Percepção materna do cuidado à recém-nascidos com baixo peso: revisão sistemática qualitativa

Resumo: Introdução: O peso ao nascer tem grande influência sobre o desenvolvimento global da criança, que por sua vez sofre influência direta dos cuidados maternos. Objetivo: Identificar na literatura científica a percepção materna sobre o cuidado de recém-nascidos com baixo peso. Métodos: Trata-se de uma revisão sistemática de estudos qualitativos conduzida em conformidade com as diretrizes metodológicas do Instituto Joanna Briggs. A pesquisa foi realizada entre abril de 2021 e março de 2023 nas bases de dados PubMed, MedLine, BVS, Scielo, EMBASE e Web of Science, com os descritores: “percepção materna”, “baixo peso ao nascer” e “cuidado da criança”, em diferentes combinações. Incluíram-se estudos em português, inglês e espanhol, publicados entre 2013 e 2023, que tratassem do cuidado materno de recém-nascidos com baixo peso no contexto hospitalar ou domiciliar. Os resultados foram categorizados de acordo com a análise de conteúdo. Resultados: Foram identificados 1129 estudos, dos quais apenas 11 foram incluídos nesta revisão, nas seguintes categorias: sentimentos maternos, necessidades e desafios do cuidado, aleitamento materno, vínculo mãe-bebê e apoio social. Considerações finais: Mães de recém-nascidos com baixo peso experimentam insegurança e problemas gerais de adaptação que afetam os cuidados para suprimento das necessidades essenciais da criança. Apoio familiar e de profissionais de saúde podem reforçar a segurança materna e contribuir para o cuidado integral de recém-nascidos com baixo peso.
Abstract: Introduction: Birth weight has a great influence on the child’s global development, which in turn is directly influenced by maternal care. Objective: To identify in the scientific literature the mother’s perception about the care of children born with low birth weight. Methods: This is a systematic review of qualitative studies conducted in accordance with the methodological guidelines of the Joanna Briggs Institute. The research was carried out between April/2021 and March/2023 in the databases: PubMed, Medline, VHL, SciELO, EMBASE and Web of Science, with the descriptors: maternal perception, low birth weight and child care, in different combinations. Studies in Portuguese, English and Spanish, published between 2013 and 2023, that addressed the maternal care of children born with low birth weight in the hospital or home context were included. Results were categorized according to content analysis. Results: 1129 studies were identified, of which only 11 were included in this review, in the following categories: maternal feelings, care needs and challenges, breastfeeding, mother-infant bonding and social support. Final considerations: Mothers of children born with low birth weight experience insecurity and general adaptation problems that affect care to meet the child’s essential needs. Family support and support from health professionals can reinforce maternal security and contribute to the comprehensive health care of low birth weight children.
Resumen: Introducción: El peso al nacer tiene una gran influencia en el desarrollo global del niño, el cual a su vez está directamente influido por los cuidados maternos. Objetivo: Identificar en la literatura científica la percepción de la madre sobre el cuidado del niño nacido con bajo peso. Métodos: Esta es una revisión sistemática de estudios cualitativos realizados de acuerdo con las pautas metodológicas del Instituto Joanna Briggs. La investigación fue realizada entre abril de 2021 y marzo de 2023 en las bases de datos PubMed, MedLine, BVS, Scielo, EMBASE y Web of Science, con los descriptores: “percepción materna”, “bajo peso al nacer” y “cuidado del niño”, en diferentes combinaciones. Se incluyeron estudios en portugués, inglés y español, publicados entre 2013 y 2023, que abordaran el cuidado materno de niños nacidos con bajo peso en el contexto hospitalario o domiciliario. Los resultados se clasificaron según el análisis de contenido. Resultados: Se identificaron 1129 estudios, de los cuales solo 11 fueron incluidos en esta revisión, en las siguientes categorías: sentimientos maternos, necesidades y desafíos de cuidado, lactancia materna, vínculo madre-hijo y apoyo social. Consideraciones finales: Las madres de niños nacidos con bajo peso experimentan inseguridad y problemas generales de adaptación que afectan los cuidados para satisfacer las necesidades esenciales del niño. El apoyo familiar y el apoyo de los profesionales de la salud pueden reforzar la seguridad materna y contribuir a la atención integral en salud de los niños nacidos con bajo peso.

RESUMO Objetivo: analisar a duração e qualidade do sono em profissionais de enfermagem que trabalham em turnos. Método: pesquisa transversal, analítica, realizada entre setembro de 2017 e abril de 2018, em hospital público do Sul do Brasil, com equipe de enfermagem. Foram utilizados questionário sócio-ocupacional e de sintomas de saúde, Escala de Sonolência de Epworth e Índice de Qualidade do Sono de Pittsburgh. Os dados são apresentados como estatística descritiva e inferencial, análise bivariada e regressão logística binária. Resultados: participaram 308 profissionais de enfermagem com predomínio de sono prolongado, ausência de sonolência e má qualidade do sono. Sono de curta duração (<6h) foi associado a turno diurno e má qualidade do sono. Qualidade do sono esteve associada à presença de sonolência diurna excessiva e trabalho diurno. Conclusão: turno de trabalho, insônia e cefaleia foram os principais fatores relacionados ao sono de curta duração para profissionais de enfermagem. Os resultados podem justificar o desenvolvimento de pesquisas de intervenção para a saúde do trabalhador. ABSTRACT Objective: to analyze sleep duration and sleep quality in nursing professionals who work in shifts. Method: this is a cross-sectional, analytical research, carried out between September 2017 and April 2018, at a public hospital in southern Brazil, with the nursing team. A socio-occupational and health symptoms questionnaire, the Epworth Sleepiness Scale, and the Pittsburgh Sleep Quality Index were used. Data are presented as descriptive and inferential statistics, bivariate analysis, and binary logistic regression. Results: participants were 308 nursing professionals with a predominance of long-term sleep, absence of drowsiness, and poor sleep quality. Short-term sleep (<6h) was associated with day shift and poor sleep quality. Sleep quality was associated with presence excessive daytime sleepiness and work day shift. Conclusion: work shift, insomnia and headache were the main factors related short-term sleep for nursing professionals. The results may justify the development of intervention research for workers’ health. La duración corta del sueño (<6 h) se asoció con turnos de día y mala calidad del sueño. La calidad del sueño se asoció con la presencia de somnolencia diurna excesiva y con el trabajo diurno. Conclusión: los turnos de trabajo, el insomnio y la cefalea fueron los principales factores relacionados con la falta de sueño de los profesionales de enfermería. ABSTRACT Objective To evaluate the direct and indirect effects of chronic pain, depressive symptoms, frailty components, and functional disability through a pathway analysis approach in a sample of community-dwelling older adults. Methods Data of 419 participants were cross-sectionally evaluated for the presence of depressive symptoms (Geriatric Depression Scale [15 items]), physical frailty components (phenotype criteria), chronic pain, and limitations in performing instrumental activities of daily living (functional disability scale by Lawton and Brody). Structural equation modeling via path analysis was used to explore the direct and indirect effects among these four variables. Statistical significance was set at p<0.05. Results Of the total participants, 69.8% were women and 59.3% had low education (1-4 years); the mean age was 80.3±4.6 years. Chronic pain and depressive symptoms were directly related and were associated to frailty. The number of frailty components and depressive symptoms were directly associated with functional disability. Frailty had an indirect effect on the association between chronic pain, depressive symptoms, and functional disabilities. Abstract This study aimed to evaluate the contribution of oral and maxillofacial pathology laboratories (OMPLs) in Brazilian public universities to the diagnosis of lip, oral cavity, and oropharyngeal squamous cell carcinoma (SCC). A cross-sectional study was performed using biopsy records from a consortium of sixteen public OMPLs from all regions of Brazil (North, Northeast, Central-West, Southeast, and South). Clinical and demographic data of patients diagnosed with lip, oral cavity, and oropharyngeal SCC between 2010 and 2019 were collected from the patients’ histopathological records. Of the 120,010 oral and maxillofacial biopsies (2010-2019), 6.9% (8,321 cases) were diagnosed as lip (0.8%, 951 cases), oral cavity (4.9%, 5,971 cases), and oropharyngeal (1.2%, 1,399 cases) SCCs. Most cases were from Brazil’s Southeast (64.5%), where six of the OMPLs analyzed are located. The predominant profile of patients with lip and oral cavity SCC was Caucasian men, with a mean age over 60 years, low schooling level, and a previous history of heavy tobacco consumption. In the oropharyngeal group, the majority were non-Caucasian men, with a mean age under 60 years, had a low education level, and were former/current tobacco and alcohol users. According to data from the Brazilian National Cancer Institute, approximately 9.9% of the total lip, oral cavity, and oropharyngeal SCCs reported over the last decade in Brazil may have been diagnosed at the OMPLs included in the current study. Therefore, this data confirms the contribution of public OMPLs with respect to the important diagnostic support they provide to the oral healthcare services extended by the Brazilian Public Health System. Resumo Este artigo analisa biografias de três mulheres ribeirinhas que têm como base investigativa os seus papéis sociais no cuidar através da medicina tradicional. Seus ‘relatos de vida’ transparecem as suas próprias percepções na construção e reconstrução de suas práticas de cura no enfrentamento de doenças, sempre levando em consideração as interações construídas com o meio e com tradições religiosas – dentre as quais se destacam a pajelança, o catolicismo e o pentecostalismo. As ilhas belenenses do Murutucum e do Combu são os ambientes antropogênicos nos quais as especialistas residem e atuam, ecossistemas de deságue hidrográfico de diversos rios paraenses e uma pequena parte do rio Amazonas no oceano Atlântico. As tipologias de curas tradicionais presentes nas ilhas foram analisadas a partir das narrativas destas três especialistas, num período de quase dois anos, com incursões, entrevistas e diversos tipos de observações in situ. Abstract This article analyzes the biographies of three riverine women whose research is based on their social roles in providing care through traditional medicine. Their “life stories” show their own perceptions of how they construct and reconstruct their healing practices to address illnesses, always involving interactions with the environment and with religious traditions (most notably pajelança, Catholicism and Pentecostalism). The islands of Murutucum and Combu in Belém are the anthropogenic environments in which these specialists reside and work, watershed ecosystems for several rivers in Pará and a small portion of the Amazon River that drain into the Atlantic Ocean. The traditional cures present on these islands were analyzed from the narratives of these three specialists over a period of almost two years, with incursions, interviews and various types of in situ observations. RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos. ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. RESUMO: Objetivou-se avaliar o comportamento de pintainhas de postura na fase de cria, alimentadas com dietas formuladas com aumento de fibra bruta e dois alimentos com composições de fibra diferentes. O trabalho foi realizado na Universidade Federal de Goiás, Goiânia, GO, utilizando-se 300 pintainhas Bovans White, em delineamento inteiramente casualizado e arranjo fatorial 2x2+1, com dois níveis de fibra bruta na dieta (3,0% e 3,5 % de FB), duas fontes de fibra (farelo de trigo e bagaço de cana) e o tratamento testemunha (ração a base de milho e farelo de soja), totalizando cinco tratamentos e seis repetições de 10 aves cada. Foram avaliados padrões de comportamento de manutenção e bem-estar aos 21 e 42 dias de idade. Não houve diferença no comportamento e na classificação por ordem de importância das atividades realizadas pelas aves alimentadas com fibra e observadas aos 21 e 42 dias pelo teste de Kruskal Wallis (P>0,05). Obteve-se condições média e boa com farelo de trigo e 3,5 e 3,0% de fibra bruta, respectivamente, com 21 dias de idade. Aos 42 dias, o uso de bagaço de cana ao nível de 3,5% de fibra bruta e o farelo de trigo ao nível de 3,0%, mostraram boas condições de comportamento. A utilização de fibra bruta na alimentação de frangas não alterou o comportamento das aves.
ABSTRACT: This study evaluated the behavior and welfare conditions of laying chicks in the starter phase, fed with diets formulated with increasing levels of crude fiber (CF) and two feedstuffs with different fiber compositions. The experiment was developed at the Federal University of Goiás, in Goiânia - GO, Brazil. Three hundred Bovans White chicks were evaluated in a completely randomized design with a 2×2+1 factorial arrangement corresponding to two levels of CF in the diet (3.0 and 3.5%) and two fiber sources (wheat bran and sugarcane bagasse), plus a control treatment (maize- and soybean meal-based), totaling five treatments with six replicates of 10 birds each. Maintenance behavioral patterns and welfare were assessed at 21 and 42 days of age. There was no difference in behavior and classification by order of importance of the activities performed by the fiber-fed birds observed at 21 and 42 days, according to the Kruskal Wallis test (P>0.05). Medium and good welfare conditions were obtained with wheat bran at 3.5 and 3.0% CF, respectively, at 21 days of age. At 42 days, the use of sugarcane bagasse at the CF level of 3.5% and wheat bran at the level of 3.0%provided good behavior conditions. The use of crude fiber in the feeding of hens did not change their behavior.

RESUMO A mastite é uma importante enfermidade que acomete rebanhos leiteiros de cabras e ovelhas. Os antimicrobianos são amplamente utilizados na terapia da mastite, contudo, o fenômeno da resistência bacteriana tem despertado o interesse por alternativas terapêuticas, destacando-se a pesquisa com óleos essenciais de diversas plantas. Dessa forma, o objetivo deste trabalho foi avaliar a atividade antibacteriana de óleos essenciais frente a patógenos de importância na mastite caprina e ovina. A obtenção dos óleos essenciais foi realizada utilizando a técnica de hidrodestilação e a composição química foi analisada através de cromatografia gasosa acoplada à espectrometria de massas (CG-EM). Para avaliar a atividade antibacteriana dos óleos essenciais (OE) de quatro plantas, utilizou-se os testes de Concentração Inibitória Mínima (CIM) e Concentração Bactericida Mínima (CBM). Foram utilizados 15 isolados bacterianos previamente identificados como Staphylococcus spp. e Staphylococcus aureus. Utilizaram-se as seguintes espécies de plantas: Lippia origanoides (alecrim-pimenta), Lippia alba (erva cidreira), Cymbopogon citratus (capim-santo) e Ocimum basilicum (manjericão) frente aos 15 isolados bacterianos. Os três primeiros óleos essenciais apresentaram atividade antibacteriana sobre 100% dos isolados (15/15), com as CIMs de 560, 1173 e 1280 µg mL-1, e CBMs de 613, 1226 e 1333 µg mL-1, respectivamente. Por outro lado, o óleo de Ocimum basilicum (manjericão), não exerceu atividade sobre nenhum isolado (0/15). O carvacrol, citral, geronial e o estragol foram alguns dos compostos majoritários encontrados. ABSTRACT Mastitis is an important infirmity that affects dairy goats and sheep flocks. Antimicrobials are widely used in mastitis therapy; however, the phenomenon of bacterial resistance has sparked interest in therapeutic alternatives, especially the research on essential oils from several plant species. Therefore, this study aimed to evaluate the antimicrobial activity of essential oils against pathogens of importance in caprine and ovine mastitis. The essential oils were obtained by hydrodistillation and the chemical composition was analyzed by gas chromatography coupled to mass spectrometry (GC-MS). The minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) tests were performed to evaluate the antimicrobial activity of the essential oils (EO) of four plants. Fifteen bacterial isolates, previously identified as Staphylococcus spp. and Staphylococcus aureus, were used. The following plant species were used against the 15 bacterial isolates: Lippia origanoides “alecrim pimenta,” Lippia alba “erva cidreira,” Cymbopogon citratus “capim-santo,” and Ocimum basilicum “manjericão.” The first three essential oils presented antimicrobial activity against 100% of the isolates (15/15), with MICs of 560 µg mL-1, 1,173 µg mL-1, and 1,280 µg mL-1, and MBCs of 613 µg mL-1, 1,226 µg mL-1, and 1,333 µg mL-1, respectively. Nevertheless, the essential oil of O. basilicum, which was also tested, exerted no activity against any of the isolates. Carvacrol, citral, geraniol, and estragole were some of the major compounds found. RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Resumo Objetivo: Este estudo tem como objetivo descrever o perfil de indicações de ressonância magnética (RM) das mamas em um hospital referência em câncer de mama. Materiais e Métodos: Estudo descritivo retrospectivo de solicitações médicas de exames e questionários de mulheres que realizaram RM das mamas em um centro de referência no diagnóstico e tratamento do câncer de mama no período de 2014 a 2018. Resultados: A indicação mais frequente foi complemento de mamografia/ultrassonografia (43,6%), seguida por estadiamento do câncer de mama (25,1%), rastreamento de pacientes de risco elevado (17,4%), avaliação de implantes (10,1%) e avaliação de resposta a tratamento por quimioterapia neoadjuvante (3,8%). Conclusão: Embora existam fortes evidências em favor do uso da RM das mamas, principalmente para rastreamento em pacientes de alto risco, o método ainda é subutilizado no nosso meio.

Materials and Methods: This was a retrospective study in which we reviewed the clinical records, including physician requests and patient questionnaires, of women who underwent breast MRI between 2014 and 2018 at a referral center for the diagnosis and treatment of breast cancer in the city of Belo Horizonte, Brazil. Results: The most common indication for breast MRI was as a complement to mammography/ultrasonography (in 43.6% of cases), followed by breast cancer staging (in 25.1%), the screening of patients at high risk (in 17.4%), the evaluation of breast implants (in 10.1%), and the evaluation of the response to neoadjuvant chemotherapy (in 3.8%). Conclusion: Although there is strong evidence supporting the use of breast MRI, mainly for the screening of high-risk patients, the imaging method is underutilized in Brazil.

Resumo Objetivos: analisar a letalidade e características clínicas em mulheres pernambucanas portadoras de neoplasia que apresentaram infecção por SARS-CoV-2. Métodos: estudo de corte transversal, retrospectivo com pacientes do sexo feminino, portadoras de neoplasias no estado de Pernambuco com registros disponibilizados pela Secretaria de Planejamento e Gestão do Estado de Pernambuco. Analisou-se dados secundários de notificações de domínio público e os fatores independentes associados ao óbito através de regressão logística. Foi considerado significativo o valor de p<0,25 na análise bivariada e para a análise multivariada foi considerado significativo o valor de p<0,05. Resultados: quarenta e nove mulheres vieram a óbito. A média da idade e desvio padrão foram 58, 75 ± 20,93 anos. 55,86% das pacientes tinham 60 anos ou mais. A taxa de letalidade global foi de 72,06% (IC95%= 59,8 - 82,2). Os sintomas mais prevalentes foram febre (70,59%), tosse (58,82%), dispneia (57,35%) e saturação de O2 <95% (48,53%). Conclusão: pacientes do sexo feminino, com câncer e infectadas pelo SARS-CoV-2 são particularmente suscetíveis a óbito, independentemente da presença de comorbidades ou da idade, sendo a saturação periférica de O2 <95% o único fator independente associado ao óbito nesse grupo.
Abstract Objectives: to analyze the lethality and clinical characteristics in Pernambuco women with neoplasia that were infected by SARS-CoV-2. Methods: a cross-sectional, retrospective study with female patients with neoplasm sin the state of Pernambuco registered and made available by the Secretariat of Planning and Management of the State of Pernambuco (SEPLAG PE). Secondary data from public domain notifications and the independent factors associated with death were analyzed through logistic regression. The value ofp<0.25 was considered significant in the bivariate analysis and for a multivariate analysis, the value ofp<0.05 was considered significant. Results: forty-nine women died. The mean age and standard deviation were 58.75 ± 20.93 years. 55.86% of the patients were 60 years old or more. The overall lethality rate was 72.06% (CI95%=59.8 - 82.2). The most prevalent symptoms were fever (70.59%), cough (58.82%), dyspnea (57.35%) and O2 saturation less than 95% (48.53%). Conclusions: female patients, with cancer and infected by SARS-CoV-2 are particularly susceptible to death, regardless of the presence of comorbidities or age, with peripheral O2 saturation <95% being the only independent factor associated with death in this group.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

Calorie and protein delivery in critically ill surgical and non-surgical patients receiving enteral nutrition therapy

ABSTRACT Providing enteral nutritional therapy to critically ill patients is a challenge, especially during the first days after starting intensive care. These challenges appear to be greater in critically ill surgical patients. Objectives: Describe and compare enteral nutrition practices in critically ill surgical and non-surgical patients. Methods: We conducted a prospective cohort study of surgical and non-surgical patients receiving exclusive enteral nutritional therapy. The values for calorie and protein delivery during the first week after admission to the intensive care unit were recorded. Results: 103 patients were enrolled (54.4% male, mean age: 63.9 years, 26.2% surgical). The median time of initiation of enteral nutritional therapy was the third day in non-surgical and the fourth day in surgical patients. Surgical patients had a lower calorie and protein delivery than non-surgical patients on the second to fourth days after admission. At the end of the first week, 20.2% of the patients had not received any diet, and there was no significant difference in nutrient delivery between the groups. Of the surgical and non-surgical patients, 42.9% and 39.3% were receiving ≥20 kcal/kg/day, and 28.6% and 34.4% were receiving ≥1.2 g/kg of protein per day, respectively. Conclusion: There was a delay in starting patients on enteral nutritional therapy and calorie and protein delivery was low, especially among surgical patients. By the end of the first week, calorie and protein delivery was similar in both groups.
RESUMEN Proporcionar terapia nutricional enteral a pacientes críticos es un desafío, especialmente durante los primeros días después de comenzar los cuidados intensivos. Estos desafíos parecen ser mayores en pacientes quirúrgicos críticos. El propósito del estudio fue describir y comparar las prácticas de nutrición enteral en pacientes críticos quirúrgicos y no quirúrgicos. Se realizó un estudio de cohorte prospectivo de pacientes quirúrgicos y no quirúrgicos que recibieron terapia nutricional enteral exclusiva. Se registraron los datos de entrega de calorías y proteínas durante la primera semana después de la admisión a la unidad de cuidados intensivos. Se estudiaron 103 pacientes (54,4% hombres, edad media: 63,9 años, 26,2% quirúrgicos). La mediana de tiempo de inicio de la terapia nutricional enteral fue el tercer día en pacientes no quirúrgicos y el cuarto día en pacientes quirúrgicos. Los pacientes quirúrgicos tuvieron una entrega de calorías y proteínas más baja que los pacientes no quirúrgicos en el segundo a cuarto día después del ingreso. Al final de la primera semana, el 20,2% de los pacientes no habían recibido ningún aporte nutricional, y no hubo diferencias significativas en el suministro de nutrientes entre los grupos. De los pacientes quirúrgicos y no quirúrgicos, 42,9% y 39,3% estaban recibiendo ≥20 kcal/kg/día, y 28,6% y 34,4% estaban recibiendo ≥1,2 g/kg de proteína por día, respectivamente. Conclusiones: Hubo un retraso en el inicio de los pacientes en terapia nutricional enteral y la entrega de calorías y proteínas fue baja, especialmente entre los pacientes quirúrgicos. Al final de la primera semana, la entrega de calorías y proteínas fue similar en ambos grupos.

Is perioperative fasting associated with complications, length of hospital stay and mortality among gastric and colorectal cancer patients? A cohort study

ABSTRACT BACKGROUND: During a surgical procedure, patients are often subjected to fasting for times that are more prolonged than the ideal, which may lead to complications. OBJECTIVE: To evaluate the duration of perioperative fasting and its association with postoperative complications, length of hospital stay (LOS) and mortality among gastric and colorectal cancer patients. DESIGN AND SETTING: Cohort study developed in a surgical oncology hospital in the city of Natal (Rio Grande do Norte, Brazil). METHODS: Patients aged over 18 years were included. The Clavien-Dindo surgical complication scale was used to evaluate occurrences of postoperative complications. LOS was defined as the number of days for which patients stayed in the hospital after surgery, or until the day of death. RESULTS: Seventy-seven patients participated (59.8 ± 11.8 years; 54.5% females; 70.1% with bowel tumor). The incidences of postoperative complications and death were 59.7% and 3.9%, respectively. The duration of perioperative fasting was 59.0 ± 21.4 hours, and it was higher among non-survivors and among patients with prolonged hospital stay (≥ 6 days). For each one-hour increase in the durations of perioperative and postoperative fasting, the odds of prolonged hospitalization increased by 12% (odds ratio, OR = 1.12; 95% confidence interval, CI 1.04-1.20) and 5% (OR = 1.05; 95% CI 1.02-1.08), respectively. CONCLUSION: Prolonged perioperative fasting, especially in the postoperative period, was observed in a sample of patients with gastric and colorectal cancer, and this was an independent predictor of LOS.
