RESUMO: Descreve-se o primeiro caso de pitiose rinofacial com disseminação em um equino adulto no Uruguai. O animal apresentava uma massa parcialmente circunscrita medindo 30 x 23 x 9 cm acometendo o lado direito da face e com envolvimento dos tecidos cutâneo, subcutâneo e muscular, cavidade nasal direita e estruturas adjacentes. Os principais achados patológicos incluem a formação de kunkers microscopicamente caracterizados por dermatite piogranulomatosa e eosinofilica, reação de Splendore-Hoeppli, e numerosas hifas intralesionais imunomarcadas com anticorpo policlonal anti-Pythium insidiosum. Lesões semelhantes foram observadas no pulmão e nos linfonodos submandibulares. O diagnóstico foi baseado nas lesões histológicas características e imunomarcação com anticorpo anti-P. insidiosum. ABSTRACT: We described the first case of rhinofacial pythiosis with dissemination in an adult horse in Uruguay. The affected horse presented a partially circumscribed mass measuring 30 x 23 x 9 cm affecting the right side of the face with involvement of cutaneous, subcutaneous, and muscular tissues, the right nasal cavity, and adjacent structures. At postmortem examination, the main lesions were “kunkers” characterized by pyogranulomatous and eosinophilic dermatitis, Splendore-Hoeppli reaction and numerous intralesional hyphae immunolabeling with polyclonal anti-Pythium insidiosum antibody. Similar lesions were observed in the lung and submandibular lymph nodes. The diagnosis was made by the characteristic histological lesions and the strong immunolabelling by anti-P. insidiosum antibody. Objective: to associate polypharmacy and medication classes with the risk of falls in the elderly. Method: cross-sectional and quantitative study, carried out in a Specialty Medical Outpatient Clinic in the city of São Paulo – SP, with 117 elderly people, from March to November 2019. Data collection was carried out by fully transcribing the prescriptions into an individual form and the Downton Falls Risk Scale. To compare the risk of falls with polypharmacy and medication classes, the Mann-Whitney and Chi-Square tests were used. In all comparative analyzes a significance level of 5% and a confidence interval of 95% were used. Results: The risk of falls showed a significant correlation with the number of medications, an average of 5.8 per day (p<0.0001) and the medication classes, antihypertensives (p<0.0001), oral antidiabetics (p=0.027), diuretics (p<0.0001) and antidepressants (p=0.042). Conclusion and implications for practice: therefore, the importance of evaluating factors related to the increased risk of falls is highlighted, for subsequent planning and implementation of strategies to deal with adverse reactions and the measures to be taken to ensure the effectiveness and safety of drug treatment.

Objetivo: associar a polifarmácia e as classes medicamentosas com o risco de quedas em idosos. Método: estudo transversal e quantitativo, realizado em um Ambulatório Médico de Especialidades na cidade de São Paulo – SP, com 117 idosos, no período de março a novembro de 2019. A coleta dos dados foi realizada pela transcrição integral dos receituários para um formulário individual e pela Escala de Risco de Quedas de Downton. Para comparar o risco de quedas com a polimedicação e as classes medicamentosas foram utilizados os testes de Mann-Whitney e o Qui-Quadrado. Em todas as análises comparativas foi utilizado um nível de significância de 5% e intervalo de confiança de 95%. Resultados: O risco de quedas apresentou correlação significativa com o número de medicamentos, média de 5,8 ao dia (p<0,0001) e as classes medicamentosas, os anti-hipertensivos (p<0,0001), os antidiabéticos orais (p=0,027), os diuréticos (p<0,0001) e os antidepressivos (p=0,042). Conclusão e implicações para a prática: portanto, ressalta-se a importância da avaliação dos fatores relacionados ao aumento do risco de quedas, para posterior realização de um planejamento e implementação de estratégias no cuidado das reações adversas e nas medidas a serem tomadas para garantir a efetividade e segurança do tratamento medicamentoso.

RESUMO Contexto: Em estudos observacionais sobre a síndrome do desconforto respiratório agudo, sugeriu-se que a driving pressure é o principal fator de lesão pulmonar induzida por ventilador e de mortalidade. Não está claro se uma estratégia de limitação da driving pressure pode melhorar os desfechos clínicos. Objetivo: Descrever o protocolo e o plano de análise estatística que serão usados para testar se uma estratégia de limitação da driving pressure envolvendo a titulação da pressão positiva expiratória final de acordo com a melhor complacência respiratória e a redução do volume corrente é superior a uma estratégia padrão envolvendo o uso da tabela de pressão positiva expiratória final baixa do protocolo ARDSNet, em termos de aumento do número de dias sem ventilador em pacientes com síndrome do desconforto respiratório agudo devido à pneumonia adquirida na comunidade. Métodos: O estudo STAMINA (ventilator STrAtegy for coMmunIty acquired pNeumoniA) é randomizado, multicêntrico e aberto e compara uma estratégia de limitação da driving pressure com a tabela de pressão positiva expiratória final baixa do protocolo ARDSnet em pacientes com síndrome do desconforto respiratório agudo moderada a grave devido à pneumonia adquirida na comunidade internados em unidades de terapia intensiva. Esperamos recrutar 500 pacientes de 20 unidades de terapia intensiva brasileiras e duas colombianas. Eles serão randomizados para um grupo da estratégia de limitação da driving pressure ou para um grupo de estratégia padrão usando a tabela de pressão positiva expiratória final baixa do protocolo ARDSnet. No grupo da estratégia de limitação da driving pressure, a pressão positiva expiratória final será titulada de acordo com a melhor complacência do sistema respiratório. Desfechos: O desfecho primário é o número de dias sem ventilador em 28 dias. Os desfechos secundários são a mortalidade hospitalar e na unidade de terapia intensiva e a necessidade de terapias de resgate, como suporte de vida extracorpóreo, manobras de recrutamento e óxido nítrico inalado. Conclusão: O STAMINA foi projetado para fornecer evidências sobre se uma estratégia de limitação da driving pressure é superior à estratégia da tabela de pressão positiva expiratória final baixa do protocolo ARDSnet para aumentar o número de dias sem ventilador em 28 dias em pacientes com síndrome do desconforto respiratório agudo moderada a grave. ABSTRACT Background: Driving pressure has been suggested to be the main driver of ventilator-induced lung injury and mortality in observational studies of acute respiratory distress syndrome. Whether a driving pressure-limiting strategy can improve clinical outcomes is unclear. Objective: To describe the protocol and statistical analysis plan that will be used to test whether a driving pressure-limiting strategy including positive end-expiratory pressure titration according to the best respiratory compliance and reduction in tidal volume is superior to a standard strategy involving the use of the ARDSNet low-positive end-expiratory pressure table in terms of increasing the number of ventilator-free days in patients with acute respiratory distress syndrome due to community-acquired pneumonia. Methods: The ventilator STrAtegy for coMmunIty acquired pNeumoniA (STAMINA) study is a randomized, multicenter, open-label trial that compares a driving pressure-limiting strategy to the ARDSnet low-positive end-expiratory pressure table in patients with moderate-to-severe acute respiratory distress syndrome due to community-acquired pneumonia admitted to intensive care units. We expect to recruit 500 patients from 20 Brazilian and 2 Colombian intensive care units. They will be randomized to a driving pressure-limiting strategy group or to a standard strategy using the ARDSNet low-positive end-expiratory pressure table. In the driving pressure-limiting strategy group, positive end-expiratory pressure will be titrated according to the best respiratory system compliance. Outcomes: The primary outcome is the number of ventilator-free days within 28 days. The secondary outcomes are in-hospital and intensive care unit mortality and the need for rescue therapies such as extracorporeal life support, recruitment maneuvers and inhaled nitric oxide. Steelmaking is an important industrial branch, but it is also associated with large emissions of pollutants into the environment, especially nickel, which is used in the manufacturing process of metallic alloys and steel. Nickel is generally associated with particulate matter and, in industrial regions, its concentration can be 14 times higher than those observed in non-industrialized regions. This study was carried out in a residential area located in the city of Volta Redonda, in the state of Rio de Janeiro, which is impacted by industrial steel activity and by a deposit of steel slag tailings. The goal of this study was to measure air contamination and its impact on the health of the population by determining Ni in the total particulate matter and in the urine of 211 residents. The results obtained showed that the average values of Ni in the atmospheric air varied in the range of 1.3-13.2 ng m-3, with a maximum value of 20 ng m-3, which is comparable to other industrialized regions. Abstract Pathogenic DNA alterations in GJB2 are present in nearly half of non-syndromic hearing loss cases with autosomal recessive inheritance. The most frequent variant in GJB2 causing non-syndromic hearing loss is the frameshifting c.35del. GJB2 encodes Cx26, a protein of the connexin family that assembles hemichannels and gap junctions. The expression of paralogous proteins is believed to compensate for the loss of function of specific connexins. As Cx26 has been involved in cell differentiation in distinct tissues, we employed stem cells derived from human exfoliated deciduous teeth (SHEDs), homozygous for the c.35del variant, to assess GJB2 roles in stem cell differentiation and the relationship between its loss of function and the expression of paralogous genes. Primary SHED cultures from patients and control individuals were compared. SHEDs from patients had significantly less GJB2 mRNA and increased amount of GJA1 (Cx43), but not GJB6 (Cx30) or GJB3 (Cx31) mRNA. In addition, they presented higher induced differentiation to adipocytes and osteocytes but lower chondrocyte differentiation. Our results suggest that GJA1 increased expression may be involved in functional compensation for GJB2 loss of function in human stem cells, and it may explain changes in differentiation properties observed in SHEDs with and without the c.35del variant. ABSTRACT Maize landraces diversity is important for many smallholder and indigenous populations. Gene flow constitutes a threat to the genetic conservation of several locally adapted maize landraces and, risk management depends on monitoring transgenic occurrence in non-GM maize. This study evaluated the occurrence of exogenous proteins from GM-maize in maize landraces and teosinte cultivated in the Alto Jequitinhonha region, State of Minas Gerais. Gene flow from GM maize to non-transgenic varieties was evaluated in eight maize groups and one teosinte variety. A transgenic maize variety was used as a positive control. Resumo O sistema de desinfecção solar, conhecido como SODIS (Solar Water Disinfection), é um método que envolve a exposição da água à radiação solar (luz UVA e calor) para eliminar microrganismos patogênicos. Coagulantes orgânicos extraídos da Moringa oleifera (T1) e Salvia hispanica (T2) foram usados em conjunto com o processo de filtração como auxiliares. Esse processo incluiu filtros de areia com tamanhos de partícula variando de 0,25 mm a 0,850 mm e uma manta geotêxtil não tecida colocada em sua base. Após passarem pelo processo de coagulação/floculação/sedimentação/filtração, as amostras foram expostas ao sol em um concentrador solar usando garrafas pintadas na metade de sua superfície (PR) e garrafas não pintadas (BR). Os parâmetros analisados incluíram propriedades físico-químicas como pH, condutividade elétrica, turbidez e cor aparente, bem como parâmetros microbiológicos, incluindo E. coli e coliformes totais. Os resultados das análises físico-químicas demonstraram a superioridade de T1 em termos de turbidez e cor aparente. Em relação às análises microbiológicas, as análises demostraram a remoção eficaz de E. coli e coliformes totais, com 100% de inativação observada nas amostras. Portanto, foi observado que a sequência de tratamento aplicada reduziu significativamente a carga microbiológica das amostras; contudo, não se pode afirmar com certeza se os coagulantes exerceram alguma influência nesse resultado. Abstract Solar Water Disinfection, known as SODIS, is a treatment method involving the exposure of water to solar radiation (UVA light and heat) to eliminate pathogenic microorganisms. Organic coagulants extracted from Moringa oleifera (T1) and Salvia hispanica (T2) were used in conjunction with the filtration process as auxiliaries. This process included sand filters with particle sizes ranging from 0.25 mm to 0.850 mm and a non-woven geotextile placed at its base. After undergoing the coagulation/flocculation/sedimentation/filtration process, the samples were exposed to sunlight in a solar concentrator using bottles painted on half of their surface (PR) and unpainted bottles (BR). The analyzed parameters included physicochemical properties such as pH, electrical conductivity, turbidity, and apparent color, as well as microbiological parameters, including E. coli and total coliforms. The results of physicochemical analyses demonstrated the superiority of T1 in terms of turbidity and apparent color. Regarding microbiological analyses, they showed the effective removal of E. coli and total coliforms, with 100% inactivation observed in the samples. It was observed that the applied treatment sequence significantly reduced the microbiological load of the samples; however, it cannot be stated with certainty whether the coagulants exerted any influence on this result. Resumo Objetivo: Fazer um levantamento sobre o uso do termo interstitial lung abnormalities nos laudos radiológicos no Brasil, propor uma tradução para o termo e fazer uma breve revisão sobre o tema. Materiais e Métodos: Foi enviada uma pesquisa, por meio de mensagem eletrônica, para diversos radiologistas de todo o Brasil, questionando sobre a familiarização com o termo, qual tradução em português utilizam e se usam os critérios propostos pela diretriz da Sociedade Fleischner. Abstract Objective: To conduct a survey on the use of the term “interstitial lung abnormalities” in radiology reports in Brazil, propose an appropriate Portuguese-language translation for the term, and provide a brief review of the literature on the topic. Materials and Methods: A survey was sent via electronic message to various radiologists in Brazil, asking about their familiarity with the term, which translation of the term they use in Portuguese, and whether they use the criteria proposed by the Fleischner Society. Results: A total of 163 responses were received, from all regions of Brazil. Although the vast majority (88%) of the respondents stated that they were familiar with the term “interstitial lung abnormalities”, there was considerable variation regarding the equivalent term they used in Portuguese. RESUMO Objetivos: avaliar o construto do University Student Depression Inventory, versão brasileira (USDI-BR). Métodos: estudo metodológico, realizado com uma amostra probabilística bola de neve constituída de 334 estudantes de graduação e pós-graduação. Foram realizadas a análise fatorial confirmatória, a confiabilidade pelo coeficiente de ômega de McDonald e o alfa de Cronbach. A análise por componentes principais foi realizada utilizando a rotação ortogonal varimax e a rotação oblíqua oblimin, por meio dos critérios de Kaiser-Meyer-Olkin, teste de esfericidade de Bartlett e scree plot. Resultados: o USDI-BR apresentou consistência interna dos itens de ω = 0,95 e permaneceu com 30 itens, com acréscimo de 1 fator (Desejo de morrer e retraimento social), totalizando 4 fatores. ABSTRACT Objectives: to assess the University Student Depression Inventory, Brazilian version (USDI-BR), construct. ABSTRACT Objective To evaluate deaths, hospitalizations, and persistence of symptoms in patients with COVID-19 after infection in an outpatient setting during the first COVID-19 wave in Brazil. Methods This prospective cohort was between April 2020 and February 2021. Hospitalized or non-hospitalized COVID-19 patients until five days after symptom onset were included. The outcomes measured were incidence of death, hospitalization, and persistence of more than two symptoms 60 days after discharge. Results Out of 1,198 patients enrolled in the study, 66.7% were hospitalized. A total of 289 patients died (1 [0.3%] non-hospitalized and 288 [36%] hospitalized). At 60 days, patients non-hospitalized during admission had more persistent symptoms (16.2%) compared to hospitalized (37.1%). The COVID-19 severity variables associated with the persistence of two or more symptoms were increased age (OR= 1.03; p=0.015), respiratory rate at hospital admission (OR= 1.11; p=0.005), length of hospital stay of more than 60 days (OR= 12.24; p=0.026), and need for intensive care unit admission (OR= 2.04; p=0.038). Conclusion COVID-19 survivors who were older, tachypneic at admission, had a hospital length of stay >60 days, and were admitted to the intensive care unit had more persistent symptoms than patients who did not require hospitalization in the early COVID-19 waves. RESUMO Objetivo: Estimar as prevalências e fatores associados com as hepatites A, B e C em mulheres trans e travestis em cinco regiões do Brasil. Métodos: Estudo transversal com mulheres trans e travestis em cinco capitais brasileiras (Campo Grande, Manaus, Porto Alegre, Salvador e São Paulo), entre dezembro/2019 e julho/2021. As amostras foram submetidas à detecção de marcadores das infecções pelos vírus das hepatites A (HAV), B (HBV) e C (HCV), utilizando-se testes rápidos e quimioluminescência. Amostras positivas foram submetidas à detecção de HBV-DNA e HCV-RNA por PCR em tempo real e genotipadas por sequenciamento de Sanger. Resultados: As análises de 1.317 amostras indicaram taxas de prevalências nas mulheres trans e travestis recrutadas de 69,1%, 24,4% e 1,5% para exposição ao HAV, HBV e HCV, respectivamente. Elevada taxa de suscetibilidade ao HBV (35,7%) e baixa prevalência do marcador vacinal (40,0%) foram observadas. Mostraram-se associadas à presença de anti-HAV: idade maior que 26 anos, autodeclarar-se preta-parda, ter apenas educação básica, história de encarceramento e uso de preservativo na última relação sexual com parceiro casual. Quanto à exposição ao HBV, foi associada a idade maior que 26 anos, cor da pele preto-parda, ter sido profissional do sexo e história de encarceramento. Idade maior de 37 anos, história de abuso sexual e consumo frequente de álcool foram associadas ao HCV. Conclusão: As maiores prevalências de HAV nessa população encontram-se nas regiões Norte e Nordeste. Com relação ao HBV, a prevalência encontrada foi superior à encontrada na população geral, sugerindo maior vulnerabilidade. A prevalência do HCV foi semelhante à encontrada na população geral. ABSTRACT Objective: To estimate the prevalence and factors associated with hepatitis A, B, and C in transgender women and travestis's networks, in 5 regions of Brazil. Methods: This cross-sectional study includedtransgender women and travestis in five Brazilian capitals (Campo Grande, Manaus, Porto Alegre, Salvador, and São Paulo), between December/2019 and July/2021. All samples were subjected to detection of serological markers of hepatitis virus A (HAV), B (HBV), and C (HCV) infections through rapid tests and chemiluminescent microparticle immunoassays. Positive samples in the screening tests were submitted to detect HBV DNA and HCV-RNA by real-time PCR and genotyped by Sanger sequencing. Results: Analysis of 1,317 samples showed network prevalence rates of 69.1%, 25.1%, and 1.5% for HAV, HBV, and HCV exposure, respectively. A high susceptibility rate to HBV infection (35.7%) and low prevalence of vaccine response markers (40%) were also observed. Age greater than 26 years, self-declared black/brown skin color, having only primary education, history of incarceration, and use of a condom in the last sexual intercourse with a casual partner were associated with total anti-HAV. Exposure to HBV was associated with age greater than 26 years, self-declared black/brown, history of being a sex worker, and incarceration. Age > 37 years, history of sexual abuse, and frequent alcohol consumption were associated with hepatitis C infection. Conclusion: The highest prevalence of HAV in this population was found in the North and Northeast regions, and the prevalence found was higher than that in the general population, suggesting greater vulnerability. The prevalence of HCV infection in our study was similar to that observed in the general population. ABSTRACT Objective: Previous studies indicated that face masks reduce the probability of infection by SARS-CoV-2 but did not examine the relationship between SARS-CoV-2 viral load and mask usage. This study analyzed this relationship. Methods: This cross-sectional study evaluated patients admitted to a public Emergency Care Unit in Belo Horizonte, MG, Brazil, between October 2020 and March 2021. Convenience samples were recruited during the study period. Adult patients with COVID-19 symptoms were invited to participate after being examined by a physician according to the Emergency Care Unit routine. This study included 441 adults with mild symptoms diagnosed with COVID-19 via RT-PCR, who were tested until the 14th day after symptom onset. Participants were interviewed about mask usage, oropharyngeal swabs were collected, and the SARS-CoV-2 viral load was measured using RT-PCR. The difference between the SARS-CoV-2 viral load in the groups of individuals who regularly used and did not use masks was subjected to a hypothesis test. Results: Of the patients who had swabs collected 1-5 days after symptom onset, 64.9% of those not using masks regularly and 45.5% of those using masks regularly presented a high SARS-CoV-2 viral load. This difference was statistically significant (p=0.0073). Considering only the patients who had swabs collected 4-8 days after symptom onset, 44.1% of those not using masks regularly and 30.8% of those using masks regularly presented a high SARS-CoV-2 viral load; this difference was statistically significant (p=0.0364). Conclusion: When people who regularly wear a face mask contract COVID-19, they have a significantly lower probability of having a high viral load of SARS-CoV-2 compared to those who do not regularly wear masks. Abstract Based on the literature on rising powers, we contend that India has historically perceived itself as a civilizational entity and benefited from the loopholes opened by power transition periods to connect its material capabilities with its normative ambition. RESUMO O estudo investigou as habilidades sociais (HS) presentes nas interações entre personagens de livros infantis. A amostra foi composta por 77 obras. Desse total, 42 livros apresentaram 168 situações de interações entre personagens que foram categorizadas segundo o sistema de classes e subclasses das HS na infância propostas por Z. Del Prette e Del Prette. A classe Assertividade (56) foi identificada em maior frequência e fazer e recusar pedidos, subclasse desta categoria, foi a que obteve mais interações (20). A classe Civilidade (16) apresentou a menor frequência. Sugere-se que a literatura infantil possa ser usada como recurso psicoeducativo para aquisição das HS desde a infância e servir como uma ferramenta indispensável no processo do letramento e alfabetização dos alunos nas séries iniciais. ABSTRACT The study investigated the social skills (SS) present in interactions among characters in children’s books. The sample consisted of 77 works. In this way, 42 books presented 168 situations of interactions among characters that were categorized according to the system of classes and subclasses of HS in childhood proposed by Z. Del Prette and Del Prette. The Assertiveness class (56) was identified most frequently and making and refusing requests, a subclass of this category, was the one that had the most interactions (20). The Civility class (16) had the lowest frequency. It suggested that children’s literature can be used as a psychoeducational resource for acquiring SS from childhood and serve as an indispensable tool in the students’ literacy process in the initial grades. RESUMEN En el estudio se investigó las habilidades sociales (HS) presentes en las interacciones entre personajes de libros infantiles. La muestra se compuso por 77 obras. De ese total, 42 libros presentaron 168 situaciones de interacciones entre personajes que se categorizaron según el sistema de clases y subclases de las HS en la infancia propuestas por Z. Del Prette y Del Prette (2013). La clase Asertividad (56) se identificó en más frecuencia y hacer y rechazar pedidos, subclase de esta categoría, fue la que obtuvo más interacciones (20). La clase Civilidad (16) presentó la menos frecuencia. Alum sludge (AS) from water treatment plants is an abundant, non-toxic residue with high Si and Al contents. These characteristics make it an ideal precursor for zeolite synthesis, not only reducing the environmental impacts from AS disposal but also adding value to the waste by promoting its reuse. Herein, we propose a route to prepare zeolitic materials from AS, aiming for its application as a component of slow-release fertilizers, using mild conditions and cost-effective hydrothermal synthesis. X-Ray Diffraction revealed the formation of sodalite (Na-SOD) in short times (5-10 h) and moderate temperatures (100-120ºC), with sodium silicate addition favoring Si-quartz consumption and zeolite crystallization, without a fusion step. The Na-SOD showed a desirable cation exchange capacity, reaching 117 mg g-1 for K+. A K-SOD fertilizer was then prepared with an average 490 nm size, 2 m2 g-1 BET surface area, and slow-release behavior. Incubation in soil confirmed that Al was inactive in K-SOD and did not immobilize phosphate, in addition to promoting high K+ availability. The results support the potential of AS reuse for application in agriculture as an efficient and sustainable fertilizer.
