RESUMO: O objetivo deste estudo foi avaliar a eficácia clínica da aplicação intra-articular de células-tronco mesenquimais alógenas derivadas do tecido adiposo (AD-CTM) no tratamento de cães portadores de osteoartrite secundária a displasia do coxofemoral (DCF). Doze cães de ambos os sexos, diferentes raças, idades e peso, portadores de graus variados de DCF comprovada em radiografia e livres de quaisquer outras alterações clínicas ou ortopédicas, foram utilizados no estudo. Todos os cães foram submetidos a avaliação ortopédica nos dias 0, 30, 60 e 90 após aplicação de AD-CTM na dose de 2 x 106. Além disso, os tutores preencheram a um questionário, baseado no Índice de dor crônica de Helsinque (IDCH) nos mesmos intervalos. Em comparação com o dia 0, observou-se melhora significativa na avalição em locomoção e físico-ortopédica assim como na avaliação dos tutores pelo IDCH ao longo dos 90 dias. Os resultados permitem inferir que as AD-CTM alógenas contribuíram significativamente para a redução dos sinais clínicos comumente associados a DCF durante o período de estudo. Entretanto, há necessidade de estudos de longo prazo para melhor determinação de protocolos terapêuticos baseados no uso de AD-CTM na rotina clínica. ABSTRACT: This study assessed the efficacy of an intra-articular injection of allogeneic adipose tissue-derived from mesenchymal stem cells (AD-MSCs) for the treatment of hip dysplasia in dogs. The study group included 12 otherwise healthy dogs of different breeds, ages, weights, and degrees of hip dysplasia diagnosed using radiography. An orthopedic assessment was performed on all dogs before and at 30, 60, and 90 days after infusion of AD-MSCs(2 × 106cells). On the same days, each dog’s owner answered a questionnaire based on theHelsinkiChronic Pain Index. The data were converted to ordinal data based on the score for each variable, and the Friedman test for multiple comparisons was used to verify the results. Compared with the corresponding values on day 0, orthopedic and gait assessments and owners’ reported pain indexes improved over the 90-day observation period. These results suggested that treatment with allogeneic AD-MSCs significantly reduced the clinical signs associated with hip dysplasia during the study period. However, long-term studies are needed to determine the optimal therapeutic protocol for routine clinical use of AD-MSCs in hip dysplasia. RESUMEN La endoscopia cervical es una de las etapas más avanzadas en la curva de aprendizaje de técnicas endoscópicas para cirugías de columna, que requiere un control preciso de los instrumentos para evitar complicaciones graves. Reportamos el caso de una paciente de 53 años con cervicobraquialgia bilateral y mielopatía causada por una estenosis central grave en C2-C3. Tras el fracaso del tratamiento conservador, se realizó una descompresión cervical posterior endoscópica over-the-top. La cirugía se completó con éxito, con una mejora significativa en el dolor y la función neurológica. A los seis meses de seguimiento, la paciente mostró un aumento en su puntuación mJOA, lo que indica una mejora en la funcionalidad. RESUMO A endoscopia cervical é um dos estágios mais avançados da curva de aprendizado em técnicas endoscópicas para cirurgias de coluna, exigindo controle preciso de instrumentos para evitar complicações graves. Relatamos o caso de uma paciente de 53 anos com cervicobraquialgia bilateral e mielopatia causada por estenose central grave em C2-C3. Após falha no tratamento conservador, foi realizada uma descompressão cervical posterior over-the-top endoscópica. A cirurgia foi concluída com sucesso, com melhora significativa da dor e função neurológica. No seguimento de seis meses, a paciente apresentou aumento de sua pontuação na escala mJOA, indicando melhora funcional. ABSTRACT Cervical endoscopy is one of the most advanced stages in the learning curve for endoscopic techniques in spine surgery, requiring precise control of instruments to avoid severe complications. We report the case of a 53-year-old female patient with bilateral cervicobrachialgia and myelopathy caused by severe central stenosis at C2-C3. After the failure of conservative treatment, an endoscopic posterior over-the-top cervical decompression was performed. The surgery was completed, with significant improvement in pain and neurological function. At six months of follow-up, the patient showed an increase in her mJOA score, indicating improved functionality. This case demonstrates that the endoscopic technique can be effective and safe in treating cervical stenosis with spinal cord compression and myelopathy. RESUMEN El objetivo de este artículo fue presentar un caso inusual de resección totalmente endoscópica de un osteoma osteoide en la vértebra S1 en un paciente con dolor lumbar sin síntomas neurológicos. El paciente era un hombre de 26 años con dolor lumbar significativo, que empeoraba por la noche y mejoraba con el uso de antiinflamatorios no esteroideos (AINE). Las imágenes lumbares revelaron una lesión osteoblástica en el cuerpo vertebral de S1, cerca del pedículo derecho, sin nidus evidente y sin captación de contraste. Se realizó una biopsia guiada por tomografía computarizada, que confirmó el diagnóstico de osteoma osteoide. El paciente fue tratado con una resección transcorporal totalmente endoscópica de S1 mediante un abordaje posterolateral extra-foraminal. La técnica quirúrgica fue descripta en detalle, y el paciente mostró una mejora clínica inmediata de los síntomas. Aunque existen varias técnicas quirúrgicas para la resección de tumores espinales, no hay consenso en la literatura sobre la mejor técnica quirúrgica o sobre los tratamientos conservadores. RESUMO O objetivo desse artigo foi apresentar um caso incomum de ressecção totalmente endoscópica de um osteoma osteóide na vértebra S1 em um paciente com dor lombar sem sintomas neurológicos. Paciente masculino de 26 anos com dor lombar baixa importante, com piora noturna e melhora com uso de anti-inflamatórios não esteróides (AINE). Exames de imagem lombar evidenciaram lesão osteoblástica no corpo de S1 próximo do pedículo direito, sem nidus evidente e sem captação por contraste. Foi realizada biópsia guiada por tomografia computadorizada, confirmando o diagnóstico de osteoma osteóide. ABSTRACT The objective of this article was to present an uncommon case of fully endoscopic resection of an osteoid osteoma in the S1 vertebra in a patient with lower back pain and no neurological symptoms. The patient was a 26-year-old male with significant lower back pain, worsening at night and improving with the use of non-steroidal anti-inflammatory drugs (NSAIDs). Lumbar imaging revealed an osteoblastic lesion near the right pedicle in the S1 vertebral body, with no evident nidus or contrast uptake. A biopsy guided by computed tomography confirmed the diagnosis of osteoid osteoma. The patient was treated with a fully endoscopic transcorporeal resection of S1 via a posterolateral extra-foraminal approach. The surgical technique was detailed, and the patient showed immediate clinical symptom improvement. Although there are several surgical techniques for the resection of spinal tumors, there is no consensus in the literature regarding the best surgical technique or conservative treatments. RESUMEN El síndrome de cola de caballo (SCC) es una emergencia neurológica grave resultante de la compresión de las raíces nerviosas lumbosacras, lo que puede ocasionar déficits motores, sensoriales y disfunciones esfinterianas. La intervención quirúrgica precoz es fundamental para evitar secuelas neurológicas permanentes. Tradicionalmente, la laminectomía abierta ha sido el enfoque de elección para la descompresión, pero las técnicas mínimamente invasivas, como la discectomía endoscópica, están ganando popularidad debido a su menor morbilidad, incisiones más pequeñas y recuperación más rápida. Este estudio presenta tres casos de pacientes con SCC tratados con discectomía endoscópica. Dos de los casos fueron causados por una hernia discal extrusa volumétrica y uno por un quiste facetario intracanal. La intensidad del dolor se evaluó mediante la Escala Visual Analógica (EVA) y la discapacidad funcional mediante el Oswestry Disability Index (ODI) en el período preoperatorio y en los seguimientos de 1, 6 y 12 meses. Los tres pacientes mostraron una mejora significativa en los síntomas, con reducción del dolor y mejora funcional. A los 12 meses, los pacientes mantuvieron su recuperación funcional y neurológica sin complicaciones reportadas. La técnica endoscópica demostró ser un método seguro y eficaz para el tratamiento del SCC en estos casos, ofreciendo una recuperación precoz y menos complicaciones postoperatorias. A intervenção cirúrgica precoce é fundamental para evitar sequelas neurológicas permanentes. Tradicionalmente, a laminectomia aberta tem sido a abordagem de escolha para descompressão, mas as técnicas minimamente invasivas, como a discectomia endoscópica, estão ganhando espaço por apresentarem menor morbidade, incisões menores e recuperação mais rápida. Este estudo relata três casos de pacientes com SCE tratados com discectomia endoscópica. Dois dos casos foram causados por hérnia discal extrusa volumosa e um por cisto facetário intracanal. ABSTRACT Cauda Equina Syndrome (CES) is a severe neurological emergency resulting from the compression of the lumbosacral nerve roots, potentially causing motor and sensory deficits, as well as sphincter dysfunctions. Early surgical intervention is essential to prevent permanent neurological sequelae. Traditionally, open laminectomy has been the treatment of choice for decompression. Still, minimally invasive techniques, such as endoscopic discectomy, are gaining popularity due to lower morbidity, smaller incisions, and faster recovery. This study reports three cases of patients with CES treated with endoscopic discectomy. Two cases were caused by a large, extruded disc herniation and one by an intracanal facet cyst. Pain intensity was assessed using the Visual Analog Scale (VAS), and functional disability was measured with the Oswestry Disability Index (ODI) in the preoperative period and at follow-ups of 1, 6, and 12 months. All three patients showed significant improvement in symptoms, with pain reduction and functional improvement. At 12 months, the patients maintained their functional and neurological recovery without reported complications. The endoscopic technique proved to be a safe and effective method for treating CES in these cases, offering early recovery and fewer postoperative complications. ABSTRACT Objective Consequently, in this study, we aimed to evaluate 1,203 cases of referral to a quaternary vascular surgical service, in São Paulo, Brazil, over a 6-year period, to assess the appropriate need for referral; in addition to the prevalence of surgical indications. Methods In this retrospective analysis, we reviewed the institutional records of participants referred from Basic Healthcare Units to a vascular surgical service inside the Brazilian Unified Health System, between May 2015 and December 2020. Demographic and clinical data were collected. The participants were stratified, as per the reason for referral to the vascular surgical service, previous imaging studies, and surgical treatment indications. Referral appropriateness and complementary examinations were evaluated for each disease cohort. Finally, the prevalence of cases requiring surgical treatment was defined as the outcome measure. Results Of the 1,203 referrals evaluated, venous disease was the main reason for referral (53%), followed by peripheral arterial disease (19.4%). A considerable proportion of participants had been referred without complementary imaging or after a long duration of undergoing an examination. Referrals were regarded as inappropriate in 517 (43%) cases. Of these, 32 cases (6.2%) had been referred to the vascular surgical service, as the incorrect specialty. The percentage of referred participants who ultimately underwent surgical treatment was 39.92%. Carotid (18%) and peripheral arterial diseases (18.4%) were correlated with a lower prevalence of surgical treatments. Conclusion The rate of referral appropriateness to specialized vascular care from primary care settings was low. This may represent a subutilization of quaternary surgical services, with low rates of surgical treatment. Abstract Background and objective: Lower limb varicose veins are a prevalent disease associated with several available treatment options, including conventional surgery and polidocanol foam sclerotherapy. However, few studies have analyzed therapeutic modality outcomes based on Patient-Reported Outcome Measures (PROMs). This large sample-size study was designed to evaluate the outcomes of polidocanol foam sclerotherapy compared to conventional surgery based on an analysis of PROMs. Methods: This was a prospective, observational, and qualitative study of 205 patients who underwent varicose vein treatment with either polidocanol foam sclerotherapy (57 patients, 90 legs) or conventional surgery (148 patients, 236 legs). Patients were preoperatively assessed and re-evaluated 30 days after the procedure using the Venous Disease Severity Score (VCSS) and specific venous disease quality-of-life questionnaires (VEINES-QoL/Sym). Results: Both treatments significantly improved VCSS and VEINES results 30 days after the procedure (p < 0.05). However, surgery promoted greater improvements in VCSS (on average 4.02-points improvement, p < 0.001), VEINES-QoL (average 8-points improvement, p < 0.001), and VEINES-Sym (average 11.66 points improvement, p < 0.001) than did sclerotherapy. Postoperative pain and aesthetic concerns about the legs were the domains of the questionnaires in which the results varied the most between the treatment modalities, with worse results for sclerotherapy. ABSTRACT The focus of this case report is to technically describe a noninvasive diagnostic evaluation of bow Hunter's syndrome using a dynamic computed tomography angiography protocol and discuss its advantages. In addition, we aimed to exemplify the quality of the study by presenting images of a 3D-printed model generated to help plan the surgical treatment for the patient. The dynamic computed tomography angiography protocol consisted of a first image acquisition with the patient in the anatomic position of the head and neck. This was followed by a second acquisition with the head and neck rotated to the side that triggered the symptoms, with technical parameters similar to the first acquisition. The acquired images were used to print a 3D model to better depict the findings for the surgical team. The dynamic computed tomography angiography protocol developed in this study helped visualize the vertebrobasilar arterial anatomy, detect vertebral artery stenosis produced by head and neck rotation, depict the structure responsible for artery stenosis (e.g., bony structure or membranes), and study possible complications of the disease (e.g., posterior cerebral circulation infarction). Additionally, the 3D-printed model better illustrated the findings of stenosis, aiding in surgical planning. In conclusion, dynamic computed tomography angiography for the evaluation of bow Hunter's syndrome is a feasible noninvasive technique that can be used as an alternative to traditional diagnostic methods. Abstract Background There is a remarkable variability in the frequency of HLA-B27 positivity in patients with spondyloarthritis (SpA), which may be associated with different clinical presentations worldwide. However, there is a lack of data considering ethnicity and sex on the evaluation of the main clinical and prognostic outcomes in mixed-race populations. The aim of this study was to evaluate the frequency of HLA-B27 and its correlation with disease parameters in a large population of patients from the Brazilian Registry of Spondyloarthritis (RBE). Methods The RBE is a multicenter, observational, prospective cohort that enrolled patients with SpA from 46 centers representing all five geographic regions of Brazil. The inclusion criteria were as follow: (1) diagnosis of axSpA by an expert rheumatologist; (2) age ≥18 years; (3) classification according to ASAS axial. The following data were collected via a standardized protocol: demographic data, disease parameters and treatment historical. Results A total of 1096 patients were included, with 73.4% HLA-B27 positivity and a mean age of 44.4 (±13.2) years. Positive HLA-B27 was significantly associated with male sex, earlier age at disease onset and diagnosis, uveitis, and family history of SpA. Conversely, negative HLA-B27 was associated with psoriasis, higher peripheral involvement and disease activity, worse quality of life and mobility. Conclusions Our data showed that HLA-B27 positivity was associated with a classic axSpA pattern quite similar to that of Caucasian axSpA patients around the world. Furthermore, its absence was associated with peripheral manifestations and worse outcomes, suggesting a relevant phenotypic difference in a highly miscegenated population. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract In 2021, rubber bales appeared along the Brazilian coastline, causing environmental impacts. In this research paper, we identified a Second World War shipwreck that was associated with this event. Numerical simulations provided robust evidence to support the hypothesis that the origin of the 2021 bales was the MV Weserland . Moreover, it was sunk to a depth of approximately 5,000 m with a cargo containing rubber, tin, and wolframite. Tin is currently considered a strategic metal, with prices rising from USD 13,375/t in March 2020 to USD 34,462/t in May 2021. We provided evidence that the departure of rubber bales from the wreck and the increase in the price of its metal cargo were temporally associated; this reinforced the hypothesis of deep-sea unauthorized salvage in international waters (areas beyond national jurisdiction). ABSTRACT Background: Patients with peripheral arterial disease have an increased risk of developing cardiovascular complications in the postoperative period of arterial surgeries known as Major Adverse Cardiac Events (MACE), which includes acute myocardial infarction, heart failure, malignant arrhythmias, and stroke. The preoperative evaluation aims to reduce mortality and the risk of MACE. However, there is no standardized approach to performing them. The aim of this study was to compare the preoperative evaluation conducted by general practitioners with those performed by cardiologists. Methods: This is a retrospective analysis of medical records of patients who underwent elective arterial surgeries from January 2016 to December 2020 at a tertiary hospital in São Paulo, Brazil. The authors compared the preoperative evaluation of these patients according to the initial evaluator (general practitioners vs. cardiologists), assessing patients’ clinical factors, mortality, postoperative MACE incidence, rate of requested non-invasive stratification tests, length of hospital stay, among others. Results: 281 patients were evaluated: 169 assessed by cardiologists and 112 by general practitioners. Cardiologists requested more non-invasive stratification tests (40.8%) compared to general practitioners (9%) (p < 0.001), with no impact on mortality (8.8% versus 10.7%; p = 0.609) and postoperative MACE incidence (10.6% versus 6.2%; p = 0.209). The total length of hospital stay was longer in the cardiologist group (17.27 versus 11.79 days; p < 0.001). Conclusion: The increased request for exams didn’t have a significant impact on mortality and postoperative MACE incidence, but prolonged the total length of hospital stay. ABSTRACT The SISBIOTA-BRASIL was a three-year multimillion-dollar research program of the Brazilian government to document plants and animals in endangered/understudied areas and biomes in Brazil. Distributional patterns and the historical events that generated them are extensively unknown regarding Brazilian fauna and flora. This deficiency hinders the development of conservation policies and the understanding of evolutionary processes. Conservation decisions depend on precise knowledge of the taxonomy and geographic distribution of species. Given such a premise, we proposed to research the diversity of Diptera of the Brazilian western arc of Amazon, Cerrado, and Pantanal in the states of Mato Grosso, Mato Grosso do Sul, and Rondônia. Three important biomes of the South American continent characterize these Brazilian states: Amazon forest, Cerrado (Brazilian Savannah), and Pantanal. Besides their ecological relevance, these biomes historically lack intensive entomological surveys. Therefore, they are much underrepresented in the Brazilian natural history collections and in the scientific literature, which is further aggravated by the fact that these areas are being exponentially and rapidly converted to commercial lands. Our project involved over 90 collaborators from 24 different Brazilian institutions and one from Colombia among researchers, postdocs, graduate and undergraduate students, and technicians. We processed and analyzed nearly 300,000 specimens from ~60 families of Diptera collected with a large variety of methods in the sampled areas. Here, we provide a detailed overview of the genera and species diversity of 41 families treated. Our results point to a total of 2,130 species and 514 genera compiled and identified for the three states altogether, with an increase of 41% and 29% in the numbers of species and genera known for the three states combined, respectively. Overall, the 10 most species-rich families were Tachinidae, Cecidomyiidae, Tabanidae, Psychodidae, Sarcophagidae, Stratiomyidae, Bombyliidae, Syrphidae, Tephritidae, and Asilidae. The 10 most diverse in the number of genera were Tachinidae, Stratiomyidae, Asilidae, Mycetophilidae, Syrphidae, Tabanidae, Muscidae, Dolichopodidae, Sarcophagidae, and Chloropidae. So far, 111 scientific papers were published regarding taxonomic, phylogenetic, and biogeographical aspects of the studied families, with the description of 101 new species and three new genera. We expect that additional publications will result from this investigation because several specimens are now curated and being researched by specialists.
