ABSTRACT Objective: To investigate the presence of chromosome mosaicism, especially for the presence of Y derived material in 45,X women with Turner syndrome (TS). Materials and methods: FISH and PCR were performed for the presence of chromosome mosaicism and Y-derived-material and genetic findings were correlated to clinical data. Results: Thirty-one participants were enrolled: 18 (58%) had chromosome mosaicisms (FISH), Y-derived material was found in 2. Yet, SRY primer was found with PCR in only one of them and DYZ3 was not found. The most frequent clinical findings were short or webbed neck (81,82%), high-arched palate (78%), breast hypertelorism, e cubitus valgus and genu valgus (57.6%, both), short fourth metacarpals (46.9%), epicanthic folds (43.8%), shield chest (43.8%), lymphedema (37.5%), and low set ears (34.4%). Both patients with Y-derived-material had primary amenorrhea, dyslipidemia and reached the height of 150 cm despite not treated with recombinant growth hormone (GHr). One of them showed 26% of leukocytes with Y-derived material and few clinical findings. Conclusions: FISH techniques proved efficient in detecting chromosome mosaicisms and Y-derived material and searching in different tissues such as mouth cells is critical due to the possibility of tissue-specific mosaicism. Phenotypical variance in TS may be a signal of chromosome mosaicisms, especially with the presence of Y-derived material.

RESUMO Objetivo Descrever o perfil vocal de indivíduos 46,XX com hiperplasia adrenal congênita, acompanhados no Ambulatório de Genética da Universidade Federal da Bahia (UFBA). Método Trata-se de um estudo descritivo e exploratório, com corte transversal. A amostra foi de conveniência e participaram do estudo 28 voluntários, 14 diagnosticados com hiperplasia adrenal congênita, acompanhados pela equipe multiprofissional do Ambulatório de Genética da UFBA, e 14 indivíduos 46,XX sem alterações vocais e ausência de patologia de cunho endócrino e/ou genético. A coleta das vozes foi realizada individualmente, em um ambiente silencioso, com as participantes devidamente sentadas. Realizaram-se análises perceptivo-auditiva (CAPE-V) e acústica. Resultados Em relação ao julgamento qualitativo do pitch, verificou-se que oito (61,54%) pacientes do grupo com hiperplasia adrenal congênita apresentaram um padrão vocal agravado e 8 (61,54%) do grupo sem a doença apresentaram um padrão vocal agudizado. Houve diferença estatisticamente significante entre os grupos apenas para as medidas da análise perceptivo-auditiva (CAPE-V) grau geral (p = 0,01), rugosidade (p = 0,00) e pitch (p = 0,01). Os demais parâmetros investigados na análise acústica não diferiram significativamente (p > 0,05). Conclusão O presente estudo demonstrou que indivíduos 46,XX com hiperplasia adrenal congênita, mesmo submetidos à terapêutica hormonal, apresentam qualidade vocal rugosa, pitch agravado e voz desviada.

ABSTRACT Purpose Describe the vocal profile of 46,XX congenital adrenal hyperplasia (CAH) patients followed up at the Genetics Outpatient Clinic of the Federal University Bahia (GOC-UFBA). Methods This is a descriptive, exploratory, cross-sectional study. The study sample consisted of 28 volunteers: 14 individuals diagnosed with CAH, followed up by the multiprofessional team of the GOC-UFBA, and 14 46,XX individuals without vocal changes and endocrine and/or genetic pathologies. Voice sample collection was performed individually in a quiet environment with participants properly seated. Acoustic (PRAAT program) and auditory-perceptual (Consensus Auditory-Perceptual Evaluation of Voice - CAPE-V) analyses were conducted. Results In the qualitative assessment of pitch, eight (61.54%) patients in the CAH group showed low vocal pattern and eight (61.54%) individuals in the group without CAH presented high vocal pattern. There were statistically significant differences between the groups only for the following vocal attributes of the CAPE-V: overall severity (p=0.01), roughness (p=0.00), and pitch (p=0.01). No statistically significant difference was observed in the other acoustic parameters investigated (p>0.05). Conclusion The present study demonstrated that 46,XX CAH individuals, even when submitted to hormone therapy, present rough, low, deviant voice.

RESUMO Objetivo: Caracterizar o controle metabólico e verificar se existe relação entre ele, variáveis socioeconômicas, demográficas e composição corporal de crianças e adolescentes com fenilcetonúria (FNC) diagnosticada no período neonatal. Métodos: Coorte com coleta retrospectiva de dados de 53 crianças e adolescentes fenilcetonúricos. Foram coletados dados de renda familiar, moradia, idade e escolaridade materna e realizaram-se medidas antropométricas de composição e distribuição corporal. Todas as dosagens de fenilalanina (Fal) dos últimos cinco anos (2015-2019) foram avaliadas e classificadas quanto à adequação (cortes: 0-12 anos: 2-6 mg/dL; 12-19 anos: 2-10 mg/dL). A proporção de dosagens adequadas ≥70% foi considerada como controle metabólico adequado. Resultados: A média (±desvio padrão) de idade, no último ano, foi de 10,1±4,6 anos. A maioria tinha menos de 12 anos (33/53; 62,3%) e apresentava a forma clássica da doença (39/53; 73,6%). Observou-se melhor controle metabólico entre os adolescentes (68,4 vs. 51,4%; p=0,019). Excesso de peso foi encontrado em 9/53 (17%) e maiores níveis séricos de Fal foram descritos nesse grupo (p<0,001). O percentual de controle metabólico com 70% ou mais de adequação dos níveis de Fal foi decrescente de acordo com a área muscular do braço (AMB; ptendência=0,042), sendo de 70% entre os de baixa reserva (AMB reduzida) e de 18,5% entre os com excesso (AMB elevada). Conclusões: Observou-se controle metabólico adequado na maioria dos avaliados e os achados sugerem que, nesta amostra, os níveis de fenilalanina podem estar relacionados com alterações da composição corporal.

ABSTRACT Objective: To characterize metabolic control and verify whether it has any relation with socioeconomic, demographic, and body composition variables in children and adolescents with phenylketonuria (PKU) diagnosed in the neonatal period. Methods: This cohort study collected retrospective data of 53 phenylketonuric children and adolescents. Data on family income, housing, and mother’s age and schooling level were collected, and anthropometric measures of body composition and distribution were taken. All dosages of phenylalanine (Phe) from the last five years (2015-2019) were evaluated and classified regarding their adequacy (cutoffs: 0-12 years: 2-6 mg/dL; 12-19 years: 2-10 mg/dL). Adequate metabolic control was considered if ≥7%) of the dosages were within desired ranges. Results: The mean (±standard deviation) age in the last year was 10.1±4.6 years. Most of them were under 12 years old (33/53; 62.3%) and had the classic form of the disease (39/53; 73.6%). Better metabolic control was observed among adolescents (68.4 versus 51.4%; p=0.019). Overweight was found in 9/53 (17%) and higher serum Phe levels (p<0.001) were found in this group of patients. Metabolic control with 70% or more Phe level adequacy decreased along with the arm muscle area (AMA) (ptendency=0.042), being 70.0% among those with low reserve (low AMA), and 18.5% among those with excessive reserve (high AMA). Conclusions: Adequate metabolic control was observed in most patients. The findings suggest that, in this sample, the levels of phenylalanine may be related to changes in body composition.

ABSTRACT The term DSD refers to disorders that affect the normal process of sexual development causing disagreement between chromosomal, gonadal and phenotypic sex, and this study aimed to describe the clinical profile of a group with DSD 46, XY joined on DSD Clinic of Hospital of Salvador, Bahia Clinics. It was a retrospective study of medical records of survey data of 93 patients with DSD 46, XY. Among the patients studied 50.5% had no defined etiology and 20.4% had androgen insensitivity syndrome (AIS), 63.4% had been initially recorded in males, 31 (33.3%) in females, being that in two it was necessary to reassignment. All patients with complete AIS pure gonadal dysgenesis and had female genitalia. Others have been diagnosed with genital ambiguity or severe hypospadias and cryptorchidism. The gonads were palpable at the first consultation in 75.3% of patients. It is important to establish an active surveillance program for these patients. The first assessment took place before the age of ten in more than 50% of cases, which shows that much needs to be done for medical education and community about the DSD. Because the phenotypic variability of sexual development disorders was noted that the clinical profile of patients studied ranged between different etiologies, including hindering the diagnostic conclusion of these individuals.

OBJETIVO: O objetivo deste estudo foi avaliar pacientes com HAC clássica antes e após tratamento com glicocorticoides e/ou mineralocorticoides e comparar o perfil metabólico entre o grupo bem controlado (BC) e mal controlado (MC). SUJEITOS E MÉTODOS: Foram selecionados pacientes recém-diagnosticados e pacientes em acompanhamento por HAC, forma clássica, em uso regular ou não de glicocorticoides/mineralocorticoides do Serviço de Genética do Hupes-UFBA, atendidos de março/2004 a maio/2006. Todos os pacientes foram submetidos a avaliação clínica detalhada e exames laboratoriais (glicemia, sódio e potássio, colesterol total, HDL, LDL, triglicerídeos, ácido úrico, leptina, 17-hidroxiprogesterona, testosterona total, peptídeo C e insulina). Os pacientes com valores normais de andrógenos foram classificados como bem controlados (BC) e os com valores elevados de andrógenos em uso ou não de glicocorticoides/mineralocorticoides foram classificados como mal controlados (MC). RESULTADOS: Foram estudados 41 pacientes com HAC: 11 no grupo BC e 30 no grupo MC. Leptina e LDL colesterol estavam mais elevados no grupo BC que no MC (p < 0,05). Valores de ácido úrico eram menores no grupo BC quando comparados com MC (p < 0,05). CONCLUSÃO: O controle adequado da HAC com glicocorticoides parece seguro, pois está associado a alterações discretas no perfil lipídico e da leptina. Não observamos outras alterações metabólicas associadas ao uso de glicocorticoides. O motivo para o menor valor de ácido úrico encontrado nos pacientes com HAC bem controlada não é conhecido e deve ser mais bem estudado.

OBJECTIVE: The objective of this study was to evaluate patients with classic CAH before and after treatment with glucocorticoids/mineralocorticoid and compare the metabolic profile of the well controlled (WC) and poorly controlled (PC) group. SUBJECTS AND METHODS: We selected newly diagnosed patients and patients monitored for CAH, classical form, regularly using or not glucocorticoids/mineralocorticoid in the Genetics Service Hupes-UFBA, seen from March/2004 to May/2006. All patients underwent detailed clinical evaluation and laboratory tests (glucose, sodium and potassium; total cholesterol, HDL, LDL, triglycerides and uric acid; leptin, 17-hydroxyprogesterone, total testosterone, C peptide, and insulin). Patients with normal androgens were classified as well controlled (WC), and those with high levels of androgens either using or not glucocorticoids/mineralocorticoids were classified as poorly controlled (PC). RESULTS: We studied 41 patients with CAH: 11 in the WC group and 30 in PC group. Leptin and LDL cholesterol levels were higher in WC than in the PC group (p < 0.05). Uric acid values were lower in WC compared with the PC group (p < 0.05). CONCLUSION: Adequate control of CAH with steroids seems safe, as it is associated with only mild changes in lipid profile and leptin values. No other metabolic abnormality was associated with glucocorticoid use. The reason for lower uric acid levels found in WC CAH patients is unknown and should be further studied.

CONTEXTO E OBJETIVO: A falta de uma definição clara da raça humana e a importância desse tema na prática médica continua a ser fonte de dúvidas para estudiosos. No presente artigo nós avaliamos o uso da variável raça por estudantes de medicina em Salvador, Brasil. TIPO DE ESTUDO E LOCAL: corte transversal, realizado numa universidade pública federal. MÉTODO: 221 estudantes, , foram incluídos. Um questionário semi-estruturado foi utilizado para a coleta dos dados. Os resultados são expressos como média e desvio-padrão da média, proporções e frequências. O teste do χ2 (qui-quadrado) foi utilizado para o cálculo estatístico. RESULTADOS: Aproximadamente metade dos estudantes (45,4%) usava a variável grupo racial na sua prática clínica em estudos. Desses, 86,8% a consideravam uma informação relevante no prontuário médico; 92,7% no raciocínio diagnóstico; 95,9% acreditavam que ela influenciava a causa, expressão e prevalência das doenças; 94,9% afirmaram que ela contribuía para estimar o risco de doenças; 80,5% informaram que a resposta terapêutica a medicamentos pode ser influenciada pelas características raciais; 41,9% consideravam que sua inclusão nas pesquisas era sempre recomendável; e 20,3% a avaliavam como indispensável. As principais características fenotípicas usadas para a classificação racial foram: cor da pele (93,2%), tipo de cabelo (45,7%), formato do nariz (33,9%) e espessura dos lábios (30,3%). CONCLUSÃO: Apesar de sua importância na prática médica, a maioria dos profissionais não usa e não sabe classificar os diversos grupos raciais. É necessário adicionar e/ou ampliar a discussão sobre as categorias raciais e étnicas no exercício da medicina e nas pesquisas médicas.

CONTEXT AND OBJECTIVE: The lack of a clear definition for human "race" and the importance of this topic in medical practice continue to create doubt among scholars. Here, we evaluate the use of the variable "race" by medical students in Salvador, Brazil. DESIGN AND SETTING: Cross-sectional study at a Brazilian federal public university. METHODS: 221 randomly selected subjects were included. A semi-structured questionnaire was used for data collection. The results were expressed as means and standard deviations of the mean, proportions and frequencies. The χ2 (chi-square) test was used for the statistical calculations. RESULTS: Approximately half of the students (45.4%) used the racial group variable in their studies on clinical practice. Of these, 86.8% considered it to be relevant information in the medical records and 92.7%, important for diagnostic reasoning; 95.9% believed that it influenced the cause, expression and prevalence of diseases; 94.9% affirmed that it contributed towards estimating the risk of diseases; 80.5% thought that the therapeutic response to medications might be influenced by racial characteristics; 41.9% considered that its inclusion in research was always recommendable; and 20.3% thought it was indispensable. The main phenotypic characteristics used for racial classification were: skin color (93.2%), hair type (45.7%), nose shape (33.9%) and lip thickness (30.3%). CONCLUSIONS: Despite the importance of different racial groups in medical practice, the majority of the professionals do not use or know how to classify them. It is necessary to add to and/or expand the discussion of racial and ethnic categories in medical practice and research.

OBJETIVO: Avaliar a translucência nucal, o ducto venoso, o osso nasal e a idade materna > 35 anos como testes de rastreamento para aneuploidias entre 12 e 14 semanas de gestação em pacientes de alto risco. MATERIAIS E MÉTODOS: Estudo prospectivo observacional envolvendo 92 gestantes entre 12 e 14 semanas submetidas a biópsia de vilo corial por alto risco de trissomia, baseado na medida da translucência nucal (17,4%) e idade materna >35 anos (78,3%). Antes da biópsia de vilo corial, realizaram-se medida da translucência nucal, avaliação de fluxo no ducto venoso e identificação do osso nasal. Calcularam-se a sensibilidade, a especificidade, o valor preditivo positivo e o valor preditivo negativo para testes realizados em paralelo e em seqüência. RESULTADOS: Encontrou-se alteração cromossômica em 12 (13,5%) fetos; 7 (58,3%) apresentavam trissomia 21. Osso nasal foi identificado em todos os fetos com trissomia. Translucência nucal, ducto venoso e idade materna isolados mostraram baixa sensibilidade (41,67-58,33%) e baixo valor preditivo positivo (10-45,45%). A associação translucência nucal + ducto venoso + idade materna apresentou o melhor resultado (sensibilidade: 100%; especificidade: 6,49%; valor preditivo positivo: 14,29%; valor preditivo negayivo: 100%). CONCLUSÃO: Em gestantes com idade > 35 anos, a associação translucência nucal + ducto venoso mostra-se como a mais sensível para a indicação de procedimento invasivo.

OBJECTIVE: To evaluate fetal nuchal translucency, ductus venosus, nasal bone and maternal age > 35 years by means of aneuploidy screening between the 12th and 14th gestational weeks in a high-risk population. MATERIALS AND METHODS: Prospective, observational study involving 92 pregnant women at 12-14 gestational weeks, who were submitted to chorionic villus sampling because of high risk for trisomy 21 based on the measurement of nuchal translucency thickness (17.4%) or on maternal age > 35 years (78.3%). Before the chorionic villus sampling, fetal nuchal translucency thickness was measured, ductus venosus flow was evaluated and the nasal bone was identified. Sensitivity, specificity, positive predictive value and negative predictive value were calculated for tests in parallel and in sequence. RESULTS: Chromosomal abnormalities were found in 12 fetuses (13.5%); 7 (58.3%) were positive for trisomy 21. The nasal bone was present in all cases with chromosomal abnormalities. Isolated nuchal translucency, ductus venosus or maternal age showed low sensitivity (41.67-58.33%) and low positive predictive value (10-45.45%). Combined nuchal translucency + ductus venosus + maternal age showed the best results (100% sensitivity; 6.49% specificity; 14.29% positive predictive value; 100% negative predictive value). CONCLUSION: In pregnant women with >35 years of age, combined nuchal translucency + ductus venosus have showed the highest sensitivity as an indication for invasive procedure.

OBJETIVO: avaliar complicações maternas e fetais após realização de biópsia de vilo corial (BVC) para diagnóstico pré-natal de alterações genéticas, na cidade de Salvador (BA). MÉTODOS: série de 958 gestantes de risco para cromossomopatias, submetidas à BVC realizada entre a nona e a 24ª semanas de gestação, por via transabdominal, utilizando agulha espinhal 18G 3½, guiada por ultra-sonografia, entre 1990 e 2006. As variáveis para a análise de complicações imediatas foram cólicas uterinas, hematoma subcoriônico, punção acidental da cavidade amniótica, dor no local da punção, amniorrexe, desconforto abdominal, bradicardia fetal e sangramento vaginal, e para complicações tardias, dor abdominal, sangramento vaginal, amniorrexe, infecção e abortamento espontâneo. Complicações obstétricas e fetais (parto prematuro, descolamento prematuro de placenta, placenta prévia e malformações anatômicas fetais) foram também estudadas. Para análise estatística, utilizaram-se o chi² e o teste t de Student ou Mann-Whitney; o nível de significância foi 5%. RESULTADOS: a média de idade das gestantes foi 36,3±4,9 anos. Complicações imediatas foram encontradas em 182 (19%) casos (cólica uterina em 14%, hematoma subcoriônico em 1,8% e punção amniótica acidental em 1,3%) e tardias em 32 (3,3%) casos (sangramento vaginal em 1,6%, dor abdominal em 1,4%, amniorrexe em 0,3% e aborto espontâneo em 1,6%). Não foi observado descolamento prematuro de placenta, placenta prévia ou malformação fetal. CONCLUSÕES: a BVC revelou-se procedimento simples e seguro. A BVC pode ser utilizada em gestantes que necessitam de diagnóstico pré-natal devido ao risco de anomalias genéticas.

PURPOSE: to evaluate fetal maternal complications after chorionic villus sampling (CVS) for prenatal diagnosis of genetic disorders in pregnant women of Salvador (BA), Brazil. METHODS: case-series study of 958 pregnancies with high risk for chromosomal abnormality submitted to CVS transabdominal between the ninth to the 24th week of gestation, using an ultrasound-guided 18G 3½ spinal needle, from 1990 to 2006. The variables for the analysis of immediate complications were uterine cramps, subchorionic hematoma, accidental amniotic cavity punction, pain in the punction area, amniotic fluid leakage, abdominal discomfort, fetal arrhythmias and vaginal bleeding, and of late complication, abdominal pain, vaginal bleeding, amniotic fluid leakage, infection and spontaneous miscarriage. Premature labor, obstetrical complications (abruption placenta and placenta previa) and newborn malformation were also studied. Qui-square, Student’s "t" or Mann-Whitney tests were used for the statistical analysis; the significance level was 5%. RESULTS: maternal mean age was 36.3±4.9 years old. Immediate complications ware found in 182 (19%) cases (uterine cramp in 14%, subchorionic hematoma in 1.8% and accidental amniotic cavity punction in 1.3%). Late complications were found in 32 (3.3%) cases (vaginal bleeding in 1.6%, abdominal pain in 1.4%, amniotic fluid leakage in 0.3% and spontaneous miscarriage in 1.6% cases). There was no case of abruption placentae, placenta previa or fetal malformation. CONCLUSIONS: CVS is a simple and safe procedure. CVS should be performed in high risk pregnant patients who need prenatal diagnosis of fetal chromosomal abnormalities.

Vários estudos têm procurado identificar marcadores genéticos para doenças oftalmológicas. Dentre eles, destaca-se o antígeno de histocompatibilidade humano (Human Leukocyte Antigens). Situado no braço curto do cromossomo 6, o sistema antígeno de histocompatibilidade humano é conhecido por sua capacidade de conferir susceptibilidade ou proteção a diferentes doenças. Em virtude do seu acentuado polimorfismo, o tipo e a força da associação variam a depender da enfermidade e da raça (etnia) estudadas. O surgimento de métodos moleculares para tipificação dos alelos antígeno de histocompatibilidade humano e as recentes atualizações de sua nomenclatura têm contribuído para o melhor entendimento desse sistema. O presente trabalho tem por objetivos revisar a estrutura e função do sistema antígeno de histocompatibilidade humano e relatar suas associações com uveíte anterior aguda, penfigóide cicatricial ocular, ceratocone de início na juventude e retinocoroidopatia "birdshot".

Many studies have been trying to identify genetic markers for opthalmological diseases, including, among others, the HLA (Human Leukocyte Antigens). Localized on the short arm of chromosome 6, the human leukocyte antigen system is well known for its capacity to confer susceptibility or resistance to different diseases. In view of its accentuated polymorphism, the strength and type of association differs with the disease and sometimes, with the studied ethnic-racial group. The development of molecular methods to typify HLA alleles and recent updates of their nomenclature has contributed to a better understanding of this system. In this review, some aspects of the human leukocyte antigen system are discussed, such as the methods of detection, nomenclature and association with acute anterior uveitis, ocular cicatricial pemphigoid, young-onset keratoconus and birdshot retinochoroidopathy.

Objetivo: discutir os aspectos clínicos, eletroencefalográficos e os mecanismos genéticos de três síndromes neurogenéticas, que se identificam como entidades nosológicas no grupo heterogêneo de patologias que cursam com retardo mental e autismo. Fontes dos dados: os autores realizaram revisão literária sobre cada síndrome do estudo, atualizando as informações, correlacionando e caracterizando as manifestações neurológicas, assim como a descrição dos mecanismos genéticos e a identificação dos marcadores biológicos. Síntese dos dados: houve a confirmação de que a síndrome de Rett é uma doença genética, conseqüente à mutação no gene MECP2, com variações clínicas que podem ser explicadas por diferentes mutações nesse gene. A síndrome de Angelman tem quatro mecanismos genéticos responsáveis pela variação fenotípica e pelos diferentes riscos de recorrência. Na síndrome do X-Frágil, o grau de comprometimento cognitivo está relacionado com o número de repetições dos trinucleotídeos. Conclusões: os diferentes mecanismos genéticos das três síndromes são responsáveis pela variabilidade clínica. Com a identificação de marcadores biológicos, o diagnóstico será mais precoce, ademais, poderão ser identificadas novas e mais sutis formas de expressão.

Objective: to discuss clinical and electroencephalographic aspects and the genetic mechanisms of three neurogenic syndromes that can be related to nosologic entities in the heterogenic pathological group presenting symptoms of mental retardation and autism. Sources: the authors carried out a bibliographic review on each syndrome involved, correlating and characterizing the neurological manifestations, as well as describing genetic mechanisms and identifying biological markers. Summary of the findings: the authors were able to confirm that Rett Sydrome is a genetic disease resulting from the mutation of the MECP2 gene and clinical variations can be explained by different mutations in this gene. Angelman syndrome has four genetic mechanisms responsible for phenotypic variations and different risks of recurrence. In Fragile-X syndrome, the degree of cognitive impairment is related to the number of trinucleotide repeats. Conclusions: different genetic mechanisms of the three syndromes are responsible for clinical variability. By identifying the biological markers, the diagnosis will be performed earlier and it will be possible to identify new subtle expressions of the disease.
