ABSTRACT Musical hallucinations and musical obsessions are distinct phenomena. The first can be understood as a manifestation of the musical ear syndrome, which produces deafferentation auditory hallucinations, while the latter is an obsessive symptom of obsessive-compulsive disorders. Both symptoms are often poorly understood and mistaken for one another or for signs of psychotic disorders. We report two cases, one characterized by musical hallucinations and the other by musical obsessions, both with comorbid hearing impairment, which is the main confounding factor in their differential diagnosis. RESUMO Alucinações musicais e obsessões musicais são fenômenos distintos. O primeiro é compreendido como uma manifestação da Síndrome do Ouvido Musical, a qual produz alucinações auditivas por desaferentação, enquanto o último é um sintoma obsessivo dos transtornos obsessivo-compulsivos. Ambos os sintomas são frequentemente pouco compreendidos e confundidos entre si ou com sinais de transtornos psicóticos. Resumo: Introdução: A pandemia da Covid-19 provocou milhares de mortes e levou a incontáveis mudanças na forma de organização de serviços de saúde e nas escolas de Medicina mundo afora. Relato de experiência: Este artigo relata a experiência da Faculdade de Medicina de Botucatu da Universidade Estadual Paulista (Unesp), cujas aulas foram suspensas em função da pandemia. Discussão: Descrevem-se as motivações para a suspensão e os procedimentos para a retomada das aulas do internato, depois de 15 semanas da interrupção. Conclusão: Ressalta-se a importância das decisões coletivas, da comunicação empática, do acolhimento e cuidado com a saúde mental e da parceria com o Hospital das Clínicas na realização de rastreamento para a presença do vírus entre os estudantes. Por fim, destaca-se o aprendizado para o professor ao se defrontar, por um lado, com a impotência diante da morte e do desconhecido, e, por outro, com a potência do cuidado que pode ser oferecido em situação tão singular quanto uma pandemia.

Abstract: Introduction: The COVID-19 pandemic has caused thousands of deaths and led to countless changes in the way health services and medical schools are organized around the world. Experience report: This article reports on the experience of the Botucatu Faculty of Medicine, UNESP, located in the interior of the state of São Paulo, where classes were suspended due to the pandemic. Discussion: The reasons for the suspension of internship classes and the procedures for their resumption, after 15 weeks, are described. Conclusions: The study highlights the importance of collective decisions, empathic communication, mental health care and attention and the partnership with the Hospital das Clínicas in performing track and trace for presence of the virus among the students. Finally, particular emphasis is given to the learning gained by teachers who found themselves feeling, on the one hand, powerless in the face of death and the unknown and, on the other, the strength of the care that can be offered in a situation as unique as the pandemic.

RESUMO Introdução Tem havido um renovado interesse pela temática da relação médico-paciente e dos valores humanitários na formação dos estudantes de Medicina, pois a capacidade de sentir empatia pelos pacientes é considerada um dos pilares da boa prática médica. A inversão de papéis é uma técnica derivada do psicodrama que pode favorecer o desenvolvimento da capacidade empática na relação médico-paciente. Objetivo Relatar a experiência de ensino-aprendizagem da anamnese psiquiátrica no curso de Semiologia em Psiquiatria com alunos da terceira série do curso médico da Faculdade de Medicina de Botucatu (Unesp), utilizando a inversão de papéis como principal método pedagógico. Método Relato de experiência. A atividade é desenvolvida em cinco etapas distintas, mas inter-relacionadas: (1) leitura e estudo autônomos da apostila de propedêutica psiquiátrica; (2) realização da anamnese em três grupos de 15 alunos, em que estes alternam o papel de médico (entrevistador) e paciente (entrevistado); (3) realização de uma anamnese completa com um colega de turma, alternando-se também o papel de paciente e de médico; (4) realização, pela mesma dupla, da anamnese psiquiátrica com um paciente internado no hospital geral universitário; (5) discussão das anamneses realizadas, atividade em que os alunos assumem o papel do paciente entrevistado para relatar, em primeira pessoa, os dados obtidos na entrevista. Resultados Os alunos mostram-se colaborativos, participando com interesse de todas as etapas da atividade, que parece favorecer não apenas o aprendizado específico da anamnese psiquiátrica, mas também a capacidade empática pelos colegas e pacientes. Nas discussões, atenção especial é dada às descobertas e dificuldades vivenciadas pelos estudantes nessas atividades práticas, assim como aos sentimentos experimentados em cada uma das etapas do processo. A atitude do preceptor de ouvir e acolher com respeito todos os relatos, dúvidas e manifestações afetivas dos alunos procura ser um modelo de postura de acolhimento sem julgamento dos problemas e sentimentos dos pacientes, postura fundamental para o estabelecimento da aliança terapêutica. Conclusão A inversão de papéis pode auxiliar os estudantes de Medicina no aprendizado da anamnese psiquiátrica, assim como no desenvolvimento da empatia, habilidade essencial à prática médica de boa qualidade.

ABSTRACT Introduction There has been renewed interest in the doctor-patient relationship and humanitarian values in the training of medical students, given that the ability to feel empathy for patients is considered one of the pillars of good medical practice. Role reversal is a technique derived from Psychodrama that can promote the development of empathic capacity in the doctor-patient relationship. Objective to report a teaching-learning experience of psychiatric anamnesis in the course of Semiology in Psychiatry with 3rd year students of Botucatu Medical School (UNESP), using the technique of role reversal as the main pedagogical approach. Method experience report. The activity has been developed in five distinct but interrelated stages: 1) autonomous reading and studying the psychiatric propaedeutic courseware; 2) performing the anamnesis in three groups of 15 students, in which they alternate between the roles of physician (interviewer) and patient (interviewed); 3) performing a complete anamnesis with a classmate, also alternating the roles of patient and physician; 4) the same pair of students performs the psychiatric anamnesis with an inpatient of the general university hospital; 5) discussion of the anamneses performed, an activity in which the students assume the role of the patient, reporting, in the first person, the data obtained in the interview. Results the students have been collaborative, participating with interest in all stages of the activity, which seems to favour not only the specific learning of psychiatric anamnesis, but also their empathic capacity towards colleagues and patients. In the discussions, special attention has been given to the discoveries and difficulties experienced by the students in these practical activities, and the feelings experienced in each stage of the process. The preceptors’ attitude of respectfully listening to and accepting all the reports, doubts and affective manifestations of the students provides a model for an accepting and non-judgemental attitude toward patients’ problems and feelings, which is fundamental for the development of the therapeutic alliance. Conclusion Role reversal can be used to help medical students develop their skills in psychiatric anamnesis, and their empathy, an essential skill for good quality medical practice.

RESUMO O desenvolvimento docente é uma necessidade nas escolas para adequar o ensino médico às intensas e contínuas transformações do mundo e da sociedade contemporânea. Iniciativas de desenvolvimento docente precisam avançar para além de cursos de capacitação, evoluindo para comunidades de prática. Com o intuito de incorporar princípios da educação médica ao cotidiano do professor, em diversos níveis, desde o planejamento até a avaliação dos processos de ensino, entendemos que ambientes informais também podem constituir espaços para o desenvolvimento da docência. Assim, a Faculdade de Medicina de Botucatu, da Universidade Estadual Paulista, criou o “café com educação médica”, uma reunião matinal quinzenal para discussão de aportes teóricos para subsidiar as ações de pesquisa e prática do Núcleo de Apoio Pedagógico. A reunião tem configuração semelhante à dos clubes de revista tradicionais no ensino de graduação e residência médica, mas com particularidades voltadas à educação em saúde. O êxito da experiência se deve à possibilidade de ocorrer desenvolvimento docente em contexto acolhedor e informal. Tendo em vista os relatos de transformação da prática docente motivados por iniciativas semelhantes ao redor do mundo e observando como nossa experiência tem sido oportuna, encorajamos as escolas médicas brasileiras a compartilharem também seus clubes de revista em educação médica e os resultados com eles obtidos.

ABSTRACT Faculty development is needed if medical schools are to adapt to the intense and constant changes taking place in the world, and in society. Initiatives for faculty development should go beyond workshops, evolving into communities of practice. To incorporate the principles of medical education into all levels of the curriculum, from planning through to assessment, the assimilation of faculty development is needed, including in informal scenarios. To this end, Botucatu School of Medicine of the Universidade Estadual Paulista created “medical education with coffee”, a morning meeting that is held twice a month to discuss theoretical approaches to medical education that supports the practices and research of the institutional center for educational support. The meetings are similar to the traditional journal clubs for undergraduate and residency programs, except that they are specially geared towards medical education. The success of these meetings lies in their capacity to develop faculty, in a cozy and informal environment. Based on reports of improvements in teaching practices around the world due to similar initiatives, we encourage other Brazilian medical schools to adopt of medical education journal clubs, and to share their results.

A importância da saúde mental nas práticas em saúde fez surgir nas escolas médicas a demanda por uma capacitação aprimorada dos graduandos nessa área. Este artigo tem como principal objetivo descrever a experiência da Liga Acadêmica de Saúde Mental (LISM) da Faculdade de Medicina de Botucatu - Unesp como atividade de ensino extracurricular. Relatam-se sua formação, objetivos, dificuldades iniciais, composição, modo de funcionamento e principais atividades desenvolvidas desde 2004. Acredita-se que a LISM tem tido êxito em complementar a formação médica nessa área, pois atua na prevenção e promoção da saúde, incluindo atividades voltadas à saúde mental dos estudantes. Discute-se também criticamente a atuação das Ligas de Saúde Mental nas escolas médicas, incluindo alguns riscos, como o de favorecer a especialização precoce em Psiquiatria, de aumentar a falsa dicotomia entre saúde física e mental ou simplesmente reproduzir atividades acadêmicas oficiais. Tais atividades extracurriculares não devem desmobilizar a comunidade acadêmica na luta pela valorização, integração e inserção sistemática e longitudinal de temas de saúde mental no currículo oficial dos cursos de graduação, indispensáveis à adequada formação dos médicos generalistas.

Considering the importance of mental health for general health practice, there has been an increasing demand by medical students for better training in this area. The main aim of the present article is to describe the experience of the Students' League on Mental Health (LISM) at the Botucatu Medical School, São Paulo State University (UNESP), in teaching mental health contents as extracurricular activities. The article describes the initial difficulties in the league's organization, as well as its structure and functioning and the main activities developed by the students since it was founded in 2004. The LISM has been successful in complementing medical training in this area, since it encompasses preventive and health promotion activities, including those targeting the students' own mental health. Some concerns regarding the role of students' leagues in mental health in medical schools are also discussed, including the risks of favoring early specialization in psychiatry, fueling the false dichotomy between physical and mental health, and simply reproducing regular curricular activities. These leagues should not discourage the academic community from struggling to value, integrate, and systematically include mental health themes in the official curriculum, which are indispensable to proper training of general practitioners.

CONTEXTO: O transtorno obsessivo-compulsivo (TOC) foi considerado pela Organização Mundial da Saúde como a 10ª causa de anos vividos com incapacidade, mas pesquisas sobre qualidade de vida (QV) nesse transtorno ainda são relativamente raras. OBJETIVOS: O objetivo deste trabalho foi fazer uma revisão convencional da literatura a respeito de estudos sobre qualidade de vida no TOC, publicados em português e inglês. MÉTODOS: A busca de artigos foi feita pelos sistemas MEDLINE, PsicoInfo e LILACS, cobrindo o período de 1980 a 2007, utilizando-se como palavras-chave: qualidade de vida, transtorno obsessivo-compulsivo, prejuízo funcional e incapacidade. RESULTADOS: Estudos populacionais apresentam indicadores indiretos de comprometimento na QV em pessoas com TOC, como mais desemprego, menor renda e menor índice de união conjugal estável, além de taxas relativamente altas de ideação e tentativas de suicídio. Estudos clínicos encontraram mais comprometimento na QV no TOC em comparação com algumas doenças clínicas crônicas, outros transtornos de ansiedade, quadros depressivos e mesmo esquizofrenia, em alguns aspectos. CONCLUSÃO: O impacto negativo do TOC sobre a QV de seus portadores pode ser grande, afetando vários domínios da vida e repercutindo também nos familiares. A reabilitação psicossocial deve ser vista como parte integrante essencial do tratamento e é preciso aumentar a conscientização da população sobre o problema, assim como o acesso a tratamentos adequados, para minimizar tal impacto, por vezes devastador.

BACKGROUND: Obsessive-compulsive disorder (OCD) has been considered by the World Health Organization as the 10th leading cause of years lived with disability, but studies on quality of life (QOL) in OCD are still scant. OBJECTIVES: A literature review of studies on quality of life in OCD. METHOD: A MEDLINE, Psychoinfo and LILACS literature review from 1980 to 2007 was conducted, using the following key-words: obsessive-compulsive disorder, quality of life, functional impairment and disability. RESULTS: Population-based studies have shown indirect indicators of QOL impairment in OCD sufferers, such as more unemployment, lower income, lower number of marriages, besides considerably high rates of suicidal thoughts and attempts. Clinical studies found more QOL impairment in OCD patients compared to individuals with some chronic physical conditions, other anxiety disorders, depressive disorders and even schizophrenia, in some aspects. CONCLUSION: The negative impact of OCD on QOL can be severe, affecting several life domains and affecting family members as well. Psychosocial rehabilitation should be an essential part of OCD treatment and it is necessary to increase both public awareness of this disorder and the availability of appropriate treatments to minimize such impact, which can be devastating.

INTRODUÇÃO: A interconsulta psiquiátrica (ICP) pode ser considerada um instrumento de pesquisa, ensino e assistência, que beneficia paciente, equipe de saúde e instituição. A sua utilização no Brasil é ainda pequena. Sabe-se que 30% a 50% dos pacientes internados em hospital geral (HG) podem apresentar uma patologia psiquiátrica, mas que apenas 1% a 12% destes são encaminhados para avaliação. Pretendeu-se com este trabalho caracterizar os transtornos psiquiátricos no HG; identificar as diferenças entre pacientes encaminhados e um grupo controle. MÉTODO: Realizou-se um estudo caso-controle de 141 pacientes (47 casos e 94 controles), analisando variáveis sócio-demográficas, clínicas, informação quanto à doença, procedimentos diagnósticos e terapêuticos; e relação pacientes- equipe de saúde. Utilizou-se o Self Report Questionnaire (SRQ), o CAGE e o Brief Psychiatric Rating Scale (BPRS), além de um questionário elaborado para o estudo. RESULTADOS: Observou-se que alterações de comportamento com manifestações de exaltação e depressivas motivaram os pedidos de ICP; 95,8% dos casos e 27,7% dos controles receberam um diagnóstico psiquiátrico; transtornos orgânicos e alcoolismo prevaleceram no grupo I (casos), e os transtornos de ansiedade e transtornos depressivos no grupo II (controles). Pacientes do grupo controle estavam mais bem informados e estabeleceram relacionamentos mais adequados com a equipe de saúde que os casos. A regressão logística mostrou que pacientes com CAGE positivo têm 12,85 vezes mais chances de serem encaminhados para ICP; os sem ocupação 2,44 vezes; ou seja, alcoolismo e alterações cognitivas foram os principais responsáveis pelo encaminhamento à ICP. DISCUSSÃO: O uso de instrumentos de triagem, como o SRQ e o CAGE, pode ser útil na formação do clínico geral. Pesquisas adicionais deverão ser feitas para avaliar de que maneira o serviço de ICP implantado melhorou as habilidades de diagnóstico e tratamento de transtornos psiquiátricos neste hospital geral universitário.

INTRODUCTION: Psychiatric consultation (PC) has been considered an efficient tool to develop research, to track and to give assistance benefiting patients, health professionals and the institution. However, it has not been much used in Brazil. Although 30 to 50% of general hospital (GH) inpatients may present a psychiatric disorder, only 1 to 12% of them are referred to assessment. The aims of this study were: to assess mental disorders in a GH; to identify which of these patients are sent to psychiatric care; to verify alleged reasons for referral to psychiatric consultation, and to examine the relationship between PC and psychiatric learning (during medical school and residence). METHODS: A case-control patient study was conducted (47 cases and 94 controls) to analyze in detail the following variables: socio-demographic; clinical; degree of information (about the disease and diagnostic/therapeutic procedures), and relationship between patient and health team. Psychiatric diagnoses were made according to the ICD-10 criteria. The Self Report Questionnaire (SRQ), the CAGE and Brief Psychiatric Rating Scale (BPRS) were used as well as a specifically designed questionnaire to collect clinical and demographic data. RESULTS: Behavioral alterations, either of elation or of depression, were the main for requesting a PC ; 95.8% of the cases and 27.7% of the controls had a mental disorder. Organic mental disorders and alcohol-related disorders were the most frequent diagnoses in group I (cases), while anxiety, depressive and alcohol-related disorders were predominant in group II (controls). Control group patients were better informed and more able to establish an appropriate relationship with the health team than case patients. The logistic regression showed CAGE-positive patients having 12.85 times greater risk of being referred to PC, followed by unemployed patients (2.44 times more PC referrals). DISCUSSION: The SRQ and CAGE were quite useful in the screening of possible patients and might be important for medical students to learn and use as generalists. Further research is needed to verify if and how the newly-established service will improve the diagnostic and treatment skills of our students.

Objetivos: A proporção de ocorrência de alcoolismo em homens e mulheres tem uma variação de 14:1 até 2:1, mostrando a necessidade de estudos específicos para a população feminina. O objetivo deste estudo foi analisar o perfil e a evolução de alcoolistas, segundo gênero e gravidade da dependência. Métodos: Realizou-se um estudo longitudinal retrospectivo de 114 homens e 57 mulheres alcoolistas (CID-10), inscritos no ambulatório da Faculdade de Medicina de Botucatu, no período de 1990-1994 e avaliados até julho de 1997. Utilizou-se um questionário semi-estruturado, e, para avaliação da gravidade do alcoolismo, o "Short Alcohol Dependence Data". Resultados/Conclusões: Os principais resultados mostraram que a estrutura familiar estava comprometida com: relacionamento difícil para 55,6% das mulheres e 65,7% dos homens; violência familiar em 74,1% das mulheres e 61,1% dos homens. As mulheres iniciaram a ingestão mais tarde que os homens (p=0,01), em geral com seus cônjuges (p=0,00). Não houve diferença de evolução no tratamento entre os gêneros. Os principais fatores associados à melhor resposta ao tratamento, independentemente do sexo, foram: nível de gravidade de dependência do álcool (dependentes leves e moderados apresentaram 5,59 vezes mais chances de melhorar do que os dependentes graves); ser praticante de alguma religião (2,3 vezes mais chances de melhora do que os pacientes que não eram praticantes); e tempo de seguimento no programa, negativamente correlacionado a chances de melhora (0,68 vezes menos chances de melhora que aqueles que permaneceram por tempo menor em tratamento).

Objective: Alcoholic men-women ratio has ranged from 14:1 to 2:1, suggesting that female alcoholism should be further studied. The purpose of the current study was to compare alcohol dependence severity and treatment outcome between alcoholic men and women. Methods: In this longitudinal study, 114 male and 57 female alcoholics (ICD-10 criteria), who started treatment between 1990 and 1994 at the Botucatu Medical School Outpatient Clinic, were retrospectively and prospectively assessed up to July 1997. Semi-structured interviews were conducted and the severity of alcohol dependence was assessed (Short Alcohol Dependence Data -- SADD). Results/Conclusions: The results showed poorly structured families, 55.6% of women and 65.7% of men reported relationship problems and 74.1% of women and 61.1% of men reported domestic violence. When compared to men, women started abusing alcohol later in life (p=0.01) and, usually, with their husbands (p=0.00). The course of treatment did not differ between genders. Regardless of gender, the main factors associated with a better response to treatment were: degree of alcohol dependence severity (mildly and moderately dependent users had 5.59-fold better chances of improvement than those severely dependent), religious practice (2.3-fold better chances of improving) and follow-up length, which was negatively associated with chance of improvement (0.68-fold less chance of improvement than those who remained under shorter treatment).
