ABSTRACT Background: In the diagnosis and follow-up of arterial hypertension (HTN) most practice guidelines recommend ambulatory blood pressure monitoring (ABPM). In this regard, there is increasing evidence supporting the superiority of nocturnal hy pertension (NHTN) as a predictor of cardiovascular events. Little is known about the relationship with cardiovascular events according to the severity of NHTN. Furthermore, it is unclear from what nighttime pressure value the risk begins to increase. Objectives: The aim of this study was to determine whether the presence of NHTN and its severity levels are associated with adverse cardiovascular outcomes during follow-up. Methods: An observational study was performed analyzing data collected from 24-hour ABPM studies obtained in a high complexity medical center in Buenos Aires. We examined patients’ clinical characteristics, laboratory findings, imaging studies and their results during the follow-up period. Our study included ≥18 year-old persons who had been diagnosed with hypertension. We defined NHTN as those cases with blood pressure values ≥120/70 mmHg during the nighttime period. Results: The final analysis included 981 patients. Among these, 53% were men, mean age was 59.4 years and NHTN was present in 63.6% of cases (n=624). Nocturnal HTN was classified into four severity strata for comparison, according to the nighttime systolic blood pressure value: 83-119 mmHg, 120-139 mmHg, 140-159 mmHg and 160-220 mmHg. Major adverse cardiovascular events were recorded in 8 (2.2%), 17 (4.1 %), 8 (5.6%) and 7 (11.3%) subjects, respectively, and this differ ence between groups was statistically significant (p=0.007). Multivariate Cox regression analysis showed that the presence of NHTN was an independent predictor of adverse cardiovascular events (HR 3.60; 95% CI 1.12-11.5; p=0.033), even when considering the presence of daytime hypertension. Conclusion: In this contemporary cohort, NHTN and its severity were independently associated with the incidence of adverse cardiovascular events.

RESUMEN Introducción: En el diagnóstico y seguimiento de la hipertensión arterial (HTA) la mayoría de las guías de práctica recomiendan el monitoreo ambulatorio de la presión arterial (MAPA). En este sentido, existe cada vez más evidencia que respalda la superioridad de la hipertensión arterial nocturna (HTAN) como predictor de eventos cardiovasculares. Se sabe poco sobre la relación con los eventos cardiovasculares según la gravedad de la HTAN. Además, no está claro a partir de qué valor de presión arterial nocturna comienza a aumentar el riesgo. Objetivos: Conocer si la presencia de HTAN y sus niveles de gravedad se asocian con resultados cardiovasculares adversos durante el seguimiento. Material y métodos: Estudio observacional. Realizamos un análisis de los datos obtenidos en un centro médico de alta complejidad de Buenos Aires, recopilados a partir de estudios de MAPA de 24 horas. Examinamos las características clínicas de los pacientes, los resultados de laboratorio, los estudios de imagen y sus resultados durante el período de seguimiento. Nuestro estudio incluyó personas de 18 años o más a las que se les había diagnosticado hipertensión. Definimos HTAN como aquellos casos con valores de presión arterial ≥ 120/70 mmHg durante el periodo nocturno. Resultados: Fueron incluidos 981 pacientes en el análisis final. De ellos, el 53 % eran hombres; la edad media era de 59,4 años. Presentaban HTAN 63,6 % (n=624). Clasificamos la HTAN en cuatro estratos de gravedad para comparar, según el valor de presión arterial sistólica nocturna: 83-119 mmHg, 120-139 mmHg, 140-159 mmHg y 160-220 mmHg. Se registraron eventos adversos cardiovasculares mayores en 8 (2,2 %), 17 (4,1 %), 8 (5,6 %) y 7 (11,3 %) sujetos, respectivamente, y esta diferencia entre grupos fue estadísticamente significativa (p=0,007). El análisis multivariado de regresión de Cox demostró que la presencia de HTAN fue un predictor independiente de eventos cardiovasculares adversos (HR 3,60; IC 95% 1,12-11,5; p=0,033), incluso al considerar la presencia de hipertensión arterial diurna. Conclusión: En esta cohorte contemporánea, la HTAN y su gravedad se asociaron independientemente con la incidencia de eventos cardiovasculares adversos.

Abstract Unlike medications that contain fixed-dose combinations, such as those recommended by clinical guidelines for treating high blood pressure, the so-called polypills contain several drugs that simultaneously treat two or more cardiovascular conditions or risk factors. They were proposed 2 decades ago, both for primary and secondary prevention with the hypothesis that they could have wide dissemination and population penetration, improving the use of therapeutics with proven benefits individually, thanks to an increase in patient adherence by reducing the number of daily tablets and also by having an equal or lower cost. In this simple review, we present a look at risk stratification different from that posed by clinical scores and summarize the benefits of polypills in the treatment of risk factors and in the reduction of major cardiovascular events. Additionally, we review the clinical messages of the HOPE-3 trial, which aim to control two of the most prevalent conditions, such as high blood pressure and high cholesterol, through a combination of candesartan, hydrochlorothiazide and rosuvastatin. Finally, we propose its potential indication in a heterogeneous health system such as that of our country, both at the population level based on intermediate or low risk, determined intuitively or using a risk calculator, as well as in the personalized care that is practiced in many health scenarios.

Resumen A diferencia de los medicamentos que contienen combinaciones de dosis fijas, como los recomendados por las guías clínicas para tratar la hipertensión arterial, las llamadas polipíldoras contienen varios fármacos que tratan simultáneamente dos o más afecciones cardiovasculares o factores de riesgo. Se propusieron hace 2 décadas, tanto para prevención primaria como secundaria, con la hipótesis de que tengan amplia difusión y penetración poblacional, es decir, que mejoren el uso de terapéuticas con probados beneficios en forma individual, gracias a un incremento en la adherencia de los pacientes al reducir el número de com primidos diarios y también al tener un costo igual o menor. En esta revisión simple, planteamos una mirada de la estratificación del riesgo distinta a la planteada por los puntajes clínicos y resumimos los beneficios de las polipíldoras en el tratamiento de los factores de riesgo y en la reducción de eventos cardiovasculares mayores. Adicionalmente, repasamos los mensajes clínicos del ensayo HOPE-3, que apuntan a controlar dos de las con diciones más prevalentes, como son la hipertensión arterial y el colesterol elevado, mediante una combinación de candesartán, hidroclorotiazida y rosuvastatina. Finalmente, proponemos su potencial indicación en un sistema sanitario heterogéneo como el de nuestro país, tanto a nivel poblacional basado en el riesgo intermedio o bajo, determinado intuitivamente o usando un calculador de riesgo, así como también en la atención personalizada que se practica en muchos escenarios sanitarios.

ABSTRACT Background: Nocturnal Hypertension (NHT) has an impact on cardiovascular morbidity and mortality. Our aim was to evaluate the association between NHT and cardiovascular events. Methods: We carried out a retrospective cohort study including adults which had an ambulatory blood pressure monitoring test between March 2017 and July 2020 at a hospital in Buenos Aires City Argentina, with up to 36 months follow-up. Univariate and multivariate analyzes were performed to identify associated factors. Results: Four hundred and ninety four patients were included in the final analysis, 48% male, average age 58.9 years. Sixty two percent (n=308) presented NHT. A higher incidence of the cardiovascular composite endpoint was found in this group compared to those who did not present it, 10.4% vs 3.8% (p=0.013). In the final multivariate Cox regression analysis NHT was an independent predictor of cardiovascular events (HR=3.9; 95% CI 1.56-9.81; p=0.001), even independently of daytime HTN and the different circadian patterns of pressure. Conclusions: In this contemporary cohort from Buenos Aires, NHT was related to cardiovascular events incidence. Different measures should be taken to increase NHT awareness and diagnosis.

RESUMEN Introducción: Nuestro objetivo fue determinar si la hipertensión arterial nocturna (HTAN) se asocia a eventos cardiovasculares o mortalidad en el seguimiento. Material y métodos: Estudio de cohorte retrospectiva que incluyó pacientes adultos que se realizaron una presurometría ambulatoria en un hospital de la ciudad de Buenos Aires, incorporados a un registro propio entre marzo de 2017 y julio de 2020, con seguimiento de hasta 36 meses de la presurometría índice. Se realizaron análisis uni y multivariados para identificar factores asociados. Resultados: De 522 sujetos, se incluyeron 494 en el análisis final. El 48% eran de sexo masculino y la edad promedio fue de 58,9 años. El 62% (n = 308) presentó HTAN. Se halló una mayor incidencia del punto final compuesto cardiovascular en este grupo en comparación con quienes no la presentaban, 10,4% vs 3,8% (p = 0,013). El análisis final por regresión de Cox multivariada demostró que la HTAN fue un predictor independiente de eventos cardiovasculares (HR = 3,9; IC 95% 1,56-9,81; p = 0,001), incluso independientemente de la HTA diurna y de los diferentes patrones circadianos de presión. Conclusión: En una cohorte contemporánea y de la ciudad de Buenos Aires, el patrón de HTAN se asoció con la aparición de desenlaces cardiovasculares. En base a estos hallazgos, se deben implementar medidas para incrementar su diagnóstico en nuestro medio.

RESUMO O diagnóstico, tratamento e controle adequados da hipertensão arterial sistêmica (HAS) continua sendo uma questão pendente em nossa tentativa de reduzir a incidência de mortalidade cardiovascular prematura e a incapacidade. Essa é a realidade no nível mundial e, particularmente, na região das Américas. Apesar de algoritmos de diagnóstico adequados e esquemas de tratamento eficientes, a lacuna entre o conhecimento médico e a realidade nos mostra como é difícil que nossas recomendações sejam adotadas por nossos pacientes. A adesão representa um grande desafio, e uma compreensão adequada de suas dimensões é essencial ao se projetar estratégias para melhorar o controle da HAS. Claramente, a disponibilidade e a divulgação de informações adequadas é parte da solução, mas não menos importantes são as ações para garantir sua implementação. Portanto, acolhemos de bom grado as diretrizes de prática clínica que visam abordar esta questão. A recente publicação das diretrizes da Organização Mundial da Saúde sobre hipertensão arterial é um claro exemplo de tais iniciativas. Os programas de implementação são indiscutivelmente um dos maiores desafios da medicina contemporânea. O desenvolvimento de intervenções multimodais na atenção primária com o objetivo de melhorar sua resolutividade e promover o desenvolvimento de redes integradas de serviços de saúde parece ser a forma mais apropriada para atingir níveis adequados de controle da HAS. Estamos convencidos do importante papel que nossas sociedades científicas desempenham nestas iniciativas ao prestar apoio, suporte e articulação aos diferentes atores envolvidos.

ABSTRACT Adequate diagnosis, treatment, and control of arterial hypertension (AHT) continues to be a pending issue in the attempt to reduce the incidence of premature mortality and disability due to cardiovascular disease, which is a reality worldwide and in the region of the Americas in particular. Despite having adequate diagnostic algorithms and efficient treatment schemes, the gap between medical knowledge and reality reveals the great difficulty in ensuring that patients follow recommendations. Adherence to treatment is a great challenge and a proper understanding of its dimensions is essential when designing strategies to improve control of AHT. It is clear that part of the solution is having adequate information and disseminating it, but actions that guarantee implementation are no less important. That is why clinical practice guidelines that try to address this problem are welcome. The recent publication of the World Health Organization's guidance on arterial hypertension is a clear example of this type of initiative. Implementation programs are arguably one of the biggest challenges in contemporary medicine. The development of multimodal interventions aimed at improving response capacity at the first level of care and promoting the development of integrated health services networks appears to be the most appropriate way to achieve adequate control of AHT. Scientific societies clearly play an important role in these initiatives by providing support and coordinating interactions among the different actors involved.

RESUMEN El adecuado diagnóstico, tratamiento y control de la hipertensión arterial (HTA) continúa siendo una asignatura pendiente en nuestro intento de reducir la incidencia de mortalidad cardiovascular prematura y discapacidades, siendo esta una realidad a nivel mundial y particularmente cierta en la región de las Américas. A pesar de contar con adecuados algoritmos diagnósticos y eficientes esquemas de tratamiento, la brecha entre el conocimiento médico y la realidad nos muestra la gran dificultad para que nuestras recomendaciones sean adoptadas por nuestros pacientes. La adherencia representa un gran desafío y la adecuada comprensión de sus dimensiones resulta básico al momento de diseñar estrategias para mejorar el control de la HTA. Es claro que disponer y diseminar información adecuada es una parte de la solución, pero no menos importante son las acciones que garanticen su implementación. Es por lo que damos la bienvenida a guías de práctica clínica que intenten abordar esta problemática. La reciente publicación de guía de la Organización Mundial de la Salud en hipertensión arterial es un claro ejemplo de este tipo de iniciativas. Los programas de implementación posiblemente sean uno de los mayores desafíos de la medicina contemporánea. El desarrollo de intervenciones multimodales en el primer nivel de atención con el objetivo de mejorar su capacidad resolutiva y promover el desarrollo de redes integradas de servicios de salud, impresiona ser el camino mas adecuado para alcanzar niveles adecuados de control de la HTA. Estamos convencidos del importante rol que nuestras sociedades científicas tienen con estas iniciativas dando apoyo, soporte y articulando la interacción con los diferentes actores involucrados.

Abstract Neglected tropical diseases (NTD) are a group of diseases with high prevalence in tropical and subtropi cal countries. They are associated to poverty and underdevelopment. Due to its high morbimortality, these conditions are considered a health crisis. The impact of these diseases can be worsened by the poor socioeconomic status of the affected countries, most of them under-developed, which affects the health care provided to patients. The morbidity of these diseases is explained by the wide organic impairment that they produce. The cardiovascular system is par ticularly affected, which explains the high morbimortality of NTD. In this article, we review the key issues of a project elaborated by Emerging Leaders of the Interamerican Society of Cardiology (SIAC), about the cardiovascular impact of NTD: the NET-Heart Project (Neglected Tropical Diseases and other Infectious Diseases affecting the Heart).

Resumen Las enfermedades tropicales desatendidas (ETD) constituyen un conjunto de afecciones altamente prevalentes en regiones tropicales y subtropicales, asociadas a pobreza y subdesarrollo. Constituyen una verdadera crisis sanitaria, incapacitando y llevando a la muerte a millones de personas anualmente. Esto se ve potenciado por las dificultades socioeconómicas que cursan estos países, mayormente en vías de desa rrollo, lo cual repercute en la calidad de la asistencia sanitaria que pueden proveer a la población. La morbilidad de estas enfermedades se explica por la amplia afectación orgánica que generan. El aparato cardiovascular resulta particularmente afectado, lo que explica en gran medida la morbimortalidad de las ETD. En el presente artículo se revisan los aspectos fundamentales de un proyecto llevado a cabo por los Líderes Emergentes de la Sociedad Interamericana de Cardiología (SIAC), cubriendo diferentes aspectos del impacto sobre el aparato cardiovascular de las ETD: el Proyecto NET-Heart (del inglés, Neglected Tropical Diseases and other Infectious Diseases Affecting the Heart).

Abstract This Consensus comes to light in the midst of one of the worst global health crises in the last 100 years. SARS-CoV-2 and its clinical manifestation, COVID-19, have caused a disruption in how doctors and patients interact. Although it is an infectious disease, one of the most notable characteristics is that its mortality increases in patients with chronic non-communicable diseases and, in particular, with a history of diabetes and cardiovascular disease. In this context, then, we believe that the recommendations made in this document, which aim to identify and protect these patients, become more relevant, while the implementation, beyond the statements, of specific cardiometabolic prevention policies becomes a priority. Methodology: this consensus is the result of the will of two Scientific Societies that have recognized the need to complement the focus on the same problem: that of patients with diabetes mellitus (DM) and cardiovascular disease (CVD), or who are at risk to suffer it. Both the Argentine Society of Cardiology (SAC) and the Argentine Society of Diabetes (SAD) have a recognized track record in the production of practice guidelines and consensus documents, although each has different work practices and cultures. Consequently, the first task that was faced was to agree, not only on the agenda and the approach to the different issues, but also on common working methods: general objective of the document, how to analyze and evalua-te the weight of the information, define the levels of evidence and determine the degrees of recommendation. It was agreed to adopt the modality used by the SAC in all the documents produced by the Consensus and Standards Area, set out in the following table: The experts who collaborated in the drafting of the Consensus were selected and invited to participate with the unanimous agreement of the group of Directors and Secretaries belonging to both Scientific Societies. Colleagues with recognized experience in the disciplines addressed were summoned to analyze the evidence and write the recommendations. All the methodological aspects and the final recommendations of this document were defined by agreement between the group of Directors and Consensus Secretaries.The information consolidation process was slow: from the decision of both companies until the writing of this document, the field of DM and CVD management underwent profound transformations that transcend the appearance of new therapeutic agents. What has been developed is a new approach model that is, in the words of Dr. Alicia Elbert, transdisciplinary. This has implied waiting and being able to understand and "process" all the information that has emerged in these years. This document, which aims to assist physicians in daily practice, has tried to adopt this new integrative perspective.

Abstract The COVID-19 pandemic has led to measures of social isolation, labor restrictions, a strong information campaign and the suspension of scheduled medical activities. The aim of this study was to describe the impact of these measures on the number of hospitalizations in Cardiovascular Intensive Care Units, with the hypothesis that the social behavior generated by this emergency promotes a decreased demand for medical care, even when severe cardiovascular disease is involved. We compared the number of admissions in March-April 2010-2019 versus March-April 2020, based on a prospective study including six institutions (three public and three private) that use Epi-Cardio® as a multicenter registry of cardiovascular care unit discharge. Altogether, we included 6839 patients discharged during the 11-year study period (2010-2020). The average number of patient admissions on March-April 2010-19 was 595 (CI 95%: 507-683) and decreased to 348 in 2020 (fall of 46.8%, p < 0.001). The reasons for hospitalization were classified into 11 groups and a statistically significant reduction was seen in 10 of these groups: cardiovascular surgery 72.3%, electrophysiological interventions 67.8%, non-ST acute coronary syndromes 52.6%, angioplasties 47.6%, arrhythmias 48.7%, heart failure 46%, atrial fibrillation 35.7%, ST elevation myocardial infarction 34.7%, non cardiac chest pain 31.8%, others 51.6%. Although with low prevalence, hypertensive crisis increased in 89%. The abrupt decrease observed in the number of admissions due to critical pathologies may be considered an “adverse effect” related to the measures adopted, with potentially severe consequences. This trend could be reversed by improving public communication and policy adjustment.

Resumen La pandemia de COVID-19 ha llevado a medidas de aislamiento social, restricciones laborales, fuerte campaña mediática y suspensión de las actividades médicas programadas. El objetivo de nuestro estudio fue relevar el impacto de estas medidas sobre las internaciones en Unidades de Cuidados Intensivos Cardiovasculares, con la hipótesis de que se ha generado un comportamiento social que puede disminuir la demanda de consultas, aun las de enfermedades graves. Comparamos las internaciones de marzo-abril de 2010-2019 con las del mismo bimestre de 2020 sobre la base del registro prospectivo multicéntrico Epi-Cardio® en seis instituciones, tres públicas y tres privadas, que utilizan la epicrisis computarizada para las altas. Fueron incluidos 6839 egresos de ese bimestre en los 11 años. El promedio del número de internaciones en ese bimestre del decenio 2010-19 fue 595 (intervalo de confianza 95%: 507-683) y se redujo a 348 en 2020 (caída del 46.8%, p < 0.001). En la agrupación por 11 causas de internación, la reducción observada fue: cirugía cardiovascular 72.3%, intervenciones electrofisiológicas 67.8%, síndrome coronario agudo sin elevación del ST 52.6%, angio-plastias 47.6%, arritmias 48.7%, insuficiencia cardíaca 46%, fibrilación auricular 35.7%, infarto con elevación del ST 34.7%, dolor no coronario 31.8% y otros 51.6%. Solo se incrementaron las consultas por crisis hipertensivas (89%), aunque la prevalencia fue baja. La caída observada en la internación de entidades clínicas críticas ha sido un “efecto adverso” de las medidas adoptadas ante la pandemia, con consecuencias potencialmente graves, que podrían revertirse con un ajuste de las políticas y la comunicación pública.

Due to the coronavirus disease 2019 (COVID-19) pandemic, a wide number of compounds are under scrutiny regarding their antiviral activity, one of them being hydroxychloroquine. Cardiac aspects of the use of chloroquine and hydroxychloroquine are reviewed in this manuscript. A non-systematic review of the medical literature was performed. Information about their safety and efficacy as antimalarials, antivirals, as well as in the long-term treatment of rheumatic diseases was collected. We found an anti-inflammatory effect with reduction of long-term cardiovascular events, a very infrequent heart disease due to a lysosomal effect of the drug, and at the hemodynamic level hypotension, tachycardia, and QT interval prolongation, exacerbated when combined with azithromycin. However, the rate of adverse cardiac events of hydroxychloroquine (and chloroquine) was low.

Ante la pandemia de COVID-19 (del inglés coronavirus disease 2019), uno de los fármacos propuesto para su tratamiento es la hidroxicloroquina. Se revisan aquí aspectos cardiológicos del uso de cloroquina e hidroxicloroquina. Se realizó una revisión no sistemática en la literatura médica orientada a la búsqueda de información acerca de su seguridad y eficacia como antimaláricos y antivirales, así como en el tratamiento prolongado de enfermedades reumatológicas. Se halló un efecto antiinflamatorio con reducción de eventos cardiovasculares a largo plazo, una cardiopatía muy infrecuente por un efecto lisosomal del fármaco, y a nivel hemodinámico hipotensión, taquicardia, y prolongación del intervalo QT, exacerbado si se combina con azitromicina. Sin embargo, la tasa de eventos adversos cardíacos de la hidroxicloroquina y la cloroquina fue baja.

background: Although various studies refer to the effect of meditation on blood pressure (BP), its impact on other cardiovascular clinical variables is unknown. Objective: The aim of this study was to evaluate the effects of a meditation program on pulse wave velocity (PWV), quality of life and ultrasensitive C-reactive protein (us-CRP) in patients with ischemic heart disease or chronic heart failure. Methods: This was a randomized study with two groups of patients: a meditation group (M) and an active control group (AC) with cardiovascular health education, evaluating the difference between initial and final values at 12 weeks of B P, PWV, quality of life (assessed by the SF-36 questionnaire) and us-CRP. results: Thirty-five patients were included in the M group and 35 in the AC group; mean age was 61 years and 80% were men. Both groups had similar baseline characteristics, except for higher number of smokers and triglyceride levels in the M group. At 12 weeks, no significant differences were found for ∆PWV: +0.51 (±1.40) in AC and +0.19 (±1.53) in M (p=0.37). Conversely, ∆SF-36 was +0.79 (±7.58) in AC vs. +5.40 (±9.69) (p=0.03) in M, and ∆us-PCR was +1.17 (±2.9) in AC vs. -0.69 (±0.89) in M (p=0.02). Conclusions: A meditation program did not significantly modify PWV at 12 weeks. However, patients allocated to this intervention improved their quality of life and us-PCR was significantly reduced. Larger studies are required to confirm these findings and explore the mechanisms involved in this improvement.

A group of interdisciplinary experts (cardiologists, clinicians, infectologists met with the purpose of analyzing the evidence revealed by the relationship between respiratory diseases caused by influenza, pneumococcal diseases and cardio-vascular events, and the role played by immunization strategies applied in cardio-vascular prevention. The present statement summarizes the conclusions reached by the expertise of the aforementioned professionals. Systematic revisions imply consistent evidence that influenza and pneumococcal infection lead to acute myocardial infarction and cardiovascular death. Studies published during the last 15 years suggest that vaccination against influenza and S. pneumoniae reduce the risk of acute coronary syndromes. With the current evidence, and considering cost-effectiveness, reducing operating expenses and safety profile of the vaccines, scientific societies, national and international government health agencies strongly recommend incorporating immunization programs in those patients with chronic cardiovascular disease.

La relación entre las infecciones respiratorias producidas por el virus de la influenza y el neumococo y los eventos cardiovasculares motivaron la reunión de un grupo interdisciplinario (cardiólogos, clínicos e infectólogos), con el objeto de analizar la evidencia entre la asociación de estos fenómenos y el rol de las estrategias de inmunización en la prevención de la enfermedad cardiovascular. El presente documento sintetiza las conclusiones del grupo de trabajo. El análisis de revisiones sistemáticas sugiere una evidencia consistente entre la infección por influenza y neumococo como desencadenantes de infarto agudo de miocardio y muerte cardiovascular. Los estudios publicados en los últimos 15 años sugieren que la vacunación para influenza y neumococo reducen el riesgo de síndromes coronarios agudos. Con la evidencia existente y teniendo en cuenta los análisis de costo-efectividad, ahorro de costos y perfil de seguridad de las vacunas, las sociedades científicas y agencias gubernamentales de salud, tanto nacionales como internacionales, recomiendan fuertemente la incorporación de la inmunización en el grupo de pacientes con enfermedad cardiovascular crónica.

O bloqueio dos efeitos adversos do sistema renina-angiotensina-aldosterona (SRAA) tem sido o foco principal no desenvolvimento de drogas para o tratamento de doença cardiovascular nos últimos 30 anos. Os níveis plasmáticos da aldosterona temporariamente diminuem depois de início do tratamento com inibidores da enzima conversora da angiotensina (IECA) e tem mostrado-se que a aldosterona ter efeitos adversos sobre o sistema cardiovascular independente da angiotensina II. Em duas reuniões consecutivas, 50 líderes de opinião se reuniram para discutir criticamente as evidências atuais. O presente documentoreflete o consensosobre o assunto: "Papel do bloqueio da aldosterona no tratamento da hipertensão arterial refratária e hiperaldosteronismo". Interesse clínico no bloqueio da aldosterona em pacientes tratados com IECA ou antagonistas do receptor de angiotensina (ARA) foi estimulado pelo estudo RALES (Randomized Aldactone Evaluation Study), que demonstrou que o antagonista receptor mineralocorticóide (RMC), espironolactona, reduziu o risco de mortalidade por qualquer causa e hospitalizações por insuficiência cardíaca (IC) em pacientes com IC severa (classe funcional III-IV, NYHA) e fração de ejeção diminuída. A hipertensão arterial refratária (HTAR) é um problema médico comum que afeta entre 10% a 30% dos pacientes hipertensos, de acordo com a população em estudo. Os estudos em HTAR são limitados pelo elevado risco cardiovascular desses pacientes. O papel da aldosterona em perpetuar a refratariedade através do seu efeito conhecido de retenção e produção de disfunção endotelial de sódio e água tem motivado o uso de bloqueadores dos receptores mineralocorticóides, demonstrando uma redução drástica na pressão arterial sistólica e diastólica, por o uso é atualmente recomendada como quarta linha de tratamento em pacientes com HTAR, que não respondem à terapia apropriada anteriormente (Guia da Sociedade Europeia de Hipertensão/Sociedade Europeia de Cardiologia 2007).

Blocking the adverse effects of the rennin-angiotensin system has been a major focus of drug development for the treatment of cardiovascular disease over the last 30 years. Plasma aldosterone levels are only transiently decreased suppressed after the initiation of angiotensin-converting enzyme (ACE) inhibitors treatment and has been shown that aldosterone causes adverse effects on the cardiovascular system independent of angiotensin II. In two consecutive meetings, 50 experts critically reviewed the available evidence. The present document reflects the consensus of the subject: "Role of aldosterone blockade in the treatment of refractory arterial hypertension and hyperaldosteronism". Clinical interest in blocking aldosterone in patients treated with ACE inhibitors or angiotensin receptor blockers (ARBs) was stimulated by the Randomized Aldactone Evaluation Study (RALES), which demonstrated that the mineralocorticoid (MC) antagonist spironolactone reduced the risk of all-cause mortality as well as hospitalizations for heart failure (HF) in patients with severe NYHA Class III-IV HF and a reduced left ventricular ejection fraction (LVEF). The refractory arterial hypertension (RAH) is a common medical problem that affects from 10% to 30% of hypertensive patients, according to the study population. Studies in RAH are limited by the high cardiovascular risk of these patients. The role of aldosterone perpetuate refractoriness through its known effect of sodium and water retention and production of endothelial dysfunction have motivated the use of mineralocorticoid receptor blockers, demonstrating a dramatic reduction in the systolic and diastolic blood pressure, by what use is currently recommended as a fourth-line treatment in patients with RAH who did not respond to appropriate therapy previously (Guide to the European Society of Hypertension / European Society of Cardiology 2007).

El bloqueo de los efectos adversos del sistema renina-angiotensina-aldosterona (SRAA) ha sido un foco importante en el desarrollo de drogas para el tratamiento de la enfermedad cardiovascular en los últimos 30 años. Los niveles plasmáticos de aldosterona disminuyen en forma transitoria luego del inicio del tratamiento con inhibidores de la enzima convertidora de angiotensina (IECA) y se ha demostrado que la aldosterona ejerce efectos adversos sobre el sistema cardiovascular en forma independiente de la angiotensina II. En dos reuniones consecutivas 50 líderes de opinión se reunieron para discutir en forma crítica la evidencia actualmente disponible. El presente documento sintetiza las conclusiones que surgieron por consenso de la mesa: "Rol del bloqueo aldosterónico en el tratamiento de la hipertensión arterial refractaria y estados de hiperaldosteronismo". El interés clínico en el bloqueo aldosterónico en pacientes tratados con IECA o antagonistas de los receptores de angiotensina (ARA) fue estimulado por el Estudio RALES (Randomized Aldactone Evaluation Study), que demostró que el antagonista de los receptores mineralocorticoides (RMC), espironolactona, redujo el riesgo de mortalidad de toda causa así como de hospitalizaciones por insuficiencia cardíaca (IC), en pacientes con IC severa (clase funcional III-IV, NYHA) y fracción de eyección disminuida. La hipertensión arterial refractaria (HTAR) es un problema médico común que afecta entre el 10% al 30% de los pacientes hipertensos, según la población evaluada. Los estudios en HTAR se ven limitados por el alto riesgo cardiovascular de estos pacientes. El rol de la aldosterona en perpetuar la refractariedad a través de su conocido efecto de retención de sodio y agua y la producción de disfunción endotelial han motivado el uso de bloqueantes de receptores mineralocorticoides, demostrándose una drástica reducción de la presión arterial sistólica y diastólica, por lo que actualmente su empleo es recomendado como cuarta línea de tratamiento en pacientes con HTAR que no respondieron a una terapia adecuada previamente (Guía de la Sociedad Europea de Hipertensión/Sociedad Europea de Cardiología 2007).
