Resumen Este estudio tuvo como objetivo determinar las concentraciones salivales de interleucina (IL)-1β, IL-2, IL-10, IL-23, factor de crecimiento transformante (TGF)-β y factor de necrosis tumoral (TNF)-α, nitrato (subproducto de la oxidación del óxido nítrico) y cortisol en pacientes con liquen plano oral (OLP). En este estudio transversal se incluyeron veinte pacientes diagnosticados con OLP y 20 voluntarios sanos (HV) del mismo sexo. Saliva entera no estimulada Se recolectó por la mañana, se determinaron las concentraciones de citocinas y cortisol en saliva mediante ensayo inmunoabsorbente ligado a enzimas (ELISA), se determinó nitrato mediante un analizador de óxido nítrico, se encontraron concentraciones salivales mayores de IL-2 (p<0,003), IL- 23 (p<0,04) y TGF-β (p=0,05) en pacientes con OLP en comparación con HV. No se encontraron diferencias significativas en los niveles salivales de TNF-α, IL-1β e IL-10. Las concentraciones de nitrato fueron marcadamente aumentó en pacientes con OLP (1227,0 ± 738,8 µM/mg de proteína total), en comparación con HV (261,6 ± 166,8 µM/mg; p<0,0001). Los niveles de cortisol salival también fueron más altos en los pacientes con OLP que en los controles (2,79 ± 1,39 vs. 1,94 ±1,21 ng/mg; p<0,048). Los niveles de óxido nítrico en saliva aumentaron notablemente en pacientes con OLP, lo que sugiere una relación de esta molécula con la muerte celular y el daño tisular observado en las lesiones de OLP.

Abstract This study aimed to determine salivary concentrations of interleukin (IL)-1β, IL-2, IL-10, IL- 23, transforming growth factor (TGF)-β, tumor necrosis factor (TNF)-α, nitrate (a by-product of nitric oxide oxidation), and cortisol in patients with oral lichen planus (OLP). Twenty patients diagnosed with OLP and 20 sex-matched healthy volunteers (HV) were included in this cross-sectional study. Unstimulated whole saliva was collected in the morning. Salivary cytokine and cortisol concentrations were determined by enzyme-linked immunosorbent assays (ELISA). Nitrate was measured in a nitric oxide analyzer. We found higher salivary concentrations of IL-2 (p<0.003), IL-23 ( p<0.04), and TGF-β (p=0.05) in patients with OLP compared to HV. No significant differences were found in salivary levels of TNF-α, IL-1β, or IL-10. Nitrate concentrations were markedly increased in OLP patients (1,227.0 ± 738.8 µM/mg total protein) when compared to HV (261.6 ± 166.8 µM/mg; p<0.0001). Salivary cortisol levels were also higher in OLP patients (2.79 ± 1.39 vs. 1.94 ±1.21 ng/mg; p<0.048). The markedly increased salivary levels of nitric oxide in patients with OLP suggest a relationship of this molecule with the cell death and tissue damage observed in these lesions.

ABSTRACT BACKGROUND: Nutritional status and growth curves can affect cognitive development, increase the risk of infection, and contribute to the development of chronic diseases. Its etiology is related to food, socioeconomic, and maternal conditions. However, to date, no data on these parameters exist in the state of Goiás, Brazil. OBJECTIVE: To compare the nutritional status and growth curves of children and adolescents in the city of Goiânia, Goiás, Brazil. DESIGN AND SETTING: This was a cross-sectional study. A total of 529 individuals were recruited from a primary health center in the municipality. METHODS: To assess nutritional status, the sample was divided into three categories: 3–4, 5–10, and 11–19 years, with z-score classification considering body mass index for age. The classification of growth curves was performed considering the median height values for age, assuming two references: (a) young Brazilian population and (b) one recommended for international use. The independent sample T-test was used to compare anthropometric variables. RESULTS: The results showed that the classification of eutrophics represents a predominant percentage between both sexes (men: 03–04 = 55.4%; 05–10 = 57.6%; 11–19 = 53.5 % and women: 03–04 = 53.5%; 05–10 = 63.9%; 11–19 = 56.9%), and growth curves showed differences in specific periods in both sexes. CONCLUSIONS: It can be concluded that children and adolescents from the city of Goiânia present as predominance the eutrophic nutritional status, followed by the risk of overweight, underweight, obesity, and malnutrition of both sexes. ABSTRACT The work was intended to evaluate the effect of different organic compounds originating from the mixture of agroindustrial and urban wastes, as well as their doses in the physiology and production of cowpea. The experiment was conducted under a randomized block design, in a 4 x 5 + 2 factorial arrangement, with four organic fertilizers (3 composed obtained from of sewage sludge, green coconut shell, carnauba butt; and sewage sludge biochar), five doses (20, 40, 60, 80 and 100 t ha-1) and two additional treatments (without fertilization and mineral fertilization). We evaluated the leaf gas exchange in three phenological stages and the productive aspects. Regarding the physiological variables, the compounds with carnauba butt and green coconut shell promoted the best results and throughout the crop cycle, the gas exchange decreased. The productive aspects were increased by about 60%, especially due to the treatments with organic compounds. The PCA can confirm the univariate results, with emphasis on compounds with green coconut shell and carnauba butt. The high percentage of explanation found in axis 1 (60.49%) may be associated with the fact that the gas exchange and production variables are highly responsive to fertilization. The treatment with biochar caused a reduction in both in the physiological and in the productive aspects. ABSTRACT Stress caused by defoliation can compromise the physiological quality of soybean seeds. However, there is evidence that the application of nitrogen (N) can be an alternative to reduce the qualitative losses in seeds caused by defoliating insects. The objective of this study was to evaluate the effectiveness of topdressing N fertilization in improving the physiological quality of soybean seeds from plants subjected to artificial defoliation. The experimental design was a randomized block arranged in a 2×4 factorial scheme with four replicates. The treatments consisted of two levels of defoliation (33 and 66%) and four doses of N (0, 50, 100, and 150 kg ha-1), using urea (45% N), applied two days after defoliation. Artificial defoliation was performed during the R3 stage by removing one (33%) and two leaflets (66%) from each trifoliate leaf under field conditions. At stage R5.1, leaf area was quantified, and after harvesting, germination, emergence, length, total dry mass, electrical conductivity, total protein content, mass of 1000 seeds, and tetrazolium (vigor and viability) tests were carried out. It was concluded that increasing the defoliation from 33 to 66% impaired the physiological quality of soybean seeds. The use of nitrogen fertilization for coverage after defoliation at the beginning of the reproductive phase cannot minimize qualitative losses in seeds. Resumo: Este estudo revisou sistematicamente a literatura sobre a associação de listas de medicamentos potencialmente inapropriados (MPI) na prática clínica e desfechos de saúde na população idosa acompanhada na atenção primária à saúde. Para tanto, o protocolo PRISMA foi usado para sistematizar a busca de artigos nas bases de dados PubMed, Web of Science, Scopus, Cochrane Central, LIVIVO e LILACS, além da literatura cinzenta. Foram selecionados estudos com ensaios clínicos randomizados, incluindo a utilização de critérios explícitos (listas) para identificar e manejar MPI em prescrições para idosos atendidos na atenção primária. Dos 2.400 artigos encontrados, seis foram utilizados para extração de dados. As intervenções reduziram significativamente o número de MPI e eventos adversos a medicamentos e, consequentemente, nas prescrições potencialmente inadequadas em idosos polimedicados. No entanto, não houve efeitos significativos das intervenções sobre desfechos clínicos negativos (como visitas a serviços de emergência, hospitalizações e óbito) ou melhora das condições de saúde dos idosos. O uso de listas de MPI pode promover a adequação da prescrição de medicamentos para idosos na atenção primária à saúde, mas mais estudos são necessários para determinar os impactos da redução de MPI em desfechos clínicos primários.

Abstract: This study is a systematic literature review of the association between lists of potentially inappropriate medications (PIM) in clinical practice and health outcomes of older adults followed up in primary health care. For this purpose, the PRISMA protocol was used to systematize the search for articles in the PubMed, Web of Science, Scopus, Cochrane Central, LIVIVO and LILACS databases, in addition to the gray literature. Studies with randomized clinical trials were selected, using explicit criteria (lists) for the identification and management of PIM in prescriptions of older patients in primary care. Of the 2,400 articles found, six were used for data extraction. The interventions resulted in significant reductions in the number of PIM and adverse drug events and, consequently, in potentially inappropriate prescriptions (PIP) in polymedicated older adults. However, there were no significant effects of the interventions on negative clinical outcomes, such as emergency room visits, hospitalizations and death, or on improving the health status of the older adults. The use of PIM lists promotes adequate medication prescriptions for older adults in primary health care, but further studies are needed to determine the impact of reducing PIM on primary clinical outcomes.

Resumen: Este estudio realizó una revisión sistemática en la literatura sobre la asociación de listas de medicamentos potencialmente inapropiados (MPI) en la práctica clínica y los resultados de salud en la población de edad avanzada monitoreada en atención primaria de salud. Para ello, se utilizó el protocolo PRISMA para sistematizar la búsqueda de artículos en las bases de datos PubMed, Web of Science, Scopus, Cochrane Central, LIVIVO y LILACS, además de la literatura gris. Se seleccionaron estudios con ensayos clínicos aleatorizados, incluyendo el uso de criterios explícitos (listas) para identificar y manejar MPI en prescripciones para adultos mayores atendidos en atención primaria. De los 2.400 artículos encontrados, seis se utilizaron para la recolección de datos. Las intervenciones tuvieron una significativa disminución en la cantidad de MPI y eventos adversos de medicamentos y, en consecuencia, en prescripciones potencialmente inapropiadas en adultos mayores polimedicados. Sin embargo, no hubo efectos significativos de las intervenciones en los resultados clínicos negativos (como consultas a servicios de urgencias, hospitalizaciones o muerte) o mejora en las condiciones de salud de los adultos mayores. El uso de listas de MPI puede promover una adecuada prescripción de medicamentos a los adultos mayores en la atención primaria de salud, si bien se necesitan más estudios para determinar los impactos de la reducción de MPI en los resultados clínicos primarios.

Abstract The scope of this article is to analyze the trend of the standardized mortality rate (SMR) for tuberculosis and its correlation with the developmental status in Brazil. An ecological time series study was conducted to analyze data of deaths from tuberculosis reported between 2005 and 2019 in all states. Data were extracted from the Mortality Information System, the Brazilian Institute of Geography and Statistics, and the Global Burden of Disease study. The temporal trend was analyzed using Prais-Winsten regression. Spearman’s correlation analysis between SMR and Socio-Demographic Index (SDI) was also performed. From 2005 to 2019, 68,879 deaths from tuberculosis were recorded in Brazil. The average mortality rate was 2.3 deaths per 100,000 inhabitants. The decreasing trend of SMR due to tuberculosis was observed in Brazil and in all regions. There was a significant negative correlation between SDI and TMP. TMP due to tuberculosis revealed a decreasing trend in Brazil and in all regions. Most states showed a decreasing trend and none of them had an increasing trend. An inverse relationship was found between developmental status and mortality due to tuberculosis.

Resumo O objetivo do artigo é analisar a tendência da taxa de mortalidade padronizada (TMP) por tuberculose e sua correlação com o status de desenvolvimento no Brasil. Estudo ecológico de séries temporais que analisou dados de óbitos por tuberculose notificados entre 2005 e 2019 de todos os estados. Os dados foram extraídos do Sistema de Informação sobre Mortalidade, do Instituto Brasileiro de Geografia e Estatística e do estudo da Carga Global de Doenças. A tendência temporal foi analisada pela regressão de Prais-Winsten. A análise da correlação de Spearman entre a TMP e o índice sociodemográfico (socio-demographic index - SDI) também foi realizada. De 2005 a 2019, foram registrados 68.879 óbitos por tuberculose no Brasil. A taxa média de mortalidade foi de 2,3 óbitos por 100.000 habitantes. A tendência decrescente da TMP por tuberculose foi observada no Brasil e em todas as regiões. Verificou-se correlação negativa significativa entre o SDI e a TMP. A maioria dos estados apresentou tendência decrescente e nenhum deles teve tendência crescente. Uma relação inversa foi verificada entre o SDI e a mortalidade por tuberculose.

Abstract The aim is to analyze the trend in breast cancer mortality rates and its correlation with the socioeconomic development status in Brazil. It involved an ecological time series study carried out in the 26 units of the federation, Federal District and regions of Brazil. Data sources included the Mortality Information System (number of deaths), the Brazilian Institute of Geography and Statistics (resident population) and the study of the Global Burden of Disease (Socio-demographic Index - SDI). Data from 2005 to 2019 were used. The trend was analyzed using Prais-Winsten regression models. The relationship between the mortality rate and the SDI was analyzed using Spearman’s correlation coefficient. During the study period, 207,683 deaths from breast cancer occurred. The average standardized mortality rate was 19.95 deaths per 100,000 women in Brazil. All the regions of Brazil showed an increasing trend in mortality. Of the total federative units, 22 showed an increasing trend. There was a positive relationship between the mortality rate and the SDI. The standardized mortality rate for breast cancer showed an increasing trend in Brazil, in all regions and in most states. There was a direct association between mortality and SDI, indicating a greater magnitude in more developed regions.

Resumo O objetivo é analisar a tendência da taxa de mortalidade por câncer de mama e sua correlação com o status de desenvolvimento socioeconômico no Brasil. Estudo ecológico de séries temporais realizado nos 26 estados, Distrito Federal e regiões do Brasil. As fontes de dados foram o Sistema de Informação sobre Mortalidade (número de óbitos), o Instituto Brasileiro de Geografia e Estatística (população residente) e o estudo da Carga Global de Doenças (socio-demographic index - SDI). Foram usados dados de 2005 a 2019. A tendência foi analisada pelos modelos de regressão de Prais-Winsten. A relação entre a taxa de mortalidade e o SDI foi analisada pelo coeficiente de correlação de Spearman. No período do estudo ocorrerem 207.683 óbitos por câncer de mama. A taxa padronizada de mortalidade média foi de 19,95 óbitos por 100.000 mulheres no Brasil. O Brasil e todas as regiões apresentaram tendência crescente da mortalidade. Do total de estados, 22 apresentaram tendência crescente. Verificou-se relação positiva entre a taxa de mortalidade e o SDI. A taxa de mortalidade padronizada por câncer de mama apresentou tendência crescente no Brasil, em todas as regiões e na maioria das unidades da federação. Verificou-se associação direta entre mortalidade e SDI, indicando maior magnitude em regiões mais desenvolvidas.

Objetivo: Descrever o protocolo e o plano de análise estatística que serão usados para testar se uma estratégia de limitação da driving pressure envolvendo a titulação da pressão positiva expiratória final de acordo com a melhor complacência respiratória e a redução do volume corrente é superior a uma estratégia padrão envolvendo o uso da tabela de pressão positiva expiratória final baixa do protocolo ARDSNet, em termos de aumento do número de dias sem ventilador em pacientes com síndrome do desconforto respiratório agudo devido à pneumonia adquirida na comunidade. Métodos: O estudo STAMINA (ventilator STrAtegy for coMmunIty acquired pNeumoniA) é randomizado, multicêntrico e aberto e compara uma estratégia de limitação da driving pressure com a tabela de pressão positiva expiratória final baixa do protocolo ARDSnet em pacientes com síndrome do desconforto respiratório agudo moderada a grave devido à pneumonia adquirida na comunidade internados em unidades de terapia intensiva. Esperamos recrutar 500 pacientes de 20 unidades de terapia intensiva brasileiras e duas colombianas. Eles serão randomizados para um grupo da estratégia de limitação da driving pressure ou para um grupo de estratégia padrão usando a tabela de pressão positiva expiratória final baixa do protocolo ARDSnet. No grupo da estratégia de limitação da driving pressure, a pressão positiva expiratória final será titulada de acordo com a melhor complacência do sistema respiratório. Desfechos: O desfecho primário é o número de dias sem ventilador em 28 dias. Os desfechos secundários são a mortalidade hospitalar e na unidade de terapia intensiva e a necessidade de terapias de resgate, como suporte de vida extracorpóreo, manobras de recrutamento e óxido nítrico inalado. ABSTRACT Background: Driving pressure has been suggested to be the main driver of ventilator-induced lung injury and mortality in observational studies of acute respiratory distress syndrome. Whether a driving pressure-limiting strategy can improve clinical outcomes is unclear. Objective: To describe the protocol and statistical analysis plan that will be used to test whether a driving pressure-limiting strategy including positive end-expiratory pressure titration according to the best respiratory compliance and reduction in tidal volume is superior to a standard strategy involving the use of the ARDSNet low-positive end-expiratory pressure table in terms of increasing the number of ventilator-free days in patients with acute respiratory distress syndrome due to community-acquired pneumonia. Methods: The ventilator STrAtegy for coMmunIty acquired pNeumoniA (STAMINA) study is a randomized, multicenter, open-label trial that compares a driving pressure-limiting strategy to the ARDSnet low-positive end-expiratory pressure table in patients with moderate-to-severe acute respiratory distress syndrome due to community-acquired pneumonia admitted to intensive care units. We expect to recruit 500 patients from 20 Brazilian and 2 Colombian intensive care units. They will be randomized to a driving pressure-limiting strategy group or to a standard strategy using the ARDSNet low-positive end-expiratory pressure table. In the driving pressure-limiting strategy group, positive end-expiratory pressure will be titrated according to the best respiratory system compliance. RESUMO: A utilização de práticas de manejo com cobertura do solo, uso de polímeros de absorção de água no solo e telas de sombreamento podem diminuir o consumo de água em sistemas irrigados, por proporcionarem proteção da planta e solo contra a presença de radiação e comprimentos de onda (luz) indesejados para o cultivo, no entanto essas tecnologias são pouco estudadas no cultivo de abacaxizeiro em regiões de clima semiárido. Assim, o trabalho foi realizado com o objetivo de avaliar o crescimento, produção e qualidade do abacaxizeiro em resposta a utilização de diferentes tipos de cobertura de solo, tela de sombreamento vermelha e polímeros de absorção água. Para isso, desenvolveu-se ensaio de campo composto por quatro coberturas de solo nas parcelas (mulching plástico; matéria orgânica; matéria orgânica + tela de sombreamento vermelha 50%; mulching plástico + tela de sombramento vermelha 50% e o controle (sem cobertura) com dois tratamentos nas subparcelas referentes a ausência e presença de polímero absorvedor de água colocado na cova de plantio (0,5 g por planta), dispostos em parcelas subdivididas, no delineamento experimental em blocos ao acaso, com três repetições. Foram avaliadas a porcentagem de plantas com florescimento natural; diâmetro do talo; índice de área foliar; área foliar de folha D e área foliar total; peso do fruto com coroa; peso do fruto sem coroa; comprimento do fruto com coroa; e comprimento do fruto sem coroa, bem como as características pós-colheita de frutos, sendo sólidos solúveis; acidez titulável; pH e relação sólidos solúveis/acidez titulável. Foram determinadas a produtividade em t ha-1 e eficiência do uso da água (EUA). O mulching plástico + tela de sombramento vermelha 50% apresentou-se mais favorável ao desenvolvimento vegetativo e produtividade das plantas de abacaxizeiro ‘IAC Fantátisco’. Os tratamentos não influenciaram nas características de qualidade pós-colheita de frutos. ABSTRACT: In irrigated systems, the adoption of management methods like soil cover, polymers which absorb the water present in the soil and the utilization of shading screens can decrease the water consumption by the plants. This is achieved as these means protect the plant and soil against radiation and wavelengths of light undesirable for cultivation. However, for pineapple cultivation in the semi-arid climate regions, very little study has been done regarding these technologies. Hence, the present study conducted to assess the pineapple growth, production and quality when various kinds of soil cover, red shading screen and water absorption polymers are employed. To accomplish this, a field trial was performed utilizing four types of soil cover in the plots (plastic mulching; organic matter; organic matter + red shading screen 50%; plastic mulching + red shading screen 50% and the control (no cover).Two treatments were included in the subplots namely, either the absence or presence of a water-absorbing polymer in the planting hole (0.5 g per plant), arranged in subdivided plots. We adopted the Randomized Complete Block (RCB) , with three replications. The following aspects of the plants were studied: naturally flowering, stem diameter; leaf area index; D-leaf area and total leaf area; fruit weight with crown; fruit weight without crown; fruit length with crown; and fruit length without crown, as well as the traits of the post-harvest fruit, soluble solids, titratable acidity, pH and the soluble solids/titratable acidity ratio. Further, the productivity in t ha-1 and efficiency of water use (EUA) were ascertained. It was evident that the plastic mulching + 50% red shading screen favored the vegetative development and productivity of the ‘IAC Fantástico’ pineapple plants. The characteristics of the quality of the postharvest fruit were unaffected by the treatments. The shading screen reduced the percentages of natural flowering in the treatments which included organic matter + red shading screen 50% and plastic mulching + red shading screen 50%. RESUMO Uma das principais etapas na bubalinocultura leiteira é a criação de bezerras, pois nesta fase é definido pela qualidade quais animais serão as futuras matrizes da propriedade. A fase de cria se estende do nascimento ao desmame, é delicada e precisa, consequentemente, atender as características de extrema importância na atividade leiteira como, aspectos comportamentais e sanitários, favorecendo com que esses animais expressem todo seu potencial genético. Devido ao fato dessa fase de vida do animal não gerar retorno econômico imediato ao produtor, ela acaba não recebendo a atenção necessária, contribuindo para índices zootécnicos baixos e custos de produção elevados. A adoção de práticas adequadas de manejo é essencial para o desenvolvimento produtivo do animal que garantirá no futuro a boa produtividade da fazenda. Diante do exposto, objetivou-se apresentar, por meio de revisão de literatura, o manejo de bezerras bubalinas, ressaltando que a alimentação adequada, o manejo correto e a prevenção de doenças são fundamentais para garantir o bem-estar e a saúde das bezerras. Esses fatores, juntamente com um ambiente limpo e seguro, podem maximizar o potencial produtivo desses animais. ABSTRACT One of the main stages in bubalinoculture is breeding calves, because this phase is defined by the quality of the animals that will be the future property matrices. The breeding phase extends from birth to weaning, and is delicate and precise; consequently, attending to characteristics of extreme importance in dairy farming activity such as behavioral and health aspects will aid these animals to express all of their genetic potential. Since this life phase of the animal does not generate immediate economic return to the producer, it ends up not receiving the necessary attention, contributing to low zootechnical indices and high production costs. Adopting adequate management practices is essential for the productive development of the animal that will guarantee the future good productivity of the farm. In view of the above, the objective of this study is to present the management of buffalo calves through a literature review, emphasizing that proper feeding, correct management and disease prevention are fundamental to ensuring the well-being and health of calves. RESUMO Objetivo: Analisar os fatores associados ao conhecimento dos Agentes Comunitários de Saúde (ACS) sobre tuberculose. Métodos: Estudo transversal realizado com 110 ACS. Utilizou-se um questionário para avaliar o conhecimento sobre tuberculose pulmonar (componente 1) e as funções laborais dos ACS no Programa Nacional de Controle de Tuberculose (componente 2). O nível de conhecimento, de acordo com os escores transformados em uma escala de 0 a 100, foi classificado como: 0-50% (baixo), 51-75% (mediano) e mais de 75% (alto). A regressão múltipla foi empregada na análise dos fatores associados. Resultados: A pontuação global (média dos escores dos componentes 1 e 2) mediana do conhecimento foi de 68,6%. O conhecimento global sobre tuberculose mostrou-se positivamente associado ao tempo de atuação profissional, à recepção de capacitação sobre tuberculose e ao acesso ao guia/cartilha de tuberculose. Conclusões: Investimentos na formação e nas estratégias de capacitação dos ACS contribuirão para o aumento do conhecimento desses profissionais, resultando em maior sucesso no controle da tuberculose. ABSTRACT Objective: To analyze the factors associated with the knowledge of Community Health Agents (ACS) about tuberculosis. Methods: A cross-sectional study was conducted with 110 ACS. A questionnaire was used to assess knowledge about pulmonary tuberculosis (component 1) and the work functions of ACS in the National Tuberculosis Control Program (component 2). RESUMO Objetivo: analisar a percepção das puérperas sobre o papel do enfermeiro no ciclo gravídico-puerperal à luz da Teoria de Peplau. Método: estudo descritivo com abordagem qualitativa, que utilizou como referencial teórico-metodológico a Teoria de Peplau. Os dados foram coletados em um Centro de Parto Normal, no Ceará - Brasil, de março a maio de 2021. Realizou-se entrevista semiestruturada com 30 puérperas. A análise ocorreu de acordo com as fases da teoria: orientação, identificação, exploração e resolução. Resultados: as informações fornecidas pelo enfermeiro não foram compreendidas de forma satisfatória e houve a restrição da autonomia da mulher durante o trabalho de parto. Recursos como plano de parto e grupo de gestantes não foram explorados efetivamente. Foi predominante o contentamento das puérperas quanto à resolução do cuidado. Conclusão: evidenciou-se a importância da aplicabilidade da Teoria de Peplau na assistência do enfermeiro no ciclo-gravídico puerperal para promover a autonomia das mulheres no processo terapêutico. ABSTRACT Objective: to analyze postpartum women’s perceptions of the role of nurses in the pregnancy-puerperal cycle in light of Peplau’s theory. Method: a descriptive study with a qualitative approach, using Peplau’s Theory as its theoretical-methodological framework. Data was collected at a Normal Birth Center in Ceará - Brazil, from March to May 2021. A semi-structured interview was conducted with 30 postpartum women. The analysis took place according to the phases of the theory: orientation, identification, exploration, and resolution. Resources such as birth plans and pregnancy groups were not effectively explored. The postpartum women were predominantly satisfied with the resolution of their care. RESUMO Contexto: Nas últimas décadas, a ecoendoscopia evoluiu de uma ferramenta diagnóstica para uma plataforma para diversas intervenções terapêuticas. Vários avanços tecnológicos surgiram desde o último Consenso Brasileiro, demandando uma revisão e atualização das recomendações baseadas nas melhores evidências científicas. Métodos: Um grupo de 32 ecoendoscopistas renomados selecionou oito tópicos relevantes para serem discutidos a fim de gerar questões clínicas. Em seguida, foi realizada uma revisão da literatura para responder a essas perguntas com base nas evidências mais atualizadas. Resultados: Trinta e três tópicos foram formulados e votados pelos especialistas para alcançar um consenso. O Sistema de Oxford foi utilizado para classificar o nível de evidência. Conclusão: Há evidências moderadas para sustentar que a forma da agulha, calibre ou técnica de aspiração não influenciam no rendimento da amostragem tecidual guiada por ultrassom endoscópico (USE) de lesões sólidas pancreáticas. Há evidências moderadas para sustentar o uso da biópsia transcutânea guiada por USE da parede cística para diferenciar entre neoplasias císticas mucinosas e não mucinosas. Existem poucas evidências para apoiar o tratamento guiado por USE de varizes gástricas. Há um alto nível de evidência para sustentar que a drenagem biliar guiada por USE e a CPRE apresentam resultados semelhantes em pacientes com obstrução biliar maligna distal. Há um alto nível de evidência para o uso da USE no diagnóstico de cistos pancreáticos neoplásicos e na detecção de necrose antes de indicar a drenagem. Há evidências moderadas para sustentar a preferência pela gastroenterostomia guiada por USE em relação ao stent duodenal para obstrução maligna da saída gástrica em pacientes com expectativa de vida superior a 2 meses. Há um alto nível de evidência para sustentar o uso da ablação por radiofrequência no tratamento de ambos os tipos, funcionantes e não funcionantes, de tumores neuroendócrinos. ABSTRACT Background: In the past decades, endoscopic ultrasound has developed from a diagnostic tool to a platform for many therapeutic interventions. Various technological advancements have emerged since the last Brazilian Consensus, demanding a review and update of the recommendations based on the best scientific evidence. Methods: A group of 32 renowned echoendoscopists selected eight relevant topics to be discussed to generate clinical questions. After that, a literature review was conducted to answer these questions based on the most updated evidence. Results: Thirty-three statements were formulated and voted on by the experts to reach a consensus. The Oxford System was used to grade the level of evidence. Conclusion: There is moderate evidence to support that the needle shape, gauge, or aspiration technique does not influence the yield of endoscopic ultrasound (EUS)-guided tissue sampling of pancreatic solid lesions. There is moderate evidence to support using EUS-TTNB of the cyst wall to differentiate between mucinous and non-mucinous cystic neoplasms. There is little evidence to support the EUS-guided treatment of gastric varices. There is a high level of evidence to support that EUS-guided biliary drainage and ERCP present similar outcomes in patients with distal malignant biliary obstruction. There is a high level of evidence for using EUS to diagnose neoplastic pancreatic cysts and detect necrosis before indicating drainage. There is moderate evidence to support EUS-GE over duodenal stent for malignant gastric outlet obstruction in patients with a life expectancy higher than 2 months. There is a high level of evidence to support the use of RFA in treating both functioning and non-functioning types of NET.
