Evaluation of the level of information of pediatricians about the diagnosis and management of cryptorchidism

Abstract Objective Evaluate the level of information of pediatricians about the diagnosis and management of cryptorchidism. Method A cross-sectional observational study was conducted using a form via the "Google Forms" platform. The study population included pediatricians and pediatric residents associated with the Brazilian Society of Pediatrics. Seven hundred twenty-eight responses were recorded and analyzed using IBM SPSS v21. Results 728 valid responses were obtained. Of these answers, only 20.5 % answered that the physical examination was sufficient for the diagnosis, and 79.4 % responded that they requested ultrasound as the best test to aid in diagnosing cryptorchidism. When questioned about the ideal age for referring a patient with cryptorchidism, the survey recorded 56.3 % of the responses defending the correct age as six months old, 30.2 % shortly after birth, and 13.2 % at two years old. Other topics were addressed in the form, such as the frequency of evaluation of testicular position and investigation for DDS, among others. Still, the answers to these questions were compatible with current manuals and guidelines on cryptorchidism. Conclusion It is evident that the understanding of the professionals consulted about the diagnosis and management of cryptorchidism needs to be updated with the current practices adopted and that pediatricians, in general, must maintain periodic programs on this subject. RESUMO Introdução: O uso de alinhadores transparentes como alternativa aos aparelhos ortodônticos fixos tem se tornado popular, devido às demandas estéticas dos pacientes adultos em busca de tratamento ortodôntico. No entanto, a falta de conhecimento dos ortodontistas sobre as consequências jurídicas de suas atividades, e a falta de evidências científicas sólidas levantam preocupações em relação à responsabilidade civil nesse tipo de tratamento. Muitas vezes, as campanhas de marketing das empresas fabricantes exageram nas promessas de resultados e desconsideram a falta de evidências científicas. O paciente, como consumidor, é protegido pelo Código de Defesa do Consumidor, e o ortodontista é considerado um fornecedor de tratamento. Portanto, ele pode ser responsabilizado por danos causados ao paciente, seja por culpa subjetiva ou objetiva. Objetivo: Identificar, por meio de uma revisão bibliográfica, as responsabilidades civis dos ortodontistas e das empresas fabricantes de alinhadores, fornecendo orientações jurídicas básicas para ajudar os profissionais a se protegerem de possíveis demandas judiciais relacionadas ao tratamento com alinhadores ortodônticos. Conclusões: O estudo destaca a importância do conhecimento de noções jurídicas em tratamentos com alinhadores ortodônticos, devendo o profissional resguardar-se juridicamente por meio de contratos individuais por escrito, prevenindo-se de assumir uma obrigação de resultado com o paciente. ABSTRACT Introduction: The use of clear aligners as an alternative to fixed orthodontic appliances has become popular due to the aesthetic demands of adult patients seeking orthodontic treatment. However, orthodontists’ lack of knowledge about the legal consequences of their activities, and the lack of solid scientific evidence raise concerns regarding civil liability in this type of treatment. Marketing campaigns of manufacturing companies often exaggerate promises of results, and ignore the lack of scientific evidence. Patients, as consumers, are protected by the Consumer Protection Code, whereas orthodontists are considered treatment providers. Therefore, they can be held liable for damage caused to patients, whether by subjective or objective fault. Objective: This article aims to identify the civil responsibilities of orthodontists and aligner manufacturing companies, by means of a literature review, providing basic legal guidance to help professionals protect themselves from possible lawsuits related to treatment with orthodontic aligners. Conclusions: The study highlights the importance of knowledge of legal notions in treatments with orthodontic aligners by orthodontists, who should legally safeguard themselves through individual written contracts, avoiding obligation of results. The genus Fridericia belongs to the family Bignoniaceae and comprises 60 species that occur in humid to dry forests, cerrado, and caatinga vegetation. It may be found from Mexico to Argentina and southern Brazil. This article reviews studies conducted on this genus, primarily focusing on its chemical composition and biological activities. Of the 60 known Fridericia species, only 14 have been the object of chemical and biological studies, accounting for 23% of the researched species. Eighty-four compounds, mainly flavonoids, terpenes, xanthones, and phenylethanoids, have been isolated from these species. The biological properties described in this review include antimicrobial, antioxidant, anti-inflammatory, hepatoprotective, antiviral, healing, antiproliferative, trypanocidal, and antileishmania activities. Gamma radiation is an effective method for microbial decontamination of plant materials. However, this treatment can induce changes in the chemical structure of plant constituents. Stem barks of Luehea ochrophylla were exposed to different doses of gamma radiation to evaluate decontamination efficiency and changes in chemical composition of plant material including phenolic fraction. The major microbial contaminants of non-irradiated samples were isolated and identified as the fungal species Eurotium chevalieri L. Mangin and Lecythophora decumbens. The dose of 5.0 kGy was effective to achieve total decontamination of the stem barks of L. ochrophylla. The formation of free radicals was verified in the plant material using electron paramagnetic resonance spectroscopy, and was supposed to be related to the trans-aconitic acid, a plant constituent. It was the only secondary metabolite to have its concentration significantly altered with radiation in phenolic fraction, as observed by liquid chromatography with diode array detector coupled to mass spectroscopy (LC-DAD-MS). The trans-aconitic acid was isolated and exposed to gamma radiation in aqueous medium. Its concentration decreased after exposition to a dose of 3.0 kGy, corroborating the supposition of its degradation. Abstract The objective of the current study was to carry out a survey of the main anatomopathological alterations in raising quails and evaluate possible interference of these in the bone tissue. To obtain the data, 23 quails were collected from farm in the central Serrana region of Espírito Santo. Necropsies with macroscopic descriptions, microbiological, coproparasitological, radiographic and histomorphometric tests were carried out. It was done data descriptive analysis and average comparision using Student T test. It was found that they presented lesions predominantly in the digestive system, followed by urinary and reproductive, and muscular system, were the altered color of the liver (47%) was the most frequent lesion. In the parasitological exams, it was found oocysts of Eimeira sp. (39.13%). In the microbiological exams, it was detected predominantly Escherichia coli (83%). Moderate osteopenia in quails, but the percentage of trabecular bone on bones was similar between healthy and diseased quails, without bone changes in histology. Microscopically, it was observed lung congestion as predominant lesion. RESUMO: Diversas técnicas são utilizadas para avaliar a qualidade das sementes; entretanto, a maioria deles é destrutiva e requer um tempo de execução considerável. O teste de raios X tem se mostrado uma técnica não destrutiva eficiente para a avaliação da qualidade de sementes. Este estudo teve como objetivo avaliar danos internos em sementes de ipê-roxo (Handroanthus impetiginosus) por meio de testes de raios X e investigar como esses danos afetam a germinação das sementes. Amostras de quatro lotes de sementes de ipê-roxo foram expostas à radiação e, posteriormente, as radiografias foram analisadas no software ImageJ, sendo as sementes divididas em quatro classes de acordo com sua morfologia interna: 1) sementes com cavidade embrionária preenchida com mais de 50%; 2) cavidade embrionária preenchida até 50%; 3) cavidade embrionária atacada por insetos; e 4) cavidade embrionária vazia. Em seguida, as sementes foram submetidas ao teste de germinação e, após 21 dias, foram classificadas como germinadas, não germinadas, mortas e plântulas normais e anormais. A análise radiográfica permitiu a identificação precisa de danos, como malformação embrionária e danos por insetos. O teste de raios X mostrou-se eficaz na avaliação da morfologia interna de sementes de ipê-roxo, com boa relação com a sua qualidade fisiológica. ABSTRACT: Several techniques are used to evaluate seed quality; however, most of them are destructive and require considerable execution time. The X-ray test has proven to be an efficient nondestructive technique for evaluating the quality of seeds. This study aimed to evaluate internal damage in ipê-roxo (Handroanthus impetiginosus) seeds through X-ray tests and to investigate how it affects seed germination. Samples from four lots of ipê-roxo seeds were exposed to radiation and, subsequently, the radiographs were analyzed using ImageJ software and the seeds divided into four classes based on their internal morphology: 1) seeds with an embryonic cavity filled with more than 50%; 2) an embryonic cavity filled up to 50%; 3) an embryonic cavity attacked by insects; and 4) an empty embryonic cavity. After that, the seeds were subjected to a germination test and, after 21 days, scored as germinated, nongerminated, dead, and normal and abnormal seedlings. The X-ray test proved to be effective in evaluating the internal morphology of ipê-roxo seeds and their relationship with physiological quality. Generalized anxiety disorder is a highly prevalent mental disorder. Previous data indicate that more than 18 million Brazilians suffer from this condition. Traditionally, generalized anxiety disorder has been considered a mild mental health disorder, despite its links to lower life expectancy, cardiovascular disease, and suicide. The aim of this article is to combine elements of systematic and critical reviews to produce a synthesis of the best evidence about generalized anxiety disorder treatment. Systematic reviews, meta-analyses, and randomized controlled trials were included. The descriptor used in the search was “generalized anxiety disorder,” which resulted in 4,860 articles and seven other studies, of which 59 were selected. Antidepressants and benzodiazepines were indicated, as was pregabalin, and atypical antipsychotics, such as quetiapine, have been studied. Individual cognitive behavior therapy (third wave) has proven effective. There is extensive literature on many effective treatments for generalized anxiety disorder. The present review summarizes the therapeutic possibilities, emphasizing those available in Brazil. Further studies are needed to compare other available medications, assess psychotherapies and new treatments in greater depth, as well as to assess the ideal duration of therapy. RESUMO Introdução: A remoção de resinas residuais é um procedimento rotineiro na clínica ortodôntica e de grande importância para o resultado final do tratamento. Objetivo: Avaliar os principais métodos para remoção de resinas residuais utilizados por ortodontistas e os principais motivos para a escolha desses métodos. Métodos: Um questionário composto por 21 perguntas: 6 relativas aos dados demográficos e 15 relativas aos métodos de remoção de resinas residuais (brocas ou alicates) foi enviado por e-mail para ortodontistas dos conselhos regionais de São Paulo e Rio de Janeiro entre os meses de abril e junho de 2023, com o total de 153 respostas. Resultados: A remoção de resinas residuais é realizada sempre com brocas em alta rotação para 44,7% da amostra e em baixa rotação para 28,7%; 61,3% utilizam irrigação. A broca carbide multilaminada é utilizada por 82,5% dos ortodontistas. Os alicates são sempre utilizados por 12,4%. O alicate removedor de resina com Wídea é utilizado em 39% dos casos. O uso da alta rotação foi justificado pelo menor tempo de trabalho e a escolha por alicates, justificada pelos menores danos ao esmalte. Conclusão: O método de remoção de resinas residual mais utilizado foi a broca carbide multilaminada em alta rotação com irrigação, justificado pelo menor tempo de trabalho. ABSTRACT Introduction: The removal of residual resins is a routine procedure in orthodontic clinics and of great importance to the final result of the treatment. Objective: To evaluate the main methods of residual resin removal used by orthodontists, and the main reasons for choosing these methods. Methods: A questionnaire consisting of 21 questions: 6 relating to demographic data and the other 15 relating to two methods used to remove residual resins (drills or pliers) was sent by e-mail to orthodontists registered with the Regional Councils of Dentistry of São Paulo and Rio de Janeiro (Brazil) within April and June, 2023. Questionnaires were sent back by 153 professionals. Results: Residual resin removal is always carried out with high speed drill for 44.7% of the sample, and with low speed drill for 28.7%; 61.3% use irrigation. The multi-laminate carbide bur is used by 82.5% of orthodontists. Pliers are always used by 12.4%. Resin-removing pliers with Widia are used in 39% of cases. The use of high speed was justified by the shorter working time, and the choice of pliers was justified by the smaller damage to the tooth enamel. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Resumo O retratamento do canal radicular e a fratura de pinos de fibra de vidro (FGP) podem exigir a remoção de FGPs. O objetivo deste estudo foi avaliar a influência das técnicas de remoção de FGP no tempo de remoção de FGP (min), no volume de dentina radicular removida (mm3), na alteração do ângulo do alinhamento do canal radicular (o) e na ocorrência de perfuração radicular usando análise de tomografia computadorizada de feixe cônico (CBCT). Trinta molares superiores extraídos foram divididos aleatoriamente em 3 grupos: Mic-Ult, FGP removido usando insertos ultrassônicos sob ampliação microscópica; Mic-DiB, FGP removido usando pontas de diamante sob ampliação microscópica, e Endo-G, FGP removido usando guia prototipado. Os CBCTs foram feitos após a abertura do acesso e após a remoção do FGP. O volume de dentina radicular, o alinhamento do canal radicular e a perfuração radicular foram realizados com os softwares InVesalius e Mimics. A alteração do ângulo do canal radicular após a remoção do FGP foi realizada usando o software ImageJ. O tempo para remoção do FGP foi calculado em min. Os dados foram analisados por análise de variância unidirecional seguida pelo teste Tukey HSD (α = 0,05). Endo-G resultou em significativamente menor remoção de dentina radicular (p< 0,001) e significativamente menor tempo para o procedimento do que Mic-Ult e Mic-DiB (p< 0,001). Mic-DiB resultou em 1 e Mic-Ult em 2 perfurações de dentina radicular. A remoção do pino FGP usando Mic-Ult e Mic-DiB exibiu alteração significativamente maior no alinhamento do canal radicular do que usando Endo-G (p<0,001). A remoção de FGP usando Endo-G resultou em maior preservação do alinhamento do canal original, preservou a estrutura da dentina radicular e exigiu menor tempo em comparação com a remoção de FGP usando Mic-DiB ou Mic-Ult sob ampliação microscópica. The aim of this study was to evaluate the effect of the FGP removal protocol on the time for FGP removal (min), the root dentin removed (mm3), the angle alteration of the root canal alignment (o), and the root perforation occurrence using cone-beam computed tomography (CBCT) analysis. Thirty extracted maxillary molars were randomly assigned to 3 groups (n = 10): Mic-Ult, FGP removed using ultrasonic inserts under microscopic magnification; Mic-DiB, FGP removed using diamond burs under microscopic magnification, and Endo-G, FGP removed using Endo-G. CBCTs were made after access opening and after FGP removal. The volume of root dentin, the root canal alignment and root perforation were performed using InVesalius and Mimics softwares. Root canal angle alteration after FGP removal was performed using ImageJ software. The time for FGP removal was calculated in min. Data were analysed by one-way analysis of variance was followed by Tukey HSD test (α = 0.05). Endo-G resulted in significantly lower root dentin removal (p< .001), and significantly less time than Mic-Ult and Mic-DiB (p< .001). Mic-DiB had 1 and Mic-Ult had 2 root dentin perforations. FGP post removal using Mic-Ult and Mic-DiB exhibited significantly greater alteration in the root canal alignment than that using Endo-G (p< .001). FGP removal using Endo-G exhibited better preservation of the original canal alignment, saved root dentin structure, and also required less time compared to FGP post removal using Mic-DiB or Mic-Ult under microscopic magnification. Abstract In the pediatric population, complex regional pain syndrome (CRPS) is a challenging condition that leads to chronic psychosocial dysfunction. This case is of a 12-year-old male patient, 50 kg, who, after twisting his right ankle, started to present intense pain. Without adequate diagnosis and treatment, his family sought a pain specialist. During the evaluation, a change in sensitivity and temperature was observed, besides hyperalgesia, allodynia, redness, edema, and motor dysfunction, being diagnosed with CRPS. RESUMO Introdução: O uso de modelos ortodônticos é essencial no diagnóstico e planejamento em Ortodontia. A demanda por modelos digitais e impressos aumentou entre os profissionais. Objetivo: O objetivo desse estudo foi avaliar a concordância das medições lineares horizontais e verticais feitas em modelos digitais, modelos impressos e medições diretas. Material e Métodos: Foram obtidos escaneamentos intrabucais de trinta indivíduos. As medições digitais foram realizadas usando os arquivos STL; as medições nos modelos impressos foram feitas usando um paquímetro digital, e as medições reais foram feitas diretamente na boca dos respectivos pacientes. Resultados: O teste t de uma amostra mostrou que não houve discrepância entre os conjuntos pareados de medições, com valor de 0 (p>0,05). A avaliação das medições foi feita com análise de Bland-Altman em pares. Os três métodos mostraram concordância nas medições horizontais e verticais. As análises de regressão linear mostraram que não houve viés proporcional nos dados (p>0,05). Conclusão: As medições horizontais e verticais avaliadas mostraram concordância quando feitas em modelos digitais, modelos impressos e diretamente na boca dos pacientes. ABSTRACT Introduction: The use of arch models is essential in diagnosis and planning in orthodontics. The demand for digital and printed models has increased among professionals. Objective: The aim of the study was to assess the agreement of horizontal and vertical linear measurements obtained from digital models, printed models, and direct measurements. Materials and Methods: Intraoral scans of 30 individuals were obtained. Digital measurements were performed using the STL files. From printed models, the measurements were done using a digital caliper, and the real measurements were done directly to the mouth of respective patients. Results: The one-sample t test showed no discrepancy between the paired sets of measurements, with the value of 0 (p>0.05). The evaluation of the measurements was done using Bland-Altman analysis in pairs. The three methods showed agreement in horizontal and vertical measurements. Linear regression analyses showed no proportional bias in the data (p>0.05). Abstract Objective: To evaluate the ability of the variables Muscle Mass percentage (%MM), Body Fat percentage (%BF), Phase Angle (PhA), and Standardized Phase Angle (SPhA) to predict overall survival rates in individuals with surgically treated HNC. Methods: A prospective cohort study of surgically treated HNC patients was carried out between 2017 and 2022 at a cancer referral center in Feira de Santana, Bahia. All subjects were seen by an experienced dietitian the week before surgery. During this visit, patients had their sociodemographic and clinical data collected, as well as anthropometric measurements (weight and height) and BIA variables. In this study, postoperative overall 5-year survival was defined as the time in months between the date of surgery and the date of death, regardless of the cause of death. Patients were followed up after surgery until death. Results: Seventy-eight older adult patients were consecutively included. Patients had a median age of 65.5-years, were mostly men (83.3%), and most had low education levels (62.3%) and low household income, ranging from zero (19.2%) to the minimum wage (60.2%). They were mostly alcohol drinkers (91%) and tobacco smokers (87.2%). Glottic cancer was diagnosed in 48.7% of patients, with 44.8% of cases with stage-IV disease. Forty (51.2%) deaths were recorded in the follow-up period, with a median survival time of 39 months. Variables %MM, %BF, and SPhA were not significantly different between groups (alive vs. dead patients). PhA was considered an independent predictor of overall survival, with an accuracy of 69% (95% CI 0.57 - 0.80). When comparing survival curves, patients with a preoperative PhA < 6.8° were more than twice as likely to die during follow-up (HR = 2.38; p = 0.02; 95% CI 1.14 - 4.97). Abstract Objectives: To review and provide evidence-based recommendations for the diagnosis and treatment of otosclerosis. Methods: Task force members were educated on knowledge synthesis methods, including electronic database search, review and selection of relevant citations, and critical appraisal of selected studies. Articles written in English or Portuguese on otosclerosis were eligible for inclusion. The American College of Physicians’ guideline grading system and the American Thyroid Association’s guideline criteria were used for critical appraisal of evidence and recommendations for therapeutic interventions. Results: The topics were divided into 2 parts: 1) Diagnosis – audiologic and radiologic; 2) Treatment – hearing AIDS, pharmacological therapy, stapes surgery, and implantable devices - bone-anchored devices, active middle ear implants, and Cochlear Implants (CI). Conclusions: The pathophysiology of otosclerosis has not yet been fully elucidated, but environmental factors and unidentified genes are likely to play a significant role in it. Women with otosclerosis are not at increased risk of worsening clinical condition due to the use of contraceptives or during pregnancy. Drug treatment has shown little benefit. If the patient does not want to undergo stapedotomy, the use of hearing aids is well indicated. Implantable systems should be indicated only in rare cases, and the CI should be indicated in cases of profound deafness.
