RESUMO: O vírus da diarreia viral bovina (BVDV) é um vírus que afeta a produção e reprodução com alto impacto econômico global nos sistemas de produção de leite. Nosso objetivo foi investigar a prevalência do BVDV em rebanhos e indivíduos na região de maior produção de leite do Brasil. A frequência do BVDV nos rebanhos foi investigada usando reação em cadeia da polimerase com transcrição reversa quantitativa (RT-qPCR) em tanques de leite em 289 fazendas leiteiras, monitoradas entre agosto de 2020 e janeiro de 2022. Dentre esses rebanhos, 68 sistemas de produção foram selecionados para investigar a prevalência de animais persistentemente infectados (PI) usando dois testes imunoenzimáticos para a detecção do antígeno (Ag-ELISAs), em intervalos de 21 dias, a partir de amostras de tecido auricular. No total, foram realizados 2.902 RT-qPCR e 23.466 Ag-ELISAs. Na investigação dos rebanhos, 23,9% (69/289) das propriedades leiteiras foram consideradas infectadas, apresentando pelo menos um teste qPCR positivo. Na pesquisa de animais Pis, 41,2% (28/68) das fazendas selecionadas tiveram pelo menos um animal positivo no teste Ag-ELISA. A associação entre os testes permitiu classificar as propriedades em quatro categorias: nível 0, negativo para ambos os testes (41,2%, 28/68); nível 1, RT-qPCR positivo e Ag-ELISA negativo (17,6%, 12/68); nível 2, RT-qPCR negativo e Ag-ELISA positivo (13,23%, 9/68); e nível 3, positivo para ambos os testes (27,9%, 19/68). A análise de correspondência múltipla (MCA) sugeriu associação entre positividade para BVDV e grandes propriedades, produção média diária de leite do rebanho, raça e contagem de células somáticas. O confinamento e a intensificação de animais de diferentes categorias e o uso de camas artificiais estão associados à infecção pelo BVDV. O uso de leite residual ou a granel também foi um fator de risco para positividade para BVDV em RT-qPCR e Ag-ELISA. Apesar da utilização de vacinas reprodutivas pela maioria dos produtores, a sua utilização parece estar associada a explorações positivas para BVDV. Este estudo apresenta as frequências epidemiológicas do BVDV nos rebanhos e indivíduos de rebanhos da região dos Campos Gerais Paranaense. A região está entre as principais áreas produtoras de leite do Brasil. ABSTRACT: Bovine viral diarrhea virus (BVDV) is a productive and reproductive virus with a high global economic impact on dairy production systems. We investigated the prevalence of BVDV at the herd and individual levels in most dairy-producing regions of Brazil. The frequency of BVDV at the herd level was investigated using quantitative reverse transcription polymerase chain reaction (RT-qPCR) in bulk milk tanks monitored at 289 dairy farms between August 2020 and January 2022. Among these farms, 68 production systems were selected to investigate the prevalence of persistently infected (PI) animals using two antigen-enzyme-linked immunosorbent assays (Ag-ELISAs), at 21-d intervals, from ear-notch samples. In total, 2,902 RT-qPCR and 23,466 Ag-ELISAs were performed. At the herd level, 23.87% (69/289) of dairy farms were considered infected, presenting at least one qPCR test positive. At the individual level, 41.2% (28/68) of the subgroup of selected farms had at least one animal positive in the Ag-ELISA test. Association between tests allowed the classification of farms into the following four categories: level 0, negative for both tests (41.2%, 28/68); level 1, RT-qPCR positive and Ag-ELISA negative (17.6%, 12/68); level 2, RT-qPCR negative and Ag-ELISA positive (13.2%, 9/68); and level 3, positive for both tests (27.9%, 19/68). Multiple correspondence analysis (MCA) suggested a possible association between BVDV positivity and large farms, average daily milk production (herd), breed, and somatic cell counts. The confinement and intensification of animals from different categories, and use of artificial bedding are associated with BVDV infection. Using waste or bulk tank milk to fed calves was also a risk factor for BVDV positivity in RT-qPCR and Ag-ELISA. Despite the use of reproductive vaccines by most producers, their use seems to be associated with BVDV-positive farms. This study presented the epidemiological frequencies of BVDV at the individual and herd levels in the Campos Gerais Paranaense region. The region ranks among the top milk-producing areas in Brazil. Additionally, the association between BVDV tests and farm characteristics indicated the farm risk for BVDV and guides specific control programs. This paper presents the stages of an acceleration program focused on chemical technologies. We demonstrate how this initiative contributes to the creation of new businesses, startups, and technology licensing from research developed at Brazilian universities. The case of the program Escale-se is discussed. There were 66 technologies submitted, and 14 were selected for the pre-acceleration phase. In this phase, various contents were worked on, such as creating a business model and aspects of scaling hard-science technologies. Teams were evaluated by panels with market specialists, and 6 were selected for the second phase: pre-scaling/proof of concept. Here, proponents were encouraged to validate the business model by connecting with potential clients and partners, as well as planning the development and testing of the product in a relevant environment. Finally, one team was classified for the last stage: scaling/go-to-market - where they received R$ 100,000.00 to scale up their technology and create strategies for raising investments. Four of the fourteen technologies selected for the program opened National Register of Legal Entities (CNPJ) and raised approximately R$ 4.5 M in investments from different sources. ABSTRACT The intensification of cattle post-weaning phase in tropical pastures depends on supplementation strategies. The objective of this study was to assess the effect of increasing levels of concentrate supplement on the intake, apparent digestibility of nutrients, feeding behavior, and performance of Girolando steers on tropical pasture during the post-weaning phase in the rainy season. Forty uncastrated male Girolando steers, initially weighing 266.62 ± 32.95 kg and aged 11 mo, were allocated into four concentrate supplement levels: 0.2%, 0.3%, 0.4%, and 0.5% body weight. The steers grazed on Urochloa brizantha (Hochst. ex A. Rich.) R.D. Webster ‘Marandu’ pasture. The intakes of supplement DM, non-fibrous carbohydrates (NFC), and total digestible nutrients increased linearly (P < 0.05) with the level of supplement offered to the steers. In a similar response, the apparent digestibility of DM and NFC from the diet also rose linearly (P < 0.05). Grazing time, total feeding time, and total chewing time decreased (P < 0.05) with the increasing levels of concentrate supplementation. In contrast, the time expended on feeding at the trough, biting rate, and the time taken to ruminate each cud increased (P < 0.05) with the supplementation level. Final body weight and average daily gain increased linearly (P < 0.05) with increasing concentrate supplementation. We recommend supplying concentrate supplement at a level of 0.5% of the body weight of steers on tropical pasture during the post-weaning phase in the rainy season.

Introdução: O canal inguinal é região propensa a hérnias que podem ter indicação cirúrgica. Embora a videocirurgia possa ser menos agressiva, a identificação dos ramos nervosos é necessária para diminuir a incidência de inguinodinia que pode ocorrer mesmo com essa via minimamente invasiva. Objetivo: Revisar a incidência e os motivos da inguinodinia nos procedimentos cirúrgicos laparoscópicos na correção das hérnias inguinais. Método: Revisão sistemática, realizada nas bases de dados PubMed, BVS e Google Scholar no período de 2018 a 2024, incluindo artigos em português e inglês. De 1.758 artigos, 10 foram selecionados. Resultado: Os principais achados da pesquisa comprovaram que o reconhecimento e a devida identificação anatômica dos principais nervos da região inguinal durante a operação – ílio-hipogástrico, ilioinguinal e ramo genital do genitofemoral - são capazes de diminuir as chances de os pacientes desenvolverem dor crônica na região inguinal após hernioplastia. A perícia e experiência do cirurgião ao evitar trauma nos nervos locais garante qualidade de vida aos pacientes e reduz a exposição deles a eventuais novas intervenções subsequentes para tratamento de inguinodinia. Conclusão: A prevenção é a medida mais importante tratando-se de erradicar a inguinodinia. Dessa maneira, o profissional deve ter conhecimento suficiente da topografia mais frequente da passagem dos principais nervos e seus ramos para evitar lesões iatrogênicas.

Introduction: The inguinal canal is a region prone to hernias that may require surgery. Although videosurgery may be less aggressive, the identification of nerve branches is necessary to reduce the incidence of inguinodynia that can occur even with this minimally invasive route. Objective: To review the incidence and reasons for inguinodynia in laparoscopic surgical procedures for the correction of inguinal hernias. Method: Systematic review, carried out in the PubMed, BVS and Google Scholar databases from 2018 to 2024, including articles in Portuguese and English. From initial number of 1,758 articles 10 were selected. Result: The main findings of the research proved that the recognition and proper anatomical identification of the main nerves of the inguinal region during the operation - iliohypogastric, ilioinguinal and genital branch of the genitofemoral - are able to reduce the chances of patients developing chronic pain in the inguinal region after hernioplasty. The surgeon's expertise and experience in avoiding trauma to local nerves guarantees quality of life for patients and reduces their exposure to possible new subsequent interventions to treat inguinodynia. Conclusion: Prevention is the most important measure when it comes to eradicating inguinodynia. Therefore, the professional must have sufficient knowledge of the most frequent topography of the passage of the main nerves and their branches to avoid iatrogenic injuries

ABSTRACT Regionalization is a logical, powerful and indispensable governance process in the health policy scenario. Faced with the challenges posed to a supportive regionalization, which enables universal access, equity and comprehensive care, the pioneering Interstate Health Network of the Middle São Francisco Valley was implemented, in the region between the states of Pernambuco and Bahia (PEBA Network). The study analyzed the complex management relationship that involves, in the coordination of care, public and private agents. The approach was qualitative, focusing on the regionalization process of the PEBA Network, through care regulation, from the perspective of managers, service professionals and users. The analysis of the supply-production-waiting line association allows us to see that qualification of clinic management, the referral process and the need for organization and communication are essential to guarantee timely access. Otherwise, there are direct effects on absenteeism, impacting the increase in queues, requiring a systemic and multifactorial approach to organizing the regulatory process. The need for organizational arrangements is evident, involving all actors in the process of formulating and developing public health policies, in order to overcome barriers to care coordination and comprehensive health.

RESUMO A regionalização é um processo de governança lógico, potente e indispensável no cenário da política de saúde. Diante dos desafios postos a uma regionalização solidária, que viabilize a universalidade do acesso, equidade e integralidade do cuidado, foi implantada a pioneira Rede Interestadual de Saúde do Vale do Médio São Francisco, na região entre os estados de Pernambuco e Bahia (Rede Peba). O estudo analisou a complexa relação gerencial que envolve, na coordenação do cuidado, agentes públicos e privados. A abordagem foi qualitativa, tendo como foco o processo de regionalização da Rede Peba, por meio da regulação assistencial, sob a perspectiva dos gestores, profissionais dos serviços e usuários. A análise da associação oferta-produção-fila de espera permite constatar que a qualificação da gestão da clínica e do processo de encaminhamento e necessidade de organização e comunicação são imprescindíveis para garantir acesso oportuno. Do contrário, ocorrem efeitos diretos no absenteísmo, impactando no aumento das filas, exigindo uma abordagem sistêmica e multifatorial para organização do processo regulatório. Fica evidente a necessidade de arranjos organizacionais envolvendo todos os atores no processo de formulação e desenvolvimento das políticas públicas de saúde, a fim de superar barreiras para a coordenação do cuidado e a integralidade em saúde.

Abstract Mucopolysaccharidosis type IIIB (MPS IIIB) is caused by deficiency of alpha-N-acetylglucosaminidase, leading to storage of heparan sulphate. The disease is characterized by intellectual disability and hyperactivity, among other neurological and somatic features. Here we studied retrospective data from a total of 19 MPS IIIB patients from Brazil, aiming to evaluate disease progression. Mean age at diagnosis was 7.2 years. Speech delay was one of the first symptoms to be identified, around 2-3 years of age. Behavioral alterations include hyperactivity and aggressiveness, starting around age four. By the end of the first decade, patients lost acquired abilities such as speech and ability to walk. Furthermore, as disease progresses, respiratory, cardiovascular and joint abnormalities were found in more than 50% of the patients, along with organomegaly. Most common cause of death was respiratory problems. Resumo Este estudo teve como objetivo examinar o efeito da idade relativa na potência muscular em crianças e adolescentes brasileiros. A amostra foi composta por 87.766 meninas e meninos com idades entre 7 e 16 anos. Os participantes foram agrupados em quatro subgrupos etários dentro de cada idade cronológica tendo como base as datas de nascimento e da coleta dos dados. A potência muscular dos membros superiores e inferiores foi avaliada por meio dos testes de arremesso de medicine ball de 2 kg, e salto horizontal, respectivamente. A distribuição normativa (fraco; razoável; bom; muito bom; excelente) dos dois testes de potência foi utilizada para a classificação. Os resultados mostraram associação significativa entre os subgrupos etários e a classificação normativa da potência muscular. Entre os meninos, houve aumento na frequência de indivíduos classificados como “muito bom/excelente” à medida que a idade relativa aumentava. Uma tendência semelhante foi observada entre as meninas. Os achados forneceram evidências do efeito de idade relativa na potência muscular de crianças e adolescentes brasileiros. Esses resultados têm implicações práticas na organização de atividades físicas estruturadas e esportes em diferentes contextos, pois indivíduos nascidos mais cedo podem ter vantagem no desempenho em tarefas que exijam potência muscular. Abstract This study aimed to examine the relative age effect on muscle power in a large sample of Brazilian children and adolescents. The sample consisted of 87,766 girls and boys aged 7-16 years, data collected in repeated cross-sectional surveillance carried out since 1999. The participants were grouped into four age subgroups within each chronological age based on birthdates. Upper and lower limbs muscle power were assessed using the 2-Kg medicine ball throw and horizontal jump tests, respectively. The normative distribution of physical fitness components was used for classification. The results showed a significant association between relative age and normative classification of muscle power. Among boys, there was a sequential increase in the frequency of individuals classified as “very good/excellent” as the relative age increased. A similar trend was observed among girls. The findings provided evidence of a relative age effect on muscle power in Brazilian children and adolescents. These findings have implications for the organization of structured physical activities and sports, as early-born individuals may have an advantage in performance. Abstract The goal of this article is to assess whether, and to what extent, the deindustrialization of the Brazilian economy has affected the geography of voting in the last two decades. Particular interest is given to examining the inflection of the vote in the Workers’ Party (PT). The study considers deindustrialization as an exogenous economic shock that, by changing the income and nature of employment available in localities, alters electoral support for the nationally incumbent party. Our main sources of empirical data are: the Instituto Brasileiro de Geografia e Estatística (IBGE) and the Superior Electoral Court (TSE); and our spatial units of analysis are municipalities. Our results confirm the hypothesis that deindustrialization negatively affected the Workers’ Party vote. ABSTRACT The pineapple, the most important fruit in the state of Paraíba, is characterized as an important generator of employment, income and regional development. With the increase in production, challenges arise in reaching distant competitive markets, including exports, making refrigerated storage necessary. However, pineapple, a tropical fruit, is subject to Chilling Injury (CI) when stored under sub-optimal temperatures, below 12 °C, which provide internal browning (IB) and quality loss. The use of pulsed electric field (PEF), a non-thermal technology, can be an alternative in ensuring the control of IB in pineapple. Therefore, the objective of this work was to evaluate the influence of PEF application on IB, enzyme expression and activity, bioactive compounds and antioxidant activity of ‘Pérola’ pineapple. Initially, to define the best strategy, 8 kV/cm of PEF was applied to pineapples directly and indirectly (drinking water), in 4 levels of electrical pulses: 0 (control), 5, 20 and 35, proving to be the most efficient the indirect pulses. Resumo Antecedentes Encefalites autoimunes (EAIs) são um grupo de doenças inflamatórias caracterizadas pela presença de anticorpos contra antígenos neuronais e gliais, que ocasionam sintomas psiquiátricos subagudos, queixas de memória e distúrbios anormais do movimento. A maioria dos pacientes é jovem, e o atraso no tratamento está associado a pior prognóstico. Objetivo Com o apoio da Academia Brasileira de Neurologia (ABN) e da Sociedade Brasileira de Neurologia Infantil (SBNI), desenvolvemos um consenso sobre o diagnóstico e o tratamento da EAIs no Brasil utilizando a metodologia Delphi. Métodos Um total de 25 especialistas, incluindo neurologistas e neurologistas infantis, foram convidados a participar. Abstract Background Autoimmune encephalitis (AIE) is a group of inflammatory diseases characterized by the presence of antibodies against neuronal and glial antigens, leading to subacute psychiatric symptoms, memory complaints, and movement disorders. The patients are predominantly young, and delays in treatment are associated with worse prognosis. Objective With the support of the Brazilian Academy of Neurology (Academia Brasileira de Neurologia, ABN) and the Brazilian Society of Child Neurology (Sociedade Brasileira de Neurologia Infantil, SBNI), a consensus on the diagnosis and treatment of AIE in Brazil was developed using the Delphi method. Methods A total of 25 panelists, including adult and child neurologists, participated in the study. Results The panelists agreed that patients fulfilling criteria for possible AIE should be screened for antineuronal antibodies in the serum and cerebrospinal fluid (CSF) using the tissue-based assay (TBA) and cell-based assay (CBA) techniques. Children should also be screened for anti-myelin oligodendrocyte glucoprotein antibodies (anti-MOG). Treatment should be started within the first 4 weeks of symptoms. The first-line option is methylprednisolone plus intravenous immunoglobulin (IVIG) or plasmapheresis, the second-line includes rituximab and/or cyclophosphamide, while third-line treatment options are bortezomib and tocilizumab. Most seizures in AIE are symptomatic, and antiseizure medications may be weaned after the acute stage. In anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis, the panelists have agreed that oral immunosuppressant agents should not be used. ABSTRACT The objective of this study was to examine the effect of the increasing levels of inclusion of palm kernel cake (PKC) in the diet on the performance of feedlot heifers. Forty-eight Nelore heifers with an initial weight of 274 ± 4.58 kg, at 24 months of age, were confined for 98 days in a feedlot. The animals were allocated to the four treatments in a completely randomized design. Treatments consisted of PKC levels of 0 (control), 10, 20, and 30% in the total dry matter of the diet. The roughage:concentrate ratio in the diets was 30:70. Ether extract intake increased, whereas the intakes of non-fiber carbohydrates and total digestible nutrients decreased with the inclusion of PKC. The apparent digestibility of all nutrients decreased, as well as the amounts of nitrogen digested and retained. Microbial protein synthesis and its efficiency also declined. The inclusion of up to 20% PKC increased feeding time and reduced rumination time of heifers. Intake and rumination efficiencies decreased with the inclusion of PKC in the diet. Generalized anxiety disorder is a highly prevalent mental disorder. Previous data indicate that more than 18 million Brazilians suffer from this condition. Traditionally, generalized anxiety disorder has been considered a mild mental health disorder, despite its links to lower life expectancy, cardiovascular disease, and suicide. The aim of this article is to combine elements of systematic and critical reviews to produce a synthesis of the best evidence about generalized anxiety disorder treatment. Systematic reviews, meta-analyses, and randomized controlled trials were included. The descriptor used in the search was “generalized anxiety disorder,” which resulted in 4,860 articles and seven other studies, of which 59 were selected. Antidepressants and benzodiazepines were indicated, as was pregabalin, and atypical antipsychotics, such as quetiapine, have been studied. Individual cognitive behavior therapy (third wave) has proven effective. There is extensive literature on many effective treatments for generalized anxiety disorder. The present review summarizes the therapeutic possibilities, emphasizing those available in Brazil. Further studies are needed to compare other available medications, assess psychotherapies and new treatments in greater depth, as well as to assess the ideal duration of therapy. Resumo Antecedentes Pacientes com doença grave por coronavírus-19 (COVID-19) podem necessitar do uso de ventilação mecânica (VM) invasiva por um período prolongado. Parâmetros agressivos de VM têm sido associados a alterações na pressão intracraniana (PIC) em pacientes com doenças intracranianas agudas. Elevações significativas da PIC podem comprometer a complacência intracraniana (CIC) e a hemodinâmica cerebrovascular (HVC). No entanto, os efeitos desses parâmetros em indivíduos sem doenças neurológicas ainda não foram sistematicamente avaliados. Objetivo Avaliar a CIC em pacientes em VM com COVID-19 comparados com outros diagnósticos, para melhor caracterizar os efeitos da VM e COVID-19 sobre a CIC. Também foi feita a comparação entre a CIC em pacientes com COVID-19 sem VM e voluntários saudáveis, para avaliar o efeito isolado da COVID-19 sobre a ICC. Métodos Trata-se de um estudo exploratório, observacional com amostra por conveniência. A CIC foi avaliada com um dispositivo não invasivo de monitoramento da PIC. Abstract Background Patients with severe coronavirus disease-19 (COVID-19) may require the use of invasive mechanical ventilation (MV) for prolonged periods. Aggressive MV parameters have been associated with changes in intracranial pressure (ICP) in patients with acute intracranial disorders. Significant ICP elevation could compromise intracranial compliance (ICC) and cerebrovascular hemodynamics (CVH). However, the effects of these parameters in individuals without neurological disorders have not yet been evaluated. Objective To evaluate ICC in patients on MV with COVID-19 infection compared to other diagnoses, to better characterize the effects of MV and COVID-19 upon ICC. We also compared between the ICC in patients with COVID-19 who did not require MV and healthy volunteers, to assess the isolated effect of COVID-19 upon ICC. Methods This was an exploratory, observational study with a convenience sample. The ICC was evaluated with a noninvasive ICP monitoring device. The P2/P1 ratio was calculated by dividing the amplitude of these two points, being defined as "abnormal" when P2 > P1. The statistical analysis was performed using a mixed linear model with random effects to compare the P2/P1 ratio in all four groups on the first monitoring day. Results A convenience sample of 78 subjects (15 MV-COVID-19, 15 MV non-COVID-19, 24 non-MV-COVID-19, and 24 healthy participants) was prospectively enrolled. There was no difference in P2/P1 ratios between MV patients with and without COVID-19, nor between non-MV patients with COVID-19 and healthy volunteers. RESUMO Objetiva-se apresentar o contexto político e social em que as avaliações sistêmicas vêm se desenvolvendo na América Latina. As discussões seguem em duas direções, uma para explicitar a centralidade da avaliação sistêmica na agenda estruturada mundialmente, e outra para desvelar os discursos que os Organismos Internacionais têm assumido nos anos 1990 e 2000. Essas duas direções bifurcam-se no ponto de confluência qualidade da educação e aprendizagem. Adota-se, como procedimento metodológico, a pesquisa documental e bibliográfica, de forma a avançar essa pauta de discussão para o contexto dos anos 2000, ainda pouco explorado sob essa vertente. A temática propicia, ainda, a conjunção de pesquisadores da área da política educacional que se dedicam às análises desses objetos: a avaliação sistêmica e as diretrizes dos organismos internacionais, o que possibilita diversos olhares, sintetizados no formato deste dossiê. ABSTRACT The objective of the paper is to present the political and social context in which systemic assessments have been developing in Latin America. The discussions are approached in two directions: the first one highlights the centrality of systemic assessment in the globally structured agenda, while the second reveals the discourses adopted by International Organizations in the 1990s and 2000s. These two directions converge at the point of focus, namely, quality of education and learning. The methodological procedure adopted includes documentary and bibliographic research, so as to advance the discussion into the 2000s, an area still little explored under such perspective. The theme also favors the collaboration of researchers in the field of education policy, who focus on the analysis of these objects: systemic assessment and the guidelines of international organizations, allowing for various perspectives, which are synthesized in the format of this dossier. Abstract Vocabulary concerns the lexical repertoire used in communication and constitutes an important aspect of language and cognitive processing. The present research aimed to develop a computerized adaptive test to assess vocabulary extension. Study 1, involving 3958 participants, analyzed the psychometric parameters of items that presented a word with four possible alternatives, only one being the correct synonym. As a result, a 96-item database was created to compose the Adaptive Vocabulary Extension Test (TAVoc). Study 2, which involved 754 participants, analyzed the TAVoc regarding its interruption criterion and validity, comparing it with external measures of verbal reasoning, fluid intelligence, emotional knowledge, and sociability.
