Abstract Objective: To describe the acute care of patients with bilateral thalamic infarctions in primary and advanced stroke centers in different regions of Colombia. Methods: Multicenter retrospective descriptive study of a series of cases of patients with an imaging diagnosis of bilateral thalamic infarction, in Colombia. 14 hospitals located in seven cities of the country participated. Results: 28 cases were registered, the mean age was 66.8 years with 53.6% men and 96.3% had a favorable previous mRs (0-2). Mean NIHSS at admission was 14 points, the most frequent clinical presentation was altered mental status in 82.1%. The most compromised structure was the bilateral anterior region of the thalamus in 12 patients (42.85%). The most common etiology was undetermined cause (39.3%). 1 patient underwent reperfusion therapy with intravenous thrombolysis (3.6%). Mean hospital stay was 19 days. Conclusions: Bilateral thalamic infarcts are a diagnostic challenge in the emergency department, due to their low reported incidence and the variable clinical presentations, stroke and research centers should be strengthened to contribute to clinical outcomes.
Resumen Objetivo: Describir la atención aguda de pacientes con infartos bilaterales del tálamo en centros de atención primaria y avanzada de ataque cerebrovascular en diferentes regiones de Colombia. Métodos: Estudio descriptivo retrospectivo multicéntrico de serie de casos de pacientes con diagnóstico por imágenes de infarto talámico bilateral, en Colombia. Participaron 14 hospitales ubicados en siete ciudades del país. Resultados: Se registraron 28 casos, la edad promedio fue de 66.8 años con un 53.6% genero masculino; el 96.3% tenía un mRs previo favorable (0-2). La escala NIHSS al ingreso promedio de 14 puntos; la presentación clínica más frecuente fue el estado mental alterado en un 82.1%. La estructura más comprometida fue la región anterior bilateral del tálamo en 12 pacientes (42.85%). La etiología más común fue de causa indeterminada (39.3%). Un paciente recibió terapia de reperfusión con trombólisis intravenosa (3.6%). Estancia hospitalaria promedio de 19 días. Conclusiones: Los infartos talámicos bilaterales representan un desafío diagnóstico en el departamento de emergencias, debido a su baja incidencia reportada y la variabilidad en las presentaciones clínicas. Se debe fortalecer la capacidad de los centros de atención de ataques cerebrovasculares e investigación, para contribuir a los resultados clínicos.

La Educación Fiscal: su impacto en la recaudación y el cumplimiento tributario

RESUMEN La educación fiscal, al orientar sus esfuerzos hacia la inculcación de valores y cualidades que fomenten la responsabilidad tributaria y desalienten las prácticas evasoras, se revela como un componente esencial para la transformación positiva de la percepción ciudadana hacia sus obligaciones tributarias. En este sentido, la presente investigación tiene como objetivo analizar el impacto de la educación fiscal en países latinoamericanos, centrándose en la recaudación tributaria, la disminución de la evasión y el nivel de cumplimiento de las obligaciones fiscales. Para lograrlo, se empleó un enfoque mixto. Los resultados obtenidos indican la necesidad de alejarse del enfoque tradicional centrado en sanciones, sugiriendo la adopción de un enfoque más educativo y participativo. Estrategias concretas, como la colaboración activa con instituciones educativas y la promoción de la cultura tributaria desde las etapas formativas tempranas, evidencian un compromiso palpable con la transformación cultural hacia una sociedad más consciente y responsable en materia fiscal.
ABSTRACT Tax education, by directing its efforts towards the inculcation of values and attitudes that promote fiscal responsibility and discourage evasion practices, is an essential component for the positive transformation of citizens' perception of their tax obligations. In this sense, the objective of this research is to analyze the impact of tax education in Latin American countries, focusing on tax collection, the reduction of tax evasion and the level of compliance with tax obligations. To achieve this, a mixed approach was used. The results obtained indicate the need to move away from the traditional approach focused on sanctions, suggesting the adoption of a more educational and participatory approach. Concrete strategies, such as active collaboration with educational institutions and the promotion of tax culture from the early formative stages, evidence a palpable commitment to cultural transformation towards a more tax conscious and responsible society.

Resumen: Los tumores suprarrenales en niños son frecuentemente neoplásicos y malignos, siendo la resección quirúrgica su primera opción de manejo. La cirugía mínimamente invasiva (CMI) ha demostrado ser una alternativa de aproximación segura, y se sugiere como alternativa preferente de abordaje. Objetivo: Comunicar los resultados quirúrgicos de pacientes con tumores suprarrenales abordados por CMI. Pacientes y Método: Estudio descriptivo retrospectivo de una cohorte de pacientes pediátricos portadores de tumor de la glándula suprarrenal sometidos a CMI en el período 2012-2023. Resultados: Se realizaron 15 CMI de la glándula suprarrenal en 14 pacientes, 60% hombres, edad promedio 1,8 años (rango: 5 días - 7 años). Los diagnósticos histológicos fueron neuroblastoma (n = 7), feocromocitoma (n = 3), ganglioneuroma (n = 2), secuestro extrapulmonar (n = 1), teratoma maduro (n = 1), y neoplasia cortical adrenal de potencial maligno incierto (n = 1). Se realizaron 14 suprarrenalectomías y 1 biopsia, todos por cirugía laparoscópica, sin necesidad de conversión a cirugía abierta. No se registraron complicaciones perioperatorias mayores a Clavien-Dindo grado 2, solo 2 pacientes requirieron transfusión de glóbulos rojos y no se registró mortalidad perioperatoria. La media del diámetro tumoral mayor fue de 3,7 cm. La mediana de estadía hospitalaria fue de 1 día. La media de seguimiento fue de 29 meses. Conclusiones: La técnica laparoscópica ha demostrado ser un abordaje seguro en pacientes pediátricos con tumores suprarrenales, ofreciendo ventajas en términos de estadía hospitalaria y complicaciones perioperatorias.
Abstract: Adrenal tumors in children are frequently neoplastic and malignant, and surgical resection is the first management option. Minimally invasive surgery (MIS) has proven to be a safe management alternative and is suggested as a preferred alternative approach. Objective: To report the surgical outcomes of patients with adrenal tumors treated by MIS. Patients and Method: Retrospective descriptive study of a cohort of pediatric patients with adrenal gland tumors undergoing MIS in the period 2012 - 2023. Results: 15 MIS of the adrenal gland were performed in 14 patients, 60% were male, mean age 1.8 years (range: 5 days - 7 years). Histological diagnoses included neuroblastoma (n = 7), pheochromocytoma (n = 3), ganglioneuroma (n = 2), extrapulmonary sequestration (n = 1), mature teratoma (n = 1), and adrenal cortical neoplasm of uncertain malignant potential (n = 1). 14 adrenalectomies and one biopsy were performed, all via laparoscopic surgery, with no need for conversion to open surgery. No perioperative complications greater than Clavien-Dindo grade II were recorded; only 2 patients required red blood cell transfusions and there was no perioperative mortality. The mean tumor diameter was 3.7 cm. The median hospital stay was 1 day. The mean follow-up period was 29 months. Conclusions: The laparoscopic technique has proven to be a safe approach in pediatric patients with adrenal tumors, offering advantages in terms of hospital stay and perioperative complications.

Abstract Soil humidity is an important environmental factor that affect composition, diversity and structure of the vegetal communities. The objective of the present study was to determine the diversity changes that occur over an environmental gradient limited by the source of water of the oasis San Ignacio and the adjoining desert of Baja California Sur, Mexico. The study area was divided in nine strips of 100 m width. Over them, there were randomly selected 140 plots of 500, 20 and 1 m2 to register the vegetal species present in the tree, shrub and herbaceous layers, respectively. Species richness, Shannon-Weiner diversity, Simpson dominance, Pielou equity and true species value indexes were estimated and compared between established strips. The qualitative index of Sörensen was estimated to evaluate species similitude and similar communities were grouped applying dendrograms. The species richness of the herbaceous and shrub layers falls abruptly at 520 and 720 m far away from the source of water. Species richness and diversity in the tree layer presented an undefined tendency. The similitude index allowed to concentrate the herbaceous, shrub and tree vegetation layers in 4, 5 and 3 clusters. This diversity analysis suggests to determine that herbaceous and shrub species are more sensitive to humidity changes than arboreal species.
Resumen La humedad del suelo es un factor ambiental importante que afecta la composición, diversidad y estructura de las comunidades vegetales. Considerando lo anterior, el objetivo de este estudio fue determinar los cambios de diversidad vegetal que ocurren entre la fuente de humedad del Oasis San Ignacio y el desierto circundante de Baja California Sur. El área de estudio se dividió en nueve franjas de 100 m de ancho, sobre las cuales se seleccionaron aleatoriamente 140 sitios de 500, 20 y 1 m2 para registrar las especies vegetales de los estratos arbóreos, arbustivos y herbáceos, respectivamente. Se estimaron los índices de riqueza, diversidad de Shannon-Weiner, dominancia de Simpson, equidad de Pielou y valor verdadero de especies y se evaluaron los cambios entre las franjas establecidas. A su vez, se estimó el índice cualitativo de similitud de especies de Sörensen, generándose dendrogramas para agrupar comunidades vegetales similares establecidas sobre el gradiente estudiado. La riqueza y diversidad de las especies del estrato herbáceo y arbústivo decrecen abruptamente a los 520 y 720 m del cuerpo de agua, respectivamente. La riqueza y diversidad de la vegetación del estrato arbóreo no presenta una tendencia definida. El índice de similitud permitió concentrar las especies del estrato herbáceo, arbustivo y arbóreo en 4, 5 y 3 grupos, respectivamente. Los análisis de diversidad, sugieren que las especies herbáceas y arbustivas son más sensibles a los cambios de humedad que las arbóreas.

Abstract Background: Loxoscelism refers to a set of clinical manifestations caused by the bite of spiders from the Loxosceles genus. The classic clinical symptoms are characterized by an intense inflammatory reaction at the bite site followed by local necrosis and can be classified as cutaneous loxoscelism. This cutaneous form presents difficult healing, and the proposed treatments are not specific or effective. This study aimed to evaluate the protective effect of mesenchymal stromal cells-derived secretome on dermonecrosis induced by Loxosceles intermedia spider venom in rabbits. Methods: Sixteen rabbits were distributed into four groups (n = 4). Except for group 1 (G1), which received only PBS, the other three groups (G2, G3, and G4) were initially challenged with 10 μg of L. intermedia venom, diluted in 100 μL of NaCl 0.9%, by intradermic injection in the interscapular region. Thirty minutes after the challenge all groups were treated with secretome, except for group 2. Group 1 (G1-control group) received intradermal injection (ID) of 60 μg of secretome in 0.15 M PBS; Group 2 (G2) received 0.9% NaCl via ID; Group 3 (G3) received 60 μg of secretome, via ID and Group 4 (G4), received 60 μg of secretome by intravenous route. Rabbits were evaluated daily and after 15 days were euthanized, necropsied and skin samples around the necrotic lesions were collected for histological analysis. Results: Rabbits of G1 did not present edema, erythema, hemorrhagic halo, or necrosis. In animals from G2, G3, and G4, edema appeared after 6h. However, minor edema was observed in the animals of G2 and G3. Hemorrhagic halo was observed in animals, six hours and three days after, on G2, G3, and G4. Macroscopically, in G4, only one animal out of four had a lesion that evolved into a dermonecrotic wound. No changes were observed in the skin of the animals of G1, by microscopic evaluation. All animals challenged with L. intermedia venom showed similar alterations, such as necrosis and heterophilic infiltration. However, animals from G4 showed fibroblast activation, early development of connective tissue, neovascularization, and tissue re-epithelialization, indicating a more prominent healing process. Conclusion: These results suggest that secretome from mesenchymal stromal cells cultured in a xeno-free and human component-free culture media can be promising to treat dermonecrosis caused after Loxosceles spiders bite envenoming. RESUMO O presente trabalho teve como objetivo identificar áreas de aptidão agroclimática para o cultivo do Eucalyptus urophyllano MATOPIBA. As classes de aptidão agroclimática para a espécie foram determinadas considerando as necessidades de temperatura e hídrica, sendo adotadas três diferentes valores de Capacidade de Água Disponível - CAD, utilizando um Sistema de Informações Geográficas - SIG. As classes de aptidão agroclimática para todos os CADs analisados identificaram a existência de pleno cultivo em algumas regiões, porém com predomínio de áreas restritas. As zonas aptas predominaram nas regiões serranas da Bahia, sul do Maranhão e noroeste do Tocantins. ABSTRACT This work aimed to identify areas of agroclimatic aptitude for Eucalyptus urophylla cultivation in MATOPIBA. The agroclimatic aptitude classes for the species were determined considering the temperature and water needs, being adopted three different values of available water capacity (AWC), using a geographic information system (GIS). The agroclimatic aptitude classes for all AWCs analyzed identified the existence of full cultivation in some regions but with a predominance of restricted areas. The fit zones predominated in the mountainous regions of Bahia, southern Maranhão and northwest of Tocantins. There was a percentage increase in the aptitude area favorable for E. urophylla (apt) cultivation, from 7.68% to 29.13%, considering the AWC values of 100 and 220 mm, respectively. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract: The relationship between the Canary Islands and Venezuela developed over years, constantly mediated by the Atlantic Ocean and marked by the comings and goings of migrants and travelers, sometimes from Spain to Venezuela, at other times from Venezuela to Spain. These historic intercontinental relationships are well documented in the works of several Venezuelan authors, configuring the Canary Islands as a non-place (Augé, 2000), marked by movement and transience. The example examined here is Arena Negra (2013), by Juan Carlos Méndez Guédez, a narrative that explores the metaforicity of the island and its subjective repercussions on the main character, providing a representative view of the Canary Islands as a fractal space based on the studies of insularity by Antonio Benítez Rojo (1989), Edouard Glissant (1990, 2008) and Ottmar Ette (2016, 2018)
Resumen: La relación entre las Islas Canarias y Venezuela fue construyéndose a lo largo de los años siempre mediada por el Océano Atlántico y marcada por los tránsitos de migrantes y viajantes, no solo en el sentido España-Venezuela, como también Venezuela-España. Encontramos la representación de estas relaciones históricas intercontinentales en la literatura de muchos escritores venezolanos a través de la figuración del espacio de las Islas Canarias como un no-lugar (Augé, 2000), marcado por el movimiento y la transitoriedad. Abstract Introduction: Sustainable management of oases needs knowledge of the conditions of composition and diversity of the species living there. Objective: Estimating and comparing the diversity and structure of vegetation in the tree, shrub and herbaceous strata of the San Ignacio oasis, Baja California Sur, Mexico. Materials and methods: A total of 140 circular sites (500 m2) were randomly located, where 20 m2 and 1 m2 sites were nested to record the vegetation in the herbaceous, shrub and tree strata. Species richness, Shannon-Wiener, Simpson, Pielou and species importance value indices were used to estimate plant diversity in the strata and to characterize their structure. Results and discussion: A total of 53 taxa were distributed in 17 orders, 25 families and 47 genera. Richness, diversity and homogeneity were higher in the herbaceous stratum (S = 41, H´ = 3.21, J´ = 8.64, respectively) and lower in the arboreal stratum (S = 21, H´ = 2.01, J´= 0.688). Regarding the importance value index, the species with the highest ecological weight was Prosopis articulata S. Watson (33.35 %) for the herbaceous stratum, Cylindropuntia alcahes (F. A. B. Weber) F. M. Kunth (65.60 %) for shrub and Phoenix dactylifera L. (92.94 %) for the arboreal stratum. Conclusions: Vegetation richness and diversity were higher in the herbaceous stratum. This study represents a baseline of indicators useful for the evaluation of changes generated by natural or anthropogenic events in the San Ignacio oasis.
Resumen Introducción: El manejo sustentable de los oasis requiere conocimiento de las condiciones de composición y diversidad de las especies que lo habitan. Objetivo: Estimar y comparar la diversidad y estructura de la vegetación en los estratos arbóreo, arbustivo y herbáceo del oasis San Ignacio, Baja California Sur, México. Materiales y métodos: Se ubicaron 140 sitios circulares (500 m2) de manera aleatoria, sobre los cuales se anidaron sitios de 20 m2 y 1 m2 para el registro de la vegetación en los estratos herbáceo, arbustivo y arbóreo. Los índices riqueza de especies, Shannon-Wiener, Simpson, Pielou y valor de importancia de las especies se utilizaron para estimar la diversidad vegetal en los estratos y caracterizar su estructura. Resultados y discusión: Se contabilizaron 53 taxones distribuidos en 17 órdenes, 25 familias y 47 géneros. La riqueza, diversidad y homogeneidad fueron mayores en el estrato herbáceo (S = 41, H´ = 3.21, J´ = 8.64, respectivamente) y menores en el estrato arbóreo (S = 21, H´ = 2.01, J´= 0.688). En lo que respecta al índice del valor de importancia, la especie con mayor peso ecológico en el estrato herbáceo fue Prosopis articulata S. Watson (33.35 %), en el arbustivo Cylindropuntia alcahes (F. A. B. Weber) F. M. Kunth (65.60 %) y en el arbóreo Phoenix dactylifera L. (92.94 %). Conclusiones: La riqueza y diversidad de la vegetación fueron superiores en el estrato herbáceo. Este estudio representa una línea base de indicadores útil para la evaluación de los cambios generados por eventos naturales o antropogénicos en el oasis San Ignacio.

Hypertension is one of the primary modifiable risk factors for morbidity and mortality worldwide, being a major risk factor for coronary artery disease, stroke, and kidney failure. Furthermore, it is highly prevalent, affecting more than one-third of the global population.
Blood pressure measurement is a MANDATORY procedure in any medical care setting and is carried out by various healthcare professionals. However, it is still commonly performed without the necessary technical care. Since the diagnosis relies on blood pressure measurement, it is clear how important it is to handle the techniques, methods, and equipment used in its execution with care.
It should be emphasized that once the diagnosis is made, all short-term, medium-term, and long-term investigations and treatments are based on the results of blood pressure measurement. Therefore, improper techniques and/or equipment can lead to incorrect diagnoses, either underestimating or overestimating values, resulting in inappropriate actions and significant health and economic losses for individuals and nations.
Once the correct diagnosis is made, as knowledge of the importance of proper treatment advances, with the adoption of more detailed normal values and careful treatment objectives towards achieving stricter blood pressure goals, the importance of precision in blood pressure measurement is also reinforced.
Blood pressure measurement (described below) is usually performed using the traditional method, the so-called casual or office measurement. Over time, alternatives have been added to it, through the use of semi-automatic or automatic devices by the patients themselves, in waiting rooms or outside the office, in their own homes, or in public spaces. A step further was taken with the use of semi-automatic devices equipped with memory that allow sequential measurements outside the office (ABPM; or HBPM) and other automatic devices that allow programmed measurements over longer periods (HBPM).
Some aspects of blood pressure measurement can interfere with obtaining reliable results and, consequently, cause harm in decision-making. These include the importance of using average values, the variation in blood pressure during the day, and short-term variability. These aspects have encouraged the performance of a greater number of measurements in various situations, and different guidelines have advocated the use of equipment that promotes these actions. Devices that perform HBPM or ABPM, which, in addition to allowing greater precision, when used together, detect white coat hypertension (WCH), masked hypertension (MH), sleep blood pressure alterations, and resistant hypertension (RHT) (defined in Chapter 2 of this guideline), are gaining more and more importance.
Taking these details into account, we must emphasize that information related to diagnosis, classification, and goal setting is still based on office blood pressure measurement, and for this reason, all attention must be given to the proper execution of this procedure.
La hipertensión arterial (HTA) es uno de los principales factores de riesgo modificables para la morbilidad y mortalidad en todo el mundo, siendo uno de los mayores factores de riesgo para la enfermedad de las arterias coronarias, el accidente cerebrovascular (ACV) y la insuficiencia renal. Además, es altamente prevalente y afecta a más de un tercio de la población mundial.
La medición de la presión arterial (PA) es un procedimiento OBLIGATORIO en cualquier atención médica o realizado por diferentes profesionales de la salud. Sin embargo, todavía se realiza comúnmente sin los cuidados técnicos necesarios. Dado que el diagnóstico se basa en la medición de la PA, es claro el cuidado que debe haber con las técnicas, los métodos y los equipos utilizados en su realización.
Debemos enfatizar que una vez realizado el diagnóstico, todas las investigaciones y tratamientos a corto, mediano y largo plazo se basan en los resultados de la medición de la PA. Por lo tanto, las técnicas y/o equipos inadecuados pueden llevar a diagnósticos incorrectos, subestimando o sobreestimando valores y resultando en conductas inadecuadas y pérdidas significativas para la salud y la economía de las personas y las naciones.
Una vez realizado el diagnóstico correcto, a medida que avanza el conocimiento sobre la importancia del tratamiento adecuado, con la adopción de valores de normalidad más detallados y objetivos de tratamiento más cuidadosos hacia metas de PA más estrictas, también se refuerza la importancia de la precisión en la medición de la PA.
La medición de la PA (descrita a continuación) generalmente se realiza mediante el método tradicional, la llamada medición casual o de consultorio. Con el tiempo, se han agregado alternativas a través del uso de dispositivos semiautomáticos o automáticos por parte del propio paciente, en salas de espera o fuera del consultorio, en su propia residencia o en espacios públicos. Se dio un paso más con el uso de dispositivos semiautomáticos equipados con memoria que permiten mediciones secuenciales fuera del consultorio (AMPA; o MRPA) y otros automáticos que permiten mediciones programadas durante períodos más largos (MAPA).
Algunos aspectos en la medición de la PA pueden interferir en la obtención de resultados confiables y, en consecuencia, causar daños en las decisiones a tomar. Estos incluyen la importancia de usar valores promedio, la variación de la PA durante el día y la variabilidad a corto plazo. Estos aspectos han alentado la realización de un mayor número de mediciones en diversas situaciones, y diferentes pautas han abogado por el uso de equipos que promuevan estas acciones. Los dispositivos que realizan MRPA o MAPA, que además de permitir una mayor precisión, cuando se usan juntos, detectan la hipertensión de bata blanca (HBB), la hipertensión enmascarada (HM), las alteraciones de la PA durante el sueño y la hipertensión resistente (HR) (definida en el Capítulo 2 de esta guía), están ganando cada vez más importancia.
Teniendo en cuenta estos detalles, debemos enfatizar que la información relacionada con el diagnóstico, la clasificación y el establecimiento de objetivos todavía se basa en la medición de la presión arterial en el consultorio, y por esta razón, se debe prestar toda la atención a la ejecución adecuada de este procedimiento.
A hipertensão arterial (HA) é um dos principais fatores de risco modificáveis para morbidade e mortalidade em todo o mundo, sendo um dos maiores fatores de risco para doença arterial coronária, acidente vascular cerebral (AVC) e insuficiência renal. Além disso, é altamente prevalente e atinge mais de um terço da população mundial.
A medida da PA é procedimento OBRIGATÓRIO em qualquer atendimento médico ou realizado por diferentes profissionais de saúde. Contudo, ainda é comumente realizada sem os cuidados técnicos necessários. Como o diagnóstico se baseia na medida da PA, fica claro o cuidado que deve haver com as técnicas, os métodos e os equipamentos utilizados na sua realização.
Deve-se reforçar que, feito o diagnóstico, toda a investigação e os tratamentos de curto, médio e longo prazos são feitos com base nos resultados da medida da PA. Assim, técnicas e/ou equipamentos inadequados podem levar a diagnósticos incorretos, tanto subestimando quanto superestimando valores e levando a condutas inadequadas e grandes prejuízos à saúde e à economia das pessoas e das nações.
Uma vez feito o diagnóstico correto, na medida em que avança o conhecimento da importância do tratamento adequado, com a adoção de valores de normalidade mais detalhados e com objetivos de tratamento mais cuidadosos no sentido do alcance de metas de PA mais rigorosas, fica também reforçada a importância da precisão na medida da PA.
A medida da PA (descrita a seguir) é habitualmente feita pelo método tradicional, a assim chamada medida casual ou de consultório. Ao longo do tempo, foram agregadas alternativas a ela, mediante o uso de equipamentos semiautomáticos ou automáticos pelo próprio paciente, nas salas de espera ou fora do consultório, em sua própria residência ou em espaços públicos. Um passo adiante foi dado com o uso de equipamentos semiautomáticos providos de memória que permitem medidas sequenciais fora do consultório (AMPA; ou MRPA) e outros automáticos que permitem medidas programadas por períodos mais prolongados (MAPA).
Alguns aspectos na medida da PA podem interferir na obtenção de resultados fidedignos e, consequentemente, causar prejuízo nas condutas a serem tomadas. Entre eles, estão: a importância de serem utilizados valores médios, a variação da PA durante o dia e a variabilidade a curto prazo. Esses aspectos têm estimulado a realização de maior número de medidas em diversas situações, e as diferentes diretrizes têm preconizado o uso de equipamentos que favoreçam essas ações. Ganham cada vez mais espaço os equipamentos que realizam MRPA ou MAPA, que, além de permitirem maior precisão, se empregados em conjunto, detectam a HA do avental branco (HAB), HA mascarada (HM), alterações da PA no sono e HA resistente (HAR) (definidos no Capítulo 2 desta diretriz).
Resguardados esses detalhes, devemos ressaltar que as informações relacionadas a diagnóstico, classificação e estabelecimento de metas ainda são baseadas na medida da PA de consultório e, por esse motivo, toda a atenção deve ser dada à realização desse procedimento.

Radial Nerve Supracondylar Block Versus Fracture Hematoma Block. Comparison of Their Efficacy in Cases of Fractures of the Distal Third of the Radius

Resumo Objetivo O estudo compara a eficácia analgésica de duas técnicas para realizar redução incruenta: o bloqueio de hematoma da fratura e o bloqueio supracondilar de nervo radial. Métodos Quarenta pacientes com fraturas do terço distal do rádio, que necessitassem redução, foram selecionados em um ensaio clínico quasi-randomizado, para receber uma das técnicas anestésicas. Todos os pacientes assinaram o termo de consentimento ou assentimento, com exceção daqueles que não desejassem participar do estudo, tivessem lesão neurológica, com contraindicação ao procedimento na sala de emergências, ou com contraindicação ao uso da lidocaína. Para aferir a analgesia foi utilizada a escala numérica da dor em quatro momentos distintos: pré-bloqueio, pós-bloqueio, durante a redução e após a redução; em seguida, foram calculadas três diferenças: a primeira entre antes e após o bloqueio; a segunda entre durante a redução e após o bloqueio; e a terceira entre antes do bloqueio e após a redução. Resultados Os grupos do bloqueio de hematoma de fratura e bloqueio supracondilar apresentaram respectivamente os seguintes valores médios: 3.90 (1–10) e 3.50 (-6–10) na diferença 1; 4.35 (-5–10) e 5.00 (-3–10) na diferença 2; e 4.65 (1–10) e 3.80 (-3–10) na diferença 3. Abstract Objective The present study compares the analgesic efficacy of two techniques to perform non-surgical reduction: fracture hematoma block and radial nerve supracondylar block. Methods Forty patients with fractures of the distal third of the radius, who required reduction, were selected in a quasi-randomized clinical trial to receive one of the anesthetic techniques. All patients signed the informed consent form, except for those who did not wish to participate in the study, had neurological injury, had contraindication to the procedure in the emergency room, or with contraindication to the use of lidocaine. To measure analgesia, the numerical pain rate scale was used at four different moments: preblock, postblock, during reduction, and after reduction; then three differences were calculated: the first between before and after blocking; the second between during reduction and after blockade; and the third between before blocking and after reduction. Results The fracture hematoma and supracondylar block groups showed the following mean values, respectively: 3.90 (1–10) and 3.50 (-6–10) in difference 1; 4.35 (-5–10) and 5.00 (-3–10) in difference 2; and 4.65 (1–10) and 3.80 (-3–10) in difference 3. RESUMEN El objetivo de este estudio fue establecer el nivel de percepción del clima laboral en colaboradores de una institución pública peruana. Para ello, se desarrolló con enfoque cuantitativo, diseño no experimental, de tipo básica y nivel descriptivo. La población fue de tipo censal de 88 colaboradores de una institución del sector de la educación referido a la gestión educativa, ubicada en la ciudad de Lima, la técnica de recolección de datos fue la encuesta y el instrumento el cuestionario tipo escala de Likert de cinco alternativas de respuestas. El instrumento se validó mediante el juicio de expertos, mientras que la confiabilidad fue calculada mediante el coeficiente Alfa de Cronbach. Los hallazgos indican que, la variable clima laboral predominó la percepción del nivel regular, seguido del nivel malo, y finalmente el nivel bueno; mientras queen la mayoría de las dimensiones prevaleció el nivel regular, seguido del nivel malo, a excepción de la dimensión motivación donde primó el nivel malo, seguido del nivel regular. Concluyendoque existe debilidad en el clima laboral en los colaboradores de la institución estudiada; se sugiere la aplicación de estrategias, que permitan la mejora del clima laboral en la referida organización.
ABSTRACT The objective of this study was to establish the level of perception of the work environment in collaborators of a Peruvian public institution. For this, it was developed with a quantitative approach, a non-experimental design, of a basic type and a descriptive level. The population was of census type of 88 collaborators of an institution of the education sector referred to educational management, located in the city of Lima, the data collection technique was the survey and the instrument was the Likert scale questionnaire of five answer alternatives. The instrument was validated through expert judgment, while reliability was calculated using Cronbach's Alpha coefficient. The findings indicate that the work climate variable predominated the perception of the regular level, followed by the bad level, and finally the good level; while in most of the dimensions the regular level prevailed, followed by the bad level, with the exception of the motivation dimension where the bad level prevailed, followed by the regular level. Concluding that there is weakness in the work environment in the collaborators of the studied institution; the application of strategies is suggested, which allow the improvement of the work environment in the aforementioned organization.
