ABSTRACT Purpose: To compare the injection of small amounts of undiluted C3F8 with the traditional gas injection in vitrectomy for macular hole treatment. Methods: This clinical trial included 26 individuals divided into two groups. Group 1 received an intravitreal injection of 0.9-1.0 mL of 100% C3F8, and Group 2 received 15-20 mL of 20% C3F8. Results: The median intraocular gas duration was 31 days in Group 1 and 34 in Group 2. The median letter gains in corrected distance visual acuity for the 26th postoperative week were 20 letters in Group 1 and 12.5 in Group 2. The median intraocular pressure was normal in both groups. Primary anatomical success was 11/13 in both groups. Conclusions: The use of C3F8 gas in a small undiluted volume is an alternative that slightly reduces the duration of the gas without negatively affecting the anatomical and visual response. Resumo O artigo analisou as implicações do Ato Médico na ordenação das relações entre as profissões de saúde e em aspectos relacionados ao controle do campo de conhecimento médico, à prática profissional e ao mercado de trabalho em saúde. Por meio de pesquisa qualitativa, investigou-se o Ato Médico mediante análise documental de material proveniente de 18 edições do Jornal Medicina do Conselho Federal de Medicina (CFM) (223 edições consultadas entre 1998 e 2018). Foram evidenciados interesses particulares no processo de negociação e de efetivação da Lei do Ato Médico, reconhecida como instrumento de controle profissional decorrente das transformações ocorridas no sistema de profissões da saúde e no mercado de trabalho em saúde. A regulamentação profissional dirigida ao exercício exclusivo de atos profissionais considerados atos médicos aparece como uma vantagem para a corporação médica. Ao pretender a exclusividade da prática médica em múltiplas abrangências, o Ato Médico termina por intensificar conflitos com profissões da saúde que compartilham seus atos profissionais. Foi concluído que o Ato Médico poderia ser também entendido enquanto resultado de um movimento pela valorização das bases liberais de atuação, objetivando a defesa da prática individual, da livre escolha, do custeio dos serviços pelo cliente e da prestação direta dos serviços.

Abstract Most critics argue that the television comedy One Day at a Time (2017), produced in the United States, is a progressive show, mainly due to its cast, its attempt at faithfully representing an ethnic minority, and its courage in advancing relevant, sensitive topics. In order to qualify such assumptions, we will review the history of the sitcom formula, particularly the genre often defined as domestic comedies of the 1970s, and argue that its formal constraints impose unsurmountable limits on a progressive agenda. Finally, we proceed with an aesthetic analysis of the first season, which further demonstrates that the genre’s need of family stability—what we call a hierarchy of values—compromises the dramatization of political content. We hope that by examining the genre’s history and analyzing the show’s aesthetic, we can contribute to a better understanding of its inherent shortcomings and compromises.

RESUMO O presente estudo avaliou os efeitos de uma intervenção com professores, baseada na Teoria da Autodeterminação, sobre a motivação de professores e alunos para a aula de educação física (EF). É um estudo quase-experimental do tipo antes e depois com 4 professores de EF e 611 alunos. Foi elaborado um manual e realizadas sessões de formação de professores. Análise estatística incluiu testes t pareados e medidas repetidas através do modelo linear geral e da regressão linear de efeito misto. Um aumento significante no escore de motivação de professores e alunos foi observado após a intervenção. Entre os professores verificou-se um aumento na motivação auto-determinada. Entre os alunos houve interação significante entre tempo por grupo (Motivação Extrínseca Regulação Identificada (F=5,6), Motivação Extrínseca Regulação Externa (F=7,41), Amotivação (F=5,32) e Motivação Autodeterminada (F=4,87). Além disso, a Motivação Intrínseca diminuiu significantemente com a idade para rapazes (β= -0.151) e moças (β= -0.121), bem como a Motivação Externa Regulação Introjetada para moças (β= -0.141). Sessões de formação podem apoiar os professores no planejamento resultando no aumento da motivação dos professores e alunos nas aulas de EF. No entanto, esta estratégia não foi suficiente para melhorar a motivação intrínseca no período investigado.

ABSTRACT The present study evaluated the effects of a teacher training intervention, based on Self-Determination Theory, on teachers’ and students’ motivation in physical education class. This is a pre-post quasi-experimental study with 4 physical education teachers and 611 students from four public schools. A handbook was developed and teacher training sessions were conducted. Statistical analysis consisted of paired t-tests and general linear model repeated measures to assess teachers’ self-determined motivation and linear mixed effect regression to evaluate students’ motivation. A significant increase in teachers’ and students’ motivation score was observed after the intervention. Among teacher, we verified an increase in self-determined motivation. Among students there were significant interaction time by group in Extrinsic Motivation Identified Regulation (F=5.6), Extrinsic Motivation External Regulation (F=7.41), Amotivation (F=5.32) and Self-determined Motivation (F=4.87). Also, Intrinsic Motivation significantly declined with age for boys (β= -0.151) and girls (β= -0.121) as well as Extrinsic Motivation Introjected Regulation for girls β= (-0.141). Training sessions can support teachers in planning lessons resulting in increasing teacher and students' motivation in physical education classes. However, this strategy was not enough to improve intrinsic motivation during the investigated period.

RESUMO Objetivo: avaliar o processo de implantação de sessões de tele-educação para os profissionais que atuam na Atenção Primária à Saúde e nos Núcleos de Apoio à Saúde da Família, com temáticas voltadas para as repercussões da tríplice endemia na saúde da comunicação humana no estado de Pernambuco, Brasil. Métodos: relato de experiência, desenvolvido após aprovação do Comitê de Ética, em parceria com um Núcleo de Telessaúde. O percurso metodológico envolveu três etapas: 1- criação do fluxograma das fases de implantação; 2- oferta das sessões educativas; 3- Avaliação da satisfação pelo público. Resultados: o processo de implantação foi constituído por cinco fases: Parceria institucional; Capacitação; Planejamento; Oferta do serviço e Avaliação da ação. Foram realizadas sete sessões educativas. O total de acessos incluiu 175 visualizações. Na análise da satisfação, 100% consideraram as temáticas interessantes e que as contribuições oriundas dessa aprendizagem foram importantes para a qualificação profissional. A maioria (86%) referiu se sentir segura quanto à retenção/apreensão do tema exposto. Conclusão: a implantação do serviço de Tele-educaçao proposto repercutiu em visualizações em todo estado e foi aprovada por 100% dos participantes, constituindo uma proposta de fácil acesso, abrangente e promissora no processo de educação permanente para os profissionais que atuam na APS e nos NASF’s.

ABSTRACT Objective: to evaluate the implementation of tele-education sessions for health professionals working in primary care and at Family Health Support Centers, addressing the repercussions of triple endemics to human communication health in the state of Pernambuco, Brazil. Methods: following approval from the ethics committee, an experience report was performed, in partnership with a telehealth center. The methods involved three steps: 1- creation of a flowchart for the implementation phases; 2- offer of educational sessions; 3- evaluation of satisfaction on the part of the public. Results: the implementation process consisted of five phases: institutional partnership, training, planning, offering of service and evaluation. Seven educational sessions were held, with a total of 175 viewings. Regarding satisfaction, 100% of the participants considered the topics interesting and that the learning experience was important to their professional qualification. Most of them (86%), reported feeling secure regarding the retention of the knowledge acquired. Conclusion: the implementation of the tele-education service had repercussions on statewide views and was approved by 100% of the participants, constituting a promising, easily accessible, comprehensive proposal in the continued education of health professionals working in primary care and at Family Health Support Centers.

Resumo Foi conduzida uma revisão sistemática da literatura de estudos epidemiológicos analíticos sobre os fatores associados à utilização dos serviços de saúde por homens idosos. Foram utilizadas quatro bases de dados, PubMed, Scopus, Web of Science e Lilacs, e os seguintes descritores “serviços de saúde”, “serviços de saúde para idosos”, “idoso”, “idoso de 80 anos ou mais”, “homem” e “saúde do homem” e seus correspondentes na língua inglesa. Foram encontrados 1.782 artigos, e após seleção com leitura independente de resumos e artigos completos, oito foram incluídos na revisão. O modelo de Utilização dos Serviços de Saúde guiou a apresentação dos resultados. Foram identificados como fatores predisponentes associados ao uso: local de residência, hábitos de vida, faixa etária, história familiar, educação e estado civil; fatores de capacitação: utilização de serviço privado de saúde e posse de seguro de saúde; por fim, fatores de necessidade: percepção do estado de saúde, susceptibilidade percebida ao stress, histórico médico, comorbidades e status funcional. A temática do envelhecimento da população masculina ainda é pouco estudada e merece atenção especial para reconhecer como os homens idosos expressam suas necessidades e como estão sendo absorvidos pelos sistemas de cuidado em saúde.

Four databases were researched, namely PubMed, Scopus, Web of Science and Lilacs and the following key words “health services,” “health services for the aged,” “aged,” “aged 80 and over,” “men” and “men’s health” and their corresponding expressions in Portuguese were used. After independent reading and selection of the abstracts and full texts of the 1782 articles located, only eight were included in this review. The Health Services Utilization model assisted in the presentation of the results. Predisposing factors associated with regard to the use of the healthcare services were identified: place of residence; lifestyle habits; age bracket; family history; education and marital status; training factors: private health services use and affiliation to a health insurance plan; effective factors of need; perception of health status; perceived susceptibility to stress; medical history; comorbidities; and functional status. The issue of aging among the male population is still poorly studied and deserves special attention to establish how elderly men express their needs and how they are being assisted by healthcare systems.

RESUMO Objetivo: conhecer as representações sociais que envolvem o desejo de engravidar de mulheres que vivem com o Vírus da Imunodeficiência Humana. Métodos: estudo exploratório-descritivo com abordagem qualitativa, realizada com 21 mulheres que se encontravam no Hospital Universitário Professor Alberto Antunes (Alagoas/Brasil) e no Posto de Atendimento Médico Salgadinho, centros de referência para pessoas que vivem com Vírus da Imunodeficiência Humana/aids, no período de maio a junho/2016. Informações coletadas mediante técnica de entrevista semiestruturada e áudio-gravada, submetidas à análise de conteúdo na perspectiva de Bardin. Utilizou-se como referencial teórico a Teoria das Representações Sociais. Resultados: foi evidenciado no estudo que as representações sociais das mulheres que vivem com o Vírus da Imunodeficiência Humana e desejam engravidar, ainda é vivenciado de forma dramática pelas mulheres, acarretando o surgimento de vários sentimentos que causam um conflito interno nas mesmas. Os sentimentos decorrentes das representações evidenciados no estudo foram: medo, frustração, impotência, culpa, tristeza, incerteza, apreensão, angústia, depressão e rejeição. Conclusão: as representações criadas ao longo da história de vida interferem nas decisões reprodutivas. Para as mulheres que vivem com o Vírus da Imunodeficiência Humana a decisão de engravidar, ou não, é influenciada pelo possível risco da transmissão vertical, a impossibilidade de amamentar, o tratamento, além dos aspectos sociais e morais que afetariam elas e seus filhos.

ABSTRACT Objective: to know the social representations that involve the desire to conceive of women living with the Human Immunodeficiency Virus. Methods: exploratory-descriptive study with qualitative approach, carried out with 21 women who were at the Professor Alberto Antunes University Hospital (Alagoas/Brasil) and at the Salgadinho Medical Care Unit, reference centers for people living with the Human Immunodeficiency Virus/AIDS, in the period from May to June/2016.The data was collected through a semi-structured and audio-recorded interview technique, submitted to content analysis from Bardin’s perspective. The Theory of Social Representations was used as theoretical reference. Results: it was evidenced in the study that the social representations of women living with the Human Immunodeficiency Virus that have the desire to conceive is still experienced in a dramatic way by women, causing the appearance of several feelings that cause an internal conflict in them. The feelings arising from the representations evidenced in the studies were: fear, frustration, impotence, guilt, sadness, uncertainty, apprehension, anguish, depression and rejection. Conclusion: the representations created throughout life history interfere with reproductive decisions. For women living with the Human Immunodeficiency Virus, the decision to conceive, or not, is influenced by the possible risk of vertical transmission, the inability to breastfeed, the treatment, and the social and moral aspects that would affect them and their children.

RESUMO Objetivo Verificar a inferência dos Diagnósticos de Enfermagem, Religiosidade prejudicada e Sofrimento espiritual em pessoas vivendo com HIV/AIDS. Métodos Trata-se de um estudo transversal, com abordagem quantitativa, realizado em um Serviço de Assistência Especializada de Recife, Pernambuco, de junho a novembro de 2015. Os resultados relacionados às 52 pessoas vivendo com HIV/AIDS entrevistadas foram analisados por três juízes enfermeiros. Resultados Estimou-se em 73,1% (38) a incidência de Sofrimento espiritual, 36,5% (19) de Religiosidade prejudicada, com número médio de características definidoras de 3,88±2,05 e 2,55±0,69. A principal característica definidora para a Religiosidade prejudicada foi: “relata necessidade de reconectar-se com crenças anteriores” (92,3%); e para Sofrimento espiritual: “expressa falta de finalidade na vida/expressa falta de significado na vida” (86,5%). Conclusões Os resultados apontam para a necessidade de considerar a dimensão religiosa-espiritual nos protocolos assistenciais e de pesquisa em enfermagem.

Métodos Se trata de un estudio transversal, cuantitativo, realizado en un servicio de atención especializada de Recife, Pernanbuco, de junio a noviembre de 2015. Los resultados de 52 encuestados fueron analizados por tres jueces enfermeras. Resultados Se estimó en un 73,1% (38) la incidencia de sufrimiento espiritual, el 36,5% (19) de la Religiosidad con deficiencias, con un promedio características definitorias de 3,88 ± 2,05 y 2,55 ± 0,69. La principal características definitorias de Religiosidad alterada: los informes tienen que volver a conectar con creencias anteriores (92,3%); y Sufrimiento espiritual: expresión de la falta de propósito en la vida/falta de sentido de la vida (86,5%). Conclusiones Los resultados apuntan a la necesidad de considerar la dimensión religiosa-espiritual en los protocolos de atención y la investigación en enfermería.

La principal características definitorias de Religiosidad alterada: los informes tienen que volver a conectar con creencias anteriores (92,3%); y Sufrimiento espiritual: expresión de la falta de propósito en la vida/falta de sentido de la vida (86,5%). Conclusiones Los resultados apuntan a la necesidad de considerar la dimensión religiosa-espiritual en los protocolos de atención y la investigación en enfermería.

RESUMO Objetivo Verificar a inferência dos Diagnósticos de Enfermagem, Religiosidade prejudicada e Sofrimento espiritual em pessoas vivendo com HIV/AIDS. Métodos Trata-se de um estudo transversal, com abordagem quantitativa, realizado em um Serviço de Assistência Especializada de Recife, Pernambuco, de junho a novembro de 2015. Os resultados relacionados às 52 pessoas vivendo com HIV/AIDS entrevistadas foram analisados por três juízes enfermeiros. Resultados Estimou-se em 73,1% (38) a incidência de Sofrimento espiritual, 36,5% (19) de Religiosidade prejudicada, com número médio de características definidoras de 3,88±2,05 e 2,55±0,69. A principal característica definidora para a Religiosidade prejudicada foi: “relata necessidade de reconectar-se com crenças anteriores” (92,3%); e para Sofrimento espiritual: “expressa falta de finalidade na vida/expressa falta de significado na vida” (86,5%). Conclusões Os resultados apontam para a necessidade de considerar a dimensão religiosa-espiritual nos protocolos assistenciais e de pesquisa em enfermagem.

OBJETIVO: Desenvolver uma versão transcultural do "ocular surface disease index" (OSDI) para a população brasileira e analisar sua validade e eficácia. MÉTODOS: Baseados nas orientações de Guillemin et al. Foram convidados 4 professores de inglês, 1 médico oftalmologista, 1 médica reumatologista, 2 residentes de oftalmologia e 1 nativo-americano que não fala o português. O procedimento seguiu as etapas: tradução para o português por dois professores de inglês, avaliação das duas traduções para uma versão única, aplicação do questionário a 27 pessoas do ambulatório de oftalmologia do Hospital Universitário Evangélico de Curitiba, questionando as dúvidas e realizando as alterações para melhor compreensão dos pacientes. Após isso foi feita uma retradução para o inglês por dois professores de inglês que moraram nos EUA e transformação para uma versão única, e leitura de um cidadão nativo dos EUA que não fala português para avaliar a compreensão do questionário. O questionário validado foi aplicado a 22 pessoas do ambulatório de oftalmologia do Hospital Universitário Evangélico de Curitiba para avaliar a eficácia inter e intraentrevistador. RESULTADOS: O estudo da concordância intraobservador mostrou que as questões 1, 2, 3, 4, 7 e 11 têm concordância substancial, a questão 8 tem concordância quase perfeita e as questões 5, 6, 9, 10 e 12 têm concordância moderada. Estudando-se a concordância interobservador nota-se que existem duas questões com concordância quase perfeita (questões 2 e 7), sete questões com concordância substancial (questões 1, 2, 4, 5, 6, 8 e 9) e três questões com concordância moderada (10, 11 e 12). CONCLUSÃO: A versão apresentada OSDI tem boa concordância intra e interobservador e pode ser usada em português para avaliação da qualidade de vida em pacientes com olho seco.

PURPOSE: To develop a cross-cultural Ocular Surface Disease Index (OSDI) for the Brazilian population and analyze its validity and effectiveness. METHODS: Based on the guidelines of Guillemin et al., four English teachers, an ophthalmologist, a rheumatologist, two ophthalmology residents and a native American who does not speak Portuguese were invited. The procedure followed the steps: translation into Portuguese by two English teachers; transformation of the two translations to a single version; application of this version of the questionnaire to 27 people in the ophthalmology clinic of the Evangelic University Hospital of Curitiba, asking questions and making changes for a better understanding by the patients; retranslation into English by two English teachers who lived in the USA.; assessment for a single version, and reading them by a native USA. citizen who does not speak Portuguese to assess the comprehension of the questionnaire. The validated questionnaire was administered to 22 people in the ophthalmology clinic of the Evangelic University Hospital of Curitiba to evaluate the effectiveness of inter-and intra-interviewer. RESULTS: The study of intra-observer for each of the questions showed that the questions 1, 2, 3, 4, 7 and 11 have substantial agreement; that the question 8 had almost perfect agreement and questions 5, 6, 9, 10 and 12 have moderate agreement. Studying the inter-observer agreement it was found that there are two questions with almost perfect agreement (2 and 7); substantial agreement with seven questions (questions 1, 2, 4, 5, 6, 8 and 9) and three questions with moderate agreement (10, 11 and 12). CONCLUSIONS: The obtained version of OSDI has good inter and intra-observer agreement and can be used in Portuguese to evaluate the quality of life of people with dry eye.
