RESUMO Este relato de caso aborda o curso clínico de um envenenamento botrópico ocorrido no município de Jarinu, SP, no ano de 2021. O paciente necessitou de fasciotomia em membro superior após síndrome compartimental aguda com enxertia dermoepidérmica em um segundo momento cirúrgico. No pós-operatório tardio, o paciente evoluiu com retração, necessitando de reabordagem cirúrgica com correção de retração de membro superior esquerdo. ABSTRACT This case study examines the clinical course of a Bothrops snakebite poisoning that occurred in Jarinu, São Paulo, Brazil, in 2021. The patient required a fasciotomy in the upper limb due to acute compartment syndrome, followed by a second surgical procedure involving dermo-epidermal grafting. In the late postoperative period, the patient experienced retraction, leading to a subsequent surgical intervention to correct the retraction in the left upper limb. The severity of the snakebite accident, the effects of venom on tissues, complications, acute compartment syndrome, as well as the indications and techniques for decompressive fasciotomy, are discussed based on the available literature. RESUMO Os Efeitos de Borda são modificações na vegetação, nas condições microclimáticas e no solo na transição entre o meio exterior e o interior da floresta. O objetivo deste estudo foi verificar até quantos metros é perceptível variações florístico estruturais ocasionadas pelo Efeito de Borda, analisando-se o estrato arbóreo e regenerativo de quatro fragmentos de Floresta Estacional Semidecidual localizados no bioma Cerrado. A partir da amostragem realizada nestes quatro fragmentos, foram analisadas, por meio de dendrogramas e regressões, se os parâmetros estruturais podem distinguir regiões de borda e de interior. Para o estrato arbóreo, nos dendrogramas de similaridade (Bray-Curtis e Sørensen), os fragmentos apresentaram grupos com parcelas semelhantes até os 40 metros de borda. Para o estrato regenerativo, os grupos foram mais difíceis em distinguir, mesmo com tendência, a dissimilaridade florística de 15 a 20 m. A partir da Análise de Similaridade (ANOSIM), os grupos foram confirmados estatisticamente. A porcentagem de similaridade (SIMPER) pode relacionar as espécies em função da sua contribuição para a divisão dos grupos e para as áreas de maior interesse. Nas análises dos dados estruturais, não foram comprovadas as relações do n° de árvores, espécies e área basal em função da borda. ABSTRACT Edge effects are changes in vegetation, microclimate conditions and soil in transition bands between the edge and interior of the forest. The objective of this study was to verify for how many meters it is possible to perceive structural floristic variations caused by the Edge Effect by analyzing the tree and regenerative strata of four seasonal semi-deciduous forests located in the Cerrado Biome, Brazil. Samples were taken from four fragments of semi-deciduous forests in the Cerrado and analyzed by dendrograms and regressions for structural parameters that could distinguish edge and interior regions. In the Bray-Curtis Index and in the Sørensen Coefficient, for the tree stratum, the fragments presented groups with plots close to 40 meters from the edge. For the regenerative stratum the groups were more difficult to distinguish, even with a tendency to floristic dissimilarity of 15 to 20 meters. The groups were statistically confirmed by the Similarity Analysis (ANOSIM). The similarity percentage (SIMPER) can relate the species according to their contribution to the division into groups and to the areas of greatest interest. In the analysis of the structural data, the relationships between the number of trees, species and basal area as a function of the edge were not proven. ABSTRACT Inside the Brazilian semi-arid region (Caatinga biome) there are vegetation refuges from other biomes that are present mainly at greater elevations in ancient geological formations such as Chapada do Araripe. This study aimed to understand the structure, diversity and floristic similarity of a fragment of humid forest in order to support conservation strategies for this community in Chapada do Araripe, southern Ceará (708m Alt.), an area of relevant ecological, cultural and scenic value. In 13 parcels of 25x25m (0.8 ha) all individuals with DBH ≥ 5cm were inventoried, observing total height. Floristic similarity was observed using the Jaccard method, comparing it with seven other areas in different regions of the Brazil. Sixty-six species were found distributed in 34 families, in a total of 1,544 individuals with DA=1,997.30 ind.ha-1 and AB =32,618 m²ha-1. Fabaceae (25.75%) and Myrtaceae (6%) had the highest number of species and Brosimum gaudichaudii had the highest IVI. The Shannon index (H') was 3.13 and the Pielou equability (J') was 0.75. The beta diversity, compared with other areas of humid forest (core and disjunct), was considered high and the statistics showed greater similarity with the area of the mesoregion of Pernambuco forest. The results allow the characterization of the area as a vegetational refuge of humid forest, corroborating the Refuge Theory and showing strong penetration of savanna species (“Cerradão”, Cerrado, Caatinga/“Carrasco”), emphasizing the action of climatic impulses that occurred in the Quaternary and pointing to the need to conservation strategies due to accelerated anthropism.

RESUMO No interior da região semiárida brasileira (bioma Caatinga), há ocorrência de refúgios de vegetação de outros biomas que se apresentam principalmente em topos de formações geológicas do período Pré-Cambriano, como a Chapada do Araripe. Este trabalho objetivou o conhecimento da estrutura, diversidade e similaridade florística de um fragmento de mata úmida na Chapada do Araripe, sul do Ceará (708m Alt.), visando subsidiar estratégias de conservação dessa comunidade, área de relevante valor ecológico, cultural e paisagístico. Em 13 parcelas de 25x25m (0,8 ha), foram inventariados todos os indivíduos com DAP ≥ 5cm, observando-se altura total. A similaridade florística foi observada pelo método de Jaccard a partir da comparação com outras sete áreas em diferentes regiões do Brasil. Foram encontradas 66 espécies distribuídas em 34 famílias, num total de 1.544 indivíduos com DA=1.997,30 ind.ha-1 e AB=32,618 m²ha-1. Fabaceae (25,75%) e Myrtaceae (6%) apresentaram maior número de espécies e Brosimum gaudichaudii apresentou maior IVI. O índice de Shannon (H’) foi 3,13 e a equabilidade de Pielou (J’) foi 0,75. A diversidade beta, comparada com outras áreas de mata úmida (core e disjuntas), foi considerada alta e a estatística apontou maior similaridade com área da mesorregião da mata pernambucana. Os resultados permitem caracterizar a área como refúgio vegetacional de mata úmida, corroborando com a teoria dos refúgios e evidenciam forte penetração de espécies savânicas (cerradão, cerrado, caatinga/carrasco) enfatizando a atuação de impulsos climáticos ocorridos no Quaternário e apontando para a necessidade de estratégias de conservação devido ao antropismo acelerado.

Conjugated linoleic acid (CLA), a fatty acid found in ruminant food products, has been associated with anticarcinogenic activity. However, its effect on cancer metabolism is unclear. In this paper we evaluated the effects of cis-9, trans-11 CLA on the metabolic profile of MCF-7 and MDA-MB-231 breast cancer cells using high-resolution magic angle spinning (HR-MAS) nuclear magnetic resonance (NMR) spectroscopy. The NMR spectra showed that phosphocholine level, a cancer malignance biomarker, was reduced in both cells treated with CLA, but the reduction was more pronounced in MCF-7 cells. The NMR spectra also showed that CLA has opposite effect on MCF-7 and MDA-MB-231 acetone metabolism. Acetone signal has been observed in the spectra of MDA-MB-231 control cells, but not in the spectra of the cells treated with 50 and 100 µM CLA. Conversely, the acetone signal is very small or not observed in the NMR spectra of MCF-7 control cells and in cells treated with 50 µM of CLA, but is very strong in the spectra of the cells treated with 100 µM of CLA. Therefore, this CLA concentration is causing a ketosis in MCF-7 cells by inducing the use of fatty acids as an energy source or by reducing acetone catabolism. These results indicate that CLA interfere in the metabolism of both cells. However, the strongest effect has been observed on the metabolism of MCF-7 cells cultivated in the presence of 100 µM CLA. Therefore, CLA could be a potential anticarcinogenic drug, especially for cells with positive estrogen receptor, such as MCF-7.

Este estudo teve como objetivo relacionar as variações na riqueza e estrutura da comunidade arbórea com variáveis edáficas em um gradiente de floresta ciliar, no Parque Estadual da Mata Seca, Norte de Minas Gerais. Os estudos das variáveis edáficas e da diversidade e estrutura da vegetação arbórea (DAP ≥ 5 cm) foram conduzidos em 39 parcelas de 400 m², divididas equitativamente em três trechos previamente selecionados, sendo eles: São Francisco (menor teor de umidade), Meio (alagamento durante a maior parte do ano) e Lagoa da Prata (alagamento durante a estação chuvosa). A análise de solo superficial (0 a 20 cm) evidenciou diferenças significativas entre os diferentes trechos florestais. Nos três ambientes foram amostrados 2.482 indivíduos, pertencentes a 36 espécies, 31 gêneros e 16 famílias botânicas. Houve diferença significativa nos diferentes trechos no índice de diversidade de Shannon, número de indivíduos, área basal e dominância absoluta. A distribuição diamétrica da comunidade apresentou grande número de indivíduos nas menores classes, decrescendo gradualmente. Vale salientar que a área basal e a densidade de indivíduos obtiveram correlações significativas com a maior parte das variáveis edáficas analisadas. Assim, a estrutura dos três trechos estudados está correlacionada com os fatores texturais, químicos e umidade do solo, ocasionando variações fitossociológicas nas comunidades estudadas.

This study aimed to relate variations in richness and tree community structure to soil characteristics in a gradient of riparian forest, at Parque Estadual da Mata Seca, north of Minas Gerais. Studies of soil characteristics and the richness and structure of trees (DBH ≥ 5 cm) were conducted in 39 plots of 400 m², divided equally into three sections previously selected, these being: São Francisco (lower moisture), Meio (flooding during most of the year) and Lagoa da Prata (flooding during the rainy season). The analysis of topsoill (0-20 cm) showed significant differences between the different forest sections. In all three environments were sampled 2482 individuals belonging to 36 species, 31 genera and 16 plant families. There were significant differences for the different sections in the Shannon diversity index, number of individuals, basal area and absolute dominance. The diameter distribution of the community had a great number of individuals concentred in the smallest classes, decreasing gradually. It is noteworthy that the basal area and density of subjects had significant correlations with soil most variables analyzed. Thus, the structure of the three sections studied is correlated with the textural, chemical and soil moisture, causing variations in phytosociological communities studied.

O objetivo foi testar a reprodutibilidade da versão em português da Escala de Qualidade PEDro. Sete fisioterapeutas avaliaram a qualidade metodológica de 50 estudos controlados aleatorizados em português, indexados na base de dados PEDro. Cada artigo já possuía sua respectiva avaliação nessa base de dados, utilizando a versão em inglês da escala PEDro. Foi calculada a confiabilidade da escala, assim como foi comparada a pontuação total de consenso com a pontuação das avaliações utilizando a escala em inglês. Os coeficientes kappa variaram entre 0,53 e 1,00 para itens individuais, e um coeficiente de correlação intraclasse (CCI) de 0,82 foi obtido para a pontuação total. O erro-padrão de medida foi de 0,58 ponto. A versão em português da escala foi comparada com a versão em inglês e foi observado um CCI de 0,78. A reprodutibilidade da versão em língua portuguesa da Escala de Qualidade PEDro foi adequada e similar à versão em inglês.

Seven physiotherapists rated the methodological quality of 50 reports of randomized controlled trials written in Portuguese indexed on the PEDro database. Each report was also rated using the English version of the PEDro Scale. Reproducibility was evaluated by comparing two separate ratings of reports written in Portuguese and comparing the Portuguese PEDro score with the English version of the scale. Kappa coefficients ranged from 0.53 to 1.00 for individual item and an intraclass correlation coefficient (ICC) of 0.82 for the total PEDro score was observed. The standard error of the measurement of the scale was 0.58. The Portuguese version of the scale was comparable with the English version, with an ICC of 0.78. The inter-rater reproducibility of the Brazilian Portuguese PEDro Scale is adequate and similar to the original English version.

INTRODUÇÃO: A Lamivudina tem-se mostrado útil no tratamento da hepatite crônica pelo vírus B (HC-VHB). OBJETIVO: Investigar os fatores preditivos da resposta à Lamivudina na HC-VHB. MATERIAL E MÉTODOS: Estudo prospectivo com Lamivudina em 35 pacientes com HC-VHB e evidência de multiplicação viral, independentemente do resultado do AgHBe. Administrou-se a Lamivudina na dose diária de 300 mg por 12 meses, seguida de 150 mg diários. Critério de resposta: DNA-VHB negativo (por técnica de PCR) aos 6 meses de tratamento. Nos pacientes não respondedores, pesquisaram-se mutações associadas com resistência à Lamivudina, através do sequenciamento do DNA viral. RESULTADOS: Observou-se resposta em 23/35 pacientes (65,7%). Dos 15 pacientes com AgHBe positivo antes do tratamento, apenas 5 (33,3%) responderam. As variáveis prévias ao tratamento que puderam prever uma má resposta foram: AgHBe positivo (p = 0,006), carga viral elevada (> 3 x 10(6) genomas/ml) (p = 0,004) e AgHBc no tecido positivo (p = 0,0028). Mutações na região YMDD foram detectadas em 7/11 pacientes não respondedores. CONCLUSÕES: Pacientes com AgHBe positivo e com alta carga viral apresentam um alto risco de desenvolver resistência à Lamivudina. Por outro lado, pacientes com AgHBe negativo, mesmo com alta carga viral, mostraram uma boa resposta à Lamivudina.

BACKGROUND: Lamivudine has been shown to be an efficient drug for chronic hepatitis B (CHB) treatment. AIM: To investigate predictive factors of response, using a quantitative method with high sensitivity. METHODS: We carried out a prospective trial of lamivudine in 35 patients with CHB and evidence for viral replication, regardless to their HBeAg status. Lamivudine was given for 12 months at 300 mg daily and 150 mg thereafter. Response was considered when DNA was undetectable by PCR after 6 months of treatment. Viral replication was monitored by end-point dilution PCR. Mutation associated with resistance to lamivudine was detected by DNA sequencing in non-responder patients. RESULTS: Response was observed in 23/35 patients (65.7%) but only in 5/15 (33.3%) HBeAg positive patients. Only three pre-treatment variables were associated to low response: HBeAg (p = 0.006), high viral load (DNA-VHB > 3 x 10(6) copies/ml) (p = 0.004) and liver HBcAg (p = 0.0028). YMDD mutations were detected in 7/11 non-responder patients. CONCLUSIONS: HBeAg positive patients with high viral load show a high risk for developing drug resistance. On the other hand, HBeAg negative patients show a good response to lamivudine even with high viremia.
