Abstract Objective To develop the second evidence-based Brazilian Society of Rheumatology consensus for diagnosis and treatment of lupus nephritis (LN). Methods Two methodologists and 20 rheumatologists from Lupus Comittee of Brazilian Society of Rheumatology participate in the development of this guideline. Fourteen PICO questions were defined and a systematic review was performed. Eligible randomized controlled trials were analyzed regarding complete renal remission, partial renal remission, serum creatinine, proteinuria, serum creatinine doubling, progression to end-stage renal disease, renal relapse, and severe adverse events (infections and mortality). The Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach was used to develop these recommendations. Recommendations required ≥82% of agreement among the voting members and were classified as strongly in favor, weakly in favor, conditional, weakly against or strongly against a particular intervention. Other aspects of LN management (diagnosis, general principles of treatment, treatment of comorbidities and refractory cases) were evaluated through literature review and expert opinion. Results All SLE patients should undergo creatinine and urinalysis tests to assess renal involvement. Kidney biopsy is considered the gold standard for diagnosing LN but, if it is not available or there is a contraindication to the procedure, therapeutic decisions should be based on clinical and laboratory parameters. Fourteen recommendations were developed. Target Renal response (TRR) was defined as improvement or maintenance of renal function (±10% at baseline of treatment) combined with a decrease in 24-h proteinuria or 24-h UPCR of 25% at 3 months, a decrease of 50% at 6 months, and proteinuria < 0.8 g/24 h at 12 months. Hydroxychloroquine should be prescribed to all SLE patients, except in cases of contraindication. Glucocorticoids should be used at the lowest dose and for the minimal necessary period. In class III or IV (±V), mycophenolate (MMF), cyclophosphamide, MMF plus tacrolimus (TAC), MMF plus belimumab or TAC can be used as induction therapy. For maintenance therapy, MMF or azathioprine (AZA) are the first choice and TAC or cyclosporin or leflunomide can be used in patients who cannot use MMF or AZA. Rituximab can be prescribed in cases of refractory disease. In cases of failure in achieving TRR, it is important to assess adherence, immunosuppressant dosage, adjuvant therapy, comorbidities, and consider biopsy/rebiopsy. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Introduction The deficiency of ADA2 (DADA2) is a rare autoinflammatory disease provoked by mutations in the ADA2 gene inherited in a recessive fashion. Up to this moment there is no consensus for the treatment of DADA2 and anti-TNF is the therapy of choice for chronic management whereas bone marrow transplantation is considered for refractory or severe phenotypes. Data from Brazil is scarce and this multicentric study reports 18 patients with DADA2 from Brazil. Patients and methods This is a multicentric study proposed by the Center for Rare and Immunological Disorders of the Hospital 9 de Julho - DASA, São Paulo - Brazil. Patients of any age with a confirmed diagnosis of DADA2 were eligible for this project and data on clinical, laboratory, genetics and treatment were collected. Results Eighteen patients from 10 different centers are reported here. All patients had disease onset at the pediatric age (median of 5 years) and most of them from the state of São Paulo. Vasculopathy with recurrent stroke was the most common phenotype but atypical phenotypes compatible with ALPS-like and Common Variable Immunodeficiency (CVID) was also found. All patients carried pathogenic mutations in the ADA2 gene. Acute management of vasculitis was not satisfactory with steroids in many patients and all those who used anti-TNF had favorable responses. Conclusion The low number of patients diagnosed with DADA2 in Brazil reinforces the need for disease awareness for this condition. Trata-se de estudo qualitativo, de natureza exploratório-descritiva, do tipo documental, que analisou os documentos públicos referentes ao planejamento do governo na especificidade do setor de saúde, o que inclui o Plano Plurianual, o Plano Estadual de Saúde e o Plano Estratégico da SES. Os achados do estudo mostram que, embora algumas ações/atividades relacionadas às diretrizes da portaria estivessem contempladas nos documentos pesquisados, ainda não foram suficientes para o estabelecimento de uma rede de atenção regionalizada de qualidade e para o fortalecimento da Atenção Primária à Saúde (APS) com capacidade de ser coordenadora do cuidado e ordenadora da rede no estado de Mato Grosso.

The findings of the study show that, although some actions/activities related to the guidelines of the Ordinance were included in the researched documents, they were not yet sufficient for establishing a quality regionalized care network and for strengthening the Primary Health Care (PHC) with the capacity to coordinate care and order the network in the state of Mato Grosso.

OBJECTIVE: To assess the possible factors that influence sleep quality in adolescents with and without chronic immunosuppressive conditions quarantined during the coronavirus disease 2019 (COVID-19) pandemic. METHODS: This cross-sectional study included 305 adolescents with chronic immunocompromised conditions and 82 healthy adolescents. Online surveys were completed, which included questions on socio-demographic data and self-rated healthcare routine during COVID-19 quarantine and the following validated questionnaires: the Pittsburgh Sleep Quality Index (PSQI), Pediatric Quality of Life Inventory 4.0 (PedsQL4.0), and Pediatric Outcome Data Collection Instrument (PODCI). RESULTS: The median current age [14 (10-18) vs. 15 (10-18) years, p=0.847] and frequency of female sex (62% vs. 58%, p=0.571) were similar in adolescents with chronic conditions compared with healthy adolescents. The frequency of poor sleep quality was similar in both groups (38% vs. 48%, p=0.118). Logistic regression analysis, including both healthy adolescents and adolescents with chronic conditions (n=387), demonstrated that self-reported increase in screen time (odds ratio [OR] 3.0; 95% confidence interval [CI] 1.3-6.8; p=0.008) and intrafamilial violence report (OR 2.1; 95% CI 1.2-3.5; p=0.008) were independently associated with poor sleep quality in these adolescents. However, the PODCI global function score was associated with a lower OR for poor sleep quality (OR 0.97; 95% CI 0.94-0.99; p=0.001). Further logistic regression, including only adolescents with chronic conditions (n=305), demonstrated that self-reported increase in screen time (OR 3.1; 95% CI 1.4-6.8; p=0.006) and intrafamilial violence report (OR 2.0; 95% CI 1.2-3.4; p=0.011) remained independently associated with poor quality of sleep, whereas a lower PODCI global function score was associated with a lower OR for sleep quality (OR 0.96; 95% CI 0.94-0.98; p<0.001). CONCLUSION: Self-reported increases in screen time and intrafamilial violence report impacted sleep quality in both healthy adolescents and those with chronic conditions. Decreased health-related quality of life was observed in adolescents with poor sleep quality.

OBJECTIVES: To prospectively evaluate demographic, anthropometric and health-related quality of life (HRQoL) in pediatric patients with laboratory-confirmed coronavirus disease 2019 (COVID-19) METHODS: This was a longitudinal observational study of surviving pediatric post-COVID-19 patients (n=53) and pediatric subjects without laboratory-confirmed COVID-19 included as controls (n=52) was performed. Twenty-three of 53 (43%) patients reported at least one persistent symptom at the longitudinal follow-up visit and 12/53 (23%) had long COVID-19, with at least one symptom lasting for >12 weeks. The most frequently reported symptoms at the longitudinal follow-up visit were headache (19%), severe recurrent headache (9%), tiredness (9%), dyspnea (8%), and concentration difficulty (4%). At the longitudinal follow-up visit, the frequencies of anemia (11% versus 0%, p=0.030), lymphopenia (42% versus 18%, p=0.020), C-reactive protein level of >30 mg/L (35% versus 0%, p=0.0001), and D-dimer level of >1000 ng/mL (43% versus 6%, p=0.0004) significantly reduced compared with baseline values. Chest X-ray abnormalities (11% versus 2%, p=0.178) and cardiac alterations on echocardiogram (33% versus 22%, p=0.462) were similar at both visits. Comparison of characteristic data between patients with COVID-19 at the longitudinal follow-up visit and controls showed similar age (p=0.962), proportion of male sex (p=0.907), ethnicity (p=0.566), family minimum monthly wage (p=0.664), body mass index (p=0.601), and pediatric pre-existing chronic conditions (p=1.000). The Pediatric Quality of Live Inventory 4.0 scores, median physical score (69 [0-100] versus 81 [34-100], p=0.012), and school score (60 [15-100] versus 70 [15-95], p=0.028) were significantly lower in pediatric patients with COVID-19 at the longitudinal follow-up visit than in controls. CONCLUSIONS: Pediatric patients with COVID-19 showed a longitudinal impact on HRQoL parameters, particularly in physical/school domains, reinforcing the need for a prospective multidisciplinary approach for these patients. These data highlight the importance of closer monitoring of children and adolescents by the clinical team after COVID-19.

ABSTRACT The present study proposes to characterize the parasites isolated during the initial phase of production in fish farms located in Mato Grosso do Sul in the central-western region of Brazil, using histopathology analysis and molecular techniques. A total of 340 hybrid surubim fish (Pseudoplatystoma reticulatum × P. corruscans) from four farms were examined during the co-feeding phase. Histopathology analysis showed that 10.9% (n = 37) of the fish were infected with parasites. Branchitis, lifting epithelium, hypertrophy of epithelial cells, lamellar fusion, aneurisms and infection in the bone tissue of the gill filament was observed. The parasite species was determined by amplification of the 18S rRNA gene followed by sequencing. The phylogenetic analysis of nucleotide sequences indicates a close relationship (99.6%) with Henneguya pseudoplatystoma reported to be infecting the hybrid Pseudoplatystoma. This study demonstrates the occurrence of H. pseudoplatystoma in hybrid surubim (P. reticulatum × P. corruscans) during the co-feeding phase in fish farms in Mato Grosso do Sul. Also, molecular techniques provide a faster and sensitive method to identify fish parasites, and may assist in the development of new management techniques aimed at improving the sanitary conditions contributing to the reduction of mortality rates in these animals.

Resumo A busca por melhores condições de vida tem levado os moradores da região semiárida brasileira a optarem por plantar culturas forrageiras, provocando a diminuição gradativa da vegetação nativa (Caatinga) desta região. A substituição da Caatinga pela palma, por exemplo, tem efeito pouco estudado, sobretudo no tocante as propriedades físico-hídricas do solo. O objetivo deste estudo foi comparar as características físico-hídricas de um Neossolo Litólico em duas áreas com diferentes condições de cobertura vegetal: uma cultivada com Palma forrageira (O. ficus-indica) e a outra com Caatinga. Observou-se que as diferenças na estrutura do solo, especialmente na porosidade, entre o solo natural e cultivado, controlam os processos hidrodinâmicos, resultando em alterações nas curvas de retenção de água no solo e de condutividade hidráulica. O solo natural apresentou uma baixa condutividade hidráulica, quando comparada a determinada para o solo cultivado. Esse aumento deve-se provavelmente ao manejo do solo, necessários para o cultivo da palma forrageira. A estrutura do solo natural, caracterizada por valores relativamente baixos de condutividade hidráulica saturada, apresentam uma infiltrabilidade que favorece o escoamento superficial. A antropização do solo na área em estudo promoveu alterações nos atributos físicos dos solos, diminuindo a densidade e aumentando a porosidade. Consequentemente há o aumento da infiltração de água no solo e a redução do escoamento superficial no solo cultivado, corroborando com resultados obtidos em estudos anteriores.
Abstract The search for better living conditions has led the residents of the Brazilian semi-arid region to plant forage crops, leading to a gradual decrease in the native vegetation (Caatinga) of this region. The effects caused by the replacement of Caatinga with palm, for example, have been little studied, especially with regard to the physical and hydraulic properties of the soil. The objective of this study was to compare the physical-hydraulic characteristics of a litholic neosol in two areas having different vegetation cover: one area cultivated with forage palm (O. ficus-indica) and the other covered by native Caatinga. Differences in soil structure, especially in porosity, between the natural and cultivated soils were observed to control the hydrodynamic processes, resulting in changes in water retention curves and hydraulic conductivity. Natural soil presents low values of hydraulic conductivity when compared to those of cultivated soil. This increase is probably due to soil management required for forage palm cultivation. The natural soil structure, characterized by relatively low saturated hydraulic conductivity values, presents an infiltrability that favors surface runoff. Human activities in the study area have promoted changes in the soil’s physical attributes, decreasing density and increasing porosity. Consequently, there is an increase in water infiltration into the soil and a reduction of runoff in cultivated areas, confirming results obtained in previous studies.

Abstract Purpose: To describe the anatomy of the swine urinary system using computed tomography and to discuss the role of this animal as an experimental model for urological procedures. Methods: Three male Landrace pigs underwent computed tomography and the anatomy of the urinary system and renal circulation was analyzed and described. Results: In all animals, 2 kidneys, 2 ureters and one bladder were identified. Each kidney presented a single renal artery vascularization, with a mean diameter on the right of 4.45 and 5.31 mm on the left (p < 0.0001) and single renal vein drainage, with a mean diameter on the right of 5.78 and 5.82 mm on the left (p = 0.0336). The average renal length was 9.85 cm on the right and 10.30 cm on the left (p < 0.0001). The average renal volume was 113.70 cm3 on the right and 109.70 cm3 on the left (p < 0.0001). The average length of the ureter was 19.78 cm on the right and 22.08 cm on the left (p < 0.0001). The average bladder volume was 423.70 cm3. Conclusions: The data obtained show similarities with human anatomy, suggesting the viability of the swine model for planning preclinical trials, basic research, refinement in experimental surgery and surgical training for urological procedures.

RESUMO Sistemas de governança de água devem ser eficientes em estabelecer políticas capazes de nortear a tomada de decisão. Os arcabouços precisam ponderar a complexidade intrínseca das interações interescala e multinível. Aspectos como descentralização e comprometimento entre as partes interessadas são pilares a serem considerados. Existem desafios a serem vencidos em bacias compartilhadas (nacionais ou internacionais) uma vez que há a necessidade de gerenciar a água em diferentes escalas e múltiplos níveis. Este estudo tem como objetivo analisar o grau de implementação da Política Nacional de Recursos Hídricos em uma bacia federal compartilhada (a Bacia Hidrográfica do Rio São Francisco) no seu nível macro e no nível micro (as 34 sub-bacias que a compõe). São analisadas as interrelações multiníveis na implementação da política hídrica como balizador da governança da água. Para tal, considera-se o Princípio 2 da Governança da Água da OECD (Gerenciar a água em escalas apropriadas) como fundamento para essa análise. Uma adaptação da metodologia do Quadro de Indicadores da Governança da Água é usada para classificar as 34 sub-bacias em distintos estágios de implementação da política hídrica. Os resultados apontam para deficiências no processo de implementação da política em sub-bacias. Há necessidade de melhores interações nos níveis federal, estadual e das bacias hidrográficas a fim de fortalecer o sistema de governança.
ABSTRACT Water governance systems must be effective in establishing policies capable of guiding decision making. Their framework needs to weigh the intrinsic complexity of inter-scale and multi-level interactions. Aspects such as decentralization and stakeholder engagement are pillars to be considered. In shared river basins (international or national), there are challenges to overcome since there is a need for managing water at different scales and multiple levels. This study aims to analyse the degree of implementation of the National Water Resources Policy in a shared federal basin in Brazil (the São Francisco River Basin) considering the basin as a whole (macro level) and its micro level (the 34 sub-basins that compose it). Multi-level interrelationships in the implementation of water policy are analysed. To this end, OECD Water Governance Principle 2 (“Manage water at the appropriate scales”) is considered as the basis for this analysis. An adaptation of the OECD Water Governance Indicator Framework methodology is used to classify the 34 sub-basins at different stages of water policy implementation. The outcomes indicate deficiencies in the process of water policy implementation in sub-basins. There is a need for better interactions at the federal, state and river basin levels to strengthen the governance system.

Resumo: Este trabalho analisa o perfil de estudantes de graduação em turismo de duas universidades públicas do sul do Brasil, com o objetivo de compreender as motivações que os levaram a escolher uma carreira no turismo e como eles se integram ao tecido social e se apropriam da universidade. Trata-se de um estudo descritivo e predominantemente quantitativo, com foco em estudantes de graduação inscritos em diferentes tipos de carreiras: diploma de bacharelado e tecnólogo em turismo. Os dados primários sobre os alunos foram coletados por meio de um questionário estruturado, com perguntas relacionadas ao perfil pessoal dos alunos, sua rota de entrada nas universidades, suas motivações para estudar turismo nessas universidades e seu senso de pertencer à universidade e à cidade em geral. Os dados foram analisados por meio da análise fatorial exploratória/confirmatória (E-CFA) para testar os fatores trazidos da literatura, uma análise do modelo de equações estruturais para avaliar as relações entre os fatores e as comparações médias para identificar diferenças significativas entre universidades e grupos. Os resultados confirmaram que as motivações para escolher a graduação em turismo são influenciadas pelas opiniões de outras pessoas e pela conveniência na universidade; eles também confirmaram que as motivações para a escolha do turismo como carreira influenciam a integração social e o sentimento de pertencimento acadêmico.
Abstract: This work analyzes the profiles of undergraduate students of tourism at two public universities in the south of Brazil, aiming to understand the motivations that led them to choose a career in tourism, and how they integrate themselves into the social fabric and appropriate the university as their own. It is a descriptive and predominantly quantitative study, focusing on undergraduate students enrolled in different types of career paths: a bachelor’s degree and a technologist degree (similar to an associate degree) in tourism. Primary data on the students were collected through a structured questionnaire, with questions related to the students’ personal profiles, their route of entry to the universities, their motivations for studying tourism at those universities, and their sense of belonging to the university and to the city in general. The data were analyzed using Exploratory/Confirmatory Factor Analysis (E-CFA) to test factors brought from the literature, a Structural Equation Model Analysis to assess the relationships between the factors, and mean comparisons to identify significant differences between universities and groups. The results confirmed that the motivations for choosing to major in tourism are influenced by the opinions of others and by the convenience at the university; they also confirmed that the motivations for choosing tourism as a career influence social integration and the sense of academic belongingness.
Resumen: Este trabajo analiza el perfil de estudiantes de graduación en turismo en turismo de las universidades publicas del sur de Brasil, con el objetivo de comprender las motivaciones que los llevaron a elegir una carrera de turismo y, como ellos se integran al tejido social y se apropian de la universidad. Se trata de un estudio descriptivo y predominantemente cuantitativo, con foco en estudiantes de graduación inscriptos en diferentes tipos de carreras: diploma de licenciatura y tecnólogo en turismo. Los datos primarios sobre los alumnos fueron colectados por medio de un cuestionario estructurado, con preguntas relacionadas al perfil personal de los alumnos, su ruta de entrada en las universidades, sus motivaciones para estudiar turismo en esas universidades y su censo de pertenecer a la universidad y a la ciudad en general. Los datos fueron analizados por medio del análisis factorial exploratorio / confirmatorio (E-CFA) para atestiguar los factores traídos de la literatura, un análisis del modelo de ecuaciones estructurales para evaluar las relaciones entre los factores y los promedios comparativos para identificar diferencias significativas entre universidades y grupos. Los resultados confirmaron que las motivaciones para elegir la graduación en turismo son influidas por las opiniones de otras personas y por la conveniencia en la universidad; ellos también confirmaron que las motivaciones para la elección en turismo como carrera influyen a la integración social y el sentimiento de perteneciente académico.

RESUMO Introdução: Modelos de cuidado contínuo baseiam recentes estratégias em HIV, infecções sexualmente transmissíveis e hepatite C (HCV). Métodos: Desenvolveram-se modelos de contínuo do cuidado em HIV, HCV e sífilis congênita incluindo todas as etapas da atenção, desde a promoção e a prevenção até o controle clínico/cura. O modelo baseou a intervenção QualiRede, desenvolvida em parceria entre universidade e Sistema Único de Saúde (SUS), direcionada a gestores e demais profissionais de 6 regiões de saúde prioritárias em São Paulo e Santa Catarina. Selecionaram-se indicadores para cada etapa do contínuo do cuidado, provenientes dos sistemas de informação do SUS e dos questionários de avaliação de processo Qualiaids e QualiAB. Os indicadores formaram a base técnica de duas oficinas com profissionais e gestores de cada região: a primeira para identificar problemas e formar um Grupo Técnico Regional; e a segunda para construir planos de ação e metas a fim de melhorar o desempenho regional. Resultados: Os indicadores estão disponíveis no website www.qualirede.org. As oficinas ocorreram em quatro regiões de São Paulo (Alto Tietê, Baixada Santista, Grande ABC e Registro) e uma região de Santa Catarina (Foz do Rio Itajaí), resultando em planos regionais em São Paulo, mas não em Santa Catarina. Observou-se domínio limitado dos novos protocolos para HIV e HCV e uso incipiente de indicadores na rotina dos serviços. Conclusão: Melhorar o desempenho do contínuo do cuidado exige apropriação dos indicadores de desempenho e coordenação integrada dos fluxos de atenção em todos os níveis de gestão.
ABSTRACT Introduction: Care continuum models have supported recent strategies against sexually transmitted diseases, such as HIV and Hepatitis C (HCV). Methods: HIV, HCV, and congenital syphilis care continuum models were developed, including all stages of care, from promotion/prevention to clinical control/cure. The models supported the intervention QualiRede, developed by a University-Brazilian National Health System (SUS) partnership focused on managers and other professionals from six priority health regions in São Paulo and Santa Catarina. Indicators were selected for each stage of the care continuum from the SUS information systems and from the Qualiaids and QualiAB facility’s process evaluation questionnaires. The indicators acted as the technical basis of two workshops with professionals and managers in each region: the first one to identify problems and to create a Regional Technical Group; and the second one to design action plans for improving regional performance. Results: The indicators are available at www.qualirede.org. The workshops took place in the regions of Alto Tietê, Baixada Santista, Grande ABC, and Registro (São Paulo) and of Foz do Rio Itajaí (Santa Catarina), which resulted in regional action plans in São Paulo, but not in Santa Catarina. A lack of awareness was observed regarding the new HIV and HCV protocols, as well as an incipient use of indicators in routine practices. Conclusion: Improving the performance of the care continuum requires appropriation of performance indicators and coordination of care flows at local, regional, and state levels of management.
