A cardiomiopatia hipertrófica (CMH) é uma forma de doença do músculo cardíaco de causa genética, caracterizada pela hipertrofia das paredes ventriculares. O diagnóstico requer detecção por métodos de imagem (Ecocardiograma ou Ressonância Magnética Cardíaca) de qualquer segmento da parede do ventrículo esquerdo com espessura > 15 mm, sem outra causa provável. A análise genética permite identificar mutações de genes codificantes de diferentes estruturas do sarcômero responsáveis pelo desenvolvimento da CMH em cerca de 60% dos casos, permitindo o rastreio de familiares e aconselhamento genético, como parte importante do manejo dos pacientes e familiares. Vários conceitos sobre a CMH foram recentemente revistos, incluindo sua prevalência de 1 em 250 indivíduos, não sendo, portanto, uma doença rara, mas subdiagnosticada. A vasta maioria dos pacientes é assintomática. Naqueles sintomáticos, a obstrução do trato de saída do ventrículo esquerdo (OTSVE) é o principal distúrbio responsável pelos sintomas, devendo-se investigar a sua presença em todos os casos. Naqueles em que o ecocardiograma em repouso ou com Manobra de Valsalva não detecta gradiente intraventricular significativo (> 30 mmHg), devem ser submetidos à ecocardiografia com esforço físico para detecção da OTSVE. Pacientes com sintomas limitantes e grave OTSVE, refratários ao uso de betabloqueadores e verapamil, devem receber terapias de redução septal ou uso de novas drogas inibidoras da miosina cardíaca. Por fim, os pacientes adequadamente identificados com risco aumentado de morta súbita podem receber medida profilática com implante de cardiodesfibrilador implantável (CDI).
La miocardiopatía hipertrófica (MCH) es una forma de enfermedad cardíaca de origen genético, caracterizada por el engrosamiento de las paredes ventriculares. El diagnóstico requiere la detección mediante métodos de imagen (Ecocardiograma o Resonancia Magnética Cardíaca) que muestren algún segmento de la pared ventricular izquierda con un grosor > 15 mm, sin otra causa probable. El análisis genético permite identificar mutaciones en genes que codifican diferentes estructuras del sarcómero responsables del desarrollo de la MCH en aproximadamente el 60% de los casos, lo que permite el tamizaje de familiares y el asesoramiento genético, como parte importante del manejo de pacientes y familiares. Varios conceptos sobre la MCH han sido revisados recientemente, incluida su prevalencia de 1 entre 250 individuos, por lo tanto, no es una enfermedad rara, sino subdiagnosticada. La gran mayoría de los pacientes son asintomáticos. En los casos sintomáticos, la obstrucción del tracto de salida ventricular izquierdo (TSVI) es el trastorno principal responsable de los síntomas, y su presencia debe investigarse en todos los casos. En aquellos en los que el ecocardiograma en reposo o la maniobra de Valsalva no detecta un gradiente intraventricular significativo (> 30 mmHg), deben someterse a ecocardiografía de esfuerzo para detectar la obstrucción del TSVI. Los pacientes con síntomas limitantes y obstrucción grave del TSVI, refractarios al uso de betabloqueantes y verapamilo, deben recibir terapias de reducción septal o usar nuevos medicamentos inhibidores de la miosina cardíaca. Finalmente, los pacientes adecuadamente identificados con un riesgo aumentado de muerte súbita pueden recibir medidas profilácticas con el implante de un cardioversor-desfibrilador implantable (CDI).
Hypertrophic cardiomyopathy (HCM) is a form of genetically caused heart muscle disease, characterized by the thickening of the ventricular walls. Diagnosis requires detection through imaging methods (Echocardiogram or Cardiac Magnetic Resonance) showing any segment of the left ventricular wall with a thickness > 15 mm, without any other probable cause. Genetic analysis allows the identification of mutations in genes encoding different structures of the sarcomere responsible for the development of HCM in about 60% of cases, enabling screening of family members and genetic counseling, as an important part of patient and family management. Several concepts about HCM have recently been reviewed, including its prevalence of 1 in 250 individuals, hence not a rare but rather underdiagnosed disease. The vast majority of patients are asymptomatic. In symptomatic cases, obstruction of the left ventricular outflow tract (LVOT) is the primary disorder responsible for symptoms, and its presence should be investigated in all cases. In those where resting echocardiogram or Valsalva maneuver does not detect significant intraventricular gradient (> 30 mmHg), they should undergo stress echocardiography to detect LVOT obstruction. Patients with limiting symptoms and severe LVOT obstruction, refractory to beta-blockers and verapamil, should receive septal reduction therapies or use new drugs inhibiting cardiac myosin. Finally, appropriately identified patients at increased risk of sudden death may receive prophylactic measure with implantable cardioverter-defibrillator (ICD) implantation.

Resumo Objetivou-se mensurar o desempenho zootécnico e a qualidade de ovos de codornas japonesas alojadas sob diferentes densidades e alimentadas com rações contendo parede celular de levedura (PCL). Foram utilizadas 576 codornas japonesas (Coturnix japonica) com 43 semanas de idade e 76% de postura, com peso inicial de 158,50 ± 5,41 g distribuídas em delineamento inteiramente ao acaso em esquema fatorial 3 × 2 (três níveis de PCL: 0; 500 e 750 g.ton−1 e duas densidades de alojamento: 81,5 e 92,4 cm2/ave), com seis repetições de 17 e 15 codornas por unidade experimental, respectivamente. Foram avaliados: consumo de ração, produção de ovos/ave/dia, produção de ovos/ave alojada, produção de ovos comercializáveis, massa de ovos, conversão alimentar por dúzia e por massa de ovos e viabilidade das aves; peso do ovo, peso específico, porcentagem de gema, de albúmen e de casca e espessura da casca. Não houve interação entre os níveis de inclusão de PCL e densidade de alojamento para os parâmetros avaliados, exceto para peso do ovo. Codornas alojadas em gaiolas com 92,4 cm2/ave alimentadas com 500 g.ton−1 de PCL apresentaram maior peso do ovo. A espessura de casca foi influenciada de forma independente pela densidade de alojamento, a menor densidade (92,4 cm|2/ave) promoveu maior espessura de casca. Abstract The objective of this study was to measure the zootechnical performance and egg quality of Japanese quails housed at different densities and fed diets containing yeast cell walls (YCWs). Five hundred and seventy-six quail (Coturnix coturnix japonica) were distributed at 43 weeks of age, and 76% were laid, with an initial weight of 158.50 ± 5.41 g, in a completely randomized design in a 3 × 2 factorial arrangement (three YCW levels: 0, 500, and 750 g.ton−1 and two housing densities: 81.5 and 92.4 cm2/quail), with six replicates of 17 and 15 quail per experimental unit, respectively. The following parameters were evaluated: feed intake, egg production/bird/day, egg production/housed quail, marketable egg production, egg mass, feed conversion per dozen eggs, egg mass and viability, egg weight, specific egg weight, percentage of yolk, albumen and shell, and shell thickness. The means of the three cycles of 21 days were subjected to analysis of variance using the statistical software Sisvar. There was no significant interaction effect between YCW inclusion level and cage density on zootechnical performance parameters or egg quality, except for egg weight, which suggested that YCW addition, regardless of cage density, did not affect the results. It was observed that the eggs of quails housed in cages with 92.4 cm2/bird feed and 500 g.ton1 YCW had greater egg weights. Shell thickness was independently influenced by cage density, and the lowest density (92.4 cm2/bird) promoted greater shell thickness. The inclusion of 500 g.ton1 of yeast cell wall material in the diet of Japanese quails housed at a density of 92.4 cm2/bird improved egg weight and shell thickness without negatively affecting the other parameters of egg quality or zootechnical performance. Abstract Mucopolysaccharidosis type IIIB (MPS IIIB) is caused by deficiency of alpha-N-acetylglucosaminidase, leading to storage of heparan sulphate. The disease is characterized by intellectual disability and hyperactivity, among other neurological and somatic features. Here we studied retrospective data from a total of 19 MPS IIIB patients from Brazil, aiming to evaluate disease progression. Mean age at diagnosis was 7.2 years. Speech delay was one of the first symptoms to be identified, around 2-3 years of age. Behavioral alterations include hyperactivity and aggressiveness, starting around age four. By the end of the first decade, patients lost acquired abilities such as speech and ability to walk. Furthermore, as disease progresses, respiratory, cardiovascular and joint abnormalities were found in more than 50% of the patients, along with organomegaly. Most common cause of death was respiratory problems. Abstract Background Systemic sclerosis (SSc) is a rare chronic autoimmune disease with heterogeneous manifestations. In the last decade, several clinical trials have been conducted to evaluate new treatment options for SSc. The purpose of this work is to update the recommendations of the Brazilian Society of Rheumatology in light of the new evidence available for the pharmacological management of SSc. Methods A systematic review including randomized clinical trials (RCTs) for predefined questions that were elaborated according to the Patient/Population, Intervention, Comparison, and Outcomes (PICO) strategy was conducted. The rating of the available evidence was performed according to the Grading of Recommendations Assessment, Development and Evaluation (GRADE) methodology. To become a recommendation, at least 75% agreement of the voting panel was needed. Results Six recommendations were elaborated regarding the pharmacological treatment of Raynaud’s phenomenon, the treatment (healing) and prevention of digital ulcers, skin involvement, interstitial lung disease (ILD) and gastrointestinal involvement in SSc patients based on results available from RCTs. New drugs, such as rituximab, were included as therapeutic options for skin involvement, and rituximab, tocilizumab and nintedanib were included as therapeutic options for ILD. Recommendations for the pharmacological treatment of scleroderma renal crisis and musculoskeletal involvement were elaborated based on the expert opinion of the voting panel, as no placebo-controlled RCTs were found. Conclusion These guidelines updated and incorporated new treatment options for the management of SSc based on evidence from the literature and expert opinion regarding SSc, providing support for decision-making in clinical practice. RESUMO A fabricação e o envase de vacinas tornaram-se primordiais no cenário mundial em função do ressurgimento da pandemia da COVID-19 desde 2020. O processo de obtenção da vacina vai desde estudos laboratoriais até o seu armazenamento em tanques reservatórios de produto. Para tanto, a vacina passa por bombas de envase e mangueiras sanitárias, até o envase efetivo nos frascos e ampolas por meio de agulhas de envase. No entanto, as mesmas são desenvolvidas em aço inoxidável 316L, que quando exposto a líquidos estagnados ou em movimento podem corroer e contaminar o fármaco. Nesse contexto, esse trabalho tem como objetivo avaliar se existe contaminação química das agulhas fornecidas pela Empresa Teksul Soluções em Envase. Para tanto, 21 agulhas de envase foram imersas em excipientes, com propriedades físico-químicas que se equivalem à vacina, durante 0, 35, 63 e 94 dias. Os excipientes foram avaliados quanto ao pH e por absorção atômica, enquanto as agulhas foram avaliadas por análise de perda e ganho de massa, análises microscópicas obtidas ao MEV (Microscópio Eletrônico de Varredura) e testes eletroquímicos de corrosão. ABSTRACT The manufacturing and packaging of vaccines have become essential on the global stage due to the resurgence of the COVID-19 pandemic since 2020. The process of obtaining the vaccine ranges from laboratory studies to its storage in product reservoir tanks. To do so, the vaccine passes through filling pumps and sanitary hoses, until it is effectively filled into vials and ampoules using filling needles. However, they are developed in 316L stainless steel, which when exposed to stagnant or moving liquids can corrode and contaminate the drug. In this context, this work aims to evaluate whether there is chemical contamination of the needles supplied by the company Teksul Soluções em Envase. To this end, 21 filling needles were immersed in excipients, with physicochemical properties that are equivalent to the vaccine, for 0, 35, 63 and 94 days. ABSTRACT Objective Currently programmed cell death protein 1 (PD-1) inhibitors in combination with other therapies are being evaluated to determine their efficacy in cancer treatment. However, the effect of PD-ligand (L) 1 expression on disease outcomes in stage III (EC III) non-small cell lung cancer is not completely understood. Therefore, this study aimed to assess the influence of PD-L1 expression on the outcomes of EC III non-small cell lung cancer. Methods This study was conducted on patients diagnosed with EC III non-small cell lung cancer who underwent treatment at a tertiary care hospital. PD-L1 expression was determined using immunohistochemical staining, all patients expressed PD-L1. Survival was estimated using the Kaplan-Meier method. Relationships between variables were assessed using Cox proportional regression models. Results A total of 49 patients (median age=69 years) with EC III non-small cell lung cancer and PD-L1 expression were evaluated. More than half of the patients were men, and most were regular smokers. The patients were treated with neoadjuvant chemotherapy, surgery, or sequential or combined chemotherapy and radiotherapy. The median progression-free survival of the entire cohort was 14.2 months, and the median overall survival was 20 months. There was no significant association between PD-L1 expression and disease progression, clinical characteristics, or overall survival. Conclusions PD-L1 expression was not correlated with EC III non-small cell lung cancer outcomes. Whether these findings differ from the association with immune checkpoint inhibitors remains to be addressed in future studies. Abstract Objectives This study aimed to evaluate quality-of-life and satisfaction outcomes in patients undergoing the MES using the Portuguese version of the Nasal Obstruction Symptom Evaluation (NOSE-p) and Rhinoplasty Outcome Evaluation (ROE), and also to evaluate the frequency of possible complications of this technique. Methods We conducted a single-center prospective study with patients who had the indication for MES, from May 2016 to September 2020 at the Facial Plastic Surgery Clinic of Otolaryngology Department of the Hospital de Clinicas de Porto Alegre. The primary outcome was the relative postoperative change in NOSE-p. Secondary outcome was the variation in ROE, a validated quality-of-life questionnaire for rhinoplasty patients. Results Of the 31 patients submitted to extracorporeal septorhinoplasty who were evaluated, twenty-seven patients were included. Preoperative and postoperative NOSE-p scale scores were 65.2 ± 29.9 and 23.5 ± 26.7, respectively (mean differences of 42.04; [95% CI 27.35-56.73]; p< 0.0001). Pre and postoperative ROE scores were 38.3 ± 24.3 vs. 67.29 ± 29.7, respectively (mean differences of −29.02; [95% CI −40.5 to −17.5]; p= 0.0001). Residual septal deviation was verified in 2 patients (7.4%). Conclusion Most of the patients submitted to modified extracorporeal septoplasty had a significant improvement in quality of life scores of nasal obstruction, with good aesthetical outcomes and low indices of postoperative complications. RESUMO RACIONAL: O bypass gástrico de uma anastomose (BGU tem ganhado destaque na busca por melhores resultados na cirurgia bariátrica. No entanto, a eficácia e segurança do BGUA, em relação ao bypass gástrico em Y de Roux (BGYR) e à gastrectomia vertical (GV), permanecem imprecisas. OBJETIVOS: Comparar a eficácia e segurança do BGUA em relação ao BGYR e GV no tratamento da obesidade. MÉTODOS: Realizamos uma busca sistemática nas bases de dados do PubMed, EMBASE, Cochrane, Lilacs e Google Scholar por ensaios clínicos randomizados que comparassem o BGUA ao BGYR ou GV na abordagem cirúrgica da obesidade. Agrupamos resultados para índice de massa corporal, porcentagem da perda de peso em excesso, remissão do diabetes mellitus tipo 2, complicações e taxas de doença do refluxo gastroesofágico. As análises estatísticas foram realizadas empregando o software R, versão 4.2.3. RESULTADOS: Dados de 854 pacientes foram extraídos de 11 ensaios clínicos randomizados, dos quais 422 (49,4%) foram submetidos ao BGUA, com seguimento médio variando de seis meses a cinco anos. Na análise combinada, o BGUA esteve associado a uma porcentagem da perda de peso em excesso significativamente maior no acompanhamento de um ano e a um índice de massa corporal significativamente menor no acompanhamento de cinco anos em pacientes submetidos ao BGUA. Por outro lado, as taxas de remissão da diabetes mellitus tipo 2, de complicações e da doença do refluxo gastroesofágico não foram significativamente diferentes entre os grupos. A qualidade geral das evidências foi considerada muito baixa. CONCLUSÕES: Nossos resultados corroboram a eficácia comparável do BGUA em relação ao BGYR e GV no tratamento da obesidade, mantendo diferenças não significativas nas taxas de remissão da diabetes mellitus tipo 2, complicações e doença do refluxo gastroesofágico. ABSTRACT BACKGROUND: One anastomosis gastric bypass (OAGB) has gained prominence in the search for better results in bariatric surgery. However, its efficacy and safety compared to Roux-en-Y gastric bypass (RYGB) and sleeve gastrectomy (SG) remain ill-defined. AIMS: To compare the efficacy and safety of OAGB relative to RYGB and SG in the treatment of obesity. METHODS: We systematically searched PubMed, EMBASE, Cochrane Library, Lilacs, and Google Scholar databases for randomized controlled trials comparing OAGB with RYGB or SG in the surgical approach to obesity. We pooled outcomes for body mass index, percentage of excess weight loss, type-2 diabetes mellitus remission, complications, and gastroesophageal reflux disease. Statistical analyses were performed with R software (version 4.2.3). RESULTS: Data on 854 patients were extracted from 11 randomized controlled trials, of which 422 (49.4%) were submitted to OAGB with mean follow-up ranging from six months to five years. The meta-analysis revealed a significantly higher percentage of excess weight loss at 1-year follow-up and a significantly lower body mass index at 5-year follow-up in OAGB patients. Conversely, rates of type-2 diabetes mellitus remission, complications, and gastroesophageal reflux disease were not significantly different between groups. The overall quality of evidence was considered very low. CONCLUSIONS: Our results corroborate the comparable efficacy of OAGB in relation to RYGB and SG in the treatment of obesity, maintaining no significant differences in type-2 diabetes mellitus remission, complications, and gastroesophageal reflux disease rates. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. RESUMO RACIONAL: O colangiocarcinoma intra-hepático (CCi) foi considerado uma contraindicação para transplante hepático. No entanto, estudos recentes têm demonstrado que casos altamente selecionados de pacientes com boa resposta à terapia neoadjuvante podem ter uma taxa de sobrevida aceitável ao serem submetidos a transplante hepático. OBJETIVOS: Apresentar dois casos de pacientes com colangiocarcinoma intra-hepático sem doença extra-hepática submetidos a transplante hepático de doador vivo, após quimioterapia neoadjuvante. MÉTODOS: Dois pacientes com diagnóstico histopatológico de Cci, localmente avançado, inelegíveis para ressecção e sem evidência de doença extra-hepática. RESULTADOS: Esses pacientes foram submetidos a pelo menos nove sessões de quimioterapia neoadjuvante incluindo Gemcitabina e Cisplatina com ou sem adição de agentes imunobiológicos e com resposta tumoral radiológica. Em seguida, foram submetidos a transplante hepático de doador vivo. O tempo médio de acompanhamento foi de 15 meses, sem sinais clínicos ou radiológicos de doença. CONCLUSÕES: Em pacientes bem selecionados, sem doença extra-hepática, o transplante de fígado de doador vivo é uma opção terapêutica potencial para colangiocarcinoma intra-hepático. ABSTRACT BACKGROUND: Intrahepatic cholangiocarcinoma (iCCA) was considered a contraindication for liver transplantation. However, recent studies have shown that highly selected cases of patients with a good response to neoadjuvant therapy may achieve acceptable survival rates when following liver transplantation. AIMS: To present two cases of patients with iCCA, without extrahepatic disease, who underwent living donor liver transplantation after receiving neoadjuvant chemotherapy. METHODS: Two cases of patients with histopathological diagnosis of locally advanced iCCA, ineligible for resection and without evidence of extrahepatic disease, are presented. RESULTS: These patients underwent at least nine sessions of neoadjuvant chemotherapy, including Gemcitabine and Cisplatin, with or without the addition of immunobiological agents, resulting in a radiological tumor response. They subsequently underwent living donor liver transplantation. The average follow-up time was 15 months, with no clinical or radiological signs of disease. CONCLUSIONS: In well-selected patients without extrahepatic disease, living donor liver transplantation represents a potential therapeutic option for iCCA. ABSTRACT This study was conducted to evaluate the relationship among digestibility, residual feed intake (RFI), and enteric methane emission in growing Nellore cattle (Bos indicus) divergently classified based on RFI phenotype and breeding value (EBV-RFI). Animals (n = 122) subjected to performance testing (forage:concentrate ratio of 60:40) in two test groups were classified based on RFI. A sample of 80 animals classified as low (−0.748±0.076 kg DM/day) or high (0.775±0.075 kg DM/day) RFI was evaluated regarding feed compounds digestibility, fecal excretion, and methane emission (CH4, g/day). Statistical mixed models included fixed effects of RFI (or EBV-RFI), linear effect of initial age as covariate, and the random effects of the test group. There was no difference in the digestibility of feed compounds between the most and least efficient animals. However, dry matter intake (DMI; 6.92 vs. 8.66 kg DM/day) and feed conversion (7.93 vs. 9.42 kg/kg) were lesser in low RFI animals. On average, low RFI animals emitted 14.3 g less CH4 per day (174 vs. 188 g CH4/day); however, CH4 emission expressed as g/kg DMI (23.1 vs. 20.1) and the percentage of gross energy intake lost as CH4 (8.13 vs. 7.08%) were greater for these animals. These results showed the benefits of using more feed efficient animals in the beef production chain, i.e., animals that exhibit lower feed intake, lower fecal excretion, and lower enteric methane emission without differences in weight gain or body weight. However, the variations in feed efficiency among them cannot be explained by differences in dry matter or feed compounds digestibility. ABSTRACT. For a banana tree to reach its productive potential, plants must be nutritionally balanced. Therefore, it is important to use methods that diagnose the nutritional status of plants through chemical leaf analysis, considering the relationships between nutrients and taking advantage of productivity and nutritional monitoring data from orchards. This study aimed to establish norms and ranges of nutrient sufficiency for ‘Nanica’ banana trees in Vale do Ribeira, São Paulo state, using the Diagnosis and Recommendation Integrated System (DRIS) method. Productivity data and chemical analysis of the leaf tissue of 291 commercial plots of Nanica banana trees, collected between 2015 and 2020 in Vale do Ribeira, were used. The DRIS method was used to establish norms and sufficiency ranges for N, P, K, Ca, Mg, S, B, Cu, Fe, Mn, and Zn. Fruit yield did not correlate with the average nutritional balance index (NBIa), indicating that its variation was affected by non-nutritional factors. The contents of K, Ca, Mg, S, B, Cu, Fe, Mn, and Zn showed high correlations with their respective nutritional indices. The sufficiency ranges and adequate nutrient contents differed from the values established for bananas in the literature.
