ABSTRACT Purpose: This study aimed to compare the safety and effectiveness of intraocular pressure reduction between micropulse transscleral cyclophotocoagulation and “slow cook” transscleral cyclophotocoagulation in patients with refractory primary open-angle glaucoma. Methods: We included patients with primary open angle glaucoma with at least 12 months of follow-up. We collected and analyzed data on the preoperative characteristics and postoperative outcomes. The primary outcomes were a reduction of ≥20% of the baseline value (criterion A) and/or intraocular pressure between 6 and 21 mmHg (criterion B). Results: We included 128 eyes with primary open-angle glaucoma. The preoperative mean intraocular pressure was 25.53 ± 6.40 and 35.02 ± 12.57 mmHg in the micropulse- and “slow cook” transscleral cyclophotocoagulation groups, respectively (p<0.001). The mean intraocular pressure was reduced significantly to 14.33 ± 3.40 and 15.37 ± 5.85 mmHg in the micropulse- and “slow cook” transscleral cyclophotocoagulation groups at the last follow-up, respectively (p=0.110). The mean intraocular pressure reduction at 12 months was 11.20 ± 11.46 and 19.65 ± 13.22 mmHg in the micropulse- and “slow cook” transscleral cyclophotocoagulation groups, respectively (p<0.001). The median preoperative logMAR visual acuity was 0.52 ± 0.69 and 1.75 ± 1.04 in the micropulse- and “slow cook” transscleral cyclophotocoagulation groups, respectively (p<0.001). The mean visual acuity variation was −0.10 ± 0.35 and −0.074 ± 0.16 in the micropulse- and “slow cook” transscleral cyclophotocoagulation, respectively (p=0.510). Preoperatively, the mean eye drops were 3.44 ± 1.38 and 2.89 ± 0.68 drugs in the micropulse- and “slow cook” transscleral cyclophotocoagulation groups, respectively (p=0.017), but those were 2.06 ± 1.42 and 1.02 ± 1.46 at the end of the study in the “slow cook” and micropulse transscleral cyclophotocoagulation groups, respectively (p<0.001). The success of criterion A was not significant between both groups. Compared with 11 eyes (17.74%) in the “slow cook” transscleral cyclophotocoagulation group, 19 eyes (28.78%) in the micropulse transscleral cyclophotocoagulation group showed complete success (p=0.171). For criterion B, 28 (42.42%) and 2 eyes (3.22%) showed complete success after micropulse- and “slow cook” transscleral cyclophotocoagulation, respectively (p<0.001). ABSTRACT The aim of this study was to characterise the lignocellulosic fractions of pruning waste from tree species showing potential for furniture production in Acaraú, Ceará. The experiment was conducted in a completely randomised, split-plot design, with the plots corresponding to two water regimes (irrigated for one year and irrigated for three years) and the sub-plots to eight tree species, with three replications. The chemical attributes were determined after seven years. Except for the insoluble lignin content, there was a significant effect from the different species in terms of the chemical properties of the wood. The results for the lignocellulosic fractions show the possibility of developing technological ways of adding value to these waste products, especially considering their lignin content. The different species also showed potential for use in producing cellulose pulp, with the exception of Handroanthus impetiginosus due to its high ash content. The results of this study offer new perspectives for future research into the use of forestry waste. Abstract A recent case in Argentina, where a child was generated through in Medically Assisted Procreation and was not the couple's biological daughter, reopened discussions about possible errors involving reproductive techniques. In this article, based on a bibliographical and documentary review, in addition to the responsibility of the centers that perform the procedure and the professionals involved, we deal with something more delicate: the rights of those who are born by “carelessness”, whose paternity is unknown or questioned or even rejected, after life has been generated in an unconventional way. Technological advances in human reproduction are undeniable and, in general, include couples prevented from having biological children without the use of such techniques, as is the case in Portugal and Brazil. However, life and ethics must be linked so that everyone's rights are respected. And, more specifically, so that the person does not have their life violated at birth, growing up amid legal disputes and rejection of parents who are faced with DNA test results that confirm the absence of the expected genetic bond. In such cases, paternity can be disputed, even by the “owners” of eggs, sperm or embryos frozen and used by mistake, to generate, by reflex, the existence of orphan newborn children, without parents.

Resumo Caso recente na Argentina, onde uma criança foi gerada por meio de Procriação Medicamente Assistida e não era filha biológica do casal, reabriu as discussões sobre possíveis erros envolvendo técnicas reprodutivas. Neste artigo, feito a partir de revisão bibliográfica e documental, para além da responsabilidade dos centros que realizam o procedimento e dos profissionais envolvidos, tratamos de algo mais delicado: os direitos de quem nasce por “descuido”, tendo a paternidade desconhecida ou questionada ou até mesmo rejeitada, após a vida ter sido gerada de modo não convencional. Os avanços tecnológicos na área da reprodução humana são inegáveis e, de modo geral, contemplam casais impedidos de ter filhos biológicos sem o uso de tais técnicas, como se verifica em Portugal e no Brasil. Entretanto, vida e ética devem estar atreladas para que os direitos de todos sejam respeitados. E, mais especificamente, para que a pessoa não tenha sua vida violada ao nascer, crescendo em meio a disputas judiciais e rejeição de pais que se depararam com resultados de testes de DNA que confirmam a inexistência de esperado vínculo genético. Em tais casos, a paternidade pode ser contestada, inclusive pelos “donos” de óvulos, esperma ou embriões congelados e utilizados por engano, a gerar, por reflexo, a existência de nascituros órfãos, sem pais.

RESUMO Objetivo: Revisar sistematicamente a literatura em busca das intervenções e indicações nutricionais mais adequadas e eficazes para o tratamento nutricional de crianças e adolescentes com paralisia cerebral (PC). Fontes de dados: Esta revisão foi conduzida de acordo com as diretrizes Preferred Reporting Items for Systematic Reviews and Meta-Analyses — PRISMA. Os artigos foram selecionados em sete bases de dados (Cochrane, Literatura Latino-Americana e do Caribe em Ciências da Saúde — Lilacs, Embase, United States National Library of Medicine — PubMed, Scientific Electronic Library Online — SciELO, Scopus, and Web of Science). Foram incluídos estudos de um grupo pediátrico (zero a 18 anos) com diagnóstico de PC e a estratégia de busca incluiu os descritores: “children” OR “childhood” AND “nutritional therapy” OR “nutritional intervention” OR “nutrition” OR “nutritional support” OR “diet” AND “cerebral palsy” OR “cerebral injury”. A qualidade metodológica foi avaliada utilizando a lista de verificação para estudos transversais analíticos, escala Newcastle-Ottawa ou ferramenta da Cochrane Collaboration, para ensaios clínicos. Síntese dos dados: Quinze estudos (n=658) publicados de 1990 a 2020 preencheram os critérios de inclusão. Todos tiveram baixo risco de viés. Os dados mostraram que crianças e adolescentes com PC apresentam pior estado nutricional do que os normalmente desenvolvidos. Aqueles que receberam suplementação nutricional hipercalórica e hiperproteica beneficiaram-se de seu uso. Estudos demonstram que a nutrição enteral deve ser considerada quando as necessidades nutricionais não são supridas pela dieta oral, principalmente nos casos em que as funções oromotoras estão prejudicadas. Além disso, houve relação direta entre a consistência dos alimentos, o nível de função motora e o estado nutricional. Conclusões: Crianças e adolescentes com PC têm maior risco de desnutrição. O uso de suplementação nutricional pode auxiliar no ganho de peso desses pacientes. ABSTRACT Objective: To systematically review the literature in search of the most suitable and effective nutritional interventions and indications for the nutritional treatment of children and adolescents with cerebral palsy (CP). Data source: This review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The articles were selected from seven databases (Cochrane, Literatura Latino-Americana e do Caribe em Ciências da Saúde — Lilacs, Embase, United States National Library of Medicine — PubMed, Scientific Electronic Library Online — SciELO, Scopus, and Web of Science). The data showed that children and adolescents with CP have worse nutritional status than those normally developed. Studies indicate that enteral nutrition should be considered when nutritional needs are not met by the oral diet, especially in cases where oral motor functions are impaired. ABSTRACT OBJECTIVE To analyze the incompleteness and trend of incompleteness of the race/color variable in hospitalizations due to COVID-19 whose outcome was death, in Brazil, between April 2020 and April 2022. METHODS Ecological time series study on the incompleteness of the race/color variable in hospitalizations due to COVID-19 whose outcome was death in Brazil, its macro-regions and Federative Units (FU), by joinpoint regression, calculation of Monthly Percent Change (MPC) and Average Monthly Percent Change (AMPC), based on data from the Hospital Information System of the Unified Health System (SIH/SUS). RESULTS The incompleteness of the race/color variable in COVID-19 hospitalizations with a death outcome in Brazil was 25.85%, considered poor. All regions of the country had a poor degree of incompleteness, except for the South, which was considered regular. In the period analyzed, the joinpoint analysis revealed a stable trend in the incompleteness of the race/color variable in Brazil (AMPC = 0.54; 95%CI: -0.64 to 1.74; p = 0.37) and in the Southeast (AMPC = -0.61; 95%CI: -3.36 to 2.22; p = 0.67) and North (AMPC = 3.74; 95%CI: -0.14 to 7.78; p = 0.06) regions. The South (AMPC = 5.49; 95%CI: 2.94 to 8.11; p = 0.00002) and Northeast (AMP = 2.50; 95%CI: 0.77 to 4.25; p = 0.005) regions showed an increase in the incompleteness trend, while the Midwest (AMPC = -2.91 ; 95%CI: -5.26 to -0.51; p = 0.02) showed a downward trend. CONCLUSION The proportion of poor completeness and the stable trend of incompleteness show that there was no improvement in the quality of filling in the race/color variable during the COVID-19 pandemic in Brazil, a fact that may have increased health inequalities for the black population and made it difficult to plan strategic actions for this population, considering the pandemic context. The results found reinforce the need to encourage discussion on the subject, given that the incompleteness of health information systems increases inequalities in access to health services and compromises the quality of health data.

ABSTRACT This study aimed to identify factors associated with colonization by community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) in adult patients admitted to a Brazilian hospital. This is a cross-sectional study, in which patients underwent a nasal swab and were asked about hygiene behavior, habits, and clinical history. Among the 702 patients, 180 (25.6%) had S. aureus and 21 (2.9%) MRSA. The factors associated with MRSA colonization were attending a gym (OR 4.71; 95% CI; 1.42 – 15.06), smoking habit in the last year (OR 2.37; 95% CI; 0.88 – 6.38), previous hospitalization (OR 2.18; CI 95%; 0.89 – 5.25), and shared personal hygiene items (OR 1.99; 95% CI; 0.71 – 5.55). At the time of admission, colonization by CA-MRSA isolates was higher than that found in the general population. This can be an important public health problem, already endemic in hospitals, whose factors such as those associated with habits (smoking cigarettes) and behaviors (team sports practice and activities in gyms) have been strongly highlighted. Resumo Objetivo: Este é um estudo de coorte retrospectivo para analisar os desfechos em longo prazo de pacientes com fratura da coluna toracolombar submetidos à fixação percutânea minimamente invasiva. Métodos: Os casos de 17 pacientes com fraturas da coluna toracolombar submetidos à fixação percutânea entre 2009 e 2011 foram objeto de análise retrospectiva. Variáveis clínicas e radiográficas foram coletadas. A avaliação clínica foi baseada nos questionários SF-36 e Oswestry. Os parâmetros radiográficos foram avaliados de acordo com a classificação de fratura baseada nos critérios de Magerls, o ângulo de cunha da vértebra fraturada e o ângulo de Cobb do segmento acometido. As medidas foram feitas em diferentes momentos: antes da cirurgia, imediatamente após a cirurgia, um ano depois e no acompanhamento tardio (cinco anos depois). Lesões associadas a traumas, complicações pós-cirúrgicas e relacionadas a implantes também foram consideradas. Resultados: O questionário SF-36 apresentou médias acima de 63,5% em todos os domínios no período pós-operatório tardio (a partir de cinco anos após a cirurgia). As respostas do questionário Oswestry mostraram limitações físicas mínimas ou nulas em 80% dos pacientes, com pontuação média de 10,8% ± 10,5%. O valor médio do ângulo de Cobb foi de 5,53º± 13,80º de cifose no período pré-operatório, 2,18º ± 13,38º de cifose no pós-operatório imediato, 5,26 ±13,95º de cifose no pós-operatório de um ano e de 8,78º ± 15,06º de cifose no período pós-operatório tardio. Abstract Objective: This is a retrospective cohort study to analyze the long-term outcomes of thoracolumbar spine fracture patients who underwent minimally invasive percutaneous fixation. Methods: The cases of 17 patients with thoracolumbar spine fractures who had percutaneous fixation between 2009 and 2011 were the subject of a retrospective analysis. Clinical and radiographic variables were collected. For the clinical evaluation the questionnaires SF-36 and Oswestry were used. Radiographic parameters were evaluated using fracture’s classification based on Magerls’s criteria, the fractured vertebra’s wedging angle, and the affected segment’s segmental Cobb angle. The measures were made at different stages: before surgery, immediately after surgery, one year later, and at a late follow-up (5 years later). Trauma associated injuries, postsurgical and implant related complications were among the additional information taken into account. Results: The SF-36 questionnaire showed averages above 63,5% in all domains in the late postoperative data (from 5 years after the surgery). Oswestry questionnaire answers showed minimal or no physical limitations in 80% of the patients with a mean score of 10,8% ± 10,5. The average preoperative Cobb angle value was 5,53º ± 13,80º of kyphosis, the immediate postoperative 2,18º ± 13,38º of kyphosis, one year postoperative 5,26 ± 13,95º of kyphosis, and the late follow-up 8,78º ± 15,06º of kyphosis. The mean correction was 3,35º, and mean loss of correction was 6,6º. There were no complications observed, no case of neurological deficit, infection or implant failure occurred. Conclusion: Thoracolumbar vertebrae fractures can be surgically treated with positive late clinical and radiological outcomes and low complication rates using a minimally invasive percutaneous method. Abstract Objective To validate the content and appearance of a graphic protocol for evaluating safe nursing care for hemodialysis patients. Methods Methodological study with a quantitative approach, organized into three procedures: theoretical from a Scoping Review; empirical in which the process of constructing the graphic protocol and checklist for the evaluation of safe care took place; finally, the analytics for the validation itself using the Delphi technique and the participation of nine expert judges in two rounds to reach agreement. Results The checklist and the graphic protocol were elaborated. As for content validity in Delphi I, three criteria obtained Content Validity Coefficient =0.77 in the checklist. Resumen Objetivo Validar el contenido y la apariencia de un protocolo gráfico para la evaluación del cuidado seguro de enfermería a pacientes en hemodiálisis. RESUMO Introdução: a sedação e analgesia são procedimentos fundamentais para crianças submetidas a intervenções invasivas, devendo-se evitar complicações durante sua realização. A simulação in situ permite, por sua vez, capacitações nos ambientes reais de prática para aprimorar as competências técnicas e não técnicas dos profissionais para tais procedimentos. Embora seja uma ferramenta de grande utilidade, muitas vezes não é aproveitada pelo despreparo para seu planejamento e aplicação. Objetivo: elaborar e validar um cenário de simulação in situ no atendimento de urgências pediátricas com uso de sedação para realização de procedimento invasivo. Método: estudo descritivo de construção e validação de conteúdo de um cenário de simulação in situ, por meio do método Delphi, seguindo os seguintes passos: 1) definição do problema e seleção de especialistas; 2) desenvolvimento do documento inicial; 3) rodadas para validação com análise das respostas e feedback (até obtenção do consenso pelo Índice de Validação de Conteúdo); 4) relatório final. Resultados: Os especialistas indicaram sugestões devidamente aproveitadas e o cenário obteve, em todos os itens, um IVC maior que 80,0% demonstrando sua alta validade e confiabilidade. ABSTRACT Introduction: sedation and analgesia are fundamental procedures for children undergoing invasive interventions, and complications must be avoided during their implementation. In situ simulation allows, in turn, training in real practice environments to improve the technical and non-technical skills of professionals for such procedures. Although it is a very useful tool, it is often not used due to lack of preparation for its planning and application. Objective: develop and validate an in situ simulation scenario in pediatric emergency care using sedation to perform an invasive procedure. Method: descriptive study of construction and content validation of an in situ simulation scenario, using the Delphi method, following the following steps: 1) definition of the problem and selection of experts; 2) development of the initial document; 3) rounds for validation with analysis of responses and feedback (until consensus is reached by the Content Validation Index); 4) final report. RESUMO RACIONAL: O câncer colorretal é o terceiro tipo de câncer mais comum no Brasil apesar dos métodos de rastreamento que reduzem seu risco. A colonoscopia é o único método com possibilidade terapêutica. O adequado rastreio por colonoscopia está relacionado à qualidade dos exames, avaliados de acordo com critérios de qualidade recomendados por diversas instituições. Dentre os fatores, o mais utilizado é a Taxa de Detecção de Adenoma, que deve ser de pelo menos 25% na população geral. OBJETIVOS: Avaliar a qualidade das colonoscopias de rastreamento do câncer colorretal realizadas em um centro brasileiro. MÉTODOS: Estudo retrospectivo avaliando os indicadores de qualidade em colonoscopias de rastreio nos pacientes assintomáticos com mais de 45 anos. O desfecho primário foi a Taxa de Detecção de Adenoma, e os secundários incluíram a taxa de detecção de pólipos e a segurança do procedimento. Subanálises incluíram a correlação dos achados endoscópicos com sexo e idade e a evolução das taxas de detecção de adenoma ao longo dos anos. RESULTADOS: Foram incluídos 2.144 pacientes, com media de idade de 60.54 anos. Pólipos foram diagnosticados em 68,6% dos procedimentos. ABSTRACT BACKGROUND: Colorectal cancer is the third most common type of cancer in Brazil, despite the availability of screening methods that reduce its risk. Colonoscopy is the only screening method that also allows therapeutic procedures. The proper screening through colonoscopy is linked to the quality of the exam, which can be evaluated according to quality criteria recommended by various institutions. Among the factors, the most used is the Adenoma Detection Rate, which should be at least 25% for general population. AIMS: To evaluate the quality of the screening colonoscopies performed in a quarternary private Brazilian hospital. METHODS: This is a retrospective study evaluating the quality indicators of colonoscopies performed at a private center since its inauguration. Only asymptomatic patients aged over 45 years who underwent screening colonoscopy were included. The primary outcome was the Adenoma Detection Rate, and secondary outcomes included polyps detection rate and safety profile. Subanalyses evaluated the correlation of endoscopic findings with gender and age and the evolution of detection rates over the years. RESULTS: A total of 2,144 patients were include with a mean age of 60.54 years-old. Polyps were diagnosed in 68.6% of the procedures. Adenoma detection rate was 46.8%, with an increasing rate over the years, mainly in males. A low rate of adverse events was reported in 0.23% of the cases, with no need for surgical intervention and no deaths. CONCLUSIONS: This study shows that high quality screening colonoscopy is possible when performed by experienced endoscopists and trained nurses, under an adequate infrastructure. RESUMO Objetivo: descrever o processo de tradução e adaptação transcultural da escala Mayo High Performance Team Scale para o português brasileiro. Método: estudo descritivo de validação e adaptação transcultural da escala, realizado de maneira virtual, seguindo pressupostos propostos por Beaton e colaboradores. Contou com uma amostra de 40 especialistas, e realizou duas rodadas, sendo uma de validação e uma de apreciação final. ABSTRACT Objective: describe the process of translation and cross-cultural adaptation of the Mayo High Performance Team Scale into Brazilian Portuguese. Method: descriptive study of validation and cross-cultural adaptation of the scale, carried out virtually, following assumptions proposed by Beaton and collaborators. Abstract The present study aimed to investigate the contextual inequalities of specialized public dental care (SPDC) in Brazil. The outcome was the trajectory of dental specialized production in municipalities with SPDC (from 2015 to 2017) obtained by group-based trajectory modeling. A Poisson regression model was used to analyze the factors associated with the high trajectory of SPDC production. The inequality indicators for SPDC production were the slope index and the concentration index according to contextual factors. The study included 954 SPDC units distributed across 893 municipalities. Among the municipalities evaluated, 62.9% had a low trajectory of SPDC. Large-sized municipalities had the highest production (IRR = 2.84, 95%CI: 1.94–4.14) and the southern region had the lowest production (IRR = 0.73, 95%CI: 0.58–0.92). Municipalities presenting a very high human development index (HDI) showed the greatest SPDC production (IRR = 3.34, 95%CI: 1.09–10.24), as well as municipalities with the highest tertile of schooling rate (IRR = 1.23, 95%CI: 1.00–1.50). The absolute inequality was 52.1 percentage points for the average monthly wage (p < 0.001), 61.0 percentage points for the HDI (p < 0.001), -22.1 for infant mortality rate (p <0.001), and 14.8 for the schooling rate (p = 0.012). RESUMO OBJETIVO Analisar a incompletude e a tendência da incompletude da variável raça/cor nas internações por covid-19 cujo desfecho foi óbito, no Brasil, no período entre abril de 2020 e abril de 2022. MÉTODOS Estudo ecológico de série temporal sobre a incompletude da variável raça/cor nas internações por covid-19 cujo desfecho foi óbito no Brasil, suas macrorregiões e Unidades Federativas (UF), pela regressão por joinpoint, cálculo da Monthly Percent Change (MPC) e Average Monthly Percent Change (AMPC), a partir de dados do Sistema de Informação Hospitalar do Sistema Único de Saúde (SIH/SUS). RESULTADOS A incompletude da variável raça/cor nas internações por covid-19 cujo desfecho foi óbito no Brasil foi 25,85%, considerada ruim. Todas as regiões do país tiveram grau de incompletude ruim, exceto a Região Sul, considerada regular. ABSTRACT OBJECTIVE To analyze the incompleteness and trend of incompleteness of the race/color variable in hospitalizations due to COVID-19 whose outcome was death, in Brazil, between April 2020 and April 2022. RESULTS The incompleteness of the race/color variable in COVID-19 hospitalizations with a death outcome in Brazil was 25.85%, considered poor. All regions of the country had a poor degree of incompleteness, except for the South, which was considered regular. In the period analyzed, the joinpoint analysis revealed a stable trend in the incompleteness of the race/color variable in Brazil (AMPC = 0.54; 95%CI: -0.64 to 1.74; p = 0.37) and in the Southeast (AMPC = -0.61; 95%CI: -3.36 to 2.22; p = 0.67) and North (AMPC = 3.74; 95%CI: -0.14 to 7.78; p = 0.06) regions. Resumo Descobertas recentes de deposições funerárias no sítio Uruguai 1 contribuem com novos elementos sobre o comportamento mortuário relativos à unidade arqueológica Guarani. Tratam-se de cinco sepultamentos que incluem uma deposição em urna e ao menos quatro diretamente no solo, sendo a primeira vez que se documenta este último tipo de deposição para o alto vale do rio Uruguai. A ocupação Guarani desse sítio se situa da primeira metade do século XV até o século XVIII (510 ± 20 e 240 ± 30 anos 14C AP). Esse sítio está inserido no contexto da área arqueológica da Foz do Chapecó, que vem sendo estudada nas últimas duas décadas e na qual já foram documentadas pelo menos outras sete deposições funerárias, todas em urna. Neste artigo, apresentamos os achados de campo e laboratório sobre as deposições do sítio Uruguai 1, assim como novas datações radiocarbônicas para o nível analisado. Abstract Recent discoveries of funerary depositions at the Uruguay 1 site provide new elements on the mortuary behavior related to the Guarani archaeological unit. Five burials including one urn deposition and at least four directly in the ground were discovered. This last behavior had not yet been documented for the upper Uruguay river. The Guarani occupation of this site dates from the first half of the 15th century to the 18th century (510 ± 20 and 240 ± 30 years 14C AP). This site is inserted in the context of the archaeological area of Foz do Chapecó, which has been studied in the last two decades and in which at least seven other burials in urns have already been documented. In this article we present new field and laboratory findings on depositions from the Uruguai 1 site, as well as new radiocarbon dates for the level analyzed.
