ABSTRACT Objective: Group A Streptococcus (GAS) are Gram-positive cocci that colonize the nasopharynx and/or skin and in rare cases may cause severe invasive infections. Although these infections decreased during the COVID-19 pandemic, some countries have observed an increased number of invasive GAS (iGAS) diseases in recent years. The objective of this study was to describe a series of iGAS diseases in a referral hospital for the treatment of pediatric infectious disease in Minas Gerais State, Brazil, between September 2022 and August 2023. The data were obtained from medical records, and GAS was isolated from blood, pleural, cerebrospinal, or joint fluid. Case description: A total of nine patients, four male and five female, between eight months and ten years of age, were admitted. Of the patients observed, three (33%) had pneumonia, three (33%) had toxic shock syndrome, three (33%) had septic arthritis, one (11%) had osteomyelitis, two (22%) had meningitis, and two (22%) cellulitis. The majority of patients (5) experienced complications, including pleural effusion, septic shock, cerebral vasculitis, and limb amputations. In total, seven (75%) patients were admitted to the intensive care unit, and one patient died. Comments: It is important to be aware of the possibility of S. pyogenes infection in children with pulmonary, neurological, osteoarticular, skin infections or in severe cases of septic shock. Given the absence of specific clinical signs, epidemiological data showing an increase in the local incidence of S. pyogenes infections is of great importance and should serve as an alert for the diagnosis of the disease. RESUMO Objetivo: Streptococcus do grupo A (SGA) são cocos gram-positivos que colonizam a nasofaringe e/ou a pele e em casos raros podem causar infecções graves. Embora essas infecções tenham diminuído durante a pandemia da COVID-19, alguns países observaram um aumento no número de doenças invasivas pelo SGA nos últimos anos. O objetivo deste estudo foi descrever uma série de doenças invasivas pelo S. pyogenes em crianças internadas em um hospital de referência para tratamento de doenças infecciosas em Minas Gerais, Brasil, entre setembro de 2022 e agosto de 2023. Os dados foram obtidos dos prontuários médicos. O SGA foi isolado do sangue, líquido pleural, cerebrospinal ou fluido articular. Descrição do caso: O total de nove pacientes, quatro do sexo masculino e cinco do sexo feminino, com idade entre oito meses e dez anos, foram admitidos. Ao todo, três (33%) tiveram pneumonia, três (33%) síndrome do choque tóxico, três (33%) artrite séptica, um (11%) osteomielite, dois (22%) meningite e dois (22%) celulite. A maioria dos pacientes (5) apresentou complicações, incluindo derrame pleural, choque séptico, vasculite cerebral e amputações de membros. Ao todo, sete (75%) pacientes foram admitidos na unidade de terapia intensiva, e um paciente morreu. Comentários: É importante estar atento à possibilidade de infeção por S. pyogenes em crianças com infecções pulmonares, neurológicas, osteoarticulares, cutâneas ou em casos graves de choque séptico. Diante da ausência de sinais clínicos específicos, dados epidemiológicos que demonstrem o aumento da incidência da doença são de grande importância e devem servir de alerta para o diagnóstico da doença. ABSTRACT Objective: To examine the associations between self-reported screen time and symptoms of stress, anxiety, and depression in adolescents. Methods: A cross-sectional study was conducted with 982 adolescents aged between 12 and 15 years, enrolled in public schools in Jacarezinho (PR), Brazil. Screen time was assessed by the question “Considering a typical day, how much time do you spend watching TV, playing videogame, using computer or smartphone?” The DASS-21 questionnaire (short form) was used to assess symptoms of depression, anxiety, and stress. Crude and adjusted analyses (age, sex, and maternal level of education) between screen time and mental disorders symptoms were performed using general linear regression models, with Poisson distribution, with significance level at p<0.05. Results: Higher depressive symptoms were observed in adolescents who reported screen time of 4–6 hours/day (PR 1.35, 95%CI 1.13–1.61) and ≥6 hours/day (PR 1.88, 95%CI 1.62–2.19), compared with their pairs with <2 hours/day. The same was observed for anxiety symptoms with screen time of 4–6 hours/day (PR 1.23, 95%CI 1.04–1.46) and ≥6 hours/day (PR 1.50, 95%CI 1.28–1.77); and stress, with 4–6 hours/day (PR 1.25, 95%CI 1.08–1.44) and ≥6 hours/day (PR 1.49, 95%CI 1.30–1.71), also compared with their pairs with <2 hours/day. Conclusions: Screen time was positively associated with depressive, anxiety, and stress symptoms in adolescents. Special attention should be given to those who spend more than four hours a day in front of a screen. RESUMO Objetivo: Examinar a associação entre o tempo de tela autorreportado e os sintomas de estresse, ansiedade e depressão em adolescentes. Métodos: Foi realizado estudo com delineamento transversal com 982 adolescentes entre 12-15 anos, matriculados na rede pública de ensino da cidade de Jacarezinho (PR). Para mensurar o tempo de tela, os adolescentes responderam à pergunta: “Considerando um dia típico, quanto tempo você passa assistindo TV, jogando videogame, usando computador ou smartphone?”. O questionário DASS-21 (versão curta) foi utilizado para avaliar os sintomas de depressão, ansiedade e estresse. Análises brutas e ajustadas (idade, sexo e nível de escolaridade materna) entre tempo de tela e indicadores de saúde mental foram realizadas por meio do modelo de regressão linear generalizado, com distribuição de Poisson, adotando a significância de p<0,05. Resultados: Maiores sintomas depressivos foram observados em adolescentes com tempo de tela de 4–6 horas/dia (RP 1,35, IC95% 1,13–1,61) e ≥6 horas/dia (RP 1,88, IC95% 1,62–2,19) quando comparados com seus pares com ≤2 horas/dia. Abstract Light wells in residential buildings represent a context that has received relatively little attention in daylighting analyses. This study aims to analyze the daylighting performance of a residential space with a window facing a light well, in order to contribute to the revision of NBR 15575:2013. The study conducted 1,152 daylighting analyses considering windows facing a light well, located on the first and fourth floors of a 4-story building. Clear and intermediate skies were considered in four Brazilian cities with different latitudes. Both the prescriptive method and point-in-time simulations were applied. The effectiveness of achieving daylit interior spaces through light wells strongly depends on the geometry of the light well and the external illuminance conditions of the city. Narrow light wells were found to be unsuitable for adequately daylighting the lower floor spaces. Furthermore, the prescriptive method was insufficient in representing daylighting originating solely from external wall interreflections. The study found that intermediate and superior performance levels, when achieved through point-in-time or spatial daylighting availability simulations, were never attained for the first-floor space. Resumo Poços de luz em edifícios residenciais representam um contexto com pouca atenção nas análises de iluminação natural. Esse estudo propõe analisar o desempenho de iluminação natural de um espaço residencial com janela voltada para poço de luz visando objetivo de contribuir com a revisão da NBR 15575:2013. O estudo realizou 1.152 análises de desempenho considerando a janela voltada para o poço de luz localizada no 1º e 4º pavimentos de uma edificação de quatro pavimentos. Foram considerados céu claros e intermediários em quatro cidades brasileiras com diferentes latitudes. O método prescritivo e simulação ponto-no-tempo foram aplicados. A solução de iluminação de espaços internos por meio de poços de luz depende fortemente de sua geometria e das condições de iluminância externa da cidade. Poços estreitos mostraram-se inaptos para levar luz suficiente para os ambientes inferiores. O método prescritivo não é capaz de representar adequadamente situações de iluminação natural oriunda exclusivamente de interreflexões de paredes externas. Os níveis intermediários e superiores de desempenho por simulação ponto-no-tempo ou por análises baseadas no clima não foram atendidos no 1º pavimento. Resumo: Este trabalho apresenta os resultados do estudo Eu Quero é Mais! A Vida de Profissionais do Sexo Durante a Pandemia da COVID-19, integrante do programa de investigação comunitária EPIC. O estudo analisou os efeitos da pandemia nas vidas de trabalhadoras sexuais cis, trans e travestis em nove Unidades da Federação e 11 cidades brasileiras ao longo de 2020 e 2021. O artigo tem como foco o componente qualitativo do estudo baseado em entrevistas semiestruturadas realizadas de forma presencial e remota com 43 trabalhadoras sexuais, e seu cotejamento com o componente quantitativo. Os efeitos são analisados em relação com o marco pandêmico brasileiro, considerando as dimensões sociais, econômicas e políticas do vírus da COVID-19. Entre as temáticas chaves da análise, se destacam: casos de adoecimento, práticas localizadas de isolamento social, práticas de prevenção e gerenciamento de cuidado, vacinação individual e estratégias coletivas de vacinação. Compartilhamos também as respostas cotidianas e ativistas traçadas por trabalhadoras sexuais numa agenda política que se contrapõe à lógica individualista, familiarista, doméstica e neoliberal de isolamento, por meio de perspectivas comunitárias de cuidado, o que se desenhou como a única linha de ação em saúde para a categoria durante a pandemia. As ações coletivas reposicionam o trabalho sexual na interface entre a saúde pública e os direitos humanos e tomam como princípio os conhecimentos das ruas, desde os ativismos, e o poder de decisão delas próprias sobre seus corpos.

La Vida de las Profesionales del Sexo Durante la Pandemia de la COVID-19, parte del programa de investigación comunitaria EPIC. El estudio analizó los efectos de la pandemia en la vida de trabajadoras sexuales cis, trans y travestis en 9 estados y 11 ciudades brasileñas a lo largo del 2020 y del 2021. El artículo se centra en el componente cualitativo del estudio basado en entrevistas semiestructuradas realizadas de forma presencial y remota a 43 trabajadoras sexuales y su comparación con el componente cuantitativo. Los efectos se analizan con relación al marco pandémico brasileño, considerando las dimensiones sociales, económicas y políticas del virus de la COVID-19. Entre las temáticas clave del análisis, destacan: casos de enfermedad, prácticas localizadas de aislamiento social, prácticas de prevención y gestión del cuidado, vacunación individual y estrategias colectivas de vacunación. Compartimos también las respuestas cotidianas y activistas esbozadas por las trabajadoras sexuales en una agenda política que se opone a la lógica individualista, familiarista, doméstica y neoliberal del aislamiento, por medio de perspectivas comunitarias de cuidado, lo que se diseñó como la única línea de acción en salud para la categoría durante la pandemia. Las acciones colectivas reposicionan el trabajo sexual en la interfaz entre la salud pública y los derechos humanos y toman como principio el conocimiento de la calle, desde el activismo, y su poder de decisión sobre sus cuerpos.
Abstract: This paper describes the results of the study I Want More! The Lives of Sex Workers During the COVID-19 Pandemic, which is part of the EPIC community research program. The study analyzed the effects of the pandemic on the lives of cis, trans and travesti sex workers in nine Brazilian states and 11 cities throughout 2020 and 2021. This article focuses on the qualitative component of the study, which was based on semi-structured, remote and face-to-face interviews carried out with 43 sex workers, and its comparison with the quantitative component. The effects are analyzed in relation to the Brazilian pandemic framework, considering the social, economic and political dimensions of the COVID-19 virus. Some of the key themes of the analysis are cases of illness, specific social isolation practices, prevention and care management practices, individual vaccination and collective vaccination strategies. We also share the daily and activist responses drawn up by sex workers in a political agenda that opposes the individualistic, familialist, domestic, and neoliberal logic of isolation by adopting community care perspectives, which was the only line of health action for this work category during the pandemic. Collective actions reposition sex work at the interface between public health and human rights and take as their principle the “street knowledge”, from activism, and the workers’ power of decision over their own bodies.

Resumo Diante das evidências científicas acerca dos malefícios à saúde do consumo de alimentos ultraprocessados, a regulação dos ambientes alimentares vem sendo priorizada como forma de promover uma alimentação adequada e saudável. Nesse contexto, foi proposto o Projeto de Lei 4198/2021 na Assembleia Legislativa do Rio de Janeiro, para proibir o comércio de ultraprocessados nas escolas do estado. Na segunda sessão em plenário da proposição, houve ampla utilização de retóricas conservadoras pelos deputados contrários. Neste presente ensaio foram classificados esses posicionamentos, encontrando-se pontos comuns entre eles, à luz da análise da tramitação legislativa. Ao realizar essa categorização, foi possível identificar três bases argumentativas exploradas: a defesa da liberdade de comércio; o direito à “pátria família”, de primazia da família na educação infantojuvenil; e a responsabilização individual da obesidade. Tais retóricas foram analisadas em conjunto com suas contraposições, presentes nas conclusões da literatura especializada. Para além da pluralidade de ideias do debate legislativo, foi possível identificar como a discussão do PL 4198/21 foi permeada pela reprodução de desinformação e violência. Desse modo, o escrutínio das retóricas conservadoras utilizadas e a compreensão do contexto dessa discussão são contribuições aqui propostas para a consolidação do repertório de defesa da saúde de crianças e adolescentes.
Abstract Considering the scientific evidence regarding the harmful effects on health from consuming ultra-processed foods, the regulation on food environments has been prioritized as a way of promoting adequate and healthy eating. In this context, Bill 4198/2021 was proposed in the Legislative assembly of Rio de Janeiro, to ban the sale of ultra-processed foods in the schools in the state. In the second voting session of the proposition conservative rhetoric was widely used by the opposing congressmen. In this essay, these positions were classified, finding common points between them, considering the analysis of the legislative process. In making this categorization, it was possible to identify three argumentative bases that were explored: the defense of freedom of trade; the right to a “family autonomy,” the primacy of the family in children’s education; and individual responsibility for obesity. These rhetorics have been analyzed together with their counterpoint, in the conclusions of the specialized literature. Beyond the plurality of ideas in the legislative debate, it was possible to identify how the discussion of Bill 4198/21 was permeated by the reproduction of disinformation and violence. In this way, the scrutiny of the conservative rhetoric used and the understanding of the context of this discussion are contributions proposed here to consolidate the repertoire of defense of the health of children and adolescents.

Abstract Objectives: to identify the scientific evidence on excessively resistant and multidrug resistant tuberculosis in pediatric patients. Methods: this is a scope review of the literature, with a guiding question: “What is the scientific evidence on multidrug-resistant tuberculosis (MDR-TB) and extensively drug-resistant tuberculosis in pediatric patients?”. The research used the descriptors: “extensively drug-resistant tuberculosis” OR “multidrug-resistant tuberculosis” AND “pediatrics”. The research was carried out in a double-blind manner in the following databases of the Medical Literature Analysis and Retrieval System Online, Regional Office for the Western Pacific’s Institutional Repository for Information Sharing, Embase/Elsevier and International Clinical Trials Registry Platform, with a temporal cut-off from 2011 to 2021, sending a final synthesized sample of 18 articles, which evaluated the methodological content through the level of evidence. Results: the results show the lack of research with a high level of evidence related to MDR-TB in children, the lack of adequate dosage of second-line drugs for the pediatric population and the importance of drug sensitivity testing for the cases of treatment Conclusions: it was identified that the obstacles to MDR-TB treatment were concentrated in the lack of detailed protocols, safe drug dosages with a low side effect, and mainly in the social health determinants and disease process involving MDR-TB. Resumo Objetivos: identificar as evidências científicas sobre tuberculose excessivamente resistente e multidroga resistente em pacientes pediátricos. Métodos: trata-se de uma revisão de escopo da literatura, tendo como questão norteadora: “Quais as evidências científicas sobre tuberculose multidroga-resistente (TB-MDR) e tuberculose extensivamente resistente em pacientes pediátricos?” A pesquisa usou os descritores: “tuberculose extensivamente resistente a medicamentos” OR “tuberculose resistente a múltiplos medicamentos” AND “pediatria”. A pesquisa foi realizada de modo duplo-cego nas bases de dados Medical Literature Analysis and Retrieval System Online, Regional Office for the Western Pacific’s Institutional Repository for Information Sharing, Embase/Elsevier e International Clinical Trials Registry Platform, com um corte temporal de 2011 a 2021, sendo a amostra final sintetizada de 18 artigos, nos quais avaliou-se o conteúdo metodológico por meio do nível de evidência. Resultados: os resultados mostraram a escassez de pesquisas de alto nível de evidência relacionadas à TB-MDR em crianças, ausência de posologia adequada das drogas de segunda linha para o público pediátrico e a importância do teste de sensibilidade a drogas para o tratamento dos casos. Conclusões: identificou-se que os obstáculos do tratamento TB-MDR se concentraram na ausência de protocolos detalhados, de dosagens medicamentosas seguras e com menor efeito colateral, e, principalmente, nos determinantes sociais do processo saúde e doença que envolvem a TB-MDR. Abstract Considering the scientific evidence regarding the harmful effects on health from consuming ultra-processed foods, the regulation on food environments has been prioritized as a way of promoting adequate and healthy eating. In this context, Bill 4198/2021 was proposed in the Legislative assembly of Rio de Janeiro, to ban the sale of ultra-processed foods in the schools in the state. In the second voting session of the proposition conservative rhetoric was widely used by the opposing congressmen. In this essay, these positions were classified, finding common points between them, considering the analysis of the legislative process. In making this categorization, it was possible to identify three argumentative bases that were explored: the defense of freedom of trade; the right to a “family autonomy,” the primacy of the family in children’s education; and individual responsibility for obesity. These rhetorics have been analyzed together with their counterpoint, in the conclusions of the specialized literature. Beyond the plurality of ideas in the legislative debate, it was possible to identify how the discussion of Bill 4198/21 was permeated by the reproduction of disinformation and violence. Resumo Diante das evidências científicas acerca dos malefícios à saúde do consumo de alimentos ultraprocessados, a regulação dos ambientes alimentares vem sendo priorizada como forma de promover uma alimentação adequada e saudável. Nesse contexto, foi proposto o Projeto de Lei 4198/2021 na Assembleia Legislativa do Rio de Janeiro, para proibir o comércio de ultraprocessados nas escolas do estado. Na segunda sessão em plenário da proposição, houve ampla utilização de retóricas conservadoras pelos deputados contrários. Neste presente ensaio foram classificados esses posicionamentos, encontrando-se pontos comuns entre eles, à luz da análise da tramitação legislativa. Ao realizar essa categorização, foi possível identificar três bases argumentativas exploradas: a defesa da liberdade de comércio; o direito à “pátria família”, de primazia da família na educação infantojuvenil; e a responsabilização individual da obesidade. Tais retóricas foram analisadas em conjunto com suas contraposições, presentes nas conclusões da literatura especializada. Para além da pluralidade de ideias do debate legislativo, foi possível identificar como a discussão do PL 4198/21 foi permeada pela reprodução de desinformação e violência. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT BACKGROUND: One anastomosis gastric bypass (OAGB) has gained prominence in the search for better results in bariatric surgery. However, its efficacy and safety compared to Roux-en-Y gastric bypass (RYGB) and sleeve gastrectomy (SG) remain ill-defined. AIMS: To compare the efficacy and safety of OAGB relative to RYGB and SG in the treatment of obesity. METHODS: We systematically searched PubMed, EMBASE, Cochrane Library, Lilacs, and Google Scholar databases for randomized controlled trials comparing OAGB with RYGB or SG in the surgical approach to obesity. We pooled outcomes for body mass index, percentage of excess weight loss, type-2 diabetes mellitus remission, complications, and gastroesophageal reflux disease. Statistical analyses were performed with R software (version 4.2.3). RESULTS: Data on 854 patients were extracted from 11 randomized controlled trials, of which 422 (49.4%) were submitted to OAGB with mean follow-up ranging from six months to five years. The meta-analysis revealed a significantly higher percentage of excess weight loss at 1-year follow-up and a significantly lower body mass index at 5-year follow-up in OAGB patients. Conversely, rates of type-2 diabetes mellitus remission, complications, and gastroesophageal reflux disease were not significantly different between groups. The overall quality of evidence was considered very low. CONCLUSIONS: Our results corroborate the comparable efficacy of OAGB in relation to RYGB and SG in the treatment of obesity, maintaining no significant differences in type-2 diabetes mellitus remission, complications, and gastroesophageal reflux disease rates. RESUMO RACIONAL: O bypass gástrico de uma anastomose (BGU tem ganhado destaque na busca por melhores resultados na cirurgia bariátrica. No entanto, a eficácia e segurança do BGUA, em relação ao bypass gástrico em Y de Roux (BGYR) e à gastrectomia vertical (GV), permanecem imprecisas. OBJETIVOS: Comparar a eficácia e segurança do BGUA em relação ao BGYR e GV no tratamento da obesidade. MÉTODOS: Realizamos uma busca sistemática nas bases de dados do PubMed, EMBASE, Cochrane, Lilacs e Google Scholar por ensaios clínicos randomizados que comparassem o BGUA ao BGYR ou GV na abordagem cirúrgica da obesidade. Abstract Mucopolysaccharidosis type IIIB (MPS IIIB) is caused by deficiency of alpha-N-acetylglucosaminidase, leading to storage of heparan sulphate. The disease is characterized by intellectual disability and hyperactivity, among other neurological and somatic features. Here we studied retrospective data from a total of 19 MPS IIIB patients from Brazil, aiming to evaluate disease progression. Mean age at diagnosis was 7.2 years. Speech delay was one of the first symptoms to be identified, around 2-3 years of age. Behavioral alterations include hyperactivity and aggressiveness, starting around age four. By the end of the first decade, patients lost acquired abilities such as speech and ability to walk. Furthermore, as disease progresses, respiratory, cardiovascular and joint abnormalities were found in more than 50% of the patients, along with organomegaly. Most common cause of death was respiratory problems. Abstract Introduction Since 2005, Brazilian cities have been adapting to organize teams and services of the Unified Social Assistance System (SUAS), and since 2011, occupational therapy has been recognized as one of the professions that compose these teams and the management of SUAS. This study addresses how the category has been involved in this policy in the state of Rio de Janeiro. Objective To map the occupational therapists working at SUAS in this state and outline the characteristics of this inclusion. Methodology A descriptive, cross-sectional mapping study, using official data from the SUAS Census. Data were analyzed from a descriptive perspective, in dialogue with the National Social Assistance/Welfare Policy, the literature in the field of occupational therapy in social assistance, and under the framework of social occupational therapy. Results In the State of Rio de Janeiro, 142 occupational therapists work at SUAS, which is equivalent to 8.9% of the professionals in this state. This workforce is composed of women (89.4%) aged 41-50 years (34.5%), hired by Civil Society Organizations (93%), under the Consolidation of Labor Laws (CLT) regime (50%), with a weekly workload of 11-20 hours (40.9%), mainly inserted in Day Centers (71%), working with people with disabilities and older people. Conclusion The inclusion of occupational therapists in SUAS of Rio de Janeiro state occurs precariously, with low insertion in this state’s devices. Resumo Introdução Desde 2005, as cidades brasileiras estão se adequando para organizar equipes e serviços do Sistema Único de Assistência Social (SUAS) e, a partir de 2011, a terapia ocupacional passou a ser reconhecida como uma das profissões que compõem tais equipes e a gestão do SUAS. Este estudo aborda como a categoria tem participado dessa política no estado do Rio de Janeiro. Objetivo Mapear as terapeutas ocupacionais que atuam no SUAS nesse estado e delinear as características dessa inserção. Metodologia Estudo de mapeamento, descritivo e transversal, utilizando informações oficiais do Censo SUAS. Os dados foram analisados a partir de uma perspectiva descritiva, em diálogo com a Política Nacional de Assistência Social, a literatura do campo da terapia ocupacional na assistência social e sob o referencial da terapia ocupacional social. Resultados No estado do Rio de Janeiro, 142 terapeutas ocupacionais atuam no SUAS, o equivalente a 8,9% das profissionais desse estado. Como retrato, obtivemos imagem formada por mulheres (89,4%) entre 41-50 anos de idade (34,5%), contratadas por Organizações da Sociedade Civil (93%), celetistas (50%), com carga horária semanal de 11-20 horas (40,9%), inseridas majoritariamente nos Centros-dia (71%), atuando com pessoas com deficiências e idosas. Conclusão A inserção de terapeutas ocupacionais no SUAS fluminense acontece de forma precarizada, com baixa inserção nos equipamentos estatais. ABSTRACT Objective Currently programmed cell death protein 1 (PD-1) inhibitors in combination with other therapies are being evaluated to determine their efficacy in cancer treatment. However, the effect of PD-ligand (L) 1 expression on disease outcomes in stage III (EC III) non-small cell lung cancer is not completely understood. Therefore, this study aimed to assess the influence of PD-L1 expression on the outcomes of EC III non-small cell lung cancer. Methods This study was conducted on patients diagnosed with EC III non-small cell lung cancer who underwent treatment at a tertiary care hospital. PD-L1 expression was determined using immunohistochemical staining, all patients expressed PD-L1. Survival was estimated using the Kaplan-Meier method. Relationships between variables were assessed using Cox proportional regression models. Results A total of 49 patients (median age=69 years) with EC III non-small cell lung cancer and PD-L1 expression were evaluated. More than half of the patients were men, and most were regular smokers. The patients were treated with neoadjuvant chemotherapy, surgery, or sequential or combined chemotherapy and radiotherapy. The median progression-free survival of the entire cohort was 14.2 months, and the median overall survival was 20 months. There was no significant association between PD-L1 expression and disease progression, clinical characteristics, or overall survival. Conclusions PD-L1 expression was not correlated with EC III non-small cell lung cancer outcomes. Whether these findings differ from the association with immune checkpoint inhibitors remains to be addressed in future studies. ABSTRACT Objective: To develop and validate the content of an auricular acupuncture point protocol for the treatment of headaches. Method: A methodological study, carried out in June 2023 in Montes Claros, Minas Gerais, Brazil. It was structured in three stages: Planning (bibliographic survey and identification of the study variables); Construction of the instruments (characterization of the experts, point protocol and treatment regime); and Content validation in charge of 15 experts. In the data analysis, an absolute agreement percentage equal to or greater than 80% was used. Método: Estudio metodológico, realizado en junio de 2023, en Montes Claros, Minas Gerais, Brasil. Abstract Background There is a remarkable variability in the frequency of HLA-B27 positivity in patients with spondyloarthritis (SpA), which may be associated with different clinical presentations worldwide. However, there is a lack of data considering ethnicity and sex on the evaluation of the main clinical and prognostic outcomes in mixed-race populations. The aim of this study was to evaluate the frequency of HLA-B27 and its correlation with disease parameters in a large population of patients from the Brazilian Registry of Spondyloarthritis (RBE). Methods The RBE is a multicenter, observational, prospective cohort that enrolled patients with SpA from 46 centers representing all five geographic regions of Brazil. The inclusion criteria were as follow: (1) diagnosis of axSpA by an expert rheumatologist; (2) age ≥18 years; (3) classification according to ASAS axial. The following data were collected via a standardized protocol: demographic data, disease parameters and treatment historical. Results A total of 1096 patients were included, with 73.4% HLA-B27 positivity and a mean age of 44.4 (±13.2) years. Positive HLA-B27 was significantly associated with male sex, earlier age at disease onset and diagnosis, uveitis, and family history of SpA. Conversely, negative HLA-B27 was associated with psoriasis, higher peripheral involvement and disease activity, worse quality of life and mobility. Conclusions Our data showed that HLA-B27 positivity was associated with a classic axSpA pattern quite similar to that of Caucasian axSpA patients around the world. Furthermore, its absence was associated with peripheral manifestations and worse outcomes, suggesting a relevant phenotypic difference in a highly miscegenated population. Abstract Objective To review current literature to support the use of mesna as a preventive therapy for hemorrhagic cystitis and bladder cancer in patients with systemic autoimmune diseases and systemic vasculitis treated with cyclophosphamide. Materials and methods The search for articles was conducted systematically through MEDLINE, LILACS, Cochrane Library, and Embase databases. Only articles in English were selected. For available records, titles and abstracts were selected independently by two investigators. Results Eighteen studies were selected for analysis. The known adverse effects of cyclophosphamide were hematological toxicity, infections, gonadal toxicity, teratogenicity, increased risk for malignancy and hemorrhagic cystitis. Long-term toxicity was highly dependent on cyclophosphamide cumulative dose. The risk of bladder cancer is especially higher in long-term exposure and with cumulative doses above 36 g. The risk remains high for years after drug discontinuation. Hemorrhagic cystitis is highly correlated with cumulative dose and its incidence ranges between 12 and 41%, but it seems to be lower with new regimens with reduced cyclophosphamide dose. No randomized controlled trials were found to analyze the use of mesna in systemic autoimmune rheumatic diseases and systemic vasculitis. Retrospective studies yielded conflicting results. Uncontrolled prospective studies with positive results were considered at high risk of bias. No evidence was found to support the use of mesna during the treatment with cyclophosphamide for autoimmune diseases or systemic vasculitis to prevent hemorrhagic cystitis and bladder cancer. In the scenarios of high cumulative cyclophosphamide dose (i.e., > 30 g), patients with restricted fluid intake, neurogenic bladder, therapy with oral anticoagulants, and chronic kidney disease, mesna could be considered. Conclusion The current evidence was found to be insufficient to support the routine use of mesna for the prophylaxis of hemorrhagic cystitis and bladder cancer in patients being treated for systemic autoimmune diseases and systemic vasculitis with cyclophosphamide. The use may be considered for selected cases.
