This study aims to propose a new method for derivatizing fatty acids (FAs) from human milk (HM), eliminating the lipid extraction step, and simplifying the preparation for gas chromatography with a flame ionization detection (GC-FID) analysis to quantify the FAs. The Design Expert software optimized the reaction times, concentrations, and sample amount. The proposed method (PM) was validated for lyophilized HM and results for the figures of merit for precision in relative standard deviation (RSD) (RSDintra-day 1.34-4.03% and RSDinter day 2.08 5.16%), accuracy (99.87-102.16%), and robustness are within a linear range of 3 to 38% lipids in HM samples. Abstract Hip arthroplasties are surgical procedures widely performed all over the world, seeking to return functionality, relieve pain, and improve the quality of life of patients affected by osteoarthritis, femoral neck fractures, osteonecrosis of the femoral head, among other etiologies. Periprosthetic joint infections are one of the most feared complications due to the high associated morbidity and mortality, with a high number of pathogens that may be associated with its etiology. The aim of the present study was to analyze aspects correlated with the occurrence of infection, diagnosis and prevention of periprosthetic joint infections in the hip associated with Staphylococcus aureus after corrective surgery for hip fractures. Abstract Analytical and computational calibrations and subsequent parametric analyses were performed on the four UHPFRC beams under flexure to estimate their actual shear strength. RESUMO Objetivo: Encontrar evidências sobre a utilização de N-acetil-cisteína (NAC) no perioperatório de transplante de fígado, uma vez que a NAC, por ser o precursor acetilado da L-cisteína e da glutationa reduzida, colabora no abastecimento hepático de glutationa ajudando o fígado a se recuperar da lesão de isquemia e reperfusão. Metodologia: Trata-se de uma revisão de escopo nas bases de dados PubMed, BVS e Web of Science. Foram utilizados os descritores “Liver transplantation”, “N-acetyl-cysteine” e “Reperfusion Ischemia”, com o operador booleano “AND”, e selecionados artigos de relevância para o tema. Inicialmente, foram selecionados 60 artigos, todos publicados nos últimos 24 anos, em português e/ou inglês. Após análise, oito artigos corresponderam ao objetivo proposto. Resultados: Os grupos que receberam NAC durante o TxF apresentaram hipotensão pós-reperfusão, menores valores de pH intraoperatório, concentrações plasmáticas mais elevadas de IL-4 e aumento significativo dos níveis de IL-10 cinco minutos antes da reperfusão. Observou-se ainda a inibição da α-glutationa S-transferase (α-GST) após a reperfusão, ao contrário do grupo controle, que apresentou aumento significativo dessa enzima. Além disso, os níveis de sVCAM-1 e sICAM-1 foram significativamente mais baixos no grupo NAC 24 horas após a reperfusão em comparação com o grupo placebo. O valor máximo de AST durante as primeiras 72 horas de pós-operatório foi semelhante em ambos os grupos, embora o pico de ALT tenha sido menor no grupo NAC do que no grupo placebo. ABSTRACT Objective: To find evidence on the use of N-acetyl-cysteine (NAC) in the perioperative period of liver transplantation, since NAC, as it is the acetylated precursor of L-cysteine and reduced glutathione, contributes to the hepatic supply of glutathione, helping the liver to recover from ischemia and reperfusion injury. Methodology: This is a scoping review of the PubMed, VHL and Web of Science databases. The descriptors “Liver transplantation”, “N-acetyl-cysteine” and “Reperfusion Ischemia” were used, with the Boolean operator “AND”, and articles relevant to the topic were selected. Initially, 60 articles were selected, all published in the last 24 years, in Portuguese and/or English. After analysis, eight articles corresponded to the proposed objective. Results: The groups that received NAC during TxF showed post-reperfusion hypotension, lower intraoperative pH values, higher plasma concentrations of IL-4 and a significant increase in IL-10 levels five minutes before reperfusion. Inhibition of α-glutathione S-transferase (α-GST) was also observed after reperfusion, unlike the control group, which showed a significant increase in this enzyme. Furthermore, sVCAM-1 and sICAM-1 levels were significantly lower in the NAC group 24 hours after reperfusion compared with the placebo group. The maximum AST value during the first 72 postoperative hours was similar in both groups, although the peak ALT was lower in the NAC group than in the placebo group. In grafts that received NAC in the perfusion solution, survival rates at 3 and 12 months were 93% and 90%, respectively, and in the control group were 82% and 70%, respectively. The incidence of postoperative complications was 23% in the NAC group and 51% in the control group. The incidence of EPD was lower for the NAC group, which was 15% versus 32% in the control group. Regarding the administration of NAC during the intraoperative TxF, the one-year patient survival rate was 78.4% in the NAC group compared to 80.9% in the placebo group. RESUMO Objetivos: Analisar as notificações de morte encefálica (ME) recebidas pela Central de Transplantes (CET) do estado de Roraima (RR) de 2017 a 2021. Métodos: Trata-se de estudo transversal, quantitativo e observacional no qual foram analisadas todas as notificações recebidas pela CET no período de 2017 a 2021, resultando em 120 pacientes, dos seguintes centros de saúde: Hospital Geral de Roraima (HGR), Hospital Confederação Nacional das Cooperativas Médicas (UNIMED), Hospital da Criança Santo Antônio (HCSA), Hospital das Clínicas (HC) e Hospital Lotty Íris (HLI). Os dados foram coletados do sistema de gerenciamento de dados da central e repassados para a ficha de coleta desenvolvida pela pesquisadora. Resultados: Este estudo analisou 119 notificações após a exclusão de uma devido a dados conflitantes. Abstract Introduction This article describes an unusual case of perianal Paget's Disease in a 42-year-old man, highlighting distinct characteristics about more typical cases. Case report 42-year-old male patient with persistent perianal lesion, recurrent abscesses, and discomfort. Magnetic resonance imaging indicated a possible fistulous path. Biopsy revealed secondary anal Paget's disease and adenocarcinoma. Chemotherapy and radiotherapy treatment demonstrated a positive response. Abstract Food processing allows a wide variation in the final quality of the product, which directly depends on the quality of the raw material used, the processing conditions, storage and commercialization. Salting, for example, is one of the oldest methods used to preserve meat. In the meat industry, the incorporation of salts into products is commonly used to improve dietary functionality and ensure food safety. Thus, the objective of this review was to understand that the discarded matrix meat can be used to make jerky meat, adding value to the final product. The processing of jerked beef is based on the osmotic dehydration process in counter flow, promoted by the penetration of salt with consequent exit of water from the inter and extra fibrillar compartments to the surface, with subsequent drying. At the same time, myosin denaturation occurs as a result of high temperatures, as well as myoglobin oxidation, promoted by salt, increasing the susceptibility to protein oxidation. The use of discarded animal meat to make jerky adds value to the by-product, improving sensory quality.

RESUMO A tecnologia da Madeira Lamelada Colada permite a execução de peças estruturais em madeira unidas com adesivo. Com o emprego desta tecnologia é possível gerar estruturas de grande porte, com controle tecnológico do material e em sistema industrial competitivo. Com ênfase ao emprego estrutural da madeira, o presente trabalho avaliou a resistência de juntas coladas com diferentes ângulos entre as fibras da madeira de Pinus sp. e unidas com o adesivo poliuretano bicomponente a base de óleos vegetais. As juntas coladas foram confeccionadas com a formação dos ângulos de 0°, 10°, 20º, 30°, 45°, 60°, 70°, 80º e 90° e avaliadas em ruptura por esforço de cisalhamento à compressão e cisalhamento à torção. Quando realizada a análise de regressão, os valores de cisalhamento à compressão apresentaram adequação com a fórmula de Hankinson, apresentando valor de R2 próximo a 0,76. Porém, os resultados de cisalhamento sob torção apresentaram valores de R2 próximos a 0,17. Desta maneira, a fórmula de Hankinson não se mostrou propícia como estimadora dos valores de resistência para o cisalhamento à torção. ABSTRACT Glued Laminated Timber technology allows the construction of wooden structural components bonded with adhesive. This technology enables the creation of large-scale structures, with technological control over the material, and in a competitive industrial system. Focusing on the structural application of wood, the present study assessed the strength of bonded joints with different angles between the fibers of Pinus sp. wood, joined with a two-component polyurethane adhesive based on vegetable oils. The bonded joints were fabricated with angles of 0°, 10°, 20°, 30°, 45°, 60°, 70°, 80°, and 90°, and were evaluated for shear strength under compression and torsion. When regression analysis was performed, shear compression values showed compliance with Hankinson’s equation, with an R2 value close to 0.76. However, shear torsion results yielded R2 values near 0.17. Thus, Hankinson’s equation was unsuitable as an estimator for torsional shear strength values. Resumo As lesões traumáticas da aorta (LTA) torácica estão associadas a altas taxas de morbimortalidade. São classificadas de acordo com a extensão do dano, e a angiotomografia computadorizada tem as maiores sensibilidade e especificidade para identificar o grau de lesão e potenciais lesões associadas. As estratégias terapêuticas para LTA são baseadas no tipo de lesão, na extensão e nas lesões associadas. Pode auxiliar na definição de conduta também o grau de estabilidade do paciente, podendo ser manejo cirúrgico convencional, endovascular (TEVAR) ou conservador em casos selecionados. Abstract Traumatic thoracic aortic injuries (TTAI) are associated with high rates of morbidity and mortality. A partir da Teoria das Representações Sociais, buscamos identificar as diferentes formas de expressão e ancoragens da homofobia a partir de princípios organizadores, nos discursos dos fãs do jogo League of Legends (LoL) na rede social Facebook. Foram selecionados 470 comentários publicados em 2017, os quais foram analisados com apoio do Iramuteq (software de análise lexicométrica) por meio de uma classificação hierárquica descendente, resultando em quatro classes: “Debate sobre a sexualidade dos campeões” (14,9%), “Representatividade no LoL” (29,8%), “Confronto entre as histórias de Varus” (39%), e “Estratégia empresarial” (16,2%). Abstract Digital games are considered an environment of male privilege that promotes heteronormative standard of masculinity. Based on the Theory of Social Representations, we seek to identify, based on organizing principles, the forms of expression and anchorages processes related to homophobia within the speeches of the fans of the game League of Legends (LoL), on Facebook. Aim This study aimed to assess the shaping ability of Reciproc Blue in the apical third and apical foramen of moderately curved canals at different working lengths (WLs), by micro-computed tomography. Methods Thirty-six mesial roots (mesiobuccal and mesiolingual canals) were included, each with 2 separate root canals and independent apical foramina, according to type IV of Vertucci’s classification of first and second mandibular molars. The canals were instrumented at three different WLs: G-1, 1mm short of the major apical foramen; G0, at the major apical foramen; G+1, 1mm beyond the major apical foramen. The groups were assessed for changes in root canal volume and untouched wall area in the apical third. Groups G0 and G+1 were also compared for percentage of untouched walls at the apical foramen. One-way ANOVA (post hoc Tukey test) and Student’s t-test adopted a 5% level of significance. Results Root canal volumes (mm3) in the apical third were 22.86±10.46, 44.48±24.91, and 55.71±21.32 in G-1, G0 and G+1, respectively. G-1 volume following instrumentation increased significantly less than that of G0 or G+1 (P>.05); G0 did not differ from G+1. The percentage of untouched wall area in the apical third did not differ among the three groups (P>.05). G0 and G+1 did not differ regarding untouched walls in the major apical foramem walls. Conclusion Extending the WL from 1mm short of the apical foramen to a point at and beyond the WL increases the apical third volume without increasing the prepared area. Untouched surface areas of the apical foramen were not modified by instrumentation at or beyond the foramen. Resumo Objetivos: avaliar a mortalidade materna de janeiro de 2019 a janeiro de 2020 no estado do Ceará. Métodos: trata-se de um estudo ecológico descritivo e retrospectivo. A coleta de dados foi realizada no período outubro a novembro de 2020, considerando o intervalo temporal de janeiro de 2019 a janeiro de 2020, através das informações registradas em banco de dados de domínio público do estado do Ceará. As variáveis consideradas para delinear o perfil dos óbitos maternos foram: faixa etária, as causas, estado civil, cor/raça e escolaridade. A análise dos dados ocorreu pela estatística descritiva por distribuição de frequências e foi apresentada por meio de tabelas. Resultados: foram registrados 135 óbitos maternos, considerando causas externas, diretas (mais prevalentes), indiretas e tardias. A razão de mortalidade materna no Ceará foi de 60.43%, cujo perfil dos óbitos foi caracterizado por mulheres com nível superior incompleto (41.5%), pardas (80%), na faixa etária de 20-29 anos (43%) e solteiras (66.7%). Conclusão: as informações apresentadas neste estudo são de extrema relevância, pois possibilitam a compreensão de uma população-chave, as mulheres, além de auxiliar no delineamento de estratégias para o enfrentamento da mortalidade materna.

Abstract Objectives: evaluate maternal mortality from January 2019 to January 2020 in the state of Ceará. Methods: this is a descriptive and retrospective ecological study. Data collection was carried out from October to November 2020, considering the time interval from January 2019 to January 2020, through information registered in a public domain database in the State of Ceará, Brazil. The variables considered to outline the profile of maternal deaths were: age group, causes, marital status, color/race and education. Data analysis was performed using descriptive statistics by frequency distribution and presented through tables. Results: 135 maternal deaths were registered, considering external, direct (most prevalent), indirect and late causes. The maternal mortality ratio in Ceará was 60.43%, whose profile of deaths was characterized by women with incomplete higher education (41.5%), brown (80%), in their 20-29 years (43%) and single (66.7%). Conclusion: the information in this study is extremely importance, as it allows the understanding of a key population, women, in addition to assisting in the design of strategies for coping with maternal mortality.

ABSTRACT OBJECTIVE Summarize the literature on the relationship between composite socioeconomic indicators and mortality in different geographical areas of Brazil. METHODS This scoping review included articles published between January 1, 2000, and August 31, 2020, retrieved by means of a bibliographic search carried out in the Medline, Scopus, Web of Science, and Lilacs databases. Studies reporting on the association between composite socioeconomic indicators and all-cause, or specific cause of death in any age group in different geographical areas were selected. The review summarized the measures constructed, their associations with the outcomes, and potential study limitations. RESULTS Of the 77 full texts that met the inclusion criteria, the study reviewed 24. The area level of composite socioeconomic indicators analyzed comprised municipalities (n = 6), districts (n = 5), census tracts (n = 4), state (n = 2), country (n = 2), and other areas (n = 5). Six studies used composite socioeconomic indicators such as the Human Development Index, Gross Domestic Product, and the Gini Index; the remaining 18 papers created their own socioeconomic measures based on sociodemographic and health indicators. Socioeconomic status was inversely associated with higher rates of all-cause mortality, external cause mortality, suicide, homicide, fetal and infant mortality, respiratory and circulatory diseases, stroke, infectious and parasitic diseases, malnutrition, gastroenteritis, and oropharyngeal cancer. Higher mortality rates due to colorectal cancer, leukemia, a general group of neoplasms, traffic accident, and suicide, in turn, were observed in less deprived areas and/or those with more significant socioeconomic development. Underreporting of death and differences in mortality coverage in Brazilian areas were cited as the main limitation. CONCLUSIONS Studies analyzed mortality inequalities in different geographical areas by means of composite socioeconomic indicators, showing that the association directions vary according to the mortality outcome. But studies on all-cause mortality and at the census tract level remain scarce. The results may guide the development of new composite socioeconomic indicators for use in mortality inequality analysis.

RESUMO Objetivo: as recomendações das decisões em Tumor Board (TB) deveriam ser acompanhadas para identificar barreiras que possam interferir na execução do melhor cuidado para o paciente decidido previamente. O objetivo do estudo é avaliar se a decisão de conduta em TB foi realizada em pacientes com tumores pancreáticos, o status de vida 90 dias após TB e analisar os motivos pelos quais a conduta não foi realizada. Métodos: estudo retrospectivo com pacientes com tumores de pâncreas, avaliados entre 2017 a 2019. Dados epidemiológicos, se a conduta de TB foi realizada, o motivo da não realização, o status de vida em 90 dias após decisão de TB e quantas vezes cada paciente foi discutido em reunião foram coletados. As variáveis categóricas foram comparadas pelo teste de qui-quadrado; variáveis numéricas foram apresentadas como médias e desvio padrão. Resultados: 111 casos, 95 pacientes, 86 (90,5%) com diagnóstico de câncer. Após 90 dias de TB, 83 pacientes (87,37%) permaneceram vivos, 9 pacientes (9,47%) faleceram e 3 (3,16%) perderam o seguimento. A conduta do TB não foi realizada em 12 (10,8%) dos casos e os motivos foram: 25% (3) por perda de seguimento, 8,33% (1) por recusa do paciente e 66,67% (8) devido à piora clínica. Os casos de pacientes com metástases tiveram menor execução de conduta de TB (p=0,006). Conclusões: a conduta do TB é realizada na maior parte dos casos e o motivo mais evidente para o não cumprimento das condutas é a piora clínica do paciente.

ABSTRACT Objective: the recommendations of the decisions made by the Tumor Board (TB) should be followed to identify barriers that may interfere with the execution of the previously decided, best care for the patient. The aim of this study is to assess whether the TB conduct decision was performed in patients with pancreatic tumors, their life status 90 days after the TB decision, and to analyze the reasons why the conduct was not performed. Methods: we conducted a retrospective study with patients with pancreas tumors, evaluated between 2017 and 2019. We collected data on epidemiological status, whether the TB procedure was performed, the reason for not performing it, life status 90 days after the TB decision, and how many times each patient was discussed at a meeting. We compared categorical variables using the chi square test, numerical variables were presented as means and standard deviation. Results: we studied 111 session cases, in 95 patients, 86 (90.5%) diagnosed with cancer. After 90 days of TB, 83 patients (87.37%) remained alive, 9 had (9.47%) died, and 3 (3.16%) were lost to follow-up. The TB decision was not observed in 12 (10.8%) cases and the reasons were: 25% (3) for loss of follow-up, 8.33% (1) for patient refusal, and 66.67% (8) due to clinical worsening. The cases of patients with metastases had a lower rate of TB conduct compliance (p=0.006). Conclusions: the TB conduct was performed in most cases and the most evident reason for non-compliance with the conducts is the patient’s clinical worsening.

SUMMARY: Azygos lobe is one of the most common pulmonary variations. Although numerous studies discuss to the clinical and surgical implications of anatomical variations, there are few investigations about this topic. The aim of this study was to discuss the anatomy of the pulmonary azygos lobe and its possible clinical and surgical correlations. In this systematic review, a total of 48 results were found between 1968 and 2020. After application of eligibility criteria, 16 articles were included to analysis. Our results showed there were no reports of surgery complications in azygos lobe presence. In all articles that described the anatomical variation, the pulmonary azygos lobe received tertiary segmental branches. Among the surgical implications in the presence of the azygos lobe, complications are of low complexity. It is necessary to teach about this anatomical variation in medical schools in cadavers. Emphasizing the obvious morphological modifications in the superior mediastinum.

RESUMEN: El lóbulo ácigos es una de las variaciones pulmonares más comunes. Aunque numerosos estudios discuten las implicaciones clínicas y quirúrgicas de las variaciones anatómicas, existen pocas investigaciones sobre este tema. El objetivo de este estudio fue discutir la anatomía del lóbulo ácigos pulmonar y sus posibles correlaciones clínicas y quirúrgicas. En esta revisión sistemática se encontraron un total de 48 resultados entre los años 1968 y 2020. Después de la aplicación de los criterios de elegibilidad, 16 artículos fueron incluidos para análisis. Nuestros resultados mostraron que no hubo informes de complicaciones quirúrgicas en presencia del lóbulo ácigos. En todos los artículos que describieron la variación anatómica, el lóbulo ácigos pulmonar recibió ramas segmentarias terciarias. Dentro de las implicaciones quirúrgicas en presencia del lóbulo ácigos, las complicaciones son de baja complejidad. En las escuelas de medicina es necesaria la enseñanza en cadáveres de esta variación anatómica, destacando las evidentes modificaciones morfológicas en el mediastino superior.
