the communication skills of children with autism spectrum disorder. We searched the PubMed and Web of Science databases using the descriptors “autism”, “Asperger”, “education”, “children” and “assistive technology” and selected articles that met the following inclusion criteria: (i) original research; (ii) written in English; (iii) based on participants with a primary diagnosis of autism spectrum disorder; and (iv) tested an instrument designed to promote communication in children with autism spectrum disorder. Our search retrieved 811 articles, of which 34 met the inclusion criteria. Data on 26 instruments were extracted, and the measurement properties of the instruments were combined with information about their outcomes and presentation. The most commonly used interventions were the Treatment and Education of Autistic and Related Communication Handicapped Children program and the Picture Exchange Communication System. The Treatment and Education of Autistic and Related Communication Handicapped Children program was shown to produce improvements in the communication skills, socialization and self-care skills of children with autism spectrum disorder. The Picture Exchange Communication System produced inconsistent results. The results of the identified studies confirm the significant importance of these instruments in improving the communicative process of autistic children.

INTRODUÇÃO: A pressão arterial de repouso demonstrou estar associada à mortalidade cardiovascular. O controle autonômico da frequência cardíaca durante a recuperação pós-exercício é estimado para detetar mudanças no sistema cardiovascular, porque tais mudanças podem levar a doenças cardiovasculares. Avaliamos o envolvimento da pressão arterial diastólica de repouso (PAD) na dinâmica da frequência cardíaca após o exercício em homens normalmente ativos fisicamente normotensos. MÉTODO: Avaliamos homens saudáveis e fisicamente ativos com idades compreendidas entre os 18 e os 22 anos divididos em dois grupos desiguais: PAD de relaxamento G1 entre 80 e 90 mmHg (n=11) e PAD <80mmHg (n=24). Os voluntários realizaram exercícios físicos em uma esteira com intensidade equivalente a 60% da Vmax. A recuperação da frequência cardíaca no primeiro (HRR1) e terceiro (HRR3) minuto após o exercício foi medida e a variação da frequência cardíaca (VFC) foi examinada no domínio do tempo e da frequência. Além disso, realizamos a análise quantitativa da trama de Poincaré. A VFC foi registrada nas seguintes fases: os períodos de 10 minutos antes do exercício, durante o exercício e os períodos de 60 minutos após o exercício. RESULTADOS: Não encontramos diferença significativa entre G1 e G2 em relação às alterações da VFC durante o exercício. O G2 apresentou uma recuperação tardia dos índices SDNN, RMSSD, RRTri, LF, HF, LF/HF, SD1 e SD2 durante a recuperação do exercício. HRR1 e HRR3 foi maior no G2. CONCLUSÃO: Homens normais e fisicamente ativos com PAD entre 80 e 90 mmHg apresentaram RHE mais rápida e recuperação mais acelerada do controle autonômico da freqüência cardíaca após o exercício aeróbio.

OBJECTIVE: Rest arterial pressure has been shown to be associated with cardiovascular mortality. Autonomic heart rate control during recovery from exercise is estimated to detect changes in cardiovascular system, which may lead to cardiovascular diseases. We assessed the involvement of rest diastolic arterial pressure (DAP) on heart rate dynamics after exercise in normotensive physically active men. METHOD: We evaluated healthy physically active men aged 18 to 22 years old divided into two unequal groups: G1- rest DAP between 80 and 90 mmHg (N=11) and G2- rest DAP < 80mmHg (N=24). Volunteers performed physical exercise on a treadmill with intensity equivalent to 60% of Vmax. Heart rate recovery in the first (HRR1) and third (HRR3) minute after exercise were measured and heart rate variability (HRV) was examined in the time and frequency domain. Additionally, we performed the quantitative analysis of the Poincaré plot. HRV was recorded in the following phases: the 10-minute period before exercise, during exercise and the 60 minute period after exercise. RESULTS: We found no significant difference between G1 and G2 concerning HRV changes during exercise. The G2 group exhibited a delayed recovery of SDNN, RMSSD, RRTri, LF, HF, LF/HF, SD1 and SD2 indices during recovery from exercise. HRR1 and HRR3 was greater in the G2 group. CONCLUSION: Normotensive physically active men with DAP between 80 and 90 mmHg presented faster heart rate recovery and an accelerated recovery of heart rate autonomic control after aerobic exercise.

INTRODUÇÃO: Exercícios com uma barra flexível têm sido usados na prática clínica para a reabilitação do membro superior. No entanto, os seus efeitos agudos sobre a regulação autonômica cardíaca não estão bem esclarecidas. OBJETIVO: Avaliar os efeitos agudos do exercício com barra flexível sobre o comportamento complexo da variabilidade da frequência cardíaca (VFC). MÉTODO: Foram investigadas 32 voluntárias saudáveis com idades entre 18 e 25 anos que realizaram uma sessão de exercício com barra flexível. A VFC foi analisada 10 minutos antes e 10 minutos imediatamente depois do exercício. RESULTADOS: Os exercícios com a barra flexível não alteraram significativamente os domínios de tempo e frequência de VFC. Análise não-linear da VFC através da Dimensão Fractal Higuchi mostra que a mesma não foi significativamente alterada durante a recuperação do exercício em relação ao controle pré-exercício. CONCLUSÃO: O exercício com a barra flexível não alterou profundamente o comportamento caótico da dinâmica da frequência cardíaca. Isto é vantajoso para a avaliação de níveis de tratamento de reabilitação necessário em tais pacientes; e sua suscetibilidade a doenças dinâmicas.

INTRODUCTION: Exercises with a flexible pole have been applied in clinical practice for upper limb rehabilitation. Nevertheless, its acute effects on cardiac autonomic regulation are unclear. OBJECTIVE: To evaluate the acute effects of exercise with flexible pole on complex behavior of heart rate variability (HRV). METHOD: We investigated 32 healthy female volunteers aged between 18 and 25 years who performed a session of exercise with a flexible pole. HRV was analyzed 10 minutes before and 10 minutes immediately after the exercise. RESULTS: Exercises with a flexible pole did not significantly change time and frequency domain indices of HRV. Non-linear analysis of HRV through the Higuchi Fractal Dimension was not significantly changed during recovery from exercise compared to the control reading at rest. CONCLUSION: Exercises with the flexible pole were unable to acutely change chaotic behavior of heart rate dynamics. This is advantageous for assessments of levels of rehabilitative treatment required in such patients; and their susceptibility to dynamical diseases.
