ABSTRACT Proper waste management is a major worldwide concern; waste is disposed of directly into the environment, affecting water, soil and air quality. The development and production of green energy sources is strongly encouraged in modern society. The aim of this study, in accordance with Sustainable Development Goals (SDGs) 1, 2 and 7 of the 2030 Agenda of the UN, is to combine the correct disposal of animal waste with the generation of energy to mitigate the energy insecurity of families in an extremely vulnerable semi-arid region in the north-east of the country using biodigesters. To this end, a conceptual proposal was drawn up for a low-cost, easy-to-maintain and easy-to-use biodigester. A small-scale prototype was used to assess its operation by evaluating the main factors that affect biogas production (pH, temperature and the biomass properties: total solids, fixed solids, volatile solids, concentration of volatile fatty acids and C/N ratio). Resumo Enquadramento: O acolhimento é uma estratégia de cuidado às crianças em situação de maus-tratos, pois compreende a organização de um espaço físico, equipa qualificada, fluxo de notificação e atendimento sensível pelos profissionais de saúde. Objetivo: Descrever o conhecimento e a prática dos profissionais de saúde no atendimento de crianças em situação de maus-tratos sob a ótica do acolhimento. Metodologia: Pesquisa qualitativa, descritiva, de caráter exploratório. A colheita foi realizada por entrevista não diretiva individual no ambiente hospitalar. Utilizou-se análise temática. Resultados: Participaram 23 profissionais de saúde. O sexo feminino foi o predominante e 17 participantes não possuíam curso de capacitação. O acolhimento foi apontado no modo de falar, agir e na rotina de cuidado. Conclusão: O conhecimento dos profissionais de saúde foi desenvolvido na prática profissional pelo tempo de formação e pelo cuidado sensível. A capacitação deficiente interfere na identificação de crianças e adolescentes em situação maus-tratos. Crianças menores são mais vulneráveis e podem sofrer negligência. O acolhimento precisa ser articulado com outras esferas com fluxo de atendimento conhecido.
Abstract Background: Welcoming child victims of maltreatment is a care strategy that requires a physical space, a qualified team, and a reporting system as well as the adoption of a more sensitive approach by health professionals. Objective: To describe the knowledge and practice of health professionals who provide care for maltreated children focusing on patient welcoming. Methodology: This is a qualitative, descriptive, exploratory study that used individual non-directive interviews in a hospital setting for data collection. A thematic analysis was conducted on the data collected. Results: A total of 23 health professionals participated in the study. Most participants were women and 17 had no formal qualifications. The welcoming practices were identified through the analysis of the statements, behaviors, and care practices of the participants. Conclusion: Health professionals have developed their knowledge through professional practice, training, and sensitivity. Inadequate training hinders the identification of child and adolescent victims of maltreatment. Younger children are more vulnerable and may suffer from neglect. Welcoming practices must be articulated with other professional sectors using a common line of care model.
Resumen Marco contextual: La acogida familiar es una estrategia para cuidar a los niños en situaciones de maltrato, ya que incluye la organización de un espacio físico, personal cualificado, flujo de notificaciones y atención sensible por parte de los profesionales sanitarios. Objetivo: Describir los conocimientos y la práctica de los profesionales sanitarios en la atención a niños en situación de maltrato desde la perspectiva de la acogida familiar. Metodología: Investigación cualitativa, descriptiva y exploratoria. Los datos se recogieron mediante entrevistas individuales no directivas en el entorno hospitalario. Se utilizó el análisis temático. Resultados: Participaron 23 profesionales sanitarios. Predominaron las mujeres y 17 participantes no tenían formación. La acogida destacó en su forma de hablar, de actuar y en su rutina asistencial. Conclusión: Los conocimientos de los profesionales sanitarios se desarrollaron en la práctica profesional mediante el tiempo de formación y el cuidado sensible. Una formación deficiente interfiere en la identificación de niños y adolescentes en situación de maltrato. Los niños más pequeños son más vulnerables y pueden sufrir desatención. La acogida familiar debe articularse con otras esferas con un flujo de cuidados conocido.

ABSTRACT This study aimed to analyze the actions taken by health management to deal with the criminal disaster of an oil spill off the coast of Pernambuco in 2019. It is a case study investigating the health sector’s actions in four municipalities in Pernambuco. Data was collected through interviews with 16 health department managers (municipal and state), using a semi-structured script with questions about performance, impacts, actions taken, difficulties, challenges, and lessons learned. The Collective Subject Discourse analysis revealed four thematic axes: a) managers’ perception of the impacts of the crime disaster; b) actions taken in the process of risk management of the disaster crime by the health sector; c) difficulties faced in the process of mitigating the disaster; d) evaluation of management actions during the disaster crime and future preparation. Weaknesses in health management were identified in disaster mitigation actions due to the need for more preparation of health services and professionals. The lack of instruments and coordination between the federated entities intensified the difficulties, generating calamity in the territory. This points to the need to build instruments and protocols to guide such actions in these situations so that activities can be carried out more efficiently and effectively.
RESUMO Este estudo objetivou analisar as ações desenvolvidas pela gestão de saúde no enfrentamento do desastre-crime de derramamento de petróleo na costa pernambucana em 2019. Trata-se de estudo de caso que investigou a atuação do setor saúde em quatro municípios de Pernambuco. A coleta de dados foi mediante entrevistas com 16 gestores de secretarias de saúde (municipal e estadual), utilizando roteiro semiestruturado com perguntas sobre atuação, impactos, ações desenvolvidas, dificuldades, desafios e lições aprendidas. A análise do Discurso do Sujeito Coletivo evidenciou quatro eixos temáticos: a) percepção dos gestores sobre os impactos do desastre-crime; b) ações desenvolvidas no processo de gestão de risco do desastre-crime pelo setor saúde; c) dificuldades enfrentadas no processo de mitigação do desastre; d) avaliação das ações da gestão durante o desastre-crime e preparação futura. Identificaram-se fragilidades da gestão em saúde nas ações de mitigação do desastre, resultado da falta de preparo dos serviços e dos profissionais de saúde. A falta de instrumentos e a desarticulação entre os entes federados intensificaram as dificuldades, gerando situações de calamidade no território. Isso aponta para a necessidade de construção de instrumentos e protocolos que guiem tais ações nessas situações, para que as atividades sejam desenvolvidas com mais eficiência e eficácia.

ABSTRACT Differentiated thyroid carcinoma (DTC) accounts for most cases of thyroid cancer, and the heterogeneity of DTC requires that management decisions be taken by a multidisciplinary team involving endocrinologists, head and neck surgeons, nuclear medicine physicians, pathologists, radiologists, radiation oncologists, and medical oncologists. It is important for nonspecialists to recognize and refer patients with DTC who will benefit from a specialized approach. Recent advances in knowledge and changes in management of DTC call for the need to raise awareness on the part of these nonspecialist physicians, including general endocrinologists and medical oncologists at large. We provide an overview of diagnostic and therapeutic principles in DTC, especially those that bear direct implication on day-to-day management of these patients by generalists. Patients with DTC may be broadly categorized as having localized, locally persistent/recurrent, or metastatic disease. Current recommendations for DTC include a three-tiered system that classifies patients with localized disease into low, intermediate, or high risk of persistent or recurrent disease. Risk stratification should be performed at baseline and repeated on an ongoing basis, depending on clinical evolution. One of the overarching goals in the management of DTC is the need to personalize treatment by tailoring its modality and intensity according to ongoing prognostic stratification, evolving knowledge about the disease, and patient characteristics and preference. In metastatic disease that is refractory to radioactive iodine, thyroid tumors are being reclassified into molecular subtypes that better reflect their biological properties and for which molecular alterations can be targeted with specific agents. ABSTRACT Objective Currently programmed cell death protein 1 (PD-1) inhibitors in combination with other therapies are being evaluated to determine their efficacy in cancer treatment. However, the effect of PD-ligand (L) 1 expression on disease outcomes in stage III (EC III) non-small cell lung cancer is not completely understood. Therefore, this study aimed to assess the influence of PD-L1 expression on the outcomes of EC III non-small cell lung cancer. Methods This study was conducted on patients diagnosed with EC III non-small cell lung cancer who underwent treatment at a tertiary care hospital. PD-L1 expression was determined using immunohistochemical staining, all patients expressed PD-L1. Survival was estimated using the Kaplan-Meier method. Relationships between variables were assessed using Cox proportional regression models. Results A total of 49 patients (median age=69 years) with EC III non-small cell lung cancer and PD-L1 expression were evaluated. More than half of the patients were men, and most were regular smokers. The patients were treated with neoadjuvant chemotherapy, surgery, or sequential or combined chemotherapy and radiotherapy. The median progression-free survival of the entire cohort was 14.2 months, and the median overall survival was 20 months. There was no significant association between PD-L1 expression and disease progression, clinical characteristics, or overall survival. Conclusions PD-L1 expression was not correlated with EC III non-small cell lung cancer outcomes. Whether these findings differ from the association with immune checkpoint inhibitors remains to be addressed in future studies. RESUMO Áreas impactadas pela deposição de rejeito de mineração de ferro possuem características físicas e químicas que podem dificultar o crescimento das raízes, e a aplicação de condicionadores pode ajudar a aliviar essas limitações. O objetivo foi avaliar o efeito de condicionadores de solo na melhoria do ambiente para o crescimento de gramínea em solo afetado por rejeitos de mineração de ferro. Foram realizados quatro experimentos testando proporções de condicionador (0, 5, 10, 25 e 50% v/v), em delineamento inteiramente casualizado com quatro repetições, utilizando Cynodon spp. Os condicionadores foram areia, vermiculita, serragem e composto orgânico. Foram avaliados densidade aparente, macro- e microporosidade, volume total de poros (VTP), pH, matéria orgânica (OM), capacidade de troca de cátions (CEC) e massa seca da parte aérea, raízes e total. Houve melhorias na porosidade e redução na densidade com a aplicação de vermiculita, serragem e composto; no entanto, a areia diminuiu o VTP. O pH foi reduzido com serragem e aumentou com composto. A serrage 6m e o composto aumentaram a OM. A CEC aumentou com vermiculita e composto. Os condicionadores de solo, de modo geral, mostraram-se eficazes na melhoria das condições de porosidade, redução da densidade, aumento de OM e promoção do crescimento das plantas, o que torna esta tecnologia eficiente para aplicação em rejeitos. ABSTRACT Areas impacted by the deposition of iron mining tailings present physical and chemical characteristics that can hinder root growth, and applying soil conditioners can help alleviate these limitations. The objective was to evaluate the effect of soil conditioners in improving the environment for the growth of grass in soil affected by iron mining tailing. Four different experiments were conducted testing conditioner proportions (0, 5, 10, 25, and 50% v/v), in a completely randomized design with four replications, using Cynodon spp. The conditioners were sand, vermiculite, sawdust, and organic compost. Bulk density, macro- and microporosity, total pore volume (VTP), pH, organic matter content (OM), cation exchange capacity (CEC), and dry mass of the aboveground, root, and total plant were evaluated. There were improvements in porosity and a reduction in density with the application of vermiculite, sawdust, and organic compost; however, sand decreased the VTP. The pH was reduced with sawdust and increased with compost. Sawdust and compost increased OM. CEC increased with vermiculite and compost. The sand and vermiculite conditioners promoted the greatest growth of Cynodon spp., with emphasis on vermiculite, which provided the greatest increases in plant biomass production. The soil conditioners, overall, proved effective in improving porosity conditions, reducing density, increasing OM, and promoting plant growth, this makes this technology efficient for application in tailings. Resumo Fundamento: A adesão à uma alimentação adequada em macronutrientes é fundamental para a prevenção secundária de doenças cardiovasculares. Objetivo: Avaliar a prevalência de adesão às recomendações de consumo de ácidos graxos para prevenção e tratamento de doenças cardiovasculares, e estimar se a presença de determinados fatores de risco cardiovascular estaria associada à adesão. Métodos: Estudo transversal com os dados de linha de base de 2358 participantes do estudo "Brazilian Cardioprotective Nutritional Program Trial". Dados de consumo alimentar, e fatores de risco cardiovascular foram avaliados. Foi considerada, de acordo com a Sociedade Brasileira de Cardiologia, uma ingestão adequada de ácidos graxos poli-insaturados (AGPI) ≥10% do consumo total de energia diária, para ácidos graxos monoinsaturados (AGM), 20% e para ácidos graxos saturados (AGS), <7%. Na análise estatística foi considerando nível de significância de 5%. Resultados: Nenhum participante aderiu a todas as recomendações de forma simultânea e mais da metade (1482 [62,9%]) não aderiu a nenhuma recomendação. A adesão exclusivamente à recomendação de AGS foi a mais prevalente, sendo cumprida por 659 (28%) dos participantes, seguida da adesão exclusivamente à recomendação de AGP (178 [7,6%]) e de AGM (5 [0,2%]). Não houve associação entre o número de comorbidades e a adesão às recomendações nutricionais (p =0,269). Os participantes da região Nordeste do país apresentaram maior proporção de adesão às recomendações para consumo de AGS (38,42%), e menor para ingestão de AGPI (3,52%) (p <0,001) em comparação às demais. Background: Adhering to a diet adequate in macronutrients is crucial for the secondary prevention of cardiovascular diseases. Objective: To assess the prevalence of adherence to recommendations for the consumption of dietary fatty acids for the prevention and treatment of cardiovascular diseases and to estimate whether the presence of certain cardiovascular risk factors would be associated with adherence. Methods: Cross-sectional study using baseline data from 2,358 participants included in the "Brazilian Cardioprotective Nutritional Program Trial". Dietary intake and cardiovascular risk factors were assessed. Adequate intake of polyunsaturated fatty acids (PUFA) was considered as ≥10% of total daily energy intake; for monounsaturated fatty acids (MUFA), 20%; and for saturated fatty acids (SFA), <7% according to the Brazilian Society of Cardiology. A significance level of 5% was considered in the statistical analysis. Results: No participant adhered to all recommendations simultaneously, and more than half (1,482 [62.9%]) did not adhere to any recommendation. Adherence exclusively to the SFA recommendation was the most prevalent, fulfilled by 659 (28%) participants, followed by adherence exclusively to the PUFA (178 [7.6%]) and MUFA (5 [0.2%]) recommendations. There was no association between the number of comorbidities and adherence to nutritional recommendations (p = 0.269). Participants from the Brazilian Northeast region showed a higher proportion of adherence to SFA consumption recommendations (38.42%) and lower adherence to PUFA intake (3.52%) (p <0.001) compared to other regions. RESUMO Objetivo Verificar a associação entre a restrição à participação decorrente de perda auditiva e a autopercepção da saúde, do suporte social e da qualidade de vida em pessoas idosas. Método Estudo com delineamento transversal, observacional, descritivo e com abordagem quantitativa dos dados. Foi utilizado um banco de dados com informações coletadas em um município brasileiro de médio porte, sendo incluídas no estudo 235 pessoas cadastradas em cinco unidades de Estratégia de Saúde da Família. Utilizou-se informações sociodemográficas, de saúde e os resultados dos questionários: avaliação à restrição à participação (Hearing Handicap Inventory for the Elderly Screening Version - HHIE-S), Suporte Social (Escala de Apoio Social do MOS), Avaliação Subjetiva de Saúde e a Qualidade de Vida (Short-Form 6 dimensions- SF-6D). Comparou-se os grupos com restrição e sem restrição à participação segundo as variáveis sociodemográficas, de saúde, suporte social e qualidade de vida. Um método de regressão logística binária multivariado foi utilizado para avaliar as associações entre as variáveis independentes e a restrição à participação. Resultados O grupo com restrição à participação é mais velho, possui menor qualidade de vida e pior autopercepção de saúde. Esta se mostrou ser o único preditor da restrição à participação relacionada à perda auditiva. Conclusão A restrição à participação está associada a uma pior autopercepção de saúde. ABSTRACT Purpose To verify the association between participation restriction due to hearing loss and self-perception of health, social support, and quality of life in elderly people. Methods This is a cross-sectional, observational, and descriptive study with a quantitative data approach. A database with information collected in a medium-sized Brazilian municipality was used. The study was conducted with 235 elderly people registered in five Family Health Strategy Units. Sociodemographic and health information and the results of the following questionnaires were used: Hearing Handicap Inventory for the Elderly – Screening Version (HHIE-S), Medical Outcomes Study (MOS) Social Support Survey, Subjective Health Assessment, and Short-Form 6-Dimension (SF-6D) Health and Quality of Life Index. Groups with and without participation restriction were compared according to sociodemographic, health, social support, and quality of life variables. A multivariate binary logistic regression method was employed to evaluate the associations between the independent variables and participation restriction. Results The group with participation restriction is composed of older individuals with lower quality of life and poorer self-perception of health. Poorer self-perception of health was the only predictor of participation restriction related to hearing loss. Conclusion Participation restriction is associated with poorer self-perception of health. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. RESUMO Objetivo: mapear as evidências científicas acerca das percepções, crenças, conhecimentos e atitudes de pais ou responsáveis legais de crianças menores de 5 anos quanto à vacinação infantil de rotina. Métodos: revisão de escopo, conduzida de acordo com o referencial do JBI. As buscas foram realizadas nas bases PubMed/MEDLINE, Web of Science, Scopus e LILACS. Retornaram 5.535 estudos e foram selecionados 77, que atenderam aos critérios de inclusão. Resultados: percepções relacionadas à interação com os profissionais e serviços de saúde, com a organização e estrutura familiar, com a interação social e políticas públicas, crenças culturais, religiosas e pessoais, conhecimentos sobre o calendário vacinal, o processo de vacinação e imunização e fontes de informação são os principais fatores mapeados e que podem influenciar positiva ou negativamente a atitudes dos pais ou responsáveis legais em vacinar crianças. ABSTRACT Objective: to map scientific evidence about perceptions, beliefs, knowledge and attitudes of parents or legal guardians of children under 5 years of age regarding routine childhood vaccination. Methods: a scoping review, conducted in accordance with the JBI framework. The searches were carried out in the PubMed/MEDLINE, Web of Science, Scopus and LILACS databases. RESUMO Este estudo objetivou analisar as ações desenvolvidas pela gestão de saúde no enfrentamento do desastre-crime de derramamento de petróleo na costa pernambucana em 2019. Trata-se de estudo de caso que investigou a atuação do setor saúde em quatro municípios de Pernambuco. A coleta de dados foi mediante entrevistas com 16 gestores de secretarias de saúde (municipal e estadual), utilizando roteiro semiestruturado com perguntas sobre atuação, impactos, ações desenvolvidas, dificuldades, desafios e lições aprendidas. A análise do Discurso do Sujeito Coletivo evidenciou quatro eixos temáticos: a) percepção dos gestores sobre os impactos do desastre-crime; b) ações desenvolvidas no processo de gestão de risco do desastre-crime pelo setor saúde; c) dificuldades enfrentadas no processo de mitigação do desastre; d) avaliação das ações da gestão durante o desastre-crime e preparação futura. Identificaram-se fragilidades da gestão em saúde nas ações de mitigação do desastre, resultado da falta de preparo dos serviços e dos profissionais de saúde. A falta de instrumentos e a desarticulação entre os entes federados intensificaram as dificuldades, gerando situações de calamidade no território. ABSTRACT This study aimed to analyze the actions taken by health management to deal with the criminal disaster of an oil spill off the coast of Pernambuco in 2019. It is a case study investigating the health sector’s actions in four municipalities in Pernambuco. Data was collected through interviews with 16 health department managers (municipal and state), using a semi-structured script with questions about performance, impacts, actions taken, difficulties, challenges, and lessons learned. The Collective Subject Discourse analysis revealed four thematic axes: a) managers’ perception of the impacts of the crime disaster; b) actions taken in the process of risk management of the disaster crime by the health sector; c) difficulties faced in the process of mitigating the disaster; d) evaluation of management actions during the disaster crime and future preparation. Weaknesses in health management were identified in disaster mitigation actions due to the need for more preparation of health services and professionals. The lack of instruments and coordination between the federated entities intensified the difficulties, generating calamity in the territory. Abstract Background Patients with immune-mediated rheumatic diseases (IMRDs) have been prioritized for COVID-19 vaccination to mitigate the infection severity risks. Patients with rheumatoid arthritis (RA) are at a high risk of severe COVID-19 outcomes, especially those under immunosuppression or with associated comorbidities. However, few studies have assessed the safety of the COVID-19 vaccine in patients with RA. Objective To evaluate the safety of vaccines against SARS-CoV-2 in patients with RA. Methods This data are from the study “Safety and Efficacy on COVID-19 Vaccine in Rheumatic Diseases,” a Brazilian multicentric prospective phase IV study to evaluate COVID-19 vaccine in IMRDs in Brazil. Adverse events (AEs) in patients with RA of all centers were assessed after two doses of ChAdOx1 (Oxford/AstraZeneca) or CoronaVac (Sinovac/Butantan). Stratification of postvaccination AEs was performed using a diary, filled out daily and returned at the end of 28 days for each dose. Results A total of 188 patients with RA were include, 90% female. CoronaVac was used in 109 patients and ChAdOx1 in 79. Only mild AEs were observed, mainly after the first dose. The most common AEs after the first dose were pain at the injection (46,7%), headache (39,4%), arthralgia (39,4%), myalgia (30,5%) and fatigue (26,6%), and ChAdOx1 had a higher frequency of pain at the injection (66% vs 32 %, p < 0.001) arthralgia (62% vs 22%, p < 0.001) and myalgia (45% vs 20%, p < 0.001) compared to CoronaVac. The more common AEs after the second dose were pain at the injection (37%), arthralgia (31%), myalgia (23%), headache (21%) and fatigue (18%). Arthralgia (41,4% vs 25%, p = 0.02) and pain at injection (51,4% vs 27%, p = 0.001) were more common with ChAdOx1. No serious AEs were related. With Regard to RA activity level, no significant difference was observed between the three time periods for both COVID-19 vaccines. Conclusion In the comparison between the two immunizers in patients with RA, local reactions and musculoskeletal symptoms were more frequent with ChAdOx1 than with CoronaVac, especially after the first dose. In summary, the AE occurred mainly after the first dose, and were mild, like previous data from others immunizing agents in patients with rheumatoid arthritis. ABSTRACT Objective: This study was conducted to investigate the distribution of placental villous vessels in pregnant women with different degrees of hyperglycemia. Methods: A cross-sectional study was performed using placental samples from 30 pregnant women without diabetes (n=10), with gestational diabetes mellitus (n=10), and with previous diabetes (type 1 and type 2 diabetes; n=10). Maternal glycemic control was evaluated using the glycemic mean and glycated hemoglobin levels. Placental samples were obtained during elective cesarean sections and processed for villous vessel analysis using immunohistochemistry for Von Willebrand factor. Vessels within 10μm of the villus margin were classified as peripheral, and vessels at a distance greater than 10μm were classified as central. The number, area, and perimeter of all vessels were evaluated, and the relationship between vessel area and total area of placental villus was calculated. Results: Pregnant women with gestational diabetes mellitus and those with previous diabetes had higher glycated hemoglobin levels. The number of vessels was reduced in the villi of the previous Diabetes Group owing to peripheral reduction. Additionally, the area, perimeter, and percentage of peripheral blood were lower in the previous Diabetes Group than in the Non-Diabetic Group.
