RESUMO Objetivo Verificar a adequação das oportunidades de ação no ambiente doméstico de crianças em risco de atraso no desenvolvimento e identificar fatores associados à sua frequência. Métodos Este estudo transversal incluiu 97 famílias que responderam ao Affordances in the Home Environment for Motor Development - Infant Scale (AHEMD-IS) para 3–18 meses (n=63) ou o Self Report (AHEMD-SR) para 18–42 meses (n=34). O teste de Mann-Whitney foi utilizado para identificar as diferenças entre as frequências de oportunidades de ação entre os grupos. A regressão linear múltipla foi utilizada para verificar a associação entre o sexo da criança, o estado civil, a escolaridade e nível socioeconômico da mãe, as idades da criança e da mãe, o número de residentes da casa, a renda per capita e os escores do AHEMD (α=0,05). Resultados A frequência das oportunidades de ação dos domicílios no AHEMD-IS variou de menos do que adequado a excelente; enquanto, no AHEMD-SR, a maior predominância foi de média frequência. A oferta de estímulos no AHEMD-IS foi significativamente maior. O maior nível socioeconômico e o número de moradores da casa foram associados a maiores oportunidades de ação. Conclusões Quanto maior o nível socioeconômico e o número de moradores, maiores são as oportunidades de ação nos domicílios das crianças em risco de atraso. Abstract Objective: The aim of this study was to verify the adequacy of affordances in the home environment of children at risk of developmental delay and to identify factors associated with their frequency. Methods: The cross-sectional study included 97 families who responded to the Affordances in the Home Environment for Motor Development — Infant Scale (AHEMD-IS) for 3–18 months (n=63), or AHEMD – Self-Report (AHEMD-SR) for 18–42 months (n=34). The Mann-Whitney U test was used to identify the differences between the frequencies of affordances between the groups. Conclusions: The higher the socioeconomic level and house residents’ number, the greater the affordances in the homes of children at risk of delay. It is necessary to provide families with some alternatives to make their home environments richer in affordances that favor child development. RESUMO Objetivo: Analisar e identificar infecções documentadas e possíveis fatores de risco para infecções por Clostridioides difficile em crianças com câncer. Métodos: Estudo retrospectivo caso-controle em um hospital pediátrico oncológico, que abrangeu os anos de 2016–2019. O pareamento foi realizado por idade e doença de base e, para cada caso, o número de controles variou de um a três. Modelos de regressão logística foram utilizados para avaliar os fatores de risco. Resultados: Analisamos 63 casos de infecção documentados por C. difficile e 125 controles. A diarreia esteve presente em todos os casos, acompanhada de febre acima de 38°C em 52,4% dos pacientes. A mortalidade foi semelhante entre casos (n=4, 6,3%) e controles (n=6, 4,8%; p=0,7). No grupo caso, 71% dos pacientes e, no grupo controle, 53% deles receberam antibióticos de amplo espectro antes da infecção. Para uso prévio de vancomicina, a Odds Ratio para infecção por C. difficile foi de 5,4 (intervalo de confiança [IC95%] 2,3–12,5); para meropenem, 4,41 (IC95% 2,1–9,2) e, para cefepima, 2,6 (IC95% 1,3–5,1). Para os agentes antineoplásicos, a razão de chances para carboplatina foi de 2,7 (IC95% 1,2–6,2), para melfalano de 9,04 (IC95% 1,9-42,3), para bussulfano de 16,7 (IC95% 2,1–134,9) e, para asparaginase, de 8,97 (IC95% 1,9–42,9). Abstract Objective: The aim of this study was to analyze and identify documented infections and possible risk factors for Clostridioides difficile infections in children with cancer. Methods: This is a retrospective case-control study, carried out in a pediatric cancer hospital, covering the years 2016–2019. Matching was performed by age and underlying disease, and for each case, the number of controls varied from 1 to 3. Logistic regression models were used to assess risk factors. Results: We analyzed 63 cases of documented infection by C. difficile and 125 controls. Diarrhea was present in all cases, accompanied by fever higher than 38°C in 52.4% of the patients. Mortality was similar among cases (n=4; 6.3%) and controls (n=6; 4.8%; p=0.7). In all, 71% of patients in the case group and 53% in the control group received broad-spectrum antibiotics prior to the infection. For previous use of vancomycin, the Odds Ratio for C. difficile infection was 5.4 (95% confidence interval [95%CI] 2.3–12.5); for meropenem, 4.41 (95%CI 2.1–9.2); and for cefepime, 2.6 (95%CI 1.3–5.1). For the antineoplastic agents, the Odds Ratio for carboplatin was 2.7 (95%CI 1.2–6.2), melphalan 9.04 (95%CI 1.9–42.3), busulfan 16.7 (95%CI 2.1–134.9), and asparaginase 8.97 (95%CI 1.9–42.9). Conclusions: C. difficile symptomatic infection in children with cancer was associated with previous hospitalization and the use of common antibiotics in cancer patients, such as vancomycin, meropenem, and cefepime, in the last 3 months. Resumo: Introdução: O curso de Medicina da Universidade Federal do Triângulo Mineiro (UFTM) foi criado em 1954. Em 2011, ocorreu a inclusão dos estágios supervisionados em medicina geral e comunitária I e II no internato, que inseriu os estudantes na rede de serviços da atenção básica do município, processo marcado por conflitos e limitações. Visando solucionar os impasses criados, a UFTM contratou, por meio de concurso público, preceptores de medicina de família e comunidade para atuar na rede municipal. Objetivo: Este estudo teve como objetivo analisar o arranjo institucional proposto para otimizar a relação ensino-serviço nos estágios supervisionados da área de medicina de família e comunidade, a partir da percepção de docentes e preceptores do Departamento de Saúde Coletiva que atuam na atenção primária. Método: Trata-se de estudo de caso apoiado em pesquisa documental e entrevistas realizadas com três docentes e três preceptores do curso de Medicina de uma instituição pública. Resultado: A análise temática revelou três núcleos que foram agrupados nas categorias “contexto difícil”, “desencontro de expectativas” e “fios soltos”. O “contexto difícil” exigia a negociação e o deslocamento de poder entre as instituições e seus agentes. A (con)fusão entre os papéis de docente e preceptor produziu “desencontro de expectativas”. Em “fios soltos”, a lacuna entre a universidade e a rede municipal parece intransponível e reiterada a partir do concurso. Conclusão: A realização do concurso e a contratação de preceptores não foram suficientes para superar os desafios da articulação entre a universidade e a rede municipal de saúde. A pesquisa demonstra que não existe caminho simples para a integração ensino-serviço e que a interdependência dos atores aponta para o desafio e a necessidade da construção de agendas e espaços de decisão coletivos.

Abstract: Introduction: The Medical course at the Federal University of the Triângulo Mineiro (UFTM) was created in 1954. In 2011, the Supervised Internships in General and Community Medicine I and II were included in the clerkship program, which immersed students in the local Primary Care network - a process marked by conflicts and limitations. Aiming to solve the impasses created, UFTM hired, through a public tender, Family and Community Medicine preceptors to work in the municipal network. Objective: To analyze the institutional arrangement proposed to optimize the teaching-service relationship in supervised clerkships in the area of Family and Community Medicine, from the perception of teachers and preceptors of the Department of Public Health who work in Primary Care. Method: Case study supported by documentary research and interviews with three professors and three preceptors from the medical school of a public institution. Result: Thematic content analysis revealed three core aspects that were grouped into the categories: “Difficult setting”; “Mismatch of expectations”; and “Loose threads”. The “Difficult setting” demanded negotiation and a power shift between institutions and their agents. The (con)fusion between the roles of teacher and preceptor produced “Mismatches of expectations”. In “Loose Threads,” the gap between the university and the municipal network seems insurmountable and reiterated was after the public tender. Conclusion: The public tender process and hiring of preceptors proved insufficient to overcome the challenges of connectivity between the university and the municipal health network. The research shows that there is no simple way to integrate teaching-service and that the interdependence of the actors points to the challenge and the need to build agendas and collective decision spaces.

Resumo No Brasil, dados da Pesquisa Nacional de Saúde (PNS) estimaram 27,0 milhões de pessoas (18,5%) que referiram Problema Crônico de Coluna (PCC), afetando mais mulheres que homens. Este trabalho tem o objetivo de identificar, entre as mulheres em idade fértil, os fatores associados ao PCC. Trata-se de um estudo transversal realizado com dados da PNS, em que a variável dependente foi a prevalência de PCC, enquanto que os fatores associados incluíram itens sociodemográficos, hábitos de vida, histórico reprodutivo, estado nutricional, diagnóstico de depressão e percepção de saúde. Foram avaliadas 22.621 mulheres com idade entre 18 e 49 anos e, destas, 14,8% referiram ter PCC. Os fatores de risco estudados foram: aumento da faixa etária; viver com cônjuge/companheiro; multiparidade; ser tabagista; sobrepeso ou obesidade, ter Circunferência da Cintura (CC) acima de 80cm e índice Circunferência/Estatura (C/E) acima de 0,5; autopercepção de saúde negativa; e diagnóstico de depressão. O único fator de proteção encontrado foi escolaridade. As associações observadas concluem que idade, viver com cônjuge/companheiro, tabagismo, multiparidade, sobrepeso e obesidade, risco aumentado para doenças cardiovasculares, diagnóstico de depressão e autopercepção de saúde negativa estão associados ao desenvolvimento de PCC em mulheres de idade fértil.

Abstract Chronic Back Problem (CBP) is a public health concern. In Brazil, data from the National Health Survey (PNS) estimated 27.0 million people (18.5%) who reported CBP, affecting more women than men. This study aims to identify the factors associated with CBP among women of childbearing age. It is a cross-sectional study carried out with data from the PNS, where the dependent variable was the prevalence of CBP, and associated factors included socio-demographic items, life habits, reproductive history, nutritional status, diagnosis of depression and health perception. A total of 22,621 women aged 18 to 49 years were interviewed. Of these, 14.8% reported having CBP. The risk factors studied were: increased age; living with spouse; multiparity; smoking; overweight or obese, having Waist Circumference (WC) above 80cm and Circumference/Height (C/E) index above 0.5; negative self-perception of health; and depression. The only protective factor associated with CBP risk reduction was education level. We conclude that age, living with a spouse/partner, smoking, multiparity, being overweight or obese, increased risk for cardiovascular diseases, depression and negative self-perceived health are associated with the development of CBP in women of childbearing age.

Resumo Objetivo: Investigar relações existentes entre excesso de peso em pré-escolares, duração do aleitamento materno e a idade de introdução de leite não materno. Métodos: Estudo transversal de amostra representativa de 817 pré-escolares, 2-4 anos de idade, de creches municipais de Taubaté. O peso e a altura das crianças foram mensurados nas creches em 2009, 2010 e 2011. Calculado o escore z de Índice de Massa Corporal (zIMC), as crianças foram classificadas como risco de sobrepeso (zIMC≥1 a<2) ou como excesso de peso (zIMC≥2). A análise dos dados foi feita por comparação de proporções, coeficiente de correlação e regressão linear multivariada. Resultados: A prevalência de risco de sobrepeso foi 18,9% e de excesso de peso (sobrepeso ou obesidade) de 9,3%. A mediana de duração do aleitamento materno e a idade de introdução do leite não materno foi de 6 meses. O zIMC da criança evidenciou correlação direta com o peso ao nascer (r=0,154; p<0,001) e com o Índice de Massa Corporal (IMC) materno (r=0,113; p=0,002). A correlação foi inversa com a duração total do aleitamento materno (r=−0,099; p=0,006) e a idade de introdução de leite não materno (r=−0,112; p=0,002). Não houve correlação entre o zIMC da criança com o comprimento ao nascer, duração do aleitamento exclusivo e idade da mãe. Conclusões: Quanto mais precoce a introdução de leite não materno, maior a correlação com excesso de peso na idade pré-escolar.

Abstract Objective: Investigate associations between excess weight in preschool children, breastfeeding duration and age of non-breast milk introduction. Methods: Cross-sectional study of a representative sample of 817 preschool children, aged 2-4 years, attending municipal day care centers in the city of Taubaté. The weight and height of children were measured in the day care centers in 2009, 2010 and 2011. The body mass index z-score (BMIz) was calculated and children were classified as risk of overweight (BMIz≥1 to<2) or excess weight (BMIz≥2). Data analysis was carried out by comparison of proportions, coefficient of correlation and multivariate linear regression. Results: The prevalence of risk of overweight was 18.9% and of excess weight (overweight or obesity) was 9.3%. The median duration of breastfeeding and age of introduction of non-breast milk was 6 months. The child's BMIz showed direct correlation with birth weight (r=0.154; p<0.001) and maternal body mass index (BMI) (r=0.113; p=0.002). The correlation was inverse with the total duration of breastfeeding (r=−0.099; p=0.006) and age at non-breast milk introduction (r=−0.112; p=0.002). There was no correlation between the child's BMIz with birth length, duration of exclusive breastfeeding and mother's age. Conclusions: The earlier the introduction of non-breast milk, the higher the correlation with excess weight at preschool age.

OBJECTIVE: To analyze maternal conceptions about excess weight in infancy and the nutritional status of their preschool-aged children. METHODS: A mixed, exploratory study was performed using semi-structured interviews. Two study groups were defined: a group of 16 mothers of children with excess weight and a group of 15 mothers of eutrophic children. The interviews were submitted to content analysis using CHIC software (Classification Hiérarchique Implicative et Cohésitive¯). RESULTS: The mothers of children with excess weight tended to conceive thin children as malnourished, while those of normal weight children emphasized the influence of family and genetics as determinants of a child’s nutritional status. Although there was a certain consensus among the mothers that an unhealthy diet contributes to the risk of a child developing excess weight, the concept of genetics as a determinant of a child’s nutritional status was also present in the dialogue from the mothers of both groups. This result indicates a lack of clarity regarding the influence of eating behavior and family lifestyle on weight gain and the formation of a child’s eating habits. Both groups indicated that the mother has a decisive role in the eating habits of her child; however, the mothers of children with excess weight did not seem to take ownership of this concept when addressing the care of their own children. CONCLUSION: Differences in conceptions, including taking ownership of care, may contribute to the development of excess weight in preschool-aged children.

Growing phenomenon, which involves high morbidity and consequently high costs for health systems, obesity has been found also among the pediatric population and is currently considered a public health problem. The aim of this study was to verify if in children in the early preschool age we can see the prevalence of overweight and if introducing complementary feeding as well as the type of food introduced, are associated with this condition in this age group. It is an observational analytic study with children born in 2011-2012 that attended public schools in Taubaté -SP during 2014. In addition to the weight and height of children, information about the history of feeding and birth were collectedusing a standardized questionnaire.The nutritional status was defined as having overweight children with z-scores for body mass index (zIMC) ≥ 1.We conducted bivariate analysis and then linear regression analysis of multiple variables.The prevalence of overweight was elevated (27.5%). Only birth weight showed significant correlation with respect to zIMC (r = 0.22, p < 0.0001). The multivariable analysis showed no relationship with the various foods, but showed birth weightas a high risk factor, the male and the total duration of breastfeeding as protective factors. As a result, we conclude that the early introduction of new foods is not a risk factor for the development of overweight at the beginning of preschool age.

Fenômeno crescente, que envolve alta morbidade e altos custos para os sistemas de saúde, a obesidade está sendo encontrada também entre a população pediátrica, sendo considerada atualmente um problema de saúde pública. O objetivo do presente estudo foi verificar se em crianças no início da idade préescolar já se observa prevalência de excesso de peso e se a introdução de novos alimentos, assim como o tipo de alimento, está associada à essa condição nutricional. Trata-se de um estudo observacional, analítico, de uma coorte histórica com crianças nascidas em 2011/12, frequentadoras de creches públicas em Taubaté - SPem 2014. Além do peso e estatura das crianças, coletou-se informações quanto aos antecedentes de alimentação e de nascimento utilizando um questionário padronizado. Quanto aoestado nutricional definiu-se como portadoras de excesso de peso (EP) crianças com escores z do índice de massa corpórea (zIMC) ≥ 1. Realizou-se a análise bivariada e posteriormente análise de regressão linear de múltiplas variáveis. A prevalência de EP encontrada foi elevada (27,5%). Apenas o peso ao nascer (PN) mostrou correlação significante em relação ao zIMC (r= 0,22, p<0,0001). Aanálise de múltiplas variáveis não mostrou relação com os diversos alimentos, mas evidenciou o PN elevado como fator de risco, o sexo masculino e a duração total do aleitamento materno como fatores de proteção. Como consequência, é possível concluir que a introdução precoce de novos alimentos não representa fator de risco para o desenvolvimento do excesso de peso no início da idade pré-escolar.

CONTEXTO E OBJETIVO: O Brasil atravessa um período de transição epidemiológica com mudanças demográfica e nutricional. A prevalência de obesidade está aumentando também entre crianças, causando inúmeros problemas de saúde, que estão se tornando questões de saúde pública. Objetivou-se avaliar a prevalência de excesso de peso em crianças de dois e três anos. TIPO DE ESTUDO E LOCAL: Estudo transversal em creches municipais de Taubaté, São Paulo, Brasil. MÉTODOS: Em amostra probabilística e randomizada de 447 pré-escolares, foram mensurados peso e altura, e calculado o índice de massa corporal (IMC). O estado nutricional foi classificado segundo pontos de corte do referencial da Organização Mundial de Saúde (2006). Compararam-se os valores médios de peso, estatura e IMC das crianças segundo a idade e sexo. RESULTADOS: A amostra final (n = 447) apresentou as seguintes médias. Idade: 38,6 meses (± 3,5), escore Z de peso para estatura (P/E): 0,50 (± 1,22), estatura para idade (E/I): -0,03 (± 1,07), peso para idade (P/I): 0,51 (± 1,23) e IMC: 0,51(± 1,23). A prevalência de excesso de peso (IMC > 1 z) foi de 28,86% e de crianças abaixo do peso ideal (IMC < -2 z) foi de 0,89%. Não houve diferença das médias de IMC entre as idades de dois e três anos (P = 0,66). CONCLUSÃO: Na população de crianças de dois e três anos de idade, já se evidencia elevada prevalência de excesso de peso, praticamente sem desnutrição, indicando que já pode estar ocorrendo uma transição nutricional importante inclusive em cidades médias de países em desenvolvimento.

CONTEXT AND OBJECTIVE: Brazil is undergoing a period of epidemiological transition associated with demographic and nutritional changes. The prevalence of obesity is also increasing in children and is causing numerous health problems that are becoming public health issues. The aim here was to evaluate the prevalence of overweight among children of two and three years of age. DESIGN AND SETTING: Cross-sectional study in municipal day care centers in Taubaté, state of São Paulo, Brazil. METHODS: Weight and height measurements were made on 447 preschool children forming a probabilistic randomized sample. Their body mass index (BMI) was calculated. Their nutritional status was classified using the World Health Organization reference cutoff points (2006). Their mean weight, height and BMI were compared according to their age and sex. RESULTS: The mean values for the final sample (n = 447) were as follows: mean age: 38.6 months (± 3.5) and Z scores for: weight/height (W/H): 0.50 (± 1.22); height/age: -0.03 (± 1.07); weight/age (W/A): 0.51 (± 1.23); and BMI: 0.51(± 1.23). The prevalence of overweight children (BMI > 1 z) was 28.86%, while the prevalence of underweight children (BMI < -2 z) was 0.89%. There were no differences in mean BMI among the two and three-year age groups (P = 0.66). CONCLUSION: A high prevalence of overweight was observed in the sample of two and three-year-old children, with practically no malnutrition, thus showing that a significant nutritional transition may already be occurring, even in medium-sized cities of developing countries.

OBJETIVO: Testar a hipótese de que colecistectomias agulhascópicas oferecem resultados superiores aos da colecistectomia laparoscópica usual (CL). MÉTODOS: Sessenta pacientes consecutivos com colecistopatia submetidos à CA ou CV foram avaliados quanto ao tempo operatório, freqüência de acidentes peroperatórios, dor pós-operatória, sintomas pós-operatórios tardios, comprimento das cicatrizes e grau de satisfação. RESULTADOS: O tempo operatório médio foi semelhante em ambos os grupos. A maioria dos pacientes, independentemente da técnica, relataram dor pós-operatória leve. Aqueles operados por CA tiveram menores níveis de dor no 7º dia de pós-operatório (PO7) (p<0.01) e menor necessidade de analgesia adicional. Menor freqüência de dor epigástrica foi observada no grupo CA até o PO4 (p<0.01). O resultado estético foi amplamente superior após CA (comprimento total das cicatrizes menor que a metade após CL). Não houve diferença quanto ao grau de satisfação entre os grupos. CONCLUSÕES: As duas técnicas foram seguras e eficazes, apresentando tempos operatórios semelhantes e baixos níveis de dor pós-operatória. A redução dos portais para 2-3 mm associou-se a menor freqüência de dor pós-operatória apenas na incisão epigástrica até o PO4. O resultado estético da agulhascopia foi significantemente superior ao da laparoscopia, apesar desta vantagem não ter influenciado o grau de satisfação dos doentes.

PURPOSE: To test the hypothesis that needlescopic cholecystectomies (NC) offer superior outcomes in comparison to common laparoscopic cholecystectomies (LC). METHODS: Sixty consecutive patients with gallbladder disease undergoing either LC or NC were evaluated with respect to differences in operative time, frequency of per-operative incidents, post-operative pain, late postoperative symptoms, length of scars and level of postoperative satisfaction. RESULTS: Mean operative time was similar in both groups. Most of the patients, irrespective of the technique, informed mild postoperative pain. NC patients had lower levels of pain on the 7th postoperative day (PO7) (p<0.01) and decreased need for additional analgesia. Less frequency of epigastric wound pain was observed in NC patients until PO4 (p<0.01). Aesthetic result was far superior after NC (total length of scars less than half after LC). No differences regarding postoperative satisfaction with the operation were observed between the studied groups. CONCLUSIONS: Both techniques were safe and effective, presenting similar operative times and low levels of postoperative pain. Downsizing the ports to 2-3 mm was associated with significantly less frequency of postoperative pain only in the epigastric wound until PO4. Aesthetic outcome of NC was significantly superior to LC, although this advantage did not influence patient level of satisfaction.
