RESUMO Introdução: A biópsia renal (BR) percutânea é fundamental para diagnóstico e manejo de diversas patologias renais. Dados nacionais sobre BR nativa em pacientes pediátricos são escassos. Nosso objetivo foi revisar características demográficas, clínicas e padrões histopatológicos em crianças submetidas a BR percutânea nativa ao longo de 24 anos. Métodos: Estudo observacional retrospectivo de pacientes submetidos a BR percutâneas nativas em unidade de nefrologia pediátrica entre 1998 e 2021, comparando três períodos: período 1 (1998–2005), período 2 (2006–2013), período 3 (2014–2021). Resultados: Constatamos que foram realizadas 228 BR, 78 (34,2%) no período 1, 91 (39,9%) no período 2, 59 (25,9%) no período 3. A idade mediana na BR foi 11 (7–14) anos. As principais indicações para BR foram síndrome nefrótica (SN) (42,9%), hematúria e/ou proteinúria não nefrótica (35,5%), lesão renal aguda (13,2%). Glomerulopatias primárias foram mais frequentes (67,1%), principalmente doença de lesão mínima (DLM) (25,4%), nefropatia por IgA (12,7%), glomerulonefrite mesangioproliferativa (GN) (8,8%). Das glomerulopatias secundárias, nefrite lúpica (NL) foi a mais prevalente (11,8%). No grupo 1, hematúria e/ou a proteinúria não nefrótica foram os principais motivos para BR, ao contrário da SN nos grupos 2 e 3 (p < 0,01). A NL apresentou tendência crescente (período 1–3: 2,6%–5,3%) e a glomeruloesclerose segmentar focal (GESF) apresentou leve tendência decrescente (período 1–3: 3,1%–1,8%), sem significância estatística. Conclusões: A principal indicação para BR foi SN, que aumentou ao longo do tempo, justificando o achado de DLM como principal diagnóstico histológico. A NL apresentou aumento na incidência ao longo do tempo, enquanto os casos de GESF não aumentaram. Introduction: Percutaneous kidney biopsy (KB) is crucial to the diagnosis and management of several renal pathologies. National data on native KB in pediatric patients are scarce. We aimed to review the demographic and clinical characteristics and histopathological patterns in children who underwent native percutaneous KB over 24 years. Methods: Retrospective observational study of patients undergoing native percutaneous KB in a pediatric nephrology unit between 1998 and 2021, comparing 3 periods: period 1 (1998–2005), period 2 (2006–2013), and period 3 (2014–2021). Results: We found that 228 KB were performed, 78 (34.2%) in period 1, 91 (39.9%) in period 2, and 59 (25.9%) in period 3. The median age at KB was 11 (7–14) years. The main indications for KB were nephrotic syndrome (NS) (42.9%), hematuria and/or non-nephrotic proteinuria (35.5%), and acute kidney injury (13.2%). Primary glomerulopathies were more frequent (67.1%), particularly minimal change disease (MCD) (25.4%), IgA nephropathy (12.7%), and mesangioproliferative glomerulonephritis (GN) (8.8%). Of the secondary glomerulopathies, lupus nephritis (LN) was the most prevalent (11.8%). In group 1, hematuria and/or non-nephrotic proteinuria were the main reasons for KB, as opposed to NS in groups 2 and 3 (p < 0.01). LN showed an increasing trend (period 1–3: 2.6%–5.3%) and focal segmental glomerular sclerosis (FSGS) showed a slight decreasing trend (period 1–3: 3.1%–1.8%), without statistical significance. Conclusions: The main indication for KB was NS, which increased over time, justifying the finding of MCD as main histological diagnosis. LN showed an increase in incidence over time, while FSGS cases did not increase. Abstract The healthcare context is undergoing a significant transformation, facing a series of complex and unprecedented challenges. This transformation is triggered by accessibility constraints to healthcare services, increased expectations from patients and sing financial burdens. Rather than being seen as unsolvable obstacles, these challenges can be perceived as powerful stimuli, encouraging the search for innovative and creative solutions, especially in a context where new digital technologies applicable to healthcare are emerging. In the context of this article, which shares a transformative vision of healthcare provision, we describe a pilot program developed for the Heart Failure ambulatory of the Internal Medicine Department at the Braga Local Health Unit (ULS-Braga). This proposal encompasses not only treatment, but also proactive health promotion, based on an approach that incorporates cutting-edge digital technology. The overall aim is to significantly expand the scope of outpatient services and provide innovative home care, bringing it closer to the specific needs of (healthy and sick) individuals.

Resumo O contexto da prestação de cuidados de saúde está a sofrer uma transformação significativa, enfrentando uma série de desafios complexos e sem precedentes. Esta transformação relaciona-se com preocupações prementes relacionadas com a acessibilidade aos serviços de saúde, a adaptação às expectativas dos doentes em constante evolução e aos crescentes encargos financeiros. Em vez de serem vistos como obstáculos intransponíveis, esses desafios podem ser percebidos como estímulos poderosos, estimulando a procura de soluções inovadoras e criativas, sobretudo num contexto onde existem novas tecnologias digitais com aplicabilidade à prestação de cuidados de saúde. No contexto deste artigo, que partilha uma visão transformadora da prestação de cuidados de saúde, descreve-se a proposta piloto desenvolvida para a consulta de Insufi-ciência Cardíaca do serviço de Medicina Interna, na Unidade Local de Saúde de Braga (ULS-Braga). Esta proposta engloba não só o tratamento, mas também a promoção proativa da saúde, assente numa abordagem que incorpora tecnologia digital de ponta. O objetivo global é expandir significativamente o alcance dos serviços de ambulatório e prestar cuidados domiciliares inovadores, aproximando-os mais das necessidades específicas das pessoas (saudáveis e com doença).

Abstract The objective is to understand the perspective of entrepreneurs from Popular Health Clinics (PHC) and representatives of the medical profession on services offered by the sector; impacts resulting from the COVID-19 pandemic; and future of the medical job market. This is a qualitative research, in the area of collective health, focused on social representations. Semi-structured interviews were carried out, from March to July 2021, with four local entrepreneurs and three representatives of the medical profession from a municipality in the Northeast region of Brazil. PHC offer assistance services restricted to consultations and exams and with financialized logic strategies. The PHC are presented as an “alternative” to the SUS, a supposed “gap” between private health plans and public services, and as a “new” medical work. Companies offer consultations with specialists at “popular” prices and without a waiting list. The assistance provided is restricted and professionals have no guarantee of labor rights. For those interviewed, access to healthcare represents geographic and temporal accessibility of services at a reduced price. The universal right to health and SUS principles are confronted with the defense of the autonomy of clients and professionals targeting their needs: health and work.

Resumo Este artigo pretende compreender a perspectiva de empresários de Clínicas Populares de Saúde (CPS) e representantes da classe médica sobre serviços ofertados pelo setor; impactos decorrentes da pandemia covid-19; e futuro do mercado de trabalho médico. Trata-se de pesquisa qualitativa, da área de saúde coletiva, com enfoque nas representações sociais. Foram realizadas entrevistas semiestruturadas, entre março e julho de 2021, com quatro empresários locais e três representantes de classe médica de uma cidade da região Nordeste do Brasil. As CPS ofertam serviços assistenciais restritos a consultas e exames e com estratégias de lógica financeirizada; se apresentam como “alternativa” ao SUS, uma suposta “lacuna” entre planos de saúde privados e serviços públicos, e como “novo” trabalho médico. As empresas ofertam consultas com especialistas a preços “populares” e sem fila de espera. A assistência prestada é restrita e os profissionais não têm garantia de direitos trabalhistas. Para os entrevistados, o acesso à saúde representa acessibilidade geográfica e temporal de serviço a preço reduzido. O direito universal à saúde e princípios do SUS são confrontados com a defesa da autonomia dos clientes e dos profissionais visando suas necessidades: saúde e trabalho.

Resumo Devido às lacunas de conhecimento sobre os representantes de Ipomoea no estado do Ceará, apresentamos aqui a sinopse desse gênero como parte do projeto “Flora do Ceará: conhecendo para preservar”. Este estudo foi baseado na análise de exsicatas e imagens de coleções-tipos, consulta à bibliografias especializadas e coletas em campo. Nós registramos 42 espécies, destas cinco são endêmicas do domínio Caatinga. Apresentamos chave de identificação, caracterização e relação morfológica entre espécies afins, dados sobre distribuição geográfica, período de floração e frutificação, ilustração dos caracteres diagnósticos, fotografias e basiônimos dos táxons estudados. Abstract Due to the knowledge gaps about the representatives of Ipomoea in the state of Ceará, we present here the synopsis of this genus as part of the “Flora of Ceará: knowing to conserve” project. This study was based on the analysis of exsiccates and images of type collections, consultation of specialized bibliographies and field expeditions. We recorded 42 species, among these five are endemic to Caatinga domain. We present an identification key, characterization and morphological relationship between related species, geographic distribution data, flowering and fruiting period, illustration of diagnostic characters, photographs and basionyms of the studies taxa. Abstract Background: Mobile applications can be useful in diabetes self-management. Knowing the needs and preferences of users is essential for these tools to provide effective support in this process. Objective: To learn about the experiences, identify needs, and explore the preferences of people with type 2 diabetes mellitus regarding mobile applications to support diabetes self-management. Methodology: Qualitative descriptive study with 12 participants selected using the snowball sampling technique. Data was collected through semi-structured interviews and analyzed using the content analysis technique. Results: Six categories emerged from the analysis: functions and technical characteristics, informative content, self-determination, emotional management, health professional, and facilitating factors. The results suggest poor digital skills, need for access to information, and preference for multifunctional and interactive applications. Conclusion: Despite the limited use of health-related apps, people are interested and realize the benefit of using these tools to manage their diabetes.

Resumen Marco contextual: Las aplicaciones móviles pueden ser útiles para el autocontrol de la diabetes. Conocer las necesidades y preferencias de los usuarios es esencial para que estas herramientas sean un apoyo eficaz en este proceso. Objetivos: Conocer las experiencias, identificar las necesidades y explotar las preferencias de personas con diabetes mellitus tipo 2 relacionadas con las aplicaciones móviles de apoyo a la autogestión de la enfermedad y al régimen terapéutico. Metodología: Estudio descriptivo, de naturaleza cualitativa, con 12 participantes seleccionados por la técnica de muestra de bola de nieve. Datos recogidos por entrevistas semiestructuradas y analizados por la técnica de análisis de contenido. Resultados: Surgieron seis categorías (funciones y características técnicas, contenidos de información, autodeterminación, gestión de emociones, profesional sanitario y factores facilitadores). Los datos sugieren bajas competencias digitales, necesidad de acceso a la información y preferencia por aplicaciones multifuncionales e interactivas. Conclusión: A pesar de la escasa utilización de aplicaciones relacionadas con la salud, las personas muestran interés y perciben el beneficio de utilizar estas herramientas para gestionar su diabetes.

Resumo Enquadramento: As aplicações móveis podem ser úteis para a autogestão da diabetes. Conhecer as necessidades e preferências dos utilizadores é fundamental para que estas ferramentas sejam um suporte efetivo neste processo. Objetivos: Conhecer as experiências, identificar as necessidades e explorar as preferências de pessoas com diabetes mellitus tipo 2 relativamente a aplicações móveis de apoio à autogestão da doença e regime terapêutico. Metodologia: Estudo descritivo, de natureza qualitativa, com 12 participantes recrutados por técnica amostral de bola de neve. Dados recolhidos por entrevistas semiestruturadas e analisados pela técnica de análise de conteúdo. Resultados: Emergiram seis categorias (funções e características técnicas, conteúdos de informação, autodeterminação, gestão de emoções, profissional de saúde e fatores facilitadores). Os dados sugerem baixas competências digitais, necessidade de acesso à informação e a preferência por aplicações multifuncionais e interativas. Conclusão: Apesar da utilização parca de aplicações relacionadas com a saúde, as pessoas assumem interesse e percecionam o benefício de utilizar estas ferramentas para gerir a sua diabetes.

Abstract Maxillofacial trauma can occur with high frequency while playing sports. Skeletal and soft tissue lesions can occur, with different degrees of severity and impact for the athlete. Sports-related facial fractures most frequently affect the nose, zygoma and mandible, regardless of the country and sport. During initial evaluation it is important to distinguish between stable injuries that can be treated on the sidelines or after the match and emergencies requiring prompt hospital transfer. Athletes could benefit from a more personalized approach to treatment, aimed at optimizing the timeframe for return to play. There is a lack of evidence-based studies on return to play after facial trauma, and currently no specific guidelines exist. Return to sport depends on a variety of factors, such as type of fracture, the risk of impact within the sport, the player's motivation for an early return and the surgeon's knowledge and experience.

Resumo Trauma maxilofacial ocorre frequentemente durante a prática de desportos. Lesões esqueléticas e de tecidos moles podem ocorrer com diferentes graus de gravidade e impacto para o atleta. As fraturas faciais relacionadas com o desporto afetam mais frequentemente o nariz, o zigoma e a mandíbula, independentemente do país e do desporto. Durante a avaliação inicial é importante distinguir entre lesões estáveis que podem ser tratadas fora do campo ou após o jogo, e emergências que requerem transferência hospitalar imediata. Os atletas podem beneficiar de uma abordagem mais personalizada no tratamento, de forma a otimizar o regresso à competição. Atualmente, e devido à falta de estudos baseados na evidência, não existem diretrizes específicas sobre o regresso à competição após traumatismos da face. O regresso à competição depende de diversos fatores, como o tipo de fratura, o risco de impacto dentro do desporto, a motivação do jogador para um regresso precoce e o conhecimento e experiência do cirurgião.

Abstract This study aimed to evaluate the contribution of oral and maxillofacial pathology laboratories (OMPLs) in Brazilian public universities to the diagnosis of lip, oral cavity, and oropharyngeal squamous cell carcinoma (SCC). A cross-sectional study was performed using biopsy records from a consortium of sixteen public OMPLs from all regions of Brazil (North, Northeast, Central-West, Southeast, and South). Clinical and demographic data of patients diagnosed with lip, oral cavity, and oropharyngeal SCC between 2010 and 2019 were collected from the patients’ histopathological records. Of the 120,010 oral and maxillofacial biopsies (2010-2019), 6.9% (8,321 cases) were diagnosed as lip (0.8%, 951 cases), oral cavity (4.9%, 5,971 cases), and oropharyngeal (1.2%, 1,399 cases) SCCs. Most cases were from Brazil’s Southeast (64.5%), where six of the OMPLs analyzed are located. The predominant profile of patients with lip and oral cavity SCC was Caucasian men, with a mean age over 60 years, low schooling level, and a previous history of heavy tobacco consumption. In the oropharyngeal group, the majority were non-Caucasian men, with a mean age under 60 years, had a low education level, and were former/current tobacco and alcohol users. According to data from the Brazilian National Cancer Institute, approximately 9.9% of the total lip, oral cavity, and oropharyngeal SCCs reported over the last decade in Brazil may have been diagnosed at the OMPLs included in the current study. Therefore, this data confirms the contribution of public OMPLs with respect to the important diagnostic support they provide to the oral healthcare services extended by the Brazilian Public Health System. Resumo O objetivo deste artigo é discutir os resultados de uma pesquisa de pós-doutoramento, que teve como propósito realizar um aprofundamento teórico acerca do conceito de estágio compartilhado e mobilizar esse estudo para a formação inicial e continuada de professores de matemática. Esses resultados estão relacionados, particularmente, às possibilidades e aos desafios que se manifestaram durante o desenvolvimento de quatro disciplinas de estágio do curso de licenciatura de matemática, no contexto remoto. Para tanto, trazem-se reflexões teóricas sobre obras literárias que dialogam com a temática do artigo. Em seguida, é feita uma contextualização acerca dessas quatro disciplinas, ministradas remotamente no segundo semestre de 2020, na Universidade Federal de São Carlos (UFSCar), campus de São Carlos (SP). A metodologia de pesquisa é qualitativa, e os dados são provenientes de tarefas teóricas realizadas pelos licenciandos e de gravações das aulas, que ocorreram via Google Meet. Abstract This article discusses the results of a post-doctoral research that conducted an in-depth theoretical exploration on the concept of shared supervised practice and mobilized this study for the initial and continuing education of mathematics teachers. These results stem from the possibilities and challenges that emerged during four internship disciplines of the Mathematics degree course, in the remote context. Thus, theoretical reflections on literary works that dialogue with the theme are presented. Then, we contextualize these four disciplines, taught remotely in the second half of 2020, at the Federal University of São Carlos (UFSCar), São Carlos (SP) campus. Abstract We evaluated the genetic diversity, coancestry and heritability of an E. benthamii trial. The 115 individuals were genotyped (13 SSR) and had their height and diameter at breast height (dbh) measured. Heritability was estimated using the RR-BLUP and the pairwise kinship coefficient method. An average of nine alleles per locus was observed. The expected heterozygosity (0.655) was similar to the observed heterozygosity, with the estimated inbreeding (0.02) being low. The group coancestry (0.051) demonstrates that the trees are related to some degree. The trees were clustered in five groups using the Evanno’s method. The average kinship within each group ranges from 0.042 to 0.082. The heritability estimated by RR-BLUP was low. The heritability estimated using the kinship coefficients is moderate, reaching estimated genetic gains of 14% for dbh. Abstract The aim of this study is to adapt a Portuguese version of the original 18 items of the Bergen Facebook Addiction Scale (BFAS), via a translation / back translation process, using Confirmatory Factor Analysis (CFA) in a Portuguese sample. The sample comprised 232 respondents from the general population. The modified BFAS acquires a different factor structure from the original, keeping 4 of the main theoretical elements (subscales) and 10 of the 18 original items. The results indicate that the Portuguese version of the original BFAS presents good psychometric qualities. The statistical techniques used in the study allowed assessing the reliability and validity of the modified BFAS. Nevertheless, further uses of this scale with other samples from the Portuguese population are necessary to confirm the obtained results.

ABSTRACT Objective: To evaluate the influence of maternal pre-pregnancy body mass index (BMI) and gestational weight gain (GWG) on blood glucose levels at diagnosis of gestational diabetes mellitus (GDM) and obstetric/neonatal outcomes. Subjects and methods: Retrospective cohort study including 462 women with GDM and singleton pregnancy delivered in our institution between January 2015 and June 2018 and grouped according to BMI/GWG. Results: The diagnosis of GDM was more likely to be established in the 1st trimester (T) in women with obesity than in normal-weight (55.8% vs 53.7%, p = 0.008). BMI positively and significantly correlated with fasting plasma glucose (FPG) levels in the 1stT (rs = 0.213, p = 0.001) and 2ndT (rs = 0.210, p = 0.001). Excessive GWG occurred in 44.9% women with overweight and in 40.2% with obesity (p < 0.001). From women with obesity, 65.1% required pharmacological treatment (p < 0.001). Gestational hypertension (GH) was more frequent in women with obesity (p = 0.016). During follow-up, 132 cesareans were performed, the majority in mothers with obesity (p = 0.008). Of the 17 large-for-gestational-age (LGA) birthweight delivered, respectively 6 and 9 were offsprings of women with overweight and obesity (p = 0.019). Maternal BMI had a predictive value only for macrosomia [aOR 1.177 (1.006-1.376), p = 0.041]. BMI and GWG positively correlated with birthweight (rs = 0.132, p = 0.005; rs = 0.188, p = 0.005). Conclusion: Maternal obesity is related with a major probability of diagnosis of GDM in 1stT, fasting hyperglycemia in 2ndT and a more frequent need for pharmacological therapy. Pre-gestational obesity is associated with GH, cesarean delivery and fetal macrosomia.

Abstract In recent years, there is a growing consensus on the importance of Corporate Social Responsibility for organisations with the aim of achieving sustainable development. This study aims to analyse the dissemination of information on Corporate Social Responsibility through the websites of Portuguese Liga NOS clubs in the 2017/2018 sport season. The study is descriptive in nature and is based on a qualitative methodology using the content analysis technique. The results show that the degree of information disclosed is low, as revealed by the total disclosure index (26.63%), and that the most disclosed information is social, followed by economic information. However, very little information is disclosed on employee recruitment and environment.

Resumen En los últimos años, está siendo consensuada cada vez más la importancia en lo que respecta a la Responsabilidad Social Corporativa para las organizaciones con el objetivo de alcanzar un desarrollo sostenible. El presente estudio tiene como objetivo analizar la divulgación de información sobre Responsabilidad Social Corporativa a través de las páginas web de los clubes portugueses de la Liga NOS en la temporada deportiva 2017/2018. El estudio es de carácter descriptivo y se basa en una metodología de tipo cualitativo utilizando la técnica de análisis de contenido. Los resultados muestran que el grado de información divulgado es bajo, como lo revela el índice de divulgación total (26,63%), y que la información más divulgada es la social, seguida de la información económica. Sin embargo, la información propagada sobre la contratación de los colaboradores y la medioambiental es muy escasa.

Pouco se sabe sobre a evolução do perímetro cefálico (PC) em crianças com síndrome congênita associada à infecção pelo vírus Zika (SCZ) em acompanhamentos contínuos. Este estudo buscou avaliar o crescimento do PC em crianças com SCZ nos primeiros três anos de suas vidas e identificar os fatores associados a ele. Os dados do PC ao nascimento e obtidos em consultas neuropediátricas de 74 crianças com SCZ foram coletados no Cartão da Criança, nos laudos paternos e em seus prontuários. Os preditores de escore-z para PC foram investigados utilizando-se diferentes modelos de efeitos mistos. O critério de informação de Akaike foi utilizado para selecionar os modelos usados. O escore-z de PC diminuiu de -2,7 ± 1,6 ao nascimento para -5,5 ± 2,2 aos 3 meses de idade, mas permaneceu relativamente estável desde então. No modelo ajustado selecionado, a presença de atrofia parênquimal cerebral grave e sintomas maternos de infecção no primeiro trimestre de sua gravidez estiveram associados a uma redução mais acentuada no escore-z de PC nos primeiros três anos de vida dos participantes. A diminuição do escore-z de PC em crianças com SCZ nos primeiros 3 meses de sua vida monstra o potencial reduzido de crescimento e desenvolvimento do sistema nervoso central dessas crianças. O prognóstico de crescimento do perímetro cefálico nos primeiros 3 anos de vida é pior quando a infecção materna ocorreu no primeiro trimestre gestacional e em crianças que tiveram atrofia parênquimal grave.

Little is known about the evolution of head circumference (HC) in children with congenital Zika syndrome (CZS). This study aims to evaluate HC growth in children with CZS in the first three years of life and identify associated factors. HC data obtained at birth and in neuropediatric consultations from 74 children with CZS were collected from the Child’s Health Handbook, parents’ reports, and medical records. Predictors of HC z-score were investigated using different mixed-effects models; Akaike’s information criterion was used for model selection. The HC z-score decreased from -2.7 ± 1.6 at birth to -5.5 ± 2.2 at 3 months of age, remaining relatively stable thereafter. In the selected adjusted model, the presence of severe brain parenchymal atrophy and maternal symptoms of infection in the first trimester of pregnancy were associated with a more pronounced reduction in the HC z-score in the first three years of life. The decrease of HC z-score in CZS children over the first three months demonstrated a reduced potential for growth and development of the central nervous system of these children. The prognosis of head growth in the first 3 years of life is worse when maternal infection occurs in the first gestational trimester and in children who have severe brain parenchymal atrophy.

Se conoce poco sobre la evolución del perímetro cefálico (PC) en niños con síndrome de Zika congénito (SZC) en los seguimientos continuos. El objetivo del estudio fue evaluar el crecimiento del PC en niños con SZC en los primeros 3 años de vida e identificar los factores asociados. Se recogieron datos del PC al nacimiento y obtenidos en las consultas de neuropediatría de 74 niños con SZC a partir de la Tarjeta del Niño, los informes de los padres y los registros médicos. Se investigaron los predictores de la puntuación Z del PC mediante diferentes modelos de efectos mixtos; se utilizó el criterio de información de Akaike para la selección del modelo. La puntuación Z del PC disminuyó de -2,7 ± 1,6 al nacer a -5,5 ± 2,2 a los 3 meses de edad, pero a partir de entonces se mantuvo relativamente estable. En el modelo ajustado seleccionado, la presencia de atrofia grave del parénquima cerebral y los síntomas maternos de infección en el primer trimestre del embarazo se asociaron con una reducción más pronunciada de la puntuación Z del PC en los primeros 3 años de vida. La disminución de la puntuación Z del PC en los niños con SZC durante los primeros 3 meses demuestra el menor potencial de crecimiento y desarrollo del sistema nervioso central de estos niños. El pronóstico del crecimiento de la cabeza en los primeros 3 años de vida es peor cuando la infección materna se produjo en el primer trimestre gestacional y en los niños que tenían una atrofia grave del parénquima cerebral.
