RESUMO: Botulismo é causado pela ingestão de toxinas pré-fromadas produzidas por Clostridium botulinum. Este estudo descrevre um surto de botulismo tipo D em um rebanho bovino, com alta taxa de mortalidade (67%), devido ao consumo de silagem mal acondicionada. Os bovinos afetados apresentaram curso clínico hiperagudo e agudo de paralisia flácida progressiva. Cinco animais foram necropsiados e não foram visualizadas alterações macroscópicas e microscópicas. Embora a toxina não tenha sido detectada no alimento, a fonte mais provável de contaminação foi considerada a silagem armazenada inadequadamente. O diagnóstico de botulismo foi baseado nos achados clínicos, epidemiológicos e pela detecção da toxina botulínica no conteúdo intestinal de dois bovinos. ABSTRACT: Botulism is a condition caused by the ingestion of pre-formed toxins produced by the bacterium Clostridium botulinum. This study described an outbreak of type D botulism with a high mortality rate (67%) in a cattle herd, caused by the consumption of fermented silage. The affected cattle presented with a hyperacute to acute clinical course with progressive flaccid paralysis. Five animals underwent necropsy with no significant macroscopic and microscopic alterations. A most probable source of contamination was deemed to be an inadequately stored silage; although, the toxin was not detected in the feed. The diagnosis of botulism was based on both clinical and epidemiological findings and the detection of botulinum toxin in the intestinal contents of two of the affected cattle. The present study emphasized the high number of fatally affected cattle, an unusual source of botulinum toxin and the importance of proper management of animal feed supply. This work evaluated the use of corncob as a support for the immobilization of an extracellular fructosyltransferase (E.C. 2.4.1.9) from Aspergillus oryzae IPT-301, aiming at the synthesis of a stable biocatalyst for the production of fructooligosaccharides. The transfructosylation activity of the immobilized enzyme was maximized via experimental design. Additionally, the biochemical properties and stability of the immobilized enzyme, as well as the reaction kinetics, were determined. The enzyme immobilized on alkali-treated corncob showed the highest transfructosylation activity at 50 °C and pH 5.5. It also presented a wide pH stability and a half-life around 1.4 times greater than the soluble enzyme. Furthermore, the immobilized enzyme showed 53% of retention of catalytic activity in a second consecutive reaction cycle, showing the potential for reuse. These results suggest that alkali-treated corncob is a promising support material for the synthesis of heterogeneous biocatalysts aiming at fructooligosaccharide production, which can contribute to the valorization of this abundant agro-waste. ABSTRACT Arbuscular mycorrhizal fungi increase water and nutrient absorption from the soil by plants. However, its benefits to plants vary depending on the inoculated species. This study aimed to determine the influence of different species of mycorrhizal fungi on the development and productivity of grain sorghum cultivars. The experiment was conducted in a greenhouse in a completely randomized design in a 2 x 5 factorial scheme, consisting of two sorghum cultivars (BRS 310 and BRS 330) and five treatments (control without inoculation and four species of arbuscular mycorrhizal fungi: Gigaspora margarita, Glomus formosanum, Acaulospora scrobiculata, and Scutellospora pellucida), with four replications. Plant height, stem diameter, leaf area, length, volume, specific surface area, root diameter, shoot dry mass, root dry mass, thousand-grain weight, grain productivity, colonization, dependence, and mycorrhizal efficiency were evaluated. The results showed that inoculation with the species S. pellucida induced higher root and shoot development of plants of the cultivar BRS 310, as well as higher productivity of sorghum grains. The cultivar BRS 330 showed higher root development when inoculated with the species G. formosanum and a higher shoot development when inoculated with the species A. scrobiculata. ABSTRACT Purpose: This study aimed to examine the prevalence of myopic eyes over 11 years (2008-2018) in a private clinic and a public assistance service. Methods: We retrospectively evaluated 6332 individuals (12,664 eyes) between 5 and 25 years old, seen at a private clinic-CEMO (2,663 individuals) and a public service-HOIP (3,669 individuals) from 2008 to 2018. We evaluated the prevalence of myopic eyes (EE ≤-0.50) and high myopic eyes (EE ≤-6.00). Results: Sex and services did not show statistical differences. The variation in the prevalence of myopic and high myopic eyes showed a random pattern during the study period (this prevalence could not be increased). Prevalences ranged from 20.7% (in 2017) to 32.4% (in 2015) for myopic eyes and from 1.6% (in 2009 and 2016) to 3.3% (in 2015) for eyes with high myopia. The prevalence of myopia showed a statistically significant increase based on the age group. Conclusion: The prevalence of myopic eyes did not increase in our study. Resumo O concreto têxtil surge como uma alternativa para o reforço de estruturas de concreto armado. Porém, por ser um material relativamente novo, inexiste ainda um órgão ou norma regulamentadora que uniformize os processos de fabricação e de aplicação do material, o que gera diferenças significativas quanto às suas características e propriedades. Sob esta perspectiva, este estudo busca avaliar experimentalmente o desempenho mecânico do concreto têxtil ao ser aplicado como reforço de vigas em concreto armado submetidas ao ensaio de flexão a quatro pontos, utilizando o têxtil de fibra de vidro álcali-resistente de produção brasileira TEXIGLASS AR-360-RA-04. De caráter exploratório, o programa experimental é composto por quatorze vigas reforçadas e uma viga testemunho de seção retangular (120 mm de largura, 200 mm de altura e 1500 mm de comprimento), totalizando quinze vigas. Abstract Textile reinforced concrete emerges as an alternative for the strengthening of reinforced concrete structures. However, as a relatively new material, there is still no regulatory body or standardization that standardizes the manufacturing and application processes of the material, which generates significant differences regarding its characteristics and properties. From this perspective, this study seeks to experimentally evaluate the mechanical performance of textile reinforced concrete when applied as reinforcement of reinforced concrete beams subjected to four-point bending tests, using the alkali-resistant fiberglass textile produced in Brazil, TEXIGLASS AR-360-RA-04. As an exploratory study, the experimental program consists of fourteen reinforced beams and one rectangular cross-section reference beam (120 mm wide, 200 mm high, and 1500 mm long), totaling fifteen beams. ABSTRACT BACKGROUND: Detraining can partially or completely reduce training-induced metabolic adaptations. However, the duration for which the rehabilitation effects persist after detraining, especially in patients with heart disease, remains unclear. OBJECTIVES: To evaluate the principle of reversibility/detraining in patients with heart disease via the 6-minute walk test (6MWT) after a period of rest. DESIGN AND SETTING: A retrospective cohort study developed at the Rehabilitation Center of the Universidade Federal do Triângulo Mineiro in Uberaba/MG, Brazil. METHODS: This clinical, retrospective longitudinal study involved 20 patients with heart disease who underwent 5 months of supervised cardiac rehabilitation (CR). The mean age of participants was 64.05 ± 9.25 years. The initial rehabilitation was followed by an interruption period and rehabilitation for another 5 months. Functional capacity was assessed using the 6MWT. RESULTS: In the specific analysis of the distance covered, values of P = 0.03 and P = 0.01 were obtained on comparing post-training (669.64 ± 58.80 meters) with post-detraining (640.82 ± 101.23 meters) and post-detraining with post-retraining (650.82 ± 96.28 meters), respectively. No significant difference was observed for the comparison between training and retraining (P = 0.83). CONCLUSION: Cardiovascular rehabilitation positively stimulates functional capacity, whereas detraining significantly reduces it. The 6MWT is effective in measuring changes in physical capacity. Resumo Diante dos impactos ambientais associados à produção do cimento Portland convencional, o estudo de cimentos especiais de reduzido impacto ambiental tem sido uma tendência. Neste contexto, os cimentos contendo ye’elimita (YCC) se destacam devido à baixa emissão de CO2. Apesar das vantagens ambientais, a alta demanda por alumínio limita economicamente a produção destes cimentos. Uma alternativa para viabilizar a sua produção é o uso de matérias-primas residuais como fonte de Al2O3, a exemplo do SFCC (catalizador gasto do craqueamento catalítico de leito fluidizado). Diante disso, este trabalho tem como objetivo avaliar a hidratação na idade inicial de cimentos YCC produzidos a partir do coprocessamento do SFCC. Abstract Given the environmental impacts associated with the production of conventional Portland cement, the study of special cements with reduced environmental impact has been a trend. In this context, cements containing ye'elimite (YCC) stand out due to their low CO 2 emissions. Despite the environmental advantages, the high demand for aluminum economically limits the production of these cements.An alternative to making its production feasible is using residual raw materials as a source of Al 2 O 3 , such as SFCC (spent fluidized catalytic cracking).In view of this, this work aims to evaluate hydration at the initial age of cements. For this, six clinkers were produced with different ye'elimite contents, with and without SFCC. These were characterized by XRD/Rietveld. The pastes were evaluated by TG/DTG and XRD/Rietveld to understand the hydration mechanism on the third day of these cements. Resumo A trabalhabilidade das argamassas resulta de outras propriedades, como consistência, plasticidade, coesão, retenção de água, exsudação, adesão inicial e densidade. Uma argamassa trabalhável distribui-se durante a aplicação, não segrega durante o transporte e permanece plástica durante o tempo de aplicação. Esta propriedade depende de vários fatores de aplicação, juntamente com os materiais constituintes. Portanto, este estudo tem como objetivo avaliar a influência da formulação, do tipo de agregado e do teor de água no comportamento reológico de argamassas de reboco, aliado a estudos de empacotamento de partículas. Para tanto, foram escolhidas três formulações de argamassas usuais. O teor mínimo de água foi calculado através dos conceitos de empacotamento de partículas e teste de squeeze flow. Abstract Mortars’ workability results from other properties, such as consistency, plasticity, cohesion, water retention, exudation, initial adhesion and density. A workable mortar distributes itself in application, doesn’t segregate during transport, and remains plastic during the application time. This property depends on several application factors, together with the constituent materials. Therefore, this study aims to assess the influence of the formulation, aggregate type and water content on rheological behavior of rendering mortars, combined with particle packing studies. For this purpose, three usual mortar formulations were chosen. The minimum water content was calculated through the concepts of particle packing and the squeeze flow test. As a result, the influence of each constituent on the workability of the mortar was determined, together with the formulations optimized by the aggregate type. Resumen Este estudio investiga manifestaciones de microagresiones raciales, con énfasis en medir el trauma racial experimentado por estudiantes negros matriculados en cursos de biblioteconomía. El objetivo general es comprender cómo se manifiestan las microagresiones raciales en este contexto. Los objetivos específicos incluyen: 1) definición conceptual de microagresiones raciales y sus características; 2) categorización de las formas en que estas microagresiones se manifiestan en las interacciones; 3) aplicación de la Escala de Microagresión Racial en la Enseñanza de la Biblioteconomía (EMREB). Desde el punto de vista metodológico, se trata de una investigación aplicada, cualitativa, que utiliza métodos documentales, bibliográficos, exploratorios y descriptivos. El estudio se divide en tres fases: 1) revisión de la literatura; 2) aplicación de cuestionarios y análisis de resultados; 3) desarrollo y aplicación de la escala. El análisis y aplicación de la escala revelan la presencia de microagresiones, que se manifiestan a través de prácticas sutiles, mensajes verbales y no verbales llenos de desprecio, sexismo y racismo, perpetuados por profesores y colegas blancos en el ámbito de la clase. Las microagresiones más frecuentes caen en las categorías de Deslegitimación/Invalidación y Amenaza/Intimidación, y las formas de opresión se cruzan con la opresión de género, racial y sexual en la experiencia educativa de los estudiantes negros en la educación superior. Con la escala, se encontró que las microagresiones raciales tienen consecuencias traumáticas y, junto con las categorías de opresión observadas, tienen un impacto duradero en la vida de los estudiantes negros observados.

Abstract The study investigates manifestations of racial microaggressions, with an emphasis on measuring racial trauma experienced by black students enrolled in Library Science courses. The overall goal is to understand how racial microaggressions manifest in this context. Specific objectives include: a conceptual definition of racial microaggressions and their characteristics; categorization of the ways in which these microaggressions manifest in interactions; and application of the Racial Microaggression Scale in Library Science Education. From a methodological point of view, it is applied qualitative research that uses documentary, bibliographic, exploratory and descriptive methods. The study is divided into three phases: 1) literature review; 2) application of questionnaires and analysis of results; and 3) development and application of the scale. The analysis and application of the scale reveal the presence of microaggressions, which are manifested through subtle practices and verbal and non-verbal messages full of contempt, sexism and racism, perpetuated by white teachers and colleagues in the classroom. The most common microaggressions fall into the categories of Delegitimization/Invalidation and Threat/Intimidation, and forms of oppression intersect with gender, racial, and sexual oppression in the educational experience of black students in higher education. With the scale, racial microaggressions were found to have traumatic consequences and, along with the categories of oppression observed, have a lasting impact on the lives of the Black students observed.

Objetivos: Investigar os fatores associados à disposição para conhecer o estado sorológico para a sífilis entre estudantes do ensino superior. Métodos: Estudo transversal e analítico realizado com estudantes do ensino superior de Portugal. A coleta de dados ocorreu por meio de questionário on- line. Os dados foram analisados pela regressão logística de Poisson com variância robusta, utilizando o SPSS-24. Resultados: Incluídos 413 estudantes, maioria do sexo feminino, idade média de 20,5 anos e matriculada em cursos de saúde. Observou -se vulnerabilidades comportamentais como não uso do preservativo e uso de álcool/outras drogas; baixa percepção de risco e conhecimento ruim sobre sífilis. Menos da metade dos estudantes se dispuseram a realizar o teste para sífilis. Conclusões: A adesão ao rastreio para sífilis foi baixa e associada a fatores incomuns como menor conhecimento, baixa percepção de risco, menor idade e número de parcerias sexuais e frequentar cursos de áreas diferentes da saúde.

Objectives: To investigate the factors associated with the willingness to know the serological status for syphilis among higher education students. Methods: A cross-sectional and analytical study was conducted with higher education students in Portugal. Data collection was carried out through an online questionnaire. The data were analyzed using Poisson logistic regression with robust variance, utilizing SPSS-24. Results: A total of 413 students were included, the majority of whom were female, with an average age of 20.5 years, and enrolled in health-related courses. Behavioral vulnerabilities such as not using condoms and using alcohol/other drugs were observed; there was also a low-risk perception and poor knowledge about syphilis. Less than half of the students were willing to undergo the syphilis test. Conclusions: Adherence to syphilis screening was low and associated with uncommon factors such as lower knowledge, low risk perception, younger age, fewer sexual partnerships, and enrollment in courses outside the health field.

Los datos se analizaron mediante regresión logística de Poisson con varianza robusta, utilizando SPSS-24. Resultados: Se incluyeron 413 estudiantes, la mayoría del sexo femenino, la edad promedio fue de 20,5 años y matriculados en carreras de salud. Observar vulnerabilidades de comportamiento como no usar condones y consumir alcohol u otras drogas; baja percepción de riesgo y escaso conocimiento sobre la sífilis. Menos de la mitad de los estudiantes estaban dispuestos a realizar la prueba de sífilis. Conclusiones: La adherencia al tamizaje de sífilis fue baja y se asoció con factores obvios como menor conocimiento, baja percepción de riesgo, menor edad y número de parejas sexuales y asistencia a cursos en áreas distintas a la salud.

Abstract Context: Female Sexual Dysfunctions (FSD) are prevalent conditions, can occur in any stage of life and can severely affect women’s quality of life. The etiology of FSD is often multifactorial, including biologic, psychological, interpersonal and sociocultural factors. Objectives: To evaluate FSD in a Portuguese population and what factors can influence the female sexual function. Methods: A cross-sectional study was conducted between August and September of 2023. Participants filled an anonymous online survey, including a demographic questionnaire and the Female Sexual Function Index (FSFI) for evaluation of sexual function. Inclusion criteria: age between 18 and 75 years old, residence in Portugal and being in a heterosexual relationship. The total score FSFI and domain scores (desire, arousal, lubrification, orgasm, satisfaction and discomfort) were analyzed using SPSS 22®. Results: A total of 401 women completed the questionnaire, 393 fulfilled the inclusion criteria. Women had a mean age of 41,6 years (range: 20-71 years). From these, 48% of women reported being moderately satisfied or very satisfied with their sex life and 9,6% reported no sexual intercourse in the past 4 weeks. Among sexually active women, the mean total FSFI score was 27.37 (range: 7.5-34.5±4.99) and 36% met the criteria for sexual dysfunction (FSFI score ≤ 26.55). Post-menopausal participants, when compared to women in reproductive age, had a lower mean total score and also had lower mean scores across all domains, except for discomfort (p<0.05). Women with children or a BMI ≥ 25 had lower mean total score and lower mean scores for desire, arousal and satisfaction domains (p<0.05). Conclusions: The prevalence of FSD was 36%. These findings highlight the importance of addressing FSD as a significant concern for women health and quality of life and the need for training and awareness among healthcare professionals.

Resumo Introdução: As disfunções sexuais femininas (DSF) são condições prevalentes. Estas podem ocorrer em qualquer fase da vida e podem afetar negativamente a qualidade de vida de muitas mulheres. A etiologia das DSF é frequentemente multifatorial, incluindo fatores biológicos, psicológicos, interpessoais e socioculturais. Objetivos: Avaliar as DSF em mulheres portuguesas e identificar possíveis fatores que possam influenciar a sexualidade feminina. Métodos: Estudo transversal observacional realizado entre agosto e setembro de 2023, através do preenchimento de questionário online anónimo, que incluía um questionário demográfico e o Female Sexual Function Index (FSFI) para avaliação da resposta sexual. A pontuação total e dos domínios (desejo, excitação, lubrificação, orgasmo, satisfação e desconforto) foram analisados utilizando o SPSS 22®. Resultados: De um total de 401 mulheres que completaram o questionário, 393 preenchiam os critérios de inclusão no estudo. A idade média das inquiridas foi de 41,6 anos (20-71 anos). Quando questionadas, 74% das mulheres referiram estar moderadamente satisfeitas ou muito satisfeitas com a sua vida sexual e 9,6% das mulheres referiu ausência de relações sexuais nas últimas quatro semanas. Entre as mulheres sexualmente ativas, a pontuação média total do FSFI foi de 27,37 (7,5-34,5±4,99) e 36% preenchiam os critérios para disfunção sexual (Pontuação FSFI ≤ 26,55). As participantes na pós-menopausa, em comparação com as mulheres em idade reprodutiva, apresentaram uma pontuação média total inferior, assim como pontuações médias inferiores em todos os domínios, exceto no desconforto (p<0,05). Mulheres com filhos ou com IMC ≥25 tiveram uma pontuação média total e pontuações médias inferiores nos domínios do desejo, excitação e satisfação (p<0,05). Conclusões: A prevalência de problemas sexuais na população em estudo foi de 36%. Estes achados destacam a importância de abordar as DSF como uma preocupação na saúde e qualidade de vida das mulheres e da necessidade de formação e sensibilização dos profissionais de saúde.

SUMMARY: The anatomical structure of wood from tree species found in floodplain forests can undergo alterations due to seasonal fluctuations in water levels, potentially affecting the volume of wood available for sustainable forest management in inundated areas. Thus, this study aims to characterize the anatomical features of Hura crepitans, Ocotea cymbarum and Eschweilera albiflora, assessing whether these characteristics differ from descriptions reported in previous studies of the same species conducted in other environments. Samples were collected from six adult trees of each species in two sustainable development reserves (SDR), Mamirauá and Amanã, located in the middle Solimões region, state of Amazonas, using a non-destructive method with an increment borer. Two samples were collected from three different heights (0.30, 1.30, and 2.30 m). Microscopic descriptions of the species were determined (porosity, axial parenchyma, rays, fibers, and vessel elements). Two-way factorial ANOVA followed by Tukey’s post hoc test was used to verify the differences in the anatomical elements of the wood samples collected at the three heights. The wood of H. crepitans has diffuse porosity, a predominance of solitary vessels, diffuse-in-aggregates, scarce axial parenchyma, non-storied rays, and libriform fibers. The wood of O. cymbarum has solitary vessels in radial multiples, vasicentric axial parenchyma, non-storied rays, and septate fibers. E. albiflora has a radial grouping of vessels, banded parenchyma, and non-storied rays. This study confirms previous descriptions of this species, revealing that its anatomical characteristics remain constant throughout the trunk.

RESUMEN: La estructura anatómica de la madera de las especies de árboles que se encuentran en los bosques de llanuras aluviales puede sufrir alteraciones debido a las fluctuaciones estacionales en los niveles de agua, afectando potencialmente el volumen de madera disponible para el manejo forestal sostenible de las áreas inundadas. Así, este estudio tiene como objetivo caracterizar las características anatómicas de Hura crepitans, Ocotea cymbarum y Eschweilera albiflora, evaluando si estas características difieren de las descripciones reportadas en estudios previos de las mismas especies realizados en otros ambientes. Se recolectaron muestras de seis árboles adultos de cada especie en dos reservas de desarrollo sustentable (RDS), Mamirauá y Amanã, ubicadas en la región media de Solimões, estado de Amazonas, utilizando un método no destructivo con barrenador incremental. Se tomaron dos muestras en cada una de tres alturas diferentes (0,30, 1,30 y 2,30 m). Se determinaron descripciones microscópicas de las especies (porosidad, parénquima axial, radios, fibras y elementos de los vasos). Se utilizó ANOVA factorial de dos vías seguido de la prueba post hoc de Tukey para verificar las diferencias en los elementos anatómicos de las muestras de madera recolectadas en las tres alturas. La madera de H. crepitans tiene porosidad difusa, predominio de vasos solitarios, difusos en agregados y escaso parénquima axial, radios no estratificados y fibras libriformes. La madera de O. cymbarum tiene vasos solitarios en múltiplos radiales, parénquima axial vasicéntrico, radios sin pisos y fibras septadas. La madera de E. albiflora tiene agrupaciones radiales de vasos, parénquima bandeado y radios sin pisos. Este estudio confirma descripciones previas de la especie, revelando que las características anatómicas permanecen sustancialmente constantes en todo el tronco.

Abstract The ‘Blue Book’: a Future for Family Medicine in Portugal, edited in 1990 by the Portuguese Association of General Practice and Family Medicine, influenced the subsequent years and built the foundation for the Primary Healthcare Reform, initiated in 2005, but not fully implemented. In the current moment of low professional satisfaction, reviewing the principles that should guide family medicine in the future is important. The family doctor is the individual’s physician, who accompanies people's health throughout their life. Family doctors are specialists in primary healthcare and act according to the core values of family medicine as defined by the WONCA. Their actions form the basis of the health system and are associated with better health outcomes. Therefore, their actions must be centred on the person and the provision of differentiated care, reducing bureaucratic tasks and those with low health value. Physical spaces, equipment, and human resources should be adequately ensured, including the adjustment of the size of patients’ lists to ensure quality and dignified work by the Family Doctor, in every working setting. The Portuguese Association of General Practice and Family Medicine advocates for a performance-based remuneration model appropriate to the family doctor’s activity that compensates and prioritizes value-based healthcare, focusing their actions on the individual and reducing inefficiencies and duplications. This model is based on a new, simplified, and modern quality system grounded in the classic Structure - Process - Results framework but considering the dimensions of patient and professional satisfaction and adequately rewarding clinical work, teamwork, and citizen-centeredness. The present document is an adaptation and translation into English of the book 'A New Future for General and Family Medicine', edited by the Portuguese Association of General Practice and Family Medicine, in 2023.

Resumo O Livro Azul: um futuro para a Medicina de Família em Portugal, editado em 1990 pela Associação Portuguesa de Médicos de Clínica Geral, marcou os anos seguintes e conceptualizou a Reforma dos Cuidados de Saúde Primários, iniciada em 2005, mas não implementada na totalidade. No momento atual de insatisfação profissional é importante rever os princípios que nortearão a medicina geral e familiar no futuro. O médico de família é Médico da Pessoa. É nessa condição que faz o acompanhamento médico ao longo da sua vida. É especialista em cuidados de saúde primários e atua de acordo com os valores da medicina de família definidos pela WONCA. A sua ação é a base do sistema de saúde em que atua e está associada a melhores resultados de saúde. É, por isso, fundamental que a sua ação seja centralizada na pessoa e na prestação de cuidados diferenciados a esta, reduzindo as tarefas burocráticas e de baixo valor em saúde. Devem ser dadas condições materiais, de espaços físicos e de recursos humanos, incluindo adequação do tamanho das listas para garantir um trabalho digno e de qualidade ao médico de família. A APMGF defende um modelo de remuneração pelo desempenho adequado à atividade do médico de família que compense e priorize o valor em saúde, que centre a sua ação na pessoa e que reduza as ineficiências e duplicações. Este modelo assenta num novo sistema da qualidade simplificado e moderno assente no clássico Estrutura - Processo - Resultados, mas que considere as dimensões de satisfação de utentes e profissionais e que compense adequadamente o trabalho clínico, em equipa e centralidade do cidadão. O presente documento é uma adaptação e tradução para inglês do livro “Um Novo Futuro para a Medicina Geral e Familiar”, editado pela Associação Portuguesa de Medicina Geral e Familiar, em 2023.

Resumo Introdução: A epidemic intelligence (EI) assegura a deteção precoce, a avaliação e a comunicação das ameaças para a saúde pública para definir prioridades e mobilizar recursos para a investigação, vigilância, prevenção e controlo. Em Portugal, a Direção-Geral da Saúde (DGS) é responsável pela EI e publica semanalmente um relatório de ameaças para a saúde pública (RONDA) que é partilhado com a rede de autoridades de saúde, instituições e profissionais de saúde pública. As alterações nas ameaças comunicadas em relatórios periódicos de ameaças desde a COVID-19 não foram descritas anteriormente. Métodos: Comparámos as ameaças reportadas na RONDA entre 2016 a 2022 em três períodos sequenciais: antes da COVID-19 (janeiro de 2016 - março de 2020), P2 durante as restrições (abril de 2020 - fevereiro de 2022) e P3 na fase pós-aguda da COVID-19 (fevereiro de 2022 - setembro de 2022). Comparamos a frequência média mensal de ameaças não COVID-19 relatadas entre todas as ameaças relatadas nesses períodos, considerando diferentes categorias dentro do grupo de doenças, foco geográfico e fontes de informação. Resultados: As ameaças não-COVID-19 tiveram uma diminuição na frequência média mensal de reporte no Período2 (14,7 vs. 2,3 p < 0.001) em comparação com o Período1, antes da COVID-19. Houve um retorno ao padrão pré-pandêmico de notificação no Período 3 (14,67 vs. 17,63 p < 0.208) para ameaças não COVID-19 com um aumento nas doenças virais emergentes (2,20 vs. 7,62 p < 0.001). O ECDC e a OMS são as fontes de informação mais frequentes, seguidas das fontes nacionais de saúde pública. Conclusões: Durante a COVID-19, houve alterações no reporte de ameaças em Portugal. A COVID-19 possivelmente afetou a EI e os relatórios de ameaças epidémicas globais, possivelmente desviando a atenção e os recursos de outras ameaças para a pandemia. No entanto, a quantidade de ameaças que justificam o acompanhamento e a comunicação pode estar a aumentar. É necessária investigação em comunicação de ameaças detetadas no âmbito da EI a fim de assegurar que todas as ameaças relevantes são avaliadas e comunicadas de acordo com os objetivos da EI, garantindo simultaneamente o investimento em recursos e a preparação para a prevenção e resposta.

Abstract Background: Epidemic intelligence (EI) ensures early detection, assessment, and communication of public health threats. Threat reporting defines priorities and mobilize resources for surveillance, prevention, and control. In Portugal, the Directorate-General of Health (DGS) is responsible for EI and publishes a weekly public health threat report (RONDA). Changes in threats in regular threat reports since COVID-19 have not been previously described. We analysed changes in non-COVID threat reporting in the weekly threat report. Methods: Using the DGS Emergency Operations Centre’s threat reporting database, we compared threats reported in RONDAs from 2016 to 2022 in three sequential periods: P1 before COVID-19 (January 2016-March 2020), P2 during acute COVID-19 restrictions (April 2020-February 2022), and P3 in post-acute COVID-19 phase (February 2022-September 2022). We described the monthly average frequency of reports on non-COVID-19 threats in those periods considering different disease groups, geographical focus, and information sources. We estimated expected non-COVID-19 reports on threats using a forecast model fitted to the time series until March 2020 and compared observed and expected values. Results: Non-COVID-19 threats had a decrease in the monthly average frequency of reporting in period 2 (14,7 vs. 2,3 p < 0.001) compared to period 1. Using the forecast methods, there were 114 fewer non-COVID threats than the 162 expected (−70%) in period 2. In period 3, there were 105 more threats than expected (+256%). The ECDC and the WHO were the most frequent sources of information followed by national Public Health sources. Conclusions: During COVID-19, there was a decrease in reports on non-COVID threats in Portugal. COVID-19 possibly affected global EI, by shifting attention and resources from other threats to the pandemic. However, the number of threats that warrant follow-up and communication is increasing. Further research is necessary to inform the EI research and development agenda, to ensure that all relevant threats are detected, accessed, and communicated according to evolving EI objectives and priorities while resources and preparedness are guaranteed.

Resumo O cateter vesical de longa permanência pode ser indicado em situações clínicas, como nas doenças crônicas do sistema genitourinário ou neurológico. Além dos riscos de infecção, traumas e sangramentos, a permanência do cateter pode afetar dimensões psicoemocionais e socioeconômicas. Objetivamos compreender como a necessidade de uso do cateter urinário por um longo prazo afeta a autopercepção, as interrelações e o autocuidado deste paciente. Realizamos um estudo qualitativo, descritivo, a partir da entrevista de 17 pacientes, e aplicamos a análise temática e o pensamento complexo. Os diferentes prognósticos e as expectativas em relação ao cateter influenciaram a autopercepção, a adaptação, sua aceitação ou negação. A presença do cateter, seja como medida curativa ou para conforto, pode afetar a autoimagem e a sexualidade, gerar inseguranças e incertezas, que requerem compreensão da multidimensionalidade das situações, que sofrem interferências do meio pessoal, familiar e social, bem como da capacidade dos sistemas de saúde para o seu enfrentamento. Abstract A long-term indwelling catheter may be indicated in clinical situations, such as chronic diseases of the genitourinary or neurological systems. In addition to the risks of infection, trauma, and bleeding, a catheter’s permanence can affect psycho-emotional and socioeconomic dimensions. We aimed to understand how the need to use a long-term indwelling catheter affects this patient’s self-perception, interrelationships, and self-care. We carried out a qualitative, descriptive study based on interviews with 17 patients, and applied thematic analysis and complex thinking. The different prognoses and expectations regarding the catheter influenced self-perception, adaptation, acceptance, or denial. The presence of a catheter, whether as a curative measure or for comfort, can affect self-image and sexuality, and generate insecurities and uncertainties, which require understanding the multidimensionality of situations that suffer interference from the personal, family, and social environment, as well as health systems’ capacity to deal with it.
