Acetic acid bacteria (AAB) are versatile organisms that catalyze the conversion of a wide range of carbon sources into biomolecules of great industrial interest. In this study, we exploited the ability of Gluconobacter spp. to synthesize levans. Among the isolated strains, Gluconobacter cerinus UELBM11 produced approximately 14.0 g L−1 of levan under non-optimized conditions. Gas chromatography-mass spectrometry (GC-MS), Fourier transform infrared spectroscopy (FTIR), and nuclear magnetic resonance (NMR) analyses confirmed that levan obtained from G. cerinus UELBM11 consisted of a β-(2→6)-D-fructose backbone with some β-(2→1) ramifications. The average molecular weight (Mw) of the purified levan was 8.78 × 105 Da. Thermogravimetric/differential thermogravimetric (TGA/DTG) analysis indicated high thermal stability, with the maximum decomposition rate observed at 227.44 °C. Scanning electron microscopy (SEM) revealed a microporous morphology, and the antioxidant activity assays demonstrated that levan had a high scavenging capacity of 2,2’-azino-bis (3-ethylbenzothiazoline-6-sulfonic acid) (ABTS) and hydroxyl radicals. Therefore, it has been demonstrated the levan produced by G. cerinus UELBM11 is a promising natural antioxidant and, owing to its microporosity and excellent thermal properties and stability, is a potential candidate as an additive in cosmetics, pharmaceuticals, and food products. Resumo No presente artigo, a partir daquilo que o filósofo José Chasin chamou de análise imanente, pretendemos analisar a relação entre a cidade e o campo na obra marxiana. Abstract In this article, from what the Brazilian philosopher José Chasin called immanent analysis, we intend to analyze the relationship between the city and the countryside in Marxian work. RESUMO Objetivo evidenciar validade de conteúdo e validade de processos de resposta de um instrumento destinado à avaliação fonológica de crianças. Método validade realizada em duas etapas por dois diferentes grupos de juízes, grupo de especialistas e grupo de não-especialistas. O primeiro grupo composto por três juízes especialistas avaliaram os 123 itens lexicais após a elaboração do instrumento, julgando a aplicabilidade das figuras no contexto da avaliação infantil e sugerindo ajustes para compor o conteúdo. A partir das observações, o instrumento foi adequado e direcionado ao grupo de juízes não-especialistas que, por meio da aplicação do instrumento, tiveram suas respostas avaliadas conforme a facilidade ou dificuldade de elicitação dos itens do instrumento. Resultados As avaliações obtiveram resultados de teor positivo para as validades de conteúdo e de processos de resposta. ABSTRACT Purpose to demonstrate the validity of content and the validity of response processes of an instrument intended for the phonological assessment of children. Methods validation was carried out in two stages by two different groups of judges, a group of specialists and a group of non-specialists. RESUMO Objetivo Investigar evidências de validade de construto para um Instrumento de Avaliação Fonológica para o Português Brasileiro, baseadas nos dados de diagnóstico gerados por sua própria aplicação a partir da análise contrastiva e do grau de severidade de fala. Método A amostra foi composta por dados de 176 crianças, com idades entre cinco até nove anos. Foram avaliadas pelo Instrumento de Avaliação Fonológica e classificadas em com transtorno fonológico ou em desenvolvimento fonológico típico, comparando tais resultados aos critérios para o transtorno no DSM-5. A busca por evidências da validade de construto contou com a concordância entre os dois métodos de avaliação, aplicando o Coeficiente Kappa. Para a diferenciação entre os grupos, utilizou-se o teste t de Student para amostras independentes. Buscou-se a investigação dos índices do instrumento pela estatística da Curva de Receiver Operating Characteristic para obter valores de área, ponto de corte, sensibilidade, especificidade, acurácia, valor preditivo positivo e negativo. Resultados O instrumento apresentou concordância e diferenciação significativa entre as classificações. Quanto aos parâmetros de desempenho, exibe ponto de corte para diagnóstico com resultados iguais ou maiores do que 96,17%, excelente valor de área sob a curva, assim como percentuais satisfatórios para as outras análises investigadas. Conclusão O conjunto de dados encontrados indicam evidências para validade de construto do Instrumento de Avaliação Fonológica, apresentando uma contribuição útil e válida ao arsenal de avaliação clínica e de pesquisa envolvendo diagnóstico de Transtorno Fonológico e, com seu resultado de acurácia, contribuiu as propriedades de desempenho dos instrumentos utilizados na Fonoaudiologia. ABSTRACT Purpose To investigate evidence of construct validity for a Phonological Assessment Instrument for Brazilian Portuguese, based on the diagnostic data generated by its application from contrastive analysis and speech severity. Methods The sample consisted of 176 children, aged between five to nine years old. They were evaluated with the Phonological Assessment Instrument and then classified as having Speech Sound Disorder or in typical phonological development, comparing these results to the criteria described for the disorder in the DSM-5. The search for evidence of construct validity relied on the agreement between the two assessment methods while applying the Kappa Coefficient. To differentiate between groups, Student's t-test was used for independent samples. We sought to investigate the instrument indexes using the Receiver Operating Characteristic Curve statistics to obtain values for area, cut-off point, sensitivity, specificity, accuracy, and positive and negative predictive value. Results The instrument showed agreement and significant differentiation between the classifications. As for the performance parameters, it shows a cut-off point for diagnosis with results equal to or greater than 96.17%, an excellent area under the curve, as well as satisfactory percentages for the other analyses investigated. Conclusion The data indicated evidence for the construct validity of the Phonological Assessment Instrument, presenting a useful and valid contribution to the arsenal of clinical assessment and research involving the diagnosis of Speech Sound Disorder and, with its accuracy result, contributed to the properties of performance of instruments used in Speech, Language and Hearing Sciences. Resumo Objetivo Apresentar evidências de fidedignidade intra e interavaliadores, e de consistência interna, dos escores do Instrumento de Avaliação Fonológica, a fim de que possa ser considerado fidedigno e válido para a utilização na prática clínica. Método Foram analisados 179 áudios dos registros de fala do instrumento, cuja coleta foi realizada a partir da sua aplicação no período de 5 meses em crianças na faixa etária dos cinco aos oito anos e 11 meses. Três juízes especialistas transcreveram no software a produção de fala de cada criança, o qual gerou relatórios de desempenho. Os dados de fala de cada criança foram comparados entre esses avaliadores, treinados e experientes em transcrição fonética, para verificar a concordância dos escores do instrumento. Para a análise da fidedignidade, foi verificada a consistência interna pelo Alpha de Cronbach e a confiabilidade intra e interavaliadores por meio do Coeficiente de Correlação Intraclasse. Resultados O Instrumento de Avaliação Fonológica apresentou evidências de alta consistência interna, com escores indicando excelente fidedignidade para avaliação dos fonemas do Português Brasileiro, como também uma adequada concordância entre os juízes acerca dos escores do instrumento. Abstract Purpose To present evidence of intra- and inter-rater reliability and internal consistency of the Phonological Assessment Instrument scores, so that it can be considered reliable and valid for use in clinical practice. Methods 179 audio recordings of the instrument's speech samples were analyzed. The collection was carried out from its application in the period of 5 months in children aged from five to eight years and 11 months. Three expert judges transcribed the speech production of each child into the software, which generated performance reports. The speech data of each child were compared between these evaluators, who were trained and experienced in phonetic transcription, to verify the agreement of the instrument scores. For the reliability analysis, the internal consistency was verified using Cronbach's Alpha and the intra and inter-rater reliability using the Intraclass Correlation Coefficient. Results The Phonological Assessment Instrument showed evidence of high internal consistency, with scores indicating excellent reliability for the assessment of Brazilian Portuguese phonemes, as well as adequate agreement among the judges regarding the instrument scores. RESUMEN Introducción: La participación femenina en deportes alcanzó un hito en 1972, cuando se implementó una reforma constitucional para garantizar igualdad de oportunidades entre hombres y mujeres. Desde entonces, el porcentaje de participantes en deportes competitivos ha crecido. En este contexto, se hizo necesario comprender el ciclo menstrual, el uso de anticonceptivos y sus respuestas individuales. Objetivo: Investigar características del ciclo menstrual, síntomas físicos, estado de ánimo, quejas relacionadas con el sangrado vaginal y los métodos anticonceptivos utilizados por atletas olímpicas brasileñas. Evaluar si estas deportistas perciben influencia del sangrado vaginal en el rendimiento deportivo, cómo lo controlan y qué se puede cambiar para mejorar. Además, ofrecemos atención especializada por una ginecóloga especialista en medicina deportiva. Métodos: Estudio observacional, descriptivo, realizado de julio a agosto de 2016, que incluyó 118 atletas olímpicas brasileñas, en menacme. Las atletas respondieron un cuestionario en línea autoadministrado y adaptado titulado “Examen ginecológico previo a la participación”. Resultados: Practicaban 28 deportes diferentes, con edad media = 27 ± 4,7 años. Para 66%, era su primera participación en un juego olímpico. Anticonceptivos usados r54%), principalmente orales (61%). ABSTRACT Introduction: Female participation in sports has reached a milestone in 1972 when a constitutional amendment was implemented to ensure equal opportunities for men and women. Since then, the percentage of participants in competitive sports has grown. In this context became necessary to understand menstrual cycle, contraceptive use, and its individual responses. Objective: To investigate menstrual cycle characteristics, physical and mood symptoms related to vaginal bleeding, and contraceptives used by Brazilian Olympic athletes. Also, to assess if these athletes relate that vaginal bleeding influences sportive performance, how they manage it and what can be changed to improve their health care and sportive performance. Additionally, we propose specialized female care by a gynaecologist specialized in sports medicine. Methods: Descriptive observational study was conducted from July to August 2016 and included 118 Brazilian Olympic female athletes, in menacme. The athletes completed a self-administered online questionnaire, adapted from Pre-Participation Gynaecological Examination of female athletes. Results: Participants practiced 28 different sports, mean age 27 ± 4.7 years. For 66% it was their first participation in an Olympic Game. Most used contraceptives (54%), mainly oral (61%). Most (76%) believed that vaginal bleeding influenced sports performance, and 63% preferred to compete after bleeding cessation. Fifty-eight percent would compete at preferred time of their cycle. Anxiety symptoms, bloating, increased appetite, depression, and dysmenorrhea were indicated by 52%. Among these, 49% reported that these symptoms deteriorated their sportive performance. Conclusion: Most in their first Olympic participation, Brazilian athletes used hormone contraceptives, mainly oral ones to manage and adapt their vaginal bleeding to the competition calendar because most of them referred those physical and mood symptoms deteriorated their sportive performance. RESUMO Introdução: A participação feminina no esporte teve um marco importante em 1972, quando uma emenda constitucional foi implementada para garantir a igualdade de oportunidades para homens e mulheres. Desde então, o percentual de participantes em esportes competitivos tem crescido. Nesse contexto tornou-se necessário compreender sobre ciclo menstrual, uso de anticoncepcionais e suas respostas individuais. Objetivo: Investigar características do ciclo menstrual, sintomas físicos e de humor, queixas relacionadas ao sangramento vaginal e métodos contraceptivos usados por atletas olímpicas brasileiras. Avaliar se percebem influência do sangramento vaginal no desempenho esportivo, como elas o controlam e o que pode ser mudado para melhorar seus cuidados e desempenho esportivo. Além disso, propõe-se atendimento específico por ginecologista especializada em medicina esportiva. Métodos: Estudo observacional, descritivo, realizado de julho a agosto de 2016 que incluiu 118 atletas olímpicas brasileiras, na menacme. As atletas responderam a um questionário online autoaplicável e adaptado intitulado “Pre-Participation Gynaecological Examination”. Resultados: As participantes praticavam 28 esportes diferentes, com média de idade = 27 ± 4,7 anos. Para 66%, foi a primeira participação em um evento olímpico. A maioria usava anticoncepcional (54%), principalmente oral (61%). A maioria (76%) acredita que o sangramento vaginal influencia o desempenho esportivo e 63% preferiam competir após o mesmo. 58% das atletas competiriam no momento preferido de seu ciclo. O risco relativo de hospitalização por diarreia em cada município foi calculado utilizando o software SaTScan, considerando-se o nível de significância estatística de 5% e 999 replicações de Monte Carlo. Resultados: O total de 13.315 hospitalizações por diarreia em crianças menores de cinco anos de idade foram registradas. As taxas anuais de hospitalização por diarreia reduziram de 8,50 para 3,45/1.000 nascidos vivos em crianças menores de um ano de idade e de 4,99 para 1,57 em crianças de 1–4 anos de idade, de 2011 a 2020. Clusters de municípios com alto risco relativo para hospitalização por diarreia, estatisticamente significantes, predominaram nas macrorregiões administrativas de saúde Norte, Nordeste e Sudoeste de Mato Grosso para crianças de ambos os grupos etários, até 2016. De 2016 a 2020, clusters de baixo risco relativo foram identificados nas macrorregiões administrativas de saúde Norte e Centro-Sul para crianças menores de cinco anos de idade. Conclusão: Os resultados mostraram redução das taxas de hospitalização por diarreia em crianças menores de cinco anos de idade, com a presença de clusters de baixo risco em Mato Grosso nos últimos anos estudados. Os monitoramentos de saúde pública devem incorporar análises espaciais na investigação da morbidade por diarreia.

ABSTRACT Objective: To examine spatiotemporal variability and identify clustering patterns of hospitalization rates for diarrhea in children younger than five years in Mato Grosso, Brazil, from 2011 to 2020. Methods: An ecological study was conducted using hospitalization records associated with diarrhea from the Brazilian Hospital Information System/Unified Health System. The relative risk of hospitalization for diarrhea in each municipality was calculated using SaTScan software considering a statistical significance level of 5% and 999 Monte Carlo replications. Results: A total of 13,315 diarrhea-associated hospitalizations for 5-year-old children were recorded. From 2011 to 2020, the annual rates for hospitalizations related to diarrhea decreased from 8.50 to 3.45/1,000 live births among children younger than one year and from 4.99 to 1.57 for children aged 1–4 years. ABSTRACT The global reemergence of measles in 2018–2019 reinforces the relevance of high-coverage immunization to maintain the disease elimination. During an outbreak in the Sao Paulo State in 2019, several measles cases were reported in individuals who were adequately vaccinated according to the current immunization schedule recommends. This study aimed to assess measles IgG antibody seropositivity and titers in previously vaccinated adults. A cross-sectional study was conducted at CRIE-HC-FMUSP (Sao Paulo, Brazil) in 2019. It included healthy adults who had received two or more Measles-Mumps-Rubella vaccines (MMR) and excluded individuals with immunocompromising conditions. Measles IgG antibodies were measured and compared by ELISA (Euroimmun®) and chemiluminescence (LIASON®). The association of seropositivity and titers with variables of interest (age, sex, profession, previous measles, number of measles-containing vaccine doses, interval between MMR doses, and time elapsed since the last MMR dose) was analyzed. A total of 162 participants were evaluated, predominantly young (median age 30 years), women (69.8%) and healthcare professionals (61.7%). The median interval between MMR doses was 13.2 years, and the median time since the last dose was 10.4 years. The seropositivity rate was 32.7% by ELISA and 75.3% by CLIA, and a strong positive correlation was found between the tests. Multivariate analyses revealed that age and time since the last dose were independently associated with positivity. Despite being a single-center evaluation, our results suggest that measles seropositivity may be lower than expected in adequately immunized adults. The indiscriminate use of herbicides makes weeds resistant, increasing the demand for more efficient herbicides. Novel potential herbicides were prepared from sorbic acid, being 8 dienamides and 6 epoxides. Subsequently, phytotoxic activities of sorbic acid and 14 synthesized compounds were evaluated against sorghum, onion, cucumber, lettuce, and beggartick seeds. Furthermore, the protein target of the most active substance in plants was identified in silico. Among the tested compounds, (2E,4E)-N-(p-fluorophenyl)hexa-2,4-dienamide (2c), (2E,4E)-N -(o-naphthyl) hexa-2,4-dienamide (2d), (E)-N-methylphenyl-3-((2R,3R)-3-methyloxiran-2-yl)acrylamide (3b), (E)-N-bromophenyl-3-((2R,3R)-3-methyloxiran-2-yl)acrylamide (3e), and (E)-N-methoxyphenyl-3-((2R,3R)-3-methyloxiran-2-yl)acrylamide (3f) showed pronounced inhibitory activities, both of shoots and roots, making them candidates for promising herbicides. ABSTRACT We investigated the impact of weaning at 30, 75, and 180 days of age on performance, body measurements, and rumen development (using the β-hydroxybutyrate biomarker) of beef calves until 190 days of age. A total of 64 Brangus calves were assigned to three treatments: hyper-early (W30), weaned at 32±0.89 days of age (n = 22, 10 males and 12 females); early (W75), weaned at 77±0.95 days of age (n = 20, 12 males and eight females); and conventional (W180), weaned at 183±0.82 days of age (n = 22, 13 males and nine females). Body weight (BW), average daily gain (ADG), β-hydroxybutyrate (βHBA), and morphometric measurements were evaluated. These variables were influenced by an interaction between treatment and days and were not affected by the sex of calves. The W30 calves experienced weight loss from 30 to 40 days of age compared with the W75 and W180 calves. Consequently, at day 75, W30 animals were lighter and had lower body length, thoracic circumference, withers height, and croup height compared with the W75 and W180 calves. However, no significant differences in BW were observed among treatments at 85 days of age. Additionally, W30 calves exhibited higher ADG and a moderate correlation with βHBA levels. RESUMO Contexto: Em estudos observacionais sobre a síndrome do desconforto respiratório agudo, sugeriu-se que a driving pressure é o principal fator de lesão pulmonar induzida por ventilador e de mortalidade. Não está claro se uma estratégia de limitação da driving pressure pode melhorar os desfechos clínicos. Objetivo: Descrever o protocolo e o plano de análise estatística que serão usados para testar se uma estratégia de limitação da driving pressure envolvendo a titulação da pressão positiva expiratória final de acordo com a melhor complacência respiratória e a redução do volume corrente é superior a uma estratégia padrão envolvendo o uso da tabela de pressão positiva expiratória final baixa do protocolo ARDSNet, em termos de aumento do número de dias sem ventilador em pacientes com síndrome do desconforto respiratório agudo devido à pneumonia adquirida na comunidade. Métodos: O estudo STAMINA (ventilator STrAtegy for coMmunIty acquired pNeumoniA) é randomizado, multicêntrico e aberto e compara uma estratégia de limitação da driving pressure com a tabela de pressão positiva expiratória final baixa do protocolo ARDSnet em pacientes com síndrome do desconforto respiratório agudo moderada a grave devido à pneumonia adquirida na comunidade internados em unidades de terapia intensiva. Esperamos recrutar 500 pacientes de 20 unidades de terapia intensiva brasileiras e duas colombianas. Eles serão randomizados para um grupo da estratégia de limitação da driving pressure ou para um grupo de estratégia padrão usando a tabela de pressão positiva expiratória final baixa do protocolo ARDSnet. No grupo da estratégia de limitação da driving pressure, a pressão positiva expiratória final será titulada de acordo com a melhor complacência do sistema respiratório. Desfechos: O desfecho primário é o número de dias sem ventilador em 28 dias. Os desfechos secundários são a mortalidade hospitalar e na unidade de terapia intensiva e a necessidade de terapias de resgate, como suporte de vida extracorpóreo, manobras de recrutamento e óxido nítrico inalado. ABSTRACT Background: Driving pressure has been suggested to be the main driver of ventilator-induced lung injury and mortality in observational studies of acute respiratory distress syndrome. Whether a driving pressure-limiting strategy can improve clinical outcomes is unclear. Objective: To describe the protocol and statistical analysis plan that will be used to test whether a driving pressure-limiting strategy including positive end-expiratory pressure titration according to the best respiratory compliance and reduction in tidal volume is superior to a standard strategy involving the use of the ARDSNet low-positive end-expiratory pressure table in terms of increasing the number of ventilator-free days in patients with acute respiratory distress syndrome due to community-acquired pneumonia. Methods: The ventilator STrAtegy for coMmunIty acquired pNeumoniA (STAMINA) study is a randomized, multicenter, open-label trial that compares a driving pressure-limiting strategy to the ARDSnet low-positive end-expiratory pressure table in patients with moderate-to-severe acute respiratory distress syndrome due to community-acquired pneumonia admitted to intensive care units. We expect to recruit 500 patients from 20 Brazilian and 2 Colombian intensive care units. They will be randomized to a driving pressure-limiting strategy group or to a standard strategy using the ARDSNet low-positive end-expiratory pressure table. In the driving pressure-limiting strategy group, positive end-expiratory pressure will be titrated according to the best respiratory system compliance. Outcomes: The primary outcome is the number of ventilator-free days within 28 days. The secondary outcomes are in-hospital and intensive care unit mortality and the need for rescue therapies such as extracorporeal life support, recruitment maneuvers and inhaled nitric oxide. RESUMO Contexto: A evidência da regressão espontânea (RE) no carcinoma hepatocelular (CHC) é rara. Objetivo: Revisar todos os casos de RE de CHC em dois centros de referência do Sul do Brasil, destacando as principais características. Métodos: Os dados de todos os pacientes com CHC foram revisados retrospectivamente buscando a ocorrência de RE em pacientes de dois centros terciários do Sul do Brasil, nos últimos 5 anos. O diagnóstico de cirrose foi estabelecido de acordo com dados clínicos, laboratoriais e de imagem, além de endoscopia digestiva alta ou exame histopatológico quando necessário. O diagnóstico de CHC foi baseado em achados típicos de acordo com critérios radiológicos (LIRADS) ou exame histopatológico. A RE foi definida como uma involução parcial ou completa de um CHC na ausência de terapia específica. Resultados: Do total de casos de CHC nos últimos 5 anos (n=433), houve cinco casos de RE. Três (60%) eram homens, a média de idade foi de 62,6 (50,0-76,0) anos, a etiologia foi virus da hepatite C em 3 (60%). A regressão completa foi observada em três pacientes (60%), um paciente (20%) apresentou regressão parcial e um (20%) apresentou recidiva e evoluiu a óbito. O tempo de seguimento variou entre 12 e 21 meses. Nesta apresentação foi destacado um caso (20%) de RE observado após infecção por COVID-19 em paciente com cirrose. Foram revisados os possíveis mecanismos envolvidos nesta situação, enfatizando os mais comuns como hipóxia e imunológicos. Houve também um paciente submetido a procedimento cirúrgico como possível fator envolvido e três pacientes sem fatores de risco evidentes. Conclusão: Este fenômeno possivelmente contribuirá para uma melhor compreensão dos mecanismos fisiopatológicos do CHC. ABSTRACT Background: Spontaneous regression (SR) is defined as the partial or complete disappearance of a tumor, in the absence of a specific treatment. Evidence of the SR in hepatocellular carcinoma (HCC) is rare. Objective: The authors aimed to review all the cases of SR of HCC in two reference centers of Southern Brazil, highlighting the main characteristics. Methods: Data of all patients with HCC were retrospectively reviewed looking for the occurrence of SR in patients from two tertiary centers in Southern Brazil, in the last five years. The diagnosis of cirrhosis was established according to clinical, laboratory and imaging data, as well as upper endoscopy or histopathological examination when necessary. The diagnosis of HCC was based on typical findings according to radiologic criteria (LIRADS) or histopathological examination. Spontaneous regression was defined as a partial or complete involution of a HCC in the absence of a specific therapy. Results: From all cases of HCC in the last 5 years (n=433), there were five cases of SR. Three (60%) were men, the mean age was 62.6 (50.0-76.0) years, and the etiology was HCV in 3 (60%). Complete regression was observed in three patients (60%), one patient (20%) presented partial regression, and one (20%) relapesed and died. The time of follow-up varied between 12 and 21 months. In this presentation, it was highlighted one case of SR observed after COVID-19 infection in a patient with cirrhosis. The possible mechanisms involved in this situation were reviewed, emphasizing the most common like hypoxia and immunological. There were also one patient submitted to a surgical procedure as a possible fator involved and three patients without obvious risk factors. Conclusion: This phenomenon will possibly contribute to a better understanding of the pathophysiological mechanisms of HCC. RESUMO Próteses totais fixas implantossuportadas objetivam reabilitar arcos edêntulos nos aspectos estético, fonético e funcional. Um preparo protético prévio sem flange no setor anterior permite avaliar o suporte labial e o espaço presente para a futura prótese, auxilinado na escolha do tipo de reabilitação e da técnica cirúrgica mais adequada. No entanto, estudos de acompanhamento tem mostrado que ao proceder dessa maneira ainda estão ocorrendo problemas no setor posterior, como falta de espaço vertical para um desenho adequado da barra e desenhos internos côncavos, que dificultam o acesso à higienização. Frente ao problema, o objetivo do presente trabalho é relatar um caso clínico no qual o preparo protético prévio incluiu a remoção do flange também do setor posterior durante a prova dos dentes e a duplicação dessa montagem em um guia multifuncional transparente permitindo a visualização da quantidade de remoção óssea necessária. A osteotomia, realizada antes da instalação dos implantes, proporcionou espaço suficiente para a barra, acrílico e dentes de estoque na prótese que foi instalada, fato importante considerando ser essa uma zona com maiores esforços mastigatórios. Também permitiu a confecção de desenho interno correto na prótese, que garantirá o acesso para correta higiene. Com base na análise do caso reabilitado, parece lícito concluir que a remoção total do flange vestibular no momento da prova dos dentes em cera e sua duplicação é um diferencial na abordagem dos casos e que deveria sempre ser adotada para garantir menor margem de erros e maior longevidade no tratamento reabilitador proposto. estético funcional adequada entanto côncavos higienização problema necessária osteotomia implantes instalada mastigatórios higiene reabilitado proposto

ABSTRACT Fixed implant-supported complete maxillary dentures aim to rehabilitate aesthetic, phonetic, and functional aspects of edentulous arches. A previous prosthetic preparation without a flange in the anterior sector makes it possible to evaluate the labial support and the existing space for the future prosthesis. Thus, it allows the most appropriate choice of the type of rehabilitation and surgical technique. However, follow-up studies have shown that when proceeding this way, problems in the posterior sector are still occurring, such as the lack of vertical space for an adequate bar design and concave internal designs, which make access to hygiene difficult. Faced with the problem, the aim of this study is to report a clinical case in which the previous prosthetic preparation included the removal of the flange also from the posterior sector during the teeth try-in and the duplication of this assembly in a transparent multifunctional guide that allowed the visualization of the amount of bone removal needed. The osteotomy, performed before the installation of the implants, provided enough space for the bar, acrylic, and prefabricated denture teeth in the prosthesis that was installed, an important fact considering that this is an area with greater chewing efforts. It also allowed for correct internal design in the prosthesis, which will ensure access to correct hygiene. ABSTRACT Objective: This study aimed to evaluate the cardiovascular risk of patients with post-surgical hypoparathyroidism through coronary calcium score (CACS) evaluation and cardiovascular risk calculators. Subjects and methods: Patients with post-surgical hypoparathyroidism (HG = 29) were compared to a control group (CG = 29), matched by sex and age. Demographic and clinical data were captured by a questionnaire or patient files. Both groups performed a thoracic-computed tomography to evaluate the CACS and the cardiovascular risk was calculated by two risk calculators. Results: In the HG, the supplementation of calcium varied between 500 to 2,000 mg/day and the mean calcitriol was 0.5 ± 0.29 mcg/day. The mean serum calcium and phosphorus were 8.32 ± 0.68 and 4.92 ± 0.87 mg/dL, respectively, and in the range recommended for hypoparathyroidism. The Brazilian Society of Cardiology's risk calculator showed a difference among groups, with no patient in the HG with low risk, but the CACS was similar. A positive CACS in the HG was associated with obesity and high BMI but not with calcium and/or vitamin D supplementation. Conclusion: In conclusion, patients with hypoparathyroidism did not show increased CACS, and it was not related to supplementation.
