Resumo Um macho de Pteronura brasiliensis (Carnivora: Mustelidae) foi encontrado morto às margens do Rio Negro, no Pantanal do estado de Mato Grosso do Sul, município de Aquidauana. Dois carrapatos encontrados aderidos à sua pele foram identificados, morfologicamente, como uma ninfa de segundo ínstar de Ornithodoros rostratus (Argasidae) e um macho de Amblyomma sculptum (Ixodidae). Para complementar a identificação morfológica, esses espécimes de carrapatos foram submetidos à extração de DNA e testados por meio de ensaios de PCR, para confirmar a identidade molecular dos espécimes. Além disso, os extraídos dos carrapatos foram testados e revelaram-se negativos nos ensaios de PCR para todos os patógenos testados. Também foram examinados 30 lotes, compostos por 174 indivíduos de O. rostratus, depositados na Coleção Acari do Instituto Butantan, e encontramos material de quatro estados brasileiros, incluindo-se um lote contendo 2 machos e 2 fêmeas de Aquidauana, estado de Mato Grosso do Sul (MS), coletado do solo. Abstract A male of Pteronura brasiliensis (Carnivora: Mustelidae) was found dead on the banks of the Rio Negro, in the Pantanal wetlands of Mato Grosso do Sul state, Aquidauana municipality. Two ticks found attached to its skin were morphologically identified as a second-instar nymph of Ornithodoros rostratus (Argasidae) and a male of Amblyomma sculptum (Ixodidae). In order to complement the morphological identification, these tick specimens were subjected to DNA extraction, and tested using PCR assays to confirm the molecular identity the specimens. Also, the tick DNA samples were tested and were negative in the PCR assays for all the pathogens tested. We also examined 30 batches, consisting of 174 individuals of O. rostratus deposited in the Acari Collection of the Butantan Institute, and we found material from four Brazilian states, including one batch containing 2 males and 2 females from Aquidauana, of Mato Grosso do Sul state, collected from the soil. Resumo Introdução: A avaliação institucional é uma importante ferramenta para o constante aperfeiçoamento dos cursos de Medicina, principalmente quando tomam a opinião discente como um de seus pontos. Objetivo: O estudo teve por objetivo averiguar, por meio de uma revisão sistemática de escopo, quais os métodos utilizados para realizar avaliações institucionais que contem com base na opinião discente através de uma revisão sistemática de escopo. Método: Em conformidade com o “Preferred Reporting Items for Systematic Reviews and Meta-Analyses for Scoping Reviews (PRISMA- ScR)”, as buscas foram realizadas de modo padronizado, envolvendo no mínimo dois pesquisadores, e consultando as bases Lilacs, SciELO, PubMed, Embase e o banco de teses da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (Capes). Por meio do software Rayyan, marcaram-se os estudos duplicados, o que permitiu a avaliação cega e independente dos dois revisores em cada etapa . Os critérios de inclusão utilizados foram: estudos publicados entre 2009 e 2019, que avaliassem cursos de Medicina e em que alguma etapa da avaliação tivesse sido realizada por estudantes. Abstract Introduction: Institutional evaluation is an important tool for the constant improvement of medical courses, especially when they take the student’s opinion as one of their focuses. Objective: The main objective of this study is to investigate which methods are used to conduct institutional evaluations involving student opinion through a scoping systematic review. Methodology: In compliance with the “Preferred Reporting Items for Systematic Reviews and Meta-Analyses for Scoping Reviews (PRISMA-ScR)”, the searches were conducted in a standardized form, involving at least two researchers and consulting the Lilacs, Scielo, Pubmed, and Embase databases and the Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES), using the software Rayyan, to signal duplicates and allow an independent and unbiased evaluation by the two reviewers. The inclusion criteria used were: Studies published between 2009 and 2019, containing the methodology of evaluation, which assessed medical courses and in which this evaluation was carried out by students at some stage. Also, the following exclusion criteria were used: studies written in languages other than Portuguese, English, French and Spanish; evaluations of a specific pedagogical intervention or limited to some subjects; and articles focusing on student self-assessment. The restriction of time and languages carries the risk of selection bias. Results: 2355 records were found and 12 articles were included. Most studies created their questionnaire and used mostly quantitative scales, although many also included some type of qualitative analysis with written questions. Most of the studies dealt with an occasional analysis and not with a project of permanent evaluation of the institution. Conclusion: Publications in the field of institutional evaluation that involve students and describe the questionnaire are still rare, especially when related to permanent evaluations. Abstract Background A cost of illness (COI) study aims to evaluate the socioeconomic burden that an illness imposes on society as a whole. This study aimed to describe the resources used, patterns of care, direct cost, and loss of productivity due to systemic lupus erythematosus (SLE) in Brazil. Methods This 12-month, cross-sectional, COI study of patients with SLE (ACR 1997 Classification Criteria) collected data using patient interviews (questionnaires) and medical records, covering: SLE profile, resources used, morbidities, quality of life (12-Item Short Form Survey, SF-12), and loss of productivity. Patients were excluded if they were retired or on sick leave for another illness. Direct resources included health-related (consultations, tests, medications, hospitalization) or non-health-related (transportation, home adaptation, expenditure on caregivers) hospital resources. Costs were calculated using the unit value of each resource and the quantity consumed. A gamma regression model explored cost predictors for patients with SLE. Results Overall, 300 patients with SLE were included (92.3% female, mean [standard deviation (SD)] disease duration 11.8 [7.9] years), of which 100 patients (33.3%) were on SLE-related sick leave and 46 patients (15.3%) had stopped schooling. Mean (SD) travel time from home to a care facility was 4.4 (12.6) hours. Antimalarials were the most commonly used drugs (222 [74.0%]). A negative correlation was observed between SF-12 physical component and SLE Disease Activity Index (- 0.117, p = 0.042), Systemic Lupus International CollaboratingClinics/AmericanCollegeofRheumatology Damage Index (- 0.115, p = 0.046), medications/day for multiple co-morbidities (- 0.272, p < 0.001), SLE-specific drugs/day (- 0.113, p = 0.051), and lost productivity (- 0.570, p < 0.001). For the mental component, a negative correlation was observed with medications/day for multiple co-morbidities (- 0.272, p < 0.001), SLE-specific medications/day (- 0.113, p = 0.051), and missed appointments (- 0.232, p < 0.001). Mean total SLE cost was US$3,123.53/patient/year (median [interquartile range (IQR)] US$1,618.51 [$678.66, $4,601.29]). Main expenditure was medication, with a median (IQR) cost of US$910.62 ($460, $4,033.51). Mycophenolate increased costs by 3.664 times (p < 0.001), and inflammatory monitoring (erythrocyte sedimentation rate or C-reactive protein) reduced expenditure by 0.381 times (p < 0.001). Conclusion These results allowed access to care patterns, the median cost for patients with SLE in Brazil, and the differences across regions driven by biological, social, and behavioral factors. The cost of SLE provides an updated setting to support the decision-making process across the country. ABSTRACT The post-planting growth of eucalyptus requires proper fertilization to provide nutrients in adequate amounts for the seedlings. This study aimed to determine the contribution of organomineral fertilizers and soil remineralizers to the initial growth of mycorrhized Eucalyptus grandis. The experiment was conducted in a greenhouse at the Federal University of Santa Maria, Frederico Westphalen campus. The experimental design was completely randomized in a factorial arrangement (2x6), combined or not with inoculation of the ectomycorrhizal fungus Pisolithus microcarpus and six fertilizers (mineral fertilizer, mineral fertilizer with remineralizer, organomineral fertilizer, with two levels of phosphorus content — high and low), with five repetitions. RESUMO Neste estudo, o objetivo foi avaliar a fauna epiedáfica em diferentes agroecossistemas utilizando dois métodos de coleta. Os organismos foram amostrados em quatro agroecossistemas: (i) cultivo de milho, (ii) cultivo de Brachiaria ruziziensis, (iii) sistema silvipastoril, e (iv) remanescente florestal secundário. Em cada agroecossistema, 5 armadilhas do método Pitfall traps e 5 do método Provid foram instaladas, espaçadas em 10 metros entre si e mantidas em campo por três dias. Os organismos coletados foram classificados ao menor nível taxonômico possível e os dados obtidos, avaliados pelos índices de diversidade de Shannon e de dominância de Simpson, e análise de componentes principais. O método Pitfall traps resultou em maior abundância da comunidade epiedáfica em relação ao método Provid. ABSTRACT In this study, the objective was to evaluate the epiedaphic fauna in different agroecosystems using two sampling methods. Organisms were sampled in four agroecosystems: (i) corn cultivation, (ii) Brachiaria ruziziensis cultivation, (iii) silvopastoral system, and (iv) secondary forest remnant. In each agroecosystem, 5 Pitfall traps and 5 Provid traps were installed at a distance of 10 meters and kept in the field for three days. The sampled organisms were classified at the lowest possible taxonomic level and the data obtained were analyzed using Shannon’s diversity and Simpson’s dominance indices, and principal component analysis. The Pitfall traps method resulted in greater abundance of the epiedaphic community compared to the Provid traps method. The corn cultivation area had greater abundance compared to the other agroecosystems evaluated. The best ecological index was observed in the B. ruziziensis crop, using both methods. ABSTRACT. This research evaluated the effects of diets containing or not containing deactivated soybean sums on productive performance, carcass attributes, carcass ultrasound, and feed cost in confined heifers with 1/2 Nellore-Angus blood. We used blood from 60 1/2 Nellore-Angus heifers aged 2 or 25 years with a body weight of 404.62 kg. A completely randomized design was used, in which the animals were divided into 2 experimental groups: Cont (concentrated for semi feedlot without added deactivated soybean) and Bocchi (concentrated for semi feedlot with the addition of deactivated soybeans). There was no significant difference in the final weight between the experimental groups or for the consumption of concentrate in the evaluation periods. However, heifers supplemented with deactivated soybean in the diet showed greater daily weight gain than did those in the Cont group, as well as greater carcass weight, arrobas gain, and carcass yield. Bocchi concentrate was less expensive than Cont (U$ 7.26 ton-1). Abstract Background Hand eczema (HE) is a highly prevalent, recurrent, and multifactorial disease. It encompasses a group of eczematous diseases that affect the hands, etiologically classified into irritant contact dermatitis (ICD), allergic contact dermatitis (ACD) and atopic dermatitis (AD). Few epidemiological studies in Latin America have investigated the characteristics of patients with this condition and the origin of the disease. Objectives To analyze the profile of patients diagnosed with HE submitted to patch tests aiming to determine its etiology. Methods A retrospective descriptive study was carried out on epidemiological data and patch tests of patients with HE treated at a tertiary hospital in the city of São Paulo from January 2013 to December 2020. Results A total of 173 patients were studied, whose final diagnosis was 61.8% of ICD, 23.1% of ACD and 5.2% of AD, with diagnostic overlap in 42.8% of the cases. The main positive and relevant patch tests were: Kathon CG (42%), nickel sulfate (33%), and thiuram mix (18%). Study limitations The number of treated cases and socioeconomic profile was limited to a vulnerable population group. Resumo Objetivo Comparamos o volume tireoidiano (VT) e a presença de bócio nodular (BN) em mulheres grávidas e não grávidas em uma área suficiente em iodo. Também avaliamos a relação entre idade gestacional, paridade e VT no grupo de gestantes e determinamos os percentis 2,5 e 97,5 de VT normal na gestação. Métodos Este estudo transversal incluiu 299 mulheres saudáveis (216 grávidas) sem doenças tireoidianas prévias. Ultrassonografias de tireoide foram realizadas e comparadas entre mulheres grávidas e não grávidas. A faixa de distribuição normal de VT (percentis 2,5 e 97,5) na gestação foi determinada após a exclusão de indivíduos com anticorpos tireoidianos positivos, BN e/ou tireotropina sérica (TSH) ou tiroxina livre (T4L) anormais. Resultados O VT foi maior entre as gestantes em comparação com as mulheres não grávidas (8,6 vs 6,1 cm3; p< 0,001) e foi positivamente correlacionado com a idade gestacional (rs = 0,221; p= 0,001), índice de massa corporal (IMC, rs 0,165; p= 0,002) e níveis de T4L (rs 0,118 p= 0,021). A frequência de BN não diferiu entre os dois grupos. Houve correlação negativa entre VT e TSH (rs -0,13; p= 0,014). Abstract Objective We compared thyroid volume (TV) and presence of nodular goiter (NG) in pregnant vs. non-pregnant women in an iodine-sufficient area. We also evaluated the relationship between gestational age, parity, and TV in the pregnant women group, and determined the 2.5th and 97.5th percentiles of normal TV in pregnancy. Methods This cross-sectional study included 299 healthy women (216 pregnant) without previous thyroid diseases. Thyroid ultrasounds were performed and compared between pregnant and non-pregnant women. The range of normal distribution of TV (2.5th and 97.5th percentiles) in pregnancy was determined after excluding individuals with positive thyroid antibodies, NG, and/or abnormal serum thyrotropin (TSH) or free thyroxine (FT4). Results Thyroid volume was larger among pregnant compared to non-pregnant women (8.6 vs 6.1 cm3; p< 0.001) and was positively correlated with gestational age (rs = 0.221; p= 0.001), body mass index (BMI, rs 0.165; p= 0.002), and FT4 levels (rs 0.118 p= 0.021). Nodular goiter frequency did not differ between the two groups. There was a negative correlation between TV and TSH (rs -0.13; p= 0.014). Thyroid volume was lower among primiparous compared to multiparous patients (7.8 vs 8.9; p< 0.001) and was positively correlated with parity (rs 0.161; p= 0.016). The 2.5th and 97.5th percentiles of TV were 4.23 and 16.47 cm3, respectively. Conclusion Thyroid volume was higher in pregnant compared to non-pregnant women and was positively related to parity, BMI, and gestational age in a normal iodine status population. ABSTRACT Introduction: Metabolic dysfunction-associated steatotic liver disease (MASLD), previously known as Nonalcoholic fatty liver disease (NAFLD), is one of the most common hepatic diseases in individuals with overweight or obesity. In this context, a panel of experts from three medical societies was organized to develop an evidence-based guideline on the screening, diagnosis, treatment, and follow-up of MASLD. Material and methods: A MEDLINE search was performed to identify randomized clinical trials, meta-analyses, cohort studies, observational studies, and other relevant studies on NAFLD. In the absence of studies on a certain topic or when the quality of the study was not adequate, the opinion of experts was adopted. Classes of Recommendation and Levels of Evidence were determined using prespecified criteria. Results: Based on the literature review, 48 specific recommendations were elaborated, including 11 on screening and diagnosis, 9 on follow-up, 14 on nonpharmacologic treatment, and 14 on pharmacologic and surgical treatment. ABSTRACT Background: SARS-CoV-2 virus originated in Wuhan (China) in December (2019) and quickly spread worldwide. Antigen tests are rapid diagnostic tests (RDT) that produce results in 15-30 min and are an important tool for the scale-up of COVID-19 testing. COVID-19 diagnostic tests are authorized for self-testing at home in some countries, including Brazil. Widespread COVID-19 diagnostic testing is required to guide public health policies and control the speed of transmission and economic recovery. Methods: Patients with suspected COVID-19 were recruited at the Hospital da Baleia (Belo Horizonte, Brazil). The SARS-CoV-2 antigen-detecting rapid diagnostic tests were evaluated from June 2020 to June 2021 using saliva, nasal, and nasopharyngeal swab samples from 609 patients. Patient samples were simultaneously tested using a molecular assay (RT-qPCR). Sensitivity, specificity, accuracy, and positive and negative predictive values were determined using the statistical program, MedCalc, and GraphPad Prism 8.0. Results: The antigen-detecting rapid diagnostic tests displayed 98% specificity, 60% sensitivity, 96% positive predictive value, and moderate concordance with RT-qPCR. Substantial agreement was found between the two methods for patients tested < 7 days of symptom onset. Conclusions: Our findings support the use of Ag-RDT as a valuable and safe diagnostic method. Ag-RDT was also demonstrated to be an important triage tool for suspected COVID-19 patients in emergencies. SUMMARY OBJECTIVE: This study aimed to determine the frequencies of Epstein-Barr virus, types 1 and 2 infection, and 30 bp del-latent membrane protein 1 viral polymorphism in gastric adenocarcinomas, as well as to investigate the association between Epstein-Barr virus infection and tumor location, type, and the patient's sex. METHODS: Samples were collected from 38 patients treated at a university hospital in Rio de Janeiro, Brazil. Epstein-Barr virus detection and genotyping were performed by polymerase chain reaction, followed by polyacrylamide gel electrophoresis and staining by the silver nitrate method. RESULTS: Overall, 68.4% of patients had Epstein-Barr virus-positive tumors. Of these, 65.4% presented infection by Epstein-Barr virus type 1, 23.1% by Epstein-Barr virus type 2, and 11.5% had coinfection with types 1 and 2. The 30 bp del-latent membrane protein 1 polymorphism was found in 42.3% of Epstein-Barr virus-positive tumors, 23.1% had the wild-type virus, and 23.1% had the wild-type and the polymorphism concomitantly. In 11.5% of Epstein-Barr virus-positive tumors, it was impossible to determine whether there was polymorphism or not. Tumor location in the antrum (22 of 38) and diffuse type (27 of 38) were predominant. There was no significant difference in Epstein-Barr virus infection or the 30 bp del-latent membrane protein 1 polymorphism between men and women. CONCLUSION: Epstein-Barr virus infection was found in 68.4% of tumors investigated in this study. To the best of our knowledge, this is the first article showing the coinfection of Epstein-Barr virus types 1 and 2 in gastric carcinoma in Brazil. RESUMO Objetivo: Analisar retrospectivamente as alterações na ceratometria e no astigmatismo corneano obtidas após cirurgia de implante de anel intraestromal, comparando o uso de um segmento de arco longo versus o implante de dois segmentos de comprimento de arco tradicional. Métodos: A partir de um estudo transversal, obtivemos os dados de 94 olhos de pacientes diagnosticados com ceratocone, que foram submetidos ao implante de anel corneano. Eles foram divididos em dois grupos, dependendo do tipo de implante recebido: Grupo A, um segmento de arco longo; Grupo B, dois segmentos tradicionais. Todos os segmentos implantados possuíam 250µ de espessura. Os dados do pré-operatório dos dois grupos foram comparados, para garantir que as amostras eram similares (as diferenças encontradas entre os dois grupos não eram estatisticamente significativas). As variáveis analisadas no pré e no pós-operatório foram acuidade visual com correção, ceratometria, astigmatismo corneano e refração. Resultados: A amostra que recebeu apenas um segmento de arco longo (Grupo A) obteve redução da ceratometria média de 4,42D (8,7%) e do astigmatismo corneano de 2,43D (40,4%). ABSTRACT Purpose: To retrospectively analyze the changes in corneal keratometry and astigmatism after intrastromal ring surgery, comparing the use of one long arch segment versus two traditional arc length segments. Methods: A cross-sectional study obtained data from 94 eyes of patients diagnosed with keratoconus that underwent surgical treatment with corneal ring implant. They were divided into two groups according to the type of implant received: one long-arch segment (Group A) or two traditional segments (Group B), both 250 microns thick. Resumen: El objetivo de este estudio fue verificar la evidencia de validez de la versión reducida adaptada al portugués brasileño del Traumatic Grief Inventory (Inventario de Duelo Traumático), mediante un análisis factorial confirmatorio y la relación con variables externas, además de evaluar la precisión y presencia de invarianza. El primer paso fue la adaptación y evaluación por jueces expertos de la versión traducida. Después de la evaluación, se aplicó la versión piloto a cinco participantes de la población objetivo para la evaluación semántica y de contenido, denominándose Inventario de Luto Traumático (ILT-BR). Luego, 211 brasileños, que pasaron por un proceso de duelo, respondieron los siguientes instrumentos: Cuestionario Sociodemográfico, Cuestionario de Autopercepción de Salud Mental e ILT-BR. El ILT-BR mostró buenos índices de ajuste y precisión, correlacionándose positivamente con el estrés, el miedo y los pensamientos obsesivos, sin encontrar variación entre grupos. A partir de estos resultados, se concluye que el ILT-BR posee propiedades psicométricas adecuadas para su uso en Brasil.

Abstract: The aim of the this study was to verify the evidence of validity of the reduced version adapted for Brazilian Portuguese of the Traumatic Grief Inventory, through confirmatory factor analysis and relationship with external variables, in addition to assessing the reliability and presence of invariance. The first step consisted of the adaptation and evaluation by expert judges of the translated version. After the evaluation, the pilot version was answered by five participants of the target population, for semantic and content evaluation, being named, in Brazil, as the Inventário de Luto Traumático (ILT-BR). Then, 211 Brazilians, who went through a grieving process, answered the following instruments: Sociodemographic Questionnaire, Mental Health Self-Perception Questionnaire, and ILT-BR. The ILT-BR presented good fit indexes and reliability, positively correlated with stress, fear, and obsessive thoughts, with no invariance found. From these results, we concluded that the ILT-BR is suitable for use in Brazil.
