ABSTRACT Despite being subject to lower AIDS-related mortality rates and having a higher life expectancy, patients with HIV are more prone to develop non-AIDS events. A low CD4+/CD8+ ratio during antiretroviral therapy identifies people with heightened immune senescence and increased risk of mortality. In clinical practice, finding determinants of a low CD4+/CD8+ ratio may be useful for identifying patients who require close monitoring due to an increased risk of comorbidities and death. We performed a prospective study on the evolution of the CD4+/CD8+ ratio in 60 patients infected with HIV (80% males), who were subjected to two different antiretroviral regimens: early and deferred therapy. The initial CD4+/CD8+ ratio was ≤1 for 70% of the patients in both groups. Older age, CD4+ cell count at inclusion, Nadir CD8+T-cell count, and Initial CD4+/CD8+ ratio ≤ 1 were risk factors for lack of ratio recovery. In the multivariate analysis, a CD4+/CD8+ ratio > 1 at the start of the treatment was found to be a determinant factor in maintaining a CD4+/CD8+ ratio > 1. The nadir CD4+T-cell count was lower in the deferred therapy group (p=0.004), and the last CD4+/CD8+ ratio ≤1 was not associated with comorbidities. Ratio recovery was not associated with the duration of HIV infection, time without therapy, or absence of AIDS incidence. A greater improvement was observed in patients treated early (p=0.003). In contrast, the slope of increase was slower in patients who deferred treatment. In conclusion, the increase in the CD4+/CD8+ ratio occurred mostly for patients undergoing early strategy treatment and its extension did not seem to be related to previous HIV-related factors. Abstract Objective To review key evidence-based recommendations for the diagnosis and treatment of peripheral facial palsy in children and adults. Methods Task force members were educated on knowledge synthesis methods, including electronic database search, review and selection of relevant citations, and critical appraisal of selected studies. Articles written in English or Portuguese on peripheral facial palsy were eligible for inclusion. The American College of Physicians’ guideline grading system and the American Thyroid Association’s guideline criteria were used for critical appraisal of evidence and recommendations for therapeutic interventions. Results The topics were divided into 2 main parts: (1) Evaluation and diagnosis of facial palsy: electrophysiologic tests, idiopathic facial palsy, Ramsay Hunt syndrome, traumatic peripheral facial palsy, recurrent peripheral facial palsy, facial nerve tumors, and peripheral facial palsy in children; and (2) Rehabilitation procedures: surgical decompression of the facial nerve, facial nerve grafting, surgical treatment of long-term peripheral facial palsy, and non-surgical rehabilitation of the facial nerve. Conclusions Peripheral facial palsy is a condition of diverse etiology. Treatment should be individualized according to the cause of facial nerve dysfunction, but the literature presents better evidence-based recommendations for systemic corticosteroid therapy. evidencebased based adults methods search citations studies inclusion Physicians Associations Association s interventions parts 1 (1 tests syndrome tumors (2 procedures grafting longterm long term nonsurgical non etiology dysfunction therapy (

ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. The importance of the CTFB is evidenced by the elevated number of citations in the scientific literature in diverse areas of biology, law, anthropology, education, forensic science, and veterinary science, among others. publications problem uptodate up date classifications context exception (CTFB http//fauna.jbrj.gov.br/, httpfaunajbrjgovbr http //fauna.jbrj.gov.br/ , jbrj gov br (http://fauna.jbrj.gov.br/) 2015 Brazil 80 specialists 1 2024 133691 133 691 133,69 125138 125 138 125,13 82.3%, 823 82 3 (82.3% 102000 102 000 102,00 7.69%, 769 7 69 (7.69% 11000 11 11,00 . 3,567 3567 567 (3,56 2,292 2292 2 292 (2,29 1,833 1833 833 (1,83 1,447 1447 447 (1,44 1000 1,00 831 (83 628 (62 606 (60 520 (52 50 users science health biology law anthropology education others http//fauna.jbrj.gov.br/ faunajbrjgovbr //fauna.jbrj.gov.br (http://fauna.jbrj.gov.br/ 201 8 202 13369 13 133,6 12513 12 125,1 82.3% (82.3 10200 10 00 102,0 7.69% 76 6 (7.69 1100 11,0 3,56 356 56 (3,5 2,29 229 29 (2,2 1,83 183 83 (1,8 1,44 144 44 (1,4 100 1,0 (8 62 (6 60 52 (5 5 http//fauna.jbrj.gov.br (http://fauna.jbrj.gov.br 20 1336 133, 1251 125, 82.3 (82. 1020 0 102, 7.69 (7.6 110 11, 3,5 35 (3, 2,2 22 (2, 1,8 18 (1, 1,4 14 4 ( 82. Abstract Objectives: To review and provide evidence-based recommendations for the diagnosis and treatment of otosclerosis. Methods: Task force members were educated on knowledge synthesis methods, including electronic database search, review and selection of relevant citations, and critical appraisal of selected studies. Articles written in English or Portuguese on otosclerosis were eligible for inclusion. The American College of Physicians’ guideline grading system and the American Thyroid Association’s guideline criteria were used for critical appraisal of evidence and recommendations for therapeutic interventions. Results: The topics were divided into 2 parts: 1) Diagnosis – audiologic and radiologic; 2) Treatment – hearing AIDS, pharmacological therapy, stapes surgery, and implantable devices - bone-anchored devices, active middle ear implants, and Cochlear Implants (CI). Conclusions: The pathophysiology of otosclerosis has not yet been fully elucidated, but environmental factors and unidentified genes are likely to play a significant role in it. Women with otosclerosis are not at increased risk of worsening clinical condition due to the use of contraceptives or during pregnancy. Drug treatment has shown little benefit. If the patient does not want to undergo stapedotomy, the use of hearing aids is well indicated. Implantable systems should be indicated only in rare cases, and the CI should be indicated in cases of profound deafness. ABSTRACT We characterized the surf-zone fish assemblage structure of an ocean beach on the central coast of Brazil (Guriri Beach, Espírito Santo) and assessed its diel variability. Like the entire Espírito Santo coast, Guriri Beach was also affected by ore tailings from the Fundão dam, which collapsed on November 5, 2015. Monthly samplings were carried out before the dam collapse along a year cycle (May 2013-April 2014), during the day and night in the neap tide, using a beach seine. A total of 2217 specimens were caught (1017 during the day and 1200 at night), distributed in 36 fish species and 20 families. Juveniles of the kingcroaker Menticirrhus spp., great pompano Trachinotus goodei, Florida pompano Trachinotus carolinus, littlescale threadfin Polydactylus oligodon, and Atlantic sabretooth anchovy Lycengraulis grossidens dominated the fish fauna, accounting for 84% of the total capture. The fish assemblage structure changed irregularly between day and night throughout the year. P. oligodon and Menticirrhus spp. were more abundant at night, whereas T. carolinus and L. grossidens were more abundant during the day. In most cases, no significant change was detected in fish abundance and richness between the day and night; when significant differences were detected, abundance and richness were higher at night. Our findings could support a monitoring program of the local surf-zone fish fauna and serve as before-impact data, allowing more accurate quantification of the ore tailings' impacts on the local fish biodiversity.

In this work, the local electrochemical activity of the zones coupled by Friction Stir Welding (FSW) of an Al-Cu-Li alloy was studied and the results were correlated to the microstructural characteristics of each zone. Electrochemical studies were carried out in the zones affected by welding using cyclic voltammetry (CV) and scanning electrochemical techniques (namely, SECM - Scanning Electrochemical Microscopy and LEIS - Local electrochemical impedance spectroscopy). The results showed that the welding joint (WJ) is predominantly cathodic relatively to the heat affected zones (HAZ). The HAZ was always anodic and showed the highest electrochemical activities among the tested ones. The high electrochemical activity of the HAZ was associated with the effect of galvanic coupling between the cathodic region (WJ) and the anodic region (HAZ). ABSTRACT This is a reflective study with the objective of analyzing the possibilities of access to employment by refugees in the Brazilian territory, in view of the socio-legal conditions in Brazil, understanding the relationship between work and health. Refugees experience the penalty of restricted access to employment, which has a significant impact on their lives. By remaining disconnected from work, they are pushed to the margins of society. In addition to this, they face difficulties in accessing adequate housing, healthcare services, education, healthy food, among others, all of which deeply affect their way of life in a foreign country with a different culture. The work becomes relevant in the discussion about access to health, goods and services necessary to live with dignity, and its implications in the work-health-disease relationship. Collaborative actions at a global level are necessary to ensure that employment opportunities are available for refugees. In this regard, this reflection articulates two basic ideas: the recognition of the importance of work in social life and living conditions, and the understanding that the determination of the health-disease process goes beyond the biological and individual choices, having historicity and a social and cultural character. territory sociolegal socio legal Brazil health lives society housing education food others culture dignity workhealthdisease disease regard ideas healthdisease choices character

Abstract The adaptation of specialized care for newborns with Down syndrome (DS) requires recognition of its characteristics at the local level. Goal: to characterize newborns diagnosed with DS and their mothers in maternal and child care units in the city of Medellín. Also, explore the factors associated with timely prenatal diagnosis. Methods: observational, cross-sectional study, based on medical records of newborns diagnosed with DS in seven institutions in Medellín, Antioquia between January 2015 and December 2019, taking into account both maternal and neonatal variables. Initially, a univariate analysis was performed using absolute and relative frequencies, then a bivariate analysis was performed taking into account neonatal outcomes according to access to prenatal care and the time at which the diagnosis was made (prenatal or neonatal), and finally a multivariate analysis for late neonatal diagnosis. Results: 50.2% of the mothers at the time of delivery were 35 years old or younger, of these 59.9% belonged to the contributory regime and 83.4% had 4 prenatal check-ups or more, despite this, it was found that only 33.7% had a prenatal diagnosis of DS. The majority (91.4%) of the neonates had a diagnosis of congenital heart disease and the prolonged hospital stay was related to related morbidity. Conclusion: DS is one of the most common chromosomal abnormalities in our environment, which can be diagnosed early. However, in our study it was possible to show that despite the high coverage of prenatal controls, the frequency of antenatal diagnosis is lower, which requires that these controls be carried out by medical personnel trained in patients with this type of pathology. This will finally be reflected in a better acceptance by the family towards the arrival of a child with this condition and, of course, better access to specialized health services.

Abstract: In recent decades, the growing obesity epidemic in Mexico has become one of the most important public health challenges faced by the country. With support from the World Obesity Federation, we formed a working group in 2021 to identify and summarize priority actions that Mexico can take to face this epidemic. More than 1 000 health professionals joined the development and discussion process. Recommendations from previously published, high-level documents and guidelines were taken into account. In commemoration of World Obesity Day 2022, this statement is presented as input for health care professionals to develop actions to address obesity. The statement includes 10 recommendations that include population-level and individual-level actions. It emphasizes the importance of social participation, comprehensive interventions with a person- centered perspective, planetary sustainability, on improving education and communication campaigns, as well as fostering a built environment that promotes active living, and shielding prevention and control efforts from conflicts of interest. The statement calls for obesity to be treated seriously, based on scientific evidence, in a timely and comprehensive manner, employing a life-course and ethical approach that does not perpetuate weight stigma in society.

Abstract The exchange rate can be a determining factor in tourist demand and it can change the tourism trade's competitiveness. This study aims to measure the dependence between international tourist demand and the exchange rate in Brazil. Empirical investigations of this relation, using the copula-GARCH model, are relatively recent in the world literature. The application is carried out with monthly data on exchange rates and international arrivals from Argentina, the United States, and Germany, between 1999 and 2018. The results indicate that the exchange rate variation is not directly associated with the number of tourist arrivals from Germany and the United States. However, for Argentina, the correlation measure was negative and statistically significant, indicating a weak association between the variables. This indicates that when the local currency depreciates against the Brazilian currency, the number of arrivals decreases. This study's conclusions can help managers of tourist organizations understand the relationship between foreign exchange and international tourist demand in Brazil.

ABSTRACT. The literature indicates that cognitive stimulation interventions have shown promising results. Abacus represents a tool with great potential in such interventions. Objectives: To carry out a systematic review of studies published in recent years that entailed the delivery of a cognitive training program using an abacus to boost target cognitive abilities of older persons and also other age groups, with or without cognitive impairment. Methods: A systematic review study was conducted in July 2020 involving PubMed, MedLine, LILACS, and SciELO databases. Results: A total of 29 studies were retrieved, of which 8 aimed to identify the effect of abacus-based mental calculation (AMC) for different age groups and to determine its applicability as a method of cognitive stimulation for older adults. In AMC technique, participants first learn to use the physical abacus (PA) and after achieving proficiency they perform calculations using a mental image of the device, manipulating the beads of the so-called mental abacus (MA). Conclusions: The number of studies addressing abacus use as a cognitive training tool was rather limited, considering the relevance of the theme. Their interventions have shown benefits for cognitive functioning of individuals of various age groups, including older adults with cognitive impairment. Future studies that involve larger samples of healthy and/or cognitively impaired older adults with a longitudinal design and a more elaborate methodological design are suggested.

Abstract People’s relationships with things and with archaeological sites in the Amazonian context are extremely plural and contextual. In each community, researchers learn other ways of thinking about the intrinsic relationships between the past and present. The Mamirauá Institute’s Archaeology Laboratory has excelled at an interdisciplinary approach to developing activities and research together with riverine communities in protected areas. In this article, we discuss archaeological practices and the challenges of preserving archaeological heritage in community contexts. Through archaeology, archaeological conservation, and education, we reflect on our role as professionals working from different knowledge and interest groups, where values, meanings, and results are revisited every time new collaborations are established (whether these involve academic research or community agents). To do so, we examine the work carried out in the Boa Esperança community, located in the Amanã Sustainable Development Reserve, which has been receiving archaeologists since 2006. This research is considered to have been transformed by the community context and by broader engagement with different agents, reorienting efforts to find medium- and long-term alternatives for collaborative and sustainable management of the Amazon’s archaeological heritage.
